Interpretation of Clinical Guidelines for Irritable Bowel Syndrome issued by American Gastroenterology Association, 2020 and comparison of diagnosis and treatment in China and the United States#br#

Irritable bowel syndrome (IBS), as a refractory functional gastrointestinal disease, has brought huge economic burden to medical systems of various countries in the world. The American College of Gastroenterology (ACG) recently updated its guidelines for the diagnosis and treatment of IBS, adopting the GRADE grading method for the first time to analyze the management of IBS from two aspects of diagnosis and treatment. Chinese scholars have also carried out a lot of studies. According to specific national conditions of China, Delphi program is adopted to update the IBS expert consensus. This paper aims to interpret the ACG 2020 Clinical Guidelines for Irritable Bowel Syndrome and make a comparative analysis with the Expert Consensus On Irritable Bowel Syndrome in China in 2020, in order to provide reference for the diagnosis and treatment of patients with IBS in China.

Effect of astragalus polysaccharide on gastrointestinal function regulation and repair mechanism of intestinal mucosa injury in rats with dampness stagnancy due to spleen deficiency#br#

Objective To investigate the regulating effect of astragalus polysaccharide (APS) on gastrointestinal function in rats with dampness stagnancy due to spleen deficiency and the mechanism of repairing intestinal mucosal injury through Wnt/β-catenin signaling pathway. Methods Fifty 4-week-old male Wistar rats were divided into five groups according to random number table method, with ten rats in each group: control group (normal feeding), model group (rat model of dampness stagnancy due to spleen deficiency induced by high-fat and low-protein diet supplemented by exhaustion swimming), APS high-dose group (after modeling, APS was given 900 mg/[kg·d], 1 ml/100 g each time), APS medium-dose group (after modeling, APS was given 600 mg/[kg·d], 1 ml/100 g each time), and APS low-dose group (after modeling, APS was given 300 mg/[kg·d], 1 ml/100 g each time), APS was administered daily. Control group and model group were given the same volume of normal saline intragastric administration for two weeks. Serum amylase, D-xylose, motilin and gastrin were detected by enzyme-linked immunoadsordent assay. HE staining was used to observe the pathological changes of small intestine in each group. The changes of Wnt1 and β-catenin protein in small intestine tissue were observed by immunohistochemistry. Results Compared with the control group, the levels of serum amylase, D-xylose, motilin and gastrin in model group were significantly decreased, with highly statistical significance (P < 0.01). Compared with model group, serum amylase, D-xylose and gastrin levels in APS high-dose group were increased, and the differences were statistically significant (P < 0.05 or P < 0.01). The levels of serum amylase and motilin in APS middle-dose and APS low-dose groups were increased, and the differences were statistically significant (P < 0.05 or P < 0.01). The results of HE staining showed that the intestinal mucosa of the model group was damaged, and the APS dose groups were improved in different degrees. Compared with control group, the expression of Wnt1 and β-catenin protein in small intestine of model group was significantly up-regulated, and the difference was highly statistically significant (P < 0.01). Compared with model group, the protein expressions of Wnt1 and β-catenin in APS dose groups were significantly down-regulated, with highly statistical significance (P < 0.01). Conclusion APS can improve gastrointestinal digestion and absorption function in rats with dampness and spleen deficiency, and its mechanism is related to the regulation of Wnt /β-catenin pathway.

Discussion on the immune regulatory mechanism of Qilian Yishen Ointment in the treatment of chronic kidney disease based on network pharmacology#br#

Objective To explore the immune regulatory mechanism of Qilian Yishen Ointment in the treatment of chronic kidney disease based on network pharmacology. Methods The active ingredients and corresponding targets of Qilian Yishen Ointment were searched through traditional Chinese medicine systems pharmacology database and analysis platform. The target information was standardized by DrugBank database and UniProt database. The targets related to chronic kidney disease were searched by DrugBank, TTD, CTD, and GeneCards database. The potential targets of overlap between Qilian Yishen Ointment and chronic kidney disease were obtained. Cytoscape 3.2.1 was used to construct the component-target network diagram, and DAVID database was used to analyze the GO biological process and KEGG signal pathway enrichment. Through literature review, the immune-related pathway of Qilian Yishen Ointment in the treatment of chronic kidney disease was obtained, and the results were verified by molecular docking technology. Results A total of 144 candidate active ingredients and 261 targets were screened from Qilian Yishen Ointment, among which 83 targets overlaps with those related to chronic kidney disease. The main components of Qilian Yishen Ointment in the treatment of chronic kidney disease were balletin, luteolin, kaempiol, etc. The pathways related to immune regulation mainly involve IL-17, TNF, Th17 cell differentiation, NF-κB, JAK-STAT, and other signaling pathways. Molecular docking preliminarily verified that the main active ingredients could bind Foxp3 and RORγt to play biological roles. Conclusion This study preliminarily reveals the immune regulatory mechanism of Qilian Yishen Ointment with multiple components, multiple targets and multiple approaches in the treatment of chronic kidney disease, and provides a basis for the clinical development and utilization of Qilian Yishen Ointment.

Cloning expression and purification of insulin-related polypeptides 3 from Oyster#br#

Objective To clone and construct a BL21 expressing strain containing insulin related polypeptide 3 (IRP3) gene, and isolate, purifiy, identifiy and renaturat the expressed protein. Methods IRP3-pET30a positive strain BL21 was cloned by DNA recombination technique, the plasmid IRP3-PET30A was successfully constructed by agarose gel electrophoresis after double enzyme digestion, and IRP3/HIS purified protein was obtained by nickel affinity chromatography, SDS-PAGE and Western blot were further used to confirm the purification effect of IRP3/HIS fusion protein. Results Plasmid IRP3-PET30A was successfully cloned and BL21 expressing strain containing plasmid IRP3-pET30a was screened, purified IRP3/HIS fusion protein was obtained from the supernatant of inclusion body lysis and renaturated by dialysis. Conclusion The successful cloning of IRP3 gene and the expression and purification of IRP3/HIS fusion protein lay a foundation for the preparation of IRP3 protein antibody and the application of IRP3 protein as in vitro growth factor additive. What we have done will surely be useful to the antibody preparation for the IRP3 protein and the application of growth factor additive in vitro.

Predictive analysis of key lncRNA and its regulatory effects in postmenopausal osteoporosis based on random forest algorithm#br#

Objective To predictive analyze the regulatory effects of key long noncoding RNA (lncRNA) in postmenopausal osteoporosis (PMOP) by random forest algorithm. Methods The chip data of patients with PMOP was obtained by searching gene expression omnibus database, and the retrieval time was from the inception to December 2020. Key lncRNAs were screened by random forest algorithm, lncRNA target genes were predicted by starBase V2.0 database, and the lncRNA-target gene regulatory network was constructed. STRING V11 was used for proteinprotein interaction analysis of target genes, and R language and KOBAS database were used for GO and KEGG enrichment analysis. Results A total of 127 lncRNA expression data were obtained, ten key lncRNAs were screened by random forest model, and 98 target genes were predicted by starBase V2.0. The target genes were mainly enriched in 13 GO processes such as RNA polymerase activity and 12 KEGG signaling pathways such as galactose metabolism. Conclusion In this study, through mining PMOP gene chip data, the key lncRNAs are screened based on random forest algorithm and the biological process and signal pathway involved in their target genes are predicted. It is concluded that lncRNAs may be involved in the regulation of proliferation and differentiation of osteoblasts and osteoclasts, transcription and translation of genetic materials, and glucose and lipid metabolism of the body.

Li Li was the author with the most published literatures on Melastoma dodecandrum Lour., and the institution was the school of landscape architecture, Fujian Agriculture and Forestry University, the research hotspot was the study of chemical constituents of Melastoma dodecandrum Lour., the research trend was the research of chemical composition and quality control. Effect of Shenling Baizhu Pills on guinea pigs model of primary hypercholesterolemia#br#

Objective To investigate the effect of Shenling Baizhu Pills on guinea pigs model with primary hypercholesterolemia. Methods Sixty male Hartley guinea pigs were divided into control group, model group, Ezetimibe group, Shenling Baizhu Pills high-dose, medium-dose, and low-dose groups, with ten guinea pigs in each group. The control group was given ordinary diet, and the other groups were given high fat diet to establish the guinea pigs model of exogenous hypercholesterolemia. Four weeks after modeling, drug gavage intervention was started. Ezetimibe group was given Ezetimibe suspension 1 mg/kg, Shenling Baizhu pills high-dose, medium-dose, and low-dose groups were given 17.00, 8.50 and 0.85 g/kg, respectively, model group and control group were given equal volume of normal saline. The levels of serum total cholesterol (TC), triglyceride (TG), low density lipoprotein cholesterol (LDL-C), high density lipoprotein cholesterol (HDL-C), bile TC and total bile acid were compared. The histomorphological changes of liver in each groups were observed. Meanwhile, the expression levels of Niemann-Pick type C1 like protein 1 (NPC1L1), ATP binding cassette transport G5 (ABCG5), acyl-CoA cholesterol acyltransferase 2 (ACAT2) and liver X receptor α (LXRα) in each groups were recorded. Results Compared with control group, the serum levels of TG, TC, LDL-C, bile TC, and total bile acid in model group were increased (P < 0.05). Compared with model group, the level of serum TG in Shenling Baizhu Pills low-dose group was decreased (P < 0.05); the levels of TG and LDL-C in Shenling Baizhu Pills medium-dose group were decreased, while the level of HDL-C was increased (P < 0.05); the serum levels of TG, TC, and LDL-C in Shenling Baizhu Pills high-dose group were decreased, while the level of HDL-C was increased (P < 0.05); the levels of bile TC and total bile acid in Shenling Baizhu Pills medium-dose and high-dose groups were decreased (P < 0.05). The liver tissue of control group was normal; in model group, diffuse bullae steatosis, extensive balloonlike change of liver cells, a small amount of inflammatory cell infiltration and dot cell necrosis were observed; steatosis and hepatocyte balloon-like change were significantly reduced in Shenling Baizhu Pills medium-dose and high-dose groups; the improvement of hepatic adipose degeneration in Shenling Baizhu Pills low-dose group was not obvious compared with model group. Compared with control group, the levels of NPC1L1, ACAT2, ABCG5, and LXRα in model group were increased (P < 0.05). Compared with model group, the levels of NPC1L1, ACAT2, and LXRα in Shenling Baizhu Pills medium-dose and high-dose groups were decreased, while the levels of ABCG5 were increased (P < 0.05). Conclusion Shenling Baizhu Pills medium-dose and high-dose can alleviate hyperlipidemia induced by high cholesterol diet and reduce hepatocyte steatosis, inhibit the expression of NPC1L1, ACAT2, and LXRα in small intestine of guinea pigs model with primary hypercholesterolemia, and up-regulate the expression of ABCG5.

Research progress of autophagy in pathogenesis of atherosclerosis#br#

Atherosclerosis (AS) is a progressive inflammatory response that occurs on the vascular wall. Its pathogenesis is complex, and many inflammatory cells and inflammatory factors are involved in this process. Autophagy is a lysosomal mediated process of removing damaged substances in cells and using them for intracellular turnover so as to maintain intracellular homeostasis, which is prevalent in eukaryotic life activities. The relationship between autophagy and AS has attracted more and more attention in the medical field. This paper introduces the relationship between AS and related protein molecules and vascular cell autophagy in the process of autophagy, providing reference for further understanding the mechanism of AS.

Glucose and glutamine metabolism modulate stem cell fates#br#

The metabolic status of stem cells varies with the developmental stage. In turn, cell metabolic patterns, particularly glucose and glutamine metabolism, affect stem cell biological activities. During differentiation, the energy metabolism of stem cells changes from glycolysis to tricarboxylic acid cycle and oxidative phosphorylation. A large number of studies have shown that cell metabolism can regulate stem cell fate through epigenetic mechanism. Metabolic intermediates of glucose and glutamine (α -ketoglutarate, acetyl-CoA, nicotinamide adenine dinucleotide and flavin adenine dinucleotide) apparently regulate self-renewal, proliferation and differentiation of stem cells by influencing methylation markers of histone, DNA and RNA and histone acetylation markers. Cell metabolism and epigenetic regulation provide new ideas for disease treatment. This paper summarizes the metabolic maps of glucose and glutamine in stem cells, and describes the metabolite mediated epigenetic mechanism regulating stem cell fate and its therapeutic application.

Research progress of cardiac power output in predicting prognosis of heart failure#br#

The number of patients with heart failure is increasing rapidly, and the mortality rate is high, and clinical lack of reliable prognostic indicators of disease. Cardiac power output (CPO) is the work done by the heart per unit of time, which is a direct indicator of cardiac function, including peak CPO, reserve CPO and resting CPO, and can be used to guide the treatment of heart failure and evaluate the prognosis. Peak CPO and reserve CPO are independent predictors of short-term and long-term prognosis in patients with stable heart failure respectively, while resting CPO is more suitable for patients with severe heart failure who cannot measure peak CPO. There is no consensus on the measurement method and reference range of CPO. Further study on the application of CPO in heart failure can provide new ideas for the diagnosis and treatment of patients with heart failure. This article reviews the research progress of CPO in the diagnosis and treatment of heart failure.

Research progress on drug resistance of EGFR-TKIs in non-small cell lung cancer and treatment plan after drug resistance#br#

Epidermal growth factor receptor tyrosine kinase inhibitors (EGFR-TKIs) have become a first-line inhibitor of epidermal growth factor receptor mutation-positive non-small cell lung cancer. Clinical effective rate can reach 70%, and the overall survival time is two to twenty-four months. It has great advantages over traditional chemotherapy, and the incidence rate of adverse reaction is low. However, the resistance of EGFR-TKIs is almost inevitable, after nine to thirteen months of EGFR-TKIs treatment, drug resistance will appear, and the disease will progress rapidly again. This paper reviews research progress on drug resistance of EGFR-TKIs in non-small cell lung cancer and treatment plan after drug resistance.

Research progress on anti-atherosclerosis mechanism of phlegm and stasis treatment#br#

Atherosclerosis (AS) is the basic disease of cardiovascular and cerebrovascular diseases, and the intermingled phlegm and blood stasis is the main formation process of AS. In this paper, by summarizing relevant studies on the mechanism of treating phlegm and stasis against AS, it is found that treating phlegm and stasis mainly exerts anti-AS effects by inhibiting the proliferation and migration of vascular smooth muscle, protecting vascular endothelial function, regulating lipid metabolism, alleviating abnormal hemodynamics, and regulating inflammatory response. However, the current research still has the defects of model selection and research methods. In the follow-up research, it is necessary to give full play to the characteristics of traditional Chinese medicine and use cutting-edge technology and methods to provide higher quality evidence-based evidence for the differentiation of AS.

Advances in the treatment of duchenne muscular dystrophy#br#

Duchenne muscular dystrophy (DMD) is an X-linked recessive genetic disease caused by mutations in the DMD gene. The DMD gene encodes dystrophin, which plays an important role in the normal contraction of muscles. When the DMD gene mutation fails to produce dystrophin, the muscle membrane is damaged, leading to continuous deterioration of muscle function. The treatment of DMD is mainly through various methods such as inhibiting the inflammatory response, up-regulating or restoring the expression of dystrophin, regulating Ca2+, and gene therapy to achieve the purpose of alleviating symptoms or curing. In recent years, some important advances have been made in the treatment of DMD, such as new glucocorticoids that have achieved good results while reducing side effects; breakthroughs have also been made in the development of new drugs for exon skip 51 and 53; new oral formulations of read-through therapy for nonsense mutations have also been developed; some breakthroughs have also been made in the treatment of Ca2+ regulators and myostatin inhibitors, in addition, gene editing holds great promise for a complete cure of DMD at the genetic level.

Prevalence, awareness, treatment, and control of hypertension among middle-aged and elderly people in China from 2011 to 2015#br#

Objective To analyze the prevalence, awareness, treatment, and control of hypertension among middle-aged and elderly people in China from 2011 to 2015. Methods The respondents were from China Health and Retirement Longitudinal Study database, the hypertension related data of the three surveys in 2011, 2013 and 2015 were collected respectively. Stata MP14 and SPSS 26.0 were used for statistical analysis. Results In 2011, 2013 and 2015, 11 562, 11 068, and 13 271 subjects aged 45 years and above were enrolled respectively. The prevalence rates of hypertension were 39.2%, 42.6% and 39.5%, the awareness rates were 64.2%, 68.7%, and 89.3%, the treatment rates were 49.1%, 52.4%, and 61.0%, while the control rates were 20.1%, 21.6%, and 27.8%, respectively. The prevalence rate did not change significantly (P = 0.86), and the awareness rate, treatment rate, and control rate all had an upward trend (P < 0.01). In 2011 and 2015, the prevalence rates of eastern, central, and western regions were compared with statistical significance (P < 0.01). The prevalence in urban areas was higher than that in rural areas (P < 0.01), and the prevalence increased with age (P < 0.01). In 2011, the prevalence of hypertension in rural women was higher than that in men (P < 0.01), while in 2015, the prevalence of hypertension in urban men was higher than that in women (P = 0.01). The awareness rate, treatment rate, and control rate of hypertension decreased in the eastern, central, and western regions in turn. The rates in urban areas were higher than those in rural areas, and women were higher than men, while the “three rates” were relatively low in the ≥75 years old group and the 45-55 years old group (P < 0.05). Conclusion The prevalence rate of hypertension among middle-aged and elderly people in China appeared high. The awareness rate, treatment rate, and control rate all showed an increasing trend. There were regional, urban-rural, age and gender differences in each rate.

Correlation analysis of PM2.5 exposure and physical activity interaction on hypertension in community residents of Suzhou City#br#

Objective To investigate the interaction between of particulate matter 2.5 (PM2.5) exposure and physical activity on hypertension in community residents of Suzhou City. Methods From January 2018 to December 2018, 13 728 study subjects were selected by multi-stage random stratified sampling, and they were divided into four groups according to the PM2.5 quartile cutoff point including low (<24.32 μg/m3), medium (24.32-<29.31 μg/m3), high (29.31- <31.24 μg/m3), and extremely high (≥31.24 μg/m3), 3432 persons in each group. The blood pressure was measured by an electronic sphygmomanometer (OmronHEM-1300), and the PM2.5 concentration was calculated according to the data published by the “National Urban Air Quality Real-time Release Platform” of the China Environmental Monitoring Station. The metabolic equivalent (MET) was used to calculate the level of physical activity (PA). The logistic regression model was used to analyze the influencing factors of hypertension, and the additive interaction was used for interaction analysis, and the relative excess risk (RERI), the synergy index (SI), interaction attribution percentage (AP) and corresponding 95%CI. Results Compared with the prevalence of patients of different genders, and the difference was statistically significant (P < 0.05). There were statistically significant differences in the age, gender, education level, family history of hypertension, marital status, smoking, and alcohol consumption among people with or without hypertension (P < 0.05). PM2.5, family history of hypertension, smoking, drinking, and physical activity were the influencing factors of hypertension (P < 0.05). After adjusting for factors such as age, gender, family history of hypertension, marital status, smoking, and drinking, etc., PM2.5 exposure and exercise have an additive interaction on the incidence of hypertension, where SI = 2.89, 95%CI: 1.96-3.04; RERI = 0.64, 95%CI: 0.53-0.71; AP=0.31, 95%CI: 0.28-0.42. Conclusion There is an interaction between PM2.5 and PA on the risk of hypertension. PA can reduce the harmful effect of PM2.5 on hypertension.

Analysis of disability acceptance, self-esteem and hope levels of patients with facial burn#br#

Objective To analyze the relationship between disability acceptance, self-esteem level and hope level of facial burn patients. Methods A total of 100 patients with facial burns admitted to West China Hospital, Sichuan University from January 2018 to January 2021 were prospectively selected as the research objects. The Rosenberger self-esteem scale (RSES) score was used to evaluate the level of self-esteem, while acceptance of disability scale (AODS) score was used to evaluate the disability acceptance. Abaseline information questionnaire to ask was designed and the patient’s baseline information in detail was recorded. The AODS scores of facial burn patients with different data characteristics was compared, and the relationship between the level of self-esteem and disability acceptance of facial burn patients was analyzed. Results The AODS score of 100 facial burn patients was (63.06±11.09) points. There was a statistically significant difference in the AODS scores of facial burn patients with different hope levels and self-esteem levels (P < 0.05). Low hope level (X1) and low self-esteem level (X2) were related to the low disability acceptance of facial burn patients (P < 0.05). The equation was obtained by multiple linear regression analysis: Y = 47.470+5.261X1+3.110X2. Conclusion Facial burn patients have low disability acceptance, which may be related to low self-esteem and low hope levels.

Analysis on influencing factors of fear of disease progression in patients with gastric cancer after operation#br#

Objective To observe the fear of disease progression in patients with gastric cancer after operation and its influencing factors. Methods A total of 105 patients who underwent laparoscopic radical gastrectomy for gastric cancer who were admitted to the Second Hospital of Chaoyang City, Liaoning Province from December 2018 to December 2020 were selected as the research objects. Three months after surgery, the Fear of Disease Progression Simplified Scale (FoP-Q-SF) was used to assess the degree of fear of disease progression, and the patients were divided into high-degree and low-degree groups according to the degree of fear of disease progression. The patient baseline data questionnaire was designed, the baseline data of the two groups was compared, and the fear of disease progression in patients after gastric cancer surgery and related influencing factors was analyzed. Results Among 105 patients with gastric cancer after surgery, a total of 18 had a high degree of fear of disease progression, accounting for 17.14%. The high-degree group had a higher education level than the low-degree group, and the brief illness perception questionaire score, general self efficacy scale (GSES) score, and family care index score were lower than the low-degree group, and the differences were statistically significant (P < 0.05). Logistic regression analysis showed that high disease awareness, high GSES score, and high family care index score were protective factors for fear of disease progression in patients after gastric cancer surgery (OR < 1, P < 0.05). Conclusion Patients after gastric cancer surgery are generally afraid of disease progression, which may be affected by the patient’s disease awareness, self-efficacy, and family care.

Analysis of influencing factors of airway injury in patients undergoing video-assisted thoracoscopic surgery for esophageal cancer#br#

Objective To analyze the related factors of airway injury in patients undergoing video-assisted thoracoscopic surgery for esophageal cancer. Methods A retrospective analysis of the clinical data of patients treated by video-assisted thoracoscopic surgery in the Thoracic Hospital of Shanghai Jiaotong University from October 2018 to February 2021. According to the presence or absence of airway injury during the operation, they were divided into an occurrence group (68 cases) and a non-occurring group (68 cases). The medical records were cherked, and the patient’s baseline data, intraoperative conditions, and postoperative pathological results were recorded. The high-risk factors of intraoperative airway injury in patients undergoing video-assisted thoracoscopic surgery for esophageal cancer were analyzed. Results The proportion of smoking history and simple general anesthesia in the occurrence group were higher than those in the non-occurring group, and the air bag pressure value and the time of one-lung ventilation were higher than those in the non-occurring group, and the differences were statistically significant (P < 0.05). The history of smoking, simple general anesthesia, high balloon pressure, and long one-lung ventilation time were risk factors for airway injury in patients undergoing video-assisted thoracoscopic surgery for esophageal cancer (OR > 1, P < 0.05). Conclusion Intraoperative airway injury in patients undergoing video-assisted thoracoscopic surgery for esophageal cancer may be affected by smoking history, anesthesia methods, balloon pressure values, and one-lung ventilation time.

Influencing factors of compliance behavior of primiparas in puerperium#br#

Objective To analyze the influencing factors of primipara’s compliance behavior during the puerperium. Methods A total of 108 primiparas who delivered in Fuyang Hospital Affiliated to Anhui Medical University (“our hospital” for short) from January 2018 to December 2020 were prospectively selected as the research subjects. All primiparas were evaluated by our hospital’s self-made compliance behavior scale at six weeks postpartum, and a baseline data filling form was designed to record the relevant baseline data of primiparas. The scores of primiparas’ compliance behaviors with different data characteristics was compared, and the related factors that may affect the puerperium’s compliance behaviors of primiparas was analyzed. Results The compliance behaviors of 108 primiparas women in the puerperium were different in different degrees, and the scores of compliance behavior scale were (60.74±8.27) points. There were statistically significant differences in the puerperium compliance behavior scores of primiparas with different educational backgrounds, social support, family monthly income, and self-efficacy (P < 0.05). Junior high school education and below (X1), low social support (X2), monthly family income (X3) <3000 yuan, and poor self-efficacy (X4) were the influencing factors leading to low compliance behaviors of primiparas during the puerperium (P < 0.05). The equation was obtained by multiple linear regression analysis: Y = 70.500-2.382X1-2.240X2-3.500X3-1.760X4. Conclusion Junior high school education or below, low social support, family monthly income <3000 yuan, and poor self-efficacy can lead to low compliance behaviors of primiparas during the puerperium.

Evaluation of bonding strength of selective laser sintering porous pile based on micro-extruded test#br#

Objective To discuss the bonding strength of Ti6Al4V porous pile made by selective laser sintering, and to provide theoretical basis for improving the bonding strength of pile core. Methods Preformed fiber piles were scanned and digital models were obtained. Ti6Al4V solid piles and porous piles were made by selective laser sintering technology, and zirconia piles were made by CAD-CAM technology. Different piles were bonded with post core resin to obtain micropush-out specimens. Micro-extrusion specimens were divided into solid pile group, porous pile group, and zirconia pile group according to material and shape, with 10 pieces in each group. The differences of bonding strength and failure of the three groups were compared by micro-extruding test. Results The bonding strength of solid pile group and porous pile group was higher than zirconia pile group, and the porous pile group was higher than solid pile group, the differences were statistically significant (P < 0.05). There were statistically significant differences in damage among the three groups (P < 0.05). Further pairwise comparison showed that the failure rate of adhesive interface of porous pile group was lower than that of zirconia pile group, the failure rate of internal adhesive was higher than that of zirconia pile group, and the failure rate of mixed adhesive was higher than that of zirconia pile group, with statistical significance (P < 0.017). Conclusion The adhesive strength of Ti6Al4V porous pile and solid pile made by selective laser sintering is obviously improved compared with that of zirconia pile and post core resin.

Effect of Jianpi Shenshi Decoction on pancreatic β cell in type 2 diabetes mellitus with dawn phenomenon #br#

Objective To investigate the effect of Jianpi Shenshi Decoction on type 2 diabetes mellitus with dawn phenomenon and its effect on the function of pancreatic β cell. Methods A prospective randomized controlled study was conducted to select 90 patients with type 2 diabetes mellitus complicated with dawn phenomenon from August to December 2020 in the Department of Endocrinology, Guangdong Provincial Hospital of Traditional Chinese Medicine. They were divided into two groups according to the random number table method, with 45 cases in each group. The control group was treated with Metformin in addition to the original hypoglycemic treatment, while study group was treated with Jianpi Shenshi Decoction in addition to the original hypoglycemic treatment, and the treatment lasted for three months. The fasting blood glucose (FBG), 2h-postprandial blood glucose (2hBG), blood glucose before sleeping, blood glucose in the morning, coefficient of variation (CV), and haemoglobin A1c (HbA1c) levels were monitored; the HOMA insulin resistance index (HOMA-IR) and homeostasis model assessment-β (HOMA-β) were measured by homeostasis assessment model to evaluate pancreatic β cell function. Results After three months of treatment, FBG, 2hBG, and CV in control group were lower than those before treatment; FBG, 2hBG, blood glucose before sleeping, and CV in study group were lower than those before treatment, and the differences were statistically significant (P < 0.05). After three months of treatment, FBG, 2hBG, and CV in study group were lower than those in control group, and the differences were statistically significant (P < 0.05). After three months of treatment, HbA1c in both groups were lower than those before treatment, and study group was lower than control group, and the differences were statistically significant (P < 0.05). After three months of treatment, HOMA-IR in both groups were lower than those before treatment, and HOMA-β in both groups were higher than those before treatment, and the differences were statistically significant (P < 0.05). HOMA-IR in study group was lower than that in control group, and HOMA-β was higher than that in control group, and the differences were statistically significant (P < 0.05). Conclusion Jianpi Shenshi Decoction can improve the function of pancreatic β cell of type 2 diabetes mellitus patients with dawn phenomenon, which is beneficial to maintain the stability of morning blood glucose.

Influencing factors of unplanned withdrawing machine in patients with severe CRRT in ICU#br#

Objective To observe the influencing factors of unplanned withdrawing of patients with continuous renal replacement therapy (CRRT) in the intensive care unit (ICU). Methods The clinical data of 80 CRRT patients admitted to the hospital from January 2017 to January 2020 were retrospectively analyzed, and baseline data were collected. According to the occurrence of unplanned withdrawing, patients were divided into unplanned and planned withdrawing units, and logistic regression analysis was used to test the influencing factors of unplanned disengagement. Results Among 80 CRRT patients, a total of 35 cases (43.75%) were unplanned withdrawing, and another 45 cases (56.25%) were planned withdrawing. The blood flow rate of the unplanned withdrawing unit was lower than that of the planned withdrawing unit, and the volume ratio of venous ampulla embolism, hematocrit, therapeutic dose, and prothrombin time activity (PTA) were higher than those of the planned unit, and the differences were statistically significant (P < 0.05). Logistic regression analysis showed that slow blood flow, high volume ratio of venous ampulla embolism, high hematocrit, high therapeutic dose, and high PTA were the risk factors for unplanned withdrawing of ICU patients with severe CRRT (OR > 1, P < 0.05). Conclusion The risk of unplanned withdrawing machine in patients with severe CRRT in ICU is high, the risk factors of unplanned withdrawing machine include slow velocity of blood flow, high volume ratio of jugular embolization, high packet cell volume, high therapeutic dose and high PTA.

Biomechanical evaluation of internal fixation combined with injured vertebrae cement reinforcement in the treatment of osteoporotic thoracolumbar burst fractures#br#

Objective To evaluate the stability of internal fixation combined with injured vertebrae cement reinforcement in the treatment of osteoporotic thoracolumbar burst fracture, and to provide reference for clinical application. Methods Six elderly thoracolumbral (T11-L3) spinal specimens were collected by Shanghai University of Traditional Chinese Medicine from January 2018 to June 2019 to prepare L1 vertebral body burst fracture model. Then biomechanical tests were performed in the order of complete state (blank group), fracture state (fracture group), 6-screw internal fixation state (6-screw group), 6-screw internal fixation combined with injured vertebrae cement reinforcement state (6-screw + cement group), and 4-screw internal fixation combined with injured vertebrae cement reinforcement state (4-screw + cement group). The specimens in each group were tested in different stages of flexion, extension, left and right bending and axial compression respectively. The deformation images of the specimens were collected by CCD camera. Based on Image J software, digital marker point centroid tracking method was used to measure the vertical displacement of T12 on the coronal plane to reflect the stiffness of the fixed segment, and the comparison was made. Results Under axial compression, flexion, extension, and left and right bending loading modes, the specimen stiffness of fracture group was significantly lower than that of blank group, 6-screw group, 4-screw + cement group, and 6-screw + cement group, with statistically significant differences (P < 0.05). The stiffness of specimens in the 6-screw group was higher than that in the blank group under extension and left and right bending loading modes, and the stiffness of specimens in the 6-screw group was lower than that in the blank group under axial compression and forward bending loading modes, with statistical significance (P < 0.05). Under axial compression, extension, and left and right bending loading modes, the stiffness of specimens in the 4 screw + cement group and 6 screw + cement group were higher than those in the blank group and 6 screw group, and the 6 screw + cement group was higher than the 4 screw + cement group, the differences were statistically significant (P < 0.05). In flexion loading mode, the stiffness of specimens in the 4 screws + cement group was lower than that of the blank group but higher than that of the 6 screws group, and the 6 screws + cement group was higher than those of the blank group and the 6 screws group, with statistical significance (P < 0.05). Conclusion The three fixation methods can achieve the stability of the fixed segment, but the 6-screw internal fixation combined with injured vertebrae cement reinforcement has the strongest stability and the best biomechanical performance.

Effect of sling exercise training on surface electromyography of lumbar and abdominal muscles in patients with nonspecific low back pain#br#

Objective To investigate the effect of sling exercise training (SET) on the surface electromyography of lumbar and abdominal muscles in patients with nonspecific low back pain (NLBP). Methods A total of 150 NLBP patients admitted to Nanping First Hospital of Fujian Province from September 2018 to August 2020 were divided into observation group and control group according to random number table method, with 75 cases in each group. Control group was given bridge exercise, observation group was given sling exercise training. Both groups trained six times a week for three weeks. Visual analogue scale (VAS) score, Japanese Orthopaedic Association (JOA) low back pain score, Oswestry disability index (ODI index), root mean square (RMS) and median frequency (MF) values of erector spinae, multifidus muscle and rectus abdominis were compared between the two groups before and after training. Results Compared with before training, VAS score, ODI index, erector spinae MF, multifidus muscle MF and rectus abdominis MF decreased after training (P < 0.05), while JOA score, erector spinae RMS, multifidus muscle RMS and rectus abdominis RMS increased after training (P < 0.05). After training, VAS score, ODI index, erector spinae MF, multifidus muscle MF and rectus abdominis MF in the observation group were lower than those in the control group (P < 0.05), while JOA score, erector spinae RMS, multifidus muscle RMS and rectus abdominis RMS in the observation group were higher than those in the control group (P < 0.05). There were no exercise-treatment-related complications in both groups. Conclusion Compared with bridge exercise, sling exercise training can effectively improve lumbar abdominal muscle strength, reduce muscle group fatigue, relieve clinical symptoms, and improve lumbar function in NLBP patients.

Meta-analysis of the effect of sling exercise training on lower limbs motor function of children with cerebral palsy#br#

Objective To systematically evaluate the effect of sling exercise training (SET) on lower limbs motor function in children with cerebral palsy (CP). Methods Clinical randomized controlled trials of SET in the treatment of children with CP were searched in CNKI, Wanfang Data, VIP, CBM, PubMed, EMbase, The Cochrane Library, Web of Science, and Science Direct databases, retrieval time was self-built database to August 2020. Two researchers screened the literature according to inclusion and exclusion criteria, extracted data and evaluated the quality of the included literature. RevMan 5.1 software was used to analyze the extracted data. Results A total of 18 articles and 1499 patients were included. The score of gross motor function measure scale (GMFM)-D area in the experimental group were higher than those in the control group, and the difference was statistically significant (WMD = 4.36, 95%CI: 3.06, 5.67, P < 0.000 01). The score of GMFM-E area in the experimental group was higher than that in the control group, and the difference was statistically significant (WMD = 5.89, 95%CI: 4.33, 7.45, P < 0.000 01). The maximum walking speed in the experimental group was higher than that in the control group, and the difference was statistically significant (WMD = 3.04, 95%CI: 2.29, 3.80, P < 0.000 01). The root mean square (RMS) value of adductor surface electromyogram signal in the experimental group was lower than that in the control group, and the difference was statistically significant (WMD = -2.23, 95%CI: -3.24, -1.22, P < 0.0001). The RMS value of gastrocnemius surface electromyogram signal in the experimental group was lower than that in the control group, and the difference was statistically significant (WMD = -2.42, 95%CI: -3.36, -1.48, P < 0.000 01). Conclusion SET can effectively improve lower limbs motor function, improve walking speed and reduce lower limbs muscle tension in CP children, but high-quality, large-sample randomized controlled studies are still needed for further demonstration.

Analysis of laparoscopic surgery for ovarian endometriosis cyst with follicular tissue#br#

Objective To analyze the effect of laparoscopic operation in the treatment of ovarian endometriosis cyst with follicular tissue. Methods Eighty patients with ovarian endometriosis cyst with follicular tissue treated in Bozhou Hospital of Traditional Chinese Medicine, Anhui Province from June 2017 to October 2020 were selected as research subjects. According to different treatment methods, they were divided into control group and study group, with 40 cases in each group. The control group was treated with laparotomy and the study group with laparoscopy. Before and after operation, serum follicle stimulating hormone (FSH), antral follicle count (AFC), estradiol (E2), anti-mullerin hormone (AMH), intraoperative blood loss, operation time, exhaust time, length of hospital stay, total incidence of complications, and recurrence rate half a year after operation of two groups were recorded and compared. Results After operation, the level of FSH in both groups were higher than those before operation, while the level of AFC was lower than that before operation, and the differences were statistically significant. After operation, the level of FSH in study group was lower than that in control group, and the difference was statistically significant (P < 0.05). There was no significant difference in AFC level between two groups (P > 0.05). After operation, the levels of E2 and AMH in two groups were lower than those before operation, and study group was lower than control group, and the differences were statistically significant (P < 0.05). The the amount of intraoperative blood loss was lower than that of control group, and operation time, exhaust time, and length of hospital stay of study group were shorter than those of control group, and the differences were statistically significant (P < 0.05). The total incidence of complications in study group was lower than that in control group, and the difference was statistically significant (P < 0.05). There was no significant difference in recurrence rate between two groups (P > 0.05). Conclusion Laparoscopic operation for patients with ovarian endometriosis cyst with follicular tissue has little impact on the follicular tissue, and will not cause follicular loss, and can effectively reduce the total incidence of operation complications and recurrence rate, which has clinical application value and is worth promoting.

Clinical effect and safety of BiPAP combined with pulmonary surfactant in the treatment of neonatal respiratory distress syndrome#br#

Objective To investigate the clinical effect and safety of bilevel positive airway pressure (BiPAP) combined with pulmonary surfactant (Poractant Alfa Injection [Curosurf]) in the treatment of neonatal respiratory distress syndrome (NRDS). Methods A total of 80 children with NRDS in Xuancheng People's Hospital of Anhui Province from August 2017 to December 2020 were selected. According to whether or not Curosurf was applied, they were divided into two groups, control group (40 cases) was treated with BiPAP, and observation group (40 cases) was treated with BiPAP combined with Curosurf. The clinical efficacy of two groups were compared, and the high pressure level (p-high), low pressure level (p-low), fraction of inspiration O2 (FiO2), arterial partial pressure of oxygen (PaO2), arterial partial pressure of carbon dioxide (PaCO2) and Oxygen therapy time, non-invasive ventilation time, clinical symptom relief time, and hospital stay of two groups were compared before treatment and two days after treatment. Results The total effective rate of observation group was higher than that of control group, and the difference was statistically significant (P < 0.05). After two days of treatment, P-high, FiO2 in observation group were lower than those before treatment, p-low in two groups were lower than those before treatment, P-high, P-low and FiO2 in observation group were lower than those in control group, and the differences were statistically significant (P < 0.05). After two days of treatment, PaO2 in two groups were higher than those before treatment, PaCO2 in two groups were lower than those before treatment, PaO2 in observation group was higher than that in control group, PaCO2 was lower than that in control group, and the differences were statistically significant (P < 0.05). The oxygen therapy time, non-invasive ventilation time, clinical symptom relief time and hospital stay in observation group were shorter than those in control group, and the differences were statistically significant (P < 0.05). Conclusion BiPAP combined with Curosurf has a significant effect on the treatment of NRDS, which can reduce mechanical ventilation parameters and promote the recovery of children with high safety.

Clinical application effect of lacrimal canaliculi fracture anastomosis combined with artificial RS-lacrimal intubation in pediatric patients#br#

Objective To observe the clinical effect of lacrimal canaliculi fracture anastomosis combined with artificial RS-lacrimal intubation implantation in pediatric patients. Methods A total of 138 pediatric patients who underwent lacrimal canaliculi fracture anastomosis combined with artificial RS-lacrimal intubation in Department of Ophthalmology, the Fourth People’s Hospital of Shenyang from January 2015 to January 2020 were collected. Extubation was performed nine months after operation, followed up three months after extubation to observe the surgical results. Results Among the 138 patients, 133 patients were cured, with a success rate of 96.38%. During the follow-up, the lacrimal duct was flushed unobobstructed, the position of lacrimal petits was normal, and there was no obvious ectopion of the eyelid and no symptom of lacrimatorrhea. One patient removed the drainage tube by himself one month after the operation, and the follow-up after the second operation reached the cure standard. One patient showed prolapse of the drainage tube six months after operation, but reached the cure standard at three months follow-up. Nine months after extubation, the lacrimal passages of three patients were flushed but not smooth, with mild lacrimatorrhea. Conclusion The artificial RS-lacrimal intubation has high success rate, does not interfere with the nasal cavity of patients, reduces the pain of patients, has less postoperative complications and simple tube drawing. It is an ideal support for children’s lacrimal canaliculi fracture, and is worthy of clinical promotion.

Objective To investigate the effect of Minocycline Hydrochloride Ointment combined with Tinidazole on the oxidative stress response and level of inflammatory factors in gingival crevicular fluid in patients with chronic periodontitis. Methods A total of 120 patients with chronic periodontitis admitted to Staff Hospital of Luoyang North Enterprise Group Co., LTD from May 2017 to May 2020 were selected and divided into treatment group and control group by random number table method, with 60 patients in each group. The control group was given Tinidazole Tablets, and the treatment group was given Minocycline Hydrochloride Ointment on this basis. Both groups were treated for two months. The efficacy, and the periodontal index, oxidative stress index, gingival crevicular fluid inflammatory factor level before and after treatment and the incidence of adverse reactions during treatment were compared between the two groups. Results The total effective rate in treatment group was higher than that in control group, and the difference was statistically significant (P < 0.05). After treatment, periodontal pocket depth, plaque index, gingival index, serum malondialdehyde, gingival crevicular fluid monocyte chemotactic protein-1, matrix metalloproteinase-8, and interleukin-6 levels were all lower than those before treatment, while serum glutathione peroxidase and superoxide dismutase were all higher than those before treatment, and periodontal pocket depth, dental plaque index, gingival index, serum malondialdehyde, gingiva crevicular fluid monocyte chemotactic protein 1, matrix metalloproteinases-8, and interleukin-6 levels in the treatment group were lower than those in the control group, serum glutathione peroxidase and superoxide dismutase were higher than those in the control group, the differences were statistically significant (P < 0.05). There was no significant difference in the total incidence of adverse reactions between the two groups (P > 0.05). Conclusion Minocycline Hydrochloride Ointment combined with Tinidazole can reduce inflammation and oxidative stress response in patients with chronic periodontitis, improve periodontal index, improve curative effect, and have good safety.

Clinical effect of Yiqi Chupi Decoction combined with Western medicine in the treatment of newly diagnosed reflux esophagitis of spleen deficiency and stagnation type#br#

Objective To explore the clinical effect of Yiqi Chupi Decoction combined with Western medicine in the treatment of newly diagnosed reflux esophagitis (RE) of spleen deficiency and stagnation type. Methods A total of 129 newly diagnosed RE patients admitted to Zhangjiagang Hospital of Traditional Chinese Medicine from September 2018 to September 2020 were selected and they were divided into two groups by random number table method. The control group (64 cases) was treated with conventional Western medicine, and the study group (65 cases) was treated with Yiqi Chupi Decoction on the basis of the control group. The therapeutic effect, traditional Chinese medicine syndrome score, serum indexes (motilin [MOT], gastrin [GAS], tumor necrosis factor-α [TNF-α], interleukin-6 [IL-6]) and adverse reactions were compared between the two groups. Results After eight weeks of treatment, the total effective rate of the study group was higher than that of the control group (P < 0.05). After eight weeks of treatment, the symptom scores of acid regurgitation, belching, epigastric distension, anorexia, heartburn and burnout in the two groups decreased compared with before treatment, and the symptom scores of the study group were lower than those of the control group (P < 0.05); the levels of MOT and GAS increased than before treatment, and those of the study group were higher than those of the control group (P < 0.05); the levels of TNF-α and IL-6 decreased than before treatment, and those of the study group were lower than those of the control group (P < 0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P > 0.05). Conclusion Yiqi Chupi Decoction combined with Western medicine in the treatment of RE with significant curative effect and good safety. Its mechanism of action may be related to the regulation of inflammatory indicators and gastrointestinal hormones.

“Ten methods of phase and qi” also known as the ten methods of wangqi, comes from Wang Hong’s Wangzhen Zunjing. This method by examining the qi, color, shape and state of each part of the body, and combined with the symptoms, four seasons, orientation, constitution, pulse manifestation, and so on to identify the disease outside and inside, yin-yang, excess-deficiency, improvement and deterioration, straight and reverse. As an important content of inspection, “ten methods of phase and qi” plays a guiding role in the application of clinical color diagnosis. In this paper, the origin and connotation of “ten methods of phase and qi” and the connection and practical application of this method with facial diagnosis, hand diagnosis, tongue diagnosis and other specific color diagnosis of traditional Chinese medicine are analyzed and summarized in order to provide new ideas for clinical diagnosis of traditional Chinese medicine.

Modern clinical application of abdominal diagnosis in children#br#

Abdominal diagnosis is an important method for disease diagnosis, which can more directly and objectively distinguish etiology, pathogenesis and location. Abdominal diagnosis is also one of the effective means to guide principle-method-recipe-medicines and treat diseases, and it has more important practical significance for pediatric clinical practice. Abdominal diagnosis should be performed quickly, accurately, and in any position in pediatrics. It is emphasized to distinguish deficiency and excess, cold and heat, qi and blood and verify the essence of syndrome from abdominal shape, abdominal skin and abdominal strength. At the same time, the combination of abdominal diagnosis and abdominal external treatment is beneficial to each other. Three cases of functional abdominal pain, precocious puberty and tourette syndrome in children are reported, which proves that abdominal diagnosis is an important basis for clinical syndrome differentiation and opens up a new idea of abdominal external treatment in pediatrics.

Rules of clinical acupoint selection for acupuncture in the treatment of functional dyspepsia based on data mining#br#

Objective To analyze the characteristics and rules of clinical acupoint selection for acupuncture in the treatment of functional dyspepsia based on data mining. Methods The clinical literatures on acupuncture in the treatment of functional dyspepsia from CNKI, VIP and Wanfang self-built database up to March 2021 were searched, and the acupoint prescription database was established. SPSS Statistics 26.0 and SPSS Modeler 18.0 were used to conduct descriptive analysis, cluster analysis and association rule analysis of the data. Results A total of 111 clinical literatures were included, involving 56 acupoints with a total frequency of 627 times, of which the most frequent ones were zúsānlǐ, zhōngwǎn, nèiguān, tàichōng, tiānshū. Acupoint meridians were mainly distributed in stomach meridian, conception channel and bladder meridian. Cluster analysis showed that it could be divided into four groups and the association rule analysis showed that the highest correlation between acupoints was zhōngwǎn-zúsānlǐ, followed by nèiguān-zúsānlǐ. Conclusion Acupuncture in the treatment of functional dyspepsia has the characteristics of attaching importance to local acupoint selection and combined with distal acupoint selection. It can provide some reference for clinical acupuncture in acupoint selection.

Detailed explanation of the standardized operation of “six forms of taijiquan” for improving cardiopulmonary function#br#

Chronic disease has become a major factor affecting the quality of life of Chinese residents. The rehabilitation of chronic diseases is closely related to cardiopulmonary function. Low levels of cardiopulmonary function can not only affect the cognitive and motor function of patients with chronic diseases, but also negatively affect the growth of adolescents and working ability of middle-aged people. Although taijiquan can improve the cardiopulmonary function, the level of evidence is low. The movements of 24-type simplified Tai taijiquan complex, which are unfavourable for practice. Therefore, six forms of taijiquan are selected from the 24-type simplified taijiquan and adapted to improve the cardiopulmonary function. According to the correlation and difficulty of the movements, the six forms of taijiquan are sorted into the following categories: commencing form, wave hands like clouds, repulse monkey, part the wild horse’s mane on both side, hold the lute, closing form. The adapted taijiquan is named “six forms of taijiquan”, which has the effect of widening the chest and relieving knots, dredging the meridians, calming the heart and mind. Because of “six forms of taijiquan” need shorter time and smaller space, it is applied to the cardiopulmonary dysfunction of chronic disease patients, working populations, college students, etc.

Clinical experience of Professor Fan Ruiqiang in treating eczema from wind and dampness theory#br#

This paper introduces Professor Fan Ruiqiang’s experience in curing eczema, which is one of the most common skin disease. Professor Fan Ruiqiang puts forward that eczema is a disease caused by the combination of wind and dampness. Professor Fan Ruiqiang enriches the connotation of wind and dampness, and points out that the release of wind open the way for the invasion of harmful insects and unclean things from the outside. At the same time, the dampness viscosity makes the pathogenic substances stagnant on the muscle surface and fill the skin. Both of them work together to cause the invasion of the guard surface and induce eczema. Professor Fan Ruiqiang points out that wind and dampness are mixed, and they fight and compete with each other, which further intensifies, and then either heat, fire, blood or yin, forming various syndrome changes. The common syndrome types in the early and progressive stage of the disease are rheumatism trapped in the surface and excessive damp heat and toxin. The common syndrome types in the chronic stage are spleen deficiency and dampness accumulation, blood deficiency and wind dryness, yin deficiency and wind dryness. In terms of treatment, we should pay attention to the method of eliminating wind and dehumidification on the basis of syndrome differentiation and treatment, and integrate different means such as oral administration of Chinese medicine, external treatment of Chinese medicine and Western medicine, so as to obtain good results.

Experience summary of Wang Weichang in the treatment of gynecologic disease by warming method#br#

Wang Weichang, a famous doctor of Longjiang medical faction, has rich clinical experience. He is good at using warming method to treat gynecologic disease especially, and the clinical efficacy is obvious. Wang Weichang believes that endometriosis is inseparable from pain, cold, blood stasis and phlegm. Warming channel for dispelling cold, removing blood stasis and dredging meridians should be carried out throughout the treatment. He uses prepared drug in pieces with the effect of warming channel for dispelling cold and simultaneous treatment of water and blood in most of premenstrual treatment, so as to promote the excretion of phlegm and blood stasis. He pays attention to drastically expel obstinate cold in postmenstrual treatment, and simultaneously applies with activating blood and resolving stasis drugs. Not all polycystic ovarian syndromes belong to amenorrhea with syndrome of phlegm stasis, most of them are caused by deficiency of tian gui. Treatment should warm and nourish essence, and benefit innate, Erxian Decoction and Wuzi Yanzong Pills are usually used. Wang Weichang uses the theory of cold coagulation, qi stagnation and blood stasis to treat primary infertility, and he uses Shaofu Zhuyu Decoction to warm channel for dispelling cold, disperse blood stasis and dredge meridian. Deficiency of qi and blood leads to the drying up of blood vessels, resulting in stagnation due to deficiency, thus blocking the breast collaterals, which can also lead to hyperplasia of mammary glands. Treatment should use warm moisturizing replenishing blood drugs, not use aggressive drugs. He advocates warming and replenishing belt channel for the treatment of flatus vaginalis, and uses modified Buzhong Yiqi Decoction and Shuilu Erxian Dan commonly. The above-mentioned clinical understanding of Professor Wang Weichang is unique and his practice has definite clinical efficacy, which can provide help for gynecological clinical treatment.

Analysis on the medication rule of traditional Chinese medicine in the treatment of infantile anorexia#br#

Objective To analyze the medication rule of traditional Chinese medicine in the treatment of infantile anorexia based on data mining technology, to provide basis for clinical medication. Methods The prescriptions for the treatment of infantile anorexia in CNKI from January 1, 2016 to December 31, 2020 were collected, and the frequency statistics of prescriptions were conducted by traditional Chinese medicine inheritance support system V2.5. The medication rule was summarized through compositional analysis of prescriptions and new prescriptions analysis. Results There were 123 prescriptions, with a total of 170 Chinese herbs. Among them, the top five Chinese herbs in frequency were Pericarpium Citri Reticulatae, Poria, Radix Glycyrrhizae, parched Fructus Crataegi, Endothelium Corneum Gigeriae Galli. The main flavour was sweet, acrid, and bitter, the main nature was warm, flat, and cold. Most of these drugs belonged to channel tropisms of spleen meridian, stomach meridian, lung meridian, and liver meridian. And 26 groups of drug combination, 20 groups of association rules and four new prescriptions were obtained. Conclusion At present, the therapeutic principles of traditional Chinese medicine in the treatment of infantile anorexia are mainly invigorating spleen and stomach, benefiting qi and invigorating spleen, dispersing liver and regulating qi. The application of traditional Chinese medicine inheritance support system for analyzing the medication rules of traditional Chinese medicine in the treatment of infantile anorexia can provide a theoretical basis for clinical medication.

Objective To explore the mechanism of Hirudo-Scolopendra couplet medicines in the treatment of diabetic erectile dysfunction (DED) by using network pharmacology method. Methods The relevant ingredients of Hirudo-Scolopendra couplet medicines were obtained by searching relevant literatures from January 1, 2017 to February 10, 2020 in CNKI. Swiss Target Prediction data platform was used to screen the main effective components and targets of Hirudo-Scolopendra couplet medicines; Cytoscape 3.8 was used to draw the drug-component-target network; OMIM and Drugbank dataBases were used to retrieve the related targets of diabetes mellitus (DM)-erectile dysfunction (ED); Venn diagram was used to obtain the common targets of Hirudo-Scolopendra couplet medicines, DM, and ED; protein-protein interaction network of components-disease common targets were constructed by using STRING platform; gene ontology (GO) enrichment analysis and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment analysis were performed by using Metascape database. Results A total of 47 active ingredients and 534 related targets were obtained from Hirudo-Scolopendra couplet medicines, DM and ED related targets were 736 and 743, respectively, and this couplet medicines had 32 targets for regulating DM and ED, and three core common targets were obtained by screening again; GO enrichment analysis showed that there were 14 biological processes and four molecular function. KEGG pathway enrichment analysis showed that there was a total of one core signaling pathway, which was the cancer pathway. Conclusion Hirudo-Scolopendra couplet medicines may treat DED by acting on AR, ESR1 through cholesterol, linolenic acid and other components, so as to restore erectile function.

[Abstract] Objective To explore the application value of enhanced recovery after surgery (ERAS) concept nursing in the surgical treatment of developmental dysplaisa of the hips (DDH) in children. Methods A total of 94 children with DDH who underwent surgery in Anhui Children’s Hospital from June 2018 to December 2019 were prospectively and randomly selected, and they were divided into control group and ERAS group according to different perioperative care methods. The differences of surgery-related indexes, thirst score, hunger score, pain score and satisfaction were compared between the two groups. Results The length of hospital stay and catheter removal time in ERAS group were shorter than those in control group, with statistically significant differences (P < 0.05). Thirst score, hunger scores in two groups were lower than before surgery and the ERAS group were lower than control group, the differences were statistically significant (P < 0.05). Overall analysis showed that there were statistically significant differences between groups, time points and interactions (P < 0.05). Further pairings within the group showed that compared with one hour after surgery, the pain scores of the two groups increased at six hour after surgery, with statistical significance (P < 0.05). Compared with six hour after surgery, the pain score of the two groups decreased 24 h after surgery, with statistical significance (P < 0.05). Compared between groups, the pain scores of patients in ERAS group at 1, 6 and 24 h after surgery were lower than those in control group, with statistical significance (P < 0.05). The scores of parental emotional and psychological support, surgical procedure arrangement and nursing treatment in ERAS group were higher than those in the control group, and nursing satisfaction was higher than that in the control group, with statistical significance (P < 0.05). Conclusion ERAS’concept of nursing can reduce thirst and hunger in children with DDH during perioperative period, alleviate postoperative pain, shorten hospital stay and improve parents’ satisfaction, which is worthy of clinical promotion.

Practical study on outcome oriented integrated zero defect nursing mode in emergency treatment of acute myocardial infarction#br#

Objective To explore the effect of the integrated zero defect nursing mode guided by outcome in the emergency treatment of acute myocardial infarction. Methods A total of 102 patients with acute myocardial infarction admitted to the First Affiliated Hospital of Air Force Military Medical University from July 2019 to May 2020 were divided into study group and control group by random number table method, with 51 cases in each group. The control group adopted routine nursing mode, and the research group adopted outcome oriented integrated zero defect nursing mode. The pre-hospital admission time, triage evaluation time, emergency room treatment time, intensive care unit (ICU) stay time, success rate of emergency treatment, incidence of short-term poor prognosis and nursing satisfaction were compared between the two groups. Results The pre-hospital admission time, triage evaluation time, emergency room treatment time and ICU stay time in the study group were shorter than those in the control group (P < 0.05). The success rate of emergency treatment in the study group was higher than that in the control group (P < 0.05), and the incidence of short-term poor prognosis in the study group was lower than that in the control group (P < 0.05). There was significant difference in the distribution of nursing satisfaction between the two groups (P < 0.05). Conclusion In the emergency treatment of acute myocardial infarction, the application of outcome oriented integrated zero defect nursing can shorten the reception time, triage time and treatment time, improve the success rate of emergency treatment, reduce short-term adverse events, and improve nursing satisfaction.

Application effect of multidisciplinary cooperative exercise intervention in patients with acute leukemia in laminar flow ward#br#

Objective To explore the effect of exercise intervention on patients with acute leukemia in laminar flow ward by multidisciplinary collaboration of medical treatment, nursing and rehabilitation, and their effects on cancer-related fatigue and negative emotions. Methods A total of 80 patients with acute leukemia admitted to the laminar flow ward of the Affiliated Huai’an No.1 People’s Hospital of Nanjing Medical University from January to December 2019 were selected and they were divided into control group (40 cases) and observation group (40 cases) by random envelope method. The control group carried out routine nursing, and the observation group carried out multidisciplinary exercise intervention on the basis of routine nursing. After four weeks, the scores of cancer-related fatigue and anxiety and depression were compared between the two groups. Results There was no significant difference in the score of cancer-induced fatigue in the observation group before and after intervention (P > 0.05). After four weeks of intervention, the score of cancer-induced fatigue in the control group was increased than before treatment (P < 0.05), while that in the observation group was lower than that in the control group (P < 0.05). After four weeks of intervention, the scores of anxiety and depression in both groups decreased than before treatment, and the observation group was lower than the control group (P < 0.05). Conclusion Multidisciplinary collaborative exercise intervention can significantly reduce the symptoms of cancer-induced fatigue and relieve the negative emotions of patients with acute leukemia in laminar flow ward.

Analysis on influencing factors of skin pressure injury in patients with PICC intubation#br#

Objective To observe and analyze the influencing factors of skin pressure injury in patients with peripherally inserted central venous catheters (PICC) intubation. Methods The medical data of 150 patients with PICC intubation admitted to the First Affiliated Hospital of Wannan Medical College from January to December 2020 were retrospectively analyzed, the patients were divided into the occurrence group (28 cases) and the non-occurrence group (122 cases) according to whether the pressure injury occurred of patients during intubation. The general data and laboratory indexes of the two groups were recorded and compared, through univariate and multivariate analysis, the influencing factors of pressure injury in patients with PICC intubation were found out. Results The levels of albumin and hemoglobin in the occurrence group were lower than those in the non-occurrence group, and the proportion of intubation season (winter) and catheter maintenance cycle delay were higher than those in the non-occurrence group, with statistical significant differences (P < 0.05); there were no statistical significant differences in other baseline data and laboratory indexes between the two groups (P > 0.05); the results of logistic regression analysis showed that the season of intubation and the delay of catheter cycle maintenance, albumin and hemoglobin were influencing factors of skin stress injury in PICC intubation patients (P < 0.05). Conclusion The occurrence of skin stress injury in PICC intubation patients may be affected by season of intubation, delay of catheter cycle maintenance, the low expression of albumin and hemoglobin and other factors.

Analysis of the hotspots of international home enteral nutrition research based on WOS core collection database#br#

Objective To understand the current status, basics, hotspots and frontiers of research in the field of international home enteral nutrition (HEN), and to provide reference for the development of HEN research in China. Methods The core collection database of Web of Science was used as the source, and CiteSpace software was used to analyze the literature. Results A total of 980 literatures were obtained. The United States ranks first in terms of the number of articles published and the publishing institutions; from the perspective of disciplines, it presents a trend of diversified development; research hotspots include: different research objects, family support, nursing application and complications; 12 highly cited articles; 13 prominent words, and the research covers three stages. Conclusion The HEN reserach has been widely concerned by dometic and foreign scholars, but it is necessary to strengthen cooperation, improve the research types, and promote the development of HEN in China.

Research on nursing APP in the application of continuous nursing of patients with PICC intubation#br#

Objective To analyze the application value of nursing APP in the continuous nursing of patients with peripherally inserted central venous catheters (PICC) intubation. Methods A total of 80 patients with PICC intubation admitted to the Lingang Branch of Shanghai Sixth People’s Hospital from December 2018 to December 2019 were prospectively included as the research subjects, all patients were intervened by nursing APP outside the hospital, the patients were followed up at 2, 4, and 6 months of intervention, the self-management ability (self-management ability of patients with PICC scale) and occurrence of complications of patients were compared at the above time points. Results The scores of each dimension and total score of self-management ability of patients at four months of intervention was higher than those at two months of intervention, and the scores of above each dimension and total score at six months of intervention were higher than those at four months of chemotherapy, the differences were highly statistically significant (P < 0.01); the incidence of complications at six months of intervention was lower than that at 2 and 4 months of intervention, the difference was highly statistically significant (P < 0.01), the incidence of complications at 2 and 4 months of intervention were compared, there was no statistical significant difference (P > 0.05). Conclusion The application of nursing APP in continuous nursing of patients with PICC intubation can effectively improve the self-management ability and reduce the occurrence of complications.

Application of nursing intervention based on resourcefulness theory in patients with gestational diabetes mellitus#br#

Objective To explore the application effect of nursing intervention based on resourcefulness theory in patients with gestational diabetes mellitus (GDM). Methods A total of 100 patients with GDM admitted to Nanjing First Hospital from January 2019 to January 2020 were prospectively selected, the patients were divided into control group and observation group by random number table method, with 50 cases in each group. The control group was given routine nursing intervention, and the observation group was given routine nursing combined with nursing intervention based on resourcefulness theory, they were all intervened until delivery. Fasting blood glucose, 2 h postprandial blood glucose between the two groups were detected and compared before intervention and during delivery; revised illness perception questionnaire (IPQ-R) and resourcefulness scale (RS) were used to evaluate and compare the disease perception and resourcefulness scores of the two groups; the pregnancy outcomes of the two groups during intervention were recorded and compared. Results Before intervention, there was no statistical significant difference in fasting blood glucose and 2 h postprandial blood glucose between the two groups (P > 0.05); during delivery, the levels of fasting blood glucose, 2 h postprandial blood glucose in both groups were decreased than before intervention, and the observation group was lower in comparison with the control group, and the differences were statistically significant (P < 0.05); before intervention, there was no statistical significant difference in IPQ-R and RS scores between the two groups (P > 0.05); at delivery, IPQ-R and RS scores of both groups were increased than before intervention, and the observation group was higher in comparison with the control group, and the differences were statistically significant (P < 0.05); there were no statistical significant differences in the incidence of premature birth, premature rupture of membranes, intrauterine distress and macrosomia, and other adverse pregnancy outcomes between the two groups (P > 0.05). Conclusion The application effect of nursing intervention based on resourcefulness theory in patients with GDM is better, which is more conducive to stabilizing the blood glucose of patients with GDM, improving the disease perception and resourcefulness of patients.

Comparison of megnetic suction pain assessment scale and visual analogue scale in the application of postoperative nursing of total hip arthroplasty#br#

Objective To compare the application value of magnetic suction pain assessment scale and visual analogue scale (VAS) in postoperative nursing after total hip arthroplasty (THA). Methods A total of 84 patients treated with THA in the Department of Orthopedics, Anqing Municipal Hospital from January 2018 to January 2020 were prospectively selected as the research subjects, they were divided into control group and observation group according to the odd or even number of medical records, with 42 cases in each group, the control group was used VAS in postoperative nursing; the observation group was used magnetic suction pain assessment scale in postoperative nursing; nursed until one day before discharge, the improvement of pain was compared between the two groups; before nursing and one day before discharge. The negative emotions (hospital anxiety and depression scale [HAD] ) and sleep quality (Pittsburgh sleep quality index [PSQI]) of the two groups were compared; the satisfaction of pain assessment was compared of patients in the two groups. Results One day before discharge, the pain scores, HAD and PSQI scores of both groups were lower than those before nursing, and the each score of the observation group was lower than that of the control group (P < 0.05); the total satisfaction rate of pain assessment in the observation group was higher than that in the control group (P < 0.05). Conclusion Compared with VAS, magnetic suction pain assessment scale in nursing after THA is more conducive to relieving sense of pain, reducing negative emotions and improving sleep quality of patients, which has certain significance to improve the satisfaction with pain assessment of patients.
