QIAO Zeyuan1 NI Jun2 ZHANG Jing2 XIE Guoqi2
Objective To observe the changes of early islet function in pigs with severe heat stroke. Methods Twelve healthy CV level male Bama miniature pigs were divided into control group (six pigs) and model group (six pigs) according to random number table method. The control group was placed at room temperature, and the model group was placed in the climate chamber with constant temperature and humidity. The severe heat stroke model of pigs was established under the conditions of 44-47 ℃ and relative humidity of (65±5) %. The core temperature of the model group > 42℃ was regarded as the success of modeling, that was the experiment ended. General conditions and core temperature of the two groups were monitored. Blood samples were collected from the two groups for examination after the end of the experiment to compare the changes in blood glucose and insulin levels. hematoxylin-exosin staining was used to observe the tissue morphology of the islets, liver and kidney of the two groups, and immunohistochemical staining was used to observe the expression level of insulin in the islets of the two groups. Results After the model group entered the constant temperature and humidity climate cabin, the core temperature of the model group showed an increasing trend, while the core temperature of the control group did not change. At the core temperature > 42℃, the model group showed depression of spirit, unstable gait, conjunctival congestion, and weakened or disappeared corneal reflex, while the control group showed no abnormal symptoms. There was no significant difference in blood glucose and insulin levels in the control group before and after the experiment (P > 0.05). After the experiment, the blood glucose level of the model group was higher than before, and the blood insulin level of the model group was lower than before, and the blood glucose level of the model group was higher than that of the control group, and the blood insulin level of the control group was lower than that of the control group, the difference was statistically significant (P < 0.05). After the experiment, there were no significant changes in islet, liver and kidney tissue in the control group. The islet margin of model group was not clear, the shape was irregular, the cell edema, a few granulocytes infiltration, and the blood stasis of the capillary. The renal tubules and collecting ducts were dilated, thrombosis was observed in the vessels, epithelial granules were degenerative, and a few inflammatory cells were infiltrated. The structure of hepatic lobules was disordered, the hepatic sinuses were slightly stasis and dilated, the hepatic cells were slightly edema and granular degeneration, and lymphocyte infiltration was observed. Immunohistochemical staining results showed that in the control group, a large number of positive expressions of brown insulin was observed. The cells were closely arranged and uniform with complete morphology. In the model group, the islets were less stained and irregular in shape. The average optical density of insulin in islets in model group was lower than that in control group, and the difference was statistically significant (P < 0.05). Conclusion Islet tissue damage, decreased insulin expression and elevated blood glucose occurred in early stage after severe heat stroke in pigs, which may be related to the impaired islet function.

TU Xiaolong1 ZHAO Ying2 SHI Guojun1 WANG Yongsheng1 ZHANG Tingsu1 HU Mengyi1
Objective To investigate the changes of serum kisspeptin-1 (Kiss-1) level in patients with pancreatic cancer, and to analyze its relationship with the prognosis, clinicopathological parameters of patients with pancreatic cancer. Methods A total of 132 patients with pancreatic cancer admitted to Ningbo Hospital of Traditional Chinese Medicine, Zhejiang Province from January 2015 to December 2017 were selected as the pancreatic cancer group, 57 patients with benign pancreatic disease were selected as the benign lesion group and 53 healthy subjects were selected as the control group. Serum levels of Kiss-1, carcinoembryonic antigen (CEA) and carbohydrate antigen 199 (CA199) in the three groups were detected to analyze the correlation between Kiss-1 and CEA and CA199, as well as the relationship between Kiss-1 level and clinical characteristics and prognosis of pancreatic cancer patients. The pancreatic cancer group was divided into low Kiss-1 group (Kiss-1 < 42.13 pg/ml, 76 cases) and high Kiss-1 group (Kiss-1 ≥ 42.13 pg/ml, 56 cases) according to Kiss-1 level. Receiver operating characteristic (ROC) curve was used to analyze the prognostic value of Kiss-1 in pancreatic cancer. Results Serum Kiss-1 level in pancreatic cancer group was lower than that in benign disease group and control group, while CEA and CA199 levels were higher than those in benign disease group and control group, with statistical significance (P < 0.05). There were no significant differences in the levels of serum Kiss-1, CEA and CA199 between the benign lesion group and the control group (P > 0.05). Serum Kiss-1 levels were negatively correlated with CEA and CA199 levels in pancreatic cancer group (r = -0.687, -0.703, P < 0.001). The serum Kiss-1 level of pancreatic cancer patients with TNM stage Ⅲ-Ⅳ, or liver metastasis was lower than that of pancreatic cancer patients with TNM stage Ⅰ-Ⅱ, or no liver metastasis, with statistical significance (P < 0.05). The 3-year survival rate of low Kiss-1 level group was lower than that of high Kiss-1 level group (P < 0.05). Multivariate Cox regression analysis showed that high TNM stage, liver metastasis and low Kiss-1 level were independent risk factors for prognosis of pancreatic cancer patients (OR > 1, P < 0.05). The ROC curve showed that the cutoff value of Kiss-1 for predicting the prognosis of pancreatic cancer was 35.16 pg/ml, the area under the curve was 0.867 (95%CI: 0.790-0.944, P < 0.001), the sensitivity was 72.73%, and the specificity was 66.67%. Conclusion Serum Kiss-1 level is decreased in patients with pancreatic cancer, which is correlated with TNM stage of pancreatic cancer and liver metastasis, and can be used to evaluate the prognosis of patients with pancreatic cancer.

123 Discussion on the treatment of diabetes upper consumption from lung with “substantial yang and functional yin”
SHI Min LIU Fulin XIA Xuting ZHANG Ting LIAO Chenmin
According to the description of the physiological characteristics and functions of the lung based on the basic theory of traditional Chinese medicine, this article sorts out the theoretical origin of the lung with “substantial yang and functional yin”, and discusses the physiological and pathological characteristics of the lung in conjunction with the connotation and interrelationship of “lung substance”and “lung function”, and discusses the pathogenesis of diabetes upper consumption. It is believed that the lung substance is the highest among the viscera, which is connected with the weather, and connects all vessels, regulates the water passages, governs purification and descending. Its substance is yang, and its function is yin. The incidence of diabetes upper consumption is closely related to the function of lung qi governing diffusion, purification and descending and the abnormal distribution of body fluid. Therefore, based on the theoretical basis of “substantial yang and functional yin”, the treatment of diabetes upper consumption should be based on the relationship between substance and function of the lung, and dialectically combine prescription medicine with the combination of benefiting qi and diffusion, promoting production of fluid and purification and descending

193 Research on the problems and countermeasures of the assessment system under the background of “traditional Chinese medicine law”
LUO Xian Ranna·Ahati YANG Zi ZHANG Xinyu HE Nan WANG Wei FAN Linjie LI Dan LI Ruifeng
The assessment system for professionals with expertise in traditional Chinese medicine is a major measure to promote the inheritance and innovative development of Chinese medicine under the background of the “traditional Chinese medicine law”. At present, the assessment system is still in the trial and exploration stage, and it faces many problems and challenges in actual progess. This article fully understands the current situation of the assessment system, and comprehensively sorts out and analyzes the material review link and the problems existing in the assessment link of the assessment system through methods such as literature search, material collection and expert interviews. This article finds that the main problems in the current assessment system are mainly these: some substantive conditions are required to be reduced, leading to increased risk factors, some assessment dimensions lack assessment standards, and the list of disease names and technical names for candidates to fill in is not complete and derived. The industrial chain of training institutions that has been released has increased the difficulty of review and supervision. In the end, this article puts forward the following suggestions in response to the problem: relevant departments should further improve the operability evaluation indicators, strictly abide by the “strict evaluation” evaluation standards, improve the list of disease names and technical names, increase regulatory enforcement and qualification review, and improve local Chinese medicine management system, etc. These suggestions provide a reference for comprehensively and smoothly advancing the work of the assessment system and improving relevant policies and regulations.
