Abstract Objective To analyze the mediating effect of perceived stress on family communication and family resilience of stroke patients. Methods Using the convenience sampling method, a total of 179 stroke patients who were hospitalized in the neurology department of a grade A tertiary hospital in Anhui Province from February to May 2020 were selected as the research objects. The general information questionnaire, family resilience rating scale, family communication scale, perceived stress scale were used to investigate it, the mediation model was constructed and tested. Results There were statistically significant differences in the total scores of family resilience of inpatients with different occupations, family per capita monthly income, and first-onset stroke inpatients (P < 0.05). Family communication score was positively correlated with perceived pressure score (r = 0.615, P < 0.01), and negatively correlated with family resilience score (r = -0.547, P < 0.01); perceived stress was negatively correlated with family resilience (r = -0.489, P < 0.01). The mediating effect test results showed that perceived stress plays a partial mediating role between family communication and family resilience, and the mediating effect was -0.212, accounting for 34.30% of the total effect (-0.618). Conclusion Family communication can not only directly predict the family resilience level of stroke patients, but also indirectly affect family resilience through the perception of pressure. Clinical nurses can take interventional measures for the patient’s family communication and perceived stress status to enhance the patient’s family resilience.
