7 Effect of acute stress on AMPA receptors trafficking and fear memory in mice
CHEN Yuanhao1 ZHOU Mengsheng1 YIN Xiaoyu1 HE Yi1 WANG Wei2 HAO Jingru1 HAN Yuan1 GAO Can1
Objective To investigate the effect of acute restraint stress on AMPA receptors trafficking and fear memory in mice and its mechanism. Methods Sixty-four male C57BL/6 mice were divided into control group, Propranolol group, restraint stress group, combined group (restraint stress + Propranolol), according to random number table method, with 16 mice in each group. In the propranolol group and combined group, 10 mg/kg Propranolol solution was administered intragastrically, at the same time, the control group and the restraintstress group were intragastrically administered with an equal volume of normal saline, and the administration volume were 20 mL/kg. After 30 minutes of administration, the restraint stress group and combined group were restrained with 50 mL EP tube for 30 minutes. The fear memory of mice was evaluated by fear conditioning test. The expression of GluA1 in hippocampus, the level of serine phosphorylation at the 831th (pS-831) and the 845th (pS-845) of GluA1, the expression of calcium/calmodulin dependent protein kinase Ⅱ (CaMKⅡ) and its phosphorylation (p-CaMKⅡ) level, and the expression of GluA1 in the hippocampus cell membrane were measured by Western blot. Results Compared with the control group, the context-dependent and tone-dependent freezing time in the restraint stress group increased (P < 0.05), the expression of pS-831, pS-845 and p-CaMKⅡ in hippocampus increasd (P < 0.05), and the expression of GluA1 in cell membrane increased (P < 0.05), while the expression of GluA1 and CaMKⅡ in total protein had no significant difference (P > 0.05). Compared with the restraint stress group, the context-dependent and tone-dependent freezing time in the combined group decreased (P < 0.05), the expression of pS831, pS845 and p-CaMKⅡ in hippocampus decreased (P < 0.05), and the expression of GluA1 in cell membrane decreased (P < 0.05), while the expression of GluA1 and CaMK Ⅱ in total protein had no significant difference (P > 0.05).  Conclusion Acute restraint stress can promote the formation of fear memory in mice, which may be related to the regulation of GluA1 subunit phosphorylation and the increased trafficking of AMPA receptors. Moreover, β-adrenergic receptors (β-ARs) may be one of the important targets for restraint stress-induced fear memory.

51 Advances in neuroendoscopic surgery for hypertensive intracerebral hemorrhage
SHI Shuang1 ZHANG Biao1 YANG Gang2 ZHANG Zhengbao1 YANG Biao1 NIU Wenyong1 DENG Wei1 QIN Chuan1
Hypertensive intracerebral hemorrhage is the most common subtype of stroke with high mortality and disability. Its prognosis is closely related to the age, location, volume, speed of bleeding, timing of operation, mode of operation and whether or not to take anticoagulants orally. On the premise of strictly grasping the indications and timing of operation, compared with local anesthesia minimally invasive puncture plus urokinase lysis (including stereotactic, CT-guided hard and soft channel puncture) and craniotomy microhematoma removal surgery, neuroendoscopic hematoma removal surgery not only reduces further trauma to brain tissue, but also can More thorough removal of intracerebral hematoma and hemostasis, with more minimally invasive, better efficacy advantages. At present, the assistant means of neuroendoscopy in the surgical treatment of hypertensive intracerebral hemorrhage include CT guidance, stereotactic guidance, B-ultrasound assistance, neuronavigation assistance and mobile phone software assistance. The use of various assistant means makes the neuroendoscopy more accurate in locating the hematoma, reduces the trauma to brain tissue and improves the prognosis of patients. This paper summarizes the assistant methods and their respective operative methods and characteristics of neuroendoscopy in the treatment of hypertensive intracerebral hemorrhage.

88 The correlation between Notch1 signaling pathway and vasculogenic mimicry in cervical cancer
FANG Hong1 ZHOU Zhongming1 LI Yunjun1 LU Lin2
Objective To explore the correlation between Notch1 signaling pathway and vasculogenic mimicry (VM) in cervical cancer. Methods Paraffin sections were collected from 68 cervical cancer patients who underwent surgical treatment in Hubei Provincial Hospital of Traditional Chinese Medicine between January 2012 and December 2015 as cervical cancer group. Paraffin sections of normal cervix were also collected from 27 patients who were surgically treated for uterine leiomyoma as control group. CD31 immunohistochemistry and periodic acid-Schiff (PAS) reaction were performed to detect VM formation. IHC was performed to detect Jagged1, Notch1 and Hes1 expression. The correlations of VM with Jagged1, Notch1 and Hes1 were analyzed. The correlations of VM, Jagged1, Notch1 and Hes1 with patients′ clinic-pathological features and postoperative prognosis were analyzed. Results The positive expression rates of Jagged1, Notch1 and Hes1 in cervical cancer group were significantly higher than those in control group (P < 0.05). VM was only in cervical cancer tissues, with the positive expression rate of 39.7%. Jagged1 was associated with lymph node metastasis of cervical cancer (P < 0.05). Notch1 was associated with tumor size and tissue differentiation (P < 0.01). Hes1 was not associated with any clinic-pathological parameters of patients (P > 0.05). VM was associated with tumor size and lymph node metastasis (P < 0.01). VM was positively correlated with Jagged1 and Notch1 expression (r = 0.283, 0.304, all P < 0.05). VM was an independent risk factor for postoperative prognosis of cervical cancer patients (HR = 2.21, 95%CI: 1.85-2.64, P < 0.05). Conclusion VM formation in cervical cancer predicts patients′ poor postoperative prognosis. The up-regulated Jagged1/Notch1 signaling pathway in cervical cancer might be involved in promoting VM formation. In the future, targeted Notch1 pathway treatment might be expected to prevent VM formation in cervical cancer and improve patients′ prognosis.

167 One case with threatened rupture of distal aortic dissection after TEVAR treated by TEVAR one more and literature analysis
HE Yiwei1,2 XIONG Pan3 ZHANG Yongheng1 LIU Jianping1 TANG Xiaojiao1
Standford type B aortic dissection is dangerous and has a high mortality rate. Once this disease is diagnosed, immediate surgery is needed. Thoracic endovascular aortic repair (TEVAR) is the first treatment for Standford type B aortic dissection, but distal stent rupture has become a complication of TEVAR, which is very important to deal with. The author analyzes the clinical data of one patient with rupture of distal aortic dissection after TEVAR treated by TEVAR one more on May 24, 2018 in Suining Central Hospital of Sichuan Province. It was found that the distal stent ruptured into thoracic cavity after TEVAR, then the patients were treated by TEVAR again and assisted by shock correction, hypoproteinemia correction, thoracic drainage and thoracentesis as the main comprehensive treatment, the postoperative recovery is good, with no serious complications. It is suggested that TEVAR is an effective treatment for Standford B aortic dissection.

ZHANG Wenjing WANG Jiahe
Objective To investigate the drug resistance of extended spectrum β-lactamases (ESBLs) producing Escherichia coli and Klebsiella pneumoniae in urinary tract infections in elderly patients so as to provide guidance for clinical rational use of antibiotics. Methods From January 2015 to December 2017, in Shengjing Hospital of Chinese Medical University, identification of bacteria and antibiotic susceptibility tests were performed on the strains isolated from the clean midstream urine specimens of elderly patients. ESBLs was detected by disk diffusion confirmatory test in the 1063 strains of Escherichia coli and 202 strains of Klebsiella pneumoniae. Results The detection rate of ESBLs-producing Escherichia coli and ESBLs-producing Klebsiella pneumoniae in three years was 42.24% and 38.61% respectively, and ESBLs-producing strains were more resistant to most drugs than non-ESBLs producing strains. The drug resistance rates of ESBL-producing Klebsiella pneumoniae to Amikacin, Nitrofurantoin, Piperacillin-tazobactam, Cefperazone-sulbactam (14.10%, 44.68%, 24.36%, 54.17% respectively) were higher than ESBL-producing Escherichia coli (4.23%, 6.70%, 2.46%, 15.00% respectively), and the differences were statistically significant (P < 0.05). Conlusion The drug resistance of ESBLs-producing Escherichia coli and ESBLs-producing Klebsiella pneumoniae is more severe. So the antimicrobial agents should be chosen reasonably according to the results of the drug susceptibility.

177 Application effect of 3D-STI in the evaluation of left ventricular function in patients with type 2 diabetes
XU Le1 WEN Dehui1▲ ZHENG Yingjuan1 SHI Baolin2 LIU Weiliang1 GU Jun3
Objective To investigate the application effect of three-dimensional speckle tracking technology (3D-STI) in the evaluation of left ventricular function in patients with type 2 diabetes mellitus. Methods From January 2017 to April 2018, 81 patients with type 2 diabetes mellitus from the First Affiliated Hospital of Hebei North University ("our hospital" for short) were selected. According to whether the intima-media thickness (IMT) of the common carotid artery was thickened, it was divided into TM1 group (IMT≤1.0 mm) (n = 38) and TM2 group (1.0 mm<IMT<1.2 mm) (n = 43), and healthy subjects with matched age and gender in the same period in our hospital were selected as the control group (n = 40). Left ventricular global longitudinal strain (GLS), circumferential strain (GCS), area strain (GAS), radial strain (GRS), left ventricular ejection fraction (LVEF), carotid IMT and related clinical indicators were compared in each group, and the correlation between strain values and IMT and glycated hemoglobin (HbA1c) was analyzed. Results Compared with the control group, GLS in TM1 group decreased, and the difference was statisticlly significant (P < 0.05). Compared with the control group and the TMI group, GLS, GCS, GAS and GRS in TM2 group decreased significantly (P < 0.01 or P < 0.05), IMT and HbA1c were negatively correlated with left ventricular strain in all directions (r < 0, P < 0.05). Conclusion Subclinical left ventricular dysfunction in early stage of type 2 diabetes mellitus can be detected by 3D-STI. If carotid IMT thickening occurs, left ventricular systolic function will be further impaired and left ventricular ejection fraction will be reduced. 3D-STI is a sensitive method to detect these slight abnormalities.

LI Banghe1 WAN Haiwei2 DONG Peng1 LI Wei1 YAN Ping1
Objective To investigate the predictive value of fragmented QRS wave for recurrence in patients with hypertrophic cardiomyopathy and atrial fibrillation after catheter ablation. Methods A total of 95 patients with hypertrophic cardiomyopathy and atrial fibrillation who underwent catheter ablation at the Aviation General Hospital from February 2016 to February 2018 were enrolled. All patients were divided into recurrent group (45 cases) and non-recurrence group (50 cases) according to whether there was recurrence after operation. The general data, levels of cardiac function related index, operation time, fluoroscopy time, fragmentation QRS wave and corrected QT interval (QTc) were compared between the two groups, and multivariate Logisitic regression analysis was performed. Results There were no significant differences in the age, gender, paroxysmal atrial fibrillation and atrial fibrillation between the two groups (all P > 0.05). The left atrial diameter of the recurrent group was greater than that of the non-recurrent group, and the difference was highly statistically significant (P < 0.05). The proportion of patients with fragmentation QRS wave and the QTc level in the recurrent group were higher than those in the non-recurrent group, the differences were highly statistically significant (all P < 0.01). Multivariate Logisitic regression analysis found that both fragmentation QRS wave and QTc were risk factors for recurrence after catheter ablation in patients with hypertrophic cardiomyopathy and atrial fibrillation (all P < 0.01). Conclusion Fragmentation of QRS and QTc have high predictive value for recurrence after catheter ablation in patients with hypertrophic cardiomyopathy and atrial fibrillation. It is worthy of clinical application.
