4 Effect of Exenatide on serum resistin in ApoE-/- mice with atherosclerosis

Objective To investigate the effect of Exenatide on serum resistin in atherosclerosis of apolipoprotein E knockout (ApoE-/-) mice. Methods Thirty six-week-old ApoE-/- male mice fed with high fat were randomly divided into model group (n = 15) and exenatide group (n = 15). The exenatide group was given Exenatide 1.0 nmol/(kg·d), subperitoneal injection twice a day. Male C57BL/6J mice fed with high fat were selected as control group (n = 15). After 12 weeks, the serum was collected and the levels of total cholesterol, high density lipoprotein cholesterol, low density lipoprotein cholesterol and triglyceride were measured by automatic biochemical analyzer. The contents of resistin, tumor necrosis factor-alpha (TNF-α) and interleukin-6 (IL-6) were determined by enzyme-linked immunosorbent assay. The aortic root was pathologically sliced and stained with hematoxylin-eosin (HE), and the corrected patch area was measured. Results The levels of total cholesterol, low density lipoprotein cholesterol, high density lipoprotein cholesterol and triglyceride in the model group were significantly higher than those in the control group(P < 0.01). There was no significant difference in total cholesterol, low density lipoprotein cholesterol, high density lipoprotein cholesterol and triglyceride between the exenatide group and the model group (P > 0.05); the levels of resistin, TNF-α and IL-6 in the model group were significantly higher than those in the control group (P < 0.01), while the contents of resistin, TNF-α and IL-6 in the exenatide group were significantly lower than those in the model group (P < 0.01). The area of corrected atherosclerosis plaque in the exenatide group was significantly smaller than that in the model group (P < 0.05). Conclusion Exenatide can effectively reduce the level of serum resistin in atherosclerotic mice, thereby inhibiting inflammatory response and atherosclerotic plaque progression, which may be one of the mechanisms of glucagon-like peptide-1 receptor agonists in anti-atherosclerosis.

8 Mechanism of persistent low back pain after intervertebral space infection

Objective To Discus on the Staphylococcus aureus supernatant of intervertebral disc nucleus pulposus cells proteoglycans (ACAN), type Ⅱ collagen (COL Ⅱ). Methods Cultured SD rat intervertebral disc nucleus pulposus cells were isolated in vitro and treated with Staphylococcus aureus supernatant at different concentrations (0%, 5%, 10%, 15%, 20%, 25%, 30%, 35%, 40%) to simulate the course of intervertebral space infection. CCK-8 was used to detect the proliferation activity of the second generation nucleus pulposus (P2) cells. RT-PCR was used to detect the gene expression of the second generation nucleus pulposus cells. Results This study found that the supernatant fluid of Staphylococcus aureus that accelerate nucleus pulposus cells to divide, stimulate ACAN, COL Ⅱ expression decreased (P < 0.05), the supernatant fluid concentration > 20%, can significantly inhibit extracellular matrix synthesis of nucleus pulposus. Conclusion The results showed that Staphylococcus aureus supernatant could inhibit the growth of nucleus pulposus cells in a concentration dependent manner. Stimulate ACAN, COL Ⅱ expression decrease may be the cause of intervertebral infection after continuous low back pain.

12 Effect of Lichong Decoction on the expression of immune escape factors in the mice with heterograft uterine fibroids

Objective To explore the effects of Lichong Decoction on the expression of indoleamine 2,3-dioxygenase (IDO) and forkhead winged helix transcription factor-3 (Foxp3) in xenograft mice with human uterine fibroids from the perspective of immune escape mechanism. Methods Thirty-two female mice of CB-17 scid were divided into normal group, model group, Lichong Decoction group and positive drug group according to completely randomized method, with 8 mice in each group. The animal model of uterine leiomyoma was established by xenotransplantation. After successful modeling, Lichong Decoction group was given Lichong Decoction, positive drug group was given Mifepristone, normal group and model group were given the same amount of saline, and tissue samples were collected after 4 weeks of continuous intervention. The histopathological changes of the uterine smooth muscle of mice in each group were observed by HE staining, and the expression of IDO in the smooth muscle tissue of mice in each group was detected by Western blot, and the expression of Foxp3 mRNA was detected by Real-time PCR. Results Compared with model group,the thickness of endometrium and muscle layer of mice in Lichong Decoction group was reduced, and the content of fibrous tissue was less compared with the model group. The expression of IDO and Foxp3 mRNA in uterine smooth muscle tissue of mice was significantly reduced in Lichong Decoction group (P < 0.05 or P < 0.01). Conclusion Lichong Decoction has the effect of inhibiting the occurrence and development of uterine fibroids, and its pharmacological mechanism may be related to blocking tumor immune escape.

16 Study on the mechanism of Edaravone on cerebral ischemia-reperfusion injury in rats

Objective To investigate the effect of Edaravone on the expression of PDCD-5 protein in rats with cerebral ischemia reperfusion. Methods Seventy-eight healthy male SD rats with a healthy weight of 250-280 g were randomly divided into blank control group (n = 6, normal saline, 3 mg/kg, twice a day, intraperitoneal injection), sham operation group (n = 24, normal saline, 3 mg/kg, twice a day, intraperitoneal injection), cerebral ischemia reperfusion group (n = 24, normal saline, 3 mg/kg, twice a day, intraperitoneal injection) and drug intervention group (n = 24, Edaravone 3 mg/kg, twice a day, intraperitoneal injection) according to random number table method, and the latter 3 groups were randomly divided into 6 h group, 1 d group, 3 d group and 7 d group. A modified Longa method was used to establish cerebral ischemia-reperfusion model. The neurological deficit, the acreage of cerebral infarction and the expression of PDCD-5 and Caspase-3 protein in hippocampus of different periods were compared. Results The cerebral infarction in cerebral ischemia reperfusion group was significantly increased compared with sham operation group (P < 0.05); the cerebral infarction in drug intervention group was significantly reduced compared with cerebral ischemia reperfusion group (P < 0.05). At the same time, the expression of PDCD-5, Caspase-3 protein in cerebral ischemia-reperfusion group at different time points was higher than those in corresponding sham-operation group and corresponding drug intervention group (P < 0.05). Conclusion Edaravone can inhibit the expression of PDCD-5 in the hippocampus of rats with cerebral ischemia-reperfusion, and protect the brain against cerebral ischemia-reperfusion injury in rats.

20 Effect of Bupi Qingfei Decoction on virulence factors of drug-resistant Pseudomonas aeruginosa

Objective To investigate the mechanism of Bupi Qingfei Decoction in the treatment of drug-resistant Pseudomonas aeruginosa (PA) infection of the lower respiratory tract, to provide evidence for the treatment of PA-induced lower respiratory tract infection. Methods A total of 30 SPF male SD rats, weighing 240-260 g, aged 2-3 months. Blank control group (n = 10) was set, no model was made, and equal volume of normal saline was given. In the model group (n = 10), normal saline of equal volume was administered after modeling. The rats in the traditional Chinese medicine group (n = 10) were given 2 times the dose (10.68 g) of traditional Chinese medicine after modeling, and the rats′ lower respiratory tract infection model were established by injecting PA solution into rats′ trachea, given Bupi Qingfei Decocion for intervention. The contents of elastase and pyocyanine were determined. Results In terms of elastase content, compared with the model group, the elastase content in the traditional Chinese medicine group decreased, and the difference was statistically significant (P < 0.05). Compared with the blank control group, elastase content in the traditional Chinese medicine group increased, and the difference was not statistically significant (P > 0.05). Compared with the blank control group, the elastase content in the model group increased, and the difference was statistically significant (P < 0.05). In terms of the content of pyocyanine, compared with the model group, the content of pyocyanine increased in the traditional Chinese medicine group, and the difference was not statistically significant (P > 0.05). Compared with the blank control group, pyocyanine content increased in the TCM group, and the difference was not statistically significant (P > 0.05). Compared with the blank control group, the content of pyocyanine in the model group increased, and the difference was not statistically significant (P > 0.05). Conclusion Bupi Qingfei Decoction can significantly improve airway inflammation, reduce the content of elastase and relieve the PA′s invasion.

24 Hemostatic effect of three different hemostasis methods in the model of anorectal hemorrhage in rats

Objective To evaluate the hemostatic effect of three different hemostatic methods in the model of anorectal bleeding in rats. Methods Twenty-seven SPF SD male rats only randomized block design was divided into air tract tourniquet group (group A) and gelatin sponge group (group B), vaseline oil gauze group (group C), each group of nine, hemorrhage model was established, with balloon tourniquet pressure cavity, vaseline oil gauze or gelatin sponge tamponade hemostasis, press 4 min, for the first time and then every 2 min to check bleeding to stop bleeding, observe the bleeding time, blood loss, after the success of the hemostatic, observe 5 min to assess any bleeding again. Two days after surgery, hematoxylin - eosin staining was used to observe the inflammation. Results Compared with group C and B, group A had shorter hemostasis time and less blood loss, and the difference was statistically significant (P < 0.01). Compared with group B, the blood loss in group C was lower, and the difference was statistically significant (P < 0.05).There was no statistically significant difference between the three hemostasis methods for re-bleeding within 5 min (P > 0.05). Histological sections showed the most severe inflammatory response in group C. Conclusion The most effective hemostasis method is the balloon cavity hemostatic device, followed by vaseline gauze, gelatin sponge used alone has poor hemostatic effect.

29 Study on the stability of Yishen Paidu Pills

Objective To investigate the change of stability of Yishen Paidu Pills with time under the influence of temperature, humidity and light, and to provide a scientific basis for the formulation of storage conditions and expiry date of Yishen Paidu Pills. Methods According to the Guidelines of Stability Testing of Raw Materials and Preparations (Part Ⅳ, General Principles 9001) of 2015 edition the Pharmacopoeia of the People′s Republic of China, factors affecting testing, accelerated testing and long-term testing were adopted to inspect the stability of Yishen Paidu Pills. Results Yishen Paidu Pills were stable under high temperature, high humidity and strong light conditions, but it still had a certain little effect on the content of its effective components. Yishen Paidu Pills should be stored in a cool, dry place away from light. The long-term test results showed that the forecast shelf life of Yishen Paidu Pills was six mouths. Conclusion Yishen Paidu Pills is very stable.

33 Study on quality standard of Qizhen Jiangtang Granules

Objective To establish the quality standard for Qizhen Jiangtang Granules. Methods Astragali Radix were identified by thin layer chromatography (TLC). The contents of citric acid and specnuezhenide were determined by high performance liquid chromatography (HPLC). The columns were Agilent ZORBAX SB-Aq (4.6 mm×250 mm, 5 μm) and Kromasil 100-5 C18 (4.6 mm×250 mm, 5 μm) with 0.1%phosphoric acid-water、methanol-water(36∶64)as the mobile phases, the flow rate was 1.0 mL/min, the detection wavelengths were 210 nm, 224 nm, the column temperature was 30℃ and the injection volumes were 5.0, 10.0 μL. Results TLC spots of Astragali Radix were clear with good resolution and without interference of negative samples. HPLC showed that the citric acid and specnuezhenide had good linear relationship with peak area in the range of 0.098 28-24.57 μg (r = 1.0000) and 0.022 08-11.04 μg (r = 1.0000) respectively. The average sample recovery rates were 100.18%, 96.08% (n = 9), and the RSD were 2.3% and 2.8%, respectively. The average contents of citric acid and specnuezhenide were 46.9, 1.02 mg/g, respectively. Considering comprehensively, the contents of citric acid and specnuezhenide in each bag is not less than 400 mg and 12 mg respectively. Conclusion The method is accurate, simple and reproducible, and can control the quality of Qizhen Jiangtang Granules.

38 Research progress on the relationship between non-coding RNA and acute kidney injury

Acute kidney injury (AKI) is a common serious disease characterized by rapid decline in renal function and high mortality. However, the molecular mechanism of AKI is still unclear and there is no effective treatment. Non-coding RNA (ncRNAs) participate in the pathophysiological process of AKI by regulating inflammatory response, programmed cell death and cell cycle in repair phase. Therefore, a variety of ncRNAs are not only considered as new biomarkers of AKI, but also potential therapeutic targets. This article reviews the changes, functions and potential therapeutic effects of ncRNAs in AKI.

42 Advances in the regulation of mitochondrial dynamic Changes by MFN2

To maintain mitochondria homeostasis, mitochondria remove damaged or excess mitochondria by autophagy. Mitochondria fission and fusion process is the basis of mitophagy, and mitofusion-2 (MFN2) is involved in the process of regulating mitochondria fission and fusion. Mutations in MFN2 could cause mitochondria dysfunction and even lead to neurodegenerative diseases such as Charcot-Marie-Tooth. This paper review recent research progress of MFN2 function and its effect on mitochondria autophagy and neurodegenerative disease, aiming at providing reference for further research in related fields.

46 Research progress on the relationship between epithelial-mesenchymal transition and chemoresistance in gastric cancer

Insensitivity of tumor cells to chemotherapeutic agents and arising drug resistance are the main cause of high mortality in gastric cancer. Epithelial mesenchymal transformation (EMT) plays an important role in the progression of gastric cancer. There is mounting evidence demonstrating that EMT participates in gastric cancer chemotherapeutic drug resistance through related transcription factors, cytokines, important signaling pathways, and regulation of stemness. This article summarizes the relationship between EMT and chemoresistance in gastric cancer to help people find new strategies in anti-drug-resistant treatment of gastric cancer.

50 Advances in the study of transforming growth factor-β1/Smads signaling pathway in ocular surface diseases

The pathogenesis of ocular surface diseases is complex and diverse, among which the imbalance of ECM is one of the important causes. Transforming growth factor -β1 (TGF-β1)/Smads is involved in the development of the disease as a major signaling pathway regulating the production, outcome and metabolic balance of ECM. The relationship between this pathway and ocular surface diseases will be reviewed in this paper, which will provide a new basis for the treatment of ocular surface diseases.

54 Advances in the relationship between FoxO1 transcription factors and liver fibrosis

FoxO1, as a member of the forkhead transcription factor family, can promote the expression of gluconeogenesis genes in hepatocytes, and promote the expression of ApoC-Ⅲ to form liver fibrosis caused by nonalcoholic fatty liver disease; FoxO1 regulates the expression of autophagy genes. Participate in autophagy of hepatic stellate cells to regulate liver fibrosis; FoxO1 can participate in hepatic stellate cell cycle arrest to regulate liver fibrosis; PDGF factor can regulate FoxO1 expression involved in hepatic stellate cell activation and regulate liver fibrosis; FoxO1 participates in fat differentiation of cells and myofibroblasts regulates the formation of liver fibrosis. This article will review the relationship between FoxO1 and liver fibrosis in the above aspects.

58 Research status of hierarchical diagnosis and treatment system in China

With the continuous introduction of policy documents related to hierarchical diagnosis and treatment, relevant government departments at all levels are committed to continuously promoting the construction of hierarchical diagnosis and treatment systems and improving the hierarchical diagnosis and treatment service system. The existing literature is analyzed from the implementation significance, implementation path, implementation effect of the hierarchical diagnosis and treatment system, the cognition of the doctors and patients on the hierarchical diagnosis and treatment system, the problems and countermeasures of the hierarchical diagnosis and treatment system in practice. It is found that there are three characteristics in the current related literature research: the research started late and the number was small, the research content is extensive but not in-depth, the research method is simple.Based on the implementation of the hierarchical diagnosis and treatment in China and the characteristics of the existing literature research, some issues worthy of further study are put forward.

62 Research progress on the effect of non-pharmacological intervention on the quality of life of patients with Alzheimer′s disease in community

With the coming of the aging population, the number of Alzheimer′s disease patients has increased dramatically. The patients have been living in their homes and communities most of the time since the disease. The disease has led to a decline in the cognitive function of patients and a serious decline in their living capacity and quality of life. While bringing pain to the patient, it increases the care burden of the carer, due to the limitation of drug therapy, non-drug intervention has been applied in the treatment of Alzheimer′s disease in recent years. This article introduces the different non-drug intervention methods reported in the literature at home and abroad in order to provide reference for the clinical intervention of Alzheimer′s disease and the care of the patients at home and abroad. It has important guiding significance for improving the quality of life of AD patients in our community.

66 Probe on ways of training Chinese medicine talents under the background of Traditional Chinese Medicine Law

At July 1, 2017, the Chinese Medicine Law was formally implemented in China, which put forward higher requirements for the development of Chinese medicine talents. Firstly, this thesis makes a detailed interpretation of the development policy of traditional Chinese medicine stipulated in the Law of the People′s Republic of China on Traditional Chinese Medicine, and explores the current situation of the training  mode of traditional Chinese medicine talents in China. In addition, the thesis found that the current training mode of traditional Chinese medicine and the teacher-to-teacher education are mostly based on practical teaching, which is closer to the training goal of traditional Chinese medicine talents, however, has some problems, such as the limitation of knowledge structure and cognitive thinking, the small scale of training objects and the uneven quality of training objects. Furthermore, the modern training mode of Chinese medicine talents and college education have the advantages of universality, fine division and openness, but at the same time, it also has the practical predicament of separating theoretical teaching from practical teaching, neglecting traditional Chinese medicine culture inheritance, disproportionate cultivation cycle and quality of training, and no successors of Chinese medicine at the grass-roots level. To solve the problems existing in the two training modes of tradition Chinese medicine talents, this thesis puts forward innovative ways of training tradition Chinese medicine talents in the integration of teacher-to-teacher education and college education, and makes a thorough analysis from five perspectives of students, training objects, trainin environment, curriculum system design and teaching staff.

70 Application and thought of Seminar combined with PBL teaching mode in teaching of toxicology experiment

Objective To evaluate the application effect of Seminar combined with PBL teaching mode in teaching of Toxicology experiment. Methods From December 2016 to January 2018, 90 students with preventive medicine and related majors from Grade 2013 and Grade 2014 in Third Military Medical University were selected, they were divided into three groups by random sampling method, with 30 students in each group. Three different teaching models (traditional teaching group, PBL group and Seminar combined with PBL group) were used for experimental teaching. Questionnaire and grade statistics were used to analyze teaching effect. Results The scores of theoretical examination and operation practice examination of Seminar combined with PBL teaching group were significantly higher than the other two groups, the teaching satisfaction, academic record, learning time, autonomous learning ability, independent thinking ability, communication ability, analytical and solving ability, team spirit in the Seminar combined with PBL teaching group were significantly better than the other groups, the differences were statistically significant (P < 0.05). Conclusion Seminar combined with PBL teaching method not only improves the quality of toxicology experiment teaching and students′ examination results, but also contribute to the comprehensive improvement of students′ comprehensive quality.

74 Discussion on issues related to postgraduate education mode of TCM general medicine

With the development of society, people put forward new demands for health. The service capacity of grassroots general medicine is the development direction of medical health and health at a certain stage, such as the report on the healthy China in the 19th National Congress of the Communist Party of China. It is pointed out that "enhance the grassroots medical and health service system and the construction of the general practitioner team". Therefore, attaching importance to TCM general medical education is one of the directions for the future development of talent cultivation. However, the lack of high-level general practitioners of traditional Chinese medicine seriously hinders the development of community health services. Postgraduate education can reserve a large number of high-quality reserve force for training high-level and specialized general medicine. Therefore, the development of postgraduate education of general medicine of traditional Chinese medicine is helpful to improve the overall quality of general practitioners,and improve the general medical teaching system, so as to better meet the community medical work. At present, the training mode of Chinese medicine postgraduate education is single, the standardized training is imperfect, the teaching staff of Chinese medicine is weak, the cultivation of clinical research ability is neglected, and the construction level of clinical community training base is uneven. This paper attempts to propose the training methods of postgraduate education in TCM general medicine. The purpose of this study is to provide reference for general practice education of Chinese medicine.

Objective To investigate Guangdong Qingyuan District IVS-Ⅱ-654 point mutations beta thalassaemia (hereinafter referred to as the "654 M beta lean") gene and genotype combination characteristic, and analysis the features of red blood cell parameters, provide a reference for better screening and prevention to poor data. Methods From January 2014 to September 2015, 6760 samples of geologically deficient gene diagnosis in the Outpatient Department of Qingyuan People′s Hospital were collected. According to the age of the subjects, the samples were divided into infant group (3 years old), child group (> 3 years old-18 years old), and adult group (≥18 years old). Gap-PCR amplification was used to detect 3 common deletion geoimpoverished genes (-α3.7/、-α4.2/、--SEA/). PCR- membrane reverse point hybridization (PCR-RDB) was used to detect 17 mutated geoimpoverished genes. And the genotypes of 654 M β. Routine blood test was used to observe the red blood cell count (RBC), hemoglobin (Hb) concentration, red blood cells deposited (Hct), mean red cell volume (MCV), mean red blood cell hemoglobin content (MCH) and red blood hemoglobin concentration (MCHC) 6 indicators, the influence of factors such as gender, age of 654 Mβ gene carriers on the red blood cell parameters was analyzed, with the fourth edition of the national clinical laboratory operation rules, 654 M heterozygote and healthy red blood cell parameters of the similarities and differences were observed. Results ①Among the investigated population, the total carrier rate of 654M was 4.79% (324/6760), including 256 cases of 654M heterozygote (3.79%), 52 cases of 654M complex ground poverty (0.77%), and 16 cases of 654M complex ground poverty (0.24%). There was no statistically significant difference in the 654M heterozygote and 654M complex α (P > 0.05). Infants and toddlers group 654 M compound β to lean gene carrying rate was higher than children series, the difference was statistically significant (P < 0.05), in not currently found 654 M compound other β to lean gene genotypes, 654 M heterozygote. ②654 M compound α to lean gene and 654 M composite β to a poverty of three genotypes of RBC, Hb, Hct influence size order for 654 M compound β to lean heterozygous > 654 M>654 M compound alpha to lean (all P < 0.05), but the three gene combinations of MCV, MCH, MCHC differences had no statistical significance (P > 0.05). ③The red blood cell parameters of 654M heterozygous carriers in different groups were higher in male RBC, Hb, Hct than in adult female, male infants and male children, and the differences were statistically significant (P < 0.05), while the differences in red blood cell parameters between other groups were not statistically significant (P > 0.05). ④Compared with healthy people, male 654M heterozygous blood phenotype was mainly reflected in RBC increased, MCV, MCH, MCHC decreased (all P < 0.05); Hb and Hct were in the critical low value (all P > 0.05). The Hb, Hct, MCV, MCH and MCHC of female heterozygote 654M were decreased (all P < 0.05). RBC′s were at a critical high (P > 0.05). Conclusion In qingyuan area 654M, the parameters of 654M deficient erythrocytes in different gene combinations are quite different. Adult male 654M RBC, Hb, Hct are significantly different from infants, children and adult females, so it is necessary to pay attention to the distinction when setting the critical value of screening indicators.

83 Effect of 28-joint Doppler ultrasound score on rheumatoid arthritis

Objective To investigate the value of 28-joint Doppler ultrasound was used to evaluate the clinical efficacy of different diseases modifying antirhenmatic drugs(DMARDs) in the treatment of rheumatoid arthritis (RA). Methods From June 2014 to June 2017, 45 RA patients who were admitted to Baiyun Hospital Affiliated to Guiyang Medical College and met the classification criteria of American rheumatic association/European anti-rheumatic association (ACR/EULAR) in 2010 were selected. According to the treatment regimen, the patients were divided into the Methotrexate group (Methotrexate alone), the Leflunomide group (Leflunomide combined with Methotrexate), and the Yisaipu group (tumor necrosis factor receptor-antibody fusion protein Yisaipu combined with Methotrexate), with 15 cases in each group. Doppler ultrasonography was performed on 28 joints of the three groups before and 6 months after treatment, respectively, and the observation indexes included joint effusion, synovial hyperplasia, blood flow signal in the synovial membrane and bone erosion. At the same time, rheumatoid factor (RF), anti-cyclic citrullinic peptide antibody (anti-CCP), C-reactive protein (CRP), erythrocyte sedimentation rate (ESR), 28 range of motion (DAS28) and adverse drug reactions were recorded to evaluate the clinical efficacy and safety of different DMARDs regimens in the treatment of RA. Results Before treatment, there was no significant difference in the baseline data of joint effusion, synovial hyperplasia, blood flow signal in the synovial membrane and bone erosion. DAS28 and laboratory examination between the three groups (P > 0.05). After 6 months of treatment, joint effusion, synovial hyperplasia, blood flow signal in the synovial membrane and bone erosion. DAS28 and laboratory examinations (RF, CCP, CRP and ESR) of the three groups were all decreased to varying degrees compared with those before treatment, and the other indexes were compared among the three groups except bone erosion indexes, with statistically significant differences (P < 0.05). The ultrasound score, DAS28 and laboratory examination indexes of Yisaipu group were the lowest, significantly lower than that of Leflunomide group and Methotrexate group (P < 0.05), while Leflunomide group was also lower than that of Leflunomide group (P < 0.05). There was no significant difference in the incidence of adverse reactions among the three groups (P > 0.05). Conclusion 28 joint Doppler ultrasound can provide a reliable basis for the evaluation of the clinical efficacy of different DMARDs regimens in the treatment of RA, and the efficacy of Yisaip is significantly better than Leflunomide combined with Methotrexate or using Methotrexate alone, with no obvious adverse reactions.

88 Study on correlated influencing factors of vascular calcification in maintenance dialysis patients

Objective To analyze main influencing factors of vascular calcification in maintenance dialysis patients and to provide the foundation for finding the methods to prevent vascular calcification. Methods A total of 300 maintenance dialysis patients treated in Urology Department in Liuzhou Worker′s Hospital between May 2013 and May 2017 were selected as research objects. The retrospective analysis of relevant clinical data was conducted, and all the correlated indexes of venous blood of patients on an empty stomach were measured. Dual energy X-ray absorptiometry was adopted to detect bone density, and then the calcification scores were done based on X-Ray films of lateral projection of abdomen, anteroposterior projection of hands and pelvis to determine the degree of vascular calcification. Logistic regression was used to analyze the influencing factors after grouping. Results Compared with patients with mild calcification and without calcification, patients with moderate and severe calcification had statistical differences in age, dialysis age, incidence rate of diabetes, low density lipoprotein (LDL), alkaline phosphatase (ALP) , blood calcium level after correction and femur density (P < 0.05). Compared with non-diabetic patients, the incidence of VC in diabetic patients significantly increased, and the difference was statistically significant (P < 0.05). Logistic regression analysis results showed that age, dialysis age, diabetes, LDL and ALP were the influencing factors of moderate and severe vascular calcification(P > 0.05). Conclusion These factor, such as age, dialysis age, diabetes, LDL, ALP, blood calcium level after correction and femur density may lead to and help the development of vascular calcification of maintenance dialysis patients, among which age, dialysis age, diabetes, LDL and ALP were the influencing factors of moderate and severe vascular calcification.

91 Analysis of the risk factors for the treatment of pulmonary nodules by video-assisted thoracoscopic surgery and its effect on immune function

Objective To explore and analyze the malignant risk factors of video-assisted thoracoscopic surgery (VATS) for small pulmonary nodules and its effect on immune function. Methods Eighty patients with small pulmonary nodules admitted to Anhui Provincial Thoracic Hospital from June 2016 to June 2018 were treated with VATS, and definite pathological diagnosis was obtained. The malignant risk factors of small pulmonary nodules were analyzed. After VATS treatment for both benign and malignant pulmonary nodules, the changes of immune function in perioperative period were compared. Results Age, nodule maximum diameter, spiculation sign, lobulation sign, pleural traction sign, cavity sign, vascular cluster sign and calcification sign were included in the Logistic regression analysis. The results showed that age, nodule maximum diameter, spiculation sign, lobulation sign and pleural traction sign could be used as independent correlative factors to judge the malignancy of pulmonary nodules (P < 0.05). All the patients were operated smoothly, and no serious complications or deaths were found during the perioperative period. The levels of CD4+, CD8+, CD4+/CD8+ and natural killer cells decreased before and after operation for benign and malignant pulmonary nodules (P < 0.05). Conclusion Age, maximum diameter of nodules, spiculation sign, lobulation sign and pleural traction are independent factors in the determination of malignant pulmonary nodules. VATS treatment of pulmonary nodules can inhibit the acute inflammatory response after operation, and does not have a strong inhibitory effect on the immune function of patients, with high safety.

95 Changes and clinical significance of serum levels of brain-derived neurotrophic factor, substance P and interleukin-6 in patients with depression

Objective To study the changes of serum brain-derived neurotrophic factor (BDNF), substance P (SP) and interleukin-6 (IL-6) levels in patients with depression and to analyze their clinical significance. Methods Sixty cases of patients with depression admitted to the Third Hospital of Quzhou City from March 2016 to March 2018 were recorded as the depression group, according to the severity of illness, they were divided into mild group (n = 15), moderate group (n = 29) and severe group (n = 16), and divided into duration of disease <5 years (n = 28) and duration of disease ≥5 years (n = 32) according to the course of disease. Another 60 healthy persons who took physical examination at the same time were recorded as healthy control group. The levels of serum BDNF, SP and IL-6 in the two groups were compared respectively, and the relationship between the level of serum BDNF, SP and IL-6 in patients was analyzed. Results The level of serum BDNF of patients in the depression group was lower than that in the healthy control group, while the levels of SP and IL-6 were higher than the healthy control group, the differences were statistically significant (P < 0.05). The levels of serum BDNF in mild group, moderate group and severe group decreased gradually, while the levels of serum SP and IL-6 increased gradually, the differences were statistically significant among the there groups (P < 0.05). The level of serum BDNF in patients with depression <5 years was higher than that in patients with depression ≥5 years, while serum SP and IL-6 levels in patients with depression <5 years were lower than those in patients with depression ≥5 years, the differences were statistically significant (P < 0.05). Pearson correlation analysis showed that serum BDNF was negatively correlated with SP and IL-6 (r = -0.632, -0.601, all P < 0.05), while SP was positively correlated with IL-6 (r = 0.676, P < 0.05). Conclusion The level of serum BDNF of patients with depression is significantly decreased, while the levels of SP and IL-6 are increased. The levels of these three indexes are related to the severity and course of depression.

99 Risk factors analysis of bile duct injury after laparoscopic cholecystectomy in patients with acute cholecystitis

Objective To study the risk factors of bile duct injury (BDI) after laparoscopic cholecystectomy (LC) in patients with acute cholecystitis (AC). Methods Fifty-eight cases of AC patients with BDI after LC treatment in Qinghai Traffic Hospital (“our hospital” for short) from March 2017 to July 2018 were selected as observation group, and 50 cases of AC patients who were not complicated with BDI after LC in our hospital were selected as the control group. Both groups were routinely treated with LC. The relationship between BDI after LC and sex, age of patients was analyzed. The risk factor of BDI after LC was evaluated by Logistic regression analysis. Complications of the two groups were compared except for BDI. Results The the proportion of thickness of gallbladder wall >4 mm, the anatomy of gallbladder triangle had variation, and the physician experience <30 cases in the observation group was significantly higher than the control group (P < 0.05). According to the results of Logistic regression analysis, the thickness of the gallbladder wall >4 mm, the anatomy of gallbladder triangle had variation, and the physician experience <30 cases were the risk factors for BDI after LC (P < 0.05). There was no significant difference in the total incidence of complications besides BDI between the two groups (P > 0.05). Conclusion The risk factors of BDI in patients with AC after LC include the thickness of gallbladder wall >4 mm, the anatomy of gallbladder triangle has variation, and the physician experience <30 cases. Clinical intervention should be directed against the above factors, which will help to improve the safety of patients.

103 Expression and significance of CD269, CD317 in bone marrow cells of patients with multiple myeloma

Objective To study the expression and significance of CD269, CD317 in bone marrow cells of patients with multiple myeloma(MM). Methods A total of 86 patients with MM who were treated in Yuebei People′s Hospital of Guangdong province from January 2016 to May 2018 were selected as the observation group, another 80 patients with iron deficiency anemia during the same period were selected as the control group. The relative expressions of CD269 and CD317 in bone marrow cells, the levels of hemoglobin (HB), β2-microglobulin (β2-MG) and lactate dehydrogenase (LDH) were compared between the two groups, the relative expressions of CD269 and CD317 in different age and sex of the observation group were compared too. The correlations between the relative expressions of CD269, CD317 and the levels of HB, β2-MG and LDH were analyzed. Results The relative expressions of CD269 and CD317 in bone marrow cells in the observation group were significantly higher than that of the control group (P < 0.05). The levels of HB and β2-MG in the observation group were significantly lower than that in the control group (P < 0.05), but there was no significant difference in the level of LDH between the two groups (P > 0.05). There was no significant difference in the relative expressions of CD269 and CD317 in different age and sex of the observation group (all P > 0.05). According to Pearson correlation analysis, the results indicated that the relative expressions of CD269 and CD317 in bone marrow cells were negatively correlated with HB, β2-MG (P < 0.05), but there was no significant correlation with LDH (P > 0.05). Conclusion The expressions of CD269, CD317 in bone marrow cells of patients with MM are significantly elevated, while the levels of HB and β2-MG are significantly decreased. The relative expression of CD269 and CD317 are negatively correlated with HB and β2-MG respectively. Clinically, the expression of these four indicators can be further monitored to provide more comprehensive feedback on the actual condition of MM patients.

107 Effects of different analgesic methods on chronic pain and sleep quality after abdominal surgery

Objective To study the effects of different analgesic methods on chronic pain and nighttime sleep quality after abdominal surgery. Methods A total of 109 patients who underwent abdominal surgery in Cancer Hospital of China Medical University, Liaoning Cancer Hospital and Institute from January 2017 to January 2018 were enrolled. This study was a retrospective study in which patients were divided into a patient-controlled epidural analgesia (PCEA) group and an ultrasound-guided transversus abdominis plane (TAP) according to postoperative analgesia. PCA (Patient-controlled intravenous analgesia, PCIA) group (PCIA + TAP group). There were 50 cases in the PCEA group and 59 cases in the PCIA+TAP group. Postoperative epidural catheters were performed in the PCEA group for PCEA, PCIA+TAP group for TAP, and a venous analgesia pump. Acute and chronic pain, sleep quality and inflammatory factor levels were measured after surgery. Results At 24 h postoperatively, the visual analogue scale (VAS) score was lower in the PCIA+TAP group than in the PCEA group (P < 0.01). The VAS score in the PCIA+TAP group was lower than the PCEA group at 48 h after operation (P < 0.01). The incidence of chronic pain (CPSP) in the PCIA+TAP group 3 months after operation was significantly lower than in the PCEA group (P < 0.05). The VAS score and ID Pain score were significantly lower than those in the PCEA group (P < 0.01). The proportion of patients with Pmtsburgh sleep quality index (PSQI) score ≤7 at 48 h after operation was significantly higher than that in PCEA group (P < 0.05). The proportion of patients with PSQI score ≤7 was significantly higher 3 months after operation(P < 0.05). The levels of tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6) and IL-8 in the PCIA+TAP group 24,48 h after operation were significantly lower than those in the PCEA group(P < 0.01). There were no significant differences in TNF-α, IL-6 and IL-8 levels 3 months after operation between two groups (P > 0.05). Conclusion TAP combined with PCIA is effective for acute pain and analgesia after abdominal surgery. It can also effectively reduce postoperative CPSP, improve sleep quality and improve patients′ quality of life.

112 Effect of transcutaneous acupoint electrical stimulation on early recovery quality of patients after laparoscopic cholecystectomy

Objective To explore the clinical application of transcutaneous acupoint electrical stimulation in improving the  early recovery quality of patients after laparoscopic cholecystectomy (LC). Methods A total of 93 patients with LC admitted to Beijing Traditional Chinese Medicine Hospital Affiliated to Capital Medical University from January 2016 to October 2018 were selected as the study objects. They were divided into control group (46 cases) and research group (47 cases) according to random number table method. The control group was given Dexmedetomidine during LC, and the research group  received transcutaneous acupoint electrical stimulation combined with Dexmedetomidine during LC. Heart rate (HR) and mean arterial pressure (MAP) were compared at before anesthesia (T1), 5 min after intubation (T2), 5 min after establishment of pneumoperitoneum (T3), 15 min after establishment of pneumoperitoneum (T4), and 5 min after operation (T5). The level of interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α), neuron specific enolation (NSE) and superoxide dismutase (SOD) at T1, T5 and postoperative 1 day(T6) were compared between the two groups. The postoperative delirium rating scale score, sedation score, visual analogue score (VAS) and comfort score were compared between the two groups. The occurrence of adverse reactions during treatment was recorded. Results The HR and MAP of control group at T2-T5 time point were higher than those at T1 time point (P < 0.05). There was no significant difference in HR and MAP between the time points of T2-T5 and T1 in the research group (P > 0.05). HR and MAP of the research group at T2-T5 time points were significantly lower than those in the control group at the same time point(P < 0.05). The SOD of the two groups at T5 time point was lower than that before treatment, but the SOD of the research group was higher than that of the control group; NSE was higher than before treatment, but lower in the research group than in the control group (P < 0.05). The levels of IL-6 and TNF-α at T5 and T6 time points in both groups were higher than those at T1 time point, but the levels of IL-6 and TNF-α in the research group were lower than those in the control group (P < 0.05). There were no significant differences in VAS score and comfort score between the two groups (P > 0.05). Postoperative delirium rating scale score in the research group was lower than that in the control group, and sedation score was higher than that in the control group (P < 0.05). There was no significant difference in the total incidence of adverse reactions between the two groups (P > 0.05). Conclusion The application of transcutaneous acupoint electrical stimulation in LC can maintain hemodynamic stability, improve the level of serum SOD, NSE, IL-6 and TNF-α, without increasing the incidence of adverse reactions, and effectively improve the early recovery quality of patients.

116 Study on analgesic effects of general anesthesia of laryngeal mask airway in senile femoral neck fracture with internal fixation

Objective To investigate the analgesic effects of general anesthesia of laryngeal mask airway and combined spinal-epidural anesthesia (CSEA) in senile femoral neck fracture with internal fixation. Methods A total of 80 senile femoral neck fracture patients with internal fixation in the hospital from January 2015 to May 2018 were selected, and they were divided into control group and observation group by random number table method, with 40 cases in each group. Control group was given CSEA, while observation group was given general anesthesia of laryngeal mask airway. The preoperative and postoperative 6 h pain of two groups were evaluated by visual analogue scale (VAS); the heart rate (HR), oxygen saturation (SpO2), mean arterial pressure (MAP) of two groups before anesthesia, after anesthesia for 5 min, during internal fixation, after operation were detected. The anesthetic effects and adverse complications in the two groups were observed. Results There was no statistical difference in the VAS score between the two groups before anesthesia (P > 0.05). After anesthesia for 6 h, the VAS score of the two groups decreded, and the VAS score of the observation group was lower than that of control group (P < 0.05). There was no statistical difference in the HR, SpO2, MAP levels between the two groups before anesthesia (P > 0.05). After anesthesia for 5 min, the HR and MAP of the observation group decreased compared with those before anesthesia, and they gradually recovered to the level before anesthesia. There was statistical difference in the HR and MAP levels after anesthesia for 5 min, during internal fixation, after operation (P < 0.05). The total excellent and good rate of anesthesia in the observation group was higher than that in the control group, and the total incidence of complications was lower than that in the control group, with statistically significant differences (P < 0.05). Conclusion The effect of general anesthesia of laryngeal mask airway in senile femoral neck fracture with internal fixation is better than that of CSEA, it can achieve stable vital signs, good analgesic effects and low incidence of complications, which is worthy of clinical promotion and application.

124 Clinical effect of Peiyuan Quyu Decoction on treating idiopathic pulmonary fibrosis with phlegm and stasis blocking collaterals

Objective To explore the clinical effect of Peiyuan Quyu Decoction in treating idiopathic pulmonary fibrosis (IPF) with phlegm and stasis blocking collaterals. Methods From July 2016 to December 2017, 60 patients with IPF in Guangdong Integrated Chinese and Western Medicine Hospital were divided into control group and treatment group by random number table method with 30 cases in each group. The control group was treated with oral Prednisone tablets and the treatment group was treated with Peiyuan Quyu Decoction for 6 months. The clinical effective rate, 6-minute walking distance, quality of life evaluation, shortness degree of breath and changes of lung function were compared between the two groups. Results The total effective rate in the control group was significantly lower than that in the treatment group and the difference is statistically significant (P < 0.05). The 6-minute walking distance, pulmonary function related index, carbon monoxide dispersion (DLCO), ATAQ-IPF score and dyspnea scale (mMRC) grade improved significantly in the treatment group, but the improvement in the control group was not obvious(P > 0.05), the difference between the two groups was statistically significant (P < 0.05). Conclusion Peiyuan Quyu Decoction can improve the clinical symptoms of phlegm and stasis obstruction collaterals type IPF, at the same time, can also improve the pulmonary function parameters, reduce the acute exacerbation of the condition, make life endurance and quality of life significantly improved, and the curative effect is reliable.

Objective To explore the medication rules of traditional Chinese medicine in the treatment of carotid atherosclerosis. Methods The articles about carotid atherosclerosis were collected by using Wanfang, VIP and CNKI from January 2005 to December 2017. A total of 109 articles were up to the requirements at last. Then the articles were selected and put into software excel to establish a database. The TCM medication rule was analyzed by SPSS statistical software, including the frequency of the drugs being used, rate, four nature of drugs, five flavours, channel tropism, drug category, as well as the therapeutic principle and method. Results Total 147 Chinese herbal medicines were used in the literatures, the total frequency was 1007 times. The most frequently used TCM medications were Radix Salviae Miltiorrhizae, Rhizoma Ligustici Chuanxiong, Radix Astragali seu Hedysari. The main drug properties were warm, calm and cold, the cumulative frequency was 92.3%. The main characters of the five flavors were sweet, bitter, pungent, the cumulative frequency was 90.4%. The main channel tropisms of the drugs were liver, spleen and heart, the cumulative frequency was 86.4%. The main types of drugs were blood-activating and stasis-removing drugs, qi-tonifying drugs, blood-tonifying drugs and warmly resolving cold-phlegm drugs, the cumulative frequency was 45.1%. Conclusion The treatment of carotid atherosclerosis with Chinese medicine decoction is mainly based on activating blood circulation to dissipate blood stasis.

132 Clinical effect of Canggui Tanxue acupuncture at ashi point in the treatment of muscle, bone and joint pain induced by aromatase inhibitor of breast cancer

Objective To explore the clinical effect of Canggui Tanxue acupuncture at ashi point in the treatment of muscle, bone and joint pain induced by aromatase inhibitor of breast cancer. Methods Seventy-two breast cancer patients with muscle, bone and joint pain because of taking aromatase inhibitor admitted to Galactophore Department of the First Affiliated Hospital of Guangzhou University of Chinese Medicine from July 2016 to September 2017 were randomly divided into control group (36 cases) and observation group (36 cases). The control group was treated with oral Western medicine, based on the control group, the observation group was given Canggui Tanxue acupuncture at ashi point. Both groups were treated for 3 courses. The clinical effect, the scores of visual analogue scale (VAS), Barthel index (BI) and the changes of lumbar spine bone mineral density (BMD) were compared between two groups. The levels of serum cytokine interferon-γ (IFN-γ), interleukin-4 (IL-4), estradiol (E2) were detected and the occurrence of adverse reactions was observed. Results ①The total effective rate of the observation group was significantly higher than the control group (P < 0.05). ②After treatment, the VAS scores of the two groups were significantly lower than those before treatment (P < 0.01), and the observation group was significantly lower than the control group (P < 0.01). ③After treatment, the BI score and BMD values of lumbar spine in the two groups were significantly higher than those before treatment (P < 0.05), which of the observation group were significantly higher than the control group (P < 0.05). ④Compared with before treatment, the levels of IFN-γ were decreased significantly, the levels of IL-4 were increased significantly in the two groups after treatment (P < 0.01), while the levels of E2 did not change significantly (P > 0.05); compared with the control group, the levels of IFN-γ were significantly decreased and the levels of IL-4 were significantly increased in the observation group (P < 0.01), but there was no significant difference in serum E2 levels between the two groups (P > 0.05). ⑤There were no adverse reactions in both groups during the course of treatment. Conclusion Canggui Tanxue acupuncture at ashi point can effectively improve the muscle, bone and joint pain caused by appliaction of aromatase inhibitor in breast cancer patients, and can significantly enhance the living ability of patients, which is safe, feasible, and worthy of clinical promotion and application.

136 Effect of acupuncture and massage combined with acupoint injection on nerve function and motor function of paralysis patients with craniocerebral trauma

Objective To investigate the effect of acupuncture and massage combined with acupoint injection on nerve function and motor function of paralysis patients with craniocerebral trauma. Methods Two hundred and fifty-six paralysis patients with craniocerebral trauma admitted to Intensive Care Unit, West China Hospital, Sichuan University from January 2016 to December 2017 were selected as research objects and divided into control group and observation group by random number table method, with 128 cases in each group. Both groups were given routine Western medicine treatment, on basis of which, the observation group was added with acupuncture and massage combined with acupoint injection. The clinical curative effect and functional recovery conditions of the two groups were observed and analyzed after treatment for 28 days. The data was analyzed by SPSS 19.0 software. Results After treatment, the total effective rate of observation group was significantly higher than that of control group, the difference was statistically significant (P < 0.05). Compared with before treatment, the aphasia score and Fugl-Meyer assessment (FMA) score in the two groups were significantly improved after treatment, while National Institute of Health stroke scale (NIHSS) score was significantly reduced after treatment, the differences were all statistically significant (P < 0.05). After treatment, the aphasia score and FMA score in the observation group were significantly higher than those in the control group, NIHSS score was significantly lower than that in the control group, the differences were statistically significant (P < 0.05). Conclusion Acupuncture and massage combined with acupoint injection in the treatment of paralysis patients with craniocerebral trauma has a significant effect, which can effectively promote language ability and intelligence, and significantly improve the degree of nerve damage and motor function, with high safety, thus it is worthy of clinical optimization and promotion.

The study of ancient medical history in the local chronicles can provide reference for medical administration nowadays; the collation of bibliography of ancient medical books in the local chronicles can make up for the lack of bibliography; the information mining of doctors, medical affairs and diseases can not only make up for the lack of medical history research, but also make up for medical humanistic materials in the teaching work, which is beneficial to the cultivation of medical humanistic spirit of medical students; there is an almost total lack of distribution of herbal resources in ancient medical books, but the study of Chinese herbal medicine in the local chronicles can make up for this shortage.

143 Analysis of effect of interventional treatment for brachiocephalic arteritis

Objective To explore the clinical efficacy of endovascular angioplasty therapy for brachiocephalic arteritis. Methods The clinical data of 19 patients with brachiocephalic arteritis in Xuanwu Hospital, Capital Medical University from April 2005 to June 2017 were analyzed. All patients underwent balloon dilatation or stent implantation treatment. Results All patients underwent balloon dilatation or stent implantation, and the operation was successful. Balloon dilatation of common carotid artery, subclavian artery and vertebral artery were 11, 9 and 1 cases respectively, and stent implantation of nameless artery, common carotid artery, subclavian artery and vertebral artery were 1, 1, 2 and 1 case respectively. The follow-up period ranged from 12 to 98 months, of which 19 cases were followed up for more than 1 year, 17 cases for more than 2 years, 9 cases for more than 3 years to less than 5 years and 8 cases for more than 5 years. The symptoms of all patients were improved, including dizziness in 10 cases, blurred vision in 4 cases, dark quinone in 3 cases, unclear speech in 3 cases, and upper limb cooling in 2 cases. During the follow-up period, 1 patient died of cerebral hemorrhage 12 months after operation, 1 patient died of nasopharyngeal carcinoma 13 months after operation, and 1 patient lost follow-up 25 months after operation. Conclusion Endovascular angioplasty is safe and effective in the treatment of brachiocephalic arteritis.

147 Dual-phase CT imaging study in rabbit liver with low-dose contrast agent and low-kV

Objective To investigate the feasibility of dual-phase CT enhanced imaging of rabbit liver in low-kV scan mode with low concentration contrast agent. Methods Fourteen rabbits (3.5-4.0 months age, weighing 2.0-2.5 kg) were selected and divided into groups A, B and C according to the scanning mode, and the tube voltages were 120, 100 and 80 kV, respectively, the contrast medium concentration was 350 mgI/mL in group A and 270 mgI/mL in group B and C. All subjects underwent dual-phase enhanced CT scanning according to the scanning conditions of group A-C. The CT value and noise of portal vein, abdominal aorta, abdominal wall muscle and liver parenchyma were measured, and the contrast noise ratio (CNR) and signal-to-noise ratio (SNR) were calculated. All images were subjectively scored by two experienced radiologists. Results There was significant difference in effective radiation dose among the three groups (P < 0.05). There was no significant difference in abdominal aorta CT value, CNR, SNR, hepatic parenchyma CT value and CNR between the three groups during arterial phase (P > 0.05), but there was significant difference in hepatic parenchyma SNR and noise between the three groups (P < 0.05), and the noise in group C was higher than that in group A and B; the difference in abdominal aorta CT value between group B and C was significant (P < 0.05); there was a systematic difference in noise between the two groups (P < 0.05). There was no significant difference in CT value of hepatic parenchyma and CNR, SNR and noise of abdominal aorta, portal vein and hepatic parenchyma among the three groups during portal vein phase (P > 0.05); there was significant difference in CT value of portal vein and abdominal aorta among the three groups (P < 0.05); CT values of portal vein in group A and B were compared with group C, group C was higher than group A and group B, the difference was statistically significant (P < 0.05); group B had higher CT value of abdominal aorta compared with group A, the difference was statistically significant (P < 0.05). There were no significant differences in the subjective scores of contrast, noise, sharpness and overall in the three groups (P > 0.05), but the contrast and noise of group A was higher than that of group C, and the differences were statistically significant (P < 0.05). There were no significant differences in the subjective scores of contrast, noise, sharpness, and overall between the three groups in the portal vein phase, and there was no significant difference between the two groups (P > 0.05). Conclusion Low-concentration contrast agent combined with low-kV scanning can reduce the effective radiation dose, increase the vascular CT value, combined with ASIR reconstruction technology, can improve the image quality, meet the diagnostic needs, and provide a theoretical basis for clinical liver enhancement scanning.

151 Application value of high frequency ultrasound in the diagnosis of congenital hip dislocation and developmental dysplasia of the hip in infants

Objective To analyze the value of high-frequency ultrasound in the diagnosis of congenital dislocation of hip and dysplasia of hip in infants. Methods From January 2017 to June 2018, 1500 infants aged 6 months who underwent hip screening ultrasound examination in Jiangyou People′s Hospital of Sichuan Province were selected as the research objects. All infants were examined by Graf′s hip ultrasonography. Based on the results of high frequency ultrasound, the shapes of infants′ hip joint were accurately evaluated to determine whether infants had congenital dislocation of hip joint and dysplasia of hip joint. The age distribution of children with developmental dysplasia of the hip was analyzed. Results Among 1500 infants aged 6 months, 15 infants had abnormal hip development, the incidence was 1.00%, and the diagnostic accuracy of high frequency ultrasound was 100%. The incidence of hip dysplasia in females was higher than that in males in 15 cases of hip dysplasia, and the difference was statistically significant (P < 0.05). Among 27 cases of hip dysplasia, the left side was more than the right side, and the difference was statistically significant (P < 0.05). Among 15 infants with dysplasia of hip joint, the incidence of infants aged >5 to 6 months was the highest, followed by infants aged >4 to 5 months and >2 to 3 months. Conclusion High frequency ultrasound is of great value in the diagnosis of congenital dislocation of hip and dysplasia of hip in infants.

155 Effects of different doses of Rosuvastatin on lipid metabolism and homocysteine levels in elderly patients with coronary heart disease and diabetes mellitus

Objective To observe the effects of different doses of rosuvastatin on lipid metabolism and homocysteine levels in elderly patients with coronary heart disease and diabetes mellitus. Methods From May 2016 to May 2017, 100 elderly patients with diabetes mellitus and coronary heart disease admitted to Beijing Haidian Hospital were selected and divided into control group and observation group according to the random number table method, with 50 cases in each group. The control group received low-dose Risuvastatin Tablets at 10 mg/time and once a day, while the observation group received high-dose Risuvastatin Tablets at 20 mg/time and the course of treatment was 6 months. Total cholesterol (TC), triglyceride (TG), low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), homocysteine (Hcy) and plasma fibrinogen (Fg) levels before and after treatment were observed in the two groups. Results Before treatment, TG, TC, LDL-C, HDL-C, Hcy and Fg of the two groups were compared, and the differences were not statistically significant (P > 0.05). At 1,6 months after treatment, TG, TC, LDL-C and Hcy in the two groups were all decreased. The levels of TG, TC, LDL-C and Hcy in the observation group were lower than those in the control group at 1 month and 6 months after treatment (P < 0.05). HDL-C increased in both groups 1,6 months after treatment, and was higher in the observation group than the control group (P < 0.05). Fg was decreased in the two groups 1,6 months after treatment, and the observation group was lower than that in the control group (P < 0.05). Conclusion Large dose of Rosuvastatin can effectively control blood lipid, Fg and Hcy levels in elderly patients with coronary heart disease and diabetes mellitus.

159 Clinical value of the ratio of neutrophils to lymphocytes in predicting the severity of acute pancreatitis

Objective To investigate the value of peripheral blood neutrophil/lymphocyte ratio (NLR) in predicting the severity of acute pancreatitis (AP). Methods A retrospective analysis was performed on the clinical data of 114 AP patients admitted to the Changzheng Hospital Affiliated to the Second Military Medical University from January 2015 to June 2017, 94 healthy volunteers were selected as the control group and 114 patients as the AP group. Blood routine, liver and kidney function, and electrolyte indexes were compared between the two groups. According to the new classification standard of Atlanta, AP patients were divided into three groups, mild (MAP), moderate (MSAP) and severe (SAP). Spearman rank correlation was used to compare the correlation between NLR and serum creatinine and calcium ion level, and the diagnostic value of NLR, serum creatinine and calcium ion in SAP was further compared by ROC curve. Results Compared with healthy control group, NLR and serum creatinine were significantly increased in AP group (all P < 0.05). There was a correlation between NLR and AP severity (P < 0.05), positive correlation with serum creatinine (r = 0.191, P < 0.05), and negatively correlated with calcium ions (r = -0.244, P < 0.01). The area under the curve of NLR diagnosis of SAP was 0.81 (95%CI: 0.703-0.924), and the difference was not statistically significant compared with serum creatinine and calcium ions (all P > 0.05). Conclusion NLR has certain clinical significance in predicting the severity of AP.

163 Effects of HPV infection on cervical immune microenviroment

Objective To compare the changes of cervical local cytokines and vaginal microecological indicators after the infection of high-risk and low-risk human papillomavirus (HPV), and to analyze the effects of different HPV infections on the cervical local immune microenvironment. Methods Collection in March 2017 to July 2018 in the First Affiliated Hospital of Hu′nan University of Medicine Outpatient Clinic women specimens of cervical and vaginal secretions was carried out, PCR - reverse dot hybridization method was used to detect HPV, according to the results, the 155 cases were divided into high risk type HPV group(55 cases), low-risk HPV group (40 cases) and control group (60 cases), who were not infected with HPV. Cervical cell preservation solution enzyme linked immunosorbent test was carried out in the gamma interferon (IFN-γ), interleukin 4 (IL-4) and IL-21 content, and according to the national clinical inspection procedures, the fungi, vaginal trichomoniasis, clue cell, cleanliness, and pH checks were carried out. Results Compared with the control group and low-risk HPV group, the content of IFN-γ and IL-21 in high-risk HPV group was significantly decreased (P < 0.01), but the content of IL-4 was significantly increased (P < 0.01). Compared with the control group, the IFN-γ content of low-risk HPV group was significantly reduced (P < 0.01), but the difference between IL-4 and IL-21 was not statistically significant (P > 0.05). After correlation analysis, IL-21 was positively correlated with IFN-γ content in high-risk HPV group, low-risk HPV group and control group (r = 0.74, P < 0.01), r = 0.70, P < 0.01; r = 0.66, P < 0.01), IL-4 and IFN-γ contents in the high-risk and low-risk HPV groups were negatively correlated (r = -0.33, P < 0.05; r = -0.44, P < 0.01); IL-21 was negatively correlated with IL-4 in the high-risk HPV group (r = -0.30, P < 0.05). In addition, there were no statistically significant differences among the three groups in microecological indicators such as fungi, vaginal trichomonas, clue cells, cleanliness and pH (P > 0.05). Conclusion High-risk HPV is more likely to cause abnormal expression of local IL-4 and IFN-γ in the cervix after HPV infection than low-risk HPV infection, which may be related to the low expression of IL-21 with immune-promoting effect. Therefore, it is very important to maintain the local normal immune microenvironment in the cervix and restore the immune regulatory function of the body to remove HPV and block the continuous infection of HPV.

167 Changes and clinical significance of thrombus elastograms in pregnant women

Objective To explore the changes and clinical significance of thrombus elastograms in pregnant women. Methods From January 2018 to December 2018, 77 pregnant women with gestational age < 36 weeks were selected as the observation group 1, 77 pregnant women with gestational age ≥ 36 weeks as the observation group 2, and 77 healthy women without pregnancy during the same period as the control group. The parameters of TEG of the three groups of women were compared, and the reference value range was established by comparing the different parameters, and the correlation analysis was conducted between the traditional blood coagulation indexes and TEG test results of pregnant women. Results There were statistically significant differences in TEG parameters comparison, pregnant women in the coagulation reaction time (R), clotting time (K), blood clots forming speed (α angle), maximum amplitude (MA), MA determine the rate of 30 min after fibrinolysis (LY30) and synthetic blood clotting index (CI) between observation group 1,2 and control group (P < 0.05). In terms of routine coagulation parameters, there were statistically significance differences between pregnant women and healthy women in prothrombin time (PT) and activated partial thrombin time (ATPP) (P < 0.05). Compared with observation group 1, except LY30, TEG test indexes of observation group 2 were significantly shortened (P < 0.05), there was no significant change in other indexes. In terms of the correlation between TEG and conventional coagulation parameters in pregnant women, except for α angle, the parameter values of other indicators had a certain correlation with traditional coagulation parameters, and PT and MA were positively correlated. APTT was positively correlated with R and negatively correlated with MA and CI. The positive correlation between INR and LY30 was low. Plt was negatively correlated with K value and positively correlated with MA and CI value. TT was positively correlated with MA. Conclusion TEG can dynamically and comprehensively observe the maternal blood coagulation function, and correct the maternal blood coagulation function reasonably and effectively according to the indicators, which has important clinical significance for reducing the surgical risk.

171 Effect of TCM multi-channel characteristic nursing program on gastrointestinal function, bladder function recovery, mental state and self-care ability of patients with cervical cancer radical surgery

Objective To investigate the influence of TCM multi-channel nursing program on mental state and self-care ability of patients and nursing satisfaction with cervical cancer radical surgery. Methods From January 2016 to March 2017, in Huangjiahu Hospital of Hubei University of Traditional Chinese Medicine, 86 patients with cervical cancer were selected, according to random number table method, they were divided into control group and observation group according to the radom number table method, with 43 cases in each group. The patients in the control group were treated with routine basic nursing, and the patients in the observation group were given multi-channel characteristic nursing program. The psychological states, self-care ability and nursing satisfaction of patients in two groups before and after intervention were recorded and evaluated. Results The psychological states of patients in observation group after intervention was better than control group, the mental normal rate in observation group was higher than control group, the incidences of negative emotions, such as anxiety, fear and pessimism in observation group were significantly lower than control group, the differences were statistically significant (P < 0.05). The self-care ability was significantly better than the control group, and the scores of health knowledge level, self-responsibility, self-concept, self-care skills and other abilities were significantly higher than the control group, the differences were statistically significant (P < 0.05). The nursing satisfaction rate of observation group was significantly higher than control group, the differences were statistically significant (P < 0.05). Conclusion Multi-channel characteristics of traditional Chinese medicine nursing is good to improve the psychological status of patients with cervical cancer, alleviate postoperative bad mood and improve self-care ability, which has high nursing satisfaction, it is worth popularization and application in the clinical practice.

178 Effects of comprehensive nursing intervention on relieving preoperative anxiety in patients undergoing thoracotomy

Objective To explore the effects of comprehensive nursing intervention on the preoperative anxiety of thoracic tumor patients. Methods Eighty-four thoracic tumor inpatients from Beijing Chest Hospital, Capital Medical University from January 2017 to January 2018 were selected. They were divided into intervention group and control group according to the random number table method, with 42 cases in each group. The control group received routine preoperative care and treatment, the intervention group received targeted personalized comprehensive nursing intervention on the basis of the control group. SAS was used for anxiety assessment on admission and before operation. Results There was no significant difference of SAS scores between the two groups on admission (P > 0.05); before operation, the SAS scores in both group were significantly higher than those on admission, and control group were significantly lower than those in intervention group, the differences were statistically significant (P < 0.01). Conclusion The Nursing intervention can reduce the anxiety state of patients with thoracic tumor.
