DOI: 10.20047/j.issn1673-7210.2022.33.01
Objective To explore the material basis and mechanism of Huoxue Jiedu Ⅰ Prescription in the treatment of atherosclerosis based on network pharmacology, and to carry out experimental verification. Methods The active ingredients and targets of Huoxue Jiedu Ⅰ Prescription were retrieved through the traditional Chinese medicine systems pharmacology database and analysis platform, and the information of protein targets was standardized by using Uniprot protein database. Target genes related to atherosclerosis were obtained from online Mendelian inheritance in man, GeneCards, and DrugBank databases, and common targets of drugs and diseases were screened. The “ingredient-target” network was constructed and key active components were screened. The STRING platform was used to construct the common target protein-protein interaction network and screen the key target genes. Matascape database was used for enrichment analysis of GO and KEGG pathways, and a multidimensional network diagram of “drug-target-pathway” was constructed. Thirty SPF 8-week-old male ApoE-/- mice, weighing 18-22 g, were fed with high-fat diet for 12 weeks to establish atherosclerosis model. They were divided into model group, statin group, and Huoxue Jiedu Ⅰ Prescription group according to the random number table method, with ten mice in each group. Ten male C57BL/6J mice of the same age and genetic background were selected as normal control group. Statin group was given 3.400 mg/(kg·d) of Atorvastatin, Huoxue Jiedu Ⅰ Prescription group was given 2.171 g/(kg·d) of drug liquid, normal control group and model group was given the same amount of normal saline. All the four groups were given gavage for 12 weeks. At the end of the experiment, colorimetric method was used to detect blood lipid indexes of the four groups, and HE staining and plaque proportion of aorta were quantitatively analyzed. The protein expressions of tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), caspase-3, and nuclear factor-κBp65 (NF-κBp65) in aortic tissues of the four groups were detected by Western blot. Results A total of 25 active ingredients of Huoxue Jiedu Ⅰ Prescription were screened, 214 potential targets were identified, and 118 targets were common with atherosclerotic diseases. GO analysis showed 2 173 results, including 165 pathways. The key active ingredients of Huoxue Jiedu Ⅰ Prescription in the treatment of atherosclerosis include quercetin, baicalin, β-sitosterol, ellagic acid, berberine, tetrandrine afrika, berberine, etc. It can regulate TNF, AKT1, IL-6, VEGFA, TP53, IL-1B, JUN, CASP3, PTGS2, PPARG, and other related targets. It may affect the occurrence and development of atherosclerosis through TNF, Toll-like receptor, NF-κB, HIF-1, PI3K-Akt, apoptosis, T-cell receptor, MAPK, Foxo, and other signaling pathways. The results of the experiment showed that serum total cholesterol (TC), triglyceride (TG), and low density lipoprotein cholesterol (LDL-C) in the model group were higher than those in the normal control group, and the level of high density lipoprotein cholesterol in the model group was lower than that in the normal control group, serum TC, TG, and LDL-C in the Huoxue Jiedu Ⅰ Prescription group were lower than those in the model group, and the differences were statistically significant (P < 0.05). The results of HE staining of aortic root showed that there was no atherosclerotic plaque deposition in the aortic root of the normal control group, and different degrees of atherosclerotic plaque deposition could be observed in the aortic root wall of the other three groups. Quantitative analysis of plaque proportion showed that the proportion of plaque area in the Huoxue Jiedu Ⅰ Prescription group was lower than that in the model group, and the difference was statistically significant (P < 0.05). The protein levels of TNF-α, IL-6, caspase-3, and NF-κBp65 in the model group were higher than those in the normal control group, while those in the Huoxue Jiedu Ⅰ Prescription group were lower than those in the model group, and the differences were statistically significant (P < 0.05). Conclusion This study preliminarily reveales the “multi-component, multi-target, multi-pathway” action network of Huoxue Jiedu Ⅰ Prescription in the treatment of atherosclerosis, and experimentally verifies that its mechanism of action is related to the inhibition of related inflammatory pathways and apoptosis, providing a research basis for the clinical application of Huoxue Jiedu Ⅰ Prescription in the treatment of atherosclerosis.

Objective To observe the effect of moxibustion of zúsānlǐ and tàixī on cell apoptosis and expression of related proteins in decidua tissues of pregnant rats with kidney deficiency-corpus luteum inhibition of abortion model. Methods Thirty-two pregnant female SD rats (SPF, 8-10 weeks, 220-250 g) were divided into normal group, model group, Dydrogesterone group, and moxibustion group according to the random number table method, with eight rats in each group. The normal group was given normal saline intragastric administration, and the other groups were given “Hydroxyurea + Mifepristone” to prepare the kidney deficiency-corpus luteum inhibition of abortion model of pregnant SD rats. At the same time, the Dydrogesterone group was given Dydrogesterone Tablets 3.02 mg/kg, 2 ml/day by gavage. In the moxibustion group, bilateral zúsānlǐ and tàixī points were moxibusted for 15 min/(time·point) every day, and the samples were collected after nine days of continuous intervention. The number of embryos in each group was calculated and the diameter of uterus was measured. The cell apoptosis of decidua tissues of each group was detected by TUNEL method, and the apoptosis index (AI) value was calculated. The protein expressions of Bax, Bcl-2, and Caspase-3 in decidua tissues of each group were determined by immunohistochemistry. Results Compared with the normal group, the uterine diameter of the model group was decreased, and the difference was statistically significant (P < 0.05). Compared with the model group, the uterine diameter of the moxibustion group was increased, and the difference was statistically significant (P < 0.05). There were no significant differences in the total number of intrauterine embryos between normal group and model group, model group and moxibustion group (P > 0.05). Compared with the normal group, the average OD value of Bax and Caspase-3 protein in the model group was increased, and the differences were statistically significant (P < 0.05). Compared with the model group, the average OD value of Bax and Caspase-3 protein in the moxibustion group was decreased, and the differences were statistically significant (P < 0.05). Compared with the normal group, the average OD value of Bcl-2 protein in the model group was decreased, and the difference was statistically significant (P < 0.05). Compared with the model group, the average OD value of Bcl-2 protein in the moxibustion group was increased, and the difference was statistically significant (P < 0.05). Compared with the normal group, the cell AI value of the model group was increased, and the difference was statistically significant (P < 0.05). Compared with the model group, the cell AI value of the moxibustion group was decreased, and the difference was statistically significant (P < 0.05). Conclusion Moxibustion at zúsānlǐ and tàixī may regulate cell apoptosis at the maternal-fetal interface by up-regulating the expression of Bcl-2 protein, down-regulating the overexpression of Bax and Caspase-3 protein and reducing the AI in decidua tissues of pregnant rats with kidney deficiency and corpus luteum inhibition of abortion

Objective To analyze the characteristics of serum metabolites and to search for differential metabolites and metabolic pathways of preterm infants in different growth patterns using metabolomics techniques. Methods Serum samples were collected from 55 preterm infants by six months of adjusted age who were hospitalized in Neonatal Intensive Care Unit and were followed by Department of Child Health Care of Huai’an Maternal and Child Health Hospital Affiliated to Yangzhou University from March 2019 to December 2020. They were divided into catch-up growth group (35 cases) and without catch-up growth group (20 cases) according to whether the Z-score difference between corrected age of six months and birth weight was ≥0.67. The serum metabolic profiles were detected by liquid chromatography-tandem mass spectrometry, the metabolites and metabolic pathways with significant differences between two groups were identified, respectively. Results A total of 28 serum differential metabolites were obtained. Among them, there were 27 upregulated substances, mainly amino acids, glycerol phospholipids, and organic heterocyclic compounds, etc.; down-regulated substance 1, belonging to fatty acids. The metabolite difference pathways mainly concentrated in metabolism and synthesis of amino acids. The most significant enrichment pathways were arginine and proline metabolism and glutathione metabolism. Conclusion Premature infants with catch-up growth in the early stage are more prone to lack of various nutrients, especially amino acids, because of their rapid growth. It should pay attention to the reasonable supplement of amino acids for preterm infants. MENG Jiaoyu1 LI Ling’e2 WANG Yajing1 LIU Qianying3 LYU Ziyue3
DOI: 10.20047/j.issn1673-7210.2022.33.07
Psoriasis vulgaris is called “Baibi” in traditional Chinese medicine. Modern medicine believes that the occurrence and development of psoriasis is related to the abnormal activation of T lymphocytes. Studies have shown that the helper T lymphocyte (Th)1/Th2 balance in patients with psoriasis is shifted toward the differentiation dominance of Th1 cells, and that Th17 cells can induce and exacerbate inflammatory responses in psoriatic lesions through the IL-23/Th17 axis. Studies on the treatment of psoriasis vulgaris by traditional Chinese medicine have found that traditional Chinese medicine decoction, Chinese patent medicine, and external treatment of traditional Chinese medicine such as fire acupuncture therapy and traditional Chinese medicine medicated bath can effectively improve the Th1 drift in patients with psoriasis and inhibit the IL-23/Th17 axis, so as to achieve the purpose of the treatment of psoriasis. Therefore, this paper reviews the literature on the clinical efficacy and mechanism of traditional Chinese medicine in the treatment of psoriasis vulgaris, in order to provide more geographical and methodological basis for the clinical application of traditional Chinese medicine in the diagnosis and treatment of psoriasis vulgaris, and to provide some new ideas and directions for the treatment of psoriasis vulgaris in the future.

FU Jia LI Shiqi ZHAO Xiaoting ZHENG Xiaodong
DOI: 10.20047/j.issn1673-7210.2022.33.08
HSPB5 is a protein with a highly conserved crystal structure, which exists in the form of dynamic oligomers in vivo. HSPB5 expression is regulated by various physiological and pathological factors, such as increased expression under heat stress, and its activity is regulated by various post-translational modifications such as phosphorylation, acetylation, and glycosylation. Under physiological conditions, HSPB5, as a chaperone-like protein can reduce the aggregation of misfolded proteins, and can also interact with various proteins to inhibit apoptosis. Under pathological conditions, HSPB5 can also affect multiple disease processes, such as Alzheimer’s disease, cardiomyopathy, tumor, and hypoxia, etc. This paper briefly reviews the role and the mechanism of HSPB5 in physiological and pathological conditions states. Regulating the expression level and activity of HSPB5 may serve as a new therapeutic strategy for a variety of diseases.

The female reproductive tract has a microbiome that lives in symbiosis with the human body, Lactobacillus is usually the dominant bacterium, this genus plays an important role in maintaining the health of the female reproductive tract. Dysregulation of reproductive tract microbiota may promote the development of human papillomavirus (HPV) infection and cervical intraepithelial neoplasias (CIN), and eventually lead to cervical malignancy. As the severity of CIN increases, depletion of dominant Lactobacillus increases, which may negatively affect the rate of disease regression, for example, leading to a pro-inflammatory environment that increases the expression of HPV E6 and E7 oncogenes and proliferation of malignant cells. This paper reviews the correlation between the composition of vaginal microbiota and the occurrence and development of cervical cancer, in order to better understand the influence of the microbiota on cervical cancer, which may contribute to explore new diagnostic or prognostic microbial markers of cervical cancer, and seek new prevention or treatment drug targets.

DOI: 10.20047/j.issn1673-7210.2022.33.17
Objective To investigate the clinical efficacy of endoscopic posterior lumbar interbody fusion for treatment of single-level lumbardegenerative disease. Methods The clinical data of 48 patients of single segment lumbar degenerative diseases treated in Baoji Hospital of Traditional Chinese Medicine from May 2018 to January 2021 were retrospectively analyzed. They were divided into minimally invasive group and open group according to different surgical methods, with 24 cases in each group. The minimally invasive group was treated with endoscopic posterior lumbar interbody fusion, and the open group was treated with traditional posterior lumbar fusion. The perioperative indexes, visual analogue scale (VAS) score of low back pain and Oswestry dysfunction index (ODI) were compared between the two groups. The clinical efficacy was evaluated according to the scoring standard of low back pain surgery in the spine group of Chinese orthopaedic society. Results The incision length of the minimally invasive group was shorter than that of the open group, the operation time was longer than that of the open group, the blood loss was less than that of the open group, and the length of hospital stay was shorter in the open group, and the differences were statistically significant (P < 0.05). Intra-group comparison: there were statistically significant differences between the two groups at each time point (P < 0.05). Comparison between groups: there were no significant differences in VAS score and ODI index between the two groups before operation (P > 0.05), and the VAS score and ODI index of the micro-wound group were lower than those of the open group at one week after operation (P < 0.05). However, there were no significant differences in VAS score and ODI index between the two groups at six months after operation and at the last follow-up (P > 0.05). There was no significant difference in clinical efficacy between the two groups (P > 0.05). Conclusion Endoscopic lumbar decompression and interbody fusion technology for the treatment of single level lumbar degenerative diseases has the advantages of small tissue trauma, less surgical bleeding, short hospital stay, fast postoperative recovery, and less damage to the posterior structure of the lumbar spine. At the same time, it can achieve similar curative effects to traditional open surgery.

DOI: 10.20047/j.issn1673-7210.2022.33.27
Ankylosing spondylitis is a chronic progressive inflammatory disease with multi-system damage and extremely high disability rate. Traditional Chinese medicine has obvious advantages in the treatment of this disease. The clinical experience of Professor Yin Keqing in the treatment of ankylosing spondylitis with acupuncture and medicine is now introduced. It believes that ankylosing spondylitis is caused by deficiency of yang, invasion of cold and dampness, stasis of meridians and collaterals, and internal and external pathogens. The treatment is based on warming the kidney and strengthening the spine, and then nourishing the liver and kidney to soften the muscles and bones, and supplementing the middle jiao products. One is to nourish qi and blood, nourish the bones; the other is to relieve the dryness of medicinal properties, and cooperate with methods such as dissipating blood stasis, dehumidification and dredging collaterals. Exorcise evil without harming righteousness. The acupoints selected from the theory of bypassing the channels and collaterals are combined with the governor channel acupoints for the treatment of dredging the collaterals and relieving pain. The combination of acupuncture and medicine is comprehensive.

DOI: 10.20047/j.issn1673-7210.2022.33.28
Rapid eye movement sleep behavior disorder (RBD) is a disease characterized by the loss of muscle atonia and abnormal behaviors occurring during rapid eye movement (REM). It is mainly manifested as muscle atonia during REM and abnormal body movements associated with vivid dreams. Western medicine treatment mainly includes Clonazepam and Melatonin, but its therapeutic effect is limited. The understanding of this disease in traditional Chinese medicine is still unclear. Professor Chen Zhigang, combining clinical experience, believes that the disease is related to kidney, and the core pathogenesis should be explored from the brain’s marrow-spirit-qi system. Brain’s marrow is the basic structure, while brain’s qi is one of the substances to maintain brain functional activities and brain’s spirit is the external manifestation of brain function. Brain’s marrow injury cause brain’s qi deficiency, then mind disorders and nightmare happens. Based on the brain’s marrow-spirit-qi system, this paper discusses the pathogenesis of the disease in traditional Chinese medicine, and provides ideas and reference for the diagnosis and treatment of Chinese medicine.

134 Tracing the origin of the thought of “the spirit of great physician” and its philosophical interpretation
KANG Jian WU Yonggang ZHANG Yajun
DOI: 10.20047/j.issn1673-7210.2022.33.30
Sun Simiao’s thought of “the spirit of great physician” on medical ethics, which has rich connotation and long history. Clarifying the origin and explaining the philosophy behind it are important to promote the creative transformation and innovative development of the traditional medical ethics thought of “the spirit of great physician”. Actually, the combination of “great physician” and “sincerity” is a evolution of the ideological history of Chinese traditional medicine from “withcraft and medicine homologous”, then to “separate the witchcraft and medicine”, and finally “intergration the medicine and philosophy”. The interpretation of “great medicine” with “sincerity” is essentially the performance of medicine obtains metaphysical support then to moves towards rationalism. With the thought of “the spirit of great physician”, Sun Simiao made medicine from “the technology of a variety of handicrafts artisan” to a new height of “no matter which kind of skill, when it develops to the extreme, it will conform to the law of nature, which is the Tao”. The spirit of mutual interpretation and integration of medical and philosophy embodied in The Spirit of Great Physician is of great practical significance to the current medical ethics education.

DOI: 10.20047/j.issn1673-7210.2022.33.40
Clear cell renal cell carcinoma (ccRCC) is the most common pathological type of renal cell carcinoma (RCC), accounting for about 3%-5% of adult malignant tumors. RCC is mainly with vascular metastasis, which is common in organs or tissues such as lung, liver, bone, adrenal gland, and brain, but it is very rare that RCC metastasis to gallbladder. This report retrospectively analyzed the case data of a patient with gallbladder metastasis found eight years after radical resection of left ccRCC in the Department of Hepatobiliary Surgery of Guangzhou Red Cross Hospital Jinan University in January 2021. The patient was found to have gallbladder space occupying lesions and right lung nodules (no cancer cells were found in the nodule by out-of-hospital biopsy before) after admission. The lesion was pathologically confirmed as ccRCC gallbladder metastasis during subsequent laparoscopic radical gallbladder cancer surgery. However, after two months’ follow-up, a new nodule was found in the left lung, and was pathologically confirmed to be metastatic ccRCC, which was intervened by delivering Sunitinib. After 15 months’ follow-up, there was no recurrence of primary or other tumors, except for the progression of lung metastases. Preoperative diagnosis of ccRCC gallbladder metastasis based on clinical symptoms, signs and imaging data is challenging, which should be further proved by pathological examination. Therefore, the current report aimed to help better understanding the disease via reviewing the corresponding literature including imaging, pathological features, diagnosis, treatment, and prognosis.

Ventricular aneurysm (VA) is one of the common complications of acute myocardial infarction, which seriously affects the prognosis of patients. In addition to surgery, there is still a lack of targeted treatment of VA in clinic. This paper reports a case of VA patient treated by traditional Chinese medicine after acute myocardial infarction. The patient underwent emergency percutaneous coronary intervention due to “coronary heart disease  acute extensive anterior myocardial infarction”. Postoperative cardiac ultrasound indicated the formation of apical VA, accompanied by palpitation, upset, fatigue, dry mouth, and difficulty in sleeping and other symptoms. The author combined tongue syndrome differentiation as qi deficiency and blood stasis, phlegm and heat disturbing the heart. After the treatment of western medicine combined with benefiting qi for activating blood circulation, cooling blood and generating muscle, the patient’s symptoms improved and the VA disappeared. It is suggested that the treatment of VA by benefiting qi for activating blood circulation, cooling blood and generating muscle has a better effect. In this paper, the traditional Chinese medicine treatment to review the analysis, explore its theory, in order to broaden the treatment of this disease, and explore the advantages of traditional Chinese medicine treatment of this disease.

[Abstract] This paper reports a case of primary hyperparathyroidism (PHPT) with kidney calculi as the main symptom. This patient had a history of dry mouth polydipsia and polyuria for more than 20 years, which was not properly diagnosed and treated. In this case, the main symptoms of renal stone obstruction were low back pain, abdominal distension, nausea and vomiting, gross hematuria, etc. After lefe parathyroidectomy, PHPT was clearly diagnosed by pathology. After laser lithotripsy, drug therapy, and parathyroid adenoma resection, blood calcium, urine and renal function were recovered, small calculi were left in both kidneys, and parathyroid hormone (PTH) was still elevated after operation, and returned to normal two months after operation. Renal and parathyroid imaging, renal function, serum calcium, PTH, and urinary calcium levels were followed up. The diagnosis of PHPT includes qualitative and locational diagnosis, laboratory examination makes qualitative diagnosis, and imaging examination is used for localization and quantification. In terms of treatment, medical treatment can alleviate the hypercalcemia emergency, and surgical treatment is the first choice for the complete cure of PHPT, the surgical method was selected according to the location and number of lesions, intraoperative PTH monitoring is beneficial to improve the cure rate, if PTH and serum calcium are still elevated during postoperative follow-up, secondary hyperparathyroidism should be considered first, and the possibility of incomplete surgery or recurrence should be excluded.
