DOI: 10.20047/j.issn1673-7210.2022.31.02
Objective To investigate the diagnostic value of the CT features of pure ground-glass nodules (pGGN) in different pathological subtypes lung adenocarcinoma, and to provide imaging support for the management and treatment of pGGN. Methods The clinical data of 215 patients (236 nodules) with pGGN ≤2 cm in diameter and pathologically confirmed early lung adenocarcinoma at the First Medical Center of General Hospital of the Chinese People’s Liberation Army from January to December 2019 were analyzed. According to the pathological subtypes of pGGN, they were divided into preinvasive lesions-minimally invasive adenocarcinoma group (117 nodules), lepidic predominant adenocarcinoma group (77 nodules), and non-lepidic predominant adenocarcinoma group (42 nodules). The clinical features and CT features of the three groups were compared; receiver operating characteristic curves of diameter, CT value, and combined detection were drawn to predict the diagnostic efficacy of invasive predominant adenocarcinoma. Results There were significant differences in lobulation, spiculation, vascular changes, diameter, and CT value among three components (P < 0.05). Diameter and CT value were positively correlated with pathological subtype (r = 0.302, 0.352, P < 0.05). Lobulation, vascular changes, diameter, and CT value were the influencing factors of invasive pulmonary adenocarcinoma (OR > 1, P < 0.05). The area under the curve of invasive predominant adenocarcinoma diagnosed by the combined detection of diameter and CT value was greater than that detected by each index alone. Conclusion The CT features of pGGN are related to its pathological subtype to some extent. Attention should be paid to morphological changes such as lobulation, spiculation, and vascular changes.

DOI: 10.20047/j.issn1673-7210.2022.31.03
Objective To investigate the effect of bone marrow mesenchymal stem cells (BMSCs) on the senescence of chronic myeloid leukemia K562 cells and Raf/MEK/ERK signaling pathway. Methods BMSCs were isolated from the bone marrow of 30 C57BL/6J male mice with SPF grade of 6-8 weeks and body weight of 20-30 g. The expression of surface antigens CD29, CD244, and CD45 was detected by flow cytometry. After induction with adipogenic and osteogenic differentiation inducing fluid, the differentiation ability was identified by oil red O and alizarin red staining. BMSCs and K562 cells were cultured at the ratios of 1∶4, 1∶2, 1∶1, and 2∶1 for 24, 48, and 72 h, respectively. The co-culture time and cell ratio with the lowest proliferation rate of K562 cells were selected as the BMSCs group, and the conventional cultured K562 cells were used as the control group for subsequent experiments. SA-β-Gal staining was used to detect the senescence of cells in the two groups, and Western blot was used to detect the protein expressions of p16INK4a, p-Raf, p-MEK, and p-ERK in the two groups. Results The positive expression rates of BMSCs surface markers CD29, CD44, and CD45 were 89.18%, 87.65%, and 1.16%, respectively. BMSCs successfully differentiated into adipocytes and osteocytes in vitro. When the ratio of BMSCs to K562 cells was 1∶2, 1∶1, and 2∶1, the proliferation rates of K562 cells at each time point of co-culture were lower than those of the control group at the same time point, and the proliferation rates of K562 cells at each time point with the ratio of 2∶1 were lower than those of the control group at the same time point (P < 0.05). When the ratio of BMSCs to K562 cells was 2∶1, the proliferation rate of K562 cells after co-culture for 48 h was lower than those after co-culture for 24 and 72 h (P < 0.05). The positive rate of senescent cells and the protein expression of p16INK4a in BMSCs group were higher than those in control group, while the protein expression levels of p-Raf, P-MEK, and p-ERK were lower than those in control group (P < 0.01). Conclusion BMSCs can induce senescence in K562 cells, and the mechanism may be related to Raf/MEK/ERK signaling pathway.

DOI: 10.20047/j.issn1673-7210.2022.31.10
Ankylosing spondylitis is an autoimmune disease with a high clinical incidence. It is characterized by abnormal ossification and rigidity of the spine and sacroiliac joints, and with the further development of the disease, the patient’s quality of life is dramatically reduced. At present, the treatment of this disease is still a difficult problem in clinic. Considering the limitations of existing therapies (nonsteroidal anti-inflammatory drugs, glucocorticoids, antirheumatics, and biologic drugs), it is necessary to seek a treatment pathway with fewer side effects and higher potential. With the development of stem cell theory and technology, mesenchymal stem cells may be a new way to treat ankylosing spondylitis. This article reviews the basic and clinical research of mesenchymal stem cells in the treatment of ankylosing spondylitis in recent years, in order to provide new ideas for future research and application.

GU Wenshu YANG Wenchen WANG Ting LI Yinghao ZHONG Fengyu TANG Meiwen
DOI: 10.20047/j.issn1673-7210.2022.31.12
Chronic atrophic gastritis (CAG) is a common and prevalent disease in splenogastroenterology. It is mainly characterized by atrophy of the epithelium and glands of the gastric mucosa, and is accompanied by intestinal chemosis or heterogeneous hyperplasia. Compared with the low eradication rate and adverse effects such as abdominal pain and constipation brought about by allopathic treatment in Western medicine, traditional Chinese medicine treatment characterized around a holistic concept and discriminative treatment has demonstrated great advantages. The literature related to the treatment of CAG by traditional Chinese medicine in recent years has been compiled and reviewed in terms of traditional Chinese medicine disease name, etiology and pathogenesis, identification and classification, and traditional Chinese medicine treatment, etc. The treatment rules and prescriptions of CAG under the guidance of traditional Chinese medicine theory have been systematically explained, and traditional Chinese medicine characteristic therapies such as acupuncture, acupuncture point embedding, auricular pressure bean, traditional Chinese medicine conditioning, and body identification have been summarized. It is easy to find that traditional Chinese medicine has the unique advantages of simplicity, convenience, inexpensiveness, experimentation, and effectiveness, however, there are still shortcomings in the modern development of traditional Chinese medicine. It is now reviewed into a paper to provide reference for clinical treatment, with a view to promoting the further development of traditional Chinese medicine.

57 Research progress of autophagy in sepsis muscle atrophy
LIN Dawei WU Jin JIANG Peng▲
DOI: 10.20047/j.issn1673-7210.2022.31.13
Sepsis muscle atrophy is one of the serious complications of sepsis, which seriously affects the motor system and respiratory function. Studies have shown that sepsis can induce muscle atrophy through a variety of pathways, including muscle protein metabolism disorders, mitochondrial damage, inflammation, and so on. Autophagy is an important pathway of substance metabolism. Normally, autophagy protects cells by degrading harmful substances in cells. But the disease process can have different effects. At present, the advantages and disadvantages of autophagy on sepsis muscular atrophy are still controversial. Autophagy can promote muscle atrophy through metabolic pathways, and also inhibit muscle atrophy through mitochondrial autophagy and other pathways. This review aims to explore the effect of autophagy on sepsis muscle atrophy and provide new ideas for clinical treatment.

DOI: 10.20047/j.issn1673-7210.2022.31.26
Chronic lymphocytic leukemia is a type of chronic B cell lymphoproliferative disease, which is more common in the elderly. In recent years, the incidence of chronic lymphocytic leukemia is increasing, and many patients simply use western medicine chemotherapy, its effect is not ideal, and side effects happen a lot, high recurrence rate and mortality. Professor Tang Xudong is good at diagnosis and treatment of chronic lymphocytic leukemia from the perspective of integrated traditional Chinese and western medicine, the effect is remarkable, quite experienced. Professor Tang mostly treats chronic lymphocytic leukemia from the treatment idea of “attacking toxin and resolving hard mass, strengthening and consolidating body resistance”. In the early stage, “attacking toxin and resolving hard mass” is used as the method to break blood and remove blood stasis, disinfect and resolve hard mass, and remove pathogenic qi. In the middle stage, the method of “strengthening and consolidating body resistance” is used to consolidat body resistance and strengthen vital qi, help the pathogenic qi go out. In the later stage, “benefiting qi and nourishing yin” is the main method, benefiting qi and consolidating body resistance, nourishing yin, and promoting fluid production, and replenishing the vital qi of the body damaged by diseases. Professor Tang to “attacking toxin and resolving hard mass, strengthening and consolidating body resistance” for treating chronic lymphocytic leukemia, in the treatment of before, during, and after use of traditional Chinese medicine treatment, at the same time of removing pathogenic qi, take care of the vital qi of the injured human body and treat and use the medicine flexibly according to the clinical symptoms and related syndrome of the patients, which has achieved good results, which is worth learning and reference.

122 Summary of Professor Xiong Shaoquan’s experience in the treatment of refractory chemo-therapy-induced thrombocytopenia in patients with malignant tumors with “Sanwei Shenghuo Dan”
LUO Yingzhe LI Ru XIONG Shaoquan HE Lifang HUANG Xingmei WU Guoyu
DOI: 10.20047/j.issn1673-7210.2022.31.28
Due to repeated chemotherapy in patients, refractory chemo-therapy-induced thrombocytopenia is a side effect of chemotherapy, which manifests as irreversible bone marrow hematopoietic dysfunction. Routine use of platelet-promoting factors such as recombinant human thrombopoietin, interleukin-11 and etc. has been difficult to achieve ideal clinical efficacy. Professor Xiong Shaoquan believes that the internal binding of static blood and poison caused the deficiency of the spleen kidney, and other viscera. Repeated chemotherapy leads to damaging the kidney yin and yin deficiency involving yang which results to the decline of vital gate fire. Therefore, Professor Xiong Shaoquan proposes that the main pathogenesis of this disease is “decline of vital gate fire and the deficiency of both kidney yin and yang” and created the “Sanwei Shenghuo Dan” to treat the disease. “Sanwei Shenghuo Dan” is composed of Aconiti Lateralis Radix Praeparata, Polygonati Rhizoma, Jujubae Fructus. The three herbs are prepared in a ratio of 1∶1∶1. According to the specific situation, “Sanwei Shenghuo Dan” need to be used in addition or subtraction others herbs on the basis of this prescription. On the one hand, replenishing the vital gate fire to strengthen the kidney yang; on the other hand, strengthening zhongzhou to produce the middle qi. When the kidney fire is strong, qi and blood grow, and when the middle qi is sufficient, the qi and blood biochemistry are active and the platelets can be restored to normal. A case study is attached in order to provide new ideas and methods for the clinical prevention and treatment of refractory chemo-therapy-induced thrombocytopenia.

131 Professor He Xiaojin’s experience in treating postmenopausal osteoporosis with Xiange Prescription based on collateral disease theory
YANG Fan1 CAO Jing2
DOI: 10.20047/j.issn1673-7210.2022.31.30
Professor He Xiaojin makes good use of collateral disease theory to treat postmenopausal osteoporosis (PMOP), and developed and improved the concept of collateral disease with clinical guiding significance. According to the traditional Chinese medicine treatment principle of treatment based on syndrome differentiation, Professor He believes that the disease of PMOP is located in the bone collaterals, and the pathogenesis is characterized by “the spleen does not governing ascending clear, the kidney essence is deficient, and the blood stasis blocks the collaterals”. Taking “combining tonifying with dredging” as the treatment principle, “raising the spleen and tonifying the kidney, benefiting essence and filling bones, promoting blood circulation for removing obstruction in collaterals” as the treatment method, self-made Xiange Prescription is designed to treat PMOP and the clinical effect is quite good. This paper summarizes and analyzes Professor He’s experience in treating postmenopausal osteoporosis with Xiange Prescription based on collateral disease theory, so as to provide the same way and serve the patients.

Objective To explore the characteristics of acute lung injury (ALI)/acute respiratory distress syndrome (ARDS) programs registered in Chinese clinical trial registries. Methods Clinical trials of ALI/ARDS registered from the inception of the center to June 30, 2021 were retrieved and the information was counted. Results Ninety-six projects were screened, with the majority of major research units in Shandong and Shanghai (12.50% each), and 79.17% were single-center trials. The 92 projects with defined sample sizes were expected to recruit a total of 23 310 patients, with a sample size range of 9 to 5 000 patients. The types of studies were mostly observational studies (47.92%), interventional randomized controlled trials (42.71%), and registration information such as blinded use and randomization concealment still needs to be improved. 44 (45.83%) studies involved comorbidities. The interventions involved combined Chinese and western medicine treatment, western medicine treatment and non-pharmaceutical treatment, etc. The specimens collected were mainly blood and bronchoalveolar lavage fluid, and the observation indexes included clinically relevant indexes, physical and chemical examinations, and scoring scales. Conclusion There is a lack of registration information in some projects, and the quality of registration needs to be further improved; multi-center and large sample size clinical studies are still needed for a variety of comorbidities and diverse interventions in ALI/ARDS; outcome indicators are abundant and scattered, and attention can be paid to the development and application of core indicator sets to promote the generation of high-quality evidence-based evidence to help improve clinical diagnosis and treatment, and provide reference for future research directions in this field.

The single multi-hospital model of public hospitals may be the development direction of general hospitals. The use of artificial intelligence consulting system to collect patient needs can provide optimization strategies for hospital management. In this paper, the model of “one hospital, three districts” in Beijing Tongren Hospital, Capital Medical University is used as the research sample of a single public hospital with multiple districts, and the analysis showed that patients’ demands included four aspects: homogeneity, accessibility, recognition, and interoperability. Hospital managers can respond to patients’ homogenization expectations through smooth information, improve service accessibility through fine management, build brand consensus and increase hospital recognition, promote the construction of smart hospitals and promote inter-hospital communication to improve patients’ satisfaction.
