LIU Lingyun* MAO Han* HUANG Peilu CHEN Shule
DOI: 10.20047/j.issn1673-7210.2022.29.04
Objective To compare and analyze the effects of FoxM1 expression on the proliferation and invasion-related cell signaling pathways in intrahepatic cholangiocarcinoma (ICC), and to reveal the molecular mechanism of its occurrence and development. Methods The mRNA data of 36 patients with ICC were downloaded from the TCGA database and divided into the high expression group and the low expression group according to the median of the overall FoxM1 expression value. The gene set enrichment analysis (GSEA) software was used for biological analysis. Informatics analysis to find possible differentially expressed genes between the high and low expressions of FoxM1 and related signal pathways that affect them. Real-time PCR (RT-PCR) was used to verify and compare the expression relationship of FoxM1 and P53 mRNA in ICC cell lines. Results GSEA analysis showed that there were significant differences in the expression of common gene signaling pathways such as P53 and MYC signaling pathways in the high FoxM1 expression group, while there were significant differences in the expression of FOXO signaling pathways in the low FoxM1 expression group (all FDR-Q values < 0.250). Rt-PCR results showed that the expression of P53 mRNA in HCC-9810-FOXM1 cells up-regulated by lentivirus transfection was significantly higher than that in control cells. The mRNA level of P53 in SSP-25-shFOXM1 cells down-regulated by FoxM1 was significantly lower than that in control cells (P < 0.000 1). Conclusion FoxM1 may play a broad role in the development of ICC and other different stages of evolution. Expression of FoxM1 may be related to P53, and FoxM1 in ICC may affect the P53 signaling pathway.

DOI: 10.20047/j.issn1673-7210.2022.29.06
Objective To explore the value of geriatric nutritional risk index (GNRI) in evaluating the prognosis of elderly patients with heart failure with preserved ejection fraction (HFpEF), and to establish a nomogram model to predict the prognosis of elderly patients with HFpEF. Methods A total of 135 patients diagnosed with HFpEF in the First Affiliated Hospital of Soochow University from January 2018 to January 2020 were retrospectively included. The patients were divided into case group (44 cases) and control group (91 cases) according to whether major adverse cardiovascular events (MACE) occurred within a year. The clinical data of patients were collected, GNRI was calculated, and multivariate logistic regression was used to analyze the risk factors of poor prognosis in elderly patients with HFpEF. R software was used to construct a nomogram model to predict the occurrence of MACE in patients within a year. Results The age, the ratio of NYHA grade ≥ Ⅲ, the proportion of diabetes, creatinine, high-sensitivity troponin T, N-terminal brain natriuretic peptide precursor, and pulmonary artery pressure of the case group were higher than those of the control group, and the differences were statistically significant (P < 0.05); while the hemoglobin, albumin and GNRI were lower than those in the control group, and the differences were statistically significant (P < 0.05). Multivariate logistic regression analysis showed that the ratio of NYHA grade ≥ Ⅲ at admission (OR = 4.213, 95%CI: 1.476 to 12.022) and diabetes (OR = 3.282, 95%CI: 1.198 to 8.990) were the risk factors for the occurrence of MACE within a year in elderly patients with HFpEF (OR > 1, P < 0.05); while higher GNRI (OR = 0.919, 95%CI: 0.853 to 0.990) was a protective factor for the occurrence of MACE in elderly patients with HFpEF within a year (OR < 1, P < 0.05). The area under the ROC curve of the Nomogram model constructed based on GNRI was 0.816 (95%CI: 0.734 to 0.897, P < 0.01). Conclusion NYHA grade ≥ Ⅲ at admission and diabetes mellitus are independent risk factors for MACE within a year in elderly patients with HFpEF, and higher GNRI is a protective factor. The nomogram model constructed in this study has a good predictive value for the prognosis of elderly HfpEF patients within a year.

Research progress on the mechanism of xanthohumol, the active ingredient of hops, in the treatment of osteoporosis
ZHAO Yifan1 LIU Tianpeng1 ZHAO Shaochuan1 RUI Chen1 YANG Feng2
DOI: 10.20047/j.issn1673-7210.2022.29.07
Xanthohumol (XN) is a special flavonoid in hops, which can promote osteoblast differentiation and inhibit osteocbast (OB) differentiation, and regulate osteoclast (OC) metabolism. In this paper, literatures related to the involvement of xanthohumol in OB and OC differentiation and related signal pathways were analyzed by searching CNKI, VIP, Wanfang, PubMed and other databases in the last 10 years. The results showed that XN could up-regulate the expression of Runx2 and promote the proliferation and differentiation of OB. In addition, the expression of PPARγ was down-regulated, and the differentiation of adipocytes was inhibited, thus regulating the balance of osteogenic and adipogenic differentiation. Meanwhile, XN can also promote the production of antioxidants in vivo by up-regulating Nrf2 expression, thus reducing the damage of oxidative stress to OB. In addition, XN inhibited NF-κB, Ca2+/NFATc1, MAPK and other pathways, and inhibited OC differentiation and maturation. This review further clarified the mechanism of XN regulating bone metabolism, and provided a new target for the development of drugs for the prevention and treatment of osteoporosis.

DOI: 10.20047/j.issn1673-7210.2022.29.13
Objective To explore the cognition and influencing factors of general practitioners in Beijing urban areas on the current situation of medication safety for chronically ill patients with long-term medication prescriptions, and to put forward reasonable suggestions for formulating medication safety interventions for chronically-ill patients with long-term medication prescriptions in the future. Methods Using the objective sampling method, 16 general practitioners (numbered A to P) from mne community health service centers in Chaoyang District, Beijing were selected as the research subjects. Using the method of one-on-one in-depth interviews, 16 general practitioners were interviewed according to the interview outline from March to June 2021. The content analysis method was used to organize and analyze the data and summarize the interview topics. Results After collating and analyzing the interview data, three themes were extracted. ①There were 10 common medication errors in patients with chronic diseases, such as increasing or decreasing the dose of medication by themselves, repeated medication, wrong medication time and wrong medication method during taking medication. ②The influencing factors that lead to drug safety problems in patients were mainly four aspects including medical staff, patients, families and policy-related factors: doctors’ lack of health education to patients and untimely follow-up; patients’ lack of knowledge and attention to drug use knowledge and skills; timely; insufficient participation of family members; untimely supply of some long-term mediation prescription drugs leads to interruption of patients’ medication and inconsistent specifications and quantities of drugs prescribed by various pharmaceutical factory directors, which indirectly affects the safety of patients’ medication. ③Various measures should be taken to improve the safety of patients’ medication. The main measures include strengthening medication safety education for patients, regular follow-up visits, and encouraging family members to participate and support. Conclusion The drug safety problem of chronically ill long-term mediation prescription patients is prominent, and the collaborative role of the family doctor’s service team should be brought into full play, and patients and family members should be encouraged to participate in drug management. The policy guarantees the unification of the supply of some long-prescription drugs and the number of drugs in each box, improves patient compliance with medication, improves patient medication safety, and promotes the further implementation of the long-prescription policy.

DOI: 10.20047/j.issn1673-7210.2022.29.18
Objective To observe the psychological crisis state of patients with viral hepatitis after liver biopsy and to analyze its related risk factors. Methods A total of 107 patients with viral hepatitis who received liver biopsy in the First Affiliated Hospital of Nanjing Medical University from October 2018 to October 2021 were selected. All patients received investigation of psychological crisis vulnerability scale (PCVS) at three weeks after puncture. The patients’ baseline data were investigated, and the differences in PCVS scores of patients with different baseline characteristics were compared, and the scores of medical coping style questionnaire (MCMQ), psychological resilience scale (CD-RISC), and perceived social support scale (PSSS) were evaluated, and the influencing factors of psychological crisis state in patients with liver biopsy were further analyzed. Results A total of 107 questionnaires were distributed and 107 were recovered with a recovery rate of 100%. The PCVS score of 107 patients with liver biopsy was (74.24±12.81) points, and the PCVS score of patients with puncture complications was higher than that of patients without puncture complications (P < 0.05). PCVS score was positively correlated with patient surrender and avoidance (r > 0, P < 0.05), and negatively correlated with face, psychological resilience, and social support (r < 0, P < 0.05). The results of multiple linear regression analysis showed that puncture complications (X1), avoidance (X3), face (X4), resilience (X5), and social support level (X6) were the influencing factors of psychological crisis in patients with liver puncture biopsy, and the equation was as follows: Y=130.664+10.572X1+0.815X3-1.475X4-0.531X5-0.185X6 (F = 147.812, R2 = 0.899, adjusted R2 = 0.893, P < 0.05). Conclusion The psychological crisis of patients with liver biopsy may be affected by puncture complications, coping modes, psychological resilience and social support.

HUANG Zhaowei1 Ling Yi2 Tan Linlin3 HUANG Fangqin1 WANG Lu1 MA Wukai2
DOI: 10.20047/j.issn1673-7210.2022.29.23
Objective To study the risk factors of ovarian cancer risk in middle-aged and elderly women with rheumatoid arthritis (RA). Methods A total of 180 middle-aged and elderly (age ≥40) female RA patients (RA group) admitted to the Second Affiliated Hospital of Guizhou University of Traditional Chinese Medicine from October 2019 to December 2020 and 80 healthy middle-aged and elderly females (healthy group) were randomly selected. The general conditions of subjects and ovarian cancer risk-related indicators of RA patients were recorded. The absolute value of T lymphocyte subsets of RA patients were detected. The levels of CA125 in the two groups were compared. The RA group was divided into 31 cases of CA125 positive group and 149 cases of CA125 negative group according to whether CA125 was abnormal or not. The correlation between the elevation of CA125 in RA patients and human epididymal protein 4 (HE4) and T cell subsets was analyzed. Results The CA125 level in the RA group was higher than that in the healthy group (P < 0.05). The levels of HE4 and CD8+ T cells in CA125-positive patients were significantly higher than those in CA125-negative patients (P < 0.05). Logistic regression analysis showed that HE4 (OR = 1.117, 95%CI:1.052-1.185) and CD8+ T cells (OR = 1.499, 95%CI:1.213-1.851) were independent risk factors for CA125 positivity in RA patients (P < 0.05). Conclusion Middle-aged and elderly female RA patients are prone to CA125 positivity. CA125 positivity is closely related to the levels of HE4 and CD8+ T cells, which may indicate an increased risk of ovarian cancer in CA125-positive RA patients. Clinically, the risk of ovarian cancer in this population should be fully paid attention to and timely investigation should be carried out.

Analysis of Professor Cui Yinglin’s theory of treating apoplexy by stage based on “new and long phlegm”
PANG Botong1 NAN Songhua1 CHEN Fangfang1 GE Wenjing1 CUI Yinglin2
DOI: 10.20047/j.issn1673-7210.2022.29.25
The second National famous traditional Chinese medicine Professor Cui Yinglin believes that phlegm pathogen is an important factor in the formation, change and prognosis of apoplexy. However, phlegm pathogen itself also changes with the course of apoplexy. Therefore, he proposed the academic idea of “treating by stages based on the new and long phlegm”. He believes that the treatment of apoplexy should start from the new and long phlegm, and according to the specific course of the disease and the different concurrent disease, stage treatment. At the same time, he also original Xingnao Emena Decoction, Kangyi Capsules and so on, applied in clinical, curative effect is very good. The author has the honor to be the successor of Professor Cui’s academic thought. In the course of learning from teachers, the author sorted out Professor Cui’s academic thought of “treating by stages based on the new and long phlegm” and its academic origins. The etiology, pathogenesis and characteristics of “phlegm-induced apoplexy” were summarized. The clinical medication experience of “phlegm-induced apoplexy” was summarized to provide theoretical basis for the treatment of apoplexy with traditional Chinese medicine.

Analysis of the theoretical basis of Yishen Jiedu Tongluo Method in the treatment of multiple sclerosis based on image thinking
DING Wenjing WANG Zhongmin REN Jiayan WANG Xiang SHANG Xiaoling
DOI: 10.20047/j.issn1673-7210.2022.29.26
Traditional Chinese medicine image thinking is the core thinking method to understand traditional Chinese medicine theory and guide clinical application. Using image thinking to distinguish and treat multiple sclerosis (MS) is the development and continuation of traditional Chinese medicine clinical diagnosis and treatment, inheritance and innovation. This paper analyzes the pathogenesis and clinical treatment of MS based on image thinking, and fully interprets the application of image thinking in the syndrome differentiation and treatment of MS, including “taking images abstracting from viewing”, “comparing images with categories” , and “taking images to understand the meaning”. Using the image thinking to summarize the symptoms and functional characteristics of MS, it is proposed that the clinical symptom of MS in traditional Chinese medicine is “flaccidity”. Combined with the original thinking of traditional Chinese medicine, it is concluded that “poison” is the key pathogenesis of MS. To guide the clinical treatment of MS with “flaccidity” and “poison”, and to use the method of “benefiting kidney, removing toxicity substance, and dredging collateral” is the extension and expansion of syndrome differentiation and diagnosis, which is of great significance to guide clinical practice and promote theoretical research and innovation.

Xie Haizhou’s academic thought of “mental disorders” in treating child autism
WANG Bo ZHANG Huadong
DOI: 10.20047/j.issn1673-7210.2022.29.32
Child autism is a neurological developmental disorder that falls under the category of mental disorders. Professor Xie Haizhou holds that mental disorders are caused by deficiency of kidney essence, deficiency of blood and qi, absence of nourishment, obstruction of phlegm and blood stasis, and obscuring the clearance of consciousness, which have important reference value for the treatment of autism in children. Treatment of the brain is the key to the treatment of autism in children, special treatment measures such as invigorating the kidney, nourishing the qi, nourishing the blood, eliminating phlegm, and removing blood stasis can be applied according to the etiology and pathogenesis of autism. For children with sudden self-mutilation, mania, convulsions, syncope, and other emergencies, priority should be given to the application of drugs to alleviate the emergencies, quickly calm the children’s mind and stabilize their mood, first standard, then both the original and the specimen.

With the progress of imaging technology, the phenomenon of lumbar disc herniation and reabsorption has been paid attention to by peers in the industry. Its clinical presentation is also different. At present, two different clinical manifestations of intervertebral discs at different lumbar levels in the same patient are rarely reported. The patient presented to the clinic because of recurrent low back pain and cold sensation in the left lower extremity. Imaging data showed that the L3-4 huge prolapse occurred significant reabsorption phenomenon, and the L4-5 protrusion calcified. After diagnosis of lumbar disc herniation, the patient was treated with Chinese medicine Spinal Ⅱ Prescription orally, and the symptoms were significantly improved. Through the analysis of this case, this paper reveals the influence of the shape of herniated intervertebral disc and its surrounding environment on the reabsorption, deepen the understanding of the diversity and complexity of the phenomenon of intervertebral disc reabsorption, and explore the effective treatment plan of traditional Chinese medicine conservative treatment for the possible reabsorption phenomenon in lumbar intervertebral disc herniation.

Investigate the research status of sicca syndrome based on National Natural Science Foundation of China
GUO Zilin1 TANG Xiaopo1▲ ZHOU Xinyao1 XU Haodong1 LI Yanting1 ZHANG Yi2
DOI: 10.20047/j.issn1673-7210.2022.29.44
Objective To analyze the research status and improvement direction based on the funding of sicca syndrome (SS) project by National Natural Science Foundation of China (NSFC). Methods From January 2000 to December 2021, all funded projects with SS in their titles, abstracts or keywords were collected from the NSFC website, Mace Medical, LetPub, and Home for researchers. Information such as project type, funding amount, supporting unit, responsible person, discipline code, keywords, and achievement transformation were extracted and analyzed by descriptive statistical method. Results From 2000 to 2021, NSFC funded 88 SS projects, including general projects, youth science funds projects, regional science funds projects, and emergency management projects. In 2003, the SS field received NSFC funding for the first time. Since then, although the number and funding of the projects had fluctuated, the overall trend was on the rise. From 2010 to 2019, the number, amount, and achievement transformation of SS funded projects were lower than those of rheumatoid arthritis and systemic lupus erythematosus. Among the 11 supporting institutions that had approved projects more than three times, Peking University was in the leading position in terms of the number of approved projects and funding. The research direction of SS project was wide, and the angle of entry was diverse. The keywords was repetition rate low, and the high-frequency terms were cell, cytokine, and pathway. Conclusion NSFC funding is of great significance to SS, but the current clinical demand is large, and the advantages of traditional Chinese medicine are not fully reflected. In the future, it should pay attention to innovation and multidisciplinary intersection, implement the guideline consensus and big data platform as soon as possible, and lead the development of SS clinical research.

Progress and future prospects of diagnosis-related groups in China during “the 13th Five-Year Plan” to deepen the reform of the medical system
TANG Jiaji
DOI: 10.20047/j.issn1673-7210.2022.29.45
During the “13th Five-Year Plan” period of medical reform, Chinese diagnosis-related groups (DRG) payment system reform achieved remarkable pilot results, and eight mainstream diagnosis-related groups (DRG) versions appeared successively. As a master, China healthcare security diagnosis related groups has initially formed a DRG payment system with Chinese characteristics. In the future, it needs to consolidate the coding system to help improve the grouping scheme, and strengthen the design of supporting system and the construction of evaluation system. DIP payment will coordinate with DRG to promote the reform and expansion of payment methods. Public hospitals should effectively develop and utilize DRG tools, strengthen cost awareness, and make concerted efforts in medical insurance management, performance management, medical management, nursing management, etc., so as to stimulate clinical endogenous dynamics and form a DRG-based three-dimensional hospital management system supported by information technology.
