Exploration of a new hybrid design sequence in drug interchangeability
LING Jia1 LIU Shengjie2 YAN Fangrong2▲
Objective To propose a new hybrid design sequence (TTRR, RRTT, RTTR, TRRT) to explore its application in drug interchangeability. Methods Based on criterion for drug interchangeability (SCDI) scaled by Chow, etc. as evaluation criteria, the application of the new sequence in drug interchangeability was explored by considering the variation among drug individuals, and was compared with the common sequence (TRTR, RTRT). Results Through computer simulation of different situations, it was found that the new sequence presented a consistent trend. In addition, the new sequence presented in this study was more sensitive than the common sequence when the key parts σWR, σD of drug interchangeability were changed. Conclusion Although the new sequences are more sensitive to variation than the common sequence in drug interchangeability, the additional two sequences make it less effective in sample size.

Advancements of microRNA in diabetic nephropathy
TIAN Shasha1 YANG Xiaopeng1 GUO Hui1,2
Diabetic nephropathy (DN) is one of the main complications of diabetic microangiopathy and has been the leading cause of chronic kidney disease in China. It′s important to understand the pathogenesis and find new diagnostic and therapeutic targets of DN. MicroRNA (miRNA), as a ubiquitous non-coding small molecular RNA, is widely involved in gene post-transcriptional and epigenetic regulations. The expression or abnormal function play an important role in the occurrence and development of DN, mediating the pathophysiological changes such as the pathophysiological changes of renal tissue fibrosis, inflammation, podocyte apoptosis in DN. Recently, the value of miRNA in the early diagnosis and prognosis judgment of DN has been confirmed and it has been gradually used in clinical treatment as a therapeutic target for DN by inhibiting or reconstructing miRNA. However, there are great challenges of miRNA in the pathogenesis and clinical application for DN in the future.

LI Jinze PENG Lei WEI Tangqiang XIA Zhongyou LI Yunxiang
Objective To systematically review the efficacy and safety of retrograde intrarenal surgery (RIRS) versus percutaneous nephrolithotomy (PCNL) in kidney stones of 2-3 cm. Methods PubMed, EMBASE, Science, WanFang, VIP and CNKI databases were electronically searched to identify all eligible comparative studies from inception of the database to October 2019. Then meta-analysis was performed by using RevMan 5.3 software. Results A total of 10 studies involving 1597 patients were included. Among them, there were 778 patients in the RIRS group and 819 patients in the PCNL group. The results of meta-analysis showed that: compared with PCNL, RIRS had lower stone-free rate (OR = 0.47, 95%CI [0.34, 0.66], P < 0.000 01), shorter hospital stay (MD = -2.45, 95%CI[-2.96, -1.94], P < 0.000 01), less decrease in haemoglobin (MD = -6.43, 95%CI[-9.05, -3.82], P < 0.000 01), and lower complication rate (OR = 0.69, 95%CI [0.51, 0.93], P = 0.02), but no difference was found in operation time between the two groups (MD = 6.27, 95%CI[-3.51, 16.05], P = 0.21). Conclusion Compared with PCNL, RIRS has a lower SFR, but it has advantages in terms of hospital stay, decrease in haemoglobin, and complication rate. Therefore, for patients with kidney stones of 2-3 cm, RIRS is a safe and effective alternative to PCNL.

Finite element analysis of proximal femoral nail anti-rotation with different length in the treatment of type A3.3 femoral intertrochanteric fracture in elderly
CHEN Xinmin1 JIANG Teng1 YANG Wei1 ZHENG Liqin2 LIN Ziling2 TU Shijie1
Objective To investigate the optimal length of proximal femoral nail anti-rotation for the treatment of type A3.3 femoral intertrochanteric fracture in elderly by using finite element method. Methods Based on one patient volunteer′s hip CT data in Fengcheng Hospital of Traditional Chinese Medicine, Jiangxi Province imported into Mimics 19.0 and Geomagic 2017 softwares and obtained proximal femur model after processed. Then the model was imported into Solidworks 2017 software to simulate A3.3 type fracture and assembled with different length of proximal femoral nail anti-rotation getting model A (short nail), B (middle nail), and C (long nail) respectively. The models were imported into Abaqus 2016 software one by one in step format to set material parameters and boundary conditions, etc. and then the calculation was carried out. The Von Mises cloud pictures were checked and the whole femur stress, internal fixation stress, fracture gap size and inversion angle of head-neck bone fragment were observed. Results The overall stress of femur in model B was the minimum, that in model C was the maximum, and that in model A was the center. Proximal femoral nail anti-rotation was the main supporting structure of head and neck bone fracture, and beared most of the stress. The stress peak was located at the connection between spiral blade and main nail, and the secondary peak was located at the entry point of external femoral wall. The fracture gap size and inversion angle of head-neck bone fragment were inversely proportional to the length of the main nail, model A had the smallest fracture gap size and inversion angle of head-neck bone fragment, model C had the largest, and model B had the middle. Conclusion The application of the short length proximal femoral nail anti-rotation in the treatment of type A3.3 femoral intertrochanteric fracture in the elderly has a smaller fracture gap size, a more stable fracture end and a better biomechanical effect.

Experience of Zang Fuke proved recipe Xiaojiye Decoction in the treatment of synovitis of knee osteoarthritis
GUO Hongyu WANG Bin YUAN Yi FAN Xiran LI Duoduo ZANG Fuke
This paper summarizes the experience of professor Zang Fuke on the treatment of synovitis of knee osteoarthritis, comprehensive the four diagnosis of traditional Chinese medicine and pathological anatomy of western medicine, emphasize on the combination of overall syndrome differentiation and local disease differentiation combined with each other in dialectics. The overall concept of syndrome differentiation and treatment is to protect the spleen and stomach and prevent recurrence, and local differentiation of disease is reflected in the principle of three stages of bone injury treatment, that is, “promoting blood circulation for removing blood stasis for the first-stage, regulating the motion of Yingqi for the mid-stage, strengthening the function of kidney and bone for the last-stage”. In the early stage, traditional Chinese medicine orthopedic diseases must injure qi and blood, damage bones and muscles, and involve liver and kidney more in the middle and late stage. Therefore, spleen, liver and kidney deficiency and wet stasis mutual junction are established as the core pathogenesis of this disease. Through the clinical diagnosis and treatment approach of “making the etiology and disease name consistent, identifying the diseased nature and position, making it disease correlated with syndrome”, the diagnosis and treatment of the disease and syndrome are divided into two types of clinical staging: “qi stagnation, blood stasis, and water accumulation type” (swelling phase) and “yang deficiency type of spleen and kidney” (non-swelling phase). In the swelling stage, the method of “soothing the liver, activating the blood and benefiting the water” should be adopted; in the remission stage, the method of “strengthening the spleen and benefiting the kidney” should be adopted; the prescription of “Xiaojiye Decoction” should be adopted to add or decrease the dosage and apply, both samples should be taken into consideration, and the curative effect is confirmed.

Thinking and discussion on the treatment of anxiety disorder with the theory of “Piwei Lun”
ZHAO Chunyi1 XIAO Rong1 ZHENG Zhuanfang1 ZHENG Liming2 LI Yan2
By combining the theory of “Piwei Lun” with clinical practices, this paper analyzes the etiology and pathogenesis of anxiety disorder, and points out that the main pathogenesis of such disorder lies in “spleen and stomach damage with imbalanced rising and falling of vital energy”. According to Professor Li Yan′s clinical experience in treating anxiety disorder for many years in Guangdong Provincial Hospital of Traditional Chinese Medicine, this paper summarizes different syndrome characteristics of anxiety disorder patients as followed: “deficiency heat in spleen and stomach”, “spleen deficiency and flaring-up of liver-fire”, “stagnation of liver qi and spleen deficiency”, “spleen deficiency and body-fluid consumption”, and “spleen and stomach disharmony”. In terms of treatment, effective prescriptions, such as Buzhong Yiqi Decoction, Bupiwei Xieyinhuo Shengyang Decoction, Shengyang Sanhuo Decoction, Huangqi Renshen Decoction and Modified Pingwei Powder are respectively selected based on the principle of “regulating spleen and stomach, benefiting primordial qi, purging yin fire and harmonizing qi and blood”.

Professor Li Yanning believes that “intermingled phlegm and blood stasis” is the key to the pathogenesis of children′s chronic persistent of asthma, and the physiological relationship of “homogeny of clear fluid and blood” must be accompanied by the pathological relationship of “intermingled phlegm and blood stasis”. “Phlegm” is the “root” of the onset of asthma and throughout the whole process of asthma. The chronic persistent of asthma is mainly intangible phlegm, which should be treated by “clearing phlegm”. However, “homogeny of clear fluid and blood”, “chronic disease and blood stasis”, phlegm turbid for a long time, blocking the vein and blocked can cause “blood stasis”. “Blood stasis” is an important pathological factor of chronic persistent of asthma. From the perspective of homology of phlegm and blood stasis, the author summarizes Professor Li Yanning′s experience in the treatment of children′s chronic persistent of asthma, discusses its unique etiology, pathogenesis, syndrome differentiation and treatment, enriches the theoretical basis of asthma treatment, and provides new ideas for clinical diagnosis and treatment.

The ancient and modern research of improving sleep by Zhuyou therapy
SUN Qiaosi HU Jingqing
“Zhuyou” originated from Neijing, which is to say the cause of illness to patients, or cooperate with the technique of incantation and forbidding evil, so as to eliminate the anxiety and depression of patients, so as to treat diseases related to emotion or caused by obvious mental factors. On the basis of consulting the papers and medical records, the origins and derived meaning of “Zhuyou” are analyzed. The principle of treating diseases is emotion-restricting, empathize, eliminating pathogen and strengthening vital qi, suggestion and solving puzzles, making use of other methods to treat diseases and pretending the name of Zhuyou. Zhuyou can improve sleeping quality through adjusting the state of mind, tranquilizing the mind, clearing up the nightmare and hypnotize the patients. So rational use of Zhuyou method can improve sleep quality effectively and it is worth further study.

TIAN Feng1 PAN Hong2▲
Objective To investigate the effect of implanting catgut in abdominal acupoint combined with Heye Jiangzhi Decoction on glucose and lipid metabolism in obese type 2 diabetes mellitus patients. Methods A total of 155 obese type 2 diabetes mellitus patients admitted to Qianjiang Central Hospital of Chongqing City from February 2016 to March 2018 were prospectively selected as the research objects. The patients were divided into control group (77 cases) and observation group (78 cases) according to the random number table method. The control group was treated with Heye Jiangzhi Decoction, and the observation group was added with implanting catgut in abdominal acupoint on the basis of the control group. The clinical effect after treatment and the level of fasting blood glucose (FBG) at different time were observed. The levels of blood lipid indexes (total cholesterol [TC], triacylglycerol [TG], low density lipoprotein-cholesterol [LDL-C], high density lipoprotein-cholesterol [HDL-C]), nesfatin-1 and apelin were recorded before and after treatment. Obesity was recorded before and after treatment. Results After treatment, the total effective rate in the observation group was higher than that in the control group, and the difference was statistically significant (P < 0.05). After treatment, the levels of obesity, TC, TG, LDL-C, nesfatin-1 and apelin in the two groups decreased, while the level of HDL-C increased, and the differences were statistically significant (P < 0.05). After treatment, the levels of obesity, TC, TG, LDL-C, nesfatin-1 and apelin in the observation group were lower than those in the control group, and the HDL-C was higher than that in the control group, and the differences were statistically significant (P < 0.05). The FBG level of the two groups showed a decreasing trend with time, and the FBG level at 30 and 60 d after treatment of the observation group were lower than those of the control group respectively, and the differences were statistically significant (P < 0.05). Conclusion Implanting catgut in abdominal acupoint combined with Heye Jiangzhi Decoction is effective in the treatment of obese type 2 diabetes mellitus, which can significantly reduce blood glucose and lipid levels, and the mechanism may be related to the regulation of nesfatin-1 and apelin.

A case of bronchial lipoma treated by bronchoscope high frequency electrical snare combined with cryotherapy
WANG Baoming1 LI Zhihao2 XING Xiqian3,4▲
Tracheobronchial lipoma is a rare disease, mostly seen in middle-aged and elderly men. Its etiology has not been determined yet, and its clinical symptoms are not specific, so it is easy to be misdiagnosed. A patient with broncholipoma who was admitted to the Department of Respiratory and Critical Care Medicine, Yan′an Hospital, Kunming City, Yunnan Province in 2016 was selected. Describe the diagnosis and treatment process and summarize the experience. The patient, female, 46 years old, was admitted for “cough and sputum for 3 weeks”. After admission, she was examined by high-resolution chest CT and bronchoscopy, and the neoplasm in the bronchial lumen was removed by high frequency electrical entrapment under the tracheoscope combined with cryotherapy, and the neoplasm was removed to reveal lipoma with saliary gland hyperplasia. Postoperative symptoms of cough and sputum were alleviated. Chest CT reexamination showed that the right middle bronchial lumen was unobtrusive and the right middle and lower lungs exudated and absorbed. Through case review and literature review, the epidemiology, clinical symptoms, imaging characteristics, and treatment selection of tracheobronchial lipoma were reviewed. Improve the level of diagnosis and treatment of tracheobronchial lipoma, reduce missed diagnosis and misdiagnosis.

The translator′s subjectivity research of Neonatal and Pediatric Respiratory Therapy and its enlightenment to medical translation team
CHEN Chen1 CHEN Lin2▲ GUO Zikai1 LIU Boyuan1 LIU Manlin1▲
Since the 1980′s, translation subjectivity has remained a heated topic, resulting in the deep exploration into translator′s subjectivity. Translation is undertaken by translators whose principal role should be well recognized. This paper based on the study of the translation process of Neonatal and Pediatric Respiratory Therapy, gives a systematic analysis of translators′ initiative to ensure translation quality throughout the translation process. In the meantime, the author is inspired by Lefevere′s “rewriting theory” to recognize that the publication of a medical book is closely related to its translation quality, which requires the medical translation team to cater to the sponsor′s request. The medical translation team should not only meet the requirements of medical book translation project, but also follow the codes and norms of the target language.
