Abstract Objective To understand the status of adolescent reproductive health, the status of unwanted pregnancy and its influencing factors, so as to provide the basis for sex education and clinical diagnosis and treatment of adolescent female clinic. Methods From June 2017 to June 2019, a total of 500 adolescent patients were randomly selected from Kunming Maternal and Child Health Hospital to conduct a questionnaire survey. The survey included two parts: the status of reproductive health and the knowledge of family planning. According to whether they had accidental pregnancy experience or not, 500 cases were divided into accidental pregnancy experience group (106 cases) and no accidental pregnancy experience group (394 cases). The related factors of unexpected pregnancy were analyzed. Results The age of menarche, the age of beginning sexual life, the experience of menstrual disorder, the experience of taking emergency contraceptives, the need to take contraceptive measures and the correct use of condoms were related to the occurrence of unexpected pregnancy, and the differences were statistically significant (P < 0.05). There was no significant relationship between the age of breast development, reproductive tract infection, the risk of induced abortion and the occurrence of unexpected pregnancy (P > 0.05). Experience of taking emergency contraceptives, understanding the need to take contraceptive measures when having sex and understanding the correct use of condom were the protective factors of accidental pregnancy (OR = 0.581, 0.459, 0.511), while menstrual disorder was the risk factor of unexpected pregnancy (OR = 1.903). Conclusion According to the development characteristics and reproductive health status of adolescent women, the knowledge of reproductive health and contraception should be publicized to reduce the occurrence of unwanted pregnancy.
