The protective effects of amniotic mesenchymal stem cells for rats early myocardial injury induced by severe burns

Objective To explore the effects and related mechanism of amniotic mesenchymal stem cells (AMSCs) for rats myocardial injury induced by severe burns. Methods The AMSCs were obtained by the method of collagenase-pancreatic enzyme digestion. Biological markers were assayed with flow cytometry. The activity of AMSCs was identified using adipogenic and osteoplastic differentiation. 24 healthy male SD rats were randomly divided into sham group, burn group and AMSCs group. The dorsal skin of rats in burn group and AMSCs group were exposed to 98℃ water for 15 s which led to the third degree, 30% TBSA burns, while the rats in the sham group were exposed to 37℃ water for 15 s. Rats in burn group and AMSCs group were given 0.9% 10 mL (50 mL/kg) saline intraperitoneally immediately after burns. PBS or AMSCs were injected through caudal vein 3 hours later. 48 hours later, rats were sacrificed and the myocardium and blood from aorta abdominalis were collected. The levels of creatine kinase (CK), lactic dehydrogenase (LDH) were detected by ELISA. The mRNA levels of caspase-3, TNF-α, IL-1β and IL-10 were detected by RT-PCR. The data were analyzed through AVONA and LSD-t test by SPSS. Results The positive rates of CD29, CD44, CD90, CDl05 were very high in the third generation of AMSCs, however, the positive rate of CD34 was low. AMSCs underwent osteogenic and adipogenic differentiation after induction. The levels of CK and LDH in rats of the burn group were significantly increased compared with the sham group (P < 0.05). The levels of CK and LDH in AMSCs group were significantly lower than those in the burn group (P < 0.05). The mRNA levels of caspase-3, IL-1β and TNF-α in the myocardium of the burn group were significantly increased compared with the sham group (P < 0.05). The mRNA levels of caspase-3, IL-1β and TNF-α in myocardium of the AMSCs group were significantly lower than those in the burn group (P < 0.05). The mRNA level of IL-10 in the burn group was significantly lower than that in the sham group (P < 0.05). In the AMSCs group, the expression of IL-10 mRNA was significantly higher than that in the burn group (P < 0.05). Conclusion AMSCs can protect against myocardial injury induced by severe burns. The levels of CK and LDH decreased, indicating that the injury of myocardial decreased. During which, the inflammatory factors, IL-1β and TNF-α mRNA decreased and anti-inflammatory factor IL-10 mRNA increased.

Study on the content changes of 5-hydroxymethyl furfural in the processing of "Jiao Sanxian"

Objective To investigate the change rule of 5-hydroxymethyl furfural (5-HMF) in the processing of "Jiao Sanxian". Methods The samples of "Jiao Sanxian" were fired with different processing time and the contents of 5-HMF in each sample were detected by high performance liquid chromatography (HPLC). The HPLC analysis was performed on an Agilent C18, column (250 mm×4.6 mm, 5 μm) and the mobile phase consisted acetonitrile-water (8∶92), the detection wavelength was set at 283 nm. Results The linear range of 5-HMF was in the range of 0.2-64.0 μg/mL (r = 0.9997), average recovery was 99.74%, and RSD was 1.37%. The contents of 5-HMF were increased remarkably at the 8 min and 6 min for Fructus Crataegi and Fructus Hordei Germinatus, respectively, while it showed a steady pattern for medicated leaven. Conclusion "Jiao Sanxian" has a Maillard reaction in the processing, which produces common element 5-HMF. The contents of 5-HMF in Fructus Crataegi and Fructus Hordei Germinatus are increased obviously with the extension of processing time, while it is not obvious for medicated leaven.

Study on standard of Guanxin Keli

Objective To establish the quality standard of Guanxin Keli. Methods TLC method was used for the qualitative identification of Salvia miltiorrhiza Bge., Panax notoginseng (Burk) F.H.Chen; the method validation was conducted by using microbial limit test in Chinese Pharmacopoeia 2015. 2 batches of Guanxin granules were tested under the validated method. HPLC was used to determine the content of salvianolic acid B. The determination was performed on KromasilC18 (250 mm×4.6 mm, 5 μm) column with mobile phase consisted of methanol-acetonitrile-1% formic acid water (29∶9∶62) at the flow rate of 1.0 mL/min. The detection wavelength was set at 286 nm. Results TLC spots were clear and well-separated without negative interference. The recoveries of each validation strain for the total aerobic micribial count, total yeasts and mold count were among 50%-200%, and did not test E. coli. This validation of method could test E. coli, and was capable. The linear range of salvianolic acid B was 0.06-0.54 mg/mL (r = 0.9998) with an average recovery of 98.61 %, RSD = 1.24% (n = 6); the content of two batchs samples were satisfactory. Conclusion The method is simple, accurate and reproducible. It is effective in controlling the quality of Guanxin Keli and providing the basis for improving the quality standas of Guanxin Keli.

Study on purification technology of syringin in Cortex Ilicis Rotundae by D-900 resin

Objective To research the method for purification technology of syringin in Cortex Ilicis Rotundae by D-900 resin. Methods The purification conditions of the resin were investigated by high performance liquid chromatography and ultraviolet spectrophotometry. The retention rate of syringin and the decolorization rate of the solution were investigated. Results The purification parameters of D-900 resin was as following: resin weight and sample volume ratio of 1∶5, resin column diameter and high ratio of 1∶5, adsorption velocity of 2 BV/h. The retention rate of syringin was above 80% and the decolorization rate of the solution was above 30% under these conditions. Conclusion The purification technology of syringin is simple and feasible.

Effect of Lyciumbarbarum polysaccharide on expression of inflammatory cytokines in keratinocytes after UVB irradiation

Objective To study the effects of the Qinghai Tsaidam Basin Lyciumbarbarum polysaccharide(LBP) on the level of interleukin1 (IL-1) and interleukin6 (IL-6) in cultured human immortalized keratinocytes (HaCaT cells) after ultraviolet (UVB) irradiation. Methods HaCaT cells were cultured in vitro and randomly divided into five groups: control group, UVB radiation group (30 mJ/cm2, radiation 40 min), LBP low dose group (0.5 mg/mL, 30 mJ/cm2,radiation 40 min) , LBP middle dose group (1 mg/mL, 30 mJ/cm2, radiation 40 min) , LBP high dose group(2 mg/mL, 30 mJ/cm2, radiation 40 min). Before 12 h of UVB irradiation, LBP was added into the culture medium. Enzyme-linked immunosorbent assay (ELISA) was applied to test the effects of LBP on groups of cells and the supernatant of the level of IL-1 and IL-6. Results Compared with the control group, the levels of IL-1 in the cell in the UVB radiation group had no significant change (P > 0.05), the level of IL-6 in the cell was significantly higher (P < 0.05). The levels of IL-1 and IL-6 in supernatant were significantly increased (P < 0.05). Compared with UVB radiation group, LBP group of cells or in the supernatant IL-1 and IL-6 levels were significantly reduced (P < 0.05). Conclusion Qinghai Tsaidam Basin LBP can reduce synthesis and secretion of inflammatory factors in HaCaT cells after UVB radiation factor, and reduce the skin damage caused after ultraviolet radiation.

Effects of different dose of Propofol on hypothalamus, pituitary, hippocampus Akt and amyloid β protein of rats

Objective To observe the effects of different Propofol anesthesia doses on rat hypothalamus, pituitary, hippocampus Akt and amyloid beta protein (Aβ). Methods 24 SPF grade male rats were used by method of low-dose tail intravenous injection (10 mg/mL) for 3 times. The rats were divided into 4 groups by methods of cumulative dose, control, Propofol group Ⅰ (10 mg/mL), Propofol group Ⅱ (20 mg/mL) and Propofol group Ⅲ (30 mg/mL), respectively. No anaesthetic management was given in control group, with 6 rates in each group. Electroencephalogram was detected by electroencephalograph; the pathology of rat hypothalamic arcuate nucleus neurons and pituitary gonadotropin cell was observed by HE staining. Western blot detection was used to detect the expression of the Akt and Aβ. Results Compared with the control group, the spike wave, α wave and β wave increased significantly with the increase of the Propofol dose (P < 0.05), and the δ wave decreased significantly with the Propofol dose increasing (P < 0.05). The density and distribution of hypothalamic arcuate nucleus were normal in the control group, with the large whole number of adenohypophysis cells, abundant sinusoid capillary between cells, and the cells were arranged regularly; no obvious lesions of rats arcuate nucleus neuron in Propofol group Ⅰ was observed, as well as no obvious reduction in adenohypophysis cells. With increasing of dose of anesthesia, the number of hypothalamic neuron cells and adenohypophysis cells reduced significantly, with unclear boundary of nuclear membrane and an increase in the number of irregular arrangement cells. The Western blot results suggested that with increasing of the Propofol dose, the Akt hippocampus protein expression reduced dramatically (P < 0.01) while Aβ expression increased significantly (P < 0.01) in anesthesia group comparing with control. Conclusion Low-dose Propofol anesthesia can not cause lesions in hypothalamus and hypophysis, but their feedback effects enhanced with Propofol dose increasing, causing pathological changes. The deposition effects of Propofol anesthesia on Aβ may be achieved by inhibiting Akt pathway.

Study on dynamic changes of total flavonoids content of Radix Puerariae in different harvest times

Objective To study the dynamic change of total flavonoids content of Radix Puerariae in the same area and different harvest times, and determine the best harvest period of Radix Puerariae. Methods UV-Vis spectrophotometry was used, the test sample was extracted with 95% ethanol for 20 min, the absorbance was measured at 250 nm, and the total flavonoid content was calculated. Results The content of total flavonoids in Radix Puerariae harvested in different months was 3.79%-11.86%, the highest from September to October, the lowest from April to May. Conclusion As to the total flavonoids as the evaluation index, the best harvest period of Radix Puerariae should be September to October, the results can provide a reliable scientific basis for determining root harvest period.

Optimization of extraction process of alkaloids from Lingnan Paotianxiong by response surface method

Objective To optimize extraction process of total alkaloids from Lingnan featured pieces Paotianxiong by response surface method. Methods Three-variables as extraction time, ratio of material to solution and ethanol concentration, and three-levels Box-Benhnken experimental design was used to optimize the effects of independent variables on the total alkaloid extraction efficiency. Results The optimum conditions were extraction time 30 min, ratio of material to solution 8.0, and 75% ethanol concentration. Under the optimum conditions, the theoretical total alkaloid extraction rate was 31.1%, and the actual rate was 32.5%. Conclusion Response surface method is an effective methodology to optimize the total alkaloid extraction process of Lingnan featured pieces Paotianxiong.

Comparison on the protective effects of dibenzocyclooctadiene lignans contents of Fructus Schisandrae Chinensis for the injured hepatocytes

Objective To evaluate the protective effects of deoxyschizandrin, schisandrin B, schisandrol A, schisandrol B, schisandrin C and schisantherin A on injured hepatocytes, respectively. Methods The H2O2 was used to induce model of injured HL-7702. In the deoxyschizandrin groups, the cells were randomly divided into normal group, model group and 1, 5, 25 μmol/L of deoxyschizandrin dosing group. The influence of deoxyschizandrin for the proliferation (A value) of injured HL-7702 was detected by cell counting kit-8, meanwhile, the influence of deoxyschizandrin for the aspartate aminotransferase (AST) released by the injured hepatocytes was detected by automatic biochemistry analyzer. The other groups of schisandrin B, schisandrol A, schisandrol B, schisandrin C and schisantherin A had the same operation with deoxyschizandrin. Results Compared with model groups, there were no significant differences of A values and AST concentration in the three concentration groups of deoxyschizandrin and schisandrol A (P > 0.05), while the A values in 1, 5 μmol/L groups of schisandrol B, schisandrin B, schisandrin C and 25 μmol/L group of schisantherin A were increased significantly (P < 0.05), the AST concentrations in which were decreased significantly (P < 0.05). Conclusion Deoxyschizandrin and schisandrol A without any methylenedioxy have no protective effects for the injured hepatocytes, while schisandrol B, schisandrin B, schisandrin C and schisantherin A with the methylenedioxy have certain protective effects for the injured hepatocytes.

Inhibitory effect of artemisinin on proliferation and invasion of gastric cancer cells

Objective To study the effect of artemisinin on the proliferation and invasion of gastric cancer cells. Methods Human gastric cancer cell line SGC-7901 was cultured and dealt with different doses of artemisinin in serum-free DMEM, respectively as 75, 150, 300 μmol/L artemisinin group, serum-free DMEM without artemisinin as control group. After 48 h of intervention, cell proliferation activity, invasion numbers and mRNA expression of proliferation gene, invasion gene were determined. Results After 48 h of intervention, cell proliferation activity, invasion numbers and Ki-67, CyclinD1, CDK4, CDK6, MMP2, MMP9 mRNA expression of 75, 150, 300 μmol/L artemisinin group were significantly lower than those of control group, TIMP1 and TIMP2 mRNA expression were significantly higher than those of control group (P < 0.05), and the higher the dose of artemisinin, the lower the cell proliferation activity, invasion numbers and Ki-67, CyclinD1, CDK4, CDK6, MMP2, MMP9 mRNA expression, the higher the TIMP1 and TIMP2 mRNA expression (P < 0.05). Conclusion Artemisinin can inhibit the proliferation and invasion of gastric cancer cells by inhibiting the expression of cell cycle proteins and matrix metalloproteinases.

Research progress of analysis of volatile organic compounds in biological fluids

Analysis of volatile organic compounds in exhaled breath, blood, sweat, urine, saliva, pleural effusion, cerebrospinal fluid, stool is a new method that was used for the assessment of health status, disease screening and diagnosis. In recent years, this method has the advantages of non-invasive, rapid, convenient operation, cheap and patients with good tolerance. The analysis of volatile organic compounds has attracted more and more attention of researchers and expanded a new research field. At present, the metabolomics research of lung cancer in exhaled breath, blood and urine show that alkanes, aromatic hydrocarbons, oxy-compound has a certain diagnostic value. There is also a link between identified various types of cancer and some special volatile organic compounds.

Research progress on stemless shoulder arthroplasty

With the development of new materials and human body engineering mechanics, shoulder arthroplasty has gradually become the core of the shoulder joint surgery. Stemless shoulder arthroplasty is the forefront progress of the shoulder joint arthroplasty, it is better than standard primary anatomic total shoulder arthroplasty and shoulder resurfacing arthroplasty. Surgical operation of stemless shoulder arthroplasty is more simple, postoperative effect of stemless shoulder arthroplasty is better, stemless shoulder arthroplasty has played an important role in the development of shoulder surgery. This paper summarizes the historical evolution, the types of prosthesis, indications and contraindications, the related research results and the advantages and disadvantages of the stemless shoulder arthroplasty, and aims to provide the basis for guiding the clinical operation program.

Clinical effect of transcutaneous electrical stimulation treatment on common peroneal nerve injury with EMG instrument

Objective To evaluate the clinical effect and safety of transcutaneous electrical stimulation treatment (TEST) on common peroneal nerve (CPN) injury with EMG instrument. Methods From January 2015 to December 2016, in the Second Affiliated Hospital, Xinjiang Medical University, 74 patients diagnosed CPN injury were collected and divided into three groups by random number table, control group (23 cases, received normal therapy without TEST by EMG instrument), short-term group (24 cases, received TEST by EMG instrument for 2 weeks) and long-term group (21 cases, received TEST by EMG instrument for 1 month). The indexes of clinical effect and the degrees of movement and perception improvement, the indexes of nerve conduction velocity (NCV), compound muscle action potential (CMAP) amplitude of extensor brevis digitorum and motor evoked potentials latent period (MEPLP), were compared among three groups at pre-therapy and 3-month post-therapy. Follow-up 1 month, the occurrences of adverse reaction were compared. Results The total effective rates of three groups were long-term group > short-term group > control group (P < 0.05). The excellent rates of three groups were long-term group > short-term group > control group (P < 0.05). The NCV and CMAP of three groups at 3-month post-therapy were higher than at pre-therapy (P < 0.05), and long-term group > short-term group > control group (P < 0.05). The MEPLP of three groups at 3-month post-therapy were lower than at pre-therapy (P < 0.05), and long-term group < short-term group < control group (P < 0.05). During treatment and follow-up 1 month, there were no serious adverse effect. Conclusion The TEST with EMG instrument can efficiently improve the clinical effect of CPN injury and the clinical effect of long-term TEST is better and the safe.

Efficacy and safety of Atorvastatin in patients with acute cerebral infarction before intravenous thrombolysis

Objective To observe the clinical efficacy and adverse reactions of Atorvastatin pretreatment before intravenous thrombolysis in patients with acute cerebral infarction. Methods From January 2013 to February 2016, in the Department of Neurology, Fourth Affiliated Hospital of Guangxi Medical University, 100 patients with primary acute cerebral infarction were collected, they were divided into two groups by random number table, study group (n = 50) and control group (n = 50). Patients in study group were given Atorvastatin (40 mg, qd) oral or nasal feeding before intravenous thrombolysis. Patients in control group were given Atorvastatin (40 mg, qd) after 7 d intravenous thrombolysis treatment. All patients were given rt-PA intravenous thrombolysis according to Recommendations of China Acute Ischemic Stroke Diagnosis and Treatment Guide 2014. The rate of 7 d neurological function improvement, 7 d neurological deterioration, 90 d good functional recovery, symptomatic intracranial hemorrhageand mortality in the two groups were observed. Results The rate of 7 d neurologic improvement in study group was 56%, that in the control group was 36%, two groups were compared, the differences were statistically significant (P < 0.05). The rate of 90 d good functional recovery in study group was 38%, that in control group was 24%, two groups were compared, the differences were statistically significant (P < 0.05). The rate of 7 d neurologic deterioration was 16%, that in control group was 34%, two groups were compared, the differences were statistically significant (P < 0.05). In study group, the rate of symptomatic intracerebral hemorrhage and 3-month mortality were 6% and 4%, those in the control group were 4% and 2%, two groups were compared, the differences were not statistically significant (P > 0.05). Conclusion Atorvastatin pretreatment before intravenous thrombolysis in patients with acute cerebral infarction could improve the short-term efficacy. Moderate dose of statin pretreatmentdoes not increase the incidence of symptomatic intracranial hemorrhage and mortality in patients undergoing intravenous thrombolysis.

Safe temperature of moderate hypothermic circulatory arrest in Stanford type A aortic dissection

Objective To explore the safe temperature range of the moderate hypothermic circulatory arrest surgery in Stanford type A aortic dissection. Methods From January 2015 to October 2016, in Tongji Hospital, 40 Stanford type A aortic dissection patients were divided into two groups by random number table, low temperature group (nasopharyngeal temperature 22-<24℃) and high temperature group (nasopharyngeal temperature 24-26℃), with 20 cases in each group, all patients were given surgery under moderate hypothermic circulatory arrest and Unilateral paraminic brain irrigation. The intraoperative and postoperative situation in the two groups was observed. Results The CPB time in the low temperature group was significantly more than that in the high temperature group (P < 0.01), and there was no statistically significant difference in the aortic clamping time and the arrest time between the two groups (P > 0.05). There was no significant difference in TND and PND between the two groups (P > 0.05). The ICU time in the low temperature group was significantly more than that in the high temperature group (P < 0.05). There was no significant difference in the hospitalization time and inpatient mortality between the two groups (P > 0.05). Conclusion 24-26℃ is a more safe nasopharyngeal temperature range in Stanford type A aortic dissection during moderate hypothermic circulatory arrest.

Comparison of hemodynamics of elderly patients during general anesthesia induction with two kinds of Etomidate target controlled infusion

Objective To compare the effect of general anesthesia induction with different kinds of Etomidate target controlled infusion on hemodynamic of elderly patients. Methods 40 cases of elderly patients underwent selective general anesthesia surgery were enrolled in this study from April to June 2015 in the Seventh People's Hospital of Hebei Province. All patients were randomly divided into two groups, each group had 20 cases. Group P was given plasma concentration target controlled infusion, and group E was given effect-site target controlled infusion. The mean arterial pressure (MAP), heart rate (HR), cardiac output (CO), stroke volum (SV), bispectral index (BIS) and the were recorded and compared immediately at T0 (before induction), T1 (loss of eyelash reflex), T2 (etomidate plasma and effect-site concentration reached a balance), T3 (intubation), T4 (1 min after intubation ), T5 (2 min after intubation). The time when eyelash reflex disappered and intubation completed, and dosages of etomidate were recorded. Results Compared with T0, at T2-5 the levels of MAP, CO, SV, BIS were dereased in the two group, with statistically significant differences (P < 0.05). At T1-3, there were statistically significant differences in MAP, CO, SV between the two groups (P < 0.05), but the levels of HR and BIS had no statistically significant differences (P > 0.05). Compared with group P, the time of eyelash reflex disappearing and intubation was shorter in group E (P < 0.05). Conclusion Adopting plasma concentration target controlled infusion for elderly patients during general anesthesia induction can help maintain stable hemodynamics.

Objective To compare the effect of of different target concentration of Propofol on pharmacodynamics and pharmacokinetics of Mivacurium Chloride during perianesthesia of patients underwent microlaryngoscopic surgery. Methods 60 cases of patients with ASA Ⅰ-Ⅱ underwent selective microlaryngoscopic surgery from June 2015 to September 2016 in 463th Hospital of PLA were selected, and divided into group Ⅰ, Ⅱ, Ⅲ by random number table, each gorup had 20 cases. The target effect-sit concentration of Propofol was 3.0, 4.0, 5.0 μg/mL respectively. After target controlled infusion of Propofol for 60 s, intravenous injection of Fentanyl 10 μg/kg, Mivacurium Chloride 0.2 mg/kg was given, and endotracheal intubation rapidly. Anesthesia was maintained with Remifentanil Hydrochloride 0.25 μg/(kg·min). Ulnar nerve train of 4 stimulation (TOF) was monitored continuously by using acceleration muscle relaxation monitor. Muscle relaxant indicators were recorded. Hemodynamics and BIS were monitored during the whole anesthesia. The pharmacokinetic effects of Mivacurium were measured. Results Comparison of the hemodynamic parameters in the three groups had no significant differences (P > 0.05). The onset time of Mivacurium in the three groups had no statistically significant difference (P > 0.05). 25%, 75%, 90% T1 recovery time of TOF and 70% T4/T1 recovery time in the group Ⅲ were significantly prolonged than those of the group Ⅰ, with statistically significant differences (P < 0.05). In the pharmacokinetic of Mivacurium, in addition to relative bioavailability (F), the rest of the indicators, compared with the groupⅠ, Ⅱ and group Ⅲ, the differences were statistically significant (P < 0.01). Conclusion The target effect-sit Propofol concentration and pharmacokinetics of Mivacurium has a dose-dependent correlation. Mivacurium pharmacodyn-amics is unsignificantly effected by target effect-sit propofol concentration of 4.0 μg/mL.

The application value of high-frequency ultrasound-guided interscalene brachial plexus nerve block anesthesia

Objective To evaluate the clinical application value of ultrasound-guided interscalene brachial plexus nerve block anesthesia. Methods 112 cases of patients underwent interscalene brachial plexus nerve block anesthesia in Baoji Hospital Affiliated to Xi'an Medical College from October 2015 to October 2016 were selected, and divided into group A, B, C, according to different anesthesia location methods. 46 cases in group A were given traditional anatomical localization; 30 cases in group B were given nerve stimulator assisted localization; 36 cases in group C were given high-frequency ultrasound-guided localization. The indicators of anesthesia completion time, block beginning time, analgesic maintain time, dosages of narcotic analgesic drugs and anesthetic effect of the three groups were compared. Results Anesthesia completion time of group A was significantly shorter than that of group B and group C (P < 0.05), while there was no significant difference between group B and group C (P > 0.05). Block beginning time of group C was significantly shorter than that of group A and group B (P < 0.05), while there was no significant difference between group A and group B (P > 0.05). Analgesic maintain time of group C was significantly longer than that of group A and group B (P < 0.05), while there was no significant difference between group A and group B (P > 0.05). Dosages of narcotic analgesic drugs of group C were significantly less than those of group A and group B (P < 0.05), while there were no significant differences between group A and group B (P > 0.05). Anesthetic effect score of group C was significantly higher than that of group A and group B (P < 0.05), while there was no significant difference between group A and group B (P > 0.05). Conclusion Ultrasound-guided interscalene brachial plexus nerve block anesthesia has shorter block beginning time, longer analgesic maintain time, less narcotic analgesic drug dosages and better anesthetic effect. It is worthy of clinical application.

Efficacy of different doses of Dexmedetomidine combined with Sufentanil for patient-controlled intravenous analgesia after spinal surgery

Objective To evaluate the efficacy of different doses of Dexmedetomidine (Dex) combined with Sufentanil for patient-controlled intravenous analgesia after spinal surgery. Methods 120 cases of patients with ASA Ⅰ-Ⅲ grade underwent elective spinal surgery with general anesthesia in Luzhou Traditional Chinese Medicine Hospital from January 2015 to December 2016 were selected, and randomly divided into group A, group B and group C, each gorup had 40 cases. The postoperative intravenous analgesia formula: Sufentanil 0.05 μg/(kg·h) in group A, Sufentanil 0.03 μg/ (kg·h) + Dex 0.05 μg/(kg·h) in group B, and Sufentanil 0.03 μg/(kg·h) + Dex 0.08 μg/(kg·h) in group C, and all added to normal saline to 100 mL, loading dose of 3 mL, background dose of 2 mL/h, PCA dose of 1 mL and locking time of 15 min. The VAS and Ramsay scores at the time points of 2, 4, 6, 12, 24 h after surgery, PCIA pressing times, the dosages of Sufentanil and complications such as nausea and vomiting, hypotension, bradycardia, respiratory depression in the three groups were recorded. Results The VAS and Ramsay scores at different time points in the three groups were tatistically significant differences (P < 0.01). Compared with group A, the VAS scores at the time points of 4, 6, 12, 24 h after surgery in group B and group C were decreased (P < 0.01), and the VAS scores of group C were lower than those of group B (P < 0.05). Compared with group A, the Ramsay scores at the time points of 4, 6, 12, 24 h in group B and group C were increased (P < 0.05), and the Ramsay scores of group C were higher than those of group B (P < 0.05). The PCA pressing times and the dosage of Sufentanil of group B and group C were less than those of group A (P < 0.01), which of group C were less than those of group B(P < 0.01). The incidence of nausea and vomiting of group A was significantly higher than that of group B and group C (P < 0.05). Conclusion Dex combined with Sufentanil for patient-controlled intravenous analgesia after spinal surgery has good effects, and Dex can enhance the analgesic effect and reduce dosage and adverse reaction of opioids.

Clinical effect of Baihe Zhimu Dihuang Decoction combined with Suanzaoren Decoction in the treatment of hyperthyroidism with insomnia

Objective To observe the clinical effect of Baihe Zhimu Dihuang Decoction combined with Suanzaoren Decoction in the treatment of the hyperthyroidism with insomnia. Methods 56 cases of hyperthyroidism with insomnia admitted to Outpatient and Inpatient Department of Endocrinology in Hainan Hospital of Traditional Chinese Medicine from August 2014 to July 2015 were selected as the observation objects, and they were divided into control group and treatment group according to the random number table method, with 28 cases in each group. The control group was treated with the Estazolam Tablets, the treatment group was given Baihe Zhimu Dihuang Decoction combined with Suanzaoren Decoction. 10 days was as one course, total for 2 courses. The effect of the two groups was observed and compared. Results The total effective rate of treatment group was 85.7%, which was higher than that of control group (57.1%), the difference was statistically significant (P < 0.05). The scores of self-rating anxiety scale, self-rating depression scale, Pittsburgh sleep quality index in treatment group after treatment was lower than those before treatment and those of control group after treatment, there were statistically significant differences (P < 0.05). Conclusion The effect of Baihe Zhimu Dihuang Decoction combined with Suanzaoren Decoction in the treatment of hyperthyroidism insomnia is significant, which is worthy of clinical promotion.

Clinical research of Duhuo Jisheng Decoction combined with lumbar manipulation in the treatment of lumbar disc herniation

Objective To observe the clinical effect and safety of Duhuo Jisheng Decoction combined with lumbar manipulation in the treatment of lumbar disc herniation (LDH). Methods 90 cases of patients with LDH were enrolled, who came from Beijing Changping Hospital of Integrated Chinese and Western Medicine from March 2015 to June 2016. All the patients were randomly divided into experimental group (45 cases) and control group (45 cases) by random number table. The experimental group was treated by Duhuo Jisheng Decoction combined with lumbar manipulation, the control group was treated by Diclofenac Sodium Enteric-coated Tablets and lumbar traction. The evaluation indicators were Japanese orthopaedic association (JOA) backache total scores, visual analogue scale (VAS) of lumbocrural pain, hepatorenal function, adverse events or adverse reactions. Results After treatment for two weeks, the JOA backache total scores in the two groups were all higher than those before treatment, the VAS scores of lumbocrural pain in the two groups were all lower than those before treatment, the differences were all highly statistically significant (P < 0.01). After treatment, the JOA backache total scores in the experimental group were significantly higher than those of control group (P < 0.01), the VAS scores of lumbocrural pain in the experimental group were significantly lower than those of control group (P < 0.05). During the study, the safety was good in both groups. Conclusion Duhuo Jisheng Decoction combined with lumbar manipulation in the treatment of LDH can improve the clinical symptoms and signs of patients, the short-term curative effect is reliable, and the safety is good.

Effects of acupuncture on cancer-related fatigue in breast cancer patients at the rehabilitation stage: a randomized controlled trial

Objective To observe the effects of acupuncture on cancer-related fatigue in breast cancer patients at the rehabilitation stage. Methods 72 breast cancer patients at the rehabilitation stage who were suffering from at least moderate fatigue were recruited from Beijing Hospital of Traditional Chinese Medicine, Capital Medical University and Guang'anmen Hospital, China Academy of Chinese Medical Sciences from January 2013 to June 2014. They were randomly divided into intervention group and control group, each 36 cases. In the intervention group, participants received traditional Chinese acupuncture on Baihui (GV 20), Neiguan (PC 6), Qihai (CV 6), Zusanli (ST 36), Sanyinjiao (SP 6). Shallow acupunctures were applied in the control group. Treatment was given twice a week for 4 weeks, and follow-up was given for 4 weeks at the end of treatment. The primary index was cancer-related fatigue, which was evaluated by Piper fatigue scale, the secondary indexes were sleep, emotion, physical functioning, which were respectively evaluated by Pittsburgh sleep quality index (PSQI), hospital anxiety and depression scale (HADS), Eastern cooperative oncology group (ECOG) performance status. Adverse events and side effects were compared between two groups. Results No significant differences were found in general fatigue or in four dimensions between the two groups after treatment and during follow-up period (P > 0.05). The general fatigue, feel dimension and cognitive/emotional dimension in the acupuncture group after treatment and during follow-up period had significant differences compared with those before treatment (P < 0.01 or P < 0.05). No significant differences were found in the scores of PSQI, HADS and ECOG scores between the two groups (P > 0.05). After treatment, the depression scores in both groups were all lower than those before treatment, the differences were statistically significant (P < 0.05). Conclusion Our study is underpowered to find a statistically significant difference in general fatigue in breast cancer patients at the rehabilitation stage. The improvement in feel dimension, cognitive/emotional dimension and depression scores may be associated with the placebo effects and the mechanisms that cancer-related fatigue is closely related to psychological factors.

Clinical observation of spread moxibustion in the treatment of 70 cases with diabetic peripheral neuropathy

Objective To investigate the effectiveness and safety of spread moxibustion in the treatment of diabetic peripheral neuropathy, to provide reference for clinical treatment. Methods 140 patients with diabetic peripheral neuropathy admitted to Qinhuangdao Hospital of Traditional Chinese Medicine from January 2014 to December 2016 were selected as the subjects, according to random number table, all the patients were divided into two groups, with 70 cases in each group. Both groups were treated with the conventional therapy, the control group was treated with Mecobalamin, based on the treatment of control group, the observation group was treated with spread moxibustion. After 2 courses of treatment, the clinical efficacy, symptom score, nerve conduction and adverse reaction rate were compared between the two groups. Results After 2 courses of treatment, the total effective rate of the observation group was 90.00%, which was significantly higher than that in the control group (71.43%), the difference was statistically significant (P < 0.05); the improvement of cold foot, numbness of lower limb, lower limb pain and sense like ants climbing in the observation group were significantly better than those in the control group, the differences were statistically significant (P < 0.05); the symptom scores in the observation group were significantly lower than those in the control group, the difference was statistically significant (P < 0.05); the nerve conduction velocity of the observation group was significantly higher than that of the control group, the difference was statistically significant (P < 0.05); there was no significant difference in the incidence of adverse reactions between the two groups (P > 0.05). Conclusion Compared with conventional western medicine treatment, moxibustion therapy can significantly improve the clinical curative effect of diabetic peripheral neuropathy, which can significantly improve the clinical symptoms and nerve conduction velocity, with no obvious adverse reactions, it is worthy of clinical attention.

Objective To analyze the current problems of Chinese medicinal material resources in Handan City and put forward solutions, so as to protect Chinese medicinal material resources in Handan City. Methods 3S modern technology was used to investigate the distribution of wild Chinese medicinal material resources and reserves of unit area; the method of interview investigation combined with field survey was used to record the varieties, acre yield and other information of planted Chinese medicinal material. Results Through investigation, it was found that there were 1300 kinds of wild Chinese medicinal materials and 25 kinds of planted breeds in Handan City currently. There were some problems such as the unreasonable resource distribution, imbalance breed distribution, diminishing wild natural preservation zone of wild Chinese medicinal material in Handan City, which influences the Chinese medicinal material resources in Handan City; there were some problems such as uneven distribution of planting base, single cultivated variety in planted Chinese medicinal material. Conclusion This paper makes clear the resource distribution and reserves of wild and planted Chinese medicinal materials in Handan city, masters the present problems, and put forward reasonable suggestions.

Objective To explore the clinical effect of Chubi Zhitong Decoction treated in cold-dampness blockage type of lubar intervertebral disc protrusion decoction and quality of sleep. Methods 80 cases with cold-dampness blockage type of lubar intervertebral disc protrusion decoction treated in Hospital of Traditional Chinese Medicine of Ju County from July 2014 to July 2016 were selected. All the patients were divided into the control group (40 cases) and observation group (40 cases), according to random number table. The control group was taken Diclofenac Sodium Sustained Release Tablets, the observation group was taken Chubi Zhitong Decoction. The urative effect, VAS and JOA score and the sleep quality of the two groups were compared. Results The total effective rate of observation group was higher than that of the control group, with statistically significant difference (P < 0.05). The VAS scores of the two groups after treatment were decreased, and the JOA scores were increased, and the improvement of observation group after treatment was more remarkable than that of the control group after treatment, with statistically significant difference (P < 0.05). The sleep disorder, sleep time, sleep efficiency, daytime function and sleeping drug scores of the two groups after treatment were decreased, and the improvement of observation group after treatment was more remarkable than that of the control group after treatment, with statistically significant difference (P < 0.05). Conclusion The appllication of Chubi Zhitong Decoction on cold-dampness blockage type of lubar intervertebral disc protrusion decoction can receive remarkable effect, relieve pain, and can improve sleep quality.

Clinical study of Huamoyan Decoction in the treatment of gonarthromeningitis with syndrome of dampness-heat blocking collaterals

Objective To observe the clinical effect of Huamoyan Decoction in the treatment of gonarthromeningitis with syndrome of dampness-heat blocking collaterals. Methods 96 cases diagnosed as gonarthromeningitis met with the syndrome of dampness-heat blocking collaterals admitted to Baoji hospital of traditional Chinese medicine from March 2012 to July 2015 were selected as research objects. All the patients were randomly divided into observation group (48 cases) and control group (48 cases) by random number table. The control group was taken microwave physical therapy, 30 min each time, once a day. The observation group was taken Huamoyan Decoction, one dose each day, external application of Chinese medicine was given at the same time. Both groups were taken 7 days as a course, treating for 4 courses. The improvement of joint swelling degree and TCM syndrome effect of the two groups after treatment were observed. The pain degree of knee-joint and joint function of the two groups before and after treatment were compared. Results After treatment for 4 weeks, the joint swelling degree of observation group was obviously milder than that of control group (P < 0.05), the TCM syndrome effect of observation group was obviously better than that of control group(P < 0.05). The scores of pain and joint function of WOMAC scale after treatment in the two groups were lower than those before treatment, and scores of pain and joint function of WOMAC scale after treatment in the observation group were decreased more significantly compared with those of control group, with statistically significant difference (P < 0.05). Conclusion Huamoyan Decoction can obviously promote the absorption of hydrops articuli in patients with gonarthromeningitis, reduce the swelling and pain of joint, improve the joint function, which is worthy of clinical promotion and application.

Efficacy and safety of Shenqi Fuzheng Injection combined with transcatheter arterial chemoembolization for primary liver cancer:a Meta analysis

Objective To systematically evaluate the efficacy and safety of Shenqi Fuzheng Injection combined with transcatheter arterial chemoembolization (TACE) in the treatment of primary liver cancer. Methods The method of systematic review from the Cochrane library was adopted and clinical controlled trials were retrieved by computer in the Cochrane library, Medline, Embase, PubMed, Wanfang database, Chinese science and technology journal full-text database retrieval (CNKI), China journal full-text database (VIP), etc. "Shenqi Fuzheng Injection", "liver cancer" and "intervention" were used as the keywords to find clinical randomized controlled trials of Shenqi Fuzheng Injection combined with TACE in the treatment of primary liver cancer which were open published from the establishment of database to December 2016. The inclusion and exclusion criteria were used to select the literature by two researchers, the full texts were sought, through quality assessment of literatures and data extraction, finally, the literatures were analyzed statistically by Review Manger 5.2 software eventually. Results There were 7 RCT studies and a total of 466 patients were included. Meta-analysis results showed that compared with single TACE in the treatment of primary hepatocellular carcinoma, the efficiency (RR = 1.45, 95%CI: 1.01-2.07, P = 0.04), the quality of life (RR = 2.29, 95%CI: 1.65-3.17, P < 0.01), CD4+/CD8+ (MD = 0.28, 95%CI: 0.09-0.47, P = 0.003) of Shenqi Fuzheng Injection combined with TACE in the treatment of primary hepatocellular carcinoma were significantly improved, the decrease of white blood cells (RR = 0.68, 95%CI: 0.56-0.83, P = 0.0001) and the occurrence rate of gastrointestinal tract reaction (RR = 0.69, 95%CI: 0.51-0.95, P = 0.02) of which were also lower than TACE. Conclusion Shenqi Fuzheng Injection can improve the efficacy and safety of TACE in the treatment of primary liver cancer.

Assessment of right atrial function in patients with pulmonary hypertension in WHO functional class Ⅰ using real-time three-dimensional echocardiography

Objective To assess right atrial function in patients with pulmonary hypertension (PH) in WHO functional classⅠ (WHO FCⅠ) using real-time three-dimensional echocardiography(RT-3DE). Methods From February 2014 to June 2016, in Affiliated Shenzhen Bao'an Hospital of Southern Medical University, 41 patients with PH in WHO FCⅠ were chosen as PH group, and 32 healthy volunteers were chosen as normal control group. RT-3DE full volume images of right atrium at apical four chamber view were collected. Right atrium volume during different cardiac cycle were acquired, and were corrected to body surface area, then right atrium maximal volume index (RAVImax),right atrium minimal volume index (RAVImin) and right atrium pre-systolic volume index (RAVIpre) were derived from QLAB 9.0 (10.5) workstation. The following right atrium total emptying volume index (RAVIt = RAVImax-RAVImin), right atrium active emptying volume index (RAVIa = RAVIpre-RAVImin), right atrium passive emptying volume index (RAVIp = RAVImax-RAVIpre), right atrium passive emptying fraction (RAEFp = RAVIp/RAVIt×100%), right atrium active emptying fraction (RAEFa = RAVIa/RAVIt×100%) were calculated, respectively. And tricuspid annular plane systolic excursion (TAPSE) during systole were measured by M-mode echocardiography. Right ventricular ejected fraction (RVEF) was derived from 2DE Simpson. The tricuspid annulus velocity (e,a) of right ventricular free wall during diastole were recorded by tissue Doppler imaging, and e/a was calculated. Results There were no significance of RVEF, TAPSE between two groups, RAVImax,RAVImin, RAVIpre, RAVIt, RAVIa, RAEFa were greater than those of normal control group (P < 0.05 or P < 0.01), except that RAVIp, RAEFp, e/a were less than those of normal control group (P < 0.05). Conclusion Right atrial reservoir function and booster pump function increase, while right atrial conduit function decreases in WHO FCⅠPH. RT-3DE has potential ability to evaluate right atrial function.

Methods 56 patients with primary knee OA given intra-articular injection of Sodium Hyaluronate in Central Hospital of Cangzhou from January 2013 to January 2016 (experimental group) and 30 healthy individuals (control group) were recruited into the study. There were no significant differences in gender, age, and body mass index between the two groups (P > 0.05). Visual analogue scale (VAS) was used to evaluate the pain level of the patients. The IL-17 concentrations in both plasma and synovial fluid were measured before intra-articular injection of Sodium Hyaluronate and at 1, 4, 8, 12 weeks after injection in the experimental group. The IL-17 levels in plasma were measured in the control group. The IL-17 levels were determined using ELISA method. Results Average VAS scores within 8 weeks after the injection were substantially lower than pre-injection periods (P < 0.05) in the experimental group. Average VAS pain score at 12 weeks after intra-articular injection of Sodium Hyaluronate was not statistically significant in comparison with pre-injection period (P > 0.05). Average plasm IL-17 concentration in the control group was (2.4±1.7) pg/mL. OA patients before the injection had higher plasma IL-17 concentration compared to the control group (P < 0.05). Average IL-17 levels in both synovial fluid and plasm at each time point for patients with OA within 8 weeks after the injection were significant lower than pre-injection periods (P < 0.05), but IL-17 levels in plasma were higher than controls (P < 0.05). Average IL-17 levels in both synovial fluid and plasm at 12 weeks after the injection were not statistically significant in comparison with pre-injection periods (P > 0.05). Conclusion For the patients with knee OA, pain symptoms have significantly been improved and IL-17 levels in both synovial fluid and plasm reduced within 8 weeks after intra-articular injection of Sodium Hyaluronate. After 12 weeks, the clinical curative effect has disappearenced, IL-17 levels in both synovial fluid and plasm are restored to the level before treatment.

Application of PBL teaching method and the multimedia technology in the teaching of orthopedic surgery

Objective To explore the application of PBL teaching method combined with multimedia technology in orthopedic surgery. Methods 42 students from the 2013 class subject of Chinese medicine bone injury of Heilongjiang University of Chinese Medicine were selected. They divided into PBL combined with multimedia technology group (22 cases) and multimedia group (20 cases) according to the drawing method. The application effect was analyzed through the examination results and case writing, skills assessment, communication skills attitude evaluation. Results The scores of test, record writing, satisfaction, skills standards, communication skills in the PBL multimedia technology combined with the group were higher than those in the multimedia group, with statistically significant difference (P < 0.05). Conclusion PBL teaching method combining with multimedia technology has significant application effect in TCM traumatology surgery teaching, students have been improved obviously in communication, skill, attitude, thinking ability, which is worth popularizing in clinical teaching.

Information management and information system major is a kind of complex specialty which integrates management, economics and computer science and technology. According to their traditional characteristics, different universities have formed different talents training models from the aspects of system development, information resources management, economic information management and medicine information management. Set up the information management and information system specialty of Traditional Chinese Medicine(TCM) colleges is not a long time. So far, in the setting of talent cultivation and curriculum, but also generally at the learning stage of exploration, it has not yet formed a training mode of universal and characteristic of Chinese medicine talents. This study is based on the characteristics of the information management and information system specialty of Shandong University of TCM——the set up of TCM basis courses group, TCM culture courses group and TCM informatization courses group which combined with years of teaching practice, put forward to "make the students understand TCM","shaping professional accomplishment by TCM culture" and "using information technology to serve TCM" that provide a reference for the cultivation of talents in information management and information system specialty in TCM colleges.

Difficulties and suggestions of ESP education of medical universities in Western China

This paper makes a reflection on the ESP education of medical universities of Western China based on two guidance documents. It points out that it is imperative to reform the college English teaching and analyses the difficulties for medical universities of Western China to implement ESP education. Various qualities of students, lack of ESP textbooks and ESP teachers are regarded as main factors that limit the ESP teaching in medical universities. Some suggestions are proposed in this article. It is believed that EGP teachers being transformed into ESP teachers, adoption of classification education and implementation of ESP in a small group, demand analysis and various teaching methods, and practical ESP teaching model can stimulate the ESP teaching of medical universities of Western China.

Practice and discussion on PBL teaching method in clinical internship of Respiratory Intensive Care Unit

Objective To explore the application effect of PBL teaching method combined with a variety of teaching methods in the clinical internship of Respiratory Intensive Care Unit (RICU) nursing. Methods 87 nursing students were enrolled in the RICU of Traditional Chinese Medicine Hospital Affiliated to Xinjiang Medical University from September 2013 to March 2016. They were divided into experimental group (44 cases) and control group (43 cases) by envelope method. The control group was received the traditional teaching mode, the experimental group was received PBL teaching methods. After the internship, the students were evaluated by theory and practice test and rated by teachers. Then the awareness, interest and learning achievement of ICU before and after the intensive were analyzed by questionnaire survey. Results The scores of the experimental group were higher than those of the control group (P < 0.05). The students in the questionnaire survey showed that the awareness, interest rate and learning achievement of the intensive care unit were better than those before the practice. Conclusion PBL teaching method can reduce the pressure of students in RICU internship, and ensure that students can fully access to the patients, increase hands-on exercise, increase internship content, improve learning enthusiasm, and fully enhance students' interest in learning.

Application and effect of Ridit analysis in hospital performance evaluation

Objective To investigate the application and effect of Ridit analysis in hospital performance evaluation. Methods From February to July 2015, the working reports during 2014 from five clinical departments of Beijing Tongren Hospital, Capital Medical University (Departments of Respiration, Gastroenterology, Orthopedics, Neurosurgery, Obstetrics and Gynecology) were investigated. Case classification and disease outcome, medical quality and medical risk were analyzed by Ridit. Results In the 9905 patients, 3394 patients were cured, 5895 patients were improved, 566 patients were not cured, 50 patients died. 5827 patients were with type A, 2218 patients were with type B, 1295 patients were with type C, 565 patients were with type D. In the five departments, Department of Obstetrics and Gynecology and Department of Orthopedics had better medical quality, followed by Department of Respiratory and Department of Neurosurgery, Department of Gastroenterology was the worst. Department of Gastroenterology and Department of Neurosurgery had higher medical risk, followed by Department of Respiratory and Department of Orthopedics, Department of Obstetrics and Gynecology was the lowest. Conclusion The application of Ridit analysis in hospital performance evaluation can effectively reflect the medical risk and the medical quality from the cases classification and the disease outcome, which can provide objective reference for the department management.

Influence of predictive nursing intervention on nursing satisfaction and negative emotion in patients with chronic hepatitis B hepatic failure

Objective To explore the influence of predictive nursing intervention on nursing satisfaction and negative emotion in patients with chronic hepatitis B hepatic failure. Methods 80 patients with chronic hepatitis B hepatic failure treated in the First Affiliated Hospital of China Medical University from August 2014 to August 2016 were selected. All the patients were divided into routine nursing group and predictive nursing intervention group according to the random number table, 40 cases in each group. The hospitalization time, nursing quality evaluation score, nursing defects score, nursing satisfaction and negative emotion between two groups were compared. Results The hospitalization time was shorter and the nursing quality score was higher and nursing defect score was lower in predictive nursing intervention group when compared with routine nursing group, with statistically significant differences (P < 0.05). The nursing satisfaction rate of predictive nursing intervention group was higher than that of routine nursing group, with statistically significant difference (P < 0.05). The scores of anxiety and depression in two groups after nursing were lower than before nursing, predictive nursing intervention group was more pronounced than routine nursing group, with statistically significant differences (P < 0.05). Conclusion The predictive nursing intervention applied in patients with chronic hepatitis B hepatic failure can shorten hospitalization time, improve the quality of nursing service and satisfaction, reduce negative emotion, when compared with routine nursing.

Effect of improved health education model on conscious patients with endotracheal intubation

Objective To study application effect of improved health education model on the conscious patients with endotracheal intubation. Methods From August 2013 to August 2014, 86 patients with endotracheal intubation in Department of Critical Care Medicine, Handan Central Hospital, Hebei Province were selected as study objects. All patients were randomly divided into intervention group and control group, 43 cases of each group. Improved health education model was used in intervention group, routine health education model was applied in control group. The effect was compared beween two groups. Results Tracheal intubation for 3 d and after extubation of 24 h, Hamilton anxiety scale score in intervention group was lower than that in control group (P < 0.05). The degree of treatment (83.7%) in intervention group was higher than that in control group (65.1%) (P < 0.05). The self-extubation rate (2.3%) in intervention group was significantly lower than that in control group (14.0%) (P < 0.05). Conclusion Junior nurses are the high risk factor of unplanned extubation. So the application of improved health education model can improve the level of health education in the junior nurses, relieve anxiety of patients, improve the degree of coordination treatment, and prevent the occurrence of unplanned extubation.

The application of performance evaluation in transfusion room based on PDA platform

Objective To count nurses' workload in scientific, fair and just way based on PDA platform, and re-allocate department bonus according to the performance. Methods Since September 2015, PDA information technology was first adopted in Transfusion Room, Affiliated Hospital of Guilin Medical University (below "our department" for short), each operation conducted by the nurse for the IV fluids patients was scanned by PDA, which can timely reflect the index of nursing workload. Performance was ranking according to the different positions, ages, nurse levels. Good performance was inclined to the positions with night shift, high risk, and great difficulty; advocating same pay for the same position, and division the bonus according to the assessment results. Results The nursing personnel of our department were more positively and proactively, and the nursing service satisfaction and the patient satisfaction were both improved after the performance assessment was implemented based on the PDA platform. The difference had statistical implication (P < 0.05). Conclusion After executing the nursing performance revolution based on the PDA platform, the infusion room's working efficiency is increased, and it realizes the nursing performance is divided according to the workload, more labor got more payment, the diligent is rewarded and the lazy is punished, the principle of fair, justice and transparent, which arouse the enthusiasm of the nurses, improve the nursing quality and nursing management.

Effect of evidence-based nursing on life quality and complications of patients with tuberculous meningitis

Objective To observe the effect of the evidence-based nursing on the quality of life and complications of patients with tuberculous meningitis. Methods From June 2015 to June 2016, 70 patients with tuberculous meningitis treated in Hebei Chest Hospital were selected. They were divided into observation group and control group according to random number table, with 35 cases in each group. The control group was received routine nursing, and the observation group was received evidence-based nursing for nursing intervention. The compliance and life quality of patients, complications and nursing satisfaction in the two groups were compared. Results The rates of compliance to medication, periodic review, conditions self-monitoring, balanced diet, regular work-and-rest time, disinfection and isolation and optimistic attitude in the observation group were higher than those in the control group (P < 0.05). The score of life quality indexes in the observation group was higher than that in the control group, with statistically significant difference (P < 0.05). Total incidence of complications, such as pressure sores, constipation, increased intracranial pressure and dyspnea in the observation group in the observation group was less than that in the control group, with statistically significant difference (P < 0.05). The satisfaction to nursing in the observation group was higher than that in the control group, with statistically significant difference (P < 0.05). Conclusion For patients with tuberculous meningitis, evidence-based nursing intervention can significantly improve the clinical efficacy, increase compliance to treatment, reduce the incidence of complications through scientific and effective nursing measure, which is beneficial to the life quality and prognosis of the patients. It is worthy of clinical application.

The influences of flush methods during daytime continuous renal replacement therapy without anticoagulation on machine pressures

Objective To explore the saline flushes and non-saline flush's influence on the machines' pressure values when renal replacement therapy (CRRT) was imposed on the critical patients with high risk of bleeding. Methods 43 patients who were in the general hospital of Clinical Department of Nephrology, PLA General Hospital from May 2014 to February 2015 with high risk of bleeding acute or chronic renal failure undergoing CRRT without anticoagulation were selected and divided into saline flush group (21 cases) and non-saline flush group (22 cases) according to whether saline flushing pipeline was adopted. To observe The TMP, PBE and filter pressure decrease value (ΔP) changes of the two groups were observed when they were in therapy at 6 points in time, which was from the start of CRRT (T0), after 1 h (T1),2 h (T2), 3 h (T3), and 4 h (T4) of CRRT, and at the end (T5) of CRRT. Results In non-saline flush group, TMP was greater on T3, T4, T5 (P < 0.01) than T0, and was greater on T5 than T1 (P < 0.01). In saline flushes group, TMP was greater on T4, T5 than T0 and T1 (P < 0.05). The change of PBE in non-saline flush group was significantly greater on T1, T2, T3, T5 than in saline flushes group (P < 0.05). Compared with non-flush group at T0, ΔP were increased at T1, T2, T3, T4 (P < 0.05), ΔP was significantly greater at T5 than T1 (P < 0.01). At T5, the ΔP change value was greater in non-saline flush group than saline flush group (P < 0.05). Due to the high pressure limits, two group patients' therapy machine were all alarmed and stopped after 5 hours of CRRT, which including 1 case of saline flush (4.76%), 4 cases of non-saline flush (18.18%), the difference has the statistical significance (P < 0.05). Conclusion Saline flushes can restrain the increase of the pressure on the machine. It makes sense for the prevention of blood coagulation of extracorporeal circulation after 5 h of CRRT.

Influence of posture intervention on the gastrointestinal function and complication in patients with colon cancer after laparoscopic cholecystectomy

Objective To explore the influence of posture intervention on the gastrointestinal function and complication in patients with colon cancer after laparoscopic cholecystectomy. Methods 96 patients with colon cancer from October 2013 to October 2015 in Sichuan Mianyang 404 Hospital were selected as the research objects, all of them were randomly divided into control group and study group by random number table, with 48 cases in each group. The control group was received conventional nursing, the study group was received posture intervention on the basis of the control group. The postoperative abdominal distension, gastrointestinal function, operation medical cost, and postoperative complications of two groups were copared. Results After different treatment, the rate of postoperative abdominal distension of the study group was significantly lower than that of control group (P < 0.05). The postoperative anal exhaust time, bowel sound recovery time and gastrointestinal peristalsis recovery time of the study group were significantly shorter than those of control group (P < 0.05). However, there were no significant differences on anesthesia time, operation time and blood loss between the two groups (P > 0.05). Moreover, after intervention, the hospital stay, ambulation time and medical cost were significantly lower in study group as compared with control group (P < 0.05).The complication rate of the study group were significantly lower than that of the control group (P < 0.05). Conclusion The implementation of the posture intervention can significantly shorter the hospital stay, decrease the incidence of abdominal distension and complications, enhance the recovery of patientsˊ gastrointestinal function, and improve life quality of patients. It is worthy of clinical application.

Nursing effect of comprehensive rehabilitation nursing of traditional Chinese medicine on cerebral hemorrhage patients with hemiplegia

Objective To explore nursing effect of comprehensive rehabilitation nursing of traditional Chinese medicine on cerebral hemorrhage patients with hemiplegia. Methods 124 cerebral hemorrhage patients with hemiplegia were enrolled in the 174th Hospital of PLA from January 2015 to December 2016. The patients were divided into comprehensive rehabilitation nursing of traditional Chinese medicine group (62 cases) and routine nursing group (62 cases) by random number table method, which were given comprehensive rehabilitation nursing of traditional Chinese medicine and routine nursing care. The clinical efficacy, motor function, daily life ability and psychological function between two groups before and after treatment were compared. Results Total effective rate in comprehensive rehabilitation nursing of traditional Chinese medicine group was higher than that in routine nursing group (P < 0.05). Fugl-Meyer motor function scale, Berg balance function scale and Barthel index scores of two groups after nursing were significantly higher than before nursing (P < 0.01), the scores in comprehensive rehabilitation nursing of traditional Chinese medicine group were significantly higher than those in routine nursing group (P < 0.01). The scores of Hamilton anxiety scale and Hamilton depression scale of two groups after nursing were significantly lower than those before nursing (P < 0.01), the scores in comprehensive rehabilitation nursing of traditional Chinese medicine group were significantly lower those in routine nursing group (P < 0.01). Conclusion The comprehensive rehabilitation model of traditional Chinese medicine can improve the clinical curative effect of cerebral hemorrhage hemiplegia patients, significantly improve the motor function, daily living ability and psychological function, and has certain clinical promotion value.
