4 Study on quality standard of Shengjiang Pills

Objective To research and establish the quality standard of Shengjiang Pills. Methods Thin layer chromatography (TLC) was used to identify Rhizoma Curcumae Longae and Radix et Rhizoma Rhei in Shengjiang Pills. High performance liquid chromatography was used to determine the contents of aloe-emodin, rhein, emodin, chrysophanol, and physcion in Shengjiang Pills. Results The characteristic spots were obvious, the resolution was good, and the negative control had no interference by TLC. The average contents of aloe-emodin, rhein, emodin, chrysophanol, and physcion in three batches of Shengjiang Pills were respectively 1.19, 0.64, 2.77, 5.12, 0.85 mg/g. In the range of 6.37-108.90 μg/mL (r = 0.9997), 6.73-115.00 μg/mL (r = 0.9994), 6.96-119.00 μg/mL(r = 0.9997), 6.96-119.00 μg/mL (r = 0.9999), 3.32-56.80 μg/mL (r = 0.9998), respectively, and had a good linear relationship with the integral value of peak area. The average recovery rates were 89.91% (RSD = 2.12%), 92.87% (RSD = 1.06%), 91.24% (RSD = 1.67%), 93.50% (RSD = 2.31%), 88.64% (RSD = 2.46%). Conclusion The method established in this experiment has good reproducibility, easy to operate, accuracy and reliability, and can be applied to the quality control of Shengjiang Pills.

8 Comparison of the diagnostic value of high-frequency ultrasound and magnetic resonance imaging in frozen shoulder

Objective To explore the diagnostic value of high-frequency ultrasound and magnetic resonance imaging (MRI) in frozen shoulder. Methods Retrospective analysis was performed on 50 patients (totally 54 affected shoulders) who underwent shoulder ultrasound and MRI from December 2017 to July 2019. Frozen shoulder was the “gold standard” for clinical diagnosis, there were 29 cases (30 affected shoulders) in the frozen shoulder group and 21 cases (24 affected shoulders) in the non-frozen shoulder group. The imaging characters and diagnostic efficacy of ultrasound and MRI were analyzed. Results High-frequency ultrasound and MRI showed that the joint capsule thickening in the frozen shoulder group were higher than those in the non-frozen shoulder group, with highly statistically significant differences (all P < 0.01). The sensitivity, specificity, accuracy, positive predictive value and negative predictive value of ultrasound diagnosis for frozen shoulder were 70.0%, 87.5%, 77.8%, 87.5%, and 70.0%, respectively; and the sensitivity, specificity, accuracy, positive predictive value and negative predictive value of MRI diagnosis for frozen shoulder were 86.7%, 91.7%, 88.9%, 92.9%, and 84.6%, respectively. Conclusion High-frequency ultrasonic examination has high application value to frozen shoulder and can be used as an important method to diagnose frozen shoulder.

12 Effects of health care moxibustion at guānyuán on serum and intestinal oxidation and inflammatory reaction in aged rats

Objective To observe the effects of health care moxibustion at guānyuán on serum and intestinal oxidation and inflammatory reaction in aged rats, and explore the possible mechanism of anti-aging. Methods According to the method of random number table, 12 normal aging female Wistar rats were divided into old group (six rats) and moxibustion group (six rats), and six young female Wistar rats were divided into young group. The abdominal hairs of rats of three groups with the diameter of guānyuán about 1.0 cm were shaved off. Young group and old group: the rats were grabbed and put into a special fixator without other intervention, 20 min/d, for 40 days. Moxibustion group: the rats were grabbed and put into a special fixator, the abdomen was aligned with the opening at the bottom of the fixator, and the ignited moxa strips were placed under the opening, aiming at the guānyuán of the rats. During moxibustion, the distance between the moxa strips and the rats was adjusted to keep the temperature basically constant. Moxibustion intervention for 20 min/d, for 40 days. The levels of SOD, MDA, IL-6, TNF-α, IL-1β and IL-10 in peripheral serum and intestine were detected. The microcirculation blood perfusion of guānyuán was measured by laser Doppler flow meter. Results Compared with young group, the serum and intestinal SOD levels in old group were decreased (P < 0.05 or P < 0.01), and the serum and intestinal MDA levels in old group were increased (P < 0.05 or P < 0.01). Compared with old group, the intestinal SOD level in moxibustion group was significantly increased (P < 0.01), and the intestinal MDA level in moxibustion group was significantly decreased (P < 0.01). Compared with young group, the expression levels of IL-6, TNF-α and IL-10 in old group were increased (P < 0.05). Compared with old group, the expression levels of IL-6, TNF-α, IL-1β and IL-10 in moxibustion group were significantly decreased (P < 0.01). Compared with young group, the peak and amplitude of microcirculation of guānyuán in old group decreased, and the blood perfusion volume decreased significantly. Compared with old group, the peak and amplitude of microcirculation of guānyuán in moxibustion group increased, and the blood perfusion volume increased significantly. Conclusion Moxibustion at guānyuán can regulate the intestinal immune system and enhance the antioxidant capacity of naturally aging rats. The mechanism may be related to reducing the level of inflammatory cytokines in serum and intestine, enhancing the ability of superoxide dismutase to scavenge oxygen free radicals and increasing microcirculatory blood flow in acupoint area.

16 Analysis of the pre-thrombotic status and the correlation study of MTHFR gene C677T polymorphism in women with recurrent spontaneous abortion

Objective To explore the relationship between recurrent spontaneous abortion (RSA) and coagulation-related indicators, as well as the relationship between the polymorphism of methylenetetrahydrofolate reductase (MTHFR) gene C677T polymorphism and the genetic susceptibility to RSA. Methods A total of 77 RSA patients in the First Affiliated Hospital of Guangzhou Medical University from September 2018 to March 2019 were selected as the RSA group, and a total of 41 normal healthy women were selected as the normal control group. EDTA anticoagulated whole blood was collected from two groups, and the differences in coagulation indicators and markers of thrombosis were compared between the two groups. The MTHFR gene was amplified by polymerase chain reaction (PCR) and then sequenced by Sanger method, and the genetic polymorphisms of MTHFR gene C677T polymorphism of the two groups were analyzed and compared. Results The fibrinogen (FIB) level of the RSA group was higher than that of the normal control group, the difference was statistically significant (P < 0.05). The incidence of PS defects in the RSA group was higher than that in the normal control group, and the difference was statistically significant (P < 0.05). The best cut-off value of PS to judge the existence of prethrombotic state (PTS) in RSA patients was 45%, the area under the curve was 0.701 (95%CI: 0.611-0.791, P < 0.01), the sensitivity was 95.1%, and the specificity was 42.9%. There was a statistically significant difference in genotype comparison between the two groups of people (P < 0.05). The comparison of allele frequencies between the two groups showed statistically significant differences (P < 0.05). Conclusion FIB levels and PS defects may be related to the occurrence of RSA, which can be used as screening indicators for RSA, and PS defects have a certain reference value for the judgment of RSA. MTHFR gene polymorphic site C677T mutation may be one of the causes of RSA.

21 Application value of digital oil drop PCR method and ARMS method in detecting BRAF gene mutation in colorectal cancer

Objective To explore the application value of digital oil drop polymerase chain reaction (ddPCR) and amplification hindered mutation system (ARMS) in the detection of serine/threonine protein kinase (BRAF) V600E mutation in colorectal cancer patients. Methods Surgical specimens of 162 patients with colorectal cancer treated in Affiliated Hangzhou First People′s Hospital, Zhejiang University School of Medicine from April 2018 to July 2019 were collected. ARMS method and ddPCR method were used to detect BRAF V600E mutation in colorectal cancer patients, and the positive rate and consistency of ARMS and ddPCR were compared. The relationship between BRAF V600E mutation and clinicopathological characteristics of patients with colorectal cancer were analyzed. Results The positive rates of BRAF V600E mutation detected by ARMS and ddPCR were 8.64% (14/162) and 9.26% (15/162). High-throughput sequencing was performed on one case of the ambiguity of the BRAF V600E mutation detected by the ARMS method, and the result of the ddPCR method was consistent with the high-throughput sequencing result. Corresponding to the ddPCR, the positive coincidence rate, negative coincidence rate and consistency of the ARMS method were 93.33%, 99.32% and 99.38%, respectively. The patients who were higher the age, the poorly differentiated histological type, in clinical stages Ⅲ-Ⅳ, the higher the distribution of BRAF V600E mutations, and the differences were statistically significant (P < 0.05). Conclusion Both ddPCR and ARMS can be used to detect BRAF V600E mutations in colorectal cancer, and ddPCR has higher detection efficiency.

25 Study on the quality standard of Changkang Capsules

Objective To establish the quality standard of Changkang Capsules. Methods Thin layer chromatography (TLC) was used to identify Coptidis Rhizoma, Aucklandiae Radix, Draconis Sanguis and Corydalis Rhizoma. Meanwhile, high performance liquid chromatography (HPLC) was used to determine the effective components of berberine and bamatine in Coptidis rhizoma. the chromatographic conditions were: Angilent XDB-C18 column (4.6 mm × 150 mm, 5 μm), acetonitrile-0.05 mol/L KH2PO4 (50∶50, v/v) (added 0.4 g of sodium dodecyl sulfate into every 100 mL solution, then adjusted the pH value to 4.0 with H3PO4), 1.0 mL/min, 345 nm, room temperature, 10 μL. Results TLC identification spots were clear and the negative control had no interference. The effective component of berberine in Coptidis rhizoma had good linear relationship in the range of 29.4-98.0 μg/mL. The average recovery was 101.00%, and the precision RSD was 0.93%. The effective component of bamatine in Coptidis Rhizoma had good linear relationship in the range of 7.1-31.8 μg/mL. The average recovery was 99.68%, and the precision RSD was 0.80%. Conclusion The research results show that this method can be used for the quality control of Changkang Capsules.

30 Reticular meta-analysis of urethral catheterization in patients with benign prostatic hyperplasia assisted by topical anesthetics

Objective To systematically evaluate the effect of different topical anesthetics on urethral catheterization in patients with benign prostatic hyperplasia. Methods Randomized controlled trials (RCT) related to PubMed, EMbase, Cochrane Library, Web of Science, CBM, CNKI, WanFang and VIP databases were searched by computer, and the retrieval time was set up until November 2019. Cochrane Handbook 5.1.0 was used to evaluate the literature quality, and Stata 13.0 and WinBUGS 1.4.3 software were used to analyze the one-time catheterization success rate and the incidence of catheterization pain in patients with benign prostatic hyperplasia in the literature. Results A total of 33 RCTs involving 5 topical anesthetics were included, with a total sample size of 3329 cases. The results of reticular meta-analysis showed that: In terms of one-time catheterization success rate, Dyclonine Hydrochloride (OR = 9.81, 95%CI [2.36, 52.59]), Tetracaine Hydrochloride (OR = 11.65, 95%CI [5.05, 31.00]), Shutai (OR = 0.07, 95%CI [0.03, 0.15]), Benoxinate Hydrochloride (OR = 0.07, 95%CI (0.03, 0.17)], Lidocaine (OR = 0.08, 95%CI [0.05, 0.15]) were superior to sterile paraffin oil, the differences were statistically significant (all P < 0.05). In terms of the incidence of catheterization pain, Dyclonine Hydrochloride (OR = 0.09, 95%CI [0.01, 0.66]), Tetracaine Hydrochloride (OR = 0.04, 95%CI [0.01, 0.25]), Shutai (OR = 7.13, 95%CI [1.04, 45.50]), Benoxinate Hydrochloride (OR = 34.09, 95%CI [7.18, 210.45]) and Lidocaine (OR = 8.58, 95%CI [2.96, 24.69]) were all superior to sterile paraffin oil, the differences were statistically significant (all P < 0.05). The result of the ranking probability graph indicated that Shutai was the best solution to increase the one-time catheterization success rate, and Benoxinate Hydrochloride was the best solution to prevent catheterization pain. Conclusion In improving the one-time catheterization success rate, Shutai is superior to other topical anesthetics. Benoxinate Hydrochloride is superior to other topical anesthetics in the prevention of catheterization pain.

35 Correlation of Skp2 high expression with neoadjuvant chemotherapy resistance in patients with osteosarcoma

Objective To explore the effect of Skp2 in neoadjuvant chemotherapy resistance of osteosarcoma. Methods Gene expression profiles of osteosarcoma and resistant osteosarcoma were collected from GSE126209, GSE16089 and TCGA datasets. The mechanisms of Skp2 resistance to osteosarcoma were identified. From February 2018 to June 2019, 17 patients with osteosarcoma admitted to the Third Clinical Medical College of Xinjiang Medical University were collected, non-drug resistance was defined as tumor cell necrosis rate (TCNR) ≥90%, and drug resistance was defined as TCNR < 90%, 11 cases of including non-drug resistant tumor tissues and 6 cases of drug resistant tumor tissues; 11 cases of non-drug-resistant paracancer tissues and 6 cases of drug-resistant paracancer tissues. RT-qPCR and Western-blot were used to detect mRNA and protein expression of Skp2 and related genes in osteosarcoma carcinoma tissues and adjacent tissues. Results The Skp2 was up-regulated in both osteosarcoma tissue and Methotrexate resistant cells. It was involved in cell cycle and FOXO pathway. Molecular experimental results showed that Skp2 and AKT expressions in drug-resistant osteosarcoma tissues were significantly increased (P < 0.05); the expressions of p27, FOXO1 and E-Cadherin in resistant osteosarcoma were significantly lower (P < 0.05). Cox analysis indicated that Skp2 (HR = 1.52, 95%CI: 0.67-3.46, P = 0.31), AKT (HR = 1.04, 95%CI: 0.48-2.28, P = 0.93), p21 (HR = 1.09, 95%CI: 0.51-2.34, P = 0.83), p27 (HR = 1.50, 95%CI: 0.67-3.38, P = 0.32) may be risk factors for prognosis of osteosarcoma. Conclusion Skp2 is involved in neoadjuvant chemotherapy resistance and in osteosarcomd and is associated with the cell cycle and FOXO pathway.

40 Progress in the diagnosis and treatment of lower-risk myelodysplastic syndrome

The lower-risk myelodysplastic syndrome (MDS) is defined by the prognostic scoring system, and the inclusion of molecular abnormalities is conducive to the accuracy of risk stratification of the system. In view of the peripheral blood cytopenia of lower-risk MDS, erythropoietin analogues (erythropoiesis-stimulating agents, darbepoetin-α), transforming growth factor-β (TGF-β) pathway blockers (luspatercept) and Lenalidomide can be selected to improve the anemia of MDS, Romiplostim and Eltrombopag are used for thrombocytopenia, demethylation drugs and antithymocyte globulin ameliorate pancytopenia of MDS, and many new drugs in clinical trials are intended for the signaling pathway of specific gene mutation. In this paper, the diagnosis and treatment of low-risk MDS are reviewed in order to guide the clinical treatment.

45 Application progress of endoscopic interventional therapy in airway stenosis

Airway stenosis is a general term for laryngotrachea, bronchus, left and right main bronchial airway stenosis caused by various causes. According to the cause, it is divided into benign airway stenosis and malignant airway stenosis. Domestic and foreign literature reports that the most common causes of benign airway stenosis are mechanical trauma, infection, Wegger′s granuloma, tracheal amyloidosis, etc., while the most common cause of malignant airway stenosis is primary or metastatic airway malignancy tumor. The common methods used to solve airway stenosis in the past include endoscopic dilation, simple endoscopic resection and open surgery. In recent years, with the rapid development of pulmonary interventional diseases, endoscopic interventional therapy has become the main treatment method for treating benign and malignant airway stenosis. This article aims to introduce the interventional treatment methods for different types of airway stenosis.

50 Research progress of leukotriene B4/leukotriene B4 receptor pathway in the mechanism of cerebral ischemic injury

Leukotriene B4 (LTB4) is one of the downstream products of arachidonic acid metabolized by 5-lipoxygenase, and it is a potent pro-inflammatory lipid mediator. By sorting out the existing literature, it is found that after cerebral ischemia, neurons, microglia and astrocytes release LTB4, which promotes neutrophil infiltration and release inflammatory factors, leading to secondary brain injury after cerebral ischemia. Further study on the role and mechanism of targeting LTB4-BLTR axis in inflammatory response after cerebral ischemia injury can find new effective therapeutic drugs for cerebral ischemia injury.

54 Research progress of CD38 in cardiovascular and cerebrovascular diseases

Cardiovascular and cerebrovascular diseases seriously affect people′s health and life in today′s society. They mainly refer to ischemic or hemorrhagic diseases of heart, brain and systemic tissues caused by hyperlipidemia, atherosclerosis and hypertension. CD38 is a type Ⅱ transmembrane glycoprotein located on the membrane and its functional structure is relatively complex. Current studies have found that the more complex functions of CD38 make it play an important role in cardiovascular and cerebrovascular diseases. Based on the latest domestic and foreign reports, this article briefly reviews the role of CD38 in different cardiovascular and cerebrovascular diseases.

58 Research progress on hyperprogression of immunotherapy for lung cancer

In recent years, due to futher understanding of tumor microenvironment, the clinical research of tumor immunotherapy has made great progress, and immunotherapy has become a hot spot in the field of tumor therapy. Since the significant survival advantage of immunocheckpoint inhibitors in non small cell lung cancer patients, immunotherapy is more and more widely used in lung cancer patients, and the special reaction phenomenon after immunotherapy has gradually attracted people′s attention. There are reports showing that the phenomenon of pseudoprogressio and hyperprogressive occurs in some lung cancer patients after immunotherapy. However, there are currently controversies about how to accurately define and distinguish them, and it is challenging to correctly assess this unique tumor response. This article reviews the research on the definition, evaluation criteria, high-risk groups and related predictors of hyperprogression after immunotherapy.

62 Research progress of CD4 T lymphocytes in the pathogenesis of post-stroke depression

The pathophysiological mechanism of post-stroke depression is unclear. Regarding its neurobiological alterations, available research evidence includes elevated monoamine system (especially serotonin) alterations, hypothalamic pituitary adrenal axis abnormalities, disruption of the prefrontal cortex circuit, alterations in neuroplasticity and glutamate neurotransmission, and excess pro-inflammatory cytokines. The immune inflammatory response may play an important role in the pathophysiological process of post-stroke depression, especially the imbalance of CD4 T lymphocytes and their related cytokines, which may damage the neuroplasticity of T lymphocytes and maintain the function of the nervous system, resulting in the imbalance of the regulation of the immune inflammatory response after stroke, affecting the synthesis of neurotransmitters and the release of metabolism, thus leading to depression.

66 Advances in molecular pathogenesis and treatment of prolactinoma

Prolactinoma is a type of tumor caused by imbalance of pituitary gland secretion. Excessive secretion of prolactin leads to prolactinoma or complications related to prolactin secretion. Studies have found that disturbances in cell signaling pathways such as phosphatidylinositol-3-kinase-protein kinase B, transforming growth factor-β could cause abnormal expansion of prolactin cells, excessive secretion of prolactin, and eventually lead to the formation of prolactinoma. The current clinical treatment for prolactinoma mainly uses dopamine agonists, but also surgical treatment and radiation therapy. With the study of the molecular mechanism of its pathogenesis, new targets have been provided for the treatment of tumors, such as programmed death-1, programmed death-ligand-1 and other immunosuppressive targets. This article reviews the molecular mechanism and the progress of treatment of prolactinoma in order to better guide the early diagnosis and clinical treatment of prolactinoma.

70 Research progress of patients with atrial fibrillation combined with anxiety and depression

Atrial fibrillation (AF) is one of the most common tachyarrhythmias. Its incidence increases year by year with the increase of age in China, and it is an important cause of severe complications such as stroke and heart failure and so on. AF often causes anxiety, depression and other psychological disorders in patients, while anxiety and depression will increase the incidence and recurrence rate of AF. AF combined with anxiety and depression can affect the quality of life, disease prognosis and clinical outcome of patients, which has attracted extensive attention of cardiovascular doctors. This artical mainly gives a brief summery of the related research progress on the epidemiology, clinical prognosis, related factors, occurrence mechanism, evaluation and diagnosis process, treatment and other related research progress of AF combined with anxiety and depression, in order to provide guidance for the clinical diagnosis and treatment of AF combined with anxiety and depression.

Methods Total of 24 students of grade 2016 and 2017 in Tongji Medical College of Huazhong University of Science and Technology were divided into VR teaching group and control group according to the random number table method, with 12 students in each group. The traditional teaching mode was used in control group, while VR technology teaching mode was used in VR teaching group. Both groups had six class hours. After the end of teaching, questionnaire survey and classroom test were carried out on two groups of students from the aspects of stimulating learning interest, improving learning efficiency, participating in classroom interaction, mastering theoretical knowledge, improving practical ability, cultivating self-learning ability, expanding learning ideas and teaching satisfaction. And the basic knowledge of written examination was implemented. The teaching effects of teaching methods of two groups were compared. Results The proportions of VR teaching group in stimulating learning interest, participating in classroom interaction, improving practical ability and teaching satisfaction were higher than those of control group, the differences were statistically significant (P < 0.05). There were no significant differences between two groups in improving learning efficiency, mastering theoretical knowledge, cultivating self-learning ability and expanding learning thought (P > 0.05). The pass rate of classroom test in VR teaching group was higher than that in control group, the difference was statistically significant (P < 0.05). There was no significant difference between two groups of theoretical examination results (P > 0.05). Conclusion The application of VR technology in the teaching of acupuncture and moxibustion is conducive to improve students′ learning interest, enhance their enthusiasm for learning, improve the effect of classroom learning, and help students to master theoretical knowledge and improve their practical ability. In particular, it can play a positive role for students in Western medicine colleges to learn the course of acupuncture and moxibustion.

78 Application effect of traditional blackboard writing in the teaching of occupational medicine

Objective To explore application effect of traditional blackboard writing in the teaching of occupational medicine. Methods Students of grade 2013 and grade 2014 majored in preventive medicine in Peking University Health Science Center were selected as the research objects. The research period was from September 2017 to May 2019. Grade 2014 was a multimedia teaching group with 77 students, multimedia teaching was used. Grade 2013 was a blackboard teaching group with 78 students, traditional blackboard writing teaching was applied. At the end of the course, students’ recognition with the teaching mode were investigated, and their test scores were compared. Results There were no significant differences in age, gender, nationality and type of candidates between two groups (P > 0.05). The recognition degree of blackboard teaching group in stimulating learning interest, improving learning effect, improving the ability of analyzing and solving problems, and worth promoting to other chapters were higher than those of multimedia teaching group, the differences were statistically significant (P < 0.05). The teaching score of multimedia teaching group was lower than that of blackboard teaching group, the difference was statistically significant (P < 0.05). Conclusion In the process of teaching, the traditional blackboard writing should be reasonably applied to improve the teaching efficiency.

Therefore, how to combine clinical practice with the cultivation of scientific research innovation consciousness in the teaching process is an urgent problem in the current professional degree teaching reform. Based on the professional master’s teaching experience, the Teaching and Research Office of Oral Medicine of School of Stomatology, Air Force Medical University preliminarily set up the oral professional degree postgraduates scientific literacy cultivating path by optimizing the curriculum design, correcting the scientific attitude, stimulating the students’ interest and strengthening the teachers configuration. In addition, we apply the flexible education mode into scientific literacy teaching, which shows satisfied teaching effects. These above results are aimed to promote the reform of clinical medical education.

96 Application of comparative teaching method in pediatric anesthesia teaching for non-pediatric anesthesiology students

Objective To study the application effect of comparative teaching method in pediatric anesthesia teaching for non-pediatric anesthesiology students. Methods Sixty students from anesthesia department of many adult hospitals were selected from Anesthesia Department of Children’s Hospital Affiliated to Suzhou University from June 2018 to December 2019. They were divided into study group and control group according to the random digital table method, with 30 students in each group. Study group was given the comparative teaching method, while control group was given the traditional teaching method. The theoretical performance, skill operation level and teaching satisfaction of students of two groups were evaluated, and the scores were analyzed and compared. Results The scores of examination and teaching satisfaction in study group were significantly higher than those in control group (P < 0.05). Conclusion The comparative teaching method can significantly improve the theoretical level and clinical operation ability of non-pediatric anesthesiology students in resident standardization training, and increase the satisfaction of students with teaching, which is worthy of further research and promotion.

99 Effect of Diltiazem on blood concentration of Cyclosporine A in renal transplant patients

Objective To explore the clinical effect of combined use of Cyclosporine A (CsA) and Diltiazem (DTZ) in patients undergoing clinical kidney transplantation, and to evaluate the effect of DTZ on the blood concentration of CsA in kidney transplant patients. Methods A total of 108 patients who underwent kidney transplantation at the First Affiliated Hospital of University of Science and Technology of China (Anhui Provincial Hospital) from January 2013 to December 2017 were collected, and they were divided into the control group (54 cases) and the study group (54 cases) by random number table method. The patients in the control group only used CsA after the operation, while the study group received both CsA and DTZ after the operation. The data of CsA dosage and blood drug concentration of two groups one to six months after operation, and the liver and kidney toxicity of CsA were analyzed. Results There were no significant differences in CsA blood concentration and interaction between the two groups after renal transplantation (P > 0.05); the difference was statistically significant in time comparison (P < 0.05). Among them, the CsA blood concentration two to six months after oparation of the two groups were lower than one month after operation, and the CsA blood concentration three to six months after operation were lower than two months after operation, and the differences were statistically significant (P < 0.05). The differences between the two groups in the dose and time of CsA administration were statistically significant (P < 0.05); while the interaction comparison, and the difference was not statistically significant (P > 0.05). Among them, two, four and six months after operation were lower than one month after operation, the dose of CsA used in three months after operation was lower than two months after operation, while four and six months after operation were lower than three months after operation, and the differences were statistical significance (P < 0.05). After treatment, the levels of serum creatinine, urea nitrogen and uric acid between the two groups were lower than those before treatment, and the differences were statistically significant (P < 0.05). Conclusion DTZ can significantly increase the blood concentration of CsA and reduce the dosage of CsA, thereby reducing the side effects of CsA and reducing the economic burden of patients.

103 Relationship between metabolic syndrome caused by Olanzapine and serum IL-6 level and cognitive function

Objective To explore the relationship between metabolic syndrome (MS) caused by Olanzapine and serum interleukin-6 (IL-6) levels and cognitive function. Methods From January 2015 to December 2017, a total of 242 cases who were treated with Schizophrenia for more than two years of single-drug use were recruited at Shanghai Mental Health Center of Shanghai Jiao Tong University School of Medicine, Yuebei Third People′s Hospital, Jinhua Second Hospital, and Wenzhou Kangning Hospital in Zhejiang Province. According to whether they were associated with MS, they were divided into 105 cases in MS group and 137 cases in non-MS group. The positive and negative symptom scale (PANSS) was used to assess the clinical symptoms of patients with schizophrenia, while the repeatable battery for the assessment of neuropsychological status (RBANS) was used to assess cognitive function, and the serum IL-6 levels were measured by enzyme-linked immunosorbent assay. Results There were highly statistically significant differences in the use time and dose of Olanzapine between the two groups (P < 0.01). The score of PANSS negative symptoms in the MS group was higher than that in the non-MS group, and the difference was statistically significant (P < 0.05). The RBANS immediate memory, attention, delayed memory and total scores of the MS group were lower than those of the non-MS group, and the differences were statistically significant (P < 0.05). The level of serum IL-6 in the MS group was higher than that in the non-MS group, and the difference was statistically significant (P < 0.05). The serum IL-6 levels of the patients were negatively correlated with the RBANS attention score (r = -0.16, P < 0.05). Conclusion The increase of IL-6 caused by MS caused by Olanzapine may cause secondary cognitive impairment in patients with schizophrenia.

107 Analysis of death causes and death time of elderly inpatients in Nanjing Drum Tower Hospital in 2018

Objective To discuss the cause of death and the distribution of death time in elderly hospitalized patients, and to provide basis for prevention and treatment of elderly health problems. Methods A retrospective collection of general data on hospitalized deaths of elderly patients over 60 years old in Nanjing Drum Tower Hospital (hereinafter referred to as “our hospital”) from January to December 2018, descriptive statistics accounted on the proportion of deaths, and circular statistical analysis was used to study the distribution of patients′ death time in months, weeks and 24 hours. Results Among the 386 elderly patients who died, the first cause of death was malignant tumors (138 cases, 35.8%), followed by circulatory system diseases (120 cases, 31.1%), and respiratory system diseases (69 cases, 17.9%). The proportion of deaths from respiratory diseases such as lung infections in patients more than 80 years old was higher. The circular statistics analysis was found that the death peak date was July 31st when analyzed by month; according to the weekly analysis, there were more deaths on Monday, however the peak period of death time was from Friday to Monday; and the peak death time point within 24 hours was 01:45, while the peak time period was 23:00 to 04:28. Conclusion Malignant tumors are the first cause of death for elderly inpatients over 60 years old in our hospital, and the peak death period occurs in the early morning. Strengthening treatment and care during the peak death period and timely adjustment of medical measures will help reduce the mortality rate of elderly inpatients.

111 Analysis of the cause of emergency consultation in 1187 cases of upper gastrointestinal bleeding

Objective To evaluate the cause of bleeding in emergency consultations with upper gastrointestinal bleeding. Methods A retrospective study of cases of upper gastrointestinal bleeding in the emergency consultation of Department of Gastrointestinal, the First Medical Center, Chinese People′s Liberation of China General Hospital from June 2018 to June 2019, relevant clinical data of patients were collected, and the etiological composition of patients of different genders and ages were analyzed. Results A total of 1187 cases of upper gastrointestinal bleeding in emergency consultation were included in this study, including 752 cases in the male group and 435 cases in the female group; 628 cases in the young and middle-aged group (<60 years old), and 559 cases in the elderly group (≥60 years old). The common causes were 480 cases of decompensated portal hypertension in liver cirrhosis, 310 cases of non-steroidal anti-inflammatory drug (NSAID) related acute gastric mucosal disease, 177 cases of peptic ulcer, 117 cases of malignant tumor, and 34 cases of vascular malformations, 19 cases of cardiac mucosal tear syndrome, 16 cases of gastric stromal tumor, and 34 cases of unknown cause. The proportion of peptic ulcers in the male group was higher than that in the female group, and the difference was statistically significant (P < 0.05). The proportions of portal hypertension, peptic ulcer, and cardiac mucosal tear syndrome during decompensated liver cirrhosis in the young and middle-aged group were higher than those of the elderly group, while the proportions of NSAID-related acute gastric mucosal lesions and malignant tumors were lower than those of the elderly group, and the differences were highly statistically significant (P < 0.01). Conclusion Age and gender factors can affect the cause of acute upper gastrointestinal bleeding.

115 Evaluation of the effect of sulfate free polyethylene glycol electrolyte solution for bowel preparation before colonoscopy

Objective To assess the cleaning efficacy of bowel preparation of sulfate free polyethylene glycol electrolyte solution before colonoscopy. Methods From September 2017 to April 2019, 105 patients undergoing colonoscopy in digestive endoscopy center, the Forth Hospital of Anhui Medical University were consecutively collected and they were devided into SF-PEG group (group A) and polyethylene glycol electrolyte solution group (PEG-ES, group B). In group A, 3 L SF-PEG was taken orally in morning and night, while in group B, 1.5 L PEG-ES was taken orally in A single time on the examination day. The bowel preparation effect, completion time of colonoscopy and polyp detection rate of the two groups were observed. Results The right colonic scores, left colonic scores and total scores of group B were higher than those of group A, and the differences were all statistically significant (all P < 0.05); there was no significant difference in transverse colon score between the two groups (P > 0.05). The completion time of colonoscopy in group A was longer than that in group B, and the difference was statistically significant (P < 0.05). There was no significant difference in the detection rate of intestinal polyps between the two groups (P > 0.05). Conclusion Both regimens could provide adequate bowel preparation. Compared with 1.5 L PEG-ES taken orally on the examination day, intestinal preparation quality of 3 L SF-PEG taken orally in morning and night is lower.

118 Comparison of clinical effects between endoscopic submucosal dissection and transanal endoscopic microsurgery for early rectal cancer

Objective To investigate the efficacy of endoscopic submucosal dissection (ESD) and transanal endoscopic microsurgery (TEM) in the treatment of early colorectal tumors. Methods A total of 211 patients with early rectal cancer (uTis-uT1) who treated in Tangshan People′s Hospital of Hebei Province from May 2010 to May 2016 were selected. They were divided into ESD group (98 cases) and TEM group (113 cases). The operative time, hospital stay, intraoperative bleeding, complete tumor resection, tumor residual and recurrence were compared between the two methods. Postoperative complications such as hemorrhage, perforation, obstruction and stenosis occurred were compared  between the two groups. Results The operation time and hospital stay in ESD group were shorter than those in TEM group, and the differences were statistically significants (P < 0.01); and there were no significant differences in intraoperative bleeding, complete tumor resection rate, tumor residual rate and recurrence rate (P > 0.05). There was no statistically significant difference in the incidence of complications between the two groups (P > 0.05). Conclusion Both ESD and TEM are safe and effective in the treatment of early rectal cancer, but the operation time and hospital stay in ESD are significantly shortened.

122 Effects of problem-oriented chronic disease intervention behavior on medication compliance and quality of life in elderly COPD patients

Objective To investigate the application value of problem-oriented chronic disease intervention behavior of clinical pharmacists in elderly patients with chronic obstructive pulmonary disease (COPD). Methods A total of 212 COPD patients admitted to Suzhou Ninth People′s Hospital of Jiangsu Province from January to April 2019 were selected. They were divided into the control group (106 cases) and the study group (106 cases) according to the random number table method. The control group was given routine intervention, while the study group was given problem-oriented chronic disease intervention. After the intervention, the self-management scale, health status questionnaire, anxiety self-rating scale (SAS) and depression self-rating scale (SDS) and medication compliance of the two groups were compared. Results The scores of daily life management, self-efficacy management, emotion management, symptom management and information management in the study group were higher than those in the control group (all P < 0.01). The improved scores of positive and optimistic, with ventilator, with home oxygen therapy, familiar with disease knowledge, drug safety, healthy diet and lifestyle, and have quit smoking or are quitting smoking in the study group were higher than those in the control group (all P < 0.01). The vitality, social function, physiological function, emotional function, physical function, physical pain, mental health and overall health scores of the study group were all higher than those of the control group (all P < 0.01). Before intervention, there were no significant differences in SAS and SDS scores of the two groups (P > 0.05); after intervention, SAS and SDS scores of the two groups were lower than those before intervention, and the study group were lower than the control group (all P < 0.01). The total compliance rate of study group was higher than that of control group (P < 0.05). Conclusion Clinical pharmacist problem-oriented chronic disease intervention behavior has higher application value in elderly patients with COPD, which can improve patients′ self-management ability and medication compliance, relieve negative emotions, and improve the quality of daily life.

126 Effect of intravenous anesthesia with different doses of Nalbuphine combined with Propofol on elderly painless gastroscopy and its effects on neurocognitive disorders

Objective To compare the effects of different doses of Nalbuphine combined with Propofol on the elderly painless gastroscopy and its effects on neurocognitive disorders. Methods From March to December 2019, a total of 150 elderly patients who in the People′s Hospital of Chaotian District In Guangyuan, Sichuan Province, planned to undergo gastroscopy were randomly divided into N1, N2 and N3 groups by envelope method, with 50 patients in each group. Nalbuphine 0.06, 0.08 and 0.10 mg/kg were intravenous injected, Propofol was injected two minutes later and gastroscopy was performed. Systolic blood pressure (SBP), diastolic blood pressure (DBP), heart rate (HR), respiratory rate (RR), and pulse oxygen saturation (SpO2) were recorded before Nalbuphine injection (T0), before Propofol injection (T1), before gastroscopy placement (T2), gastroscopy reaching the duodenum (T3), at the end of examination (T4) and after awakening (T5). Onset time of anesthesia, gastroscope time, time of waking and leaving hospital, Propofol dosage, and incidence of complications were recorded. The montreal cognitive assessment scale (MoCA) was used to evaluate the neurocognitive function at the time of discharge (T6), postoperative 24 h (T7) and postoperative 72 h (T8). Results SBP, DBP and HR of the three groups were compared at different time points and the differences were statistically significant (all P < 0.05), while the intergroup comparison and interaction among the three groups were not statistically significant (all P > 0.05). Compared with T0 and T1, SBP, DBP and HR in the three groups were all decreased in T2 to T4. There were no statistically significant differences in RR and SpO2 at time points, intergroup comparison and interaction among the three groups (all P > 0.05). Compared with N1 group, the time of waking and leaving hospital in N2 and N3 groups were shorter and Propofol consumption were reduced, with statistically significant differences (P < 0.05 or P < 0.01). The time of waking and leaving hospital in N3 group was longer than that in N2 group, and the difference was statistically significant (P < 0.05). There were no significant differences in the other indexes (all P > 0.05). SpO2 < 90% and postoperative pain incidence in N2 and N3 groups were lower than N1 group, and the differences were highly statistically significant (all P < 0.01), while the differences in the incidence of other indicators were not statistically significant (all P > 0.05). MoCA score time points and intergroup comparison among the three groups showed statistically significant differences (all P < 0.05), while the interaction difference was not statistically significant (P > 0.05). Intra-group comparison: there were statistically significant differences among the three groups at each time point (all P < 0.05). Comparison between groups: the MoCA score of N2 and N3 groups increased at T6 and T7 compared with N1 group, and the differences were statistically significant (all P < 0.05); the MoCA score of N2 and N3 groups showed no statistically significant difference at T6 to T8 (P > 0.05). Conclusion Nalbuphine 0.08 mg/kg combined with Propofol can shorten the recovery time, waking the dosage of Propofol and reduce the occurrence of neurocognitive disorders in the elderly patients during the anesthesia of painless gastroscopy.

The definition of “heart governing exterior” means heart which governs all the skin tissues and accessory organs to maintain the physical functions, the heart is closely related to the exterior in terms of the movement trend of the air engine. Because currently it is rarely applied the theory of “heart governing exterior” to the treatment of dermatosis, this article will be focused on the treatment thoughts derived from this theory in treatment of dermatosis. The domination mechanism is based on the principle that heart belongs to yang, stores spirit and governs the blood. Therefore, dermatosis should be treated primarily from the heart with regulating the mind firstly, adjusting heart yang necessarily, clearing the blood vessels systematically and expelling pathogenic factors felicitously. Moreover, it is necessary for overall consideration and the whole treatment to prescribe herbs flexibly after considering qi activity of the other viscera and specific circumstances.

142 Effects of a new type of acupuncture and moxibustion plaster combined with rhubarb root and rhizome plaster on the treatment of functional constipation in the elderly

Objective To explore the effect of a new type of acupuncture and moxibustion plaster combined with rhubarb root and rhizome plaster on the treatment of functional constipation (FC) in the elderly. Methods A total of 120 patients with FC in the Nursing Homes in Chang′an District, Shaanxi Province from May to December 2019 were selected and divided them into four groups according to the principle of randomized control method, with 30 cases in each group. Blank group A was treated with no treatment for constipation, control group B was treated with vinegar-made rhubarb root and rhizome plaster patched on shénquē point, control group C was treated with acupuncture and moxibustion plaster patched on tiānshū point (both sides), test group D was treated with acupuncture and moxibustion plaster patched on tiānshū point (both sides) combined with vinegar-made rhubarb root and rhizome plaster patched on shénquē point, 10 d as a course of treatment. The number of bowel movements, first bowel movement time, bowel speed and stool characteristics of the four groups were compared and observed. Results After treatment, the total effective rate of defecation of patients in the test group D, control group B and control group C were higher than that of the blank group A (P < 0.01). The total effective rate of defecation in the experimental group D was higher than that in the control group B and C (P < 0.05). The first bowel movement time and bowel speed of patients in the test group D, control group B and control group C were shorter than those in the blank group A (P < 0.05); the first bowel movement time and bowel speed of patients in the test group D were shorter than those in the control group B and control group C (P < 0.05). The improvement of stool characteristics of patients in the test group D, control group B and control group C were better than that of the blank group A (P < 0.01). The improvement of stool characters in experimental group D was higher than that in control group B and C (P < 0.05). Conclusion The new type of acupuncture and moxibustion plaster combined with rhubarb root and rhizome plaster has a remarkable clinical effect on the improvement of FC among the elderly, which is simple to operate, easy to use and easy to promote.

146 Study of the characteristics of benefit population-based on patients with taking the first generation of EGFR-TKIs and drug resistance prolonged by traditional Chinese medicine

Objective To explore the characteristics of the benefit population-based on patients with taking the first generation of EGFR-TKIs and drug resistance prolonged by traditional Chinese medicine. Methods A retrospective collection of patients with non-small cell lung cancer taking EGFR-TKIs who were hospitalized in Guang′anmen Hospital of Chinese Academy of Chinese Medical Sciences from January 2006 to December 2017 and continued to receive traditional Chinese medicine in the outpatient clinic of this hospital after discharge. Progression-free survival (PFS) time was used as the main efficacy index. Results This study included 90 patients with a median PFS time of 9.6 months. Kaplan-Meier univariate survival analysis showed that PFS time was prolonged in 90 patients with the female, no smoking history, no drinking history, peripheral lung cancer, adenocarcinoma, no adrenal metastasis, no liver metastasis, no atelectasis, adverse reactions after taking EGFR-TKIs, syndrome of qi deficiency of lung and spleen and phlegm and blood stasis. Multivariate Cox regression analysis, found that adverse reactions occurred after taking EGFR-TKIs, peripheral lung cancer, no adrenal metastasis were independent protective factors (P < 0.05). Compared with other types of syndromes, syndrome of qi deficiency of lung and spleen and phlegm and blood stasis may be protective factors. Conclusion The characteristics of the benefit population-based on taking the first generation of EGFR-TKIs and drug resistance prolonged by traditional Chinese medicine may be peripheral lung cancer, no adrenal metastasis, syndrome of qi deficiency of lung and spleen and phlegm and blood stasis.

150 The value of saffron in the rehabilitation of sequela of apoplexy

Objective To explore the value of saffron in the treatment of rehabilitation of sequela of apoplexy. Methods A total of 120 patients with sequela of apoplexy who were treated at the Baoding First Central Hospital from January 2018 to December 2019 were selected as the research objects. According to the random number table method, they were divided into the conventional group and the saffron group, with 60 cases in each group. The conventional group received routine standardized treatment, and the saffron group added saffron treatment on the basis of the conventional group. After eight weeks of treatment, the treatment effects of the two groups were compared, including the National Institutes of Health Stroke Scale (NIHSS) score, activities of daily living (ADL) score, hemorheological indicators [hematocrit, erythrocyte sedimentation rate, fibrinogen, whole blood viscosity (low cut), whole blood viscosity (high cut), platelet aggregation index] and therapeutic efficacy. Results After treatment, the NIHSS scores of the two groups were significantly lower than before treatment, and the saffron group was significantly lower than that of the conventional group (P < 0.05). After treatment, the ADL scores of the two groups were significantly higher than those before treatment; and the saffron group was significantly higher than that of the conventional group (P < 0.05). After treatment, the hematocrit, erythrocyte sedimentation rate, fibrinogen, whole blood viscosity (low cut), whole blood viscosity (high cut), platelet aggregation index and other hemorheological indicators of the saffron group were significantly lower than those of the conventional group (P < 0.05). Compared with before treatment, the hematocrit, erythrocyte sedimentation rate and fibrinogen of patients in the conventional group were significantly reduced after treatment, and all hemorheological indexes in the saffron group were significantly reduced (P < 0.05). The total effective rate of the saffron group was significantly higher than that of the conventional group (P < 0.05). Conclusion Saffron can significantly improve the neurological function of patients with sequela of apoplexy, can also improve their daily living ability and down-regulate the hemorheology indicators, such as the hematocrit, erythrocyte sedimentation, fibrinogen, whole blood viscosity (low cut), whole blood viscosity (high cut), platelet aggregation index, to improve the treatment efficiency of patients, which is worthy of clinical promotion.

158 Clinical effect of Bushen Tongluo Decoction combined with filiform and fire needle therapy in the treatment of lumbar disc herniation

Objective To observe the effect of Bushen Tongluo Decoction combined with filiform and fire needle therapy in the treatment of lumbar disc herniation (LDH). Methods A total of 72 patients with LDH who were treated in the Affiliated Hospital of Inner Mongolia University for Nationalities from January to November 2019 were selected and randomly divided into treatment group and control group by random number table method, with 36 cases in each group. Patients of both groups were given conventional Western medicine treatment (nonsteroidal anti-inflammatory drugs). The treatment group was treated with Bushen Tongluo Decoction combined with filiform and fire needle therapy in the basis of conventional treatment. Patients in both groups were treated for two weeks. The Japanese orthopaedic association (JOA) score, Oswestry disability index (ODI) score, pain rating index (PRI) score, visual analog scale (VAS), pain score (PRIS) and adverse reactions of the two groups were observed before and after treatment. The therapeutic effects of the two groups were compared. Results The total effective rate of the treatment group was higher than that of the control group (P < 0.05). After treatment, the ODI, PRI, VAS, PRIS scores of the two groups were decreased, while the JOA scores were increased (all P < 0.05). After treatment, the ODI, PRI, VAS, PRIS scores in the treatment group were lower than those of the control group and JOA score was higher than that of the control group (all P < 0.05). During the treatment, there was no abnormal change of liver and kidney function, no drug-related gastrointestinal adverse reactions and other obvious discomforts. Conclusion Bushen Tongluo Decoction combined with filiform and fire needle therapy in the treatment of LDH has a positive effect, which is worthy of further study.

162 Application of serum amyloid A combined with tumor markers in the diagnosis and treatment of senile lung cancer

Objective To investigate the value of serum amyloid A (SAA) combined with neuron-specific enolase (NSE), carcinoembryonic antigen (CEA) and carbohydrate antigen (CA)125 in the diagnosis of senile lung cancer. Methods A total of 210 senile patients with lung cancer (including 119 cases of adenocarcinoma, 66 cases of squamous cell carcinoma, 25 cases of small cell carcinoma ) in Cancer Hospital of the University of Chinese Academy of Sciences from January to December 2018 were selected as lung cancer group and 120 senile patients with lung benign diseases as benign control group,and 50 healthy senile persons during the same period were selected as health control group. The levels of serum SAA, NSE,CEA and CA125 were detected, and receiver operating characteristic curve was applied to analyze the diagnostic value of each index in senile patients with lung cancer. Results The levels of SAA, NSE, CEA and CA125 in lung cancer group were higher than those in the benign control group and healthy control group (all P < 0.05). SAA level in adenocarcinoma was lower than that in squamous cell carcinoma and small cell carcinoma, and NSE level in adenocarcinoma and squamous cell carcinoma were lower than those in small cell carcinoma (all P < 0.05), while there were no significant differences in CEA and CA125 levels among the three groups (P > 0.05). SAA, NSE, CEA and CA125 levels of Ⅲ-Ⅳ period patients were higher than those of Ⅰ-Ⅱ period patients, and SAA, NSE, CEA and CA125 levels of patients with distant metastasis were higher than those of patients without distant metastasis (all P < 0.01). The AUC, sensitivity and specificity of SAA in the diagnosis of senile lung cancer were 0.569, 67.14% and 46.67% respectively; the AUC, sensitivity and specificity of NSE in the diagnosis of senile lung cancer were 0.694, 55.71% and 73.33% respectively; the AUC, sensitivity and specificity of CEA in the diagnosis of senile lung cancer were 0.583, 30.48% and 95.00% respectively; the AUC, sensitivity and specificity of CA125 in the diagnosis of senile lung cancer were 0.649, 42.38% and 85.00% respectively; and the AUC, sensitivity and specificity of combined detection in the diagnosis of senile lung cancer were 0.745, 47.62% and 96.67% respectively. Conclusion Combined detection of SAA, NSE, CEA and CA125 can significantly improve the diagnostic efficiency of elderly lung cancer.

166 Plasma metabolomics of type 2 diabetes mellitus based on GC-MS

Objective To conduct a study of type 2 diabetes mellitus (T2DM) patients and healthy people by metabolomics techniques, to explore the pathogenesis of T2DM. Methods Gas chromatography-mass spectrometry (GC-MS) combined with multiple multivariate statistical analysis methods was used to analyze plasma samples from healthy group (60 people) and the T2DM group (100 cases) in the Department of Nephrology, Jiamusi Central Hospital of Heilongjiang Province from June to July 2018. Results The method of plasma metabonomic analysis of T2DM based on GC-MS was established. Ten different metabolites such as L-alanine, lactic acid and L-valine in plasma were determined. Conclusion Based on metabolomic GC-MS analysis showed that the metabolic pattern of T2DM patients is significantly different from that of healthy group, which is related to metabolic pathways such as glucose and amino acid metabolism.

174 Effects of group cognitive behavioral therapy on sleep quality and anxiety and depression levels in insomniac breast cancer survivors during chemotherapy

Objective To investigate the effects of group cognitive behavioral therapy on sleep quality, anxiety and depression in patients with insomniac breast cancer survivors during chemotherapy. Methods From August 2018 to February 2019, 62 patients with insomniac breast cancer survivors received chemotherapy in Beijing Tiantan Hospital, Capital Medical University, random number table method was used to divide the patients into intervention group and control group, 31 cases in each group. The control group received routine nursing, and the intervention group received group cognitive behavioral therapy intervention for six weeks on the basis of the control group. The pittsburgh sleep quality index (PSQI) and hospital anxiety and depression scale scores were compared between the two groups before and after the intervention. Results Intervention group after intervention, time to fall asleep, sleep efficiency, sleep disorder, daytime dysfunction, and total score of PSQI were all lower than before intervention (P < 0.05), in addition, the four dimensions of sleep quality, time to fall asleep, sleeping time and sleeping efficiency and the total score of PSQI were significantly lower than the control group (P < 0.05). The scores of all dimensions and total scores of all dimensions of the hospital anxiety and depression  cale in intervention group were significantly lower than those before intervention and lower than those of the control group, with statistically significant differences (P < 0.05). Conclusion Group cognitive behavioral therapy is helpful to improve the sleep quality of patients with Insomniac breast cancer survivors during chemotherapy, and has a good effect on reducing their anxiety and depression.

178 Application of nursing quality sensitive indicators in perioperative safety management of patients with gastrointestinal surgery

Objective To analyze the application effect of nursing quality sensitive indicators in perioperative safety management of patients with gastrointestinal surgery. Methods From July 2017 to June 2018, 214 gastrointestinal surgery patients admitted to The Affiliated Huai′an NO.1 People Hospital of Nanjing Medical University were selected. According to the implementation of the quality of nursing indicators, the patients were divided into 104 cases before implementation (July-December 2017) and 110 cases after implementation (January-June 2018). The traditional nursing quality control method of gastrointestinal surgery was adopted before the implementation, and the nursing quality sensitive indicators was adopted after the implementation. The achievement rate of priority rectification indicators and nursing adverse events were compared before and after implementation. Results After implementation, the gastrointestinal perioperative nursing technical specification implementation rate, risk assessment correct rate, drug delivery correct rate, patient identification correct rate, gastrointestinal function rehabilitation training implementation rate, postoperative return visit rate were significantly higher than before implementation (P < 0.05 or P < 0.01). The incidence of adverse events such as nausea and vomiting, wound infection, pulmonary infection, anastomotic leakage, falling/falling in patients with gastrointestinal surgery, which was significantly lower than that of before implementation (P < 0.01). Conclusion The application of sensitive indicators of nursing quality can improve the quality of perioperative safety management of gastrointestinal surgery patients.

181 Application of multi-dimensional supportive psychological intervention in primary caregivers of elderly maintenance hemodialysis patients

Objective To investigate the effect of multi-dimensional supportive psychological intervention on the psychological flexibility and quality of life of the primary caregivers of patients with elderly maintenance hemodialysis (MHD). Methods From January 2017 to December 2018, 80 patients with MHD primary caregivers in Jiangsu Provincial Hospital were selected. According to the study time, they were divided into 42 cases in the treatment group (January-December 2018) and 38 cases in the control group (January-December 2017). The control group was given routine psychological support, while the treatment group was given multi-dimensional supportive psychological intervention. Followed for three months, primary caregivers of the two groups were compared for their psychological flexibility, quality of life, and psychological support satisfaction. Results The total scores of toughness, self-reliance, optimism, and psychological flexibility of the primary caregivers of the two groups were significantly higher than those of this group before intervention, the differences were statistically significant (P < 0.05 or P < 0.01). The total scores of toughness, self-reliance, optimism, and psychological flexibility of the primary caregivers in intervention group were significantly higher than control group, the differences were statistically significant (P < 0.05), the total scores of toughness, self-reliance, optimism, and psychological flexibility of the main caregivers of the two groups were significantly higher than those before intervention, the differences were statistically significant (P < 0.05 or P < 0.01). The total scores in the areas of physiology, psychology, social relations, and quality of life in intervention group were significantly higher than those of the control group, the differences were statistically significant(P < 0.05). The psychological support satisfaction rate in intervention group was significantly higher than the control group, the difference was statistically significant (P < 0.05). Conclusion Multi-dimensional supportive psychological intervention is helpful to enhance the level of psychological flexibility of the primary caregivers of MHD patients, improve the quality of life, and increase their psychological support satisfaction.

185 Application of closed-loop process management in the hospital transportation of severe neurosurgery patients

Objective To explore the application effect of closed-loop process management in the hospital transportation of severe neurosurgery patients, and to provide basis for clinical diagnosis and treatment. Methods A total of 100 critically severe neurosurgical patients from the Second Affiliated Hospital of Nantong University in Jiangsu Province from October 2019 to March 2020 were selected as study subjects. Random number table method was used to divide into observation group and control group, 50 cases in each group. During the transportation in the hospital, routine management measures were adopted in the control group and closed-loop process management measures were adopted in the observation group. The two groups were compared in terms of time-consuming in hospital transportation, omission rate of handover content, transportation complaint rate, nursing quality and satisfaction. Results The total transit time, inspection waiting time, and transfer time of the observation group were significantly shorter than those of the control group; the rate of omission of transfer contents and the rate of complaints of transfer in the observation group were significantly lower than those of the control group, the differences were statistcally significant (P < 0.05). The observation group′s transfer nursing, basic nursing, ward management, communication and specialty nursing scores were significantly higher than the control group, the differences were statistically significant (P < 0.05); the nursing satisfaction of the observation group was significantly higher than that of the control group, the difference was statistically significant (P < 0.05). Conclusion Closed-loop process management measures can shorten the transfer time in hospital for severe neurosurgery patients, improve work efficiency and quality of care, ensure the safety of transfer, increase satisfaction, and are recommended for clinical use.

Health industry is an important measure to implement health China strategy and an important industry for future development. First of all, this paper expounds the current situation of health industry in Hebei province, analyzes the six aspects of medical resources and services, health food, drug production and management, medical devices, traditional Chinese medicine industry and health management and promotion, and finds that there are problems in the development of health industry in Hebei Province, such as the single mode of medical service, the violation of some production and operation of medical and pharmaceutical devices, the scattered market of health management, the lack of high-end talents, and further strengthening of innovation and research and development. Specific suggestions are put forward for the above problems, including the full development of internet plus smart medicine, the improvement of the independent research and development capability of the pharmaceutical industry, and the specific measures for the training and introduction of high-end talents in the health industry, so as to provide a reference for the development of Hebei Province′s health industry.
