4 Effect of low-level vagus nerve stimulation on atrialfibrillation induced by obstructive sleep apnea in dogs
MENG Qingjun ZHANG Ling Jiasuoer·Xiaokereti WANG Xuesheng ZHOU Xianhui TANG Baopeng▲
Objective To investigate the mechanism of low-level vagus nerve stimulation (LL-VNS) in the treatment of atrial fibrillation (AF) induced by obstructive sleep apnea (OSA) in dogs. Methods Eighteen adult male healthy beagle dogs were randomly divided into three groups:control group (n = 6),OSA group (n = 6) and OSA + LL-VNS group (n = 6). All animals were intubated under general anesthesia. Both the OSA group and the LL-VNS group performed the same OSA process, which was suffocated for 2 min, ventilated for 8 min, and 10 min was a cycle lasting 6 h. The OSA + LL-VNS group was given LL-VNS from the third hour to the end of the experiment and the control group was not suffocated. The changes of blood gas indexes at 0, 1, 3 h, and 6 h, and inflammatory markers including tumor necrosis factor-α (TNF-α), interleukin-2 (IL-2) and interleukin-6 (IL-6) at 0, 3 h and 6 h after modeling were detected. The canine arterial blood pressure was recorded throughout the whole experiment. At the end of the experiment, the atrial muscle tissue was taken for hematoxylin-eosin (HE) staining. Results OSA model was successfully established (P < 0.05). The systolic blood pressure of OSA group and OSA + LL-VNS group had significantly changed before and after OSA (P < 0.05), but there was no significant difference between the two groups (P > 0.05). Inflammation indicators-TNF-α, IL-2 at 6 hours in OSA group were significantly higher than the control group, but in OSA + LL-VNS group, it was significantly lower than the control group, the differences were statistically significant (P < 0.05). IL-6 showed no significant change in each time period (P > 0.05). In OSA group the atrial relative refractory period (ERP) was gradually shortened, and AF duration was gradually prolonged (P < 0.05). In OSA + LL-VNS group, from 0 h to 3 h the atrial ERP was gradually shortened and the duration of AF was gradually prolonged (P < 0.05); from 3 h to 6 h after given LL-VNS, the atrial ERP was gradually extended and AF duration reduced compared with 0 h to 3 h (P < 0.05). Compared with the control group, the HE slices of the atrial muscle in the OSA group showed that the atrial myocytes were disordered and the cell gap widened and After LL-VNS intervention, the above changes were reversed to some extent. Conclusion Inflammatory factors are positively correlated with the genesis and development of AF, and LL-VNS can reduce the incidence of AF. Therefore, reducing the body's inflammatory response may be one of the mechanisms by which LL-VNS reduces AF inducibility.

22 Research progress of pathogenic mechanism of periodontitis and active oxygen
FANG Hui LIU QI
Recent studies suggest that the direct and indirect participation of oxidative stress is also a key factor in the destruction of periodontal tissue. Reactive oxygen species (ROS) is a product of oxidative stress in the body. It has become a hot research topic in the pathogenesis of periodontitis in recent years. Physiological levels of ROS can participate in physiological activities such as homeostasis, signal transduction and apoptosis in cells as second messengers. Excessive ROS play a cytotoxic role, causing oxidative damage to proteins , lipids and DNA, interfering with cell growth and cell cycle progression , and induce gingival fibroblast apoptosis. These effects of ROS together cause irreversible and direct damage to the periodontal tissue. In addition, ROS can also indirectly cause serious damage to periodontal tissues by activating inflammatory factors, nuclear factor κB (NF-κB), c-Jun N-terminal kinases and autophagy. This article summarizes the relationship between the excessive production of ROS and its activated signal transduction pathway and periodontitis, so as to provide evidence for further exploration of the correlation between ROS and periodontitis.

YAN Xuemei1 HAN Yan2 MA Lijuan1 SU Jianping3
Objective To investigate the relationship between symptom distress and quality of life in patients with thoracic and laparoscopic radical resection of esophageal carcinoma. Methods From December 2016 to April 2017, 94 patients with esophageal cancer treated by thoracic laparoscopy combined with radical resection of esophageal carcinoma were investigated by convenient sampling method, and the status of symptom distress and the quality of life score were analyzed. Pearson correlation analysis was used to analyze the correlation between symptom distress and quality of life after radical resection of esophageal carcinoma. Results The most plagued for patients with thoracic laparoscopy combined with radical esophagectomy was the concern for the prospects, the rest in turn were the appearance changes, fatigue, pain, insomnia, loss of appetite and so on. The score of cognitive functional dimension was the highest and the somatic functional status were the lowest in all functional areas of postoperative quality of life. There were no significant differences in the scores of cognitive function, emotional function, diarrhea when compared with Norwegian norm in symptom domains and single item (P > 0.05);the scores of physical function, role function, social function, general health status, constipation and pain were lower than the norm, with statistically significant differences (P < 0.05); Shortness of breath, loss of appetite, fatigue, nausea and vomiting, insomnia, economic difficulties were higher than the norm (P < 0.05). Pearson correlation analysis showed that the scores of symptom distress were negatively correlated with the overall health status, physical function, emotional function, social function, cognitive function and role function of quality of life (P < 0.001). Conclusion After radical resection of esophageal cancer, the symptoms of the patients are common and suffer from multiple symptoms, the quality of life is at a low level. Nursing staff should pay attention to the evaluation and management of patients' symptoms, pay attention to the outcome of nursing care, so as to improve the quality of life in patients with esophageal cancer after operation.

Objective To investigate the body temperature protective effect of inflatable warming blanket during transurethral prostatectomy (TURP) and its influence on the coagulation function of the patients. Methods From February 2016 to July 2017, 126 patients with benign prostatic hyperplasia in Changhai Hospital were selected and divided into the control group and the observation group, with 63 cases in each group. The control group was given conventional body temperature intervention, while the observation group was given inflatable warming blanket on the bases of control group. The perioperation body temperature, coagulation function before and after surgery and the incidence of complications were compared between the two groups. Results The one-way analysis of variance showed that there were statistically significant differences in the two groups before and after different time points of anesthesia (F=46.24, 92.86, P < 0.05). There were no statistically significant differences before and after anesthesia of 15 min between the two groups (P > 0.05). After anesthesia of 30, 45, 60 min and at the end of operation, the temperature in the observation group were higher than those of control group, with statistically significant differences (P < 0.05). There were no statistically significant differences of prothrombin time, activated partial thrombin time and fibrinogen in the observation group (P > 0.05), but the indicators above in the control group after the operation were higher than those of before operation, with statistically significant differences (P < 0.05). The incidences of hypothermia, shiver and cystospasm in the observation group were lower than those of control group, with statistically significant differences (P < 0.05). Conclusion The application of inflatable warming blanket during TURP has a good body temperature protection effect, which can avoid the coagulation dysfunction caused by hypothermia.

CHEN Xiaolin ZHANG Zuowen▲
Objective To investigate the clinical effect of Atorvastatin in the treatment of fracture patients with unstable carotid plaque occurred ischemic stroke in perioperative. Methods From January 2015 to January 2017, 86 fracture patients combined with carotid artery plaques occurred ischemic stroke in perioperative in Jiangjin District Central Hospital were selected. They were divided into study group (43 cases) and observation group (43 cases) according to the therapeutic schedule. They were treated by the routine treatment, such as anticoagulation, improved microcirculation and neuroprotective function, and the patients in the study group were treated with Atorvastatin based on those treatments. All the patients were treated for 6 months. Before and after treatment, the levels of total cholesterol (TC), triglyceride (TG), high density lipoprotein cholesterol (HDL-C) and low density lipoprotein cholesterol (LDL-C), carotid intima-media thickness (IMT) and carotid artery plaque were detected. The nerve function damage of patients was evaluated by neurological deficit score (NIHSS). Then the transient ischemic attack (TIA), recurrent stroke and adverse reactions of the two groups were compared. Results After treatment, the levels of TC, TG, LDL-C, IMT, maximum diameter, plaque area, and NIHSS scores in the study group were lower than those in the control group, while the level of HDL-C was higher than that in control group, with statistically significant difference (P < 0.05). The incidence of TIA and ischemic stroke recurrence rate in the study group were lower than those in the control group (P < 0.05). However, there was no significant difference in the incidence of adverse reactions between the two groups (P > 0.05). Conclusion The effect of Atorvastatin in fracture patients combined with carotid artery plaques occurred ischemic stroke in perioperative is significant. Atorvastatin can obviously improve the patient's blood lipid levels and carotid artery plaque instability state, promote nerve functional recovery, reduce the incidence of cerebral ischemic events, and has a certain safety, so it is worthy of clinical application.
