5 Analysis of clinical trial registration status of male infertility in China

Objective To explore the clinical trial registration status of male infertility in China. Methods From the establishment of Chinese Clinicle Trail Registry and National Library of Medicine Clinical Trial Registry database to October 2022, all registered clinical trials for male infertility in China were searched. Registration studies were screened according to the exclusion criteria, and information such as registration time, registration location, intervention measures, outcome indicators, and study design were extracted. Descriptive statistical methods were used for the analysis. Results A total of 96 studies were retrieved, 31 of which were eventually included, with a total sample size of 37 507 cases. Since 2018, the number of registrations had been increasing year by year. The main research methods were intervention studies (21 studies 67.74%), and the most research designs were randomized parallel controls (18 studies, 58.06%). The geographical distribution of sponsoring units was mainly in Zhengzhou, Beijing, Nanjing, Guangzhou, and Shanghai, and the nature of sponsoring units was mainly in hospitals and reproductive centers; funding mainly came from self-financing of hospitals (18 studies, 58.06%); the intervention measures mainly included drugs and non-drugs, among which the drugs were mainly proprietary Chinese medicines (7 studies, 33.33%), and the non-drugs included assisted reproduction and surgery. The outcome indexes were mainly semen quality, sex hormone level, and pregnancy rate. Conclusion Chinese medicine intervention measures include in the study accounted for a relatively high proportion and significant characteristics, mainly proprietary Chinese medicines with the effect of tonifying kidney and essence. Clinical trial registration in this field still has problems such as fewer registrations, uneven geographical distribution, and registration information not updated in time.

10 Analysis of depressive symptoms and influencing factors in elderly people in China

Objective To explore the depressive symptoms and influencing factors of the elderly in China. Methods The study used data from the 8th round of the Chinese longitudinal healthy longevity survey (CLHLS) conducted in 2017—2018. CLHLS adopted multi-stage stratified sampling method. After screening the data according to inclusion and exclusion criteria, 6 318 elderly people were finally included as study subjects. The study was conducted from five dimensions: dem- ographic characteristics, social support, lifestyle, health status, and economic status. Logistic regression was used to analyze the influencing factors of depressive symptoms in elderly people in China. Results There were statistically significant differences in gender, body mass index(BMI), education level, smoking, drinking, exercise, marital status, residence status, annual income, health status, and place of residence among elderly people with different depressive symptoms (P < 0.05). Logistic regression analysis showed that BMI<18.5 kg/m2 (OR=1.39, 95%CI: [1.15, 1.69]), widowed (OR=1.33, 95%CI: [1.06, 1.65]), and living alone (OR=1.35, 95%CI: [1.10, 1.66]) were risk factors for depressive symptoms in elderly people (P<0.05). Exercise (OR=0.65, 95%CI: [0.53, 0.80]), good health (OR=0.22, 95%CI: 0.18, 0.27]), and affluence (OR=0.79, 95%CI: [0.64, 0.98]) were the protective factors (P<0.05). Conclusion The likelihood of depressive symptoms in elderly people who are thin, widowed, and living alone is higher than that in elderly people who are of normal weight, married and living with their spouse, and living with their family. Exercise, good health, and affluence are less likely to cause depressive symptoms in elderly people than those who do not exercise, have poor health, and are very poor.

14 Predictive value of CTA combined with CTP in the treatment and rehabilitation of patients with unilateral middle cerebral artery infarction

Objective To investigate the predictive value of CT angiography (CTA) combined with CT perfusion imaging (CTP) in the treatment and rehabilitation of patients with unilateral middle cerebral artery infarction. Methods A total of 89 patients with unilateral middle cerebral artery infarction who were treated in Suzhou Integrated Traditional and Western Medicine Hospital from January 2020 to October 2022 were selected, all patients were given routine treatment, follow-up to 90 days after treatment, the clinical data, CTA and CTP parameters of patients with effective (65 cases) and ineffective (24 cases) treatment, good prognosis (57 cases) and bad prognosis (32 cases) were analyzed and compared. Results There were significant differences in collateral circulation and relative cerebral volume (rCBV) between effective and ineffective patients (P<0.05). The area under receiver operator characteristic(ROC) curves for collateral circulation (poor =1, good =0), rCBV and combined prediction of treatment failure were 0.780, 0.724, and 0.745, respectively (P<0.05). There were significant differences in collateral circulation, rCBV, and relative mean transmit time between good and bad prognosis patients (P<0.05). The area under ROC curve of collateral circulation, rCBV and combined prediction of poor prognosis were 0.836, 0.767 and 0.778, respectively (P<0.05). The area under ROC curve of relative mean transmit time for predicting poor prognosis was 0.620 (P>0.05). Conclusion CTA combined with CTP has a certain predictive value in the treatment effect and prognosis of patients with unilateral middle cerebral artery infarction, which is worth further study.

19 Effect of ApoE and SLCO1B1 gene polymorphisms on lipid-lowering therapy in patients with hyperlipidemia

Objective To explore the polymorphisms of ApoE and SLCO1B1 genes and their effects on the lipid-lowering effect of statins in patients with hyperlipidemia. Methods A total of 247 patients with hyperlipidemia found during routine physical examination in Northern Jiangsu People’s Hospital, Jiangsu Province, from January 2020 to January 2022 were selected, and the polymorphisms of ApoE and SLCO1B1 gene loci of the patients were detected by PCR-pyrosequencing method, and the Hardy-Weinberg law was analyzed for genotype typing. The distribution characteristics of different genotypes in sex were compared. The patients were classified according to ApoE and SLCO1B1 gene polymorphisms, and the differences in blood lipid indexes of patients with different genotypes were compared before and after taking Statins for three months. Results The observed ApoE and SLCO1B1 gene loci in 247 patients with hyperlipidemia were consistent with Hardy-Weinberg law (P>0.05). The ApoE alleles of the patients were E2 (133 cases), E3 (173 cases), and E4 (41 cases), involving six different genotypes, namely E2/E2, E2/E3, E2/E4, E3/E3, E3/E4, and E4/E4, and there was no statistical significance in the gender distribution of different genotypes (P>0.05). SLCO1B1 gene phenotypes were detected in three classes (six genotypes), including Class Ⅰ (*1a/*1a, *1a/*1b, *1b/*1b, 185 cases), Ⅱ (*1a/*15, *1b/*15, 58 cases), and Ⅲ (*15/*15, 4 cases) genotypes. The gender distribution of SLCO1B1 genotype patients was compared, and the difference was statistically significant (P<0.05). After three months of treatment, total cholesterol (TC), triglyceride (TG), and low-density lipoprotein cholesterol (LDL-C) in ApoE2, E3, and E4 patients were lower than those before treatment, the differences were statistically significant (P<0.05). After three months of treatment, TC, TG, and LDL-C in SLCO1B1 genotype ClassⅠ, Ⅱ, and Ⅲ patients were lower than those before treatment, and the differences were statistically significant (P<0.05). Conclusion The gene frequencies of ApoE and SLCO1B1 are regularly distributed, and there are sex differences in the SLCO1B1 genotype. The polymorphism of ApoE and SLCO1B1 genes may have no effect on statin therapy in patients with hyperlipidemia.

24 Effect of Shenqi Zhilong Decoction on expression of FcγRⅠ, FcγRⅡB, and FcγRⅢA in renal tissue of membranous nephropathy model mice

Objective To explore the effect of Shenqi Zhilong Decoction on expression of FcγRⅠ, FcγRⅡB, and FcγRⅢA in renal tissues of membranous nephropathy (MN) model mice. Methods Sixty SPF grade ICR mice, six-week old and weig- hting (23±4) g, half male and half female, were divided into blank group (10 mice) and model group (50 mice). The model group mice were injected with cationic-bovine serum albumin to replicate MN mouse model. The 40 mice successfully modeled were divided into model group, Shenqilong Decoction high-, low-dose group (24, 12 g/kg) and Tripterygium Glycoside Tablets group (14 mg/kg) according to the random number table method, with 10 mice in each group. After four weeks of administration, the mice were killed. HE and Masson staining were used to observe the pathological changes of renal tissues; immunofluorescence method was used to observe the protein fluorescence intensity of FcγRⅠ, FcγRⅡB, and FcγRⅢA in renal tissues; Western blot was used to detect the protein expression of FcγRⅠ, FcγRⅡB, and FcγRⅢA in renal tissues; the mRNA expression of FcγRⅠ, FcγRⅡB, and FcγRⅢA in renal tissues was detected by RT-PCR. Results HE staining showed no significant abnormalities in the morphology and structure of the epithelial cells, basement membrane, renal tubules, and renal interstitium in blank group. In model group, the glomerular volume increased, the glomerular capillary loop dilated, blood cell overflow, the mesangial thickened, the cystic cavity enlarged, and the skin on the wall of the cystic cavity was severely damaged. Compared with model group, the glomerular volume decreased, the basement membrane thickened and the skin on the wall of the renal capsule was not damaged or partially damaged in Shenqi Zhilong Decoction high-dose group. Masson staining showed that the glomerular basement membrane was normal in the blank group. In model group, there was a large amount of subepithelial photophilin deposition, basal membrane matrix reaction, vacuol-like steatosis of renal tubules, and a large amount of renal interstitial lymphatic hyperplasia. Compared with model group, the lesion of each administration group was reduced, and the improvement was more obvious in Shenqi Zhilong Decoction high-dose group. Compared with blank group, the protein fluorescence of FcγRⅠ and FcγRⅢA in model group was enhanced, while the protein fluorescence of FcγRⅡB was weakened (P<0.05); compared with model group, the protein fluorescence of FcγRⅠ and FcγRⅢA in Shenqi Zhilong Decoction high-dose group was decreased (P<0.05), while the protein fluorescence of FcγRⅡB in Shenqi Zhilong Decoction high- and low-dose group was increased (P<0.05); compared with Shenqi Zhilong Decoction low-dose group, the protein fluorescence of FcγRⅠ and FcγRⅢA in Shenqi Zhilong Decoction high-dose group was decreased, and the protein fluorescence of FcγRⅡB was increased (P<0.05). Compared with blank group, the protein expressions of FcγRⅠ and FcγRⅢA were increased in model group, while the protein expression of FcγRⅡB was decreased (P<0.05); compared with model group, the protein expressions of FcγRⅠ and FcγRⅢA in Shenqi Zhilong Decoction high-dose group were decreased, while the protein expressions of FcγRⅡB in Shenqi Zhilong Decoction high- and low-dose groups were increased (P<0.05); compared with Shenqi Zhilong Decoction low-dose group, the protein expression of FcγRⅡB in Shenqi Zhilong Decoction high-dose group was increased, and the protein expression of FcγRⅢA was decreased (P<0.05). Compared with blank group, the mRNA expressions of FcγRⅠ and FcγRⅢA in model group were increased, while the mRNA expressions of FcγRⅡB was decreased (P<0.05); compared with model group, the mRNA expressions of FcγRⅠ and FcγRⅢA in Shenqi Zhilong Decoction high- and low-dose groups were decreased, and the mRNA expression of FcγRⅡB was increased (P<0.05). compared with Shenqi Zhilong Decoction low-dose group, the mRNA expressions of FcγRⅠ and FcγRⅢA in Shenqi Zhilong Decoction high-dose group were decreased, while the mRNA expression of FcγRⅡB were increased (P<0.05). Conclusion The therapeutic effect of Shenqi Zhilong Decoction on MN mice is related to improving the pathological changes of renal tissues, reducing the expressions of FcγRⅠ and FcγRⅢA, and increasing the protein expression of FcγRⅡB, and the effect of Shenqi Zhilong Decoction with high concentration is better.

31 Expression of JAG2, INHBA, and PD-L1 proteins in elderly colorectal cancer tissues and their relationship with prognosis

Objective To investigate the relationship between the expression of Jagged 2 protein (JAG2), inhibin βA (INHBA), programmed cell death-ligand 1 (PD-L1) and prognosis in elderly patients with colorectal cancer. Methods A total of 112 elderly patients with colorectal cancer who received surgical treatment in Hunan Cancer Hospital from May 2017 to May 2019 were selected as the study objects. JAG2, INHBA, and PD-L1 expression in cancer tissues and adjacent tissues were detected by immunohistochemical method. The survival time of patients was followed up, and the relationship between JAG2, INHBA, PD-L1 expression and prognosis of elderly patients with colorectal cancer was analyzed by Cox regression model.Results The positive expression rates of JAG2, INHBA, and PD-L1 in cancer tissues were higher than those in adjacent tissues, and the differences were statistically significant (P<0.05). The positive expression rates of JAG2, INHBA, and PD-L1 in poorly differentiated patients were higher than those in middle and highly differentiated patients, and the positive expression rates of JAG2, INHBA, and PD-L1 in patients with TNM stage Ⅲ-Ⅳ were higher than those in patients with stage Ⅰ-Ⅱ (P<0.05). Cox regression model showed that low tumor differentiation, TNM stage Ⅲ-Ⅳ, JAG2, INHBA, and PD-L1 expression were independent risk factors for progression-free survival in elderly patients with colorectal cancer (OR>1, P<0.05). The overall survival rate of JAG2, INHBA, and PD-L1 negative patients with colorectal cancer were higher than those of JAG2, INHBA, and PD-L1 positive patients, and the differences were statistically significant (P<0.05). The progression-free survival rate of JAG2, INHBA, and PD-L1 negative patients were higher than those of JAG2 positive patients, and the differences were statistically significant (P<0.05). Conclusion The positive expression rates of JAG2, INHBA, and PD-L1 in cancer tissues of elderly patients with colorectal cancer are high, which is of great significance for prognosis evaluation.

37 Research progress of CC chemokine receptor 9/CC chemokine ligand 25 in related diseases

Chemokine is a structure-related protein with a relatively small molecular weight, which can target cells to chemotaxis and promote inflammatory response. CC chemokine receptor 9 (CCR9) and CC chemokine ligand 25 (CCL25) are involved in the regulating the occurrence and development of various diseases. Early research have confirmed various physiological functions including migration and maturation of inflammatory cells. Subsequent studies have confirmed that CCR9/CCL25 plays an important role in the pathological processes of various inflammatory and non inflammatory diseases such as inflammatory bowel disease, rheumatoid arthritis, cardiovascular disease, tumors, and other diseases. Therefore, this review provides an overview of the latest progress of CCR9/CCL25 in various diseases, which is beneficial for in-depth research on CCR9/CCL25. CCR9/CCL25 is expected to become a potential target for drug therapy.

41 Research progress on comorbid sleep disorders in children with tic disorders

Tic disorders (TD) and sleep disorders are common chronic diseases in children and adolescents. However, there are few reports on the comorbidity of TD and sleep disorders in China, and the pathogenesis of comorbidity is unknown. In general, sleep problems such as insomnia, abnormal sleep, drowsiness, sleep-related dyskinesia, and respiratory disorders are more common in children with TD. Reduced sleep time, reduced sleep efficiency, and decreased percentage of rapid eye movement sleep are significant characteristics of sleep structure in children with TD. At present, the comorbidity of sleep disorders in children with TD is considered to be related to neurobiology, genetics, family, psychological and social, immune, and other factors. At the same time, the treatment of TD and other comorbidities may also increase the risk of comorbidity of sleep disorders in children with TD, so clinical attention should be paid.

45 Relationship between general anesthetics and brain development and autism spectrum disorders in children and its clinical prospect

Autism spectrum disorder is a type of brain neurodevelopmental disorder that occurs frequently in children, mainly in childhood. Its mechanism of occurrence is not yet fully elucidated, and the disease lasts for a lifetime, which is difficult to reverse and causes a huge burden on families and society. Therefore, it has gradually become a hot topic in research in social psychology, clinical medicine, and other fields. Studies have shown that multiple or long-term use of general anesthesia and anesthetic analgesics may affect the brain development of infants and young children, leading to long-term autism like behavior. However, the exact mechanism is still unclear. This article reviews the potential mechanisms of childhood autism spectrum disorders, explores the relationship between general anesthesia related drugs and childhood autism, and its clinical significance. The aim is to provide new ideas for studying the mechanism of brain development related damage caused by general anesthesia and clinical treatment.

50 Discussion on the immunological strategies of traditional Chinese medicine for ulcerative colitis based on the theory of “spleen acting as the defense for host”

The theory of “spleen acting as the defense for host” believes that the spleen is the source of qi and blood biochemistry, transforming defensive qi, which can resist external evil and protect the body, which is mainly related to the physiological functions of “spleen governing transportation and transformation”, “spleen governing ascending and clearing”. The abnormal physiological function of the spleen is manifested by “spleen losing its duty as the defense”, and all diseases are born. In the development of ulcerative colitis, the breakdown of intestinal mucosal immune barrier plays a key role, and the disturbance of immune cell balance leads to inflammatory damage. It can be seen that the function of immune cells is closely related to the distribution and movement of defensive qi, and the mechanism of intestinal mucosal immune barrier is coupled with the connotation of the theory of “spleen acting as the defense for host”. This paper traces the theoretical origin of “spleen acting as the defense for host”, and combines with the immunological mechanism in modern medicine to explore the new idea of traditional Chinese medicine in the treatment of ulcerative colitis, which is helpful for traditional Chinese medicine to play a greater role in the immunotherapy of ulcerative colitis.

54 Research progress on risk factors and preventive measures of bone cement leakage after vertebroplasty

Osteoporotic and pathological fractures can be treated by vertebral augmentation, these techniques are widely applied in clinical practice. However, bone cement leakage during operation is the most common complication, which may lead to vascular embolism, nerve injury, and refracture of adjacent vertebrae. This article reviews the risk factors of bone cement leakage, including anatomy, fracture, surgery, bone cement, disease factors, and common preventive measures, such as optimal surgical methods and the use of novel materials. It also identifies the gaps and limitations in the existing research and suggests future research directions, aiming to provide guidance for clinical work and enhance surgical safety.

58 Research progress on pathogenesis of rheumatoid arthritis combined with interstitial lung disease

Rheumatoid arthritis combined with interstitial lung disease has a high incidence and mortality. Clinical research on its pathogenesis is of great significance, which can provide evidence for the targeted treatment of the disease, effectively control the disease early, improve the quality of life of patients, and reduce the mortality. In addition, research on the risk factors involve in the disease can provide evidence for the prevention of high-risk groups, early prevention, and diagnosis. It is of great significance to improve the prognosis of the disease. This paper mainly reviewes the environmental level of smoking population susceptibility, microbial pathogens, gene level of genetic susceptibility, gene variation, and molecular level of autoantibodies, immune cells and cytokines, and elaborated on the pathogenesis of various factors involved in rheumatoid arthritis complicated with interstitial lung disease.

62 Sinicization and validation of clinical stress perception scale for nursing interns

Objective To Chinesize the clinical stress perception scale (NSPCSS) of nursing interns and test its reliability and validity. Methods The English version of NSPCSS was translated, back-translated and cross-cultural debugging to form the Chinese version of NSPCSS. A questionnaire survey was conducted among 363 nursing interns in the First Affiliated Hospital of Anhui Medical University from March to May 2023 to test the reliability and validity of the Chinese version of NSPCSS. Results The Chinese version of NSPCSS included six dimensions, including limited clinical ability, insufficient clinical environment, insufficient knowledge and skills, inefficient clinical teaching planning, improper teaching behavior, and nursing concerns. The overall Cronbach’s α coefficient, retest reliability, and broken half reliability of the scale were 0.945, 0.941, and 0.901, respectively. I-CVI was 0.833 - 1.000, S-CVI was 0.983; the Chinese version of the perceived stress scale was used as the criterion variable, and the criterion association validity coefficient was 0.632 (P< 0.05). Six common factors were extracted by exploratory factor analysis, and the cumulative variance contribution rate was 71.424%. Conclusion The Chinese version of NSPCSS has good reliability and validity, and can be used to evaluate the stress perception of nursing interns in China during clinical practice.

66 Analysis of the occurrence and influencing factors of post-traumatic growth in patients with leukemia

Objective To investigate the post-traumatic growth (PTG) of leukemia patients and its influencing factors. Methods A total of 240 leukemia patients admitted to Dongguan People’s Hospital in Guangdong Province from June 2019 to December 2022 were evaluated with the PTG rating scale, perceptive social support scale (PSSS), general self-efficacy scale (GSES), and the Chinese version of the event-related ruminative meditation inventor (C-ERRI). The patients were divided into PTG group (57-105 scores, 132 cases) and non-PTG group (0-56 scores, 108 cases) according to the PTG assessment results. General data, disease-related conditions, PSSS score, GSES score, and C-ERRI score were compared between the two groups. Logistic regression analysis was used to determine the influencing factors of PTG in leukemia patients. Results The total PTG scores of 240 patients ranged from 20 to 89, with an average of (59.63±7.10) points. There were statistically significant differences in education level, family per capita monthly income, disease status, pain, disease course, PSSS score, GSES score, and C-ERRI score between the two groups (P <0.05). Logistic regression analysis showed that family per capita monthly income (OR=1.618, 95%CI:1.107-2.365), disease status (OR=1.728, 95%CI: 1.105-2.702), pain (OR=0.352, 95%CI:0.153-0.810), PSSS score (OR=2.221, 95%CI:1.327-3.717), GSES score (OR=1.969, 95%CI:1.215-3.191), C-ERRI score (OR=1.724, 95%CI:1.141-2.605) were the influencing factors of PTG in leukemia patients (P<0.05). Conclusion PTG can be promoted by effective intervention measures to improve patients’ social support, self-efficacy and rumination.

70 Analysis of clinical features of measles complicated with pneumonia in children

Objective To explore the clinical characteristics of measles complicated with pneumonia in children and to provide reference for the diagnosis and treatment of children with measles. Methods A retrospective study was conducted to collect the clinical data, including demographic data, measles infection, and laboratory results, of 0-18 year old children with measles who were first diagnosed with measles in Beijing Youan Hospital, Capital Medical University from June 2010 to June 2022. The children were divided into pneumonia group and non-pneumonia group according to whether they were complicated with pneumonia. The clinical data were analyzed and summarized. Results Among 671 children with measles, a total of 601 cases were complicated with pneumonia, most of them were male, and few complications were complicated. The clinical manifestations were fever, cough, and rash. The age of onset in the pneumonia group was lower than that in the non-pneumonia group, and the proportion of previous history was higher than that in the non-pneumonia group, and the differences were statistically significant (P<0.05). There were significant differences in the proportion of cough, Collinia spot, total complication rate, and gastroenteritis between the two groups (P<0.05). The median indexes of white blood cell count, neutrophil count, C-reactive protein, and creatinine in pneumonia group were higher than those in non-pneumonia group, and the differences were statistically significant (P<0.05). The proportion of antibiotics used in pneumonia group was higher than that in non-pneumonia group, and the difference was statistically significant (P<0.05). The proportion of antiviral drugs used in pneumonia group was lower than that in non-pneumonia group, and the difference was statistically significant (P<0.05). The hospitalization days in the pneumonia group were longer than that in the non-pneumonia group, and the difference was statistically significant (P<0.05). Conclusion Pneumonia is the main complication of measles, and the clinical manifestations of children with measles comp- licated with pneumonia are more serious than those without measles complicated with pneumonia. Understanding the clinical manifestations and laboratory examination results of measles complicated with pneumonia is helpful to judge the prognosis and provide guidance for correct treatment.

74 Application value of heart fatty acid binding protein combined with high-sensitivity cardiac troponin I in rapid diagnosis of acute myocardial infarction

Objective To explore the value of heart fatty acid binding protein (H-FABP) combined with high-sensitivity cardiac troponin I (hs-cTnI) in rapid diagnosis of acute myocardial infarction. Methods A total of 105 suspected AMI patients with chest pain as the first symptom and 200 normal reference population admitted to the First People’s Hospital of Huzhou, Zhejiang Province from March 2021 to March 2022 were selected. Serum H-FABP and hs-cTnI levels were measured immediately after admission (0 h) and 1 h after admission, and the cut-off values of H-FABP and hs-cTnI were determined. The rapid triage strategy of H-FABP combined with hs-cTnI was formulated and the diagnostic efficacy of H-FABP combined with hs-cTnI for AMI was analyzed. Results The cut-off values of serum H-FABP and hs-cTnI were 6.30 ng/ml and 10.65 pg/ml, respectively. Serum H-FABP and hs-cTnI levels in patients with suspected AMI at 0 h and 1 h after admission were higher than those in normal reference population, and the differences were statistically significant (P<0.05). Serum H-FABP and hs-cTnI levels of patients with suspected AMI 1 h after admission were higher than those of 0 h after admission, and the differences were statistically significant (P<0.05). According to the H-FABP combined hs-cTnI rapid triage process, 45 cases (42.86%) were included in the AMI exclusion group, 50 cases (47.62%) in the AMI inclusion group, and 10 cases (9.52%) in the observation group. The sensitivity, specificity, positive predictive value, and negative predictive value of rapid triage for AMI were 100.00%, 88.71%, 86.00%, and 100.00% respectively. The area under the curve of H-FABP+hs-cTnI in the diagnosis of AMI 1 h after adm- ission were higher than those 0 h after admission, and the differences were statistically significant (P<0.05). Conclusion The changes of serum H-FABP and hs-cTnI levels at 0 h and 1 h after admission can rapidly diagnose or exclude most AMI, which has positive significance for realizing rapid triage of AMI and reducing the mortality and misdiagnosis rate of patients.

79 Analysis of clinical features and risk factors of re-admission to ICU after cardiac surgery

Objective To summarize the clinical characteristics of patients re-admission to the intensive care unit (ICU) after cardiac surgery and to evaluate the risk factors for re-admission to the ICU. Methods A total of 2 722 patients admitted to the Department of Cardiac Surgery in Peking Union Medical College Hospital from January 2016 to December 2022 were admitted to ICU after open heart surgery, 2 102 patients were admitted to ICU after surgery, of which 2 083 patients were admitted to ICU only once and 19 patients were re-admitted to ICU. The clinical characteristics of re-admission to ICU were analyzed, and 57 patients who were admitted to ICU only once were selected as the control group according to the ratio of 1∶3 and were similar in age, gender, and admission time to the study group. The risk factors of re-admission to ICU were explored by conditional logistic regression. Results Respiratory failure was the most important reason for re-admission to ICU after cardiac surgery. There were significant differences in cardiac function grade, left ventricular ejection fraction, cardiopulmonary bypass time, first ICU stay time, and continuous renal replacement therapy between the two groups (P<0.05). Univariate conditional logistic analysis showed that preoperative cardiac function grade, left ventricular ejection fraction, intraoperative CPB time, first ICU stay time, continuous renal replacement therapy were related to the occurrence of postoperative ICU re-admission (P<0.05). Multivariate logistic regression showed that CRRT (OR:12.986, 95%CI:2.839-59.395, P=0.001) was an independent risk factor for postoperative re-admission to ICU. Conclusion CRRT after open heart surgery is a risk factor for re-admission to ICU after open heart surgery, which can help doctors identify high-risk patients in time and promote diagnosis and treatment.

84 Short-term efficacy of Shexiang Baoxin Pills combined with thrombus aspiration on patients with acute ST-elevation myocardial infarction after primary percutaneous coronary intervention

Objective To explore the short-term efficacy of Shexiang Baoxin Pills combined with thrombus aspiration on patients with acute ST-elevation myocardial infarction (STEMI) after primary percutaneous coronary intervention (PCI). Methods A total of 144 patients with acute STEMI who accepted primary PCI in Nanchang First Hospital, Jiangxi Province from April 2020 to December 2022 were selected. They were divided into group A (72 cases) and group B (72 cases) by random number table method. Patients in group B were treated with thrombus aspiration, and patients in group A were treated with Shexiang Baoxin Pills combined with thrombus aspiration(taking Shexiang Baoxin Pills 90 mg sublingually before operation and taking Shexiang Baoxin Pills 45 mg/time, three times a day after operation). The proportion of postoperative thrombolysis in myocardial infarction (TIMI) blood flow grade three immediately after primary PCI and the proportion of ST segment regression>50% at two hours after PCI were compared between two groups; left ventricular ejection fraction (LVEF) and left ventricular end diastolic diameter (LVEDd) were compared between two groups before and one month after operation; and the incidence of major adverse cardiovascular events (MACE) in hospital and within 30 days after hospitalization was compared between two groups. Results The proportion of postoperative TIMI blood flow grade three immediately after primary PCI and the proportion of ST segment regression>50% at two hours after PCI in group A were higher than those in group B, and the differences were statistically significant (P < 0.05). One month after operation, LVEF was higher than that before operation, LVEDd was lower than that before operation; and LVEF in group A was higher than that in group B, LVEDd was lower than that in group B, and the differences were statistically significant (P <0.05). The total incidence of MACE in hospital and within 30 days after hospitalization in group A was lower than that in group B, and the difference was statistically significant (P <0.05). Conclusion In the primary PCI treatment of acute myocardial infarction, the use of Shexiang Baoxin Pills combined with thrombus aspiration can effectively improve myocardial perfusion and cardiac function, and reduce the incidence of recent MACE.

88 Diagnostic value of serum CD40 ligand and QTc interphase level in postoperative arrhythmia of coronary heart disease

Objective To explore the diagnostic value of serum CD40 ligand (CD40L) combined with QTc interphase level of ambulatory electrocar-diogram in postoperative arrhythmia of coronary heart disease. Methods A total of 89 patients with coronary heart disease admitted to Suzhou Hospital of Integrated Traditional Chinese and Western Medicine from January 2020 to January 2022 were selected. Clinical medical records of the patients were collected and collated, and ambulatory electrocar-diogram and CD40L levels were detected after admission. According to the occurrence of arrhythmia within one month after percutaneous coronary intervention, the patients were divided into occurrence group (ten cases) and non-occurrence group (79 cases). Logistic regression model was used to analyze the influencing factors of postoperative arrhythmia, and the diagnostic value of QTc interphase level and serum CD40L level in arrhythmia was further evaluated by receiver operating characteristics (ROC) curve. Results The levels of serum potassium, QTc interphase level, and CD40L in the occurrence group were higher than those in the non-occurrence group, the differences were statistically significant (P<0.05). Analysis of influencing factors showed that blood potassium (OR=3.684, 95%CI: 1.259-5.685), QTc interval (OR=2.743, 95%CI: 1.259-6.666), and serum CD40L (OR=3.340, 95%CI: 1.374-8.118) were the influential factors for postoperative arrhythmia in patients with coronary heart disease (P<0.05). ROC curve analysis showed that the combined QTc interphase level of ambulatory electrocar-diogram, and serum CD40L level were more effective in diagnosing arrhythmias than those predicted by them alone (P<0.05). Conclusion The QTc interphase level of ambulatory electrocar-diogram and serum CD40L are abnormal in patients with post-coronary heart disease arrhythmia, and the levels of serum CD40L combined with QTc interphase level are of high diagnostic value for postoperative arrhythmia of coronary heart disease.

Objective To investigate the correlation between serum lipoprotein a (Lp[a]), lipoprotein-associated phospholipase A2 (LP-PLA2), and clinical prognosis in patients with metabolic syndrome complicated with ischemic stroke treated with intravenous thrombolytic therapy. Methods A total of 107 patients with metabolic syndrome combined with hyperacute ischemic stroke admitted to Linfen Central Hospital of Shanxi Province from January 2020 to March 2022 were selected as the study objects. All patients received intravenous thrombolytic therapy with Alteplase after completing routine examination. Clinical data and Lp[a] and LP-PLA2 tests were collected. According to the changes of the National Institutes of Health stroke scale at admission and seven days after admission, the patients were divided into a recent improvement group (67 cases) and a recent no improvement group (40 cases), and according to the changes of the modified Rankin scale score at admission and 90 days after admission, the patients were divided into a long-term improvement group (72 cases) and a long-term no improvement group (35 cases). The clinical data and the differences of Lp[a] and LP-PLA2 were compared respectively, and the influencing factors of recent and long-term prognosis were analyzed, and the predictive value of the influencing factors was further analyzed. Results The levels of total cholesterol, triglyceride, low density lipoprotein, Lp[a], and LP-PLA2 in recent no improvement group were higher than those in recent improvement group (P<0.05). The levels of total cholesterol, low density lipoprotein, Lp[a], and LP-PLA2 in the long-term no improvement group were higher than those in the long-term improvement group (P<0.05). Logistic regression analysis showed that low-density lipoprotein (OR=1.073, 95%CI: 1.011-1.734), Lp[a] (OR=1.833, 95%CI: 1.631-2.341), LP-PLA2 (OR=1.725, 95%CI: 1.237-2.113) were the influencing factors of recent prognosis in patients with metabolic syndrome complicated with ischemic stroke treated with intravenous thrombolytic therapy (P<0.05); Lp[a] (OR=1.743, 95%CI: 1.531-2.293), and LP-PLA2 (OR=1.685, 95%CI: 1.106-2.131) were the factors influencing the long-term prognosis of patients with metabolic syndrome complicated with ischemic stroke treated with intravenous thrombolytic therapy (P< 0.05). Lp[a] and LP-PLA2 could predict the recent and long-term prognosis of patients with metabolic syndrome complicated with ischemic stroke (P<0.05). Conclusion Serum Lp[a] and LP-PLA2 levels in patients with metabolic syndrome complicated with intravenous thrombolysis of ischemic stroke have an influence on the near and long term prognosis, and have a certain prognostic value, which is worthy of attention in clinical work.

98 Relationship between serum melatonin and osteopontin levels and cerebral vasospasm after intracranial aneurysm rupture

Objective To investigate the relationship between serum melatonin (MT), osteopontin (OPN) levels, and cerebral vasospasm (CVS) after intracranial aneurysm rupture. Methods A total of 149 patients with intracranial aneurysm rupture admitted to Guangdong Sanjiu Brain Hospital from March 2017 to October 2022 for concurrent surgical treatment were selected as the study objects, and they were divided into CVS group (60 cases) and non-CVS(NCVS) group (89 cases) according to whether the patients had concurrent CVS after surgery. The differences of serum MT, OPN levels, and other clinical data between the two groups were compared. The influencing factors of postoperative CVS were analyzed by logistic regression model, and the predictive value of MT and OPN on postoperative CVS was calculated by receiver operating characteristic (ROC) curve. Results The proportion of Fisher Grade three to four, the proportion of World Federation of Neurosurgical Societies Grade three to four, and serum MT, OPN in CVS group were higher than those in NCVS group, and the Glasgow coma scale score was lower than that in NCVS group, the differences were statistically significant (P<0.05). Logistic regression model analysis showed that serum levels of MT (OR=2.303, 95%CI: 1.517-3.496) and OPN (OR=2.494, 95%CI: 1.383-4.499) were independent influencing factors for postoperative CVS in patients with ruptured intracranial aneurysm (P<0.05). ROC curve analysis showed that the area under the curve predicted by MT and OPN was higher than that predicted by them alone (P<0.05). Conclusion Serum MT and OPN levels in patients with CVS after intracranial aneurysm rupture are significantly increased, both of which are independent risk factors for patients with CVS after surgery. Combined detection is helpful for early evaluation of patients with CVS after surgery.

102 Comparison of the effect of CT-guided puncture combined with urokinase drainage in the treatment of hypertensive cerebral hemorrhage

Objective To investigate the effect of CT-guided puncture combined with urokinase drainage in the treatment of hypertensive cerebral hemorrhage. Methods A total of 90 patients with hypertensive cerebral hemorrhage who received treatment in Xinyi People’s Hospital of Jiangsu Province from October 2020 to October 2022 were selected, and they were divided into experimental group (CT-guided puncture combined with urokinase drainage treatment, 45 cases) and control group (urokinase drainage, 45 cases) by random number table method. The treatment effect and survival rate of one, three, and six months after treatment were compared between two groups; the levels of serum myelin basic protein (MBP) and serum ferritin (SF) were compared between two groups before treatment and three months after treatment; operation time, intraoperative blood loss, hematoma before and after operation, and postoperative bed time were compared between two groups; the occurrence of complications in two groups was recorded. Results There was no significant difference in treatment effect between two groups (P>0.05); there were no significant differences in survival rates at one, three, and six months after treatment between two groups (P>0.05). At three months after treatment, the levels of MBP and SF in both groups were lower than those before treatment, and experimental group was lower than control group (P<0.05). The operative time and postoperative bed time of experimental group were shorter than those of control group, intraoperative blood loss was lower than that of control group, and the difference of hematoma before and after operation was greater than that of control group (P<0.05). The total incidence of adverse reactions in experimental group was lower than that in control group (P<0.05). Conclusion Both CT-guided puncture combined with urokinase drainage and urokinase drainage alone are effective in the treatment of hypertensive cerebral hemorrhage. CT-guided puncture combined with urokinase drainage is better than urokinase drainage alone in postoperative complications and neurological rehabilitation.

Objective To investigate the clinical effect of continuous passive motion (CPM) machine on improving knee hyperextension in stroke patients with hemiplegia. Methods A total of 60 stroke hemiplegia patients with knee hyperextension admitted to Xuzhou Central Hospital, Jiangsu Province from January to October 2022 were selected. They were divided into two groups according to the random number table method, with 30 cases in each group. The control group received routine rehabilitation training and squatting training, while observation group received routine rehabilitation training and lower limb CPM machine training, both groups were trained continuously for six weeks. Knee hyperextension frequency in 6-minute walking test (6MWT), knee extension angle in the support phase of the affected side, stride length, and stride speed were compared between two groups before treatment and six weeks after treatment. Results Six weeks after treatment, knee hyperextension frequency in 6MWT and knee extension angle in the support phase of the affected side in both groups were lower than those before treatment, and observation group was lower than control group (P<0.05). Six weeks after treatment, stride length and stride speed in both groups were higher than those before treatment, and observation group was higher than control group (P<0.05). Conclusion As a simple and easy to operate auxiliary treatment method, CPM machine combined with conventional rehabilitation training can greatly improve the treatment effect of knee hyperextension in stroke patients with hemiplegia.

Objective To study the relationship between Notch signaling pathway related molecules, procalcitonin (PCT), and serum 25-hydroxyvitamin D3 (25 [OH] D3) expression, and prognosis in children with community-acquired pneumonia (CAP). Methods The clinical data of 160 children with CAP admitted to the First People’s Hospital of Lianyungang, Jiangsu Province from January 2018 to January 2021 were retrospectively analyzed. The patients were followed up for 60 days after treatment and they were divided into good prognosis group and poor prognosis group according to clinical symptoms and imaging results. The expression of relevant basic data and laboratory indicators (total cholesterol [TC], triglyceride [TG], high density lipoprotein cholesterol [HDL-C], PCT, and 25 [OH] D3), and Notch signaling pathway related molecules (Notch1, Notch2, Notch3, Notch4, Jagged1, DLL1, and DLL3) were compared between two groups; the risk factors of poor prognosis in children with CAP were analyzed; the predictive value of Notch1, Notch3, Jagged1, DLL1, PCT, and 25[OH] D3 for poor prognosis in children with CAP was analyzed. Results A total of 31 out of 160 children with CAP had poor prognosis. There were no significant differences in gender, age, TC, TG, HDL-C, Notch2, Notch4, and DLL3 between two groups (P>0.05); Not- ch1, Notch3, Jagged1, DLL1, and PCT level in poor prognosis group were higher than those in good prognosis group, and 25 [OH] D3 level was lower than that in good prognosis group (P< 0.05). Notch1, Notch3, Jagged1, DLL1, PCT were risk factors for poor prognosis of children with CAP (OR>1, P<0.05); 25 [OH] D3 was protective factor for poor prognosis of children with CAP (OR<1, P<0.05). The areas under the curve of Notch1, Notch3, Jagged1, DLL1, PCT, and 25 [OH] D3 predicting poor prognosis were 0.830, 0.883, 0.935, 0.934, 0.708, and 0.867, respectively. Conclusion The prognosis of children with CAP is influenced by many factors, and is closely related to Notch1, Notch3, Jagged1, DLL1, PCT, and 25 [OH] D3, which should be paid close attention to clinically.

Objective To investigate the predictive value of peripheral blood systematic immune-inflammation index (SII) combined with 24 h urinary protein quantification for adverse pregnancy outcome of early-onset severe preeclampsia (EOSP). Methods A total of 65 EOSP pregnant women with adverse delivery outcomes who were admitted to Cangzhou People’s Hospital, Hebei Province from June 2021 to December 2022 were enrolled as adverse outcomes group; in addition, 65 EOSP pregnant women with good delivery outcomes during the same period were enrolled as good outcomes group. The clinical data of two groups were collected; the influencing factors of EOSP adverse pregnancy outcome and the predictive value of SII and 24 h urinary protein inflammation quantification on EOSP adverse pregnancy outcome were analyzed. Results White blood cell count, 24 h urinary protein quantification, neutrophil count, and SII in adverse outcome group were higher than those in good outcome group, and lymphocyte count was lower than that in good outcome group, with statistical significances (P<0.05). SII and 24 h urinary protein quantification were risk factors for adverse pregnancy outcomes in patients with EOSP (OR>1, P<0.05). SII and 24 h urinary protein quantification were positively correlated with adverse pregnancy outcomes in EOSP patients (r=0.301, 0.479, P<0.05). The area under the curve of peripheral blood SII combined with 24 h urinary protein quantification prediction of EOSP adverse pregnancy outcome was higher than that of each index alone (P<0.05). Conclusion Abnormal peripheral blood SII and 24 h urinary protein quantification are independent risk factors for adverse pregnancy outcomes in patients with EOSP, and have certain predictive value.

Objective To explore the clinical efficacy of traditional Chinese medicine Reyanbao on postoperative rehabilitation of unilateral thyroid cancer patients. Methods This study was conducted on 92 patients who underwent surgical treatment for unilateral thyroid cancer at the Department of General Surgery, the First Affiliated Hospital of Anhui University of Chinese Medicine from February 2017 to November 2021. The patients were assigned into the observation group (43 ceses) and the control group (49 cases) by random coin toss method. The control group received routine nursing and routine symptomatic treatment after thyroid cancer operation. On the basis of the treatment in the control group, the observation group was also treated with traditional Chinese medicine Reyanbao. The clinical effect of the two groups was compared. The visual analogue scale (VAS) scores of headache and neck, shoulder and back muscle soreness for three consecutive days after operation were compared between the two groups. The volume of neck drainage for three consecutive days after operation was compared between the two groups. The levels of thyroid-stimulating hormone (TSH) on day 3rd after operation were compared between the two groups.Results The clinical efficacy of the observation group was better than that of the control group, and the difference was statistically significant (P<0.05). VAS scores and neck drainage in the observation group were lower than those in the control group at day 1, 2 and 3 after operation, the differences were statistically significant(P<0.05). There were significant differences in VAS scores and neck drainage between the two groups at each time point (P<0.05). There was no significant difference in TSH level between the two groups on the 3rd day after surgery (P>0.05). Conclusion Traditional Chinese medicine Reyanbao can significantly relieve the discomfort symptoms of neck, shoulder and back muscular soreness, dizziness, headache, nausea, and vomiting, neck swelling and other discomfort symptoms of patients with unilateral thyroid cancer after surgery through the effect of acupoints and local penetration. At the same time, it can markedly reduce the exudation of neck wounds, lower the amount of drainage fluid, accelerate postoperative rehabilitation, and has outstanding clinical efficacy.

124 Clinical efficacy of airway stents in patients with central airway tumors

Objective To investigate the effect of airway stents in the treatment of central airway tumors. Methods A total of 80 patients with central airway tumors who were treated in the First Affiliated Hospital of Nanjing Medical University and Yixing Hospital Affiliated to Jiangsu University from January 2020 to March 2023 were selected, the airway stents was given. Blood gas analysis, lung function index, and quality of life were observed before and after treatment. Results After treatment, the partial pressure of arterial oxygen and saturation of arterial oxygen were higher than those before treatment, and the fraction of arterial carbon dioxide was lower than that before treatment (P<0.05). After treatment, forced expiratory capacity in first second, forced vital capacity, maximum expiratory flow, maximum gas volume per minute, total lung, and functional residual capacity were higher than those before treatment (P<0.05). After treatment, the overall symptom score of the European Organization Research and treatment quality of life scale was lower than that before treatment, and the physical, role, emotional, cognitive, and social dimension scores were higher than those before treatment (P<0.05). Conclusion Airway stents in the treatment of central airway tumors has a good effect, significantly improving the patient’s blood gas, lung function, and quality of life indicators. The treatment effect may be influenced by age, pathological stage, and stenosis degree.

128 Effect of β-elemene on inflammatory response and interferon-γ of patients with lung adenocarcinoma after chemotherapy

Objective To investigate the effect of β-elemene on inflammatory response and immune factor interferon-γ (INF-γ) in patients with lung adenocarcinoma after chemotherapy. Methods Ninety-two patients with lung adenocarcinoma treated in Hai’an Hospital of Traditional Chinese Medicine from February 2018 to January 2021 were selected and they were divided into study group and control group according to random number table method, with 46 cases in each group. The control group received postoperative chemotherapy (Carboplatin combined with Pemetrexed), and the study group received β-elemene therapy on the basis of the control group. The clinical efficacy, serum interleukin-2 (IL-2), tumor necrosis factor-α (TNF-α), carbohydrate antigen 199 (CA199), IFN-γ, and adverse reactions of the two groups were compared. Results There were three cases fell of in the control group during the study. The clinical effect of the study group was better than control group (P<0.05). After treatment, the serum levels of TNF-α, IL-2, and CA199 in two groups were lower than before treatment (P<0.05), and those of the study group were lower than control group (P<0.05). After treatment, the INF-γ level in two groups was higher than before treatment (P<0.05), and the study group was higher than control group (P< 0.05). There were no differences in the grade distribution of phlebitis, liver and kidney damage, and digestive tract reaction between the two groups (P>0.05). Conclusion β-elemene is effective in the treatment of patients with lung adenocarcinoma after chemotherapy. It can reduce inflammatory response, regulate the level of tumor markers, improve immune function, and is safe and reliable.

Objective To compare the efficacy and prognosis of stent and EP stent assisted embolization for communicating wide-neck aneurysms. Methods The clinical data of 84 patients with posterior communicating wide-neck aneurysms treated in South Taihu Hospital Affiliated to Huzhou College from September 2019 to December 2022 were collected for prospective analysis. According to the random number table method, they were divided into two LVIS group (LVIS stent) and EP group (EP stent), with 42 cases in each group. The clinical efficacy, biomarker levels, incidence of complications, and prognosis between the two groups were compared. Results After six months of follow-up, the clinical effect of EP group was better than that of LVIS group (P<0.05). There was no significant difference in recurrence rate between the two groups (P>0.05). 24 h after operation, the levels of serum vascular endothelial growth factor (VEGF) and matrix metalloproteinase-9(MMP-9) in two groups were lower than those before operation, with statistical significance (P<0.05). There were no significant differences in serum VEGF and MMP-9 levels between the two groups 24 h after operation (P>0.05). There was no significant difference in the incidence of complications and clinical prognosis between the two groups (P>0.05). Conclusion LVIS stents and EP stents have similar therapeutic and prognostic effects in assisting embolization of wide-neck aneurysms, with high safety, and stents can be selected according to specific needs in clinical practice.

There are many types of psychiatric disorders secondary to heart failure, and due to the heaviness of the underlying disease and the complexity of the pre-morbid mechanism. There is a significant difference between mental and psychological disorders that occur solely or are secondary to other diseases, and it is a more challenging type of bicardiac disease. The differentiation and treatment concept of “syndrome based disease” is in line with the characteristics of the pathogenesis of this disease. Professor Zhao Haibin believes that the basic pathogenesis of mental and psychological disorders after heart failure is inseparable from the four aspects of “deficiency, stasis, heat, and stagnation”. Objective To explore the effect of Tongxieyao Prescription combined with moxa stick moxibustion on diarrhea type-irritable bowel syndrome (IBS-D) and its influence on levels of immunoglobulin and serum inflammatory factors. Methods A total of 94 patients with IBS-D of liver depression and spleen deficiency type admitted to Department of Spleen and Stomach Diseases, Beijing Changping District Hospital of Traditional Chinese Medicine from July 2019 to October 2022 were selected and they were divided into observation group and control group according to random number table method, with 47 cases in each group. The control group was given Piverium Bromide Tablets, and the observation group was given Tongxieyao Prescription combined with moxa stick moxibustion. Both groups were treated for 14 days. Clinical efficacy, traditional Chinese medicine symptom score, irritable bowel syndrome-symptom severity scale (IBS-SSS) score, irritable bowel syndrome-quality of life (IBS-QOL) score, immunoglobulin level, inflammatory factor level of the two groups were compared, and the occurrence of adverse reactions between the two groups were recorded. Results The clinical effect of the observation group was better than that of the control group (P<0.05). After treatment, the scores of symptoms of emotional changes cause or aggravate diarrhea, abdominal pain, pain reduction after diarrhea, hypochondriacal swelling pain, dilatation, and often sighing in two groups were lower than before treatment, and those in the observation group was lower than those in the control group (P<0.05). After treatment, IBS-SSS scores in both groups were lower than before treatment, and that in the observation group was lower than that in the control group (P<0.05). After treatment, IBS-QOL scores in both groups were higher than before treatment, and that in the observation group was higher than that in the control group (P<0.05). After treatment, the levels of serum immunoglobulin(Ig)A, IgG and IgM in the two groups were lower than before treatment, and the IgA and IgG in the observation group were lower than those in the control group (P<0.05). After treatment, the serum levels of interleukin(IL)-6 and tumor necrosis factor-α in both groups were lower than before treatment, and those in the observation group was lower than those in the control group (P<0.05). After treatment, the serum IL-10 level in both groups was higher than that before treatment, and that in the observation group was higher than that in the control group (P<0.05). No obvious adverse reactions were observed during treatment. Conclusion Tongxieyao Prescription combined with moxa stick moxibustion can improve the clinical efficacy of IBS-D patients, regulate immunity, control inflammation in vivo, reduce pain and improve symptoms, and is worthy of clinical application.

Lumbar tendon injury, including lumbar contusion, transverse process syndrome of the third lumbar vertebra, lumbar disc herniation, lumbar spinal stenosis, piriformis syndrome, etc. is one of the most popular and young diseases at present. Most of the commonly used western medicine and surgical therapy will produce certain side effects, and some patients are afraid of surgical treatment. Therefore, it is particularly critical to explore a treatment idea with less side effects and more prominent effects. As the inheritor of Qinggong bone-setting, Professor Wang Qingfu created the “three-part method” based on 40 years of clinical experience, dividing lumbar tendon injury into three stages, namely “injury of flesh, injury of tendon, and injury of bone”. In diagnosis, he not only pays attention to MRI, CT, and other diagnostic reports, but also pays attention to patients’ subjective feelings and palpation feelings of his hands, and applies syndrome differentiation and treatment thinking to the diagnosis of lumbar tendon injury. 154 Mesh meta-analysis of different acupuncture therapies for sudden deafness

Objective To evaluate the clinical effect of different acupuncture therapies for sudden deafness by mesh meta-analysis. Methods Randomized controlled trials on acupuncture and moxibustion in treatment of sudden deafness were searched from PubMed, Embase, Cochrane Library, CNKI, Wanfang Data, VIP, and SinoMed database, and the search time was from inception to March 2023. Stata17.0 was used for mesh meta-analysis. Results A total of 23 articles were included, covering six different interventions. The total response rate included six interventions and the average hearing threshold included four interventions. In terms of total effective rate, filiform needle (OR=4.00, 95%CI: [1.37 to 11.70]), filiform needle + conventional western medicine (OR=3.86, 95%CI:[2.54 to 5.87]), electro-acupuncture (OR=3.09, 95%CI:[1.61 to 5.92]), electro-acupuncture + conventional western medicine (OR=5.14, 95%CI:[1.86 to 14.19]), acupoint injection + conventional western medicine (OR=3.38, 95%CI:[1.94 to 5.89]) were superior to conventional western medicine. In terms of average hearing threshold, filiform needle + conventional western medicine (OR=-1.90, 95%CI: [-2.99 to -0.81]), electro-acupuncture + conventional western medicine [OR=-3.55, 95%CI: (-6.50 to -0.61)] were superior to conventional western medicine. Total effective rate: electro-acupuncture + conventional western medicine > filiform needle + conventional western medicine > filiform needle > acupoint injection + conventional western medicine > electro- acupuncture > conventional western medicine; average hearing threshold: electro-acupuncture + conventional western medicine > filiform needle + conventional western medicine > acupoint injection + conventional western medicine > conventional western medicine. Conclusion Acupuncture ther- apy or acupuncture therapy combined with conventional western medicine is better than conventional western medicine in the treatment of sudden deafness, and electro-acupuncture com- bined with conventional western medicine has advantages in improving the average hearing threshold and total effective rate.

159 Comparison of clinical haematological indices in patients with primary immune thrombocytopenia of different traditional Chinese medicine syndromes

Objective To explore the differences in clinical blood indicators among different traditional Chinese medicine syndromes in primary immune thrombocytopenia (ITP), and to provide reference for the clinical diagnosis and treatment of ITP. Methods A total of 124 hospitalized patients with confirmed ITP from January 2019 to December 2022 in the Department of Hematology and Oncology, the First Affiliated Hospital of Hunan University of Chinese Medicine were collected. Differences in clinical blood indexes (coagulation function, blood routine, lymphocyte subpopulations, cytokines) were compared between different traditional Chinese medicine evidence types of ITP. Results According to different syndrome types, 124 ITP patients were divided into failure of keeping fluid due to qi-deficiency group(36 cases), yin deficiency and blood heat group(27 cases), internal blockage due to blood stasis group (15 cases), and bleeding due to blood heat group (46 cases). There was significant difference in gender composition among all groups (P<0.001). The age of yin deficiency and blood heat group and internal blockage due to blood stasis group was higher than that of failure of keeping fluid due to qi deficiency group, and the difference was statistically significant (P<0.05 or P<0.01). The age of the yin deficiency and blood heat group was higher than that of the bleeding due to blood heat group, and the differences were statistically significant (P<0.05). The hospitalization days in the internal blockage due to blood stasis group were longer than those in the failure of keeping fluid due to qi deficiency and bleeding due to blood heat group, and the differences were statistically significant (P<0.01). The fibrinogen of internal blockage due to blood stasis group was lower than that of yin deficiency and blood heat group, and the difference was statistically significant (P<0.05). The white blood cells count in the internal blockage due to blood stasis group were lower than those in the yin deficiency and blood heat group and the bleeding due to blood heat group, and the differences were statistically significant (P<0.05). The hemoglobin of yin deficiency and blood heat group was higher than that of failure of keeping fluid due to qi deficiency group, the difference was highly statistically significant (P<0.01). The platelets count in the internal blockage due to blood stasis group were higher than those in other syndrome groups, and the differences were statistically significant (P < 0.05). The ratio of CD4+T cells and CD4 / CD8 in the bleeding due to blood heat group was lower than that in the yin deficiency and blood heat group, the differences were statistically significant (P<0.05). The CD3 / CD8 ratio in the bleeding due to blood heat group was lower than that in the yin deficiency and blood heat group and the internal blockage due to blood stasis group, and the differences were statistically significant (P<0.01). The levels of interleukin-6 and interleukin-10 in the bleeding due to blood heat group were lower than those in other syndrome groups, and the differences were statistically significant (P<0.05). Tumor necrosis factor-α in the bleeding due to blood heat group was higher than that in other syndrome groups, and the differences were statistically significant (P<0.05). Conclusion There are differences in blood indicators among different traditional Chinese medicine syndrome of ITP, which can provide objective basis for the Traditional Chinese medicine diagnosis and treatment of primary immune thrombocytopenia.

164 Clinical observation of Auricular Point Pressing Pills in the treatment of respiratory tract infection in children

Objective To explore clinical effect of Auricular Point Pressing Pills in the treatment of respiratory tract infection in children. Methods Eighty-two children with respiratory tract infection treated in Tianchang Hospital of Traditional Chinese Medicine in Anhui Province from March 2016 to December 2021 were selected as the research objects. They were divided into control group and observation group according to the random number table method, with 41 cases in each group. The control group was treated with acupoint application, and the observation group was treated with Auricular Point Pressing Pills on the basis of the control group. Traditional Chinese medicine syndrome scores, clinical efficacy, serum inflammation and immunological indicators, and adverse reactions between two groups were compared. Results Before treatment, there were no statistically significant differences in the main syndrome score and secondary syndrome score of traditional Chinese medicine between two groups (P>0.05). After treatment, the main syndrome score and secondary syndrome score of traditional Chinese medicine in two groups were lower than those before treatment, and those in observation group were lower than those in control group, the differences were statistically significant (P<0.05). Clinical efficacy of observation group was better than that of control group, the difference was statistically significant (P<0.05). Before treatment, there were no statistically significant differences in the levels of hypersensitive C-reactive protein (hs-CRP) and immunoglobulin A (IgA) between two groups (P>0.05). After treatment, the levels of hs-CRP in two groups were lower than those before treatment, the levels of IgA in two groups were higher than those before treatment, the level of hs-CRP in observation group was lower than that in control group, and the level of IgA in observation group was higher than that in control group, the differences were statistically significant (P<0.05). There was no statistically significant difference in the total incidence rate of adverse reactions between two groups (P>0.05). Conclusion Acupoint application combined with Auricular Point Pressing Pills has superior curative effect and good safety in the treatment of children with respiratory tract infection.

Objective To evaluate the application effect of phased training mode in respiratory support equipment training for nurses with low seniority in emergency department. Methods A total of 80 junior nurses in the Department of Emergency the Second Affiliated Hospital of Wenzhou Medical University from January 2022 to November were selected as the study objects, and they were divided into control group and experimental group according to random number table method, with 40 cases in each group. The control group received traditional training, and the experimental group received phased training. The differences of theoretical assessment scores, mastery of relevant theories, operational skills, emergency response ability, and self-efficacy were compared between the two groups before training, and four and seven months after training. Results The overall analysis results showed that there were statistically significant differences in the time effect, inter- group effect and interaction effect of theoretical assessment scores, relevant theoretical knowledge, operational skills assessment, emergency response ability assessment, and self-efficacy between the two groups at different time points (P<0.05); intra-group comparison: the results of theoretical assessment, mastery of relevant theories, assessment of operational skills and emergency response ability, and self-efficacy of the two groups were compared before, and four and seven months after training, the differences were statistically significant (P<0.05); inter-group comparison: the test group’s theoretical assessment scores, mastery of relevant theories, operational skills assessment, emergency response ability assessment, and self-efficacy were higher than those of the control group at four and seven months after training, the differences were statistically significant (P<0.05). Conclusion The phased training mode can greatly enhance the theory, skills, and self-confidence of the respiratory support instrument for nurses with low seniority in the emergency department. The training mode of step-by-step and checking for deficiencies can greatly meet the clinical work needs and is worthy of promotion and application.

Objective To analyze the application effect of four-in-one short-distance communication extended nursing based on network platform in patients with colorectal cancer. Methods A total of 80 patients with colorectal cancer admitted to Liyang People’s Hospital in Jiangsu Province from April 2021 to September 2022 were selected as the study objects. Patients with odd and even numbers were randomly numbered from 1 to 80 and included in control group and observation group, with 40 cases in each group. The control group was given routine extended care, and on the basis of the control group, the observation group was given four-in-one short-distance communication extended nursing based on network platform. The quality of extended care, the score of self-care ability, and the incidence of complications were compared between the two groups. Results After nursing, the nursing assessment scale scores of both groups were higher than those before intervention, and the observation group was higher than that of the control group, the difference was statistically significant (P<0.05). After nursing, the self-care ability scale score of the two groups was higher than those before intervention, and the observation group was higher than that of the control group (P<0.05). The total complication rate of observation group was lower than that of control group (P<0.05). Conclusion The application of four-in-one short-distance communication extended care based on the network platform in colorectal cancer patients can significantly improve the quality of extended care for patients, help patients avoid the risk of related complications, and has high clinical promotion and application value.

181 Application of cognitive-behavioral intervention based on human perspective in the clinical nursing of benign epilepsy of childhood with centro-temporal spinke

Objective To explore the application of cognitive-behavioral intervention based on human perspective in the clinical nursing of children with benign epilepsy of child hood with centro-temporal spike (BECT). Methods A total of 77 children with BECT admitted to Anhui Children’s Hospital from March 2020 to June 2022 were selected as the study objects, and they were divided into control group (38 cases) and observation group (39 cases) by random number table method. The control group was given routine nursing intervention, and the observation group was given cognitive-behavioral intervention based on human perspective on the basis of control group. The intervention period was two months. After intervention, the differences of medication compliance and cognitive function before and after intervention were compared between the two groups. Results After intervention, the compliance score and cognitive function score of the observation group were higher than those before intervention, and the observation group were higher than those of the control group (P<0.05). Conclusion Cognitive-behavioral intervention based on the human perspective is helpful to improve the children’s medication compliance and cognitive function.

185 Stiff-person syndrome: a report of two cases and literatures analysis

Stiff-person syndrome (SPS) is characterized by fluctuating, progressive stiffness of the trunk and proximal limbs, accompanied by painful spasticity. With the progression of the disease, SPS patients are prone to permanent skeletal abnormalities, eventually leading to inability to walk and disability. However, most patients can be improved by medical treatment in the early stage. Therefore, early diagnosis and treatment are essential to prevent or delay the progression of the disease. However, the clinical manifestations of SPS are complex and variable, which brings great challenges to medical workers. Therefore, doctors need to further strengthen the study of relevant theoretical knowledge and improve the understanding of this disease. This paper reports the medical records of two cases with SPS treated in Tongji Hospital of Tongji University in 2023. Among them, one was SPS patient with Hashimoto thyroiditis, and the other was SPS patient with thymoma. By discussing the clinical characteristics of the two special SPS cases, it is helpful to improve the early recognition and diagnosis of doctor and reduce misdiagnosis and mistreatment.

189 Application value of six Sigma theory in the management of blood culture samples

The quality of blood culture samples is directly related to the accuracy of test results. Improving the quality of blood culture samples is crucial for accurate anti-infection treatment and reducing the generation of drug-resistant bacteria. At present, the quality of blood culture specimens is not high, so it is urgent to adopt effective management methods to improve the quality of blood culture specimens. Taking the specific data and specific measures of Yongchuan Hospital of Chongqing Medical University as examples, this paper expounds the methods and significance of applying six Sigma theory in the management of blood culture specimen delivery. Through the five implementation steps of six Sigma theory, namely definition, measurement, analysis, improvement, and control, problems are found, analyzed, and solved, so as to standardize the collection and delivery of clinical blood culture specimens. The quality of blood culture samples can be improved by effective quality control of the key links of blood culture samples.

194 Effect of multi-disciplinary treatment management mode on the incidence of high-rate medical records

Objective To explore the effect of multi-disciplinary team (MDT) on the incidence of high rate cases. Methods The data came from the hospital of municipal medical insurance system. A total of 33 263 medical records of medical insurance settlement discharged from a hospital from May to June 2021 and from May to June 2022 were taken as the research object, among which 17 683 medical records of medical insurance settlement discharged from May to June 2022 were taken as the intervention group, and 15 580 medical records of medical insurance settlement discharged from May to June 2021 were taken as the control group. Set up an MDT management team, formulate measures through MDT meetings, implement them in collaboration with multiple departments, and the incidence of high rate cases in the two groups was observed. Results The incidence of high rate cases in the intervention group was lower than that in the control group, and the difference was statistically significant (P<0.05). The total hospitalization cost, drug cost, consumables cost, drug proportion, and consumption proportion in the intervention group were lower than those in the control group, and the differences were statistically significant (P<0.05). The average length of stay in the pre-treatment group was shorter than that in the control group, and the difference was statistically significant (P<0.05). Conclusion The MDT management mode under the diagosis-related groups payment can reduce the incidence of high rate cases, and is an effective management method under the diagosis-related groups payment.
