4 Lysis effect of phage Guo1 and Leo on Mycobacterium tuberculosis in the dormant stage of different models

Objective To investigate the lysis effect of phage Guo1 and Leo on Mycobacterium tuberculosis in dormant stage. Methods ClustalX software was used for multiple sequence comparison. Motif3 motifs of phage Guo1 and Leo were searched. The model of Mycobacterium tuberculosis in dormant stage was constructed by hypoxia model and potassium deficiency model respectively. Hypoxic and potassium deficiency mode induce domant TB bacterial fluid, they were respectively randomly divided into 5 groups, each group was set with 3 duplicate tubes, and each tube was filled with 2 mL bacterial liquid. Then 100 liters of Isoniazid (final concentration: 5 g/mL, Isoniazid group), Rifampicin (final concentration: 5 g/mL, Rifampicin group), phage Leo (titer: 2.6×108 PFU/mL, Leo group), and phage Guo1 (titer: 3.26×108 PFU/mL, Guo1 group) were added to the test tubes of each group, and the control group was added with the same amount of ddH2O. The drug resistance to Isoniazid and Rifampicin in dormant tuberculosis bacteria and the lysis of phage in dormant Tuberculosis bacteria were detected by most probable number method. Results The motif3 motif was found in the tape rule proteins of phages Guo1 and Leo, and the motif3 motif contained 6 highly conserved amino acid residues and 1 highly conserved isoleucine-tryptophan site. When used as hypoxia model for dormant Tuberculosis bacteria, compared with Rifampin group and Isoniazid group, the bacteria quantity of Guo1 group decreased significantly (P < 0.05). Compared with Rifampicin group, the amount of bacteria in Leo group decreased significantly (P < 0.05). The drug resistance rate of potassium deficiency model to Isoniazid and Rifampicin was significantly higher than that of hypoxia model. Conclusion Compared with hypoxia model, potassium deficiency model can induce Tuberculosis bacterium to enter deeper dormant state. Phage Guo1 has cleavage effect on dormant tuberculosis bacteria induced by hypoxia model, and phage Leo has cleavage effect on dormant Tuberculosis bacteria induced by potassium deficiency model.

8 Effect of Rhodiolae Herba on pulmonary injury induced by intermittent hypoxia in rats

Objective To investigate the effects of Rhodiolae Herba on pulmonary injury induced by intermittent hypoxia (IH) in rats. Methods Rat model of IH was established. 45 male and Sprague-Dawley rats aged 4 weeks were randomly divided into 5 group according to weight (9 rats in each group): normoxia control group (group A), IH group (group B), IH with low-dose Rhodiolae Herba group (group C), IH with medium-dose Rhodiolae Herba group (group D), IH with high-dose Rhodiolae Herba group (group E). After 45 days, the contents of nitric oxide (NO), superoxide dismutase (SOD), malondialdehyde (MDA), erythropoietin (EPO), tumor necrosis factor-α (TNF-α) and the expression levels of hypoxia inducible factors-1α (HIF-1α) mRNA and substance P-1 (SP-1) mRNA in the lung homogenate of rats were detected. The pathological changes under microscope were observed, and the relationship between the indicators, oxidative stress and hypoxic reactions were analyzed. Results Compared with group A, the contents of NO and SOD in group B decreased and the content of MDA increased (P < 0.01); compared with group B, the content of SOD in group C increased (P < 0.01), the contents of NO and SOD in group D and group E increased, and the content of MDA decreased (P < 0.05 or P < 0.01). Compared with group A, EPO content and HIF-1α mRNA expression level in group B increased (all P < 0.01); compared with group B, EPO content and HIF-1α mRNA expression level in group C, group D and group E decreased (all P < 0.01). Compared with group A, TNF-α content and SP-1 mRNA expression level in group B increased (P < 0.01); compared with group B, the TNF-α content in group C decreased (P < 0.01), TNF-α content and SP-1 mRNA expression level in group D and group E decreased (P < 0.01). Correlation analysis showed that TNF-α content and SP-1 mRNA level were negatively correlated with NO content (r = -0.55, -0.57), SOD content (r = -0.84, -0.69). TNF-α content, SP-1 mRNA level were positively correlated with MDA content (r = 0.55, 0.56), EPO content (r = 0.69, 0.57), HIF-1α mRNA level (r = 0.86, 0.75) (all P < 0.01). HE results showed that the lung tissue of IH group showed inflammatory and anoxic changes, and the prognosis was improved after intervention. Conclusion Rhodiolae Herba has protective effect on lung injury induced by IH, which may be achieved by antioxidant stress, reducing hypoxia damage and inhibiting inflammatory response.

14 Effect of Digoxin on apoptosis of MDA-MB-231 cells in triple negative breast cancer

Objective To investigate the effect of Digoxin on apoptosis of MDA-MB-231 cells of triple negative breast cancer in vitro, and to reveal its possible mechanism. Methods Breast cancer MDA-MB-231 cells were cultured in vitro and divided into control group and drug administration group (25, 50, 100, 200 nmol/L Digoxin). CCK-8 method was used to detect the effects of Digoxin at different concentrations on the activity of MDA-MB-231 cells at different action time (24, 48, 72 h). The effects of Digoxin at different concentrations on apoptosis and mitochondrial membrane potential were quantitatively investigated by flow cytometry. Expression of Bax, Bcl-2, P62 and LC3B in MDA-MB-231 cells was detected by Western blot. The effects of Digoxin at different concentrations on autophagosomes in MDA-MB-231 cells were observed by transmission electron microscopy. Results CCK-8 results indicated that, compared with the control group, the activity of MDA-MB-231 cells decreased in the drug administration group at different time, with statistically significant differences (P < 0.05 or P < 0.01). Annexin V/PI double staining results indicated that compared with the control group, the apoptosis rate of the treated group increased with a statistically significant difference (P < 0.05 or P < 0.01). The results of JC-1 staining indicated that compared with the control group, 50, 100 and 200 nmol/L of Digoxin reduced the mitochondrial membrane potential of MDA-MB-231 cells, and the proportion of JC-1 decreased, with statistically significant differences (P < 0.05 or P < 0.01). The results of fluoroscopy indicated that, compared with the control group, the cell morphology of the drug administration group underwent apoptotic changes, and the formation of autophagosomes was observed in the drug administration group. Western blot results indicated that, compared with the control group, the administration group up-regulated the expression of pro-apoptotic factor Bax, down-regulated the expression of anti-apoptotic factor Bcl-2, and up-regulated the expression of autophagy marker protein LC3B, with statistically significant differences (P < 0.05 or P < 0.01); compared with the control group, 50, 100 and 200 nmol/L of Digoxin decreased the expression of P62, with statistically significant differences (P < 0.05 or P < 0.01). Conclusion Digoxin inhibits the activity of MDA-MB-231 cells, promotes apoptosis, induces autophagy, and may have a potential effect on triple negative breast cancer.

Objective To explore the effect of Genistein inducing liver cancer MHCC97-L cells apoptosis and activity of caspase-3 and caspase-9 through inhibiting JNK pathway. Methods The experiment was divided into control group and SP600125 (SP) 10, 20, 30 μmol/L groups. Western blotting was carried out to search the optimum working concentration of SP. The experiment was divided into control, Gen, SP and combined groups. Apoptosis morphology, apoptosis index, activity of caspase-3 and caspase-9 were detected by inverted microscope, fluorescence microscope and spectrophotometry respectively. Results SP obviously blocked the expression of P-JNK protein at a minimum working concentration of 20 μmol/L (P < 0.01). Under inverted microscope, chromatin became darker or condensate in each drug group, and cell changes most obvious in combined group. Early apoptotic cells and late apoptotic cells were observed under fluorescence microscope. Compared with control group, apoptotic index of all drug groups was significantly increased (P < 0.01), among which apoptotic index of combined group was highest and pro-apoptotic effect was stronger than that of Gen group and SP group (P < 0.01). Compared with control group, the activity of caspase-3 and caspase-9 in all drug groups were significantly increased (P < 0.01). Compared with Gen group and SP group, the activity of caspase-3 in combination group was the highest (P < 0.01), and the activity of caspase-3 in SP group was higher than that in Gen group (P < 0.01). The activity of caspase-9 in the combination group was higher than that in the Gen group (P < 0.01), while the activity of caspase-9 in the SP group and Gen group and the combined group and the SP group have no significant differences (P > 0.05). Conclusions These results suggest that inhibition of JNK pathway can promote the apoptosis of Gen induced MHCC97-L cells by activating the caspase-dependent mitochondrial pathway.

24 Advance in the immunological detection technologies research of serum protein biomarkers

Serum protein biomarkers are important molecular markers for labeling changes in tissues, organs and cell functions during disease, which have important application value in disease diagnosis and treatment. The development of immunological assays such as enzymatic chemiluminescence immunoassays, multiplex fluorescence immunoassays, and electrochemiluminescence immunoassays provides various methods for serum protein biomarkers detection. In this review, the technical characteristics, application research, advantages and disadvantages of the commonly used serum protein biomarker detection methods are analyzed and compared in order to provide useful reference for researchers.

29 Advances on the role of Wnt signaling pathway in rheumatoid arthritis

The typical pathological changes of rheumatoid arthritis (RA) are synovitis and bone destruction, which eventually lead to joint deformity and disability. Wnt signaling pathway plays a wide role in synovitis and bone destruction, and is an important factor in the progression of RA. Wnt signaling pathway is closely related to the dysfunction of RA chondrocytes, osteoblasts and osteoclasts, and promotes the production of RA-related pathological factors. This review summarizes the relationship between Wnt signaling pathway and synovitis and bone erosion in RA, and the treatment targeting Wnt signaling pathway.

33 Research progress of non-coding RNA in diabetic cardiomyopathy

Diabetic cardiomyopathy (DCM) is one of the important causes of death in diabetic patients. It is defined as a ventricular dysfunction disease independent of coronary artery disease and hypertension. It is defined as ventricular dysfunction independent of coronary artery disease and hypertension, it can induce heart failure, malignant arrhythmia and other heart disease, there is also a risk of sudden cardiac death. Cardiac tissue in DCM patients often undergoes multiple morphological changes at the cellular level, such as cardiomyocyte hypertrophy, interstitial fibrosis, and intramyocardial microvascular disease. Non-coding RNAs (ncRNAs) play an important role in this process. This paper summarizes some of the current literature on ncRNAs in DCM studies. To explore the use of long non-coding RNAs (lncRNAs) and microRNAs (miRNAs) as therapeutic targets and biomarkers for DCM, and provide new ideas and new horizons for the specific treatment of DCM.

37 Research progress on pharmacological effect of Banxia Xiexin Decoction

Banxia Xiexin Decoction, the original prescription begins in Shanghan Zabing Lun, founded by the medical saint Zhang Zhongjing, has the function of harmonizing liver and spleen, mildly regulating cold and heat, relieving oppression and masses. Its main treatment cold and hot mixed ruffian syndrome and has been widely used in clinical practice by doctors of previous generations. Modern pharmacological studies have shown that it can protect the gastric mucosa, regulate the metabolism of blood glucose and lipid, resist cell apoptosis, regulate the intestinal flora, regulate neurotransmitter and other pharmacological effects. The author reviews the literatures on the pharmacological effect of Banxia Xiexin Decoction at home and abroad in recent years, and summarizes its modern pharmacological effect, especially its research progress on tumor, in order to provide certain reference for the in-depth research of scientific research workers and the clinical work of medical workers.

40 Research progress of the application of ultrasound elastography in transplanted kidney

Ultrasonic elastography (UE) is an imaging technique sensitive to tissue elasticity. According to the principle, UE can be divided into two types of elastic imaging: strain-type and shear-wave. The strain-type elastography uses the probe to compress the tissue to make the tissue deform, and converts the degree of deformation into color image. The non-determinable factors of external force make the repeatability become worse and strain-type elastography is qualitative or semi-quantitative. The shear-wave elastography does not need to apply pressure to the tissue. The acoustic radiation force is used to propagate and deform the tissue, and then the shear wave velocity or young′s modulus of the corresponding tissue is calculated. The repeatability is good, realizing real-time visualization of dual-mode imaging and quantitative reaction of tissue hardness. With the development of ultrasound elastography in evaluating the postoperative status of renal transplantation, some achievements have been made in renal fibrosis, renal function and acute and chronic rejection. With fibrosis of renal parenchyma and renal function impairment, renal tissue elasticity index increased gradually, and studies have found that in transplant renal pathology reaction than conventional color doppler and renal function index sensitive, due to the complex structure of kidney, internal structure distribution. The application of ultrasound elastography to kidney transplantation is affected by many factors, and further research is needed to make it play a greater role in the clinical diagnosis and treatment of kidney transplantation.

46 Research progress on parenteral nutrition associated cholestasis in neonates

With the widespread use of parenteral nutrition in newborns, parenteral nutrition associated cholestasis (PNAC) has become one of the common complications in neonates, especially preterm infants. The disease often causes hepatocyte steatosis and cholestasis of the child, leads to biliary cirrhosis, which can cause liver failure and even death. Therefore, the disease should be highly valued by neonatal physicians. Due to the pathogenesis of PNAC is still unclear and controversial, this paper studys and summarizes a large number of recent literatures on PNAC, and found that PNAC is associated with premature birth and low birth weight, prolonged fasting, infection, parenteral nutrition application time, nutritional imbalance, and genetic factors. This article will review the known pathogenesis, diagnosis, treatment and new research progress.

50 Application progress of two dimensional speckle tracking echocardiography in experimental rabbits

The two-dimensional speckle tracking echocardiography can track the speckle echo of cardiac tissue to obtain the global and regional deformation parameters of myocardium. Compared with other small rodents, rabbit hearts have a comparative advantage in the evaluation of structure and function. Among the application fields, ischemic heart disease rabbit models are the most widely used. Other fields, such as hypertensive ventricular hypertrophy, hyperthyroidism cardiomyopathy and hypercholesterolemia, trauma, infection induced myocardial damage have also been studied. This paper reviews the principle of two-dimensional speckle tracking technology, the advantages and disadvantages of the rabbit models and the application of speckle tracking in the rabbit models, so as to provide reference for preclinical experimental study.

54 The status and influencing factors analysis of compliance behavior of prisoners with hypertension

Objective To explore the compliance behavior and the related influencing factors of prisoners with hypertension. Methods A total of 198 patients diagnosed with hypertension from January 2018 to January 2019 in Hainan Judicial Hospital were selected, and a questionnaire survey to determine compliance behavior were conducted. According to the compliance behavior, the study subjects were divided into the good compliance group (116 cases) and the poor compliance group (82 cases). The normal data and laboratory indicators of the two groups of patients were compared and the relevant factors affecting compliance were analyzed. Results The differences in gender, age, body mass index (BMI), marital status, educational level, hypertension classification, and hypertension risk stratification between the two groups were showed statistically significant (P < 0.05). There were statistically significant differences in the systolic blood pressure, uric acid, and fasting blood glucose between the two groups of patients (P < 0.05). In addition, gender, age, BMI, education level, hypertension classification, hypertension risk stratification, and systolic blood pressure were the influencing factors of patient compliance (P < 0.05). Conclusion Gender, age, BMI, education level, hypertension classification, hypertension risk stratification, and systolic blood pressure level are related to compliance behaviors of hypertension offenders.

58 Analysis of the serum vitamin A levels and related influencing factors in children aged 0 to 6 years

Objective To investigate the serum vitamin A (Vit A) levels and related influencing factors in children aged 0 to 6 years. Methods From April 2017 to April 2019, a total of 445 children aged 0 to 6 years who were treated in the pediatric clinic of Cangzhou Hospital of Integrated Traditional Chinese Medicine and Western Medicine of Hebei Province were selected for questionnaire survey. Serum Vit A levels were measured, and the levels of Vit A and the deficiency of Vit A among children aged 0 to 6 years of different gender, season and age were observed and analyzed. Results There were no significant differences in Vit A levels and deficiencies between different genders (P > 0.05). There was a statistically significant difference in serum Vit A levels between different seasons (P < 0.05). Further comparison, the Vit A levels in summer and autumn were significantly higher than those in winter, and the differences were statistically significant (P < 0.05). Comparison of serum Vit A at different ages, the differences were statistically significant (P < 0.05). Among them, the levels of Vit A were the lowest in children less than 6 months (P < 0.05). There were no significant differences in the levels of Vit A among children of other ages (P > 0.05). While the difference in serum Vit A deficiency was statistically significant (P < 0.05). Regression analysis showed that season and age were influencing factors of Vit A levels. Conclusion It is necessary to perform Vit A test and appropriate Vit A supplement in time for children in winter and low age.

61 Construction and effect evaluation of disaster rescue capability training program for nurses in military hospitals

Objective To construct and evaluate the effect of the nursing disaster rescue training program in the military hospital. Methods A total of 120 paramedics from March to May 2019 in the Second Affiliated Hospital of Air Force Medical University of the emergency care team were selected to training using by purpose sampling method. The core competence evaluation scale of disaster nursing for clinical paramedics was used to evaluate the scores of disaster nursing ability, theoretical knowledge and operation skills before and after paramedics training. Results After the disaster rescue capability training, the scores of the total disaster rescue capacity and each dimension, including disaster nursing basic knowledge, disaster process nursing, vulnerable population care, disaster nursing professional development, and disaster nursing management of the paramedics were all higher than before the training, and the differences were statistically significant (P < 0.05). After training, the theoretical and operational assessment scores of the paramedics were higher than before training, and the differences were all statistically significant (all P < 0.05). Conclusion The disaster rescue training program can effectively improve the disaster rescue ability, theory and operation levels of paramedics, and provide a basis for paramedics disaster rescue related training.

65 Study on the correlation between anticipatory sadness and psychological resilience in breast cancer patients during chemotherapy

Objective To explore the correlation between anticipatory sadness and psychological resilience of breast cancer patients during chemotherapy, and to provide theoretical basis for formulating measures to improve anticipatory sadness of breast cancer patients in the later stage. Methods A cross-sectional survey was conducted to investigate the anticipated sadness and resilience of 188 breast cancer patients who were received chemotherapy in Shandong Province Qianfoshan Hospital from January to March 2019 by using the general information questionnaire, the Chinese version of the anticipatory grief scale and the resilience scale. Results The total score of anticipatory grief in breast cancer patients during chemotherapy were (34.16±15.97) points, and there was generally anticipatory grief. The median total score of resilience were 69 points, which was at a moderate level, while that was higher than the domestic norm. The anticipatory grief scores were negatively correlated with tenacity (r = -0.149,P < 0.05), strength (r = -0.253,P < 0.01), and total resilience scores (r = -0.232, P < 0.01). Conclusion Anticipatory grief of breast cancer patients during chemotherapy period is closely related to psychological resilience. Clinical medical staff should take active and effective measures to improve their psychological resilience level, while alleviate their sadness and improve their quality of life.

69 Expression and clinical significance of serum FFA, β-arrestin2 and RBP4 in patients with type 2 diabetes mellitus

Objective To investigate the expression and clinical significance of serum free fatty acids (FFA), β inhibition of protein 2 (β-arrestin2), retinol-binding protein 4 (RBP4) in patients with type 2 diabetes mellitus (T2DM). Methods From February 2016 to December 2018, 123 T2DM patients admitted to Zhangjiagang Hospital Affiliated To Soochow University (hereinafter referred to as “our hospital”) were selected as the observation group. The observation group was divided into the obese T2DM group (BMI≥28 kg/m2, n = 62) and the non-obese T2DM group (BMI<28 kg/m2, n = 61) according to body mass index (BMI). In addition, 60 volunteers who came to our hospital for physical examination during the same period were selected as the control group. The serum levels of  FFA, β-arrestin2, RBP4 and glucose and lipid metabolism of the three groups were compared. Pearson correlation was used to analyze the correlation between the serum levels of FFA, β-arrestin2 and RBP4 and the level of glucose and lipid metabolism. Results The serum levels of FFA and RBP4 in the T2DM obesity group and the T2DM non-obesity group were higher than those in the control group, and those in the T2DM obesity group were higher than those in the T2DM non-obesity group (P < 0.05). The level of β-arrestin2 in the T2DM obesity group and the T2DM non-obesity group was lower than that in the control group, and that in the T2DM obesity group was lower than that in the T2DM non-obesity group (P < 0.05). The glycosylated hemoglobin (HbA1c), total cholesterol (TC), low density lipoprotein cholesterin (LDL-C), triglyceride (TG), insulin resistance index (HOMA-IR) and BMI in the T2DM obesity group and the T2DM non-obesity group were higher than the control group, and those in the T2DM obesity group were higher than those in the T2DM non-obesity group (P < 0.05). However, there was no statistically significant difference in high-density lipoprotein cholesterol (HDL-C) between the three groups (P > 0.05). Serum FFA and RBP4 of T2DM patients was positively correlated with HbA1c, TC, TG, LDL-C, HOMA-IR and BMI (r > 0,P < 0.05),  β-arrestin2 was negatively correlated with HbA1c, TC, TG, LDL-C, HOMA-IR and BMI (r < 0,P < 0.05). FFA, RBP4 and β-arrestin2 were not correlated with HDL-C (P > 0.05). Conclusion In T2DM patients, the serum levels of FFA, β-arrestin2 and RBP4 were all abnormally expressed, which may jointly promote the disease progression of T2DM.

73 Characteristics of thyroid hormone changes and their correlation with albumin in patients with schizophrenia

Objective To investigate the change characteristics of serum thyroid hormone and its correlation with albumin (ALB) in patients with schizophrenia. Methods Eighty patients with schizophrenia admitted by the outpatient department of Xi′an Mental Health Center were selected as the disease group, and forty-six healthy patients from Xi′an Friendship Hospital were selected as the control group. Serum levels of thyrotrophin (TSH), triiodinated thyroxine (T3), thyroxine (T4), free triiodinated thyroxine (FT3), free thyroxine (FT4), and reverse triiodothyronine (rT3) in two groups were detected by chemical radiometry. At the same time, the ALB level of patients in the disease group was detected by the end point method of bromocresol purple. Results TSH, FT4 and rT3 levels in the disease group were all higher than those in the control group, while T3 and FT3 levels were lower than those in the control group, with statistically significant differences (P < 0.05). There was no significant difference in T4 between the two groups (P > 0.05). There were no statistically significant differences in the effects of disease course and gender on thyroid hormone indexes (TSH, T3, T4, FT3, FT4, rT3, T3/rT3) in the disease group (all P > 0.05). In the disease group, the T3/rT3 ratio was negatively correlated with  ALB level (r = -0.262,P = 0.019). Conclusion In schizophrenia patients with thyroid dysfunction, rT3 elevation is the characteristic change, and the T3/rT3 ratio is negatively correlated with ALB level.

77 Analysis of clinical features and treatment measures of severe pneumonia after renal transplantation

Objective To investigate the clinical features and treatment measures of severe pneumonia after renal transplantation. Methods The clinical data of 31 cases of severe pneumonia after kidney transplantation in Renmin Hospital of Wuhan University from January 2017 to July 2019 were retrospectively analyzed. The clinical features were summarized, and the effects of rational use of antibiotics, timely adjustment of immunosuppressant use, combined with respiratory support, continuous renal replacement therapy (CRRT) and other comprehensive treatment measures were analyzed. Results The mean age of 31 patients with severe pneumonia after renal transplantation was (43.9±11.7) years old. In 31 cases, 17 cases (54.84% ) infected within 6 months and 14 cases(45.16%) occurred 6 months after the operation. The main initial symptoms included fever in 19 cases (61.29%), cough in 15 cases (48.39%), sputum in 8 cases (25.80%), dyspnea in 6 cases (19.35%), pharyngeal pain in 2 cases (6.45%) and chest tightness in 1 case (3.22%). Among the 31 patients, single pathogen infection accounted for 16 cases(51.61%), two or more pathogen infections accounted for 8 cases (25.81%), and unknown pathogen infection accounted for 7 cases (22.58%). Among the 31 patients, single pathogen infection accounted for 16 cases (51.61%), two or more pathogen infections accounted for 8 (25.81%), and unknown pathogen infection accounted for 7 cases (22.58%). After admission, 25 patients improved and 6 patients died. Conclusion Severe pneumonia usually occurs within 6 months after kidney transplantation, and the first symptoms are mostly fever. Bacterial infection and mixed infection are the main pathogens. Early identification of pathogens and combined treatment are the key to successful treatment.

81 Effect of arthroscopic debridement combined with platelet-rich plasma in the treatment of patellofemoral arthritis

Objective To investigate the efficacy of arthroscopic debridement combined with platelet-rich plasma in the treatment of patellofemoral arthritis. Methods A total of 106 patients with patellofemoral arthritis admitted to Xuzhou Cancer Hospital, Affiliated Hospital of Xuzhou Medical University, from April 2017 to April 2019, the patients were randomly divided into control group and observation group according to the method of random number table, 53 cases in each group. The control group was treated with arthroscopic debridement, and the observation group was treated with platelet-rich plasma on the basis of the control group. The visual analogue scale (VAS) score, Lysholm knee function scale (Lysholm) score and complication rate were compared before and after treatment. Results The VAS score of the observation group was lower than that of the control group after 5 weeks of treatment, and the difference was statistically significant (P < 0.05). After 5 weeks of treatment, pain, lameness, support, upper and lower floors, instability, joint locking, squatting, joint swelling, and total Lysholm score in the observation group were all higher than those before treatment, and the above indicators were all higher than the control group, with statistically significant differences (all P < 0.05). There was no significant difference in the incidence of complications between the two groups (P > 0.05). Conclusion Arthroscopic debridement combined with platelet-rich plasma for the treatment of patellofemoral arthritis can effectively alleviate pain and promote knee function recovery, with fewer complications and worth promoting.

85 Clinical study of cranial traction reduction combined with anterior cervical decompression and fusion in the treatment of lower cervical spine fracture and dislocation combined with articular process interlocking

Objective To investigate the clinical effect of cranial traction reduction combined with anterior cervical decompression and fusion in the treatment of lower cervical spine fracture and dislocation combined with articular process interlocking. Methods A total of 96 patients with fracture and dislocation of the lower cervical vertebra combined with articular process lock admitted to the 8th Medical Center of PLA General Hospital from January 2015 to January 2018 were selected. According to the random number table method, all patients were divided into control group and study group with, 48 cases in each group. The study group was treated with skull traction reduction combined with anterior cervical decompression and fusion, while the control group was treated with anterior pry and fixation. The clinical efficacy, patient recovery, spinal cord nerve function and perioperative indexes of the two groups were compared, and postoperative complications of the two groups were recorded. Results The total effective rate of the study group was significantly higher than that of the control group (P < 0.05). The scores of sensory function and motor function in the two groups increased one year after operation, and the score in the study group were higher than those in the control group (P < 0.05). The operation time of the study group was shorter than that of the control group, and the amount of bleeding during the operation was less than that of the control group (P < 0.05). The improvement of American spinal cord injury association (ASIA) grading of the study group was significantly better than that of the control group (P < 0.05). The incidence of complications in the study group was significantly lower than that in the control group (P < 0.05). Conclusion The curative effect of  cranial traction reduction combined with anterior cervical decompression and fusion in treatment of  lower cervical spine fracture and dislocation combined with articular process interlocking is remarkable, it can effectively improve the spinal nerve function, and reduce the incidence of complications.

89 Effect of PVP on elderly patients with osteoporotic thoracolumbar fracture and its effect on bone mineral density and osteocalcin

Objective To explore the therapeutic effect of percutaneous vertebroplasty (PVP) on elderly patients with osteoporotic thoracolumbar fracture and its effect on bone mineral density and osteocalcin. Methods A total of 82 elderly patients with osteoporotic thoracolumbar fractures who were treated in Lishui People′s Hospital from March 2017 to April 2018 were selected and divided into the observation group (43 cases) and the control group (39 cases) according to the random number table. The control group received conservative treatment, while the observation group received PVP treatment, the clinical efficacy, vertebral compression rate, Cobb angle, bone mineral density, osteocalcin level and daily activity ability of the two groups were compared. Results The total effective rate of the observation group was significantly higher than that of the control group (P < 0.05). Vertebral compression rate and Cobb angle in the observation group were significantly lower than those before treatment and in the control group (P < 0.05). The vertebral compression rate in the control group was significantly lower than that before treatment (P < 0.05), but the Cobb angle had no significant difference (P > 0.05). The bone mineral density and osteocalcin levels in the two groups were significantly higher than those before treatment (P < 0.05), and the bone mineral density and osteocalcin levels in the observation group were significantly higher than those in the control group (P < 0.05). The daily activity ability of the patients in the observation group was better than that in the control group and before treatment (P < 0.05). Conclusion PVP has a good clinical effect on elderly patients with osteoporotic thoracolumbar fracture, it can significantly improve the healing of vertebral body, promote vertebral fusion, improve bone mineral density, osteocalcin and daily activity ability.

92 Cognition investigation of GnRH-a in combination with add-back therapy in ethnic minority patients with uterine leiomyoma in Xinjiang District and discussion the new model of health education

Objective To survey cognition level of gonadotrophin releasing hormone agonist (GnRH-a) in combination with add-back therapy in ethnic minority patients with uterine leiomyoma in Xinjiang District and discuss the new model of health education. Methods From January 2017 to December 2018, 80 ethnic minority cases with uterine leiomyoma received GnRH-a combination with add-back therapy in the First Affiliated Hospital of Xinjiang Medical University were collected as observational objects. According to the random number table method, they were divided into control group and observation group, with 40 cases in each group. Control group was given conventional health education, while observation group was given multi-dimensional medical care integration mode health education. Cognitive scores of GnRH-a combined with add-back therapy, levels of hormone, adverse reactions and levels of bone mineral density (BMD) were compared between two groups. Results Before health education, there were no statistically significant differences in the cognitive scores of GnRH-a combination with add-back therapy between two groups (P > 0.05). After health education, the cognitive scores of GnRH-a combination with add-back therapy in both groups were increased, and observation group was higher than control group, with statistically significant differences (P < 0.05). The levels of follicule-stimulating hormone (FSH), lutropin (LH), estradiol (E2) was decreased in both groups, and the levels of FSH, LH in observation group was lower than those in control group, while the level of E2 in observation group was higher than that in control group, with statistically significant differences (all P < 0.05). The BMD levels of 1-4 lumbar spine (L1-L4) and femoral head in both groups were decreased, and observation group was higher than control group, and the differences were statistically significant (all P < 0.05). The incidence of adverse reactions in observation group were lower than that in control group, and the differences were statistically significants (P < 0.05). Conclusion Ethnic minority patients with uterine leiomyoma in Xinjiang District has low level of cognition of GnRH-a combined with add-back therapy. And multi-dimensional medical care integration mode health education can increase cognition level, improve levels of sex hormones, reduce incidences of adverse reactions and has less influences on bone loss.

96 Effect evaluation of hysteroscopic niche resection and laparoscopic repair with hysteroscopy in the treatment of post-cesarean scar diverticulum

Objective To investigate the efficacy of laparoscopic repair with hysteroscopy and hysteroscopic niche resection in the treatment of post-cesarean scar diverticulum. Methods From January 2016 to November 2018, 42 cases with post-cesarean scar diverticulum in the Affiliated Changzhou NO.2 People′s Hospital of Nanjing Medical University were selected. According to the surgical methods, they were divided into the hysteroscopic niche resection group and the laparoscopic repair with hysteroscopy group, with 21 cases in each group. The hysteroscopic niche resection group was performed simple hysteroscopic niche resection, while the laparoscopic repair with hysteroscopy group was performed laparoscopic repair with hysteroscopy. The operation time, intraoperatve blood loss, hospital stay length and hospital expenses and postoperative pregnancy were compared between two groups. Results The operation time, intraoperatve blood loss, hospital stay length, hospital expenses of the hysteroscopic niche resection group were lower than those of the laparoscopic repair with hysteroscopy group, and the differences were statistically significant (P < 0.05 or P < 0.01). There  was no significant difference in the clinical effect at 1 month after surgery between two groups (P > 0.05). The clinical effect at 3 and 6 months after surgery of the laparoscopic repair with hysteroscopy group were higher than those of the hysteroscopic niche resection group, and the differences were statistically significant (P < 0.05). Postoperative pregnancy of the laparoscopic repair with hysteroscopy group were higher than those of the hysteroscopic niche resection group. Conclusion Both surgical methods have achieved certain clinical effect in the treatment of post-cesarean scar diverticulum.The laparoscopic repair with hysteroscopy of PCSD is safer and more effective, has more satisfactory menstrual recovery, should widely use to treat patients with PCSD.

100 Analysis of etiology, characteristics and risk factors of dyspnea in 1135 neonates

Objective To analyze the common etiology, clinical characteristics and risk factors of dyspnea in neonatal period. Methods From January to December 2018, 2270 neonates admitted to Neonatal Medical Center, Huai′an Maternal and Child Health Hospital of Jiangsu Province were selected as the study subjects. According to the occurrence of dyspnea, they were divided into dyspnea group(1135 cases) and control group(1135 cases). The etiology of dyspnea neonates was analyzed, and the incidence of dyspnea neonates with different gestational ages and birth weights were compared, as well as the univariate and multivariate analyses of dyspnea neonates. Results The incidence of neonatal dyspnea was the highest in the respiratory system, in which neonatal pneumonia was the main etiology and neonatal respiratory distress syndrome was the second etiology. The incidence of dyspnea in neonates with different gestational ages and birth weights were significantly differents (P < 0.01). And gestational age, cesarean section, intrauterine distress, maternal anemia, asphyxia were the risk factors for neonatal dyspnea (P < 0.01). Conclusion Dyspnea is caused by many etiology. Early detection, early diagnosis and early treatment are of great significance to improve the cure rate of neonatal diseases, reduce the length of hospital stay, lower the mortality, and reduce the incidence of sequelae.

104 Relationship between serum endothelin-1, von willebrand factor and Tei index and pulmonary artery systolic pressure in neonates with hypoxic pulmonary hypertension

Objective To investigate the relationship between serum endothelin-1 (ET-1), von willebrand factor (vWF), Tei index and pulmonary artery systolic pressure (PASP) in neonates with hypoxic pulmonary hypertension (HPH). Methods From January 2016 to January 2019, 113 cases with HPH neonates who were admitted to the People′s Hospital of Langfang City of Hebei Province (“our hospital” for short) were selected. They were divided into mild group (n = 37), moderate group (n = 38) and severe group (n = 38), according to the level of PASP. In the same period, 39 cases with healthy neonates in our hospital were selected as control group. The levels of serum ET-1 and vWF, arterial blood gas analysis index, Tei index and PASP in each group were compared. Pearson correlation analysis was used to analyze the correlation between serum ET-1, vWF levels, Tei index and PASP. Results The serum vWF, ET-1 levels, Tei index and PASP in four groups showed statistically significant differences (all P < 0.01). The serum vWF, ET-1 levels, Tei index and PASP in mild group, moderate group and severe group were higher than those in control group, and the differences were statistically significant (all P < 0.05). Arterial blood gas analysis index in four groups showed statistically significant differences (all P < 0.01). SaO2, PaO2 in mild group, moderate group and severe group were lower than those in control group, PaCO2 in mild group, moderate group and severe group were higher than that in control group, and the differences were statistically significant (all P < 0.05). Pearson correlation analysis showed that serum ET-1 and vWF levels and Tei index of HPH neonates were positively correlated with PASP (r = 0.839, 0.866, 0.657, P < 0.05). The serum ET-1 and vWF levels were positively correlated with Tei index (r = 0.530, 0.618,P < 0.05). Conclusion The changes of ET-1,vWF levels and Tei index are closely related to HPH newborns. Clinically, serum ET-1 and vWF levels can be detected to help evaluate the changes in cardiac function and disease progression of children.

108 The relationship between retinal ganglion cell complex thickness and retinal nerve fiber layer thickness and its effect on neovascular glaucoma

Objective To research the relationship between the thickness of retinal ganglion cell complex (mGCC) and retinal nerve fiber layer (pRNFL) and its effect on neovascular glaucoma. Methods From May 2016 to May 2019, 40 cases with NVG patients receiving fourier domain optical coherence tomography (FD-OCT) examination in the First Affiliated Hospital of Kunming Medical University were selected as NVG group. In the same period, 40 cases with normal persons receiving FD-OCT examination were selected as normal group. The mGCC thickness and pRNFL thickness were compared between two groups. Pearson accumulates correlation analysis was used to analyze the correlation between the mGCC thickness and pRNFL thickness. And the diagnostic effectiveness for NVG was evaluated. Results The thickness of GCC-average, GCC-superior, GCC-inferior and the thickness of RNFL-average, RNFL-temporal, RNFL-superior, RNFL-nasal, RNFL-inferior in NVG group were lower than those in normal group, and the differences were statistically significant (all P < 0.05). Pearson accumulates correlation analysis showed that mGCC thickness at all sites were positively correlated with pRNFL thickness. mGCC thickness and pRNFL thickness at all sites have diagnostic effect on NVG (P < 0.01). Conclusion In the diagnosis of NVG, mGCC thickness can be used as an important supplementary measure of pRNFL thickness, which is worthy of application and promotion.

111 Study on quality standard of Mastitis Granules

Objective To establish the quality standard of Mastitis Granules. Methods Paeoniflorin content in Mastitis Granules was determined by high performance liquid chromatography (HPLC). Agilent EClipse SB-C18 column (4.6 mm×250 mm, 5 μm) was used as the column, methanol-0.05 mol/L potassium dihydrogen phosphate solution (40∶60) was used as the mobile phase, the volume flow rate of 1.0 mL/min, with the detection wavelength of 230 nm. The colomn temperature maintained at 30℃. The injection volume was 10 μL. The identification of Cyperus rotundus L., Angelica sinensis and Paeoniae Radix Rubra in Mastitis Granules were determined by thin-layer chromatography (TLC). The limits of heavy metals and arsenic salts in Mastitis Granules were determined by heavy metal test method and arsenate test separately. Results The content of paeoniflorin was determined by HPLC. The paeoniflorin in the range of 5.01-501 μg/mL(r = 0.9996) showed a good linear relationship with the peak area. The average recovery rate was 99.92%, RSD = 1.26% (n = 6). The spots of TLC samples were clear and there was no negative interference, the limit tests of heavy metals and arsenic salts in Mastitis Granules were in accordance with the regulations. Conclusion The results of determination by HPLC are accurate, sensitive and reproducible. TLC method is simple and easy to operate, with short test period and clear and reliable results. The current pharmacopoeia method for heavy metals and arsenic salts can be used for the limit examination of heavy metals and arsenic salts in Mastitis Granules. The above methods provide a basis for establishing the treatment standard of Mastitis Granules.

Objective To investigate clinical effect of Total Glucosides of Paeony Capsules combined with Hydroxychloroquine Sulfate in the treatment of Sjogren′s syndrome (SS). Methods One hundred SS patients admitted to Anqing Hospital Affiliated to Anhui Medical University from September 2015 to March 2019 were divided into control group (50 cases) and observation group (50 cases) according to random number table method. Control group was treated with Hydroxychloroquine Sulfate, while observation group was treated with Total Glucosides of Paeony Capsules combined with Hydroxychloroquine Sulfate. Both groups were treated for 3 months. Clinical effect of two groups was evaluated and compared. The dry mouth score, dry eye score, fever score, joint pain score, erythrocyte sedimentation rate (ESR), immunoglobulin G (IgG) and C reactive protein (CRP) level were compared between two groups before and after treatment. The occurrence of adverse reactions during the treatment was recorded. Results Total clinical effective rate of observation group was significantly higher than that of control group, there was a significant difference between two groups (P < 0.05). After treatment, the scores of dry mouth, dry eye, fever and joint pain in two groups were significantly lower than those before treatment (P < 0.05), while scores of dry mouth, dry eye, fever and joint pain in observation group were significantly lower than those in control group (P < 0.05). ESR, IgG and CRP in two groups were significantly lower than those before treatment (P < 0.05), while ESR, IgG and CRP in observation group were significantly lower than those in control group (P < 0.05). During the treatment period, there was no significant difference between two groups (P > 0.05). Conclusion Total Glucosides of Paeony Capsules combined with Hydroxychloroquine Sulfate in the treatment of SS has significant clinical efficacy, which can significantly improve clinical symptoms, and has good safety.

Objective To investigate the effect of Levetiracetam on children with febrile seizure (FS) and its effect on the expression of neuron-specific enolase (NSE) and S-100 Mongolian protein (S-100β) in serum. Methods From November 2017 to April 2019, 70 children with FS in Department of Pediatrics, Changxing People′s Hospital of Zhejiang Province were selected as the study objects, According to the random number table method, the patients were divided into control group and observation group, 35 cases in each group. The control group was treated with oral Diazepam Tablets, while the observation group was treated with oral Levetiracetam Tablets. The course of treatment was set as 3 d. After one course of treatment, the patients were followed up for 6 months to 1 year. The recurrence rate of convulsion and incidence of adverse reactions were compared between the two groups. NSE and S-100 were measured before and after treatment. Results During the study and follow-up, 5 patients in the control group dropped out due to active withdrawal, poor compliance and follow-up release, while 3 patients in the observation group dropped out. In order to maintain the quantity balance, 30 patients were eventually included in each group. Follow-up for 6 months to 1 year showed no statistically significant difference in the recurrence rate of convulsion between the two groups (P > 0.05). The incidence of adverse reactions in the observation group was lower than that in the control group, and the difference was statistically significant (P < 0.05). Before treatment, there was no significant difference in NSE and S-100 between the two groups (P > 0.05). NSE and S-100 in both groups were lower than those before treatment, and NSE and S-100 in the observation group were lower than those in the control group, with statistically significant differences (all P < 0.05). Conclusion Levetiracetam can effectively prevent FS recurrence, and its relapse prevention effect is equivalent to that of Diazepam. With high clinical safety and less adverse reactions, it can effectively improve the level of cerebral nerve factors NSE and S-100 in children with FS, and has a good effect on brain protection.

123 Clinical study of Liqi Huoxue Decoction combined with Western medicine in the treatment of chronic prostatitis with the symptom of stagnation of qi and blood stasis

Objective To explore the clinical effect of Liqi Huoxue Decoction combined with Western medicine on chronic prostatitis (CP) with stagnation of qi and blood stasis. Methods A total of 92 CP patients admitted to Wuxi Third People′s Hospital from April 2018 to March 2019 were divided into study group (46 cases) and control group (46 cases) by random number table method. The control group was treated with conventional Western medicine, while the study group was treated with Liqi Huoxue Decoction on the basis of the control group. The curative effect, traditional Chinese medicine symptom score, inflammatory factor levels and the incidence of adverse reactions were compared between the two groups. Results After 8 weeks of treatment, the symptom scores of perineum pain, post urine drip, perineum distention, urination tingling and the levels of interleukin-8 (IL-8), interferon-γ (IFN-γ) and tumor necrosis factor -α (TNF-α) in prostatic fluid in the two groups were lower than before the treatment, while the study group were lower than those in the control group, and the differences were statistically significant (P < 0.05). The total clinical effective rate of the study group was significantly higher than that of the control group, and the difference was statistically significant (P < 0.05). There was no difference in the incidence of adverse reactions between the two groups (P > 0.05). Conclusion Liqi Huoxue Decoction combined with Western medicine is effective in the treatment of CP patients with the symptom of stagnation of qi and blood stasis. It can rapidly improve the clinical symptoms of the patients, reduce the levels of inflammatory factors in the body, and the drug is safety.

127 Revaluation of traditional Chinese medicine in the treatment of peptic ulcer by meta-analysis

Objective To comprehensively summarize the meta-analysis on peptic ulcer treated with traditional Chinese medicine (TCM), and to evaluate the methodological quality of meta-analysis and evidence quality of outcomes. Methods Eight databases including CNKI, Wanfang, VIP, OVID, CBM, PubMed, Embase, Cochrane Library were searched to collect the published meta-analysis on peptic ulcer treated with TCM. The time was from the inception to March 2019. Two researchers conducted literature screening and data extraction independently. The methodological quality of meta-analysis was assessed by using AMSTAR2 tool. The evidence quality was evaluated by using GRADE system. Results Finally, 11 meta-analysis were included, including 38 outcomes. The results of AMSTAR showed that 2 meta-analysis had moderate methodological quality, 3 had low quality, and 6 had very low quality. Results of GRADE showed that 21 outcomes were low quality evidence, 14 outcomes were very low quality evidence, and only 3 outcomes were moderate quality evidence. Conclusion The overall methodological quality of meta-analysis on peptic ulcer treated with TCM is low, and the evidence quality of outcome is generally low, indicating low reliable results. High quality meta-analysis should be conducted in the future for further verification.

132 Clinical experience of the treatment of insomnia from Shaoyang

Insomnia is a common clinical disease, which seriously affects the quality of life of patients. Traditional Chinese medicine calls it “unswerving”, and currently there are more side effects in the treatment of insomnia with western medicine. Professor Liu Longtao has extensive experience in the treatment of this disease. He draws the essence from Treatise on Febrile Diseases and Nei Jing, and combined with clinical experience, he believes that the pathogenesis of insomnia is closely related to the failure of Shaoyang Shuji operation, and should be treated from Shaoyang. And the using of Xiaochaihu Decoction addition and subtraction, the main prescription of Shaoyang disease, to reconcile Shaoyang can achieve good clinical results, which is worth promoting. The author is fortunate to learn from the teacher and have gained a lot. Now the author discusses Professor Liu′s experience of treating insomnia from Shaoyang.

136 Discussion on professor Wang Youpeng′s experience in treating cough variant asthma from the perspective of treatment before symptoms arising

Cough variant asthma is a common clinical disease in pediatrics at present. Its pathogenesis is complex, rapid evolution, long course of disease and lingering, which requires the timeliness and effectiveness of clinical treatment. The theory of “treatment based on syndrome differentiation” is initiated in Neijing, which inspired us to give predictive prevention and treatment on the basis of traditional Chinese medicine syndrome differentiation and treatment. Professor Wang Youpeng, starting from the theory of “syndrome first and treatment first”, thinks that the focus of the occurrence and development of the disease lies in the pathological factors throughout the disease-wind, phlegm and blood stasis. In the treatment, Professor Wang Youpeng advocates that the syndrome should be recognized before the syndrome, and the medicine should be given before the syndrome. It is suggested that we should master the objective factors of the disease, grasp the regular progress of the disease after it acts on the body, deduce its transmission and transformation, make active discovery and prediction, achieve early intervention and control, strengthen the traditional Chinese medicine which is conducive to cutting off the development of pathogenic factors, do the treatment before the disease and medicine before syndrome, which provides a new idea for the treatment of cough variant asthma.

139 Discussion on pathogenesis of ischemic white matter lesions from sweat pore theory

Based on sweat pores, this paper elucidates the traditional Chinese medicine pathogenesis of ischemic white matter lesions. Based on the theory of sweat pore, this paper analyzes the physiological status of sweat pores, qi and body fluid, the main pathogenesis of opening and closing function of sweat pore of ischemic white matter lesions, and the pathogenesis of different stages. The opening and closing function of sweat pore is closely related to the function of the neurovascular unit. The pathogenesis of ischemic white matter lesions mainly includes too much or too little opening of sweat pore, and qi stagnation, phlegm, excessive fluid, blood stasis, and toxin can be aggregated in sweat pore. So the sweat pore theory can elucidate the etiology and pathogenesis of ischemic white matter lesions.

142 Meta analysis of the effect of traditional Chinese medicine on knee osteoarthritis

Objective To systematically evaluate the clinical efficacy of traditional Chinese medicine in the treatment of knee osteoarthritis (KOA) by evidence-based medicine. Methods CNKI, WanFang Data, VIP, PubMed databases were searched by computer, and the randomized controlled trials (RCT) on the treatment of knee osteoarthritis in traditional Chinese medicine were collected. The search time was from the establishment of the database to December 2018. After 3 researchers independently screened the literature, extracted data and evaluated the risk of bias in the included studies, the RevMan 5.3 software was used for analysis. Results A total of 31 RCT were included, with a total of 3140 cases. Meta analysis showed: compared with the comprehensive treatment of Western medicine, the comprehensive treatment of traditional Chinese medicine had a better overall effective rate [OR = 4.47, 95%CI (3.57, 5.59), P < 0.000 01]. Meta analysis results of the comprehensive treatment of traditional Chinese medicine compared with the four subgroups showed that the internal and external treatment of cartilage protectant [OR = 5.62, 95%CI (2.52, 12.52), P < 0.0001], oral cartilage protectant [OR = 4.06, 95%CI (2.17, 7.61), P < 0.0001], oral NSAIDs [OR = 4.46, 95%CI (3.43, 5.80), P < 0.000 01], external use of chondroprotective agents and NSAIDs [OR = 4.04, 95%CI (1.66, 9.85), P = 0.002]. In the four subgroups, the effect of traditional Chinese medicine comprehensive treatment was the best, and the difference was statistically significant. Conclusion The comprehensive treatment of knee osteoarthritis by traditional Chinese medicine has good curative effect, which is worthy of clinical promotion.

149 Experience of Professor Xuan Guowei in the treatment of children Henoch-Schonlein purpura nephritis

Henoch-Schonlein purpura nephritis is the most common secondary nephropathy in children. It is lingering and difficult to cure. According to the characteristics of children′s constitution, Professor Xuan Guowei considers that this disease is mainly caused by insufficient natural endowment, and accumulation of heat toxin is the superficial cause. He put forward treatment ideas that “reinforcing the vital essence and strengthening the primordial qi, and solving disorders of blood system”. Combining characteristics of Lingnan region, the treatment mainly consists of nourishing yin and tonifying the kidney, combined with the Chinese herbs of clearing heat and cooling blood, activating blood and stop bleeding, removing toxin and stasis. Thus, he inventing Xueguanyan Decoction and pay attention to protect children′s spleen and stomach by using light and mild herbs to invigorating vital qi without stagnation and purgation without damage vital qi and get satisfied curative effects by judging and weighing deficiency and excess and differential diagnosis of stage of disease in clinical.

153 A case of POEMS syndrome misdiagnosed as chronic inflammatory demyelinating polyneuropathy

The clinical data of a case of retroperitoneal schwannoma in the Dongfang Hospital Attached to Beijing University of Traditional Chinese Medicine in 2018 were analyzed retrospectively in order to investigate the diagnosis and treatment of POEMS syndrome. Multiple peripheral neuropathy is not only the main clinical manifestation of POEMS syndrome patients, but also the first symptom. Almost all patients with this disease have the protein cell separation phenomenonin in the cerebrospinal fluid, which is easy to be misdiagnosed with chronic inflammatory demyelinating polyneuropathy (CIDP). Early treatment is beneficial to reduce the degree of disease damage and increase the survival rate, so early diagnosis is very important. This patient was misdiagnosed as CIDP for two and a half years. During this period, the patient developed multiple system symptoms, and went to many departments of many hospitals for treatment. While, the patient was not diagnosed correctly all the time. The patient was suffering physically and mentally, which cost a lot of human, material and financial resources. In this study, the case report combined with literature review can help guide clinicians to correctly diagnose patients with POEMS syndrome as soon as possible, so that patients can benefit the most.

157 One case of gastric metastasis from breast cancer

Breast cancer is one of the most common malignant tumors in women. Most early breast cancer do not have typical symptoms and signs. Cell or histopathology is used diagnose the disease after physical examination or screening and treated by comprehensive treatment methods such as surgery, chemoradiotherapy, targeting and endocrine therapy. The prognosis of breast cancer is affected by many factors, for example pathological type, stage of disease, molecular markers, and so on. Although early breast cancer has become one of the most effective solid tumors after comprehensive treatment, the survival time will be shortened rapidly once recurrence and metastasis occur. The common metastatic sites are bone, lymph node, soft tissue, lung, brain, liver, and so on. The gastric as the first site of metastasis is relatively rare and the clinical, imaging and endoscopic manifestations are lack of specificity, making it difficult to differentiate from primary gastric cancer. This paper reports a case of invasive lobular breast carcinoma with gastric metastasis and analyzes the clinical characteristics, diagnosis, treatment and prognosis of the disease combined with the literature, in order to improve the clinical understanding of gastric metastasis from breast cancer.

165 Research on efficiency of health resources allocation in national traditional Chinese medicine hospitals based on DEA model

Objective To understand the efficiency of allocation of health resources in Chinese medicine hospitals and offer suggestions. Methods Based on the health input and output data of Chinese medicine hospitals in 31 provinces in 2017, DEA analysis of health resources was carried out from both regional and provincial perspectives. Results The DEA analysis was effective in 3 provinces, was weak in 10 provinces, was ineffective in 18 provinces, and was effective in eastern, central and western regions. Conclusion The allocation efficiency of regional macro-health resources is reasonable, and the allocation efficiency of micro-health resources in provinces is poor. In most provinces, there are excessive investment and insufficient output. It is recommended to control the growth of hospital scale, integrate existing resources, balance regional health resources, improve hospital management and hospital efficiency.

168 Study on the equity of health resource allocation in Zhangjiakou from the perspective of health poverty alleviation

Objective To explore the rationality and fairness of health resource allocation in Zhangjiakou area from the perspective of health poverty alleviation. Methods From January 2013 to December 2016, the number of health care institutions, beds, health technicians, the number of practitioners (assistant doctors) and registered nurses in the region were collected, and the equity of the two dimensions of resident population and geographical area was evaluated by using the curve and Gini coefficient. Results In 2016, the Gini coefficient of resource allocation of health institutions in Zhangjiakou was 0.19, which was in a highly fair state. The Gini coefficient of bed resource allocation in health institutions was 0.28, which was fairer. Gini coefficient of 1000 nurses was 0.43, which was unfair. The Gini coefficient of health resources according to population allocation was 0.43 for registered nurses, 0.34 for doctors, 0.34 for health technicians, 0.28 for beds and 0.19 for health institutions, the allocation of nurses resources was slightly unfair. The average number of medical institutions was 1.04 per 1000 square kilometers, and the Gini coefficient of bed resource allocation was 0.55, which was very unfair; the average number of health technicians was 4.03 per 1000 square kilometers, and the Gini coefficient of professional technicians was 0.61, which was highly unfair. The average number of doctors per thousand square kilometers was 1.49, and the Gini coefficient of doctors per thousand square kilometers was 0.56, which was very unfair. The average number of nurses per thousand square kilometers was 0.12, and the Gini coefficient of nurses per thousand square kilometers was 0.63, which was highly unfair. Except for health institutions the allocation of resources was very unfair. From 2013 to 2016, the Gini coefficient of health resources allocated by population in Zhangjiakou area did not change much. Except for the Gini coefficient of medical and health institutions fluctuated greatly in 2015 and decreased to the previous level in the next year, the Gini coefficient showed a declining trend. The Gini coefficient of doctors according to population distribution showed a slightly higher increase trend, the average Gini coefficient of doctors according to population distribution was 0.27, which was fairer. Gini coefficient of health resources allocated by region was 0.53, showed a very unfair trend. Gini coefficient of nurses decreased significantly compared with 2016, and that of health technicians decreased suddenly in 2014, and increased steadily to the previous level in the next year. The Gini coefficient of health institutions was basically stable, and the Gini coefficient of beds increased slightly. Therefore, the Gini coefficient of health resources allocated by population was better than that allocated by region. Conclusion In recent years, the distribution of medical and health resources in Zhangjiakou has been unfair, the proportion of medical and health care is unbalanced, and the resources are significantly concentrated and unreasonable.

174 The problems and countermeasures of current infectious disease teaching

Due to the attack of new and old infectious diseases, which seriously affects people′s life and health, it is necessary to pay attention to the talent cultivation of infectious diseases. However, there are problems such as lags in teaching materials, lack of clinical cases, and insufficient teacher experience in the teaching, it influence the level of the talent cultivation of infectious diseases in China. This paper makes an in-depth analysis of these issues, and proposes strategies to strengthen the teaching resources, integrate teaching resources, integrate modern means, and pay attention to the teaching reform of infectious diseases such as cross-teaching, in order to help improve level of the talent cultivation of infectious diseases.

178 Relationship between serum IL-1β, IL-17 levels and the therapeutic effect in patients with first-episode schizophrenia

Objective To explore the relationship between serum interleukin-1β (IL-1β), interleukin-17 (IL-17) levels and the therapeutic effect in patients with first-episode schizophrenia. Methods A total of 200 patients with first-episode schizophrenia admitted in Hangzhou Seventh People′s Hospital from January 2017 to January 2019 were selected as the research group. Olanzapine was given to the patients on the day of admission and the treatment time was 4 weeks. At the same time, sixty healthy people were selected as the control group. The positive and negative symptoms scale (PANSS) scores were recorded before and 4 weeks after treatment. The serum IL-1β and IL-17 levels of two groups were measured by enzyme-linked immunosorbent assay. According to the effective rate of treatment, the research group was divided into group A (recovery, 30 cases), group B (significant progress, 60 cases), group C (improvement, 72 cases) and group D (ineffective, 38 cases). The serum IL-1β and IL-17 levels were compared before and after treatment in each group. Pearson correlation was used to analyze the correlation between serum IL-1β, IL-17 levels and PANSS score in patients. Results The levels of serum IL-1β and IL-17 in the research group were higher than those in the control group, and the differences were statistically significant (P < 0.05). The levels of serum IL-1β and IL-17 in the research group after treatment were significantly lower than those before treatment, and the differences were statistically significant (P < 0.05). Compared with before treatment in the same group, the serum levels of IL-1β in groups A, B and C were significantly reduced, and the serum levels of IL-17 in groups A and B were significantly reduced, the differences were statistically significant (P < 0.05). Further pairwise comparison, the differences in serum IL-1β levels of the four groups were statistically significant (P < 0.05), and the level in group A was the lowest. In addition, the serum IL-17 levels in group A were significantly lower than those in the other three groups, and the serum IL-17 levels in group B were significantly lower than those in group D, and the differences were statistically significant (P < 0.05). The levels of serum IL-1β and IL-17 were positively correlated with PANSS score(r = 0.386, 0.544, P < 0.05). Conclusion IL-1β and IL-17 are closely related to the onset of schizophrenia, which can reflect the therapeutic effect to a certain extent, and have an important value in evaluating clinical effect of patients.
