LI Qian1 JIANG Fujin2 WEN Rumin1 WANG Wentian3
Objective To investigate the clinical values of plasma miRNA-21 and miRNA-142-5p expression levels in the diagnosis of renal cell carcinoma (RCC). Methods Blood samples of patients with RCC and bebign renal disease from July 2015 to October 2017 in the Second People′s Hospital of Huai′an were collected, there were 55 cases and 49 cases respectively. The expression levels of plasma miRNA-21 and miRNA-142-5p were determined by fluorescence quantitative PCR in these patients. The correlations between the two miRNAs levels in plasma and clinicopathological characteristics of patients with RCC were analyzed. The diagnostic performance of each miRNA to distinguish RCC from benign renal disease was evaluated by the receiver operating characteristic (ROC) curve analysis. Results The levels of plasma miRNA-21 and miRNA-142-5p in RCC patients were higher than those in benign renal disease (P < 0.01). There was no significant correlation between plasma miRNA-142-5p levels and clinicopathological characteristics of RCC patients (P > 0.05), whereas the expression levels of plasma miRNA-21 was correlated with clinical stages (P < 0.05). There was no significant correlation between the expression of miRNA-21 and miRNA-142-5p in the plasma of 55 patients with RCC (P > 0.05). The area under ROC curve of the two miRNAs were 0.826 (95%CI:0.794-0.912) and 0.805 (95%CI:0.744-0.919). The sensitivity and specificity were 81.8% and 83.7% for miRNA-21, and 80.0% and 77.6% for miRNA-142-5p, respectively, for diagnosing RCC. And the sensitivity was increased to 87.3% with miRNA-21 in combination with miRNA-142-5p, despite slightly decreased specificity which was 75.5%. Conclusion These findings demonstrated that plasma miRNA-21 and miRNA-142-5p may serve as a promising biomarker in the diagnosis of RCC.

23 Study advance in the relationship between Parkinson′s disease and melatonin
HU Yingying WANG Xijin
Parkinson′s disease is a chronic progressive neurodegenerative disease. The main clinical manifestations include motor syptoms such as bradykinesia, tremor, rigidity, and postural instability, as well as non-motor symptoms, such as impaired olfaction, sleep disorder, cognitive impairment, and emotional disorder. Melatonin is a hormone synthesized mainly by the pineal gland, regulating the circadian rhythm. Besides it acts as a free-radical scavenger, with neuroprotective, anti-inflammatory, and antioxidant properties. The unbalance of pineal melatonin synthesis disposes the body to a variety of neurodegenerative diseases, including Alzheimer's disease and Parkinson′s disease. Parkinson′s disease is due to the impairment of melatonin concentration and the expression of its receptor in the brain. Exogenous melatonin treatment improves its motor symptoms. Besides, it also shows effects on non-motor symptoms, such as sleep disorders, congnitive disorders, anxiety and depression. The aim of this review is to summarize the knowledge about the potential role of the melatoninergic system in the pathogenesis and treatment of Parkinson′s disease.

27 The research progress of association between Helicobacter pylori and inflammatory bowel disease
CHENG Yi′an TAN Shiyun LI Ming
Inflammatory bowel disease (IBD) is a chronic inflammatory disease affecting intestinal tract. Existing researches suggest that environment, genetics, immunity and infection may be involved in its pathogenesis. Helicobacter pylori (H.pylori) is a common human pathogen, which is associated with a variety of gastric and extra-gastric diseases. In recent years, a lot of data show a negative correlation between them, and studies find that this association may be related to the immune regulation characteristics of H.pylori or other confounding factors such as antibiotics, sulfasalazine and environment. In addition, there are cases of newly diagnosed IBD after the eradication of H.pylori and it may trigger Crohn disease by affecting the balance between Th1 and Th2 immune response. The association between H.pylori and IBD as well as the mechanism are disscussed in this paper, and the article also reviews the development of IBD after the eradication of H.pylori.

31 Research progress on the IL-17 and pathogenesis of dengue virus infection
ZHENG Yuanfei LIU Fenghui CHEN Weifeng WU Huiwen ZHANG Yunqin ZHOU Bingxua ZHENG Biying
Dengue virus is transmitted by Aedes mosquito and causes dengue fever, dengue hemorrhagic fever and dengue shock syndrome. Early studies have found that dengue virus infection causes cytokines to be oversecreted, leading to cytokine storms. Different cytokines play different roles in dengue virus infection, aggravate or inhibit the progression of disease, and affect the outcome of the disease. As an important pro-inflammatory cytokine, interleukin-17 (IL-17), secreted by Th17 cells, can induce the aggregation and proliferation of neutrophils, thus effectively mediating the inflammatory response of tissues, which is related to the pathogenesis of various viral infections. The immune mechanism of dengue virus infection and cytokine storm IL-17 and the relationship between IL-17 and dengue virus infection were reviewed.

Results Gram-negative bacilli, gram-positive cocci and fungi accounted for 58.38%, 23.24 % and 18.38% respectively, and it is most common with Pseudomonas aeruginosa, coagulase negative staphylococci and white candidiosis. The antibiotic resistance rate of gram-negative bacilli was lower than 50.00% in Cephalosporin, and the drug resistance rate was close to 0.00%. The drug resistance rate of gram-positive cocci to quinolones was more than 50.00%, while the resistance rate of Cephalosporin antibiotics was close to 100.00%, but the drug resistance rate of Vancomycin was only 0.00%. The drug resistance rate of Candida Albicans was close to 0.00%. Conclusion Compared with western hospitals, the main pathogenic bacteria from AIDS patients with pulmonary infection in our hospital have low resistance to common antibiotics. But the monitoring of multidrug resistant bacteria should be strengthened, the role of combined treatment of traditional Chinese medicine and western medicine treatment in the prevention and control of hospital infection is emphasized.

170 Relationship of plasma levels of Aβ1-42 and Tau protein with cognitive impairment in patients with severe Parkinson′s disease
DU Jie WEI Liang ZHAO Xi ZHAO Lige LIU Fengbing
Objective To investigate the relationship of plasma levels of β-amyloid 1-42 (Aβ1-42) and Tau protein with cognitive impairment in patients with severe Parkinson′s disease (PD). Methods Sixty-four cases of severe PD that were treated in Department of Neurology , the People′s Hospital of Kaizhou District from April 2016 to April 2018 were enrolled. They were divided into three groups according to the scores of Montreal cognitive assessment scale (MoCA): severe cognitive impairment group (13 cases), mild-to-moderate cognitive impairment group (29 cases) and normal cognitive function group (22 cases). The levels of plasma Aβ1-42 and Tau protein were compared among the groups. Pearson correlation analysis was used to analyze the correlation between levels of plasma Aβ1-42 and Tau protein with the MoCA scores in PD patients. Results There were statistically significant differences in the levels of plasma Aβ1-42 and Tau protein among the three groups (P < 0.05). The level of plasma Aβ1-42 in severe cognitive impairment group was significantly lower than that in mild-to-moderate cognitive impairment group and normal cognitive function group, while the Tau protein level of severe cognitive impairment group was significantly higher than that in mild-to-moderate cognitive impairment group and normal cognitive function group; the plasma Aβ1-42 level in mild-to-moderate cognitive impairment group was significantly lower than that in normal cognitive function group while the Tau protein level was significantly higher than that in normal cognitive function group, the differences were significant (P < 0.05). Pearson correlation analysis showed that plasma Aβ1-42 level was positively correlated with MoCA score (r = 0.345, P < 0.05), and plasma Tau level was negatively correlated with MoCA score (r = -0.416, P < 0.05). Conclusion Level changes of plasma Aβ1-42 and Tau may be associated with cognitive impairment in PD patients, or they may be used as potential early predictors of cognitive impairment in PD patients.
