4 Effects of atmospheric particulate matter PM2.5 on bone metabolism in aged male rats

Objective To observe the effect of atmospheric fine particulate matter PM2.5 on bone mineral density and trabecular structure of aged male rats, and the effect of receptor activator of nuclear factor-kappa B ligand (RANKL)/osteoprotegerin (OPG) expression of function, and to explore PM2.5 effect on bone metabolism of aged rats. Methods A total of 20 male SD rats aged 19 months were divided into the cleaning group and the PM2.5 group according to the random number table method using a haze warehouse exposure system, with 10 rats in each group. They were kept in a clean warehouse and a haze warehouse respectively, and their weight was monitored weekly. After 12 weeks, femoral tissue was collected and bone mineral density of rats was detected. Bone morphology was observed by hematoxylin-eosin (HE) staining. The mRNA in bone marrow was extracted and the expression of RANKL/OPG was detected by Real-time PCR. Results The body weight of the two groups increased steadily with the extension of time, but there was no signifant difference in body weight between the two groups (P > 0.05). HE results showed that compared with the cleaning group, the bone trabeculae in the cancellous bone of the PM2.5 group were reduced, arranged sparsely and partially fractured. Compared with the clean group, bone mineral density, trabecular number, trabecular area and percentage of trabecular area in the PM2.5 group all decreased significantly, with statistically significant differences (P < 0.05 or P < 0.01). Real-time PCR results showed that compared with the clean group, OPG mRNA content in the PM2.5 group was decreased, while RANKL mRNA content was increased, with statistically significant differences (all P < 0.05). Conclusion Long-term exposure to PM2.5 may increase osteoclast activity by increasing the expression of osteoblasts RANKL and decreasing the expression of OPG, thereby reducing bone mineral density and bone trabeculae and aggravating osteoporosis in aged rats.

8 The expression and the clinical significance of monocarboxylate transporter 4 in colorectal cancer

Objective To explore the expression and the clinical significance of monocarboxylic acid transporters (MCTs) in colorectal cancer (CRC). Methods The expression status and clinical significance of MCT1-4 in CRC tissues were analyzed online by GEPIA2 database. Fifty-three cases of CRC tissue specimen wax blocks of Xuzhou Central Hospital in 2019 were selected, and the expression of MCT4 protein was detected by immunohistochemistry. The results of positron emission computed tomography (PET/CT) were obtained from the patients before operation. Results In the cancer genome atlas project (TCGA) database, only the expression of MCT4 mRNA in colon adenocarcinoma (COAD) tissues (275 cases) and rectal adenocarcinoma (READ) tissues (92 cases) were significantly higher than that in normal tissues, and the differences were statistically significant (P < 0.05). High expression of MCT4 was associated with poor disease-free survival (DFS) in patients with CRC (P < 0.05). Moreover, the expression of MCT4 protein in CRC tissue was significantly higher than that in normal tissue, and the difference was highly statistically significant (P < 0.01). For patients with high expression of MCT4, the standardized uptake value (SUV) of 18-fluorodeoxyglucose (18F-FDG) on PET/CT was higher than that of low expression of MCT4, and the difference was highly statistically significant (P < 0.01). Conclusion MCT4 plays an important role in the development of CRC and is expected to become a new therapeutic target.

12 Optimizating roasted technology of Aucklandiae Radix based on multi-index evaluation

Objective To optimize the processing technology of roasted Aucklandiae Radix by multi index comprehensive score combined with box-behnken design (BBD) response surface methodology. Methods The appearance character, volatile oil, costunolide and dehydrocostuslactone of the roasted Aucklandiae Radix were taking as the comprehensive evaluation indexes. The BBD response surface method was used to investigate the number of tile layers, paper simmering temperature and paper simmering time, select the best processing parameters, and verify the optimized processing process. Results The optimal processing technology of roasted Aucklandiae Radix was as follows:the wet Aucklandiae Radix was wrapped with a layer of straw paper, laying them in five layers at intervals, place them in a 100℃ oven and bake them for about 2 hours, and taking them out when they present brown. Conclusion The optimized processing technology of the paper-roasted Aucklandiae Radix is simple, feasible and stable, which can provide a basis for the setting of processing technology and standard specifications of the paper-roasted Aucklandiae Radix.

17 Pharmacokinetic study of genistein -loaded methoxy-terminated polyethylene glycol-lactic acid glycolic acid copolymer micelles after multiple dosing in rats

Objective To discuss the pharmacokinetic behavior of genistein (GEN) methoxy-terminated polyethylene glycol-lactic acid glycolic acid copolymer (MePEG-PLGA) micelles administered multiple times in healthy rats. Methods Twenty-four healthy Wistar rats were randomly divided into four groups according to their body weight, with 6 rats in each group, namely the GEN emulsion group and the GEN micelle low, medium and high dose groups. In each group, GEN emulsion and GEN micelles were injected into the tail vein several times, respectively, with the emulsion dose of 40 mg/kg and the micelle dose of 20, 40 and 60 mg/kg. The high performance liquid chromatography (HPLC) was used to analysis drug delivery in different time points after plasma GEN concentration. Results The differences in minimum blood drug concentration (Cmin), half-life (t1/2), area under the curve (AUC0-t), clearance rate (CL) and fluctuation coefficient (DF) between GEN emulsion group and GEN micelles group were statistically significant (P < 0.01). There were no statistically significant differences in the major pharmacokinetic parameters Tmax, t1/2, CL, apparent distribution volume (Vd) and DF in the GEN micelles groups with low, medium and high doses (P > 0.05). Compared with the low-dose group of GEN micelles, the maximum blood concentration (Cmax) and AUC0-t in the medium-dose group of GEN micelles were increased, while the Cmax and AUC0-t in the high-dose group of GEN micelles were increased compared with the low-dose and medium-dose group of GEN micelles, and the differences were highly statistically significant (all P < 0.01). The differences of AUC0-t, AUC0-∞, Cmax and average steady stateconcentration (Cav) in each dose group of GEN micelles increased with the increase of the dose (P < 0.01), and there was a linear relationship between AUC0-t, AUC0-∞, Cmax and Cav and the dose (r > 0.97). There were no significant differences in the major pharmacokinetic parameters Cmax, t1/2, AUC0-t, CL, Vd, MRT0-t between single and multiple doses at the same dose (P > 0.05). Conclusion GEN micelles can significantly delay the retention time of GEN in rats and improve the bioavailability. Moreover, there is a linear relationship between blood drug concentration and dose, and there is no accumulation in rats after repeated drug administration.

22 Effect of heat acclimation training on people with high intensity physical activity in high temperature and humidity environment

Objective To explore the effect of heat acclimation training on people with high intensity physical activity in high temperature and humidity environment. Methods From August 2014 to August 2018, 70 amateur long-distance runners in Xianshan Community participate in 5 km long-distance training were divided into the oservation group and the control group by simple random grouping method, with 35 in each group. The observation group was received heat acclimation training before training. The changes of interleukin-1β (IL-1β), γ interferon (IFN-γ), tumor necrosis factor-α (TNF-α), acute physiology and chronic health evaluationⅡ (APACHEⅡ), glasgow coma scale (GCS), troponin I (cTnI), urine creatinine (Cr), creatine kinase isoenzymes (CK-MB) level. The incidence of exertional heat stroke, multiple organ dysfunction syndrome and the organ involvement rate in the two groups were compared. Results Before training, there were no significant differences in the levels of IL-1β, IFN-γ and TNF-α in the two groups (P > 0.05). After training, the IL-1β, IFN-γ and TNF-α levels in the two groups were increased than those before training, and the observation group was lower than the control group, with statistically significant differences (P < 0.05). Before training, there were no significant differences in APACHEⅡ and GCS scores between the two groups (P > 0.05). After training, APACHEⅡ scores of the two groups were increased than those before training, GCS scores were decreased than those before training, and APACHEⅡ scores of the observation group were lower than those of the control group, and GCS scores were higher than those of the control group, the differences were statistically significant (all P < 0.05). Before training, there were no significant differences in cTnI, Cr and CK-MB levels between the two groups (P > 0.05). After training, cTnI, Cr and CK-MB levels in the two groups were increased than those before training, the observation group was lower than the control group, with statistically significant differences (all P < 0.05). The incidence of exertional heat stroke, multiple organ dysfunction syndrome and organ involvement in the observation group were lower than those in the control group (P < 0.05). Conclusion Heat acclimation training has a significant effect on high intensity physical activity in high temperature and high humidity environment. It can effectively improve the inflammatory response and reduce the incidence of multiple organ dysfunction syndrome. It is worth popularizing and applying.

26 Effect of hydrogen inhalation on lung injury induced by severe acute pancreatitis in rats

Objective To investigate the effect of hydrogen(H2) inhalation on lung injury in rats with severe acute pancreatitis (SAP). Methods Forty-eight healthy male SPF Wistar rats were selected, four groups (12 in each group) were divided into 4 groups by random number table method, sham operation group (SH group), severe pancreatitis group (SAP group), 1%H2 group, 2%H2 group. With the exception of SH group, the rat SAP model was made by injecting 5% sodium taurocholate into each group through retrograde cholangiopancreatine. H2 group 1% and H2 group 2% continuously inhaled H2 at the corresponding concentration during the process of model making and 24 h after successful model making. Blood gas analysis and oxygenation index (PaO2/FiO2) were calculated after 24 h of femoral arterial blood collection after modeling. The rats were then sacrificed to determine the lung wet-to-dry weight ratio (W/D) and the lung tissue injury score. The enzyme linked immunosorbent method was used for determination of levels of 8-hydroxyl deoxyguanosine (8-OHdG), toll-like receptor 4 (TLR-4), nuclear factor-κB (NF-κB), tumor necrosis factor-α (TNF-α), interleukin 1β (IL-1β), IL-6 in the lung tissues. Results Compared with SH group, the W/D ratio of SAP group, 1%H2 group, 1%H2 group were analyzed, the lung tissue damage rating, 8-OHdG, TLR-4, NF-κB, TNF-α, IL-1β, IL-6 and IL-1 increased significantly, PaO2/FiO2 decreased (P < 0.05). Compared with SAP group, W/D ratio, lung tissue damage rating, 8-OHdG, TLR-4, NF-κB, TNF-α, IL-1β, IL-6 and IL-1 in 1%H2 group and 2%H2 group significantly reduced, PaO2/FiO2 elevated (P < 0.05). There was no significant difference in the above indicators between the 1%H2 group and the 2%H2 group (P > 0.05). Conclusion Inhalation of H2 can reduce lung injury caused by severe acute pancreatitis, and the mechanism may be related to inhibition of inflammatory response and oxidative stress.

29 Clinical significance of soluble tumorigenic source 2 and brain natriuretic peptide in the diagnosis of heart failure with preserved ejection fraction

The prevalence of heart failure with preserved ejection fraction (HFpEF) is increasing. HFpEF is characterized by increased left ventricular diastolic blood pressure, and pathophysiological changes are myocardial hypertrophy, progressive myocardial fibrosis, and increased myocardial cell hardness. Myocardial fibrosis is closely related to HFpEF. At present, HFpEF diagnostic criteria are still controversial and used to mainly rely on echocardiographic to evaluate left ventricular diastolic sfunction and filling pressure. In recent years, studies have found that the sensitivity and specificity of brain natriuretic peptide and soluble tumorigenic source2 (sST2) are satisfactory in the diagnosis of HFpEF. This review starts with myocardial fibrosis and its detection methods and discusses the changes and significance of sST2 in HFpEF patients, the diagnostic value, prognostic judgment and clinical guidance of sST2 combined with BNP for HFpEF.

34 Research progress on the relationship between exosomes and tissue fibrosis

Tissue fibrosis is a pathological process in which the necrosis of parenchymal cells is reduced and the fibrous connective tissue is increased, which can lead to the destruction of tissue and organ structure and loss of function, even threaten the health and life of human. Exosomes are tiny vesicles that can be secreted by almost all cells, carrying intracellular mRNA, protein and other substances, mediating intercellular communication and participating in various biological processes. In recent years, there have been more and more studies on the relationship between exosomes and fibrosis. This article mainly reviews from the perspective of exosomes regulating fibrosis, as biological markers for diagnosing fibrosis and therapeutic targets.

37 Research progress on the effects and mechanisms of epithelial growth factor receptor on tight junction

Tight junction is widely distributed in various organs and tumor tissues of the body, and plays an important role in the injury and repair of the barrier tissues, as well as in the development of the tumor tissues. Epithelial growth factor receptor (EGFR) is also expressed in these tissues, and its activation affects the state changes of tight junction of barrier and tumor tissues. However, the effects are significantly different: activation of the EGFR signaling pathway can enhance tight junction in barrier tissues and inhibit their destruction. Whereas, it reduces the tight junction between tumor cells and promotes the discrete cell metastasis. Therefore, studying these detailed effects and mechanisms of EGFR signaling pathway on tight junction may provide new therapeutic strategies on several barrier injury-associated diseases and tumors.

41 Research and application of traditional Chinese medicine for thyroid diseases based on systems biology omics technology

Traditional Chinese medicine has unique advantages in treating thyroid diseases, especially in shortening the onset time, rapidly relieving symptoms, enhancing the efficacy of Western medicine, and reducing the toxic and side effects of Western medicine. However, the current research lacks innovation and characteristics of traditional Chinese medicine. Systems biology is a large science characterized by holistic research, which is similar to wholism concept of traditional Chinese medicine. This paper expounds the current situation of traditional Chinese medicine research on genomics, transcriptomics, proteomics and metabolomics of thyroid diseases, and puts forward the assumption and prospect of thyroid disease research from the perspective of omics technology. The omics technology based on the systems biology platform can be integrated with each other and applied to the research of traditional Chinese medicine, which can further broaden the research thought of traditional Chinese medicine for thyroid diseases.

45 Research progress on gastrointestinal dysfunction in sepsis with traditional Chinese medicine treatment

Sepsis is still a serious threat to human health disease in today′s society, it can lead to multiple organ dysfunction, gastrointestinal dysfunction is a common complication in patients with sepsis, concurrent gastrointestinal dysfunction can make deterioration or poor prognosis, patients in the intensive care unit time longer, obviously improve the cost of treatment, and reduce survival rate significantly. Therefore, active treatment of gastrointestinal dysfunction is of great significance for the development and prognosis of sepsis. Through literature review, this paper summarizes the etiology, pathogenesis and treatment methods of traditional Chinese medicine of sepsis gastrointestinal dysfunction, providing ideas for the treatment of sepsis gastrointestinal dysfunction.

48 The status quo of postoperative rehabilitation self-efficacy and influencing factors of patients after total knee arthroplasty surgery

Objective To investigate the status quo of postoperative rehabilitation self-efficacy in patients with total knee arthroplasty (TKA) and to analyze the influencing factors. Methods Using the convenience sampling method, a total of 110 patients with TKA who were treated in the General Hospital of Eastern Theater Command from December 2018 to June 2019 were selected as the research object. The Western Ontario and McMaster University Osteoarthritis Index (WOMAC) score and hip and knee self-efficacy for rehabilitation outcome scale (SER) were used for the investigation. Results A total of 104 valid questionnaires were recovered in this study, with an effective recovery rate of 94.5%. The total score of rehabilitation self-efficacy of TKA patients was (71.68±10.71) points 1 month after operation. Pain, stiffness, dysfunction scores and the WOMAC total score were negatively correlated with postoperative rehabilitation self-efficacy (r = -0.388, -0.306, -0.651, -0.617, P < 0.05). The total scores of rehabilitation self-efficacy of patients with supervised training or not, different comorbidities and preoperative course were statistically significant (P < 0.05). Factors such as preoperative WOMAC score, pain, with supervised training and the duration of preoperative disease were the influencing factors of rehabilitation self-efficacy (P < 0.05). Conclusion The overall level of postoperative rehabilitation self-efficacy of TKA patients is not good, and is affected by factors such as preoperative WOMAC score and course of disease.

52 The status quo of the application of the management model of patients with severe mental disorders in the “five in one” community in Jinshan District, Shanghai

How to effectively manage and control the serious mental disorders in the community, prevent the serious mental disorders from causing accidents, which are related to the health of the people, the establishment of safety, and the overall situation of social stability, are also important management issues facing the health administration and the government. To this end, Zhujing Town of Jinshan District explored the establishment of a departmental collaboration mechanism, unblocked a rescue channel, opened a helpline, established an emergency rescue team, and determined a “five-in-one” community management model for severe mental disorders in a permanent parent in order to carry out the management of severe mental disorders in the jurisdiction while manages the patients with severe mental disorders comprehensively and promotes the development of mental health in the jurisdiction effectively.

56 Comparison of the early stages of the development of independent student enrollment

Objective To explore the laws and similarities between the independent enrollment sources and the general enrollment sources in the early stages of school development after admission. Methods From September 2018 to August 2019, part of the independent admissions students (experimental group) and ordinary admissions students (control group) admitted by Beijing University of Chinese Medicine in 2017 were used as research objects. Campus cultural activities were compared with the four dimensions of social practice, psychological quality and personal development satisfaction score. Results In 2018, a total of 190 questionnaires were issued, and 176 were effectively recovered, with an effective recovery rate of 92.63%. While in 2019, 350 questionnaires were issued, and 316 were effectively recovered, with an effective recovery rate of 90.29%. In 2018, the interpersonal skills and relationship scores of the experimental group were higher than those of the control group, and the differences were highly statistically significant (P < 0.01). In 2019, the interpersonal communication ability, the relationship with the teacher, and the relationship with the classmates of the experimental group were higher than those of the control group, the differences were statistically significant (P < 0.05 or P < 0.01). In 2018, the participation in campus cultural activities score of the experimental group was higher than that of the control group, the difference was statistically significant (P < 0.05). In 2019, the views on social practice and participation in campus cultural activities of the experimental group were higher than those in the control group, and the differences were highly statistically significant (P < 0.01). In 2018, the positive emotion score of the experimental group was significantly higher than that of the control group, while the negative emotion score was lower than that of the control group, and the differences were highly statistically significant (P < 0.01). In 2019, the positive affective score of the experimental group was significantly higher than that of the control group, and the difference was highly statistically significant (P < 0.01). In 2018, the learning ability, innovation ability, practical hands-on ability, and adaptability satisfaction score of experimental group were significantly higher than those of the control group, the differences were statistically significant (P < 0.05 or P < 0.01). In 2019, the satisfaction scores of academic development, innovation ability and practical ability of the experimental group were higher than those of the control group, and the differences were statistically significant (P < 0.05). Conclusion As far as the surveyed items are concerned, the comprehensive quality of independent enrollment students is relatively higher than that of ordinary enrollment students in the early stages of university education.

60 Design and application of hanging type dressing change bracket

Objective To design the hanging type dressing change bracket and observe its application in dressing change of orthopedic patients. Methods From January to June 2018,80 cases with trauma patients who were admitted to Department of Orthopedics, Haikou People′s Hospital were enrolled. They were divided into the observation group and the control group, according to the random number table method, with 40 cases in each group. The observation group assisted by the self-designed hanging type dressing change bracket to assist the dressing change, and the control group was assisted by the medical staff to lift the injured limb dressing. The differences in comfort level, average duration of the doctor′s dressing change and the number of assistants needed were compared between the two groups. Results The comfort level scores of the observation group were lower than those of the control group and the difference was highly statistically significant (P < 0.01). There was no significant difference in average duration of the doctor′s dressing change between the two groups (P > 0.05). The doctors did not need an assistant when changing the dressing in the observation group, while the control group needed assistants (1-2 people) when changing the dressing, and the difference was highly statistically significant (P < 0.01). Conclusion The self-designed hanging type dressing change bracket assists the doctor to change the medicine for the patients with orthopedic lower limb injury, which can increase the comfort of the patient during dressing change and effectively save the labor of the medical staff.

63 Clinical study of Prostaglandin E combined with continuous renal replacement in the treatment of sepsis with acute  kidney injury

Objective To investigate the clinical effect of Prostaglandin E combined with continuous renal replacement therapy (CRRT) in sepsis with acute kidney injury (AKI). Methods A total of 83 patients with sepsis complicated with AKI who admitted to Hefei Binhu Hospital of Anhui Province from March 2015 to January 2019 were selected and they were divided into the control group (41 cases) and the study group (42 cases) according to the random number table method. The control group was treated with CRRT, and the study group was treated with Prostaglandin E on the basis of the control group. The clinical efficacy, inflammatory factors, kidney function indexes, clinical outcome and adverse reactions were compared between the two groups. Results The total clinical effective rate of the study group was significantly higher than that of the control group after 7 days of treatment (P < 0.05). There was no statistically significant difference in interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α) and high-sensitivity C-reactive protein (hs-CRP) between the two groups before treatment (P > 0.05). Compared with before treatment, Il-6, hs-CRP and TNF-α in the two groups decreased after 7 days of treatment, and the study group was lower than the control group (P < 0.05). There was no significant difference in blood urea nitrogen (BUN), serum creatinine (SCr) and cystatin (CysC) between the two groups before treatment (P > 0.05). Compared with before treatment, BUN, SCr and CysC were all increased in the two groups after 7 days of treatment, but the study group was lower than the control group (P < 0.05). The length of stay in the study group was shorter than that in the control group (P < 0.05), and there was no statistically significant difference in the case fatality rate and the incidence of adverse reactions between the two groups within 28 days (P > 0.05). Conclusion Prostaglandin E combines with CRRT in the treatment of sepsis complicated with AKI has remarkable curative effect. It can effectively alleviate kidney injury, improve the level of inflammatory factors and prognosis, and has good drug safety and high clinical application value.

67 Analysis of risk factors of renal function damage induced by anti-hepatitis B virus drugs

Objective To analyze the risk factors of renal function damage caused by anti-hepatitis B virus drugs. Methods A total of 100 cases of chronic hepatitis B (CHB) in the Fifth Affiliated Hospital of Zhongshan University from June 2015 to June 2016 were selected as the research objects. The sex, age and medication status of all patients were recorded, and the changes of estimated glomerular filtration rate (eGFR) in all patients during treatment was observed during treatment. The incidence of renal impairment was recorded in all patients during treatment. The single factor analysis of renal function damage caused by anti-hepatitis B virus drugs was analyzed. Logistic regression model was used to analyze the multi-factor analysis of renal function damage caused by anti-hepatitis B virus drugs. Results Compared with the initial treatment, the eGFR values of all patients decreased gradually after 1, 2 and 3 years of treatment (P < 0.05). According to the condition of kidney injury, the patients were divided into kidney injury group (34 cases) and non-kidney injury group (66 cases). Univariate analysis showed that age, eGFR level, anti-hepatitis B virus drugs were associated with renal impairment in patients with CHB (P < 0.05), but there was no correlation with gender (P > 0.05). Logistic regression model was included. The results showed that age ≥40 years old, eGFR < 90 mL/(min·1.73m2) and the anti-hepatitis B virus drug Adefovir Dipivoxil (ADV) were the risk factors of renal dysfunction in CHB patients (P < 0.05). Conclusion Age≥40 years old, eGFR level <90 mL/(min·1.73m2) and anti-hepatitis B virus drugs ADV are risk factors for renal function damage in CHB patients. Clinical can pay attention to the above situation to prevent the occurrence of renal injury.

71 Finite element analysis of proximal femoral nail anti-rotation in the treatment of non-anatomical reduction of basicervical femoral neck fracture in elderly

Objective To investigate the biomechanics of proximal femoral nail anti-rotation in the treatment of femoral neck basicervical fracture in elderly with three different reduction states by using finite element method. Methods Based on the CT data of the volunteers′ hips, the proximal femoral model was extracted and optimized by Mimics, Geomagic and other softwar, then assembled with proximal femoral nail anti-rotation in Solidworks software and segmented into simulated fracture lines. Three models of different reduction states (positive support fracture, neutral support fracture, negative support fracture) were obtained by moving fracture fragments. Finally, all the three models were imported into Abaqus software to set material parameters, boundary conditions, etc., and then the calculation was carried out to check the Von Mises nephogram of the model. Results The stress distribution of the positive support fracture end was the most uniform and the displacement of the fracture end was the least. The stress of negative support fracture end was concentrated and the displacement of the fracture end was the greatest. The stress distribution of neutral support fracture end was uniform and the displacement of the fracture end was larger. Conclusion The use of proximal femoral nail anti-rotation in the treatment of basicervical femoral neck fracture should avoid the negative support reduction state, and try to maintain the anatomical reduction or positive support.

76 Effects of long segment fusion and fixation on lower limb gait in patients with degenerative scoliosis

Objective To analyze the effects of long segment fusion fixation surgery on lower limb gait in patients with degenerative scoliosis (DS). Methods A retrospective analysis was performed to evaluate 49 patients with DS who underwent long segment fusion fixation surgery in Cangzhou Hospital of Integrated Traditional Chinese and Western Medicine, Hebei Province from January 2015 to June 2017. The surgery were observed before and after treatment of measuring spatiotemporal parameters (step size, step speed and step width), and the kinematic data (hip joint motion angle, knee joint motion angle and plantar pressure distribution), and measurement of coronal plane scoliosis angle (Cobb), lumbar lordosis angle (LL), sacral slope (SS), visual analogue scale (VAS) score of lumbago and lower limb pain, lumbar Oswestry disability index (ODI) score Results Compared with the preoperative results, the postoperative step size, step speed and hip joint motion angle of DS patients all increased, while the knee joint motion angle decreased, with statistically significant differences (all P < 0.05). There was no significant difference of the step width between the postoperative and the preoperative (P > 0.05). The preoperative ground reaction force (GRF) of DS patients showed a high single peak curve, and the postoperative GRF changed into a low level bimodal curve. Compared with preoperative results, the postoperative Cobb angle of DS patients decreased, spinal balance parameters of LL and SS increased, VAS score and ODI score decreased, and the differences were all statistically significant (all P < 0.05). Conclusion Long segment fusion fixation surgery can improve patients′ gait parameters, such as step size and step speed, by improving patients′ spinal deformity, so as to achieve the ultimate treatment of patients′ limb function and gait and improve their quality of life.

80 Study on the influencing factors and control of unplanned readmission within 31 days in orthopedic patients

Objective To analyze the influencing factors of unplanned readmission within 31 days in orthopedic patients, and to propose effective countermeasures for controlling and managing the unplanned readmission rate, so as to reduce the burden on patients and provide references for improving hospital quality management. Methods The medical records of 14 950 patients discharged from Department of Orthopedics, the Second Affiliated Hospital of Inner Mongolia Medical University from January to December 2018 were collected. Among them, 93 cases of unplanned readmission patients were named as the unplanned readmitted group, and the remaining patients were named as the remaining patients group. Descriptive analysis and correlation analysis were carried out on the two groups of patients with age, gender, complications, intraoperative use of internal plants and postoperative infection, which were used as the observation indicators. Results The readmission rate was 0.62% (93/14 950). The differences in gender, intraoperative use of internal plants and postoperative infection between the two groups were all statistically significant (all P < 0.05), while there were no significant differences in age and complications between the two groups (P > 0.05). Conclusion Each department of orthopedics should strengthen preoperative examination to clarify the surgical complications. Pay attention to the reasonable use of intraoperative internal plants; strengthen postoperative monitoring to prevent postoperative infection, correctly guide patients′ postoperative rehabilitation training, and improve patients′ rehabilitation consciousness. Hospitals, doctors and patients are working together to reduce the rate of unplanned readmission.

84 Clinical effect of external fixator pinning in anterior process of calcaneus for treatment of AO/ASIF 43.Type A3 distal tibial fractures

Objective To investigate clinical effect of the external fixator pinning in anterior process of calcaneus for treatment of AO/ASIF 43.Type A3 distal tibia fracture. Methods Thirty-six patients of AO/ASIF 43.Type A3 distal tibia fracture fixed by external fixator pinning in anterior process of calcaneus in the Department of Orthopedics, Cangzhou Central Hospital, Hebei Province from January 2013 to January 2018 were selected, and the relevant data were retrospectively analyzed. All cases were associated with fracture of shaft of distal fibula, and all fractures of distal tibia and fibula were closed fractures. During the operation, the distal end of the fracture was fixed with an external pin about 1 cm in the posterior medial side of the calcaneus and the anterior side of the sustentaculum of talus of calcaneus, and internal fixation of fracture of shaft of distal fibula was fixed with a posterolateral incision plate. Ankle flexion and extension training began 4 weeks after the operation, and relaxed the external fixator once a week. The operative time, intraoperative blood loss, fracture healing time and perioperative complications were recorded. The patients were followed up for 18 months after the operation. The clinical efficacy was evaluated by Maryland score, Lowa ankle joint score, and visual analogue scale (VAS) score. Results The average operative time was (42.6±23.8) min, the average intraoperative blood loss was (149.5±28.6) mL, and the average length of stay was (9.2±2.9) d. After the operation, one patient (2.8%) had superficial inflammatory reaction of fibula incision and one case (2.8%) had infection of external pin hole, and all healed after corresponding treatment. No vascular and nerve injury complications occurred in all cases. The overall incidence rate of perioperative complications was 5.6%. All cases were followed up for 18 months after the operation. Fracture healing was achieved in all cases without reduction loss. There were no statistically significant differences between the overall Lowa ankle score and VAS score at each postoperative time point (P > 0.05). According to the Maryland score, the good rate of the curative effect was 88.9%. Conclusion The closed reduction and external fixator pinning in anterior process of calcaneus is an alternative treatment method in the early stage of AO/ASIF 43.Type A3 distal tibia fracture injury, which has good clinical efficacy, reliable fixation and low complication rate.

88 Effects of multiple infections and viral load on severity of respiratory infection in children

Objective To investigate the effects of multiple infections and viral load on the severity of respiratory infections in children. Methods A retrospective analysis of 97 children with respiratory infections who were treated at the Jiangjin Central Hospital of Chongqing from December 2017 to December 2018 was carried out. All children were divided into three groups: mild group (n = 72), moderate group (n = 15) and severe group (n = 10) infections according to the severity of disease. All patients underwent respiratory pathogen screening, and the differences in general conditions, symptoms, infections, complications, visits, laboratory-related indicators, and viral load were compared among the three groups. The risk factors that affect the condition of the child were analyzed. Results The multiple infection rate, procalcitonin (PCT) and total viral load in the severe group were significantly higher than those in the other two groups. The multiple infection rate, PCT and total viral load in the moderate group were significantly higher than those in the mild group (P < 0.05). PCT (OR = 4.951, 95%CI: 1.005-9.473, P = 0.003) and multiple infections (OR = 1.731, 95%CI: 8.782-122.363, P < 0.001) were independent risk factors affecting the severity of disease. Conclusion PCT and multiple infections are independent risk factors for the severity of disease in children with respiratory tract infection, and should be taken into account in future clinical work.

92 Analysis of clinical signs and biochemical indexes of patients with phlegm-dampness polycystic ovary syndrome infertility

Objective To explore the clinical signs, sex hormones and glycolipid metabolism characteristics of patients with phlegm-dampness polycystic ovary syndrome (PCOS) infertility. Methods From January 2017 to January 2019, 126 patients with PCOS infertility first diagnosed in the gynecology clinic of the First Affiliated Hospital of Heilongjiang University of Chinese Medicine were retrospectively analzed, including 65 cases of phlegm-dampness syndrome and 61 cases of non-dampness syndrome. The general clinical characteristics and biochemical parameters of the patients were recorded, and the clinical specificity of patients with phlegm-dampness syndrome PCOS infertility was analyzed. Results General clinical features: systolic blood pressure, diastolic blood pressure, body weight, body mass index (BMI), waist circumference and hip circumference were significantly higher in the phlegm-dampness syndrome group than those in the non-dampness syndrome group, and the incidence of overweight or obesity (BMI>24 kg/m2) in the phlegm-dampness syndrome group was significantly greater than that in the non-dampness syndrome group, and the differences were statistically significant (P < 0.05). Sex hormones index: luteinizing hormone and luteinizing hormone/follicle-stimulating hormone in the phlegm-dampness syndrome group were significantly lower than those in the non-dampness syndrome group, and the differences were statistically significant (P < 0.05). Glucose and lipid metabolism index: fasting glucose, fasting insulin and insulin resistance index in the phlegm-dampness syndrome group were significantly higher than those in the non-dampness syndrome group, and the incidence of insulin resistance in the phlegm-dampness syndrome group was significantly higher than that in the non-dampness syndrome group, and the differences were statistically significant (P < 0.05). Conclusion Patients with phlegm-dampness PCOS infertility may have significant insulin resistance and higher BMI. Therefore, it is important to pay attention to the screening of glucose and lipid metabolism in patients, so as to prevent and cure long-term complications such as impaired glucose tolerance and diabetes as early as possible.

96 Clinical study on the treatment of cervical high-grade squamous intraepithelial lesionswith fertility requirements by hysteroscopic “pushpin” cervicalresection and cervical cold knife conization

Objective To explore the value of hysteroscopic “pushpin” cervical resection in the treatment of high-grade cervical squamous intraepithelial lesions with fertility requirements. Methods Clinic data of 152 patients with cervical intraepithelial neoplasias (CIN) grade Ⅲ from January 2010 to December 2015 were retrospectively analyzed. According to the different surgical methods, they were divided into hysteroscopic “pushpin” cervical resection group (hysteroscopy group) and thecervical cold knife conization group (CKC group), including 70 patients in hysteroscopic group and 82 patients in CKC group. The changes of operation time, intraoperative blood loss, pathological results between postoperative and preoperative, operation effect and follow-up, postoperative pregnancy (pregnancy rate, abortion rate, premature delivery rate, vaginal delivery rate and cesarean section rate) and postoperative satisfaction were compared between the two groups. Results The operation time and intraoperative blood loss in hysteroscopy group were lower than those in CKC group, the differences were statistically significant (P < 0.05). There were no significant differences between the two groups in postoperative pathological results, TCT and recurrence rate (P > 0.05). There were no significant differences in pregnancy rate and delivery mode between the two groups (P > 0.05). The preterm delivery rate of hysteroscopy group was lower than that of CKC group, the difference was statistically significant (P < 0.05). The length of stay, postoperative physical recovery, postoperative psychological recovery and overall satisfaction score of the hysteroscopic group were all higher than those of the CKC group, the differences were statistically significant (P < 0.05). Conclusion The hysteroscopy has shorter operation time, less bleeding during operation and lower rate of preterm pregnancy after operation. It is the preferred treatment for fertility requirements with high-grade cervical squamous intraepithelial lesions and it is worthy of clinical promotion.

100 Effect of Buqi Liangxue Decoction combined with Oxytocin in the treatment of postpartum hemorrhage and its influence on inflammatory factors and oxidative stress

Objective To explore the effect of Buqi Liangxue Decoction combined with Oxytocin in the treatment of postpartum hemorrhage and its influence on inflammatory factors and oxidative stress. Methods One hundred and sixty-eight patients with postpartum hemorrhage admitted to the Second Affiliated Hospital of Suzhou University from August 2017 to August 2019 were selected retrospectively. According to the different treatment methods, they were divided into control group (n = 83, Oxytocin treatment) and study group (n = 85, Buqi Liangxue Decoction combined with Oxytocin treatment). Both groups were treated for 7 days. Clinical effect, bleeding, uterine regurgitation on the 1st, 3th and 7th day of postpartum, serum levels of C-reactive protein (CRP), tumor necrosis factor-α (TNF-α), interleukin-8 (IL-8), lipid peroxide (LPO), glutathion peroxidase (GSH-Px), superoxide dismutase (SOD) before and after treatment and adverse reactions were compared between two groups. Results Total clinical effective rate of study group was higher than that of control group (P < 0.05). The bleeding time of study group was shorter than that of control group, the amount of blood loss and the number of bleeding cases in the later period were less than control group (P < 0.05). There were significant differences in the height of uterine fundus among groups, time points and interaction (P < 0.05). Intra group comparison: with the prolongation of treatment time, the height of uterine fundus in both groups decreased gradually (P < 0.05). Comparison between groups: there was no significant difference between two groups in the height of the uterine fundus on the first day after delivery (P > 0.05); on the third and seventh days after delivery, the height of the uterine fundus in study group were lower than those in control group (P < 0.05). After treatment, TNF-α, CRP and IL-8 levels in two groups were lower than those before treatment, and study group was lower than control group (P < 0.05). LPO level of two groups after treatment was lower than that before treatment, and study group was lower than control group (P < 0.05). The levels of SOD and GSH-Px in two groups after treatment were higher than those before treatment, and those in study group were higher than those in control group (P < 0.05). There was no significant difference in the incidence of adverse reactions between two groups (P > 0.05). Conclusion The effect of Buqi Liangxue Decoction combined with Oxytocin in the treatment of postpartum hemorrhage is definite. The condition of uterine regurgitation is good, it can decrease the level of serum inflammatory factors, reduce the oxidative stress response of patients, which can provide reference for clinical treatment.

104 Analysis on the factors affecting the pregnancy rate of ovulatory cycle artificial insemination

Objective To analysis the influencing factors of success rate of intrauterine artificial insemination (IUI) in ovulation-promoting cycle. Methods A retrospective analysis of 500 cycles of ovulation-promoting IUI and clinical data of patients at Hainan Maternal and Child Health Hospital and Suzhou Hospital Affiliated to Nanjing Medical University from July 2015 to June 2017, the female age, infertility years, IUI cycle, the number of IUI times per cycle, the number of ovulation inducing scheme, the number of dominant follicles, and whether or not human chorionic gonadotropin (HCG) inducing ovulation and its correlation with the clinical pregnancy rate were explored. The independent factors affecting clinical pregnancy rate of IUI were analyzed by logistic regression analysis. Results A total of 117 patients were successfully pregnant after IUI, with a pregnancy rate of 23.40%. The univariate analysis showed that the effect of female age and the number of dominant follicles on the clinical pregnancy rate of IUI was significant (P < 0.05), there were no statistically significant differences in the effects of infertility years, IUI cycles, the number of IUI in each cycle, the ovulation-promoting regimen, and whether or not HCG induced ovulation on the clinical pregnancy rate of IUI (P > 0.05). logistic regression analysis showed that the age of women (OR = 2.128, 95%CI: 1.018-4.162, P < 0.05) and the number of dominant follicles (OR = 3.617, 95%CI: 0.855-7.282, P < 0.05) were independent influencing factors of the clinical pregnancy rate of IUI in the ovulation-promoting cycle. Conclusion Female age and the number of dominant follicles are key factors in the clinical pregnancy rate of the IUI during the ovulation-promoting cycle, age is a risk factor, and the number of dominant follicles is a protective factor.

108 Investigation study on the epidemiological of hypothyroidism in pregnancy

Objective To explore the epidemiological characteristics of hypothyroidism in pregnancy, and to analyze its influencing factors. Methods The clinical data of 1000 pregnant women who were admitted to Dingzhou People′s Hospital in Hebei Province from December 2017 to December 2018 were retrospectively selected. The prevalence of hypothyroidism in pregnancy was screened. Univariate and multivariate logistic regression were used to analyze the influencing factors of hypothyroidism in pregnancy. Results Of the 1000 pregnant women included in this study, 392 pregnant women had hypothyroidism, the prevalence rate was 39.20%. Among them, 127 pregnant women had clinical hypothyroidism, accounting for 32.40%, 265 pregnant women had subclinical hypothyroidism, accounting for 67.60%. Univariate analysis showed that the incidence of hypothyroidism in pregnancy with age≥30 years, number of pregnancies≥2 times, education below junior college, parity≥3 times, history of spontaneous abortion, diabetes mellitus, hypertensive disorder of pregnancy, low iodine intake, thyroid peroxidase antibody positive, birth interval≥48 months were higher (P < 0.05). Multivariate logistic regression analysis showed that age≥30 years, number of pregnancies≥2 times, education below junior college, parity≥3 times, history of spontaneous abortion, diabetes mellitus, hypertensive disorder of pregnancy, low iodine intake, birth interval≥48 months were independent risk factors for hypothyroidism in pregnancy (P < 0.05). Conclusion Hypothyroidism has a high prevalence rate in pregnant women, which should be detected early and given high attention clinically. They can make scientific preventive measures to reduce the occurrence of disease according to its influencing factors

Objective To observe clinical effect of MOTOmed virtual scene training combined with electromyographic biofeedback in the treatment of spasmodic diplegia with cerebral palsy. Methods Seventy-one cerebral palsy children with spastic diplegia treated in Department of Child Rehabilitation of Huai′an Maternal and Child Health Care Hospital from January 2015 to December 2017 were collected and divided into control group of 33 cases and treatment group of 38 cases according to random number table. The routine rehabilitation therapy was used in control group. MOTOmed virtual scene training combined with electromyographic biofeedback therapy were used in treatment group in addition to routine rehabilitation therapy. Before and after treatment, the modified Ashworth scale was used to evaluate the gastrocnemius muscle tension of children′s lower extremities, the joint protractor was used to measure the foot dorsiflexion angle, and the two functional areas of D and E in the 88 items of gross motor function measure scale were used to evaluate the standing, walking and running and jumping functions. Results The score of gastrocnemius muscle tension in two groups after treatment was significantly lower than that before treatment, the difference was statistically significant (P < 0.05). There was no significant difference in the score of gastrocnemius muscle tension between two groups after treatment (P > 0.05). The dorsiflexion angle of foot in two groups after treatment was significantly smaller than that before treatment, the difference was statistically significant (P < 0.05). There was no significant difference in dorsiflexion angle between two groups after treatment (P > 0.05). D-area score of two groups after treatment was significantly higher than that before treatment, and treatment group was higher than control group, the difference was statistically significant (P < 0.05). E-area score of two groups after treatment was significantly higher than that before treatment, and treatment group was higher than control group, the difference was statistically significant (P < 0.05). Conclusion MOTOmed virtual scene training combined with electromyographic biofeedback for spasmodic diplegic cerebral palsy has a clear curative effect, which can significantly promote the recovery of lower limb motor function in children.

Objective To investigate the prevalence of refractive anomalies and its affecting factors in 4237 children aged 7-12 years in Wuxi City. Methods A total of 4237 children were randomly selected from November to December 2018 to study in Wuxi Hechang Primary School, Rongcheng Primary School, Taihu New Town Primary School, Pre-school Campus of Wuxi Normal Primary School and Xiaoyangguang Campus of Wuxi Normal Primary School. According to the diopter, they were divided into the normal group (n = 2362) and the abnormal group (n = 1875). The distribution of vision and prevalence of refractive anomalies in children of different ages were observed, the factors affecting children aged 7-12 years were analyzed, and a multivariate logistic regression model was established to identify the risk factors for children with refractive anomalies. Results The proportion of children aged 7-<11 years old with vision of > 0.8 was the highest, the proportion of children aged 11-<12 years old with vision of 0.5-0.6 was the highest, and the proportion of children aged 12-<13 years old with vision of 0.1-0.4 was the highest. The prevalence of refractive anomalies was highest in children aged 12- < 13 years, and lowest in children aged 7- < 8 years. Daily sleep duration, good eye habits, balanced nutrition, regular visual acuity examination, and age were associated with refractive anomalies in children aged 7-12 years (P < 0.05). Age≥9 years old, daily sleep time < 8 h, poor eye habits, nutritional imbalance and not regularly examined vision were risk factors for refractive anomalies in children aged 7-12 years old (P < 0.05). Conclusion During the prevention of refractive anomalies in children aged 7-12 years old, close attention should be paid to children′s age, sleep time, eye habits, balanced nutrition and regular visual examination, so as to take timely measures to prevent and reduce the incidence of refractive anomalies in children.

Objective To observe the efficacy of oral microscopic biological ceramics Iroot BP Plus combined with warm gutta-percha obturation in treatment of chronic periapical periodontitis with apical non-closure. Methods From March 2017 to March 2018, 120 cases with chronic periapical periodontitis with apical non-closure who admitted in Xining Stomatological Hospital were selected. They were divided into control group and research group by random number table method, with 60 cases in each group. Both groups were received oral microscopic biological ceramics Iroot BP Plus apical barrier surgery, and on this basis, research group was treated with vertical condensation of warm gutta-percha obturation. Clinical efficacy was evaluated 1 year after surgery. Operation time and pain duration were compared between two groups. The levels of inflammatory factors in gingival crevicular fluid were measured before and 2 weeks after treatment. Retention rates of affected teeth were compared between the two groups. Results The total effective rate of research group was higher than that of control group, the retention rate of affected teeth in research group was higher than that in control group (P < 0.05). There was no statistical difference in the operation time between two groups (P > 0.05). The pain duration in research group was shorter than that in control group (P < 0.05). There was no significant difference in the inflammatory factors of gingival crevicular fluid between two groups before treatment (P > 0.05). After 2 weeks of treatment, the levels of inflammatory factors of gingival crevicular fluid in two groups were lower than those before treatment, and research group was lower than control group (all P < 0.05). Conclusion Oral microscopic biological ceramics Iroot BP Plus combined with warm gutta-percha obturation in treatment of chronic periapical periodontitis with apical non-closure can shorten pain duration, increase the retention rate of affected teeth, and have better relief effects of apical inflammation.

123 Determination of blood concentration of active component in Bupleurum Injection

Objective To establish high performance liquid chromatography (HPLC) for the determination of saikosaponin d of Bupleurum Injection in rat plasma. Methods Six SD male rats were intraperitoneally injected with Bupleurum Injection of 5 mL/kg, and the blood was collected from the tail vein at different time points. The blood samples were treated with ethyl acetate precipitated protein method, and the blood concentration of saikosaponin d was determined by HPLC. The column Swell chromplus-C18 (250 mm×4.6 mm,5 μm) was used as the column. The mobile phase was water-acetonitrile (gradient elute), the detection wavelength was 204 nm, the column temperature was 30℃ and with the flow rate of 1.0 mL/min. Results The linear range of saikosaponin d in rat plasma was 0.1-2 mg/mL (r = 0.9955). The accuracy recovery rates of low, medium and high concentrations were 92.95%, 105.52% and 97.86%, respectively. The RSD values of intra-day and intra-day precision were all < 6%, and the detection limit was 2 μg/mL. Plasma samples with low, medium and high concentrations were kept stable for 12 h at room temperature, frozen at -20℃ for 10 d, and repeatedly frozen and thawed 3 times. Conclusion The method is accurate, specific, and suitable for the determination of saikosaponin d of Bupleurum Injection in rat plasma.

127 Clinical observation on Tenofovir Disoproxil treating the poor effect of Lamivudine resistance combined with Adefovir Dipivoxil

Objective To observe the clinical efficts and safety of Tenofovir Disoproxil Fumarate (TDF) treating the poor effect of Lamivudine resistance combined with Adefovir Dipivoxil. Methods From January 2010 to December 2018, 49 patients with chronic hepatitis B with poor effects by Lamivudine resistance combined with Adefovir Dipivoxil, were chosen from the Handan Infectious Diseases Hospital in Hebei Province. Patients were divided into TDF group of 30 cases (Tenofovir Dipivoxil) and the control group of 19 cases (Entecavir combined Adefovir Dipivoxil) by random number table method. After changing the treatment plan, at the 12th, 24th and the 48th weeks after treatment, HBV DNA, renal function, blood calcium and phosphorus were tested to compare the treatment effects of the two groups at the above different stages. Results At the 12th week after treatment, the negative conversion rate of HBV DNA in TDF group was significantly higher than that in the control group (P < 0.05), at the 24th and the 48th weeks after treatment, there was no significant statistic difference in the negative conversion rate of HBV DNA (P > 0.05). At the 12th and the 24th weeks after treatment, the HBV DNA levels in TDF group were significantly lower than those in the control group (P < 0.05), at the 48th week after treatment, there was no significant difference in HBV DNA level in the two groups (P > 0.05). Compared with the pre-treatment level, HBV DNA in TDF group decreased significantly at the 12th, 24th and the 48th weeks after treatment (P < 0.05), while in the control group, HBV DNA also decreased significantly at the 24th and the 48th weeks after treatment (P < 0.05). Both groups had good tolerability without severe adverse reaction. Conclusion For patients with poor efficts of Lamivudine resistance combined with Adefovir Dipivoxil, within a short time TDF can quickly restrain virus replication and improve the negative conversion rate of HBV DNA, meanwhile with good safety.

131 Effect of Silibinin Capsules on metabolic index and vascular endothelial function in patients with metabolic syndrome

Objective To study the effect of Silibinin Capsules on metabolic index and vascular endothelial function in patients with metabolic syndrome. Methods From June 2017 to June 2019, a total of 82 patients with metabolic syndrome admitted to Nanjing Hospital of Traditional Chinese Medicine were included in the study. According to the random number table method, they were divided into control group and observation group, 41 cases in each group, the control group received basic treatment, and the observation group added Silibinin Capsules on the basis of the control group. Before and after treatment, the endothelium function indexes of the two groups were compared, basic vessel diameter (D0), congestive vessel diameter (D1), endothelium-dependent vasodilation (FMD); metabolism indexes; body mass index (BMI), waist circumference (WC), triglyceride (TG), total cholesterol (TC), fasting insulin (FINS), fasting blood glucose (FBG) and glucose metabolism rate (M); serum Inflammatory factors and liver function indexes: serum interleukin-8 (IL-8), IL-18, tumor necrosis factor-α (TNF-α), alanine aminotransferase (ALT), aspartate aminotransferase (AST). Results After treatment, FMD, D0 and D1 in the two groups were significantly higher than before treatment, and the observation group was significantly higher than the control group (P < 0.05). The TG and FINS levels in both groups were significantly lower than those before treatment, and the observation group was lower than the control group (P < 0.05). The M value in both groups was significantly higher than that before treatment, and the value in the observation group was higher than that in the control group (P < 0.05). The levels of IL-18, IL-8, TNF-α, ALT and AST in both groups were significantly lower than those before treatment, and were lower in the observation group than in the control group (P < 0.05). Conclusion Silibinin Capsules can effectively regulate metabolic disorder and improve endothelial function in patients with metabolic syndrome.

139 Exploration on the rule of traditional Chinese medicine in external treatment of hand foot syndrome based on association rule and cluster analysis

Objective To search and analyze medication rules of traditional Chinese medicine (TCM) for hand foot syndrome (HFS). Methods Articles were searched from CNKI, Wanfang data and VIP network by the words of “hand foot syndrome” or “hand foot skin reaction” before September 2019. The formulas of external use for HFS was screened, and the data was logged in excel 2019. The association rules were conducted using “arules” of R 3.6.0 to search for TCM composition with strong association and cluster analysis was conducted by SPSS 20.0 for herbs of high frequency to research main combination of TCM for HFS. Results Eighty TCM formulas was included. There were 15 herbs that using frequency was more than 10. According to medication rules, 11 herb-partners had strong combination. Herbs which had the most strong internal relationship were Radix Astragali seu Hedysari, Flos Carthami, Ramulus Cinnamomi, Radix Angelicae Sinensis. Cluster analysis produced two main treatment aspects: benefiting qi for activating blood circulation, warming womb and channels and dredging channels. Conclusion The external application of TCM in treating HFS is mainly through benefiting qi for activating blood circulation, warming womb and channels and dredging channels, as well as clearing heat and removing toxicity and nourishing blood for moistening dryness.

143 Clinical study of auricular acupoint embedding combined with acupuncture in the treatment of perimenopausal insomnia

Objective To explore clinical effect of auricular acupoint embedding combined with acupuncture in the treatment of perimenopausal insomnia. Methods From December 2017 to June 2019, 100 cases of perimenopausal insomnia were selected from the Outpatient Department and Inpatient Department of the First Affiliated Hospital of Heilongjiang University of Chinese Medicine. They were divided into observation group (50 cases) and control group (50 cases) by random number table method. Observation group was treated by pressing auricular point and embedding acupuncture combined with acupuncture, while control group was treated by Estazolam. The levels of serum estrogen (E2), follicle stimulating hormone (FSH), 5-hydroxytryptamine (5-HT), noradrenaline (NE), sleep time and total scores of Pittsburgh sleep quality index (PSQI) were observed before and after treatment in two groups. Results The scores of sleep time and total scores of PSQI after treatment of two groups were lower than those before treatment, which of observation group were lower than those of control group, the differences were statistically significant (P < 0.05 or P < 0.01). The serum E2 level of two groups after treatment was higher than that before treatment, and that of observation group was higher than that of control group; the serum FSH levels of two groups after treatment were lower than those before treatment, and that of observation group was lower than that of control group, the differences were statistically significant (P < 0.05 or P < 0.01). The serum 5-HT levels of two groups after treatment were higher than those before treatment, and that of observation group was higher than that of control group; the levels of serum NE after treatment in two groups were lower than those before treatment, and that in observation group was lower than that in control group, the differences were statistically significant (P < 0.05 or P < 0.01). Conclusion Auricular acupuncture embedding combined with acupuncture can improve the sleep quality of perimenopausal patients by promoting the expression of serum E2, 5-HT and inhibiting the expression of serum FSH, NE.

Objective To summarize and discuss the rules of acupoint selection of professor Shao Ying in the treatment of infantile functional dyspepsia with infantile massage. Methods From January to September 2019, 139 outpatient medical record prescriptions of children with functional dyspepsia of professor Shao Ying in acupuncture clinic of the First Affiliated Hospital of Guangzhou University of Chinese Medicine and Guangzhou Dongyuantang National Medical Center were collected and sorted out, and 108 infants met the inclusion criteria were statistically analyzed. The prescription database of professor Shao Ying′s massage for children′s functional dyspepsia were established. Microsoft excel 2017, SPSS 23.0 and SPSS Modeler 18.0 were used for frequency analysis, cluster analysis, association analysis and network diagram data mining. Results Among 108 pieces of infantile massage prescriptions, 66 acupoints were involved, with a total frequency of 2519 times. In addition, 10 efficient clusters were acquired from cluster analysis, correlation analysis, and network graph. Among them, the core group was the spleen, the internal gossip, the ridge, and the abdomen. The core of the acupoints were spleen channel, internal eight trigrams, spine and abdomen. Conclusion Professor Shao Ying in treating infantile functional dyspepsia with massage is based on traditional Chinese medicine theory, it is flexible in clinical matching point, and she is good at according to Lingnan climate and infantile constitution to dialectical addition and subtraction. Professor Shao Ying′ academic experience can provide new ideas and references for clinical treatment of this disease.

Myositis ossificans progressiva is a rare and serious crippling disease. It is mainly manifested in the formation of heterotopic bone, such as tendon, ligament, fascia, aponeurosis, striated muscle, etc. The pathophysiological mechanism has not been fully defined. At present, the study shows that the disease is related to the mutation of activin A receptor, and there is no specific and effective treatment. There is no relevant record of “myositis ossificans progressiva” in ancient books of traditional Chinese medicine. Modern doctors mostly classify this disease as “arthralgia” and mainly treat it by promoting blood circulation and removing blood stasis, relaxing muscles and activating collaterals. Professor Jiang Liangduo thinks that the pathogenesis of the disease is kidney deficiency and heat toxin accumulated in the blood. Therefore, the treatment is based on tonifying the kidney, clearing the blood and detoxifying the blood. At the same time, on the basis of traditional Chinese medicine treatment, combined with local low-dose radiotherapy, the effect is quite remarkable.

155 Exploration on the mechanism of Zuojin Pills in the treatment of gastric precancerous lesions based on network pharmacology

Objective To study possible mechnism of Zuojin Pills in the treatment of gastric precancerous lesions (GPL) using network pharmacology. Methods The active components of Zuojin Pills were obtained by TCMSP database, the corresponding targets of the active components were obtained by TCMSP and Pharmapper, the related targets of GPL were obtained by Gene Card, the common targets were obtained, the PPI network map was constructed by String and Cytoscape, and the targets were analyzed by David. Results There were 41 active components corresponding to 636 targets in Zuojin Pills, 1352 targets in GPL, 165 targets in Zuojin Pills and GPL. The enrichment of GO biological process showed that Zuojin Pills and GPL were involved in transcription regulation, drug binding, cell apoptosis and other biological processes. The enrichment of KEGG showed that the common targets were obviously enriched in cancer pathway, PI3K-Akt pathway, and so on. Conclusion Zuojin Pills can treat GPL through multiple targets and multiple pathways, so as to provide reference for future research.

159 Effect of clearing heat and removing dampness in the treatment of acute gouty arthritis

Objective To discuss the effect of Western medicine treatment combined with clearing heat and removing dampness in the treatment of acute gout arthritis. Methods Ninety-four patients with acute gout arthritis diagnosed and treated in the First Hospital of Wuhan City from June 2017 to August 2019 were selected and divided into control group and experimental group by random number table method, with 47 cases in each group. Control group was treated with conventional Western medicine, and experimental group was treated with heat clearing and dampness removing therapy on the basis of control group. Clinical efficacy, laboratory index, joint symptom score and adverse reaction rate were compared between two groups. Results There was no significant difference in clinical efficacy between two groups (P > 0.05). After treatment, the levels of serum uric acid, C-reactive protein, erythrocyte sedimentation rate and interleukin-6 in two groups were lower than those before treatment (P < 0.05), and experimental group was lower than control group (P < 0.05). After treatment, the scores of joint function, joint pain and joint swelling in two groups were lower than those before treatment (P < 0.05), and experimental group was lower than control group (P < 0.05). There was no significant difference in the incidence of adverse reactions between two groups (P > 0.05). Conclusion Western medicine combined with clearing heat and removing dampness can improve clinical symptoms of patients with acute gout arthritis, reduce the level of laboratory indicators, and have good safety.

163 Study on the rule of traditional Chinese medicine in the treatment of radiation dermatitis based on data mining

Objective To explore and discuss the therapeutic rules and characteristics of traditional Chinese medicine external prescription for radioactive dermatitis. Methods The literatures of CNKI, Wanfang data knowledge service platform, and VIP database (from July 1977 to September 2019) were searched by computer. The literatures related to external treatment of radioactive dermatitis by traditional Chinese medicine were collected and sorted out. Traditional Chinese medicine inheritance support system software (V2.5) was applied to establish a unified standard prescription database, and relevant data mining was carried out. Results Through screening, 61 formula of traditional Chinese medicine external treatment for radioactive dermatitis were obtained. Among them, 76 medicinal herbs were involved, among which, cold and warm medicinal herbs, bitter and sweet medicinal herbs account for a larger proportion, and liver, spleen and heart meridians were the ones with a higher frequency of meridian return. The top three drugs used in frequency were Borneolum Syntheticum, Cortex Phellodendri, Radix Lithospermi, the most frequently used drug combination was Borneolum Syntheticum-Cortex Phellodendri, Borneolum Syntheticum-Radix Lithospermi, Radix Scutellariae-Cortex Phellodendri, Rhizoma Coptidis-Cortex Phellodendri, excavating 10 core drug combinations and 5 new prescriptions. Conclusion The currently known prescriptions for the treatment of radiation dermatitis are mainly for clearing away heat-evil and dispelling dampness, cooling blood, detoxifying and relieving pain, as well as promoting granulation and astringing sores, benefiting qi and nourishing yin, etc. The core drug group and new prescription can provide reference for theoretical research, new drug development and clinical treatment of radioactive dermatitis.

The world health organization (WHO) collaborating centre in China is a partner in supporting WHO in achieving its organizational goals and is also an important part of China′s participation in global health governance. This paper applies the internalization theory of international norms to analyze the initial development stage, capacity improvement stage and rapid improvement stage of who cooperation centers in China from three dimensions of cognitive change, institutional change and policy practice. Based on literature review, field research, in-depth interviews and expert consultation, this paper analyzes the current problems and challenges faced by who cooperation centers in China, including the lack of cognitive channels, institutional guarantees and policy support at the country level. In addition, it proposes to strengthen top-level design and strategic layout planning, promote the development of cooperation centers in China as a whole, establish a close coordination mechanism, open communication channels, actively mobilize subjective initiative, and further enhance the brand effect and international influence of cooperation centers in China.

Objective To analyze the scientific research layout of the National Cancer Institute (NCI) in the United States in the past 20 years, and to provide reference for the policy formulation of cancer prevention and control strategy and scientific research investment layout in China. Methods The funding data for fiscal years 1998-2018 were retrieved from the national cancer institute database to analyze the overall situation, funding structure, key support areas, institutions, and the relationship with the cancer burden. Results During the 1998-2018 fiscal year, NCI′s overall funding and research grants had progressively increased. In the structure of funding, research grants had been accounted for absolute proportion; at the meantime, stable support for institutions and non-competitive project funding were focused. The key supporting area had matched the actual demand in cancer prevention and control of America. Conclusion Scientific research plays an important role in supporting the prevention and control of cancer in the United States, and the relevant layout experience of the national cancer institute can be used for reference in formulating strategies to strengthen the scientific and technological support for cancer prevention and control in China.

Currently, the interventions with high accessibility are limited. The natural history of disease is an important part of the clinical study of disease, and its role in the description of clinical manifestations and pathogenesis is fundamental. The results of research show that communication studies around the components of the communication model, comprehensively examines the macroscopic process of the international communication of traditional Chinese medicine culture, and analyzes the main internal and external influencing factors of the communication system of traditional Chinese medicine culture, and proposes countermeasures for the international dissemination of traditional Chinese medicine culture. However, limited to the theory and practice of international communication of traditional Chinese medicine culture, the research at this stage needs to be deepened. In particular, there are two aspects of cultural difference and communication carrier that need to be updated and improved.

185 SWOT analysis of “Internet +” pension model under the background of aging population

As the effects of aging population become more serious,the traditional pension model unable to content the expectations of the new era. Develope the “Internet +” pension model will be a feasible way to solve the problem about China′s aging population. This paper through SWOT analysis, sorts out the internal advantages and disadvantages, external opportunities and threats of “Internet +” pension mode. And it also puts forward measures to speed up the implementation of policies, pilot the reform of the old-age care model, unify the standards for the smart old-age care industry, strengthen network security, standardize nursing education, improve the job access mechanism for nursing workers, and strengthen spiritual care for the elderly.

189 Analysis and application of standardized general practice thinking model for general practice services

Objective To explore a set of standardized general practice thinking models suitable for community general practitioners′ general practice services at the current stage to achieve an effective, structured and complete general practice process. Methods A total of 160 general practitioners from 20 social health centers in Baoan District, Shenzhen City, Guangdong Province, from January 2017 to December 2018 were selected as research subjects. Ten social health centers included in the whole person care pilot were taken as the observation group, 80 general practitioners were taken as the observation subjects, and 400 patients treated in this period were taken as the questionnaire subjects. At the same time, 10 social health centers with similar personnel structure, social health scale and number of patients were selected as the control group, 80 general practitioners were taken as the observation subjects, and 400 patients treated in this period were takehzas the questionnaire subjects. The control group adopted the traditional independent diagnosis and treatment model, and the observation group adopted the standardized general practice thinking diagnosis and treatment model. The general practitioners′ general practitioners′ professional achievement, self-confidence, communication skills, professional knowledge self-learning ability of two groups were compared. The accuracy of disease diagnosis and treatment, patient satisfaction and follow-up compliance of the hospitalized patients of the two groups were observed. Results The professional achievement, self-confidence, communication ability and self-learning ability of general practitioners in the observation group were higher than those in the control group (P < 0.05). Patients in the observation group had higher disease diagnosis and treatment accuracy, patient satisfaction and patient compliance than those in the control group (P < 0.05). Conclusion Standardized general practice thinking model based on general practice service can effectively improve the quality of community general practice service and improve the level of community general practice service.

193 Turkish experience in creating an integrated health service system

Objective To Refine and analyze a series of measures for the reform of the Turkish health system, so as to seek empirical support for building a “people-oriented” medical and health service system in China. Methods Based on systematic literature review, the relevant documents, documents, bills and other contents of Turkey′s integrated health service system were searched and collected, while these contents were analyzed and researched. Results In the health reform measures in Turkey, adjusting the functions of health managers, strengthening of primary health care, integrating social health insurance, and optimizating health financing systems had been improved in terms of management efficiency, service continuity, service accessibility, and service quality, which had important value for building an integrated health system. Conclusion Turkey′s health transformation plan has improved financing equity, service accessibility and service quality to a certain extent, but it has not fully addressed the problems of insufficient investment and manpower shortage.
