4 Preparation of braining-targeting liposomes and the structural stability of Ac4MAN on the surface of liposome in vitro

Objective To construct a modified Ac4MAN (p-carboxybenzene-α-D-acetylmannose) brain-targeted liposome and investigate the structural stability of Ac4MAN. Methods Ac4MAN was incubated with porcine liver esterase (PLE) at 37°C, pH 7.4 and sampled at different time points. High performance liquid chromatography (HPLC) and ultra performance liquid chromatography-mass spectrometry (UPLC-MS) were used to analyze it. Ac4MAN liposomes were prepared and characterized. Flow cytometry was used to detect the uptake of Ac4MAN liposomes and Ac4MAN liposomes plus PLE by glioma cells U87. Results The results of HPLC showed that the area of Ac4MAN decreased from 578.4 to 37.9 after incubation for 0-3 h. The area of Ac4MAN peak was close to 0 after incubation for 4-7 h. The peak area of the formed MAN (p-aminobenzene-α-D-pyran mannoside) increased from 0 to 363.0, indicating that with the prolongation of incubation time, Ac4MAN gradually decreased and MAN gradually increased. Conclusion The Ac4MAN liposome prepared in this study has a particle size of (120.3±2.0) nm and a potential of (-15.76±1.23) mV. The uptake of AC4MAN liposomes by U87 cells is enhanced after adding enzymes.

9 Effect of pVAX-eNOS transfection on the early endothelial progenitor cells from the patients with acute myocardial infarction

Objective To investigate the feasibility of transfection of early endothelial progenitor cells (EPCs) in peripheral blood of patients with acute myocardial infarction (AMI) in vitro using pvax1-mediated nitric oxide synthase (eNOS) gene. Methods From December 2017 to June 2018, 30 cases of AMI patients admitted to Shaanxi Provincial People′s Hospital due to heart disease and diagnosed by coronary angiography were selected. The Ficoll density gradient centrifugation method was used to isolate mononuclear cells in the peripheral blood of patients with fasting veins and induce them to culture into EPCs for identification and purification of pvax-enos plasmids. EPCs were divided into three groups: eNOS transfection group, cationic polymer JetPEITM was used to transfect eNOS gene into EPCs in vitro. After transfection for 24 h, nitric oxide (NO) content in EPCs was detected by nitrate reductase method, and the ability of eNOS and vascular endothelial growth factor (VEGF) secretion by EPCs was detected by ELISA and Western blot. The proliferation of EPCs was detected by Wst-1 method, and the migration and adhesion of EPCs in each group were detected by Transwell chamber. Empty plasmid transfection group, only pVAX empty plasmid was transfected into EPCs. Control group, no transfection of EPCs. Results Pvax-enos gene was successfully transfected into early EPCs of AMI patients, and 24 h after transfection, the content of NO was significantly increased compared with the blank plasmid transfection group and the control group (P < 0.01), The expression of eNOS was significantly increased (P < 0.01), and the ability to secrete VEGF was significantly increased (P < 0.01). The ability of proliferation, migration and adhesion of EPCs was significantly increased (P < 0.01), and there was no significant difference between the empty plasmid transfection group and the control group (P > 0.05). Conclusion Cationic polymer JetPEITM can successfully mediate the effective transfection of pvax-enos into early EPCs in AMI patients, and effectively enhance the proliferation, migration, adhesion and secretion of EPCs, which lays a foundation for the improvement of myocardial ischemia in AMI patients by autologous EPCs transplantation.

14 Anti myocardial fibrosis effects of aucubin based on ROS-MAPK signaling pathway

Objective To investigate the effect and mechanism of aucubin (AU) on cardiac fibroblasts (CFs) proliferation induced by angiotension Ⅱ (Ang Ⅱ). Methods Rat CFs were cultured by the methods of trypsin digestion and differential adhesion. They were divided into blank control group, Ang Ⅱ (1×10-6 mol/L) intervention group, low concentration of AU intervention group [Ang Ⅱ (1×10-6 mol/L) + AU (1 μmol/L)], medium concentration of AU intervention group [Ang Ⅱ (1×10-6 mol/L) + AU (10 μmol/L)], high concentration of AU intervention group [Ang Ⅱ (1×10-6 mol/L) + AU (100 μmol/L)]. CCK8 method was used to test cell proliferation activity, collagen type Ⅰ, Ⅲ content in cell supernatant were determed by enzyme linked immunosorbent (ELISA), fluorescence enzyme standard instrument was used to detect reactive oxygen species (ROS) level, Western blot method was used to detect p38MAPK and p-p38MAPK protein expression. Results Compared with Ang Ⅱ intervention group, CFs proliferation and p38MAPK phosphorylation were significantly inhibited in the medium and high concentration AU intervention groups (P < 0.05). Compared with Ang Ⅱ intervention group, low, medium and high concentration of AU intervention group ROS, the production of collagen type Ⅰ, Ⅲ decreased significantly (P < 0.05). Conclusion AU can restrain by ROS-MAPK pathway Ang Ⅱ induction of CFs proliferation and collagen formation.

18 Effect of Kaiwei Xiaoshi Mixture on motilin and somatostatin in rats model of functional dyspepsia

Objective To study the therapeutic effect of Kaiwei Xiaoshi Mixture on functional dyspepsia (FD) rats and its related mechanism. Methods Sixty male Wistar rats were randomly divided into blank group, model group, positive drug group, low dose of Kaiwei Xiaoshi Mixture group, middle dose of Kaiwei Xiaoshi Mixture group and high dose of Kaiwei Xiaoshi Mixture group. The blank group rats were fed normally. The rest of rats were fed with starvation and satiety disorders and tail-clipping irritation for 14 days to establish FD model. The gavage treatment lasted for 14 days. At the end of the experiment, the mental state of rats in each group was observed and the weight of rats was recorded. The gastric emptying rate and small intestinal propulsion rate of rats in each group were measured by gavage with blue dextran 2000. The contents of motilin (MTL) and somatostatin (SS) in serum and duodenal tissue were detected by enzyme-linked immunosorbent assay (ELISA). Results Compared with the blank group, the body weight of the model group decreased (P < 0.01); compared with the model group, the body weight in the middle and high dose of Kaiwei Xiaoshi Mixture groups increased (P < 0.05 or P < 0.01). Compared with the blank group, the gastric emptying rate of the model group decreased (P < 0.01); compared with the model group, the gastric emptying rate in high dose of Kaiwei Xiaoshi Mixture group increased (P < 0.05). Compared with the blank group, the small intestine propulsion rate of the model group was significantly decreased (P < 0.01); compared with the model group, the intestinal propulsion rate in low, middle and high dose of Kaiwei Xiaoshi Mixture groups increased, while there were no statistically significant differences (P > 0.05). Compared with the blank group, the MTL content in the serum and duodenum of the model group was significantly decreased (P < 0.05 or P < 0.01); compared with the model group, the MTL content in serum and duodenum of high dose of Kaiwei Xiaoshi Mixture group increased (P < 0.05 or P < 0.01). Compared with the blank group, the SS content in the serum and duodenum of the model group was significantly increased (P < 0.05); compared with the model group, the SS content in serum and duodenum of high dose of Kaiwei Xiaoshi Mixture group reduced (P < 0.05). Conclusion Kaiwei Xiaoshi Mixture has the effect of treating FD and can significantly increase gastric emptying rate and small intestinal propulsion rate, and its mechanism is closely related to the improvement of gastrointestinal hormones SS and MTL secretion.

23 Research progress on gut microbiota and anti-atherosclerosis treatment with Chinese medicine

Atherosclerosis is the pathological basis of many kinds of cardiovascular and cerebrovascular diseases. With the deepening understanding of the relationship between the alteration of gut microbiota and the parenteral diseases, the connection between the alteration of gut microbiota and arteriosclerosis has been gradually revealed. The new idea of treating atherosclerosis through regulating gut microbiota has been explored. This review aims to explain the relationship between the alteration of gut microbiota and atherosclerosis, as well as its risk factors. Meanwhile, the effect and mechanism of Chinese medicine in anti-atherosclerosis through regulating gut microbiota are introduced in this article.

27 Progress in the expression of insulin-like growth factor-1 in renal cell carcinoma

Insulin-like growth factor 1 (IGF-1) and its receptor (IGF-1R) are important components of insulin-like growth factor, IGF-1, the activation of IGF-1R may be closely related to the occurrence and development of various malignant tumors. It is suggested that the relationship between IGF-1, IGF-1R and malignant tumors can be studied in order to provide reference for the clinical diagnosis and treatment of malignant tumors. Renal cell carcinoma is a common malignant tumor. The expression of IGF-1, IGF-1R in RCC and its clinical significance can be helpful for the diagnosis and treatment of renal cell carcinoma. This article is mainly about the structure and function of IGF-1, IGF-1R, the relationship between IGF-1, IGF-1R and malignant tumor, the expression of IGF-1, IGF-1R in renal cell carcinoma, and the analysis of IGF-1, IGF-1R is a molecular targeted therapy for renal cell carcinoma.

30 Advances in the role of cytokines secreted by tumor-associated macrophages in tumor microenvironment

Tumor microenvironment (TME) contains multiple cells and cytokines, which play an important role in tumor proliferation, invasion and metastasis. Tumor-associated macrophages (TAMs), as an important component of TME, mainly interact with tumor cells through the cytokines secreted by them to further regulate tumor progression. In this paper, the role of cytokines secreted by TAMs in TME was summarized, including tumor invasion, migration and angiogenesis. Targeting TAMs secreted cytokines in TME for intervention is expected to be a new therapeutic strategy for tumors.

34 Advances in research on gene polymorphisms of antithrombotic drugs in Kawasaki disease

Kawasaki disease (KD) with coronary artery aneurysm causing thrombosis is a serious complication of KD and a serious heart disease in children and their adulthood. Antithrombotic drugs include Aspirin, Clopidogrel, Warfarin and Dipyridamole. The individualized differences in the clinical treatment of these drugs are closely related to heredity. At present, there is no definite conclusion on Aspirin-related gene research, while more studies on Clopidogrel and more definite and unified conclusion are CYP2C19 and Warfarin-related are CYP2C9 and VKORC1, and no Dipyridamole-related gene research has been found. Most of these findings come from adult studies, and the related gene polymorphisms have been divided into different levels of evidence, which provides a direction for the study of antithrombotic therapy in children.

38 Research progress on Difluoromethyl-containing drugs and Difluoromethylation reagents

This paper presentes all Difluoromethyl-containing drugs approved by FDA in the United States from 2011 to 2018 for the first time. And briefly introduces their clinical applications. The Difluoromethylation reagents reported domestic and foreign are reviewed and classified based on their electrical effect. Among them, the nucleophilic difluoromethylation reagents are the most common ones. They mainly include TMS, PhXCF2H, BrCF2PO(OEt)2, Phenylsulfonyl and Phenylsulfonyl Difluoroacetic acid metal salts. The researches on electrophilic Difluoromethylation reagents are relatively fewer. Difluoromethyl radical donors and Difluorometallic salts have been reported.

42 Analysis of influencing factors and depression status quo of bedridden elderly in old-age care institutions in Urumqi

Objective To understand the depression status of bedridden elderly in old-age care institutions in Urumqi and analyze the influencing factors. Methods A total of 546 bedridden elderly people from 8 old-age care institutions in Urumqi from January to July 2018 were selected by stratified cluster sampling. A questionnaire survey was conducted with the geriatric depression scale (GDS-15) and the Barthel index rating scale. Results The incidence of depression in bedridden elderly in Urumqi′s old-age care institutions was 65.4%, including 40.3% for mild depression and 25.1% for moderate-to-severe depression. Multivariate Logistic regression analysis showed that widowhood, low education level, heavy bedridden grading, long bedtime, poor daily living ability, and the number of diseases were important factors in the incidence of depression in bedridden patients (P < 0.05). Conclusion The incidence of depression among bedridden elderly in old-age care institutions in Urumqi is high. Care workers should implement targeted intervention measures according to the characteristics of bedridden elderly, so as to improve their mental health level.

46 A study on the influence of social support on the choice of old-age financing mode for middle-aged and elderly people

Objective To understand the impact of different levels of social support on elderly people′s choice of financing. Methods A self-designed questionnaire was used to select 300 middle-aged and elderly residents in Fangshan district of Beijing from June to July 2017 through convenient sampling method, 294 valid questionnaires were recovered, with an effective recovery rate of 98%, χ2 was used to analyze the factors influencing the choice of elderly financing. Results In terms of demographic characteristics, there were statistically significant differences in the choice of old-age financing modes among middle-aged and elderly people with different marital status and the largest source of income (P < 0.05). In terms of social support, middle-aged and elderly people with different levels of support had statistically significant differences in the choice of old-age financing mode (P < 0.05). Conclusion The middle-aged and senior citizens with high level of social support have more diversified financial management methods. The elderly with low level of social support have a single way of financing. In general, the choice of financial management methods for middle-aged and elderly people is still in the primary stage, and the old age financial market for the elderly has great potential for development.

50 The role of TEG in evaluating the effect of antiplatelet therapy in patients with coronary heart disease after PCI and monitoring the occurrence of ischemia and hemorrhage events

Objective To monitor the effect of antiplatelet drugs after percutaneous coronary intervention (PCI) in patients with coronary heart disease by thromboelastography (TEG), and to analyze its relationship with ischemic and hemorrhagic events after PCI. Methods A total of 300 patients with coronary heart disease diagnosed by the First People′s Hospital of Huizhou City of Guangdong Province ("our hospital" for short) and treated with PCI were collected from July 2017 to May 2018. All patients were treated with aspirin and clopidogrel for antiplatelet therapy. The arachidonic acid (AA) pathway platelet aggregation inhibition rate ≥ 50% defines Aspirin onset; the adenosine diphosphate (ADP) pathway platelet aggregation inhibition rate ≥ 30% defines Clopidogrel onset. According to the results of TEG AA/ADP inhibition rate, 200 cases were divided into Aspirin + Clopidogrel effective group (AA and ADP inhibition rates were up to standard), 30 cases in Aspirin ineffective group, 50 cases in Clopidogrel ineffective group and 20 cases in Aspirin + Clopidogrel ineffective group. The platelet inhibition rate and the total incidence of ischemic events within half a year were compared among the four groups. According to the results of the maximum clot intensification (MA-ADP) induced by adenosine diphosphate in TEG, the patients were divided into MA-ADP<31 mm group, 31 mm≤MA-ADP≤47 mm group and MA-ADP>47 mm group. The platelet inhibition rate and the total incidence of bleeding events were compared among the three groups. Results The platelet inhibition rate and the total incidence of ischemic events in the four groups were significantly different (P < 0.05). The platelet inhibition rate of aspirin + clopidogrel group was the highest, which was significantly higher than that of other groups (P < 0.05). The total incidence of ischemic events in aspirin + clopidogrel group was 35.00%, which was significantly higher than that in other groups (P < 0.05). The 300 patients were divided into MA-ADP < 31 mm group, 31 mm < MA-ADP < 47 mm group and MA-ADP > 47 mm group according to the results of TEG MA-ADP. The platelet inhibition rate and the total incidence of hemorrhagic events in the three groups were significantly different (P < 0.05). The platelet inhibition rate of MA-ADP > 47 mm group was significantly lower than that of other two groups (P < 0.05). The total incidence of hemorrhage events in MA-ADP > 47 mm group was only 5%, which was significantly lower than that in other two groups (P < 0.05). Conclusion For patients receiving antiplatelet therapy after PCI, monitoring with TEG can guide the adjustment of antiplatelet therapy and ensure the inhibition of platelet aggregation activity, thus reducing the incidence of cardiovascular adverse events.

54 Effects of Atorvastatin Calcium on myocardial microcirculation, endothelial microparticles and C-reactive protein in patients with acute coronary syndrome after PCI

Objective To investigate the effects of Atorvastatin Calcium on myocardial microcirculation, endothelial microparticles (EMPs) and C-reactive protein (CRP) in patients with acute coronary syndrome (ACS) after percutaneous coronary intervention (PCI). Methods A total of 128 patients with ACS admitted to the Department of Cardiology, Langzhong People′s Hospital of Sichuan Province from January 2014 to November 2016 were selected as the research objects. They were divided into control group and observation group according to random number table method, 64 cases in each group. Among them, patients in the observation group were given 40 mg Atorvastatin Calcium 12 and 2 hours before PCI respectively, while patients in the control group were given the same amount of placebo. The other treatment processes of the two groups were consistent. TIMI blood flow classification at 7 days after operation was compared by coronary angiography, and the levels of EMPs and ACS in venous blood at different time points before and after treatment were compared between the two groups. Results The blood flow of the distal part of the observation group was significantly better than that of the control group (P < 0.05). The results of repeated measurement variance analysis showed that there were significant differences in CPR and EMPs levels between the two groups during treatment, at different time points, and the interaction between groups and time (P < 0.05). There was no significant difference in EMPs and CRP levels between the two groups before operation (P > 0.05). Compared with before operation, the levels of EMPs and CRP in the two groups were significantly increased at 1 day after operation, and decreased significantly at 14 days after treatment (P < 0.05). The levels of EMPs and CRP in the observation group at 1 and 14 days after operation were significantly lower than those in the control group, and the differences were statistically significant (P < 0.05). Conclusion Atorvastatin Calcium can effectively improve myocardial microcirculation, reduce EMPs and CRP levels and protect endothelial cells in ACS patients after PCI, which is worthy of clinical promotion.

58 Effects of Naoxueshu oral liquid on hemorheology homocysteine and vascular endothelial growth factor in patients with cerebral infarction

Objective To observe the effect of Naoxueshu oral liquid on cerebral infarction and its effect on hemorheology, homocysteine and vascular endothelial growth factor (VEGF). Methods from February 2017 to February 2018, 70 cases of cerebral infarction patients admitted to Tangjiazhuang Hospital, Kailuan, Hebei Province were selected and divided into observation group and control group by random number table method, with 35 cases in each group. Patients in both groups were given symptomatic treatment such as anticoagulation, anti-platelet aggregation, dehydration, reducing intracranial pressure, and scavenging oxygen free radicals, etc, and the observation group was given cerebrohemohy Naoxueshu therapy on this basis. The therapeutic effect, NIHSS, mRS score, hemorheology, Hcy and hs-CRP were compared between the two groups. Results After treatment, the NIHSS and mRS scores of patients in the two groups were lower than those before treatment, and the differences were statistically significant (P < 0.05), the NIHSS and mRS scores of the observation group were lower than those of the control group, and the differences were statistically significant (P < 0.05). Hcy and hs-CRP in the two groups decreased compared with that before treatment, and the differences were statistically significant (P < 0.05). Hcy, hs-CRP and VEGF levels in the observation group were lower than those in the control group, with statistically significant differences (P < 0.05), fibrinogen, plasma viscosity, low whole blood viscosity, medium whole blood viscosity and high whole blood viscosity of the two groups were all lower than before treatment, and the differences were statistically significant (P < 0.05), the indexes of hemorheology in the observation group were lower than those in the control group, and the differences were statistically significant (P < 0.05). The total effective rate in the observation group was higher than that in the control group, and the difference was statistically significant (P < 0.05). Conclusion The treatment of cerebral infarction patients with cerebral blood thinning oral liquid is helpful to improve blood coagulation function, promote nerve function repair, and improve the quality of life of patients.

62 Effect of stereotactic radioneurosurgery robot CYBER knife system in the treatment of pituitary adenoma

Objective To explore the approaches to CYBER knife system for stereotactic radiosurgery (SRNS) in the treatment of pituitary adenoma, and to open up a new non-invasive way for the treatment of pituitary adenoma. Methods A total of 12 patients with pituitary tumor Admitted to 960 Hospital of PLA and Affiliated Hospital of Shandong Acodemy of Medical Sciences from May 2015 to Decmber 2018 were selected and treated with CYBER knife system, a fifth-generation non-invasive robot from the United States. 126 radiating nodes were used, and the treatment was divided into three times, with each interval of 24 h and three times of total dose of 20 Gy. The dose of the optic nerve at the edge of the tumor was 20 Gy, combined with the clinical symptoms and endocrine changes, the efficacy of CYBER knife in the treatment of pituitary adenoma was analyzed. Results There were 6 cases of PRL tumor and 6 cases of GH tumor. The tumor diameter was 2.0 - 3.5 cm, with an average (2.30±0.78) cm. The preoperative PRL in 4 patients was significantly higher than 200 ng/L, with an average (390.00±45.32) ng/L, 2 patients increased, with an range value of 37.5 - 68.6 ng/L, and with an average (46.00±17.69) ng/L, GH of 6 patients elevated, and with an average (14.00±9.13) ng/L. MRT2 reexamination 2 weeks after treatment showed tumor radionecrosis. Postoperative reexamination showed that 4 cases of hormone completely returned to normal, 8 cases of neuroendocrine disorder, of which 6 cases returned to normal, 1 case of mild decrease, and 1 case without improvement. The patients were followed up for 3 to 11 months, with an average of 5.8 months. One patient had low endocrine function and received oral prednisone and thyroxine replacement therapy. One case of recurrence was treated by surgery. No permanent diabetes insipidus or death from surgery. Conclusion It is safe and feasible to apply the CYBER knife system of stereotactic non-frame radioneurosurgery to treat pituitary adenoma at a dose of 20 Gy and above 100 nodes.

65 Analysis of quality of life in patients with advanced ovarian cancer based on WHOQOL-BREF scale

Objective To explore the quality of life and it′s influencing factors in patients with ovarian cancer. Methods From January 2017 to October 2018, in Wuhan University People′s Hospital, 86 patients with ovarian cancer were selected. The World Health Organization Quality of Life-BREF (WHOQOL-BREF) scale was used to conduct a questionnaire survey on the quality of life of the patients, and applied multiple stepwise regression methods was used to analyze the influencing factors of quality of life. Results The total life quality score in 86 cases was (58.17±13.22) scores. Scores in physiology domain was (17.73±2.99) scores, (16.23±4.20) scores in psychological domain, and (17.13±3.00) scores in environment. The multivariate Logistic regression results indicate that independent factors affecting the quality of life of ovarian cancer include: lower degree (OR = 0.604, 95%CI: 0.305-0.925), family monthly income ≤3000 yuan (OR = 2.506, 95%CI: 1.214-10.841), self-funded (OR = 2.577, 95%CI: 1.396-6.937), and a large number of symptom groups (OR = 4.578, 95%CI: 1.138-18.153). Conclusion The overall quality of life of patients with ovarian cancer is low, and it is affected by factors such as academic qualifications, medical expenses, family monthly income, and number of symptom groups.

69 Study on the efficacy of high intensity focused ultrasound combined with postoperative uterine clearing in the treatment of placenta accreta

Objective To study the effect of high intensity focused ultrasound (HIFU) combined with uterine clearing on placenta accreta and its complications. Methods From January 2016 to June 2018, in University City Hospital Affiliated to Chongqing Medical University and the First Hospital Affiliated to Chongqing Medical University, 60 patients with placenta accreta were selected, according to random number table method, they were divided into control group and observation group, with 30 cases in each group. The control group was treated with transuterine arterial methotrexate embolization combined with uterine clearing, and the observation group was treated with high intensity focused ultrasound combined with postoperative uterine clearing. The blood loss, postoperative blood loss and one-off uterine clearance success rate, the rate of hysterectomy, the incidence of postoperative complications, postoperative pain score, serum human chorionic gonadotropin β(β-HCG), hospitalization time, uterine recovery time, menstrual recovery time, immune function index in the two groups were compared. Results There was no significant difference in the success rate of one-off uterine clearing between the two groups (P > 0.05), but the amount of intraoperative and postoperative bleeding in the observation group was lower than that in the control group (P < 0.05). The hysterectomy rate and the total incidence of postoperative complications in the observation group were lower than those in the control group (P < 0.05). The postoperative pain score was lower than that in the control group (P < 0.05). After operation, the serum β-HCG in the observation group was lower than that in the control group (P < 0.05). The recovery time of serum β-HCG, hospitalization time, uterine recovery time and menstrual recovery time in the observation group were shorter than those in the control group (P < 0.05). The indexes of CD3+ and CD4+/CD8+ in the two groups were lower than those before operation (P < 0.05), but the immune function indexes in the observation group were higher than those in the control group (P < 0.05). Conclusion High intensity focused ultrasound combined with postoperative hysterectomy can effectively reduce the amount of bleeding, complications and hysterectomy of patients on the basis of ensuring the effect of hysterectomy, which is conducive to the reduction of postoperative pain and abnormal immune function, and the prompt recovery of serum betahcg, uterus and menstruation of patients.

73 Clinical effect of Xiaoer Yanbian Granules in treatment of upper respiratory tract infection complicated with febrile convulsion

Objective To investigate the clinical efficacy and safety of Xiaoer Yanbian Granules in treatment of upper respiratory tract infection complicated with febrile convulsion. Methods From January 2015 to January 2018, in Yichang First People's Hospital, 238 children with upper respiratory tract infection complicated with febrile convulsion were selected, according to random number table method, they were divided into control group and treatment group, with 119 cases in each group. The control group was treated with conventional therapy, the treatment group was treated with Xiaoer Yanbian Granules on the basis of conventional therapy, the two groups were treated for 3-7 d. The clinical efficacy of the two groups was evaluated and compared, the defervescence time, fading time of coughing, fading time of nasal obstruction and nasal discharge,the fading time of convulsion and the cases of convulsion recurrence in the two groups was recorded and compared, serum level of neuron-specific enolase (NSE) and S-100β protein were measured and compared between the two groups before and after treatment. The adverse reactions in the two groups were recorded and compared. Results The total effective rate of the treatment group was higher than the control group, the difference was statistically significant (P < 0.05). The defervescence time, the fading time of coughing, the fading time of nasal obstruction and nasal discharge, fading time of convulsion and the cases of convulsion recurrence in the treatment group were significantly less than the control group, the differences were statistically significant (P < 0.05). After treatment, the serum NSE, S-100β level of the two groups were significantly lower than before treatment, and these in the treatment group were significantly lower than the control group, the differences were statistically significant (P < 0.05). During the treatment, the incidence of adverse reactions in the treatment group was higher than the control group, but the difference was not statistically significant (P > 0.05). Conclusion Xiaoer Yanbian Granules is effective in the treatment of upper respiratory tract infection complicated with febrile convulsion. It can shorten the duration of clinical symptoms and improve the brain injury of children, it is safe and worthy of clinical application.

77 Meta-analysis of the value of long-chain non-coding RNA-UCA1 in the diagnosis of bladder cancer

Objective To systematically evaluate the diagnostic value of long-chain non-coding RNA urothelial carcinoma associated gene 1 (lncRNA-UCA1) for bladder cancer by meta-analysis. Methods PubMed, Web of Science, EMBASE, CNKI, Wanfang and VIP databases were retrieved. Literatures related to the use of lncRNA-UCA1 in the diagnosis of bladder cancer were collected, starting from the establishment of the database until July 31, 2018. According to inclusion and exclusion criteria, literatures were screened and data were extracted; QUADAS-2 item tool was used to evaluate the quality of literature, Meta-Disc 1.4 and Stata 12.0 software were used for statistical analysis, and statistics on UCA1 for the diagnosis of bladder cancer were combined. Results A total of 6 articles were included, including 919 cases of bladder cancer and 418 cases of non-cancer control. The combined sensitivity of lncRNA-UCA1 was 0.78 (95% CI: 0.76-0.80), specificity was 0.87 (95% CI: 0.85-0.90), and area under curve (AUC) was 0.93. Subgroup analysis showed that the detection efficiency of lncRNA-UCA1 based on GAPDH as internal reference (AUC: 0.98) was higher than that of non-GAPDH (AUC: 0.85); the detection efficiency of lncRNA-UCA1 based on urine as detection matrix was higher (sensitivity: 0.78; specificity: 0.86; AUC: 0.93). Conclusion lncRNA-UCA1 has high clinical value in the diagnosis of bladder cancer.

81 Diagnostic and therapeutic value of preoperative CT-guided Hook-wire localization for synchronous multiple primary lung cancer

Objective To evaluate the efficacy and safety of CT-guided Hook-wire localization in the treatment of synchronous multiple primary lung cancer (SMPLC) before video-assisted thoracoscopic surgery (VATS). Methods From February 2011 to December 2016, 47 patients with SMPLC admitted to the thoracic surgery department of Jiangyin People′s Hospital Affiliated to Southeast University were retrospectively analyzed. All patients were treated with Hook-wire localization needle under CT guidance, and VATS was performed to remove the lesions. Further surgical methods were determined according to the frozen pathological results during the operation. Positioning accuracy, the success rate of VATS lung lesion resection surgery, postoperative complications and other indicators were calculated. Results Among 47 patients, there were 19 patients with unilateral SMPLC and 28 with bilateral SMPLC. All resections of lesions which guided by the inserted Hook-wire were successfully performed by VATS (success rate was 100%). The mean procedure time for the CT-guided Hook-wire localization was (34.98±5.95) min. The major complication of CT-guided Hook-wire localization was slight pneumothorax (lung compression<20%) in 7 patients(14.9%), no one needed chest tube drainage. Moderate pneumothora (lung compression ≥20%) were observed in 4 patients(8.6%). Contralateral pneumothorax occurred during operation in 2 patients (4.3%). Two patients (4.3%) had minimal hemothorax (≤50 mL) during VATS. One patient (2.1%) underwent additional wedge resection by VATS during the same procedure because no lesion was identified in the surgical specimen. Conversion thoracotomy were required in 2 patients (4.3%) for pleural adhesions. None of Hook-wires dislodged or displaced during the transport to the surgical suite. Conclusion Preoperative CT-guided Hook-wire localization is accurate, safe and has less complications. It is of great clinical value in the diagnosis and treatment of SMPLC for VATS.

85 Expression and correlation analysis of miRNA-181a and miRNA-373 in bone marrow tissue of patients with multiple myeloma

Objective To investigate the expression of microRNA-181a (miRNA-181a) and miRNA-373 in bone marrow of patients with multiple myeloma (MM) and to explore its clinical significance. Methods A total of 67 patients with MM who were treated in Taizhou First People′s Hospital from March 2015 to March 2017 were selected as the case group, 40 patients with other blood diseases in the same period were selected as the case control group and 40 healthy people were selected as the healthy control group. Quantitative  real-time PCR (qRT-PCR) was used to detect the expression of miRNA-181a and miRNA-373 in bone marrow tissues of each group. The differences in the expression of miRNA-181a and miRNA-373 between these groups and their correlation with clinicopathological features of MM were analysed. Pearson linear correlation analysis were used to show the correlation between miRNA-181a and miRNA-373 expression. Results Compared with the case control group and the healthy control group, the expression of miRNA-181a was significantly higher in the case group and the expression of miRNA-373 was significantly lower (P < 0.05), but there was no difference in the expression of miRNA-181a and miRNA-373 between the case control group and the healthy control group (P > 0.05). The expression of miRNA-181a and miRNA-373 in bone marrow tissue of MM patients was related to tumor stage and tumor differentiation (P < 0.05), but not related to age, gender, cytological type and lymph node metastasis (P > 0.05). There was a significant negative correlation between miRNA-181a and miRNA-373 expression in the bone marrow of the case group (r = -0.696, P < 0.05). There was no significant correlation between the case control group and the healthy control group (r = 0.151, 0.179, P > 0.05). Conclusion The expression of miRNA-181a is increased in MM bone marrow tissue, and the expression of miRNA-373 is decreased. Both of them participate in the tumor development and progression, which may become a new marker for diagnosis, treatment and prognosis of MM.

89 Expression and clinical significance of cIAP and Caspase-7 in endometrial adenocarcinoma

Objective To study the expression and clinical significance of apoptosis inhibitory protein (cIAP) and caspase-7 in endometrial adenocarcinoma tissues. Methods The expression of cIAP and Caspase-7 in 80 cases of endometrial adenocarcinoma, 50 normal endometrium tissues and 50 cases of endometrial dysplasia were examined by immunohistochemical SP method in the Second Affiliated Hospital of Chongqing Medical University from January 2015 to December 2017, and the expression of cIAP, Caspase-7 and intrauterine were analyzed. The relationship between the clinicopathological characteristics and the expression of cIAP and Caspase-7 in endometrial adenocarcinoma. Results The positive rate of cIAP in endometrium adenocarcinoma was higher than that of endometrium atypical hyperplasia and normal endometrium. The positive rate of Caspase-7 was lower than that of endometrium atypical hyperplasia and normal endometrium (P < 0.05). The endometrium of endometrium was atypical hyperplasia tissue and endometrium was positive. There was no significant difference in cIAP positive rate and Caspase-7 positive rate between normal tissues (P > 0.05). The positive rate of cIAP in the tissues of G1 to G2, FIGO stage Ⅰ to Ⅱ, and non lymph node metastasis of endometrial adenocarcinoma was lower than that of histological classification G3, FIGO stage Ⅲ to Ⅳ, and lymph node metastasis. The positive rate of Caspase-7 was higher than that of histological classification G3, FIGO stage Ⅲ to Ⅳ, and lymph node metastasis, all of them was statistically significant (P < 0.05). Spearman correlation analysis showed that the expression of cIAP and Caspase-7 in endometrial adenocarcinoma was negatively correlated (r = -0.843, P = 0.039). 93 Risk factors analysis of intraoperative vascular injury during laparoscopic-assisted gastrectomy for gastric cancer

Objective To explore the risk factors of vascular injury in laparoscopic radical gastrectomy and its influence on postoperative recovery. Methods The clinical data of 136 consecutive patients with gastric cancer who underwent laparoscopic radical gastrectomy in Department of Gastrointestinal Surgery, the First Hospital of Jiaxing Zhejiang Province, from January 2015 to December 2017 were retrospectively analyzed. The risk factors of vascular injury and its effect on postoperative recovery were analyzed. Results Among the 136 patients, 84 patients were in the injured group and 52 patients were in the non-injured group. In the comparison study between the injured group and the non-injured group, age > 55 years (OR = 2.304, 95%CI: 1.072-4.950, P < 0.05), diameter of tumor>3.5 cm (OR = 2.318, 95%CI: 1.027-5.232, P < 0.05), lymph node metastasis (OR = 5.015, 95%CI: 1.348-18.656, P < 0.05) were independent risk factors for intraoperative vascular injury. Compared with the non-injured group, the postoperative hospitalization days were significantly longer in the injured group (P < 0.05), and the amount of blood loss was higher (P < 0.01). Postoperative complications were more likely to occur when intraoperative blood loss was 200 mL or above (P < 0.01). Conclusion Advanced age, large tumor and lymph node invasion are independent risk factors for vascular injury in laparoscopic radical gastrectomy. When vascular injury occurs during the operation, the postoperative hospitalization time is significantly prolonged, even affecting the occurrence of complications.

97 Effect of Tegafur, Gimeracil and Oteracil Porassium Capsules combined with Oxaliplatin on the expression of gastrin and survivin antibodies in serum of patients with advanced gastric cancer

Objective To investigate the clinical efficacy of Tegafur, Gimeracil and Oteracil Porassium Capsules  combined with Oxaliplatin in the treatment of advanced gastric cancer and its effect on the expression of gastrin (GAS) and survivin antibodies in serum. Methods A total of 100 patients with advanced gastric cancer treated in the 4th People′s Hospital of Shenyang City from January 2015 to June 2017 were divided into observation group and control group by random envelope method, with 50 cases in each group. The control group was treated with Capecitabine combined with Oxaliplatin, and the observation group was treated with Tegafur, Gimeracil and Oteracil Porassium Capsules combined with Oxaliplatin. Both groups were treated for 4 cycles. After treatment, the short-term clinical efficacy was assessed, and adverse reactions and quality of life were evaluated. Blood GAS and survivin antibody levels were detected before and after treatment. Results The short-term total effective rate of treatment in the observation group was significantly higher than that in the control group(P < 0.05). The total improvement rate of the quality of life in the observation group after treatment was significantly higher than that of the control group (P < 0.05). GAS and survivin of patients in the two groups significantly decreased after treatment (P < 0.05). The GAS and survivin antibodies in the blood of the patients were significantly lower than those in the control group (P < 0.05), and the incidence of nausea, vomiting and diarrhea in the observation group was significantly lower than that of the control group (P < 0.05). Conclusion The combination of Tegafur, Gimeracil and Oteracil Porassium Capsules and Oxaliplatin can effectively improve the clinical efficacy and reduce the levels of serum GAS and survivin antibody in patients with advanced gastric cancer. It is of high clinical value.

101 The clinical result of progressive gastric cancer after operation by three-dimensional conformal radiotherapy combined with Kapetabine chemotherapy

Objective To investigate the clinical effect of postoperative three-dimensional conformal radiotherapy combined with capecitabine chemotherapy in the treatment of advanced gastric cancer. Method The 150 cases' clinical data of progressive gastric cancer in the Fifth People's Hospital Qinghai province from January 2012 to December 2017 were analyzed retrospectively, and they were divided into two groups, 50 patients in the chemotherapy group (capecitabine chemotherapy) and 100 cases in the radiotherapy combined chemotherapy group (three-dimensional conformal radiotherapy combined Kapetabine chemotherapy) were treated for 2 consecutive cycles. The quality of life of patients in the two groups before and after treatment was observed, and the clinical efficacy and incidence of adverse reactions were observed. Results The life quality scores of patients in the two groups before treatment had no significant differences (P > 0.05). Quality of life scores in the two groups after treatment were higher than before the treatment, chemotherapy and radiotherapy patients quality of life score in the radiotherapy combined chemotherapy group was higher than chemotherapy group, remission rate of patients in the radiotherapy combined chemotherapy group was higher than chemotherapy group, leukopenia, hemoglobin reduces, the incidence of nausea and vomiting of patients in chemotherapy group were higher than chemotherapy group, the difference had statistical significance (P < 0.05), There was no significant difference in the incidence of thrombocytopenia and extremity numbness between the two groups (P > 0.05). Conclusion The progressive gastric cancer after operation by three-dimensional conformal radiotherapy combined with kapetabine chemotherapy, which could increase life quality and the clinical treatment remission rate, but the incidence of leucopenia, reducing hemoglobin, and nausea vomiting rate are high.

105 Risk factors for type 2 diabetes mellitus with atrial fibrillation

Objective To investigate the risk factors of atrial fibrillation (AF) in type 2 diabetes mellitus (T2DM). Methods From January 2016 to January 2018, 530 cases of T2DM patients (control group), 533 patients with T2DM combined with AF (T2DM combined with AF group) admitted to the Affiliated Hospital of Xuzhou Medical University were selected, 533 patients with T2DM combined with AF were divided into two subgroups, including 220 patients with paroxysmal AF (T2DM combined with PAF group) and 313 patients with non-paroxysmal AF (T2DM combined with NPAF group). Relevant indicators were collected for univariate anova and Logistic regression analysis, influencing factors were screened out, and their predictive value was analyzed by ROC. Results Univariate analysis showed that there were statistically significant differences in the levels of uric acid, blood glucose, glycosylated hemoglobin, bilirubin, low-density lipoprotein and cholesterol between the control group and T2DM combined with AF group (P < 0.05). Logistic regression analysis showed that uric acid (OR = 1.005, P < 0.05) and glycosylated hemoglobin (OR = 1.398, P < 0.05) were risk factors for the occurrence of AF in T2DM. Total bilirubin (OR = 0.929, P < 0.05), low-density lipoprotein (OR = 0.285, P < 0.05) and cholesterol (OR = 0.610, P < 0.05) were protective factors for the occurrence of AF in T2DM patients. ROC curve analysis showed that the above indicators have certain predictive value for the occurrence of atrial fibrillatione. Conclusion Uric acid and glycosylated hemoglobin are independent risk factors for atrial fibrillation in patients with T2DM, while total bilirubin, low-density lipoprotein and cholesterol are the protective factors of atrial fibrillation.

109 Expression and significance of serum heparin binding protein in acute renal failure in patients with sepsis

Objective To investigate the purpose of this study was to investigate the expression and significance of serum heparin binding protein (HBP) in the process of acute renal failure in patients with sepsis and to understand its clinical value. Methods From September 2014 to September 2015, 124 patients with sepsis admitted to the emergency Department of Beijing Luhe Hospital Affiliated to Capital Medical University were selected. According to the occurrence of acute renal failure, 42 patients in the acute renal failure group with sepsis (group A), 82 patients in the group without acute renal failure group with sepsis (group B), 60 patients with acute renal failure without sepsis (group C), and 60 healthy volunteers (group D) were selected as the healthy control group. Venous blood was collected in each group within 24 h after admission and after corresponding treatment, and serum HBP and serum creatinine concentrations were measured. Results Before treatment, compared with the healthy control group, the HBP level of group A and group B was significantly higher than that of group C and group D (P < 0.05), while the serum HBP level of group A with acute renal failure was significantly higher than that of group B (P < 0.05). After treatment, only group A showed A significant decrease in HBP level (P < 0.05). Conclusion HBP can be used as a method to evaluate the occurrence of acute renal failure in patients with sepsis and to evaluate its therapeutic effect.

112 Effect of magnesium valproate on serum total bilirubin, albumin, inflammatory factors and quality of life in patients with bipolar affective disorder

Objective To analyze the effect of magnesium valproate on serum total bilirubin (TBIL), albumin, inflammatory factor level and quality of life in patients with bipolar affective disorder. Methods The clinical data of 100 patients with bipolar affective disorder admitted in Guangxi Zhuang Autonomous Region Brain Hospital from August 2017 to July 2018 were retrospectively analyzed and divided into control group and observation group according to different treatment methods, each group had 50 cases. The patients in the control group were treated with lithium carbonate, and the patients in the observation group were treated with magnesium valproate, to analyze the effect of the two groups after treatment. Results The total effective rate in the observation group was significantly higher than that in the control group, the difference was statistically significant (P < 0.05). After treatment, the levels of tumor necrosis factor- α (TNF- α) and uric acid (UA) decreased, TBIL, serum albumin increased after treatment, and the levels of TNF- α and UA in the observation group were lower than those in the control group, and the serum albumin level of TBIL was higher than that of the control group, and the differences were statistically significant (all P < 0.05). After treatment, the scores of hamilton depression scale (HAMD), mania scale (BRMS) and health survey summary form (SF-36) were decreased, and the HAMD and BRMS scores of the patients in the observation group were lower than those of the control group, and the SF-36 score was higher than that of the control group, and the differences were statistically significant (all P < 0.05). The incidence of adverse reactions in the observation group was significantly lower than that in the control group, the difference was statistically significant (P < 0.05). Conclusion The effect of magnesium valproate on bipolar affective disorder patients is obvious. It can improve the quality of life, reduce the condition of mania and depression, and reduce the incidence of adverse reactions. It is worth popularizing and applying in clinic.

116 Effects of procalcitonin, Hypersensitive C-reactive protein, erythrocyte sedimentation rate and S100A9 on pulmonary tuberculosis complicated with bacterial infection

Objective To compare the effects of procalcitonin (PCT), hypersensitive C-reactive protein (hs-CRP), erythrocyte sedimentation rate (ESR) and S100A9 protein on pulmonary tuberculosis complicated by bacterial infection. Methods Form September 2017 to August 2018 in Beijing Chest Hospital Affiliated to Capital University of Medical Sciences, 132 cases were diagnosed as tuberculosis, and according to whether the pulmonary bacterial infection they were divided into bacterial infection group (62 cases) and simple tuberculosis group (70 cases), the peripheral blood plasma samples application immunofluorescence test PCT, ELISA method for determination of S100A9 level, immunoturbidimetry was used to determine hs-CRP, the ESR was determined by the Weiss method. Results The levels of PCT, S100A9 and hs-CRP in the bacterial infection group were significantly higher than those in the pure tuberculosis group (P < 0.01). Multivariate analysis showed that PCT and S100A9 were independent risk factors for pulmonary tuberculosis complicated by bacterial infection. Conclusion The levels of PCT and S100A9 in pulmonary tuberculosis patients have good clinical application value in monitoring whether the patients are complicated with pulmonary bacterial infection.

120 Meta analysis of the treatment of dislocation of acromioclavicular joint by loop plate and clavicular hook plate

Objective To systematically evaluate the curative effect of Endobutton plate and clavicular hook plate(CHP) in the treatment of acromioclavicular joint dislocation. Methods The Cochrane Library, PubMed, CNKI and Wanfang, CBM databases were searched by computer (from the database establishment to November 2017). Two researchers independently screened literature, assessed the methodological quality of literature, and extracted data. Then Meta-analysis was performed with Review Manager 5.3, according to the treatment divided into Endobutton plate group and Clavicular hook plate group. Results In the end, 33 articles met the inclusion criteria, and a total of 1718 patients were included. Among them Endbutton plate group 825 cases, Clavicular hook place group 893 cases. The results of Meta analysis showed that Endobutton plate group were superior to Clavicular hook plate group in the shoulder-joint functional Constant-Murley score [WMD = 6.64, 95%CI (3.70, 9.57), P < 0.000 1], shoulder Karlsson classification [OR = 3.92, 95%CI (2.72,5.66), P < 0.000 01], shoulder mobility recovery [WMD = 13.85,95%CI (10.36,17.34), P < 0.000 1], intraoperative blood loss [WMD = 11.38, 95%CI (-18.22, -4.54), P = 0.001]. However, there was no significant difference between the two groups in the time of operation [WMD = 1.28, 95%CI (-4.38,6.94), P = 0.66]. Conclusion Compared with the internal fixation of clavicular hook plate in the treatment of acromioclavicular joint dislocation, the internal fixation with endobutton plate can better improve the function of shoulder joint, increase the activity of shoulder joint and reduce the intraoperative blood loss, which is an effective and safe operation the way.

126 Effert compartion of vertebral artery atherosclerotic stenosis by ISR and drug treatment

Objective To investigate the effect of vertebral artery stenting and drug therapy on vertebral artery atherosclerosis stenosis. Methods The clinical data of 80 patients with vertebral artery atherosclerotic stenosis from January 2012 to December 2017 in the First People′s Hospital of Ziyang of Sichuan Province were retrospectively analyzed. They were divided into control group (given drug treatment) and observation group (given vertebral artery stenting) according to the treatment method, 40 cases in each group. Relative cerebral blood flow (rCBF), relative mean transit time (rMTT), relative cerebral blood volume (rCBV) and relative peak time (rTTP) were observed before and after treatment in the two groups. The clinical efficacy, stroke and mortality in the two groups were observed after 6 months. Results There was no significant difference in rCBF, rMTT, rCBV and rTTP between the two groups before and after treatment (P > 0.05); there was no significant difference in rCBF, rMTT, rCBV and rTTP in the control group before and after treatment (P > 0.05); there was no significant difference in rCBV in the observation group before and after treatment (P > 0.05); after treatment, rCBF in the observation group was higher, while rMTT, rTTP were lower than before treatment, the difference were statistical significance (P > 0.05). The clinical treatment efficiency of the observation group was higher than that of the control group, the difference was statistically significant (P < 0.05). There was no significant difference in stroke and mortality between the two groups 6 months after treatment (P > 0.05). Conclusion Vertebral artery stenting can significantly improve rCBF, rMTT and rTTP in patients with vertebral artery atherosclerotic stenosis, and its clinical effect is good, which is worthy of clinical application.

130 Clinical effect of antibacterial silver ion dressing combined with Prandll liquid dressing on diabetic foot

Objective To investigate the clinical effect of antibacterial silver ion dressing combined with Prandll liquid dressing in the treatment of diabetic foot. Methods The clinical data of 120 patients with diabetic foot admitted to Xinhua Hospital Affiliated to Shanghai Jiaotong University School of Medicine from February 2017 to June 2018 were analyzed. According to the different dressings, there were 60 patients in the combined dressing group and 60 patients in the Prandll liquid dressing group. The debridement time, granulation tissue growth time and epithelial formation time of the two groups were observed. The Wagner classification and treatment effects of the two groups were observed. Results The debridement time, granulation tissue growth time and epithelial formation time of the combined dressing group were lower than those of the Prandll liquid dressing group, and the differences were statistically significant (P < 0.05). The Wagner classification of the combined dressing group was superior to the Prandll liquid dressing group, and the difference was statistically significant (P < 0.05). The total effective rate of clinical treatment in the combined dressing group was higher than that in the Prandll liquid dressing group, and the difference was statistically significant (P < 0.05). Conclusion Antibacterial silver ion dressing combined with Prandll liquid dressing in the treatment of diabetic foot patients has less trauma and faster recovery. Wagner grading improves significantly, and the effect is good, which is worthy of clinical application.

134 Efficacy and safety of Sitagliptin combined with insulin in the treatment of type 1 brittle diabetes

Objective To investigate the clinical efficacy and safety of Sigliptin combined with insulin in the treatment of type 1 brittle diabetes mellitus. Methods A total of 80 patients with type 1 brittle diabetes were selected from September 2016 to April 2018 in Tianyou Hospital, Wuhan University of Science and Technology. They were divided into control group and observation group by odd even random method, with 40 patients in each group. The control group was treated with insulin, while the observation group was treated with Sigliptin on the basis of the control group. The course of treatment was three months. The blood sugar index, islet function, incidence of adverse reactions and frequency of hypoglycemia were compared between the two groups. Results There was no significant difference in FBG, 2 h PBG and HbA1c between the two groups before treatment (P > 0.05). After treatment, FBG, 2 h PBG and HbA1c in the two groups were lower than those before treatment (P < 0.05), and PBG, 2 h PBG and HbA1c in the observation group were lower than those in the control group (P < 0.05). There was no significant difference in fasting C-peptide and postprandial 2 h C-peptide between the two groups before treatment (P > 0.05). After treatment, the fasting C-peptide and postprandial 2 h C-peptide in the two groups were higher than those before treatment (P < 0.05), and the fasting C-peptide and postprandial 2 h C-peptide in the observation group were higher than those in the control group (P < 0.05). The incidence of adverse reactions and the frequency of hypoglycemia in the observation group were significantly lower than those in the control group (P < 0.05). Conclusion Insulin can reduce blood sugar and improve islet function in patients with type 1 brittle diabetes mellitus, but the effect of Sitagliptin combined with insulin in the treatment of type 1 brittle diabetes mellitus is more significant, and the occurrence of hypoglycemic events is less, which can stabilize blood sugar well, and the incidence of adverse events is low. It has high safety and is worth promotion in clinic.

138 Clinical effect of Fuzheng Huayu Capsules combined with Magnesium Isoglycerate in the treatment of decompensated cirrhosis

Objective To investigate the effect of Fuzheng Huayu Capsules combined with Magnesium Isoglycerate on liver function and hepatic fibrosis in patients with decompensated cirrhosis. Methods A total of 120 cases with decompensated cirrhosis hospitalized in Tangshan City Hospital for Infectious Diseases and Affiliated Hospital of North China University of Technology from January 2015 to June 2017 were selected. According to random number table method, the patients were divided into control group and case group with 60 cases in each group. The control group was given conventional liver protection plus Magnesium Isoglycyrrhizate, and the case group was given conventional liver protection plus Magnesium Isoxalate and Fuzheng Huayu Capsules. Both groups were treated for 2 weeks. Clinical efficacy, liver fibrosis index and liver function of the two groups before and after treatment were observed. Results The effective rate was 95.00% in the case group, which was significantly higher than 81.67% in the control group, the difference was statistically significant (P < 0.05). After treatment, compared with before treatment, The levels of total bilirubin (TBIL), alanine aminotransferase (ALT), aspartate aminotransferase (AST), layer adhesion protein (LN), type Ⅳ collagen (CIV), hyaluronic acid (HA), and before collagen Ⅲ type (PCⅢ) were decreased, while the levels of albumin (ALB) and cholinesterase (CHE) were increased the difference was statistically significant (P < 0.05). After treatment, liver function and liver fibrosis index TBIL, ALT, AST, LN, CIV, HA, PCⅢ levels decreased, levels of ALB and CHE increased in the two groups, the differences were statistically significant (P < 0.05). After treatment, compared with the control group, levels of TBIL, ALT, AST, LN, CIV, HA, PCⅢ decreased, while the levels of ALB and CHE increased in the case group, the differences were statistically significant (P < 0.05). Conclusion The effect of Fuzheng Huayu Capsules combined with Magnesium Isoserate in the treatment of chronic liver cirrhosis is significant. It can obviously improve the liver function and the degree of liver fibrosis, and it has a synergistic effect.

142 Application effect of rational emotive therapy on psychological intervention in patients with hepatitis B cirrhosis in compensatory phase

Objective To explore the effect of rational emotional behavior therapy (REBT) on psychological intervention in patients with hepatitis B cirrhosis in compensatory phase. Methods A total of 92 patients with compensatory hepatitis B cirrhosis admitted to the First Affiliated Hospital of Xinjiang Medical University from January 2015 to December 2017 were selected as the study subjects. According to random number table method, they were randomly divided into observation group and control group, with 46 cases in each group. The control group received routine nursing, while the observation group on the basis of control group combined with REBT. The mental health, self-efficacy and self-management behaviors of the patients were evaluated and compared with the corresponding scale. Results The scores of interpersonal sensitivity, depression and anxiety in the psychosomatic symptom checklist 90 (SCL-90) in the observation group were significantly lower than those in the control group (P < 0.05). The scores and total scores of chronic self-efficacy scale (CDSES) and self-management behavior were significantly higher than those of the control group (P < 0.05). Conclusion Reasonable emotional therapy can obviously alleviate the negative emotions of patients with hepatitis B cirrhosis in the compensatory period, and then improve self-efficacy, promote the development of self-management behavior, which is of great significance to improve the quality of life of patients.

146 Effect of TCM multi-channel characteristic nursing program on gastrointestinal function and bladder function recovery

Objective To explore the effect of multi-channel characteristic nursing program of traditional Chinese medicine on the recovery of gastrointestinal function and bladder function in patients with cervical cancer after radical operation. Methods From January 2016 to March 2017, 86 patients with cervical cancer who underwent radical resection in Huangjiahu Hospital, Hubei University of Traditional Chinese Medicine were selected and they were divided into observation group and control group by random number table method with 43 cases in each group. The patients in the control group were given conventional basic nursing, while the observation group was given traditional Chinese medicine multi-channel nursing plan. The gastrointestinal function was observed and recorded at the first day after operation, and the bladder function recovery was compared between the two groups. Results The first defecation time, anal exhaust time and bowel sounds recovery time in the observation group were significantly shorter than those in the control group (P < 0.05). The recovery of bladder function in the observation group was significantly better than that in the control group, the incidence of urinary retention and catheterization was significantly lower than that of the control group, and the rate of self-relieving urination was relatively higher in the observation group; residual urine volume decreased significantly after extubation in the observation group (P < 0.05). Conclusion The multi-channel characteristic nursing scheme of TCM can obviously improve the gastrointestinal dysfunction and promote the recovery of bladder function in patients with cervical cancer after operation, which is worthy of clinical promotion and application.

150 Study on the application effect of clinical pathway nursing in patients with acute respiratory failure

Objective To evaluate the effectiveness and safety of clinical pathways on nursing work of patients with acute respiratory failure. Methods Eighty patients with acute respiratory failure treated in ICU of the Second Affiliated Hospital of Xi′an Jiaotong University from June 2017 to July 2018 were selected, they were divided into control group and observation group according to random number table method, 40 cases in each group. The patients in control group were given with routine nursing and observation group was intervented with clinical pathway nursing. ICU time, ventilation time, respiratory function recovery time, hospitalization cost, complications, and nursing service satisfaction of patients were compared between two groups, blood gas indexes of patients before and after nursing intervention were compared between two groups. Results The ICU time, ventilation time, respiratory function recovery time, hospitalization costs and the incidence of complications in the observation group were lower than those in the control group, the differences were statistically significant (P < 0.05). After the intervention, the partial oxygen pressure (PO2) of the patients in both groups were higher than those before the intervention, and the partial carbon dioxide pressure (PCO2) were lower than those before intervention, and PO2 of patients in the observation group was higher and PCO2 was lower than those in the control group, and the differences were statistically significant (P < 0.05). Conclusion Clinical pathway nursing might shorten the hospitalization time, the ventilation, and recovery time of respiratory function in patients with acute functional failure, reduce the hospitalization costs and the incidence of complications, improve the blood gas indexes and the patients′ satisfaction.

154 A case of Sipple syndrome and literature review

Sipple syndrome (MEN-2A) is a tumor syndrome that can affect multiple endocrine organs simultaneously or sequentially. As a rare familial inherited disease, it is associated with mutations in the RET gene located on the autosome. The RET gene is usually expressed on thyroid C cells, chromaffin cells and parathyroid cells, so Sipple syndrome is easily missed as one or more of Medullary carcinoma of the thyroid gland (MTC), pheochromocytoma (PHEO) and primary hyperparathyroidism (PHPT). One patient was admitted to the First Affiliated Hospital of Guangdong University of Traditional Chinese Medicine with a left adrenal mass, with the typical PHEO symptoms: palpitations and hyperhidrosis. During the diagnosis and treatment, multiple endocrine organs (adrenal, pancreas, parathyroid, thyroid) and non-endocrine organs (left femur) were found in the diagnosis, and MEN (multiple endocrine neoplasia) were diagnosed. MEN includes MEN-1 caused by mutation of MEN-1 gene and MEN-2 caused by mutation of RET gene.Aim to definite diagnosis, the patient took RET gene test and MEN-1 gene test after the surgery. The results were reported as MEN-2A caused by RET gene mutation. The lesions of Sipple syndrome often involve the adrenal, thyroid and parathyroid glands, so early diagnosis depends on the localization and qualitative diagnosis of pheochromocytoma, medullary thyroid carcinoma, and parathyroid adenoma. RET gene is the gold standard for the diagnosis of Sipple syndrome,and surgery is the main treatment for it.

Currently, there is no uniform standard scheme for calorie restriction programs, which is obviously controversial. The length of time, the composition of food, the feeding time and the type of animal may affect the results of the experiment. This paper reviews the relevant influence factors in the process of calorie restriction.

161 Distributions and antibiotic resistances of bloodstream infectious pathogens in the department of hyperbaric oxygen

Objective To investigate the distributions and antibiotic resistances of pathogens isolated from the patients with bloodstream infections (BSI) in Department of Hyperbaric Oxygen (HBO), the Sixth Medical Center of PLA General Hospital (“our hospital” for short). Methods There were 266 BSI cases (male 175, female 91) in the HBO department from January 2014 to December 2018. The blood samples to be inspected were cultured with BACT/ALERT 3D system of Biomerieux, France. The 282 isolated pathogens were identified and processed antimicrobial susceptibility tests with Microflex LT mass spectrometer of Bruker Daltonics, American and VITEK 2 Compact microbiology analyzer of Biomerieux, France. The data was processed and analyzed by WHONET 5.6 software and Excel software. Results Gram-negative bacteria with the isolate rate 55.0% were the predominant BSI pathogens in the HBO Department of our hospital during the 5 years, and the proportions of Gram-positive bacteria and fungi were 40.8% and 4.2%, respectively. Klebsiella pneumoniae, total 46 strains, with the ratio of 16.3% was the major one among the Gram-negative bacteria and presented the higher resistance rate to most antimicrobial, even to the Carbapenems, which was 39.1%. Staphylococcus, total 76 strains and with the ratio of 66.1%, was the major pathogen among the Gram-positive bacteria. The sensitivity of Staphylococcus aureus and Coagulase negative staphylococcus (Cons) to Vancomycin and Tigecycline were the lowest and no resistance strains were found. Totally 12 fungi were all Candida, with the resistance rates to Flucytosine, Fluconazole, Voriconazole and Amphotericin B were all 0.0%. Conclusion To regularly surveillance the distributions and antibiotic resistances of pathogens from the BSI patients in the HBO department is very significant for clinicians to perform empiric and preemptive therapy.

173 Research summary on the advantages of traditional Chinese medicine service

Under the background of “Healthy China” strategy and population aging, it is of great significance to make full use of the advantages of traditional Chinese medicine (TCM) services to promote the development of high quality health services. Through literature research, this paper sorts out the advantages of TCM services from the aspects of chronic diseases prevention and treatment, meeting the needs of public health services and improving health literacy. This paper summarizes the factors that weaken the advantages of TCM services from the aspects of insufficient development of TCM services ability, unbalanced development of TCM services and restrained demand of TCM services, and put forward relevant suggestions, which will be conducive to broaden the horizon of TCM services research, and can provide a basis for further study of TCM services superiorities.

177 The research of corporate government, corporate social responsibility and financial performance in relation to Chinese medicine companies

To study the change of corporate social responsible brought about by corporate governance, this paper uses the panel data of Chinese medicine corporate from 2014 to 2016 as samples, and uses empirical methods to analyze the relationship between CSR and financial performance. The result shows that the CSR of Chinese medicine has a positive impact on financial performance. The size of the board of directors, the proportion of independent directors in the board of directors and the separation of the chairman and the CEO have positive effects on the relationship between the CSR and financial performance of Chinese medicine companies. So it can be proved that the fulfillment of the CSR of Chinese medicine can promote their financial performance, the CSR under the effective corporate governance has a positive effect on the financial performance.
