HUAN Ke1,2 BAI Bo1 BAI Xiaozhi3 SU Fei1 CHEN Xiao4 SANG Hongxun1,5
Objective To study the therapeutic effect of the new nanocrystalline silver-zinc dressing on deep partial-thickness scald burn infection. Methods 90 male SD rats were divided into three groups by random number table, with 30 rats in each group. Two deep partial-thickness scald wounds infected with Pseudomonas aeruginosa were established in the back of rats with consistent area and depth. Group A was treated with nano-silver dressing, group B was treated with nanocrystalline silver-zinc dressing, and group C was treated with ordinary sterile dressings when dressing change. The concentrations of interleukin -1 (IL-1), tumor necrosis factor alpha (TNF-α), interleukin -10 (IL-10) and transforming growth factor beta (TGF-β) in the tissues were detected by ELISA at 3, 6, 10 and 14 days after injury. Immunohistochemistry staining was used to observe the expression of inflammatory cytokines in group B, and the wound healing time of each group was recorded. Results At 3, 6, 10 and 14 days after injury, the levels of inflammatory factors IL-1 and TNF-α in group B were lower than those in group A and C, and the trend decreased with time prolonged, with statistically significant difference (P < 0.05). The concentration of IL-10 and TGF-β in group B was higher than those in group A and C, and the trend increased with time prolonged, with statistically significant difference (P < 0.05). Immunohistochemical staining was showed that the expression of inflammatory factor TNF-α in group B decreased with the prolongation of treatment time, and the expression of IL-10 increased gradually. The wound healing time of group B was shorter than those of group A and C, with statistically significant difference (P < 0.05). Conclusion The new nanocrystalline silver-zinc dressing has a better anti-infection and healing effect on deep partial-thickness scald burn infection.

13 Study on the mechanism of Cisplatin chemosensitivity in cervical cancer cells based on miR21
YUE Xiaoxue MIAO Jinwei LU Pan
Objective To detect the mechanism of miR21 gene on the sensitivity of Hela and Cisplatin-resistant Hela/DDP cells to Cisplatin. Methods Mature miR21 mimic and negative control (NC) miRNA were transfected into Hela cells, while miR21 inhibitor and negative control (NC) miRNA were transfected into Hela/DDP cells by riboFECTTM  CP. Therefore, Hela cells were divided into mimic group, inhibitor group, NC group and blank group. Real-time PCR was used to measure the expression of PTEN mRNA in each group. The cell cycle was measured through PI method and apoptosis rate of cells after Cisplatin treatment was detected through AnnexinⅤ/PI by fluorescene activated cell sorter. Results Real-time PCR results showed that the expression of PTEN mRNA was an average of (0.410±0.046) fold higher in Hela/DDP than in Hela (P < 0.01). The expression of PTEN mRNA in mimic group was obviously lower than those in NC group and blank group (P < 0.01). The expression of PTEN mRNA in inhibitor group was significantly higher than those in NC group and blank group (P < 0.01). There was no statistical difference between NC group and blank group (P > 0.05) in Hela and Hela/DDP cells. The results of Annexin Ⅴ/PI showed that the apoptosis rate of mimic group was lower than NC group and blank group (P < 0.05), while the inhibitor group showed that the apoptosis rate was more than NC group and blank group (P < 0.05). The of cell proportion of S period in mimic group was higher than those of NC group and blank group (P < 0.05), and there was no significant difference between NC group and blank group (P > 0.05). In the meantime, the cell proportion of S period in inhibitor group was less than those of NC group and blank group (P < 0.05), and there was no significant difference between NC group and blank group (P > 0.05). Conclusion PTEN mRNA is low expressed in Hela/DDP cells and highly expressed in Hela cells. The up-regulated expression of miR21 in Hela can significantly reduce the expression of PTEN mRNA, reduce apoptosis and increase the proportion of S phase in the cell cycle, thereby resulting in Cisplatin resistance. The down-regulated expression of miR21 in Hela/DDP can increase the expression of PTEN mRNA, increase the apoptosis rate and reduce the proportion of S phase in the cell cycle, so as to increase the chemosensitivity of Cisplatin.

28 Research progress of incretin-based therapies on nonalcoholic fatty liver disease
PANG Baoying KUANG Hongyu▲
Type 2 diabetes mellitus is closely associated with non-alcoholic fatty liver disease (NAFLD), which is the most prevalent chronic liver disease worldwide. Currently, the only validated treatment for NAFLD is lifestyle intervention. Glucagon like peptide-1 receptor agonists and dipeptidyl peptidase-4 inhibitors, collectively termed incretin-based therapies, represent two of the most widely used classes of novel antidiabetic drugs. A number of studies of human trials have evaluated the effects of incretins on liver fat content and suggest that the treatment could represent a new alternative for NAFLD management. In this paper, the main clinical studies regarding the effects of incretins on NAFLD are reviewed.

31 Research progress in application of heart rate variability in pregnancy disease
ZHU Meilin ZHENG Wenhui CHENG Lili DONG Youjing
Particular physical and psychological changes in the maternal body during pregnancy can lead to the occurrence of certain diseases, such as gestational hypertension, gestational diabetes, gestational sleep apnea syndrome, etc. The development of pregnancy disease is closely related to the change of maternal autonomic nervous function during pregnancy, for example, the very low frequency (VLF) and low frequency (LF) of pregnant women with preeclampsia increased significantly. The high frequency (HF) of pregnant women with gestational diabetes decreased significantly at night. The LF/HF of pregnant women with sleep apnea syndrome increased. The 24 hours standard deviation of NN intervals, HF and LF of the patients with congenital heart disease were significantly decreased, and the Heart rate variability (HRV) variation of the mothers with arrhythmia was controversial. HRV connected by safe noninvasive means can monitor the change of autonomic nerve and it has important clinical value in the evaluation and prediction of pregnancy maternal system disease.

Objective To observe the clinical effect of open reduction with cannulated compression screw combined with medial support plate via direct anterior approach (DAA) in the treatment of Garden Ⅲ and Ⅳ femoral neck fractures in young adults. Methods Clinical data of 37 young patients with femoral neck fractures treated by cannulated compression screw combined with medial support plate via DAA approach in He′nan Luoyang Orthopedic Traumatological Hospita from November 2014 to September 2015 were enrolled in this retrospective study. Fracture healing and complications were followed up and observed. The hip function was evaluated by hip Harris score and the fracture reduction was evaluated by Garden reset index. Results In this study, 37 patients were followed up for more than 2 years. All patients achieved bone healing in 3-6 months after operation, the average healing time was (4.40±0.96) months. No loosening of internal fixation or displacement of fracture occurred. Two years after operation, one patient had osteonecrosis of the femoral head, then the conservative treatment was performed, the activity of the hip joint was not significantly affected. One patient experienced a decrease in sensation of the skin outside the thigh, which was considered to be caused by injury of the lateral femoral cutaneous nerve and was given the drug symptomatic treatment, and recovered after 5 months. The final follow-up of the Harris hip score was (89.66±4.52), which was significantly higher than the preoperative score (40.41 ± 6.98) (P < 0.05). There was no significant difference in the Garden reset index between 3 d after operation and 2 years after the operation (P > 0.05). Conclusion By using open reduction with hollow compression screw combined with medial support plate via DAA approach in the treatment of Garden Ⅲ and Ⅳ femoral neck fracture in young adults, the injury is small, the reduction is accurate and the internal fixation is firm, which is beneficial to early active rehabilitation after operation and to reduce the incidence of nonunion and necrosis of the femoral head. It is worth popularizing in clinic.

141 Analysis on the characteristics of dynamic electrocardiogram of premature ventricular contractions with syndromes of stirring wind due to yin deficiency
CHEN Jin1,2 ZHANG Limei1 SONG Qingqiao1 DU Bai1
Objective To observe the characteristic changes of dynamic electrocardiogram in patients who were diagnosed as premature ventricular contractions with syndromes of stirring wind due to yin deficiency. Methods From November 2011 to August 2016, 90 patients who were diagnosed as premature ventricular contractions with syndromes of stirring wind due to yin deficiency in Guang'anmen Hospital, China Academy of Chinese Medical Sciences (“our hospital” for short) were selected as the premature ventricular contraction group, and 30 healthy subjects in our hospital were selected as the control group. All subjects underwent 24 hours dynamic electrocardiogram. The average heart rate, slowest heart rate, heart rate variability (HRV) of the two groups and the number of premature ventricular contraction in morning and night of premature ventricular contraction group, the correlation between TCM syndrome classification and HRV index in the premature ventricular contraction group were observed and analyzed. Results The levels of the average heart rate and the slowest heart rate in premature ventricular contraction group were all higher than those in control group (P < 0.05). The levels of standard deviation of NN intervals (SDNN), standard deviation of average 5 min NN intervals (SDANN), percentage of differences exceeding 50 ms between adjacent normal number of intervals (PNN50), square root of the mean squared differences of successive RR interval (RMSSD) and HRV trigonometric index in premature ventricular contraction group were lower than those in control group (P < 0.05). The premature ventricular contraction frequency of premature ventricular contraction group in the night was more than that in the morning (P < 0.05). The syndrome classification of traditional Chinese medicine in premature ventricular contraction group was positively correlated with age, which was negatively correlated with SDNN, SDANN, PNN50, RMSSD, HRV triangular index. Conclusion The premature ventricular contraction frequency of patients in premature ventricular contraction group in the night is more than that in the morning, and the HRV is lower than control group. It is speculated that the occurrence of premature ventricular contraction with syndrome of stirring wind due to yin deficiency is positively related to autonomic nerve dysfunction, mainly vagus function decline.

149 Professor Wang Guohua's experience in treating acne caused by polycystic ovary syndrome
PANG Ying WANG Guohua ZHAO Xiuping GAO Yue
In recent years, the incidence of polycystic ovary syndrome (PCOS) was increased year by year, among which, acne is the most distressing and concerned problem for most patients, while there are only few related reports. Professor Wang Guohua believes that as the changes of lifestyle, the main Chinese etiology and pathogenesis of acne caused by PCOS are qi depression and deficient fire. The main etiology are the disorder of qi movement caused by stagnation of liver qi and blood stasis and stagnated fire in lung meridian, and the flaring up of deficient fire caused by upper heat and lower cold and disharmony between heart and kidney. Professor Wang Guohua pays attention to “treatment aiming at its root causes” in clinical treatment, and focuses on the regulation of health education and lifestyle, she is also good at using modified classic prescriptions, which has received good clinical effects. It is worthy of learning and promotion.

175 Repair of rabbit steroid-induced avascular necrosis of femoral head by ADSCs injection through the joint puncture
TAN Weiyuan AN Rongze QI Xinwen XIE Jiaqing
Objective To investigate the effect of adipose derived stem cells (ADSCs) injection through the joint puncture on the repair of steroid-induced avascular necrosis of femoral head in rabbits. Methods ADSCs were isolated and passaged from the neck adipose tissue of adult New Zealand white rabbits. Thirty-six New Zealand white rabbits were used to establish the model of steroid-induced avascular necrosis of femoral head by horse serum combined with hormone. These rabbits were randomly divided into control group, core decompression group and ADSCs group (12 rabbits in each group), and all for bone density detection. Control group:no intervention; core decompression group: femoral head drilling decompression; ADSCs group:by intra-articular injection of ADSCs. At 4 weeks and 8 weeks after intervention, 6 rabbits in each group were examined with bone mineral density (BMD) and X-ray examination. After the death, their femur samples were collected for gross observation, HE staining and PCR were used for the measurement of BMP-2 mRNA. Through the comparison of each group to explore the effect of ADSCs repairing steroid-induced avascular necrosis of the femoral head by joint puncture injection. Results 4 weeks and 8 weeks after intervention three groups of BMD, but the control group decreased, compared with core decompression group and ADSCs group were statistically significant (P < 0.05), but there was no significant difference between the core decompression group and the ADSCs group (P > 0.05). General observation, imaging and histological findings at 4 weeks and 8 weeks after intervention, compared with the control group, ADSCs treatment by joint puncture injection and core decompression both can delay the progress of femoral head necrosis. There had a repair effect on femoral head necrosis, and the effects in ADSCs group were better than those in core decompression group; at 4 weeks and 8 weeks after intervention, the expression of BMP-2 mRNA in the ADSCs group and the core decompression group were both higher than those in the control group, the differences were statistically significant (P < 0.05), while there was no significant difference between the core decompression group and the ADSCs group (P > 0.05). Conclusion ADSCs injected through the joint puncture can repair the steroid induced avascular necrosis of the femoral head in rabbits.
