Effect of genistein on fertility of parental rats in the period of mating

[Abstract] Objective To explore the effect of genistein on body weight and fertility of parental rats in the period of mating. Methods After adaption for 1 week, 8 weeks female and male SD rats were randomized to 4 groups: high-dose group, middle-dose group, low-dose group and control group, 8 female rats and 4 male rats in each group. Female and male rats were mating at ratio of 2∶1. Different dosage of genistein were given at the beginning of mating fed with high-fat diet for 3 weeks. The high, meddle and low dose groups were given 5400, 1800, 600 mg genistein into every kg fodder diet respectively. No genistein was added in control group. Body weight was weighted every week. The organ weights of male and female rats were weighted, the sperm density of male rats and pragnancy rate of female rats were observed at the end of experiment. Results Compared with control group, body weight of female rats in high-dose group decreased, the difference was statistically significant (P < 0.05). Meanwhile, the sperm density of male rats and pregnancy rate of female rats reduced in high-dose genistein group. Conlusion Genistein can reduce fertility of parental rats in the period of mating.

Study of simulated microgravity affecting antitumor response of mesenchymal stem cells against A549 transplantation tumor

[Abstract] Objective To estimate the whole cell antigens (WCAs) of mesenchymal stem cells (MSCs) cultured in different gravity can generate different immune response against cancer in mice which induced by lung cancer cell line A549. Methods The MSCs was isolated and cultured adherent cells from marrow and a 2-dimensional clinostat (2D clinostat) was used to keep cells in continuous 2D rotation at a speed of 30 r/min around the horizontal axis, to mimc the microgravity (MMG), and the normal gravity (NG) was used as control. And then MSCs was to irradiate to X-ray (15 Gy) to get the WCAs. The experiment group (MMG group) was immunized with WACs (once/3 d, 2 weeks) and then the mice were challenged with A549 cells; the control mice immunized subcutaneously with PBS control (PBS group), MSCs-NG control (NG group) and A549 control (A549 group), mice were monitored for their tumor growth by measure the diameter; then the weight and volume was plotted; the expression of PCNA was monitored by immunohistochemisty. Results A well-established lung cancer (A549) model was established, MMG could improve the immune responses against A549 lung carcinoma (P < 0.05). The A549 group had strongest inhibit effect, while the PBS group had the biggst tumor, the tumor of MMG group and NG group were found beginning to grow until 6 d; and the size in MMG group was smaller than NG group (P < 0.05); the weight of tumor were measured on 12 d, it showed that the weight of NG group was (458.7±21.6) g, while in MMG was (315.6±18.5) g; the resist effet in MMG group was bigger than NG group (P < 0.05). HRP stain showed MMG could down-regulated the PCNA expression (P < 0.05). Conclusion Immunization of with MSCs getting WCAs can suppress tumor growth, MMG can enhance this suppressing effect, it can be observed PCNA expression down, the further immune mechanism need more exploring.

Incidence of low response of Clopidogrel and its risk factors in patients with coronary heart disease and type 2 diabetes treated with percutaneous coronary intervention

[Abstract] Objective To assess the incidence of low response of Clopidogrel and its risk factors in patients with coronary heart disease and type 2 diabetes treated with percutaneous coronary intervention (PCI). Methods A retrospective case-control study was adopted in this study. 210 patients with coronary heart disease and type 2 diabetes who underwent selective PCI in Beijing Anzhen Hospital Affiliated to Capital Medical University from January to September 2015 were selected. All patients received a loading dose of Clopidogrel 300 mg at least 6 h before PCI. All the patients were taken blood samples before Clopidogrel administration as the baseline and at least 24 h post-medication. ADP induced platelet aggregation was measured by light transmittance aggregometry. Low response of Clopidogrel was defined as the absolute difference of the platelet between baseline and post-medication ≤10%. The patients were divided into the normal group (n=133) and the low-response group (n=77), the incidence of low response of Clopidogrel was calculated, and the correlation between age, gender, smoking, blood pressure, blood lipid, HbA1c and low response of Clopidogrel was analyzed. Results The incidence of low response of Clopidogrel in patients with coronary heart disease and type 2 diabetes after selective PCI was 36.67%. There was no significance in the ADP induced platelet aggregation rates between the normal group and the low-response group at baseline (P > 0.05); the platelet aggregation rates of the two groups on 24 h post-medication were (62.62±9.23)% and (32.71±11.77)% respectively, there was significant difference between two groups (P < 0.01). Multivariate Logistic regression analysis found that high level of HbA1c was an independent risk factor of low response of Clopidogrel (P=0.037, OR=1.241, 95%CI 1.013-1.519). Conclusion High level of HbA1c is an independent risk factor of low response of Clopidogrel in patients with coronary heart disease and type 2 diabetes. Low response of Clopidogrel exists significantly in patients with coronary heart disease and type 2 diabetes, making controlled levels of HbA1c is clinically crucial.

Effects of calcitonin gene-related peptide receptor antagonist on ventricular arrhythmia after acute myocardial infarction in rats

[Abstract] Objective To observe the effect of calcitonin gene-related peptide (CGRP) receptor antagonist CGRP8-37 on ventricular arrhythmia after acute myocardial infarction in rats. Methods Twenty-four healthy adult male Sprague-Dawley rats weighing 250-300 g were selected and randomly assigned into three groups (n = 8): control group (sham group), coronary artery occlusion group (CAO group) and CGRP8-37 intervention for coronary artery occlusion in advance group (CGRP8-37-CAO group). The times of ventricular arrhythmia from 10 min before CAO to 60 min after CAO were calculated and analyzed statistically according to the types of ventricular arrhythmia including one ventricular premature beat, two consecutive ventricular premature beats, three consecutive ventricular premature beats, more than three consecutive ventricular premature beats. Results CAO could induce the occurrence of ventricular arrhythmias, the occurrence time was concentrated on 0-12 min of early ischemia and peaked around 6 min after CAO. Pretreatment of CGRP8-37 before CAO could significantly increase the frequency of arrhythmias, and the time course was significantly extended from 0-12 min in CAO group to 0-20 min in CGRP8-37-CAO group. Peaked time of ventricular premature beats appeared in 12 min after CAO. The occurrence of ventricular arrhythmias in sham group, CAO group and CGRP8-37-CAO group had no significant differences 10 min before CAO (P > 0.05). Compared with sham group, the occurrence of ventricular arrhythmias was increased in CAO group and CGRP8-37-CAO group, the differences were highly statistically significant (P < 0.01). Compared with CAO group, the occurrence of ventricular arrhythmia in CGRP8-37-CAO group was increased, the difference was highly statistically significant (P < 0.01). The time of differences appeared in ventricular arrhythmia was concentrated on 20, 30, 40 min after CAO. Conclusion CGRP8-37 can significantly increase the occurrence of ventricular arrhythmia after acute myocardial infarction in rats.

Molecular dynamics simulation on the molecular interactions of peptide deformylase and actinonin

[Abstract] Objective To investigate the binding modes between actinonin and peptide deformylases and to characterize the informations about how to design the specific HsPDF inhibitor. Methods The combination of molecular dynamics simulation and MM/GBSA free energy calculation was employed to study the interactions between several PDF enzymes and actinonin. The detailed interactions of each residues of protein and actinonin were calculated by MM/GBSA free energy decomposition. Results Three important differences in actinonin binding to PDFs were obtained as followed: actinonin had stronger binding interaction with the motif 1 of HsPDF than the motif 2, while actinonin had lower binding interaction with the motif 1 than the motif 2 for PDF1A and PDF2. Moreover, actinonin had stronger binding affinities with Leu131 and Met145 of HsPDF, but had no interaction with the corresponding ones of PDF1A and PDF2. In addition, actinonin interacted strongly with Trp207 of HsPDF but weakly with the corresponding residue of PDF1A and PDF2. Conclusion By using the method of molecular dynamics simulation, the interaction of actinonin and PDF model are studied, differences of HsPDF and PDF in the active site with other types are expounded, based on the design thinking of specific HsPDF inhibitors of actinonin is proposed.

Expression of transforming growth factor β in the liver and its biliary epithelial cells of rats with cholestatic liver disease

[Abstract] Objective To explore the expression of transforming growth factor β (TGF-β) in the liver and its biliary epithelial cells of rats with cholestatic liver disease. Methods Sixty healthy female SPF SD rats aged 6-8 weeks were divided into control group (d0 group), 7 days after common bile duct ligation group (d7 group), 14 days after common bile duct ligation group (d14 group) and 21 days after common bile duct ligation group (d21 group), with 15 rats in each group, among which, the common bile duct of d0 group was touched softly after laparotomy and then abdomen was closed immediately. Knodell histological activity index (HAI) grading was used to evaluate the hepatic histologic changes of rats, the expression of TGF-β mRNA in liver tissue and biliary epithelial cells was detected by PCR, the amount of biliary epithelial cells within 4 d after cell culture was calculated by colorimetric method, the proliferative effect of TGF-β1 for biliary epithelial cells was observed. Results The Knodell HAI scores were improved with the extension of postoperative time (P < 0.05); the levels of mRNA expression of TGF-β1, TGF-β2 and TGF-β3 were increased with the extension of cholestasis time (P < 0.05); the study in vitro showed that TGF-β1 could inhibit the proliferation of biliary epithelial cells. Conclusion In the cholestatic liver disease caused by common bile duct ligation, the liver damage and expression of TGF-β mRNA are increased with the extension of cholestasis time, the level of TGF-β1 is closely related to the genesis and development of cholestatic liver disease, with the increase of the expression of TGF-β1 in hyperplastic biliary epithelial cells, the incidence of cholestatic hepatic fibrosis will be increased relevantly.

Building and application of LNR surgical staging system in cervical tuberculous lymphadenitis

[Abstract] Objective To build a new type of surgical staging system and verify the predictive value and curative effect in patients with cervical tuberculosis lymphadenitis. Methods Medical records of 179 cervical tuberculosis lymphadenitis patients who underwent surgical treatment from January 2013 to July 2015 in Tuberculosis Hospital of Shaanxi Province were reviewed. The data of age, body mass index, lesion size, nodal involvement area, whether complicated with diabetes as well as the existence of multidrug-resistant were analyzed to screen the risk factors which affected curative effect and complications. According to these risk factors, the surgical staging system was built. Results The lesion diameter ≥3 cm, lymph node involvement ≥2 areas and multidrug-resistant positive were risk factors which significantly increased the occurrence of postoperative complications. The LNR surgical staging system based on lesion (L), node (N) and resistence (R) was divided into three stages. The incidences of complication had statistically significant differences among each stage (P < 0.05). Conclusion The new surgical clinical staging system for cervical tuberculous lymphadenitis has good predictive value for complications and guiding significance for curative effect.

Study on release and permeation behavior of Wenjingzhitong Gel Paste in vitro

[Abstract] Objective To study the release and permeation behavior of Wenjingzhitong Gel Paste in vitro to be used in the treatment of dysmenorrehea prevention and care and to study the influence of inclusion technique to the permeation behavior of essential oil. Methods Dialysis membrane and rat skin were used as the barriers respectively. The release and permeation experiment were carried out with Franz diffusion cell in vitro. Ligustilide, the main active ingredient of the monarch drug (Angelica sinensis) and ministerial drug (Ligusticum wallichii) in the prescription, was used as a marker, and measured by high performance liquid chromatography (HPLC) method. The accumulated and permeation amount were regressed linearly to time to calculate the release and permeation rate separately. Results The release equation of ligustilide from Wenjingzhitong Gel Paste was Q=4.37t+1.79, r=0.993; the release rate was 4.37 μg/(cm2·h) and the accumulated release ratio in 12 h was 90.9%. Permeation equation of Wenjingzhitong Gel Paste with inclusion technique applied to essential oil in 24 h was Q=1.45t+6.06 and the release rate was 1.45 μg/(cm2·h), while permeation equation without inclusion technique was Q=2.65t+11.99 and the release rate was 2.65 μg/(cm2·h). Conclusion The release rate of ligustilide from Wenjingzhitong Gel Paste is much greater than the permeation one, which can make ligustilide permeate sustainedly. Inclusion technique makes the permeation rate decrease little, thus the essential oil can be included in the cyclodectrin first and then be added into the hydrogel patch.

Determination of polysaccharide content in Jianpizhixie Capsules by UV- spectrophotometry and HPLC-ELSD method

[Abstract] Objective To establish a determination method for polysaccharide in Jianpizhixie Capsules Methods Anhydrous glucose was used as reference substance. Total sugar content was determined by phenol-sulfate acid method; 1.5mol/L hydrolyzed polysaccharide sulfate solution was used, the method of alkali neutralization and ethanol precipitation was used to get simple sugars. The HPLC-ELSD method was used to determine its content. Chromatographic condition: chromatographic column was Xbridge Amide column (4.6 mm×150 mm, 3.5 μm); Mobile phase was acetonitrile-0.1%triethylamine (75∶25), column temperature was 30℃, flow rate was 1.0 mL/min, temperature of ELSD was 95℃, gas flow rate was 3.0 L/min. Results Phenol-sulfate acid method of anhydrous glucose was linear in the ranges of 6.8-68.0 μg/mL (r2 = 0.9995), the average recovery rate was 96.1%. HPLC-ELSD of anhydrous glucose was linear in the ranges of 0.0507-0.8110 mg/mL (r2 = 0.9998), the average recovery was 97.1%, RSD = 1.4%. The average contents of two kinds methods for determination of polysaccharide were 22.3%, 15.2%. Conclusion The method is stable and feasible, has good repeatability and can be used as the polysaccharide content determination of Jianpizhixie Capsules.

Detection of antimutagenic effect of Genistein

[Abstract] Objective To evaluate the antimutagenic effect of Genistein. Methods L5178Y cells were cultivated, and grouped according to the different processing methods, the L5178Y cells were respectively exposed to the Genistein concentrations of 0.078, 0.156, 0.312, 0.625 μg/mL with 5 μg/mL methyl methanesulfonate (MMS)or 0.5 μg/mL mitomycin C (MMC) (positive mutation group), 5 μg/mL MMS or 0.5 μg/mL MMC (positive mutation control group), 0.5% DMSO (solvent control group), water (negative control group), 5 μg/mL MMS or 0.5 μg/mL MMC and 100 μg/mL VitC + 5 μg/mL MMS or 0.5 μg/mL MMC (anti mutagenesis positive control group). The antimutagenic effect of genistein were determined by the mouse lymphoma Assay in L5178Y cells, in the evaluation of antimutagenic effect three kinds of treatment (pre-treatment, silmutaneous-treatment and post-treatment) were used to detect the antimutagenic effects of Genistein and puerarin against gene mutations induced by MMS and MMC. Determination of mutant frequency of tk locus (TFT) was performed according to the microcell method. Results In pre-treatment and post-treatment, at all the tested concentrations the total mutation frequencies were lower than the MMS or MMC positive mutation control group the differences were statistically significant (P < 0.01). In simultaneous treatment, the total mutation frequencies at concentrations of 0.312, 0.625 μg/mL were lower than MMS and MMC positive mutation control group, the differences were statistically significant (P < 0.01). Conclusion Under the three kinds of treatment, Genistein shows a good role for preventing TK gene mutation induced by MMS or MMC, pre-treatment has the strongest inhibitory effect, had a good application and development prospect.

Advances in the research and application of dissolution method for oral solid dosage forms

[Abstract] Solid oral drugs must undergo dissolution in gastrointestinal fluid before they can be absorbed and reach the systemic circulation. Dissolution test is an in vitro method that simulates the disintegration and dissolution of solid oral drugs in the gastrointestinal tract, which plays a more and more important role in the prescription optimization and quality evaluation. A lot of studies on dissolution technology have been made to obtain higher levels of in vivo and in vitro correlation and ensure the consistency of the quality and efficacy of oral dosages. This paper summarized the research progress of dissolution medium and dissolution device and their applications in some particular dosages, it is hoped to provide some reference for other researchers committed to quality evaluation of oral solid preparation and development of generic drugs.

Relationship between calcium and stroke

[Abstract] Calcium plays a critical role in cerebral vascular system. However, calcium intake is generally low in Chinese population. It is still a debate on the relationship between calcium and stroke diseases. Herein, it is found that different conclusions have been made by recent studies. Most of them indicate that calcium has a negative correlation in the incidence of stroke. Some demonstrate that there is no obvious relationship between calcium and stroke. However, some even show that high calcium intake may increase the risk occurrence of stroke diseases. Recent meta-analysis presents that calcium is a protective factor of the stroke in the long-term follow-up. This article will review the relationship between calcium and stroke in detail.

Advances in computer-aided navigation technology in orthopedics

[Abstract] With the development of information science and life sciences, computer-assisted surgery technologies has opened up a new field for the development of surgical techniques. Computer-assisted surgical techniques is the use of modern digital imaging techniques such as CT, MRI, PET image data obtained by computer processing and analysis, simulated surgical procedure, combined with the spatial positioning and navigation system, intraoperative real-time three-dimensional visual positioning to carry out surgical navigation (3D Navigation), so that surgery tends to be more accurate and minimally invasive. Especially in recent years navigation in orthopedic applications, the navigation carried out under trauma, spine, joint, has become a recent hot spot, which is characterized by precise positioning, improving the accuracy of surgery, reducing the exposure time and surgery complications. From the development of navigation, system composition, clinical application, existing problems and so on are summarized in this artical, and the trend of the development outlook is put forward.

Application progress of intraperitoneal ovarian transposition in the patients of young SCC concurrent radiotherapy after radical resection and chemotherapy

[Abstract] Treatment of young patients with cervical squamous cell carcinoma is concerned in the clinic, treatment of previously mostly is concurrent radiotherapy after radical surgery and chemotherapy, radiotherapy for the treatment of ovarian function impairment is a thorny issue. In recent years, retained ovarian function operation of peritoneal ovarian transposition in young patients is widely used in clinical practice, and gradually becomes a hot topic of young patients with comprehensive treatment of cervical squamous cell carcinoma. Therefore, this article introduces the advanced applications of intraperitoneal ovarian transposition in the patients of young SCC concurrent radiotherapy after radical resection and chemotherapy. Preserving ovarian function of displacement of ovary transposition abdominal cavity.

Study on the correlation between work pressure, coping style and social support among nurses in private hospitals

[Abstract] Objective To investigate the work pressure, coping style and social support of nurses with different work experiences in private hospitals, and analyze the correlation between them, in order to provide guidance for further reducing nurses' vocational pressure and improving the work efficiency. Methods 402 registered nurses in 6 private hospitals of Huizhou City were selected by cluster random sampling method from May 2013 to July 2014, depending on different work experiences, the nurses were divided into two groups: 0-5 years group and >5-10 years group, each group had 201 cases. Vocational pressure source scale (including of patient care, nursing profession and work, time allocation and workload, work environment and resources, management and interpersonal aspects) was used to evaluate nurses' vocational pressure, scored with grade 1-4; simplified coping style questionnaire (including of positive response and negative response dimensions) was used to investigate and evaluate nurses' coping style; social support rating scale (including of objective and subjective support and utilization degree of social support) was used to evaluate social support of the nurses; the correlation of vocational pressure and coping style, social support was analyzed with Pearson correlation. Results In 0-5 years group and >5-10 years group, the results of vocational pressure source scale showed that, the scores of the nursing profession and work [(2.25±0.31) vs (2.85±0.55) points, t=5.521, P < 0.05], the environment and equipment [(2.45±0.50) vs (2.68±0.52) points, t=7.974, P < 0.05], the overall average working pressure [(2.56±0.48) vs (2.66±0.52) points, t=2.865, P < 0.05) were compared, the differences were statistically significant. Coping style scale in 0-5 years group and >5-10 years group, positive response dimension [(1.84±0.36) vs (1.79±0.41) points, t = 1.621, P > 0.05], negative coping dimension [(1.08±0.12) vs (1.11±0.15 ) points, t = 0.517, P > 0.05] were compared, the differences were not statistically significant. There was no significant difference (t = 1.750, P > 0.05) in social support [(20.50±2.15) vs (20.94±1.46) points] between 0-5 years group and >5-10 years group. The correlation analysis of the vocational pressure, coping style and social support of the nurses with different work experience in private hospitals showed that, vocational pressure was negatively correlated with positive coping style (r = -0.374, P < 0.05), positively correlated with negative coping (r = 0.421, P < 0.05), and negatively correlated with social support (r = -0.406, P < 0.05). Conclusion In private hospitals, vocational pressure is current among the nurses with different work experiences, but there is no difference about coping style and social support. Vocational pressure is correlated with coping style and social support in the nurses of private hospitals.

Effects of Sanqi Panaxnotoginseng Injection combined with Gliclazide and Melbine for diabetic nephropathy

[Abstract] Objective To discuss the clinical therapy effect and safety degree of type Ⅱ diabetic nephropathy by the intervention treatment of Sanqi Panaxnotoginseng Injection combined with Gliclazide and Melbine. Methods From September 2011 to December 2014, in Department of Endocrinology, the Fourth Affiliated Hospital of Guangxi Medical University, 80 patients with diabetic nephropathy (DN) were selected as research subjects, all of them belonged to the newest diagnosis standard of WHO of type Ⅱ diabetes patients and they were divided into observation group and control group according to random number table, with 40 patients in each group. The patients of two groups were educated with basic knowledge of diabetes before medical treatment, patients in control group were treated with Gliclazide sustained-release tablets by oral administration, 60 mg/time, 3 times/d and metformin hydrochloride enteric coated tablets by oral administration, 0.5 g/time, 3 times/d; patients in observation group were treated with injection of Sanqi Panaxnotoginseng Injection as supplement, 10 mL/time, twice/d. The therapy period involved three weeks. Before and after the therapy, all of the blood flow indexes, coagulation indexes, excretion of urinary protein and adverse reaction were recorded. Results After the therapy, patients in observation group had significant improvements in low shear blood viscosity, plasma viscosity and red cell assembling index, the differences were statistically significant (P < 0.05); the coagulation indexes of patients in observation group recovered to within the range of reference value and displayed great improvement compared with that of control group, the differences were statistically significant (P < 0.05); the urinary albumin excretion rate of patients in observation group were lower compared with that of control group, the differences were statistically significant (P < 0.05), while the testing result of serum creatinine of two groups, the differences was not statistically significant (P > 0.05). During the three-week therapy period patients from both groups showed no adverse effect. Conclusion There is good therapy effect and high safety degree of intervention treating type Ⅱ diabetic nephropathy with Sanqi Panaxnotoginseng Injection, which deserves further development.

Analysis on insulin requirement and glucose metabolism of diabetic nephropathy patients at different development stages

[Abstract] Objective To discuss the changes of blood glucose, insulin dosage of patients with diabetic nephropathy at the different stage of the disease. Methods From January 2009 to December 2014, in Guangzhou First People's Hospital, the clinical data of 63 patients with diabetic nephropathy were retrospectively analyzed, 5 periods were divided according to CKD and the relationship of hematodialysis, the insulin dosage of patients at CKD1-2 period (period Ⅰ), CDK3-4 period (period Ⅱ), CKD5 period before hemodialysis (period Ⅲ), hemodialysis 1-3 months(period Ⅳ), hemodialysis > 6 month (period Ⅴ), blood glucose, serum albumin, hemoglobin, hypoglycemia of patients were recorded. Results The insulin dosage of patients at periodⅠ[(0.63±0.21) U/kg] was higher than those at period Ⅱ and period Ⅲ[(0.43±0.18), (0.41±0.19) U/kg], the differences were statistically significant (P < 0.05); this at period Ⅲ was higher than those at period Ⅳ and period Ⅴ[(0.32±0.19), (0.28±0.21) U/kg], the differences were statistically significant (P < 0.05); the occurrence rates of hypoglycemia at period Ⅳ and period Ⅴ were higher than that at period Ⅲ, the differences were statistically significant (P < 0.05). Conclusion From CKD1-2 period to CKD3-4 period, insulin dosage of patients with diabetic nephropathy need to reduce, from CKD3-4 period to CKD5 period before dialysis, insulin dosage has no significant decrease, but after hemodialysis for a short time (1-3 months), insulin dosage need to further reduce, after long-term hemodialysis (more than 6 months), insulin dosage has no significant decrease, but the occurrence rate of hypoglycemia increases.

Clinical treatment study on acute renal failure caused by upper urinary tract calculi

[Abstract] Objective To explore the effective therapy for acute renal failure caused by upper urinary calculi, and to provide reference for the study on diagnosis and treatment of the future. Methods 67 cases of acute renal failure patients caused by upper urinary calculi from November 2013 to November 2014 in Affiliated Hospital of Hebei University of Engineering were selected, and they were divided into observation group (33 cases) adopted by percutaneous nephrolithotomy with holmium laser lithotripsy and control group (34 cases) took double positioning in vitro lithotripsy according to their order of treatment and individual will. Serum creatinine and urea nitrogen levels before and after surgery, postoperative renal function and urine output recovery time between two groups were recorded and compared. Results After surgery, serum creatinine and urea nitrogen levels in two groups were lower than those before surgery, with statistical differences (P < 0.05). Serum creatinine and urea nitrogen levels in two groups before and after surgery were compared, with no statistical differences (P > 0.05). Postoperative renal function and urine output recovery time in control group was (7.98±1.32), (7.23±1.54)d respectively, postoperative renal function and urine output recovery time in observation group was (6.14±1.99), (6.38±1.33)d respectively, the differences were highly statistically significant (P < 0.01). Conclusion Percutaneous nephrolithotomy with holmium laser lithotripsy and double positioning in vitro lithotripsy in clinical indicator have no significant difference, but patients with long time existence calculi or urethra narrow and there severe adhesion with relevant organization, should be preferred percutaneous nephrolithotomy with holmium laser lithotripsy, in order to improve postoperative recovery time and to ensure maximum completely get rid of stone.

Effect observation of behavior change theory in stages intervening exercise in patient with hemodialysis

[Abstract] Objective To explore effect of behavior change theory in stages intervening exercise in patient with hemodialysis. Methods 56 patients with maintenance hemodialysis in Hemodialysis Room of the Third People′s Hospital of Luoyang City, He′nan Province from December 2013 to December 2014 were selected as research objects. Behavior change theory in stages was applied in intervening exercise of patient. Followed up for 12 months, exercise status of hemodialysis patients before and after intervention were investigated by using exercise benefit and barrier scale of hemodialysis patient. The number of adherence to exercise and exercise adherence before and after intervention were monitored. The dates were counted and analyzed. Results After intervention, exercise benefit and barrier scores of hemodialysis patients were increased significantly, the number of adherence to exercise was increased, exercise adherence was improved, compared with before intervention, the differences were statistically significant (P < 0.05). Conclusion Behavior change theory in stages intervening exercise in patient with hemodialysis can effectively improve exercise behavior of patient.

Changes of 25 hydroxy vitamin D3 in postmenopausal women between 50 and 60 years of age with fractures

[Abstract] Objective To investigate the changes of bone metabolism markers in postmenopausal women between 50 and 60 years of age with fractures. Methods From July 2014 to June 2015, in Shanghai Pudong New Area Gongli Hospital, 60 postmenopausal women between 50 and 60 years old [average age was (56.7±3.3) years] with fractures treated in were selected. The patients were divided into two groups by years after postmenopause, including 30 cases of 0-5 years (average: 54.5±3.3) after postmenopause, 30 cases of 5-10 years [average age was (58.8±0.9) years] after postmenopause. All the patients were measured height, weight, and bone metabolism markers, including 25 hydroxy vitamin D3 [25-(OH)D3], bone isoenzyme of alkaline phosphatase (BALP), Osteocalcin (OC), C-terminal crosslinking telopeptides of type Ⅰ collagen (u-NTX), type Ⅰ procollagen carboxy-terminal peptide (PICP) and N-terminal crosslinking telopeptides of type Ⅰ collagen (s-CTX). The differences in general and bone metabolism between the two groups were analyzed. Results Serum levels of 25-(OH)D3 in 5-10 years after postmenopause group decreased significantly compared with 0-5 years after postmenopause group, the differences were statistically significant (P < 0.05). There was no significant difference in serum BALP, OC, s-CTX, PICP, and u-NTX between 0-5 years postmenopause group and 5-10 years after postmenopause group, the differences were not statistically significant (P > 0.05). Conclusion Serum levels of 25-(OH)D3 decreased while the years after postmenopause increased. Postmenopausal women between 50 and 60 years old should promptly add vitamin D3 in order to prevent and delay the onset of fracture in postmenopausal women between 50 and 60 years old.

Occurring sites of anterior cruciate ligament rupture and multivariate Logistic regression analysis for its influencing factors

[Abstract] Objective To investigate the occurring sites of anterior cruciate ligament rupture and explore its influencing factors. Methods From October 2004 to August 2015, in People's Hospital of Tangshan City, the clinical data of 78 patients with injury and confirmed by arthroscopic probe substance section ligament complete rupture were selected. The incidence of the occurring upper, middle, lower site of ACL was analyzed. The influence of age, sex, knee injury mechanisms and injury joint position on the fracture site was analyzed by multivariate Logistic regression analysis. Results Three sections of the fracture incidence were 78.21% (61/78), 20.51% (16/78), 1.28% (1/78) (P < 0.05); the probability of patients with different age and different injury mechanism occured ACL upper site probability was different (P < 0.05); gender, lesion side, damage joints were not related to site of ACL (P > 0.05); compared with ≥30 years, those who were <30 years were more prone to ACL on segment fracture, the risk of increased 4.855 times (OR = 4.855, 95%CI: 2.659-9.004, P = 0.000); compared with the non-contact, the risk of rupture of the segment ACL contact occurs rose 10.549 times (OR = 10.549, 95%CI: 5.156-15.266, P = 0.000). Conclusion Nearly three-quarters of ACL occurs on the first segment, about 1/4 occurs in the middle segment and lower segments rarely occurs; cleavage site may influenced by age, injury mechanisms.

Influence of Gucixiao Mixtures on clinical curative effect and serological related indicators in patients with knee osteoarthritis

[Abstract] Objective To observe influence of Gucixiao Mixtures on clinical curative effect and serum superoxide dismutase (SOD), nitric oxide (NO) and tumor necrosis factor-α (TNF-α) in patients with knee osteoarthritis. Methods 60 patients with knee osteoarthritis in Orthopaedics Outpatient of Yangzhou Affiliated Hospital of Nanjing University of Traditional Chinese Medicine from January 2013 to March 2015 were selected and divided into two groups by random number table method. Patients in treatment group (n=30) received 40 mL oral Gucixiao Mixtures combined with 0.5 g centipede powder once daily after supper and discontinuation for one day per week for 5 weeks; patients in control group (n=30) received 3 tablets Jinwugutong Capsules thrice daily and discontinuation for one day per week for 5 weeks. Clinical efficacy, WOMAC score and serum SOD, NO and TNF-α before and after treatment between two groups were observed. Results Total effective rate of treatment group (90.00%) was significantly higher than that of control group (76.67%), the difference was statistically significant (P < 0.05). After treatment, WOMAC scores of treatment group and control group [pain score (2.26±1.24), (7.32±1.51) point; ankylosis score (0.51±0.28), (2.63±0.51) point; daily activity score (16.87±2.27), (30.76±4.16) point; comprehensive score (22.13±22.13), (40.81±5.32) point] were lower than those before treatment [pain score (15.46±3.23), (15.58±3.43) point; ankylosis score (6.51±1.24), (6.81±1.36) point; daily activity score (56.89±8.36), (57.41±6.55) point; comprehensive score (86.77±86.77), (84.81±9.81) point] (P < 0.05). WOMAC scores of treatment group after treatment were significantly lower than those of control group (P < 0.05). After treatment, serum SOD levels of treatment group and control group [(146.47±22.48), (126.28±19.57) U/mL] were significantly higher than those before treatment[(104.37±16.21), (105.53±14.78) U/mL], NO levels of treatment group and control group [(40.24±14.92), (49.29±16.38) μmol/L] and TNF-α levels of treatment group and control group [(54.33±17.69), (66.43±21.07) ng/L] were significantly lower than those before treatment [(57.42±21.78), (58.03±20.36) μmol/L; (97.25±32.06), (95.79±30.27) ng/L], the differences were statistically significant (P < 0.05 or P < 0.01). Serum SOD, NO and TNF-α levels after treatment between two groups were compared, the differences were statistically significant (P < 0.05). Conclusion Gucixiao Mixtures in the treatment of knee osteoarthritis have good clinical effect. It can protect the patient′s knee joint cartilage and its matrix from secondary damage, delay degeneration of knee articular cartilage, improve the nutrition and metabolism of knee joint cartilage, promote the repair of knee joint cartilage. To a certain extent, it can be prevention and treatment of knee osteoarthritis.

Influence of prophylactic oophorectomy on hormone level and life quality of postmenopausal women

[Abstract] Objective To explore the change of hormone levels of the postmenopausal women after prophylactic oophorectomy. Methods From December 2010 to March 2011, 29 patients with benign uterine lesions and given prophylactic bilateral ovaries line excision in Dalian Maternity Hospital were selected as the research objects, and all of the patients were postmenopausal women. According to duration of menopause, they were divided into three groups: group A (1-3 years), 9 cases; group B (> 3-10 years), 10 cases; group C (> 10 years), 10 cases. Kupperman scoring was used to evaluate the feel of patients in three groups before and after surgery; the follicle-stimulating hormone (FSH), estrogen (E2) and testosterone levels at preoperative, 3 months postoperative and 12 months postoperative of patients in three groups were tested and compared. Results 3 months postoperative, the total Kupperman scores of group A and B were significantly higher than that of preoperative (P < 0.05); scores of hot and sweating, heart palpitations, fatigue and urinary tract disease of patients in group C before and after surgery had statistically significant differences (P < 0.05); before operation, score of heart palpitations between group C and group A had statistically significant differences (P < 0.05), urinary tract symptoms between group C and group B had statistically significant differences (P < 0.05). 3 months postoperative levels of E2, FSH and testosterone of patients in group A were obviously decreased than those before operation (P < 0.05), levels of E2 and testosterone of patients in group B were obviously decreased than those before operation (P < 0.05), and 12 months postoperative risen again; FSH showed opposite change tendency, higher at 3 months postoperative, fall at 12 months postoperative, but was also higher than preoperative (P < 0.05). Conclusion Menopause patients especially patients with duration of menopause less than 3 years, within 3 years androgen and estrogen will significantly decrease after preventive ovaries remove, and increases FSH significantly, aggravate the menopausal symptoms, and lower the quality of life.

Effect of systematic goal-setting combined micro channel interaction model on the management of gestational diabetes mellitus

[Abstract] Objective To investigate the effects of systematic goal-setting combined micro channel interaction model on the management of quality of life and glucose metabolism outcome in patients with gestational diabetes mellitus (GDM) at postpartum 6-12 weeks. Methods A total of 160 patients who had been diagnosed as GDM from January 2011 to June 2014 in Fuzhou General Clinical Teaching Hospital of Fujian Medical University and the Second Affiliated Hospital of Fujian Medical University were investigated in this study. The patients were randomly divided into two groups: control group and observation group, each of them with 80 patients. In terms of the ways for the management, regular traditional diabetes health education intervention were used for the patients in control group, and full systematic goal-setting combined micro channel interaction model intervention was used for the patients in the observation group. The diabetes knowledge awareness, the quality of life and the glucose metabolism outcome at postpartum 6-12 weeks in two groups before and after intervention were respectively analyzed and compared. The follow up period was from the date of diagnosis to postpartum 6-12 weeks. Results The diabetes knowledge awareness difference between the two groups before intervention had no statistical significance(P > 0.05), whereas it exhibited statistical difference after intervention: patients in observation group had significantly increased diabetes knowledge awareness than those patients in the control group (P < 0.05); there was no significant difference in the dimensions of DSQL and total scores between the two groups before intervention (P > 0.05), whereas the dimensions of DSQL and total scores in observation group were significantly increase compared with those in the control group after intervention (P < 0.05). Based on the results from oral glucose tolerance test obtained from postpartum 6-12 weeks, the ratio of patients with abnormal glucose metabolism turning to with normal glucose metabolism was significantly increased while the ratio of patients turning to have impaired glucose regulation function and suffer from overt diabetes were significantly decreased in observation group compared to those in the control group (P < 0.05). Conclusion The full systematic goal-setting combined micro channel interaction model intervention has better application effects on the management of patients with GDM, which can effectively improve the disease awareness and the quality of life so as to achieve the purpose of improving the treatment for this disease.

Correlation between end-tidal carbon dioxide partial pressure and arterial partial pressure of carbon dioxide in nasal vestibule of the spontaneously breathing neonates

[Abstract] Objective To understand the correlation between end-tidal carbon dioxide partial pressure (PetCO2) and arterial partial pressure of carbon dioxide (PaCO2) in nasal vestibule of spontaneously breathing neonates, and verify the reliability of PetCO2 in reflecting PaCO2 in spontaneously breathing neonates. Methods From May 2014 to July 2015, 85 spontaneously breathing hospitalized neonates were randomly selected in Neonatal Medical Center of Huai'an Maternity and Child Healthcare Hospital Affiliated to Yangzhou University Medical Academy. PetCO2 values in nasal vestibule were recorded, while extracting radial arterial for blood gas analysis. According to gestational age, neonates were divided into preterm group (39 cases) and term infant group (46 cases); according to the presence of respiratory system disease, neonates were divided into respiratory disease group (25 cases) and without respiratory disease group (60 cases). PaCO2 and PetCO2 levels were compared by t test and correlated using Pearson correlation. Results The overall average mean value of PetCO2 was lower than that of PaCO2 [(37.6±6.3) vs (40.2±7.4) mmHg, t = 2.406, P = 0.017] (1 mmHg=0.133 kPa). There was a good correlation between PetCO2 and PaCO2 levels among all paired samples (r = 0.947, P = 0.000). PetCO2 of the preterm group was lower than PaCO2, there was no statistically significant difference (t = 1.314, P = 0.193). PetCO2 of the term infant group was lower than PaCO2, the difference was statistically significant (t = 2.032, P = 0.045). The difference between PetCO2 and PaCO2 levels was significant only in the respiratory disease group [(40.1±6.2) vs (45.0±7.6) mmHg, t = 2.494, P = 0.016], but not in the without respiratory disease group [(36.6±6.1) vs (38.4±6.2) mmHg, t = 1.368, P = 0.174]. The correlation between PetCO2 and PaCO2 in the respiratory disease group (r = 0.920, P = 0.000) was lower than that in the without respiratory disease group (r = 0.980, P = 0.000). The difference of PaCO2 and PetCO2 [P(a-et)CO2] in the respiratory disease group [(4.9±2.1) mmHg] was higher than that of the without respiratory disease group [(1.8±0.8) mmHg], and the difference was statistically significant (t = 9.901, P = 0.000). Conclusion PetCO2 of nasal vestibule can provide an accurate and noninvasive estimate of PaCO2 levels in spontaneously breathing neonates, especially in the neonates without respiratory disease.

Clinical effect comparison of different ways of inhaled Budesonide Aerosol in the treatment of children with combined allergic rhinitis and asthma syndrome

[Abstract] Objective To discuss the clinical curative effect of different ways of inhaled Budesonide Aerosol in the treatment of combined allergic rhinitis and asthma syndrome (CARAS) in children. Methods A total of 113 children with mild and moderate CARAS in Department of Pediatrics of Shenzhen People's Hospital form June 2011 to January 2015 were selected as the research objects, and according to random number table method they were divided into group A (39 cases) treated through the mouth inhaled budesonide aerosol, 200 μg/time, 2 times/d; group B (36 cases) treated through the nasally inhaled Budesonide Aerosol, 200 μg/time, 2 times/d; and group C (38 cases) treated through the mouth inhaled Budesonide Aerosol (100 μg/time, 2 times/d) and the nasally inhaled budesonide nasal spray 200 μg/time (each side nasal spray 100 μg each time, 1 time/d); each group were treated for 8 weeks. Correlation parameters of patients in the three groups before and after the treatment were recorded and compared. Results Compared with the group A, the clinical symptom score, pulmonary function, EOS absolute value, and serum total IgE levels of the group B were improved significantly (all P < 0.05), the total effective rate was significantly higher (χ2 = 5.943, P < 0.05); compared the group B with the group C, the difference was not obviously significant (P > 0.05), but the incidence of adverse reactions of group C was significantly higher than the group A and B, and the difference between group B and group C was statistically significant (χ2 = 11.402, P < 0.05). Conclusion The curative effect through the nasally inhaled Budesonide Aerosol, 200 μg/time, 2 times/d (400 μg/d) treated with mild and moderate CARAS is distinct, with less adverse reactions, simple operation, it is worthy of clinical popularization and application.

Expression and clinical significance of GRP78 in human trabecular meshwork cells

[Abstract] Objective To detect protein and mRNA expression differences of GRP78 in human trabecular meshwork cells of normal eyes (NTM) and POAG patients (GTM) in vitro. Methods Human trabecular meshwork cells from NTM and GTM in vitro were divided into 3 groups: Tunicamycin (Tm) group, Staurosporine (STS) group and control group. Real-time RT-PCR and Western blot were used to detect the expression of mRNA and protein of GRP78 in trabecular meshwork cells after 6, 24 h treatment. Results The expression of GRP78 in primary culture GTM cells was significantly lower than that in NTM cells, with statistically significant difference (P < 0.05). Tm could increase the expression of GRP78 in NTM and GTM cells, the expression in GTM cells was lower than that in NTM cells, with statistically significant difference (P < 0.05). The response of GTM cells for STS was bigger than NTM cells. Conclusion GTMs showed insensitivity reactions when stimulated with inducer of endoplasmic reticulum stress, reduce the expression of GRP78, lead to the increased sensitivity of GTMs to damage stimulus. GRP78 may play a cytoprotective role in the apoptosis in trabecular meshwork cells caused by endoplasmic reticulum stress.

Clinical effect of Nimotuzumab combined with Cisplatin concurrent che-moradiotherapy in the treatment of advanced nasopharyngeal carcinoma

[Abstract] Objective To evaluate the clinical effect of Nimotuzumab combined with Cisplatin concurrent chemoradiotherapy in the treatment of advanced nasopharyngeal carcinoma. Methods According to different treating methods, 66 cases of patients with advanced nasopharyngeal carcinoma treated in the First Affiliated Hospital of Xi'an Jiaotong University from January 2011 to January 2013 were assigned into two groups. 33 cases in the control group received Cisplatin concurrent chemoradiotherapy treatment, while 33 cases in the observation group received Nimotuzumab combined with Cisplatin concurrent chemoradiotherapy. The efficacy of the two groups was compared. Results The total effective rate of observation group and control group was 84.85%, 60.61% respectively, the difference was highly statistically significant (P < 0.01). The incidence of adverse reactions such as red blood cells decrease, thrombocytopenia, neutropenia, myelosuppression, skin reactions, radiation-induced oral mucositis, gastrointestinal reactions, electrocardiogram abnormalities, fever and others in the observation group was all significantly lower than those of control group, the differences were statistically significant (P < 0.05 or P < 0.01). The progression-free survival (PFS) of observation group was (6.4±0.4) months, and the median survival time (MST) was (14.6±1.8) months. For control group, the PFS was (4.2±0.6) months, and the MST was (11.1±1.5) months. The PFS and MST of observation group was obviously longer than those of control group (P < 0.01). Conclusion Nimotuzumab combined with Cisplatin concurrent chemoradiotherapy in the treatment of advanced nasopharyngeal carcinoma can improve clinical effects significantly, with few side effects and high safety, which is worthy of clinical application.

Morbidity situation of macular edema in patients with diabetic cataract after phacoemulsification

[Abstract] Objective To analyze the morbidity situation of macular edema in patients with diabetic cataract after phacoemulsification. Methods The clinical data of 113 cases (122 eyes) of patients treated with cataract ultrasonic emulsification and intraocular lens implantation in Shandong Qingzhou Ophthalmic Hospital from January 2013 to January 2015 was analyzed retrospectively, the patients were divided into diabetic group (59 cases, 64 eyes) and non-diabetic group (54 cases, 58 eyes) according to the presence of diabetes. Occurrence of macular edema, vision improved conditions, and macular retinal thickness of central sunken area of the two groups before and after treatment were observed and compared. Results Three months after surgery, there were 22 eyes of macular edema in the diabetic group, the incidence was 48.44%; there were 5 eyes of macular edema in the non-diabetic group, the incidence was 8.62%, the incidence of macular edema in the diabetes group was obviously higher than that of non-diabetic group, the difference was statistically significant (P < 0.05). Fluorescence fundus angiography examination showed that, diabetes patients showed mainly with leakage type of localized and diffuse leakage, the non-diabetic group was given priority to cystic oedema leakage, the leakage type between the two groups was statistically significant (P < 0.05). One month after surgery, the best corrected visual acuity of non-diabetic group was obviously better than that of diabetes group, and macular retinal thickness of central sunken area was lower than that of diabetes group, the differences were statistically significant (P < 0.05). Preoperative vision of the two groups had no statistically significant difference, postoperative visual acuity of two groups was improved significantly (P < 0.05), and 1 week, 1 month, and 3 months after surgery, vision of patients in diabetes group was all lower than those of non-diabetic group, the differences were statistically significant (P < 0.05). Conclusion The incidence of diabetic cataract patients with macular edema after phacoemulsification is significantly higher than that of non-diabetic cataract patients, and postoperative retinal thickness is increased, which significantly affects the recovery of vision.

Long-term comparison of stability of soft-tissue and hard tissue under different implant schemes

[Abstract] Objective To investigate the stability of soft-tissue and hard-tissue after different implant schemes in front teeth area. Methods 80 cases of patients (implant of 92 teeth) underwent dental implant in Ningbo Lihuili Hospital from January 2012 to January 2013 were selected and divided into immediate implant group (30 cases, 36 teeth) and conventional implant group (50 cases, 56 teeth), follow-up for 24 months. The implant stability, peri-implant soft tissue, bone level were detected and compared between the two groups. Results After 4 months surgery and before permanent prosthesis implant, there was no significant difference in implant stability coefficient between the two groups (P > 0.05). After repair of 2 years, there were no significant differences in the average increase of proximal mesial gingiva papilla, the average increase of distal mesial gingiva papilla, the average loss of lip side gingival margin level between the two groups (P > 0.05). After repair of 2 years, there were no significant differences in bone resorption of proximal gingiva mesial and distal gingiva mesial between the two groups (P > 0.05). Conclusion Immediate implant and conventional implant in front teeth area can both get good stability of soft-tissue and hard tissue, but immediate implant has course advantage, should give priority to intended population.

Effect of Hedan Tablets combined with Atorvastatin on serum lipids and inflammatory factors in cerebral infarction with sleep apnea-hypopnea patients

[Abstract] Objective To investigate the effects of Hedan Tablets combined with Atorvastatin for serum lipids and inflammatory factors of cerebral infarction with sleep apnea-hypopnea patients. Methods Total 74 cases of cerebral infarction with sleep apnea-hypopnea patients admitted to the Affiliated Hospital of Hebei University from January 2013 to June 2015 were selected as research objects. The patients were randomly divided into control group (n = 37) and observation group (n = 37). The control group was treated with oral administration of Atorvastatin for 4 weeks, the observation group was treated with oral administration of Hedan Tablets combined with Atorvastatin for 4 weeks. Before and after treatment the serum triglyceride (TG), total cholesterol (TC), low density lipoprotein C (LDL-C), high-sensitivity C-reactive protein (hs-CRP) and soluble intercellular adhesion molecules-1 (sICAM-1) were determined. Results After treatment, the levels of serum TG [(2.549±0.441), (2.854±0.444) mmol/L], TC[(2.865±1.035), (2.992±0.991) mmol/L], LDL-C[(1.778±0.486), (1.827±0.527) mmol/L], hs-CRP[(1.846±0.607), (2.230±0.799) mg/L] and sICAM-1[(315.532±39.717), (345.108±30.733) ng/mL] in observation group and control group were all lower than those [(3.630±0.548), (3.862±0.683) mmol/L;(5.878±0.999), (5.757±1.062) mmol/L; (3.687±0.581), (3.538±0.658) mmol/L; (4.500±1.432), (4.422±1.540) mg/L; (397.762±33.752), (409.243±39.001) ng/mL] before treatment, the differences were all statistically significant (P < 0.01). After treatment, there were no statistically significant differences of the levels of serum TC and LDL-C between observation group and control group (P > 0.05). The levels of serum TG, hs-CRP and sICAM-1 in control group were lower than those of observation group after treatment, the differences were statistically significant (P < 0.05 or P < 0.01). Conclusion Hedan Tablets combined with atorvastatin has strong effects of lipid-lowering in cerebral infarction with sleep apnea-hypopnea patients. Hedan tablet combined with atorvastatincan strengthens the inflammatory response.

Effect of Methotrexate combined with Iguratimod in the treatment of patients with refractory rheumatoid arthritis

[Abstract] Objective To observe the effect of Methotrexate (MTX) combined with Iguratimod in the treatment of patients with refractory rheumatoid arthritis. Methods Sixty patients with refractory rheumatoid arthritis were all coming from Department of Rheumatology, Huai'an First People's Hospital, Nanjing Medical University from December 2013 to February 2014, they were randomly divided into experimental group and control group, with 30 cases in each group. The experimental group was given Iguratimod (50 mg/d) and MTX (15 mg one week), the control group was given the same dose of MTX, the course was 16 weeks. Before treatment and after treatment for 16 weeks, the disease activity score 28 (DAS28) was evaluated and recorded. And serum samples were retrieved, levels of tumor necrosis factor alpha (TNF-α) and vascular endothelial growth factor (VEGF) were quantified by enzyme-linked immunosorbent assays. Results There was no statistically significant difference of DAS28 score before treatment between the two groups [experimental group: (5.97±1.62) points; control group: (6.40±1.90) points; P > 0.05]. After treatment for 16 weeks, DAS28 score of the two groups decreased significantly than before treatment [experimental group: (3.02±1.24) points; control group: (5.65±1.86) points], the differences were all statistically significant (P < 0.01, P < 0.05), and DAS28 score of the experimental group was reduced more obviously than that of control group, the difference was highly statistically significant (P < 0.01). There was no statistically significant difference of TNF-α before treatment between the two groups [experimental group: (379.27±114.70) pg/mL; control group: (368.86±127.06) pg/mL, P > 0.05]. After treatment for 16 weeks, the levels of TNF-α of the two groups were significantly reduced compared with those before treatment [experimental group: (230.42±67.23) pg/mL; control group: (309.65±95.59) pg/mL], the differences were all statistically significant (P < 0.01, P < 0.05), and the level of TNF-α of the experimental group decreased more obviously than that of control group, the difference was highly statistically significant (P < 0.01). There was no statistically significant difference of VEGF before treatment between the two groups [experimental group: (1124.11±305.06) pg/mL; control group: (1142.02±174.02) pg/mL; P > 0.05]. After treatment for 16 weeks, the levels of VEGF of the two groups were significantly reduced compared with those before treatment [experimental group: (818.89±178.78) pg/mL; control group: (1000.21±245.87) pg/mL], the differences were all statistically significant (P < 0.01, P < 0.05), and the level of VEGF of the experimental group decreased more obviously than that of control group, the difference was highly statistically significant (P < 0.01). Conclusion MTX combined with Iguratimod has a good efficacy for treatment of patients with refractory rheumatoid arthritis, with high safety.

Effect of Tirofiban in the percutaneous coronary intervention of elderly patients with hish-risk non-ST-elevation acute coronary syndrome

[Abstract] Objective To observe the effect of Tirofiban in the percutaneous coronary intervention (PCI) for elderly patients with hish-risk non-ST-elevation acute coronary syndrome (NSTE-ACS). Methods Three hundred and twenty cases of elderly patients with hish-risk NSTE-ACS receiving PCI in Daping Hospital and the Research Institute of Surgery of the Third Military Medical University from February 2012 to February 2015 were selected and divided into two groups including 162 cases of control group (conventional treatment) and 158 cases of observation group (conventional treatment + Tirofiban). The TIMI myocardial perfusion grading before and after PCI of the two groups was compared, the clinical efficacy of the two groups was analyzed, the platelet aggregation levels and platelet count of the two groups were measured, the incidence of major adverse cardiovascular event (MACE) was recorded after a follow-up period of 30 d, and the incidence of bleeding complications was observed. Results The TIMI myocardial perfusion grades after PCI were significantly better than the control group (P < 0.05); the total effective rate of observation group was 93.7%, which was significantly higher than that of control group (87.0%) (P < 0.05); the platelet aggregation rate after PCI in observation group (0.29±0.09) was also significantly lower than that of control group (0.40±0.10) (P < 0.05); follow-up for 30 d after PCI, the incidence of MACE in observation group (8.9%) was significantly lower than that of control group (14.2%) (P < 0.05); while the incidence of bleeding complications between the two groups had no significant differences (P > 0.05). Conclusion For elderly patients with hish-risk NSTE-ACS, Tirofiban can significantly improve TIMI myocardial perfusion levels, reduce platelet aggregation, improve clinical efficacy and short-term clinical prognosis, without increasing the risk of bleeding, which is worthy of clinical application.

Effect of Tiotropium Bromide combined with Salmeterol/Fluticasone Propionate treatment on lung function of COPD patients

[Abstract] Objective To explore effect of Tiotropium Bromide combined with Salmeterol/Fluticasone Propionate treatment on lung function of COPD patients. Methods clinical data of 75 cases of COPD patients in the Central Hospital of Fushun City from January 2013 to January 2015 were analyzed, According to different treatment method, they were divided into two groups, general treatment group (Spasmolysis and asthma foundation treatment) 35 cases and combined treatment group (Tiotropium Bromide unite Salmeterol/Fluticasone Propionate treatment) 40 cases. The clinical symptom scores, lung function, total effective rate of clinical treatment of two groups of patients before treatment and after treatment were detected. The lung function was tested by lung function instrument. Results There was no statiscal difference of clinical symptom scores [(6.60±2.21) vs (7.14±2.32) scores] between two groups of COPD patients before treatment (t=1.03, P > 0.05). There was no statiscal difference of FEV1 [(1.12±0.67) vs (1.13±0.58) L], FEV1/FVC% [(52.28±3.29)% vs (52.48±3.64)% L], FEV1%Pred [(52.34±4.03)% vs (53.16±3.92)%] between two groups of COPD patients before treatment (t=0.07, 0.25, 0.89, P > 0.05). The clinical symptom score [(4.23±1.62)scores] of combined treatment group COPD patients after treatment was better than general treatment group [(5.66±1.45)scores, t=4.00, P < 0.05]. The FEV1 [(1.96±0.55)L], FEV1/FVC% [(59.92±3.47)%], FEV1%Pred [(63.12±5.24)%] of combined treatment group after treatment were better than general treatment group [(1.54±0.48)L, (56.20±3.24)%, (56.72±4.87)%] (t=3.50, 4.78, 5.45, P < 0.05), the clinical effective rate (100.0%) of combined treatment group COPD patients was higher than general treatment group (74.3%), the difference were statistically significant (χ2=29.49, P < 0.05). Conclusion The clinical effacy of tiotropium bromide combined with salmeterol/ fluticasone propionate treatment on COPD patients is obvious, has good prognosis, which is worthy of clinical application.

Clinical study of oral medicinal herbs combined with external application of qi goiter acupoint in the treatment of benign thyroid nodules

[Abstract] Objective To observe the clinical effect of oral Sanjie Ⅰ Prescription combined with local application of qi goiter acupoint in the treatment of benign thyroid nodules. Methods Eighty patients with benign thyroid nodules admitted to Zengcheng Hospital of Traditional Chinese Medicine in Guangzhou City from July 2014 to July 2015 were selected and randomly divided into two groups. 40 cases of the treatment group were taken oral Chinese medicine Sanjie Ⅰ Prescription combined with local application of Sanjie Ⅱ Prescription on bilateral qi goiter acupoint; 40 cases of the control group were taken oral Levothyroxine Tablets. The effects of two groups after treatment, the levels of thyroid hormones [thyroid stimulating hormone (TSH), serum free three iodine thyroid gland (FT3) and serum free thyroid hormone (FT4)] before and after treatment, the changes of the size of thyroid nodules and the changes of blood flow imaging in the nodules were observed. Results The total effective rate of treatment group was 92.5%, which of control group was 47.5%, the difference between the two groups was statistically significant (P < 0.05). B ultrasound showed that the maximum diameter of the nodules [(9.23±4.52) mm] and the maximum cross-sectional area of the nodules [(65.53±55.42) mm2] were significantly decreased compared with the maximum diameter of the nodules [(12.36±4.68) mm] and the maximum cross section area [(75.66±52.08) mm2] of the control group, the differences were statistically significant (P < 0.05). The levels of thyroid hormone (TSH, FT3, FT4) in the treatment group had no statistically significant differences compared with those before treatment (P > 0.05). In the control group, the level of TSH after treatment [(0.73±0.34) mU/L] was decreased significantly compared with that before treatment [(1.96±0.79) mU/L] (P < 0.05), the levels of FT3 [(3.95±2.33) pmol/L], FT4 [(13.95±2.61) pmol/L] after treatment were significantly increased compared with those before treatment [FT3: (3.52±1.06) pmol/L, FT4: (13.49±3.65) pmol/L] (P < 0.05). There was a significant difference of blood flow classification after treatment between the two groups (P < 0.05). Conclusion Oral medicinal herbs combined with external application of qi goiter acupoint in the treatment of benign thyroid nodules has a good effect, which is worthy of clinical promotion and application.

Screening of Mongolian anti-inflammatory drugs and its working mechanism study based upon the mitochondrial function

[Abstract] As a traditional national medication, Mongolian medication has unique characteristics of national, geographical, theoretical and clinical characteristics. Mongolian drug has particular effect on anti-inflammation. However, the anti-inflammation mechanism of a variety of drugs is not clear due to limited modern science methodologies and techniques. In order to develop Mongolian anti-inflammation drug market and exploit its utilization, urgently require to find out an innovative research strategy and methodology. Mitochondria, an important organelle for cellular energy transduction, has been functionally linked to many kinds of metabolic and immunological diseases. From micro-systematic viewpoint, mitochondria are crucial factors for human physiological status. Digging into this factor through its interaction with the whole system, and also its response to external stimulation is always an important direction for research. And this theory is perfectly agreed with the idea of Mongolian medication. Uncoupling protein 2 (UCP2) and peroxisome proliferator-activated receptor-γcoactivator-1α (PGC-1α) are two important functional indicators for mitochondria. In this study, the expression and activity of both UCP2 and PGC-1α are analyzed to evaluate the mitochondrial status, and so as to check whether cells or tissues were normally functioning. Based on this study, a generalized research methodology for studying the physiology and selective active ingredients of Mongolian anti-inflammation drugs is presented and proved.

Study of the influence factors of non-medical technology service

[Abstract] Objective To research the influence factors of non-medical technology service, in order to provide basis for the further improvement of non-medical technology service, help to improve the overall quality of medical service. Methods Doctors and patients were surveyed by questionnaires in 10 hospitals including primary, secondary and tertiary hospitals in Hohhot City of Inner Mongolia Autonomous Region. The method of consulting literature materials was used, the influence factors about non-medical technology service were extracted. The method of expert interview was used to do screen for the influence factors which were extracted. The questionnaire was used to survey doctors and patients. Frequency analysis, arithmetic average index and assignment method were used for data statistics and analysis. Results The top of factors both doctors and patients among the 33 influence factors of the non-medical technology service were as follow: the doctor's work ambition (ranked first in both doctors and patients); doctors complying with law and working system (ranked second in doctors); the doctor's service consciousness (ranked third in doctors and forth in patients); doctors strictly performing diagnosis standard and medication guide (ranked fourth in doctors and second in patients); doctors paying attention to communicate with patients (ranked fifth in doctors); doctors love their job (ranked third in patients); doctors strictly performing medical services and drug pricing policy(ranked fifth in patients). Conclusion Non-medical technical service should be strengthened, in order to improve the whole medical service level.

Effect of preoperative standard visit combined with visit card in laparoscopic surgery for patients with pancreatic and duodenal resection

[Abstract] Objective To investigate the effect of preoperative standard visit combined with visit card in laparoscopic surgery for patients with pancreatic and duodenal resection. Methods From August 2013 to August 2015, in Traditional Chinese Medical Hospital of Guangdong Province, 50 patients with laparoscopic pancreatic resection were divided into experiment group and control group, with 25 cases in each group. In the experiment group, the patients were treated according to the standard of preoperative visit; patients in the control group was treated by the circuit nurses according to their own experience and the interview was conducted in the control group. The anxiety scale (SAS) and depression scale (SDS) were used to investigate the psychological mood of the two groups, and the systolic blood pressure and heart rate were detected, and the satisfaction of patients was investigated. Results The scores of SAS and SDS in the experiment group were significantly lower than those in the control group (P < 0.01). The systolic blood pressure and heart rate in the experiment group were significantly lower than those in the control group (P < 0.01), and the patients in the experiment group were significantly higher than those in the control group (P < 0.01). Conclusion The preoperative visit to the standard combined with visit card can reduce the anxiety of patients and more initiative to cooperate with the operation anesthesia. Nurses through the interview process can fully understand the assessment of patients which helped them to prepare for the operation and to provide the basis for the operation of tacit understanding ahead of time so that the operation can carry on smoothly.

Discussion on practice of professional titles evaluation in a collectivized hospital

[Abstract] As the new medical reform gets implemented, public hospitals in Anhui Province are also undergoing reforms in evaluating professional titles. This article systematically summarizes the works that have been done in evaluating professions titles by a collectivized hospital in Anhui Province from 2009 to 2015. This hospital established a set of scientific, practical, and highly operative evaluating system, so as to form fair, objective, and justified evaluation and assessment system of medical professionals, formulating effective evaluation system for professional technical personnel. Through systematic surveying the current state of evaluation system of professional titles in collectivized hospitals, major problems at the moment and achievement in evaluation system reform, this articles further discusses the standards and conditions of the evaluation system for medical staff, so as to better improve the evaluating system and procedures, to adapt the new demand of the new medical reform, and finally, to provide effective experience for other hospitals to learn. This article provides assessment foundation for the application of medical staff. It is also instructive in that it offers strategy for the application and evaluation of professional titles in other hospital groups.

Influence of the teaching mode of simulation class combined with bed patient rounds on practice ability of nursing students

[Abstract] Objective To explore the modern nursing teaching mode in order to meet the clinical needs. Methods Three hundred and two nursing students were chosen from Suizhou Vocational and Technical College. And the students were randomly divided into trial group (151 cases) and control group (151 cases) by random number table. The teaching contents were reformed and new cases were designed. The trial group was under the teaching mode of simulation class combined with bed patient rounds, and the control group was still under the traditional teaching mode. Grades of the two groups of students, the passing rate of national license exam and the graduate employment rate were analyzed and compared. Results The learning achievement (Z = -11.04, P = 0.000), the passing rate of national license exam (χ2=34.91, P = 0.000) and employment rate (χ2=13.08, P = 0.000) between the two groups had statistically significant differences, the results of trial group were all better than those of control group. Conclusion The teaching mode of simulation class combined with bed patient rounds is well-organized and operable, and it can improve students' practice ability, so it is worthy of promotion and application in the medical colleges and universities.

Influence factors of students malnutrition from a university in Shijingshan District of Beijing

[Abstract] Objective To explore the influence factors of students malnutrition from a university in Shijingshan District of Beijing. Methods 300 cases students from a university in Shijingshan District of Beijing were collected from February 2014 to June 2015 as the research objects. There were 30 cases of male students, and 270 cases of female students, aged from 18 to 22. Questionnaire and physical examination were done. Based on the nutritional status, all the students were divided into the malnutuitin group and normal group. Then the nutritional status of the two groups students and the influence factors were analyzed by statistics. Results 81 cases had malnutrition among 300 students, the proportion was 27.0%. Logistic regression analysis found that female (P=0.005), living expenses insufficient (P=0.034), low basal metabolic rate (P=0.009), low body fat mass (P=0.012), low limbs fat (P=0.024), low trunk fat (P=0.015), bad appetite (P=0.031), lack of physical activity (P=0.035), like to eat sweet food (P=0.042) and dietary bias (P=0.006) were the independent risk factors of malnutrition in college students. Conclusion The detection rate of malnutrition is high in students from a university in Shijingshan District of Beijing. Gender, cost for living, basal metabolic rate, quality of body fat, limbs fat mass, trunk fat, appetite, every day's exercise, like sweets food and dietary bias all influence the nutritional status of the college students. Comprehensive intervention should be taken for malnourished college students by improving their lifestyle.

[Abstract] Scientific and reasonable assessment on the overall operation of the hospital has positive significance for the sustainable development of hospital. The current financial evaluation system has so many shortcomings, such as scattered, the neglect about non-financial indicators, and the not fitting the reform process, that couldn't meet the needs of application. This article presented theapplication of a new public hospital financial analysis index system constructed by combining the DuPont financial analysis and Waldo proportion score, which could evaluate the financial situation more comprehensive, and promote the smooth work of hospital.
