33 Research progress of traditional Chinese medicine based on “lung-intestinal” axis regulating intestinal flora in the treatment of severe pneumonia
LUO Cheng1 YE Yuanhang1 ZHENG Lan2 KE Jia2
DOI: 10.20047/j.issn1673-7210.2023.06.07
Severe pneumonia (SP) is one of the common critical respiratory diseases. With the development of the “lung- intestinal” axis theory, the relationship between gut microbiota and lung diseases has become a research hotspot. In recent years, many studies have been carried out on the regulation of intestinal microbiota in the treatment of SP by traditional Chinese medicine, and the results show that intestinal microbiota will become a new target for the prevention and treatment of SP by traditional Chinese medicine. Based on the basic theory of traditional Chinese medicine, this paper takes intestinal microbiota as the target, and sorts out the correlation research of traditional Chinese medicine regulating intestinal microbiota in the treatment of SP through the “lung- intestinal” axis, in order to provide guiding significance for the clinical treatment of SP.

37 Study on mechanism of action, slow release research, and improvement strategies of antimicrobial peptides
MENG Yupeng1 LIU Hong2 XIANG Yanyan1 ZHU Song1 WANG Min1
DOI: 10.20047/j.issn1673-7210.2023.06.08
Antimicrobial peptides as new anti-infective drugs have been paid more and more attention. By studying the mechanism of action of  antimicrobial peptides, it is found that antimicrobial peptides can act on multiple different targets of bacteria and fungi, making it difficult for pathogenic bacteria to respond to them in a single way, and thus difficult to develop drug resistance. However, the slow release of antimicrobial peptides is still a difficult problem to be solved. From the point of view of the combination and release of antimicrobial peptides with different carriers, the slow release mechanism of antimicrobial peptides on different carriers is explained. It is considered that the electrostatic binding force between ions is the key to slow release. At the same time, this paper discussed the improvement and optimization of antimicrobial peptides and the current clinical application progress, hoping to provide the direction for further clinical application of antimicrobial peptides.

DOI: 10.20047/j.issn1673-7210.2023.06.09
Ulcerative colitis (UC) is a chronic immune-mediated disease affecting the intestinal tract. Its course is prolonged, the recurrence rate is high, and the lack of effective treatment affects the health of patients. In recent years, the number of UC patients has been increasing, which has attracted wide attention, but its etiology and pathogenesis remain unclear. More and more studies have shown that intestinal microecological imbalance is an important pathogenic mechanism of UC. Traditional Chinese medicine can play a role in the treatment of UC by regulating intestinal microecological balance, enhancing the stability of intestinal microbial barrier and improving intestinal immune function. This article mainly reviews the research progress of the regulation of intestinal microbiome by traditional Chinese medicine monomer and compound in the treatment of UC, and provides reference for further treatment of UC.

HUANG Can MAO Tangyou SUN Zhongmei LI Junxiang ZHANG Wenji WANG Zhibin
DOI: 10.20047/j.issn1673-7210.2023.06.12
The pathogenesis of ulcerative colitis (UC) is still unclear, but it is generally considered to be caused by immune disorders caused by environmental factors, microorganisms, and metabolic disorders in the background of genetic susceptibility. As an essential amino acid in mammals, tryptophan can regulate intestinal barrier function, mucosal immune cells, and gut microbiota through different metabolic pathways, thereby participating in the occurrence and development of UC. Therefore, regulating the metabolic pathways or metabolites of tryptophan may become a new strategy for the treatment of UC in the future. This article will review the research progress of tryptophan metabolism and mechanism of the action in ulcerative colitis, in order to provide new ideas and methods for its clinical diagnosis and treatment.

DUAN Jun1 ZHANG Ruijing2 REN Wanting1 LI Yingjuan1 LIU Xinyan2
DOI: 10.20047/j.issn1673-7210.2023.06.14
Objective To investigate the influencing factors of hyperuricemia (HUA) in patients with type 2 diabetic kidney disease in non-dialysis state (ND-DKD). Methods Clinical data of 326 patients with ND-DKD admitted to the Department of Nephrology, the Second Hospital of Shanxi Medical University from January 1, 2018 to December 31, 2021 were collected and divided into HUA group and non-HUA group according to the diagnostic criteria of HUA. Based on the estimated glomerular filtration rate (eGFR), patients were divided into stages 1 to 2 of CKD and 3 to 5 of CKD. The clinical data of the two groups were compared, and the influencing factors of ND-DKD patients with HUA were analyzed by logistic regression. Results The incidence of HUA in 326 patients with ND-DKD was 50.3%, and the incidence of HUA in patients with CKD stage 1 to 2 and CKD stage 3 to 5 was 34.0% and 58.2%, respectively. The level of platelet, glutamyltransferase (GGT), albumin, globulin, total protein, urea nitrogen, serum creatinine, cystatin C (CysC), α1 microglobulin, β2 microglobulin, blood potassium, blood phosphorus, QTc interval, the history of hypertension, the incidence of anemia, 3 to 5 of CKD, and hypertriglyceridemia in HUA group were higher than those in non-HUA group, and the differences were statistically significant (P<0.05). The glycosylated hemoglobin, bicarbonate, eGFR, and urine pH in HUA group were lower than those in non-HUA group, and the differences were statistically significant (P<0.05). Logistic regression analysis showed that GGT (OR=1.013, 95%CI: 1.003-1.024), total protein (OR=1.071, 95%CI: 1.040-1.104), CysC (OR=2.760, 95%CI: 1.750-4.353), blood phosphorus (OR=3.717, 95%CI: 1.296-10.658), hypertriglyceridemia (OR=2.424, 95%CI: 1.440-4.081) were the factors influencing the occurrence of HUA in ND-DKD patients (P<0.05). Conclusion The incidence of HUA in ND-DKD patients increases with the decline of renal function, and GGT, total protein, CysC, blood phosphorus, and hypertriglyceridemia are the influencing factors for the occurrence of HUA in ND-DKD patients. Dietary guidance and health education for patients with ND-DKD should be strengthened. Low-fat and low-phosphorus high-quality protein diet can reduce the incidence of HUA in patients with ND-DKD, which has a positive significance in delaying disease progression.

Sjögren’s syndrome (SS) belongs to an inflammatory autoimmune disease mainly involving exocrine glands, in addition to exocrine gland manifestations such as dry eyes and dry mouth, pulmonary interstitial fibrosis is common clinically, and pigmented villonodular synovitis (PVNS) is rare. PVNS is a rare disease in clinical practice, with dual characteristics of inflammation and tumor, non-specific clinical manifestations, frequent joint swelling, pain, and limited activity at the onset of disease. Clinical misdiagnosis of PVNS is easy, pathological examination is the gold standard for diagnosis of PVNS, and surgical resection of the affected joint synovium is the main treatment for PVNS. Radiotherapy can be used as postoperative adjuvant therapy for this disease. In this paper, a patient with left lower limb distension and pain as the primary clinical manifestations accompanied by dry eyes and mouth and recurrent bloody joint effusion in the left knee was reported. After histomathological examination, SS and PVNS were confirmed to be present at the same time. The previous literature was reviewed in order to provide reference for clinical diagnosis and treatment of this disease with integration of Chinese and western medicine.

DOI: 10.20047/j.issn1673-7210.2023.06.42
Cadmium poisoning is a rare heavy metal poisoning. Acute oral cadmium poisoning has the characteristics of acute disease, rapid progress, and difficult identification. This article introduces a patient with “abdominal pain and palpitations” as the first manifestation in the Third Affiliated Hospital of Guangzhou University of Chinese Medicine. The patient rapidly progressed to respiratory failure, liver and kidney failure after onset, and was found to have acute cadmium poisoning through blood cadmium examination. There is no specific antidote for cadmium poisoning, so the main treatment is to maintain organ function and life support. The patient’s multi-organ function continued to deteriorate and eventually died. Combined with the problems in treatment, the authors of this paper reviews the relevant literature, summarizes the characteristics of cadmium poisoning, and proposes that when the diagnosis of liver and kidney function damage is not clear, possible poisoning should be considered in time, and blood and urine toxicology tests should be carried out in time to help diagnose.

DOI: 10.20047/j.issn1673-7210.2023.06.43
With the increasing number of operations, the traditional preoperative evaluation model of anesthesia can no longer meet the medical needs of patients. Therefore, it is important to explore a new evaluation model to improve the medical service capacity of the hospital. Establish a center with full-time anesthesiologists and anesthesiologists to jointly conduct preoperative visit and evaluation, provide the diagnosis and treatment services of waiting, pre-examination, and evaluation visit, and realize one-stop preoperative evaluation of anesthesia with patients’ active participation, assisted evaluation by anesthesia nurses, and paperless medical record preservation. The anesthesia preoperative evaluation center not only improves the work efficiency of anesthesia visits and the rate of preoperative visits, but also plays a positive role in alleviating the tension and fear of patients during surgery and anesthesia, thus improving the doctor-patient relationship and providing patients with higher quality medical services.

DOI: 10.20047/j.issn1673-7210.2023.06.44
With the increasingly serious problem of population aging, the management of chronic diseases in the elderly is of great significance. At present, there is a shortage of geriatric specialists in China. The standardized training of geriatric specialists can not only meet the basic medical needs of the current aging society, but also adapt to the development plan of the modern chemistry department of geriatric medicine. At present, there is still a gap between the development of geriatrics in China and the advanced countries in the world. Therefore, the standardized training of geriatric medical specialists is particularly important. It should focus on strengthening the training of theoretical knowledge of geriatrics and standardizing the use of comprehensive evaluation methods, paying attention to multidisciplinary team cooperation and improving the cognitive level of palliative medical concept, promoting the establishment of scientific research thinking, improving the ability of crisis management in response to public health emergencies, paying attention to the cultivation of core literacy and improving the establishment of training and assessment system and so on, can improve the training effect.

194 Traditional Chinese medicine translation theory and discourse power remodeling
LI Chenghua1 KONG Ranran1 SUN Huiming2
DOI: 10.20047/j.issn1673-7210.2023.06.45
Chinese language is the medium of ancient dissemination of traditional Chinese medicine (TCM). However, recently China does not have sufficient discourse power in TCM dissemination, namely, the lack of discourse power in the selection of standard TCM English terms and the dwarfing of TCM culture in the shadow of modern medicine make it urgent for us to remodel the discourse power in TCM translation. Having distinguished the conceptual logic in translation studies, the author proposes that to reshape the discourse power in TCM translation from a theoretical perspective, translators should adhere to the thought of TCM cultural translation, take the foreignization translation strategy as the leading role, build a TCM translation theory of multi-dimensional integration of “translation thought - translation strategy - translation methods”, which serves to guide the translation practice of TCM classics and win the leading role in the selection of standard TCM English terms, avoid the tendency of desinicization in TCM translation, and promote the international dissemination of TCM culture.
