6 The value of exosomal ABCA1 protein in the diagnosis of Alzheimer′s disease

Objective To preliminary study the value of exosome ABCA1 protein as a diagnostic marker for Alzheimer′s disease (AD) through mouse experiments and clinical specimen detection. Methods Cerebrospinal fluid (CSF) and serum exosomes of APP/PS1 AD model mice in nine-month-old group were extracted and analyzed by mass spectrometry and ABCA1 protein detected by enzyme linked immunosorbent assay (ELISA). After three, six, nine months old APP/PS1 mice cerebrospinal fluid exosomes were injected into the third ventricle of three-month-old wild type (WT) mice, serum and CSF of zero, two, four, six-hour-groups were detected respectively exosome ABCA1 level. Serum exosome ABCA1 levels were detected in 69 patients with subjective cognitive decline (SCD group), 43 patients with mild cognitive impairment (MCI group) and 35 patients with dementia (DAT group) and 30 cases of control group. Results The results of mass spectrometry and ELISA showed that the levels of CSF and ABCA1 protein in serum of APP/PS1 double transgenic mice in three-month-old group, six-month-old group and nine-month-old group were higher than those of WT mice at the same month age (P < 0.05). CSF and serum exosomes ABCA1 protein of APP/PS1 double transgenic mice in six-month-old group were significantly higher than those in three-month-old group (P < 0.05). CSF and serum exosomes ABCA1 protein in nine-month-old APP/PS1 double transgenic mice were significantly higher than those in six-month-old APP/PS1 double transgenic mice (P < 0.05). The serum level of ABCA1 protein in WT mice was significantly increased in two, four and six hour-groups after exosomes injection (P < 0.05), and the highest level was found in four-hour group. The levels of ABCA1 protein in CSF exosomes of WT mice were significantly decreased in two, four and six hour-group after exosomes injection (P < 0.05), and the lowest in six-hour-group. There was no significant difference in ABCA1 protein between SCD group and control group (P > 0.05). The level of ABCA1 protein in serum of MCI group and DAT group was significantly higher than that of control group (P < 0.05), and that of DAT group was significantly higher than that of MCI group (P < 0.05). When the cut-off value was 0.39, the sensitivity and specificity of ABCA1 protein was 72.5% and 75.4% respectively, when the cut-off value was 0.52, the sensitivity and specificity of ABCA1 protein was 70.1% and 69.2% respectively in the diagnose of MCI. The diagnostic efficiency of ABCA1 protein in MCI and DAT was significantly higher than that in SCD (P < 0.05). Conclusion ABCA1 exosomes can pass through the blood-brain barrier and reach the peripheral blood, which can be used as a candidate marker for AD diagnosis. However, more research data need to be accumulated in the diagnosis of SCD.

10 Expression of TLR4 in maternal peripheral blood before and after parturition and its relationship with activation of downstream signaling pathways

Objective To analyze the expression of Toll-like receptor 4 (TLR4) in maternal peripheral blood before and after parturition and its relationship with activation of downstream signaling pathways. Methods The peripheral blood and uterine muscle tissue of pregnant women admitted to Putuo Hospital Affiliated to Shanghai University of Traditional Chinese Medicine from March 2018 to August 2019 were collected and divided into term labor group (21 cases) and term non labor group (20 cases) according to whether or not labor. The expression of TLR4 was detected by flow cytometry and enzyme-linked immunosorbent assay. RT-PCR was used to detect the expression of TLR4, contraction related protein, nuclear factor (NF)-κB, JNK, ERK and P38 mRNA in uterine muscle, and their relationships were analyzed. Results There was no significant difference in age, gestational age and body mass index between two groups (P > 0.05). There were significant differences in the expression levels of peripheral blood neutrophils, monocytes and serum TLR4 between two groups (P < 0.05). There were significant differences in TLR4, CX43 and OTR between two groups (P < 0.05). There were significant differences in the mRNA levels of NF-κB, ERK, P38 and JNK between two groups (P < 0.05). TLR4 was positively correlated with CX43, OTR, JNK, ERK, P38 and NF-κB (R2 = 0.619, 0.416, 0.438, 0.426, 0.55, 0.332, all P < 0.01). JNK, ERK, P38 and NF-κB were positively correlated with CX43 (R2 = 0.548, 0.393, 0.702, 0.600, all P < 0.01), JNK, ERK, P38 and NF-κB were positively correlated with OTR (R2 = 0.308, 0.608, 0.532, 0.514, all P < 0.01). Conclusion TLR4 may play an important role in labor initiation by activating downstream NF-κB/MAPK pathway, promoting the expression of contraction related proteins and promoting uterine contraction.

14 Research on the quality standard improvement of Danzhi Shuxin Capsules

Objective To improve the quality standard of Danzhi Shuxin Capsules. Methods Thin layer chromatography (TLC) was used to identify the three components of Danzhi Shuxin Capsules, including danshen root, borneol and rhizoma corydalis, and high performance liquid chromatography (HPLC) was used to determine the content of salvianolic acid B in the preparation. Chromatographic conditions: Agilent C18 column (4.6 mm ×250 mm, 5 μm), mobile phase conditions: acetonitrile-formic acid-water (22∶1∶77), 286 nm wavelength was used for detection, flow rate was 1.0 mL/min, column temperature was 25℃, injection volume was 10 μL. Results In TLC identification of danshen root, borneol and rhizoma corydalis, the component spots were clear, the separation degree was good, and the negative control samples had no interference. The content of salvianolic acid B was within the range of 19.4582-311.3312 μg/mL, which showed a good linear relationship with the peak area, R2=1. The average recovery was 100.84% and the RSD was 1.37% (n = 9). Conclusion This method is accurate and feasible, and can be used as the quality standard of Danzhi Shuxin Capsules.

18 Analysis and thinking of scientific research management based on research projects and articles of a tertiary general hospital in Beijing from 2009 to 2018

Objective To analyze the scientific research projects and Chinese and English papers of a tertiary general hospital in Beijing from 2009 to 2018, so as to provide a quantitative basis for the future scientific research management decision of the hospital. Methods According to the data of research projects and papers in Chinese and English from 2009 to 2018, the number of projects and funding amount, the number of papers in Chinese and English published as the first author or corresponding author, SCI impact factors and so on were retrospectively analyzed. Results From 2009 to 2018, the institute′s scientific research developed rapidly. The number of projects and the amount of funding, the number of papers in Both Chinese and English, and SCI article average influence factors all showed fluctuations. However, there were still problems such as unstable growth of the number of research projects and SCI papers, and declining efficiency of scientific research output. Conclusion In order to further enhance the hospital′s scientific research ability, the hospital should improve the incentive mechanism, strengthen the introduction and training of scientific research talents, consolidate the platform foundation, and cultivate scientific research management professionals.

22 Molecular mechanism of Pingchuan Granules regulated EOS for asthmatic based on Notch/STAT signaling pathway

Objective To investigate the therapeutic target and molecular mechanism of Pingchuan Granules in the treatment of asthma. Methods Eighty Wistar rats were randomly divided into Control group, Model group, Pingchuan Granule (PCG) group and Dexamethasone (DXMS) group, with 20 rats in each group. Asthma model rats were prepared by the method of combined inhalation of ovalbumin and aluminum hydroxide. Five days before the end of modeling, the corresponding drug intervention treatment was given by gavage. According to the modeling condition, ten of each group were selected for subsequent experiments. Bronchoalveolar lavage fluid (BALF) and serum eosinophilic granulocyte (EOS) counts, EOS cationic protein (ECP) and activated chemokine expression (Eotaxin) were compared among the four groups. Expression of proteins and mRNA related to Notch and STAT signaling pathways in lung were measured by RT-PCR and Western blot. Results BALF and serum EOS counts in Model group were higher than those in Control group (all P < 0.01), BALF and serum EOS counts in PCG group were lower than those in Model group (all P < 0.01). The content of ECP and Eotaxin in Model group were higher than those in Control group (all P < 0.01), and the content of ECP and eotaxin in PCG were lower than those in Model group (P < 0.05 or P < 0.01). Notch1, 2, 4, Hes, Jagged1, Delta4 protein and mRNA expression in Model group were higher than those in Control group, and the expression of Notch3, Delta1, 3 protein and mRNA were lower than those in Control group (all P < 0.01). Notch1, 2, 4, Hes, Jagged1, Delta4 protein and mRNA expression in PCG group were lower than those in Model group and the expression of Notch3, Delta1, 3 protein and mRNA were higher than those in Model group (P < 0.05 or P < 0.01). The expression of P-STAT1, 3, 5, 6 protein and mRNA in Model group were higher than those in Control group, and P-STAT4 protein and mRNA were lower than those in Control group (all P < 0.01). The expression P-STAT1, 3, 5, 6 protein and mRNA in PCG group were lower than those in Model group, and P-STAT4 protein and mRNA were higher than those in Model group (all P < 0.01). Conclusion PCG can reduce EOS by regulating Notch/STAT signaling pathway in the treatment of asthma.

28 Analysis on the effect of antiviral therapy and correlation with viral load in patients with HIV/HBV co-infection

Objective To investigate the effect of antiviral therapy and correlation with viral load in patients with human immuncodeficiency virus/hepatits B virus (HIV/HBV) co-infection. Methods From January 2017 to December 2018, 147 patients with HBV and HIV infection in the Department of Infectious Diseases of the Third People′s Hospital of Guilin, Guangxi Zhuang Autonomous Region were selected as the research subjects. According to the type of infection, the patients were divided into three groups: 49 cases in HBV group, 50 cases in HIV group, and 48 cases in HIV/HBV group. Prospective analysis was conducted to calculate the correlation between liver and kidney function indexes of three groups of patients before and after treatment with HIV ribonucleic acid (HIV-RNA) and HBV deoxyribonucleic acid (HBV-DNA). Results The levels of alanine aminotransferase (ALT), total bilirubin (TBil) and direct bilirubin (DBil) in the HBV group after 24 weeks of treatment were lower than those after 12 weeks, and the levels of ALT, TBil and DBil after 48 weeks of treatment were lower than those after 24 weeks of treatment, and the differences were statistically significant (all P < 0.05). The levels of TBil and DBil in the HIV group after 12 and 24 weeks of treatment were higher than those before treatment, aspartate aminotrransferase (AST) levels after 24 weeks of treatment were lower than those before treatment, after 48 weeks of treatment, the levels of AST, TBil and DBil were lower than those after 24 weeks of treatment, and the differences were statistically significant (P < 0.05). After 12 weeks of treatment, the DBil level in the HIV/HBV group was lower than that before treatment, after 24 weeks of treatment DBil level was lower than that after 12 weeks of treatment, and that after 48 weeks of treatment was lower than that after 24 weeks of treatment, and the differences were statistically significant (P < 0.05). After 24 weeks of treatment, the HBV-DNA level of the HIV/HBV group was higher than that of the HBV group, and the difference was statistically significant (P < 0.05). After 24 weeks and 48 weeks of treatment, the levels of HIV-RNA in the HIV/HBV group were higher than that in the HIV group, and the differences were statistically significant (P < 0.05). Conclusion The antiviral therapy is effective in the recovery of liver function for those HIV/HBV co-infected, but it is still difficult to eliminate HIV and HBV.

32 Research progress on the drug resistance mechanism of Osimertinib in patients with non-small cell lung cancer

Osimertinib is a third-generation epidermal growth factor receptor-tyrosine kinase inhibitors (EGFR-TKIs). It is currently approved for first-line treatment of patients with advanced non-small cell lung cancer (NSCLC) with EGFR mutation and second-line treatment with T790M mutant NSCLC after the first-generation EGFR-TKIs treatment progresses. Clinical studies have shown that Osimertinib can significantly improve the progression-free survival (PFS) of NSCLC patients, but it also brings serious drug resistance problems. In recent years, studies have found that the first-line application of Osimertinib resistance mechanism mainly depends on the activation of EGFR pathway and bypass signaling pathway; the drug resistance mechanism produced by the second-line and second-line application of Osimertinib depends on the activation of the EGFR pathway, the bypass signaling pathway, and the transformation of the cell phenotype. Osimertinib has different drug resistance mechanisms at different times. In order to provide a theoretical basis for the prevention and treatment of Osimertinib resistance, this article will review the different drug resistance mechanisms and treatment strategies of Osimertinib from the perspective of clinical drug use time.

36 Progress in the effects of nitrous oxide exposure on the nervous system

Long-term nitrous oxide (N2O) exposure can cause neurological damage mainly manifested as spinal cord lesions. Patients generally have a gradual onset and progressive condition. The clinical manifestations are limb paresthesia and ataxia, some patients have symptoms of peripheral nerve injuries and extrapyramidal tracts injuries, it may cause cognitive dysfunction and mental abnormalities. Since the increase in clinical case reports of N2O abuse, it is of great significance to research the effects of N2O exposure on the nervous system. Therefore, after a systematic review of related literature and case reports, this paper summarizes the injury mechanism, clinical manifestations, exposure methods, diagnosis, treatment and prognosis of N2O exposure. We hope to strengthen the awareness of the disease, and to provide some ideas for diagnosis and treatment.

40 Recent progress on anti-aging and molecular mechanisms of curcumin

Curcumin is a polyphenolic active ingredient extracted from the traditional Chinese medicine Turmeric, which has been widely used as medicine and seasoning at home and abroad. Due to its multiple targets, it has various effects such as anti-oxidation, anti-inflammation, anti-atherosclerosis, anti-tumor and so on. In recent years, its anti-aging effect has attracted much attention from researchers. This review mainly summarizes the anti-aging effects of curcumin on nervous system, cardiovascular system and skin from a molecular perspective in the recent five years, expecting to shed new light on prospective drug discovery and rational application.

In this paper, the research status and progress of bone metabolism markers are reviewed. It provides a new idea for clinical intervention of osteoporosis.

48 Research progress on rehabilitation assessment and treatment of dysphagia after stroke

Dysphagia is a common complication of cerebral apoplexy. During the course of the disease, it is easy to cause aspiration pneumonia, recurrent cough and asphyxia, even life-threatening. This paper reviews the progress of bedside screening, instrument evaluation and rehabilitation for patients with dysphagia after stroke. Understanding the methods, advantages and disadvantages and application scope of commonly used swallowing assessment tools is very important to identify the causes of swallowing disorders and determine the degree of dysphagia in patients. It also provides scientific guidance for clinical rehabilitation treatment. The treatment of dysphagia in stroke includes oropharyngeal muscle movement sensory training, feeding training, airway protection, neuromuscular electrical stimulation, intermittent oral feeding, balloon catheter dilatation. The aim is to improve the swallowing function and reduce the complications, so as to provide a theoretical basis for clinical rehabilitation therapy to treat the dysphagia after stroke.

52 Research progress on risk factors and assessment of PICC related thrombosis in tumour patients

PICC related thrombosis is one of the most serious complications related to PICC. The influencing factors are complicated, involving patient factors, disease types and course of disease, catheter placement factors and catheter location, catheter factors and drug factors. PICC related thrombosis risk assessment tools for tumour patients, such as various assessment scales (Michigan risk prediction scale, Caprini scale, Maneval scale), nomogram model, thromboelastography, etc., it can guide clinical to take active and effective clinical nursing intervention measures. This paper reviews the research progress of influencing factors and risk assessment tools of PICC related thrombosis in tumour patients through the analysis of relevant literature, analyzes the advantages and disadvantages of existing research, and puts forward the future research direction, so as to provide reference for the selection of prevention, treatment and evaluation tools of PICC related thrombosis in tumour patients.

56 Relationship between amyloid β-protein and Alzheimer′s disease and the research progress of acupuncture intervention

Alzheimer′s disease (AD) is a neurodegenerative disorder characterized by cognitive impairment and memory loss. Amyloid β-protein (Aβ) is the initiating factor that triggers the course of AD, and a potential intervention for the treatment of AD is to reduce the deposition of Aβ in the brain. This paper summarizes the generation and cleanup of Aβ, and the relationship between Aβ and AD, and reviews the basic research on the regulation of Aβ by acupuncture in the treatment of AD. It is found that acupuncture can reduce the production of Aβ, promote the cleanup of Aβ by regulating Aβ-degrading enzyme and microglia-related factors, and improve the transport ability of the blood-brain barrier to Aβ and other pathways, and promote the dearance of Aβ. At the same time, the future research trends are prospected to provide new ideas for disease research.

60 Reformation and cogitation of teaching of medicinal chemistry of natural products in medical universities

Cell Biology is a required basic course in modern medical education, its curriculum is rich in ideological and political elements, ideological and political education is introduced into the teaching of cell Biology, it is helpful to cultivate students′ rigorous scientific style and patriotic thought. Based on the course content of Cell Biology, the author further excavates and extracts the Curriculum ideological and political education of the course. By means of embedding, infiltration and extension, the concept of Serengthen morality eduction was integrated into important knowledge points in cell biology, such as cell intima, cell signal transduction, cell proliferation and cell cycle, cell differentiation, etc., so as to make it go hand in hand with ideological and political education and form a synergistic effect.

67 Application of augmented reality combined with flipped classroom in the orthopedics teaching

Orthopedics is a clinical subject based on anatomy and physiology, involving many subjects such as imaging and biomechanics, and has a strong professionalism. Due to the many interdisciplinary subjects and abstract knowledge concepts, orthopedics teaching practice has certain difficulties. In order to meet the urgent needs of high-quality medical talents in modern society, the call for medical education reform is increasing. The search for effective methods that can improve the quality of orthopedics teaching has attracted the attention of many medical educators. In recent years, the flipped classroom teaching model has become a hot spot in the reform of medical education, and augmented reality technology has gradually been applied to classroom teaching. Based on the teaching content of orthopedics, the article designs and implements an augmented reality technology-assisted flipped classroom teaching method based on Complete Anatomy software, and summarizes the advantages and problems of this method in the process of educational practice.

71 Construction of simulation training curriculum system of higher vocational nursing based on virtual reality

Nursing posts in Internet + era put forward higher requirements for professionals. In order to improve the core post competence of nursing students and help nursing students adapt to clinical nursing work smoothly, this paper analyzes the post needs of clinical nurses based on the nursing work process. It is proposed to construct a four-step and two-wing higher vocational nursing simulation training course system with the combination of virtual and reality in the information age. That is, through basic nursing simulation training-specialized nursing simulation training-comprehensive nursing simulation training-intensive training of nursing skills before clinical practice of four-step, step by step, until the teaching goal of nursing training course is achieved. It not only emphasizes the “hard skills” of knowledge and operation, but also pays attention to the “soft skills” of nursing students′ professional accomplishment and emotion, the combination of software and hardware, and the fullness of the “two wings” to cultivate excellent nurses.

75 Discussion on online and offline mixed teaching practice of traditional Chinese medicine identification based on BOPPPS teaching mode

Combined with the subject characteristics and training objectives of traditional Chinese medicine identification, this paper discusses the online and offline mixed teaching practice of traditional Chinese medicine identification using  BOPPPS teaching mode. Taking the identification of golden thread medicinal materials as an example, this paper introduces the specific methods of rational designing, allocating and utilizing online and offline resources in six stages according to the BOPPPS teaching model, including bridge-in, objective, pre-assessment, participatory learning, post-assessment and summary, and uses comprehensive index to evaluate the teaching effect. 82 Application of emergency care simulator combined case based study in anesthesia teaching

Objective To explore the guiding role and teaching effect of applying emergency care simulator combined case based study to undergraduates of anesthesiology before clinical practice. Methods From September 2019 to January 2020, 50 students in the fifth grade of anesthesiology major who were interned in the First Clinical Medical College of Inner Mongolia Medical University were selected as subjects. They were randomly divided into EC group and control group, with 25 students in each group. The training will be conducted by the same teacher. Control group was adopted the traditional teaching method and EC group was adopted the emergency care simulator combined case based study method. The teaching effect of two groups were evaluated and the teaching satisfaction of the two groups were compared. Results The scores of theoretical knowledge, practical skills and teaching satisfaction in EC group were all higher than those in control group (all P < 0.05). Conclusion The application of emergency care simulator combined case based study in anesthesia teaching has a significant effect, which is helpful to consolidate the theoretical knowledge, enhance the practical skills, improve the quality of anesthesia teaching and the teaching satisfaction significantly. It is worth applying and popularizing in teaching.

88 Application value of A2DS2 score in risk assessment of pneumonia after ischemic stroka

Objective To investigate the application value of A2DS2 score in predicting the risk of stroke associated pneumonia (SAP). Methods Retrospective study method was used, 294 cases with ischemic stroke hospitalized in the Department of Neurology of Minzu Hospital of Guangxi Zhuang Autonomous Region from January 2017 to December 2018. The risk factors affecting SAP was analyzed, and receiver operating characteristics curve was used to evaluate the diagnosis of the A2DS2 score for predicting SAP. Results Consciousness disorders, dysphagia, and NIHSS score≥5 points were risk factors for the occurrence of SAP. A2DS2 score was four points, the area under the curve was 0.835, the sensitivity was 0.788, specificity was 0.774. Conclusion A2DS2 score can effectively predict the risk of developing pneumonia after ischemic stroke.

91 Changes of serum KL-6 level in patients with connective tissue disease associated interstitial lung disease and its clinical significance

Objective To investigate the changes of serum krebsvon den lungen-6 (KL-6) level in patients with connective tissue disease associated interstitial lung disease (CTD-ILD) and its clinical significance. Methods From May 2018 to October 2019, 80 cases with CTD-ILD treated in the Second People′s Hospital of Lianyungang of Jiangsu Province (“our hospital” for short) were selected as the CTD-ILD group, and 69 cases with CTD without ILD treated in our hospital during the same period were selected as the CTD-NILD group. Patients in the CTD-ILD group were further divided into usual interstitial pneumonia (UIP) group with 20 cases, nonspecific interstitial pneumonia (NSIP) group with 46 cases and undefined group with 14 cases according to high resolution CT result, and serum KL-6 level of each groups were compared. The correlation between serum KL-6 and lung function index was analyzed by Pearson method, and the diagnostic value of serum KL-6 level in CTD-ILD was analyzed by receiver operating characteristic (ROC) curve. Results The serum KL-6 level of the CTD-ILD group was higher than that of the CTD-NILD group, while maximum oxygen uptake (VO2max), lung diffusion capacity (DLco/SB) and lung dispersion per unit lung volume (DLco/VA) were lower than those of the CTD-NILD group (all P < 0.05). Serum KL-6 level in UIP group was higher than that in NSIP group and undefined group, and serum KL-6 level in NSIP group was higher than that in undefined group (all P < 0.05). Serum KL-6 level was negatively correlated with VO2max, DLco/SB and DLco/VA (r = -0.777, -0.711, -0.553, P < 0.05). ROC curve showed that the when cut-off value of serum KL-6 was 384.68 U/mL, the area under the curve was 0.832 (95%CI: 0.767-0.897), and the sensitivity and specificity were 0.81 and 0.87, respectively. Conclusion Serum KL-6 level in CTD-ILD patients is elevated abnormally, while lung function decreased significantly. The reduced lung function is closely related to the elevation of serum KL-6 level.

96 Clinical and pathological features of 116 patients with proliferative and sclerosing IgA nephropathy

Objective To investigate the clinical and pathological features of patients with proliferative and sclerosing IgA nephropathy. Methods Retrospective analysis was performed on 116 patients with proliferative and sclerosing IgA nephropathy confirmed by renal biopsy in Department of Nephropathy, Xiyuan Hospital, China Academy of Chinese Medical Sciences from January 2015 to March 2019. According to chronic kidney diseases (CKD) stage, they were divided into CKD stage 1-5. The 24 h urinary protein quantification and comorbidities of each group were compared (including the presence or absence of hypertension, hyperuricemia, etc.). They were divided into group with ischemic kidney injury and group without ischemic kidney injury according to the presence or absence of ischemic kidney injury, and they were divided into group with IgM deposition and group without IgM deposition according to the presence or absence of IgM deposition group. The 24 h urinary protein quantification, serum creatinine level and estimated glomerular filtration rate (eGFR) of each group were compared. Results There were statistically significant differences among the five groups in age, 24 h urine protein quantification, and proportion of hyperuricemia, pharyngitis and tonsillitis (all P < 0.05). The serum creatinine and proportion of hypertension in group with ischemic kidney injury were higher than those of the group without ischemic kidney injury, and eGFR was lower than that of the group without ischemic kidney injury, with statistically significant differences (all P < 0.05). The age and eGFR level of group with IgM deposition were lower than those of group without IgM deposition, the serum creatinine level was higher than that of group without IgM deposition, and the differences were statistically significant (all P < 0.05). Conclusion The clinical and pathological lesions in patients with proliferation and sclerosing IgA nephropathy are severe, and 24 h urine protein quantitative, blood creatinine, ischemic kidney injury and IgM deposition may be risk factors for renal progression and poor renal prognosis.

100 Influence of case management on social function of psychotic major depression patients

Objective To explore the influence of case management on social function of psychotic major depression patients. Methods A total of 87 cases of depression psychotic major patients who were treated in Nanhui Mental Health Center, Pudong New Area, Shanghai from January 2013 to October 2014 were selected as the study subjects, and they were divided into study group (43 cases) and control group (44 cases) according to the random block group method. The study group implemented the case management program, and the control group carried out the general outpatient follow-up program. At the time of enrollment, three, six, and twelve months after treatment, Hamilton depression scale-17 items (HAMD-17), brief psychiatric rating scale (BPRS), and family assessment device (FAD) and insight and treatment attitude questionnaire (ITAQ) were used for evaluation and comparison. Results A total of 79 cases (41 in the study group and 38 in the control group) were finally completed. The differences in BPRS and HAMD scores among groups, time points and interaction were statistically significant (all P < 0.05). BPRS and HAMD scores of the two groups at three, six and twelve months of treatment in the two groups were all lower than those at the time of enrollment, those at six and twelve months of treatment were less than three months of treatment, and those at twelve months of treatment were less than six months of treatment, and the differences were statistically significant (all P < 0.05). The BPRS scores of the study group at three, six and twelve months of treatment were lower those of the control group, and the HAMD scores of the study group at six and twelve months of treatment were lower those of the control group, with statistically significant differences (all P < 0.05). The differences in FAD scores among groups and time points were statistically significant (P < 0.05), but the interaction difference was not statistically significant (P > 0.05). The FAD scores of the two groups at three, six and twelve months of treatment in both groups were lower than those at enrollment, and those at six and twelve months of treatment were less than three months of treatment, and those at twelve months of treatment were less than six months of treatment, and the differences were statistically significant (all P < 0.05). The FAD scores of the study group were lower than those of the control group at six and twelve months of treatment, and the differences were statistically significant (all P < 0.05). There were statistically significant differences in ITAQ score at time point and interaction (all P < 0.05), while there were no statistically significant differences between groups (P > 0.05). ITAQ scores of the two groups at three, six and twelve months of treatment were all higher than those at the time of enrollment, and those at six and twelve months of treatment were higher than those at three months of treatment, and those at twelve months of treatment were higher than those at six months of treatment, with statistically significant differences (all P < 0.05). There were no significant differences in ITAQ scores between the two groups at each time point (P > 0.05). The study group was significantly better than the control group in terms of medication compliance, recurrence and re-employment within twelve months of treatment, with statistically significant differences (all P < 0.05). Conclusion Compared with general outpatient follow-up, case management can improve patients′ medication compliance, thus effectively relieve patients′ depressive symptoms and accompanying psychiatric symptoms, improve social function, and increase employment rate.

105 Significance of macroprolactin detection in the female inpatients with schizophrenia

Objective To explore the significance of macroprolactin (MPRL) detection in the female inpatients with schizophrenia. Methods From January 2018 to June 2019, 65 schizophrenics patients were collected in Department of Psychiatry, the Third People′s Hospital of Huzhou City, Zhejiang Province and all of them were treated with Risperidone Tablets. The menstrual situation of the patients was recorded during the treatment, and the contents of prolactin (PRL) and MPRL were measured by luminescent immunoassay and enzyme-linked immunosorbent assay respectively before and at the end of the sixth weekend of the treatment. The content of PRL and MPRL were compared by self-control study before and after the treatment, at the same time, according to the content of PRL and MPRL at the end of the sixth weekend, 65 patients were divided into the normal PRL group (16 cases) and the hyperprolactinemia group (49 cases). The hyperprolactinemia group was then divided into the macroprolactinemia group (21 cases) and the non-macroprolactinemia group (28 cases), and the menstrual delay in different groups was compared. Results The levels of PRL and MPRL in 65 patients after treatment were significantly increased compared with those before treatment, and the differences were highly statistically significant (all P < 0.01). The number and incidence of menstrual delay of the normal PRL group , the hyperprolactinemia group, the macroprolactinemia group and the non-macroprolactinemia group were 2 cases (12.5%), 30 cases (61.2%), 5 cases (23.8%) and 25 cases (89.3%), respectively. The incidence of menstrual delay in four groups was compared in pairs, there was no significant difference between the macroprolactinemia group and the normal PRL group (P > 0.05), the incidence of menstrual delay in the non-macroprolactinemia group was higher than that in the hyperprolactinemia group, the macroprolactinemia group and the normal PRL group, with highly statistically significant differences (all P < 0.01). The incidence of menstrual delay in the hyperprolactinemia group was higher than that in the macroprolactinemia group and normal PRL group, with highly statistically significant differences (all P < 0.01). Conclusion Macroprolactinemia caused by Risperidone may not lead to menstrual delay. The clinical detection of macroprolactin is helpful to guide the treatment of hyperprolactinemia.

109 Effect of long-term standardized inhaled drugs and home oxygen therapy on pulmonary function and quality of life in patients with COPD at stable stage

Objective To explore the effect of long-term standard inhaled drugs and home oxygen therapy on pulmonary function and quality of life in patients with chronic obstructive pulmonary disease (COPD) at stable stage, and to provide evidence for the diagnosis and treatment of patients. Methods A total of 100 patients with COPD at stable stage admitted to the Department of Respiratory Medicine, the Central Hospital of Maanshan, Anhui Province from April 2018 to April 2019 were selected as study subjects, and they were divided into observation group and control group by random number table method, with 50 patients in each group. The control group was treated with conventional therapy, while the observation group was treated with long-term standard inhaled drugs combined with home oxygen therapy for six months. The results after treatment were compared, and the changes of lung function, dyspnea score and quality of life score before and after treatment were recorded. Results The total effective rate of the observation group was significantly higher than that of the control group, with statistically significant difference (P < 0.05). After treatment, forced vital capacity (FVC) and forced expiratory volume in the first second (FEV1) in both groups were increased compared with that before treatment, and the observation group was higher than the control group, ratio of forced expiratory volume in the first second to forced vital capacity (FEV1/FVC) in the observation group was increased compared with that before treatment, and the observation group was lower than the control group, the differences were statistically significant (P < 0.05). After treatment, both dyspnea and quality of life scores were lower in the observation group than in the control group, and the differences were statistically significant (P < 0.05). No significant adverse reactions were observed during treatment in both groups. Conclusion Long-term standardized inhaled drugs and home oxygen therapy have significant effects on COPD at stable stage, which can improve the symptoms of dyspnea, promote the recovery of pulmonary function of patients, improve the quality of life, and can be popularized in clinical use.

Granulomatous lobular mastitis is a chronic inflammation of the breast that is common in multipara. The etiology and pathogenesis are still unclear in modern medicine. Adrenal cortical hormone has a certain therapeutic effect on the treatment of this disease, but the treatment cycle is long, and abscesses and wound infections are repeatedly formed after the drug withdrawal. The patients have to undergo repeated operations or even total mastectomy in order to be cured. Professor Lin Yi believes that the main cause of this disease is the accumulation of foreign bodies, and the treatment is based on the principle of “Qufu Shengji”, and the key to the treatment is to thoroughly remove slough and pus. 117 Analysis of Professor Yuan Zhaokai′s prescription for insomnia based on data mining

Objective To analyze the rules of Professor Yuan Zhaokai′s prescriptions for insomnia by data mining. Methods Prescriptions of patients diagnosed as “insomnia” were collected from Yuan Zhaokai′s Master Heritage Studio of Jinrun Hospital of Traditional Chinese Medicine from January 2018 to December 2019. The methods such as association rule, entropy clustering of complex systems were used to analyze the law of medication in prescriptions. Results A total of 206 prescriptions were included, involving 92 traditional Chinese medicine and generating 22 high-frequency drugs (frequency greater than 40 times). In terms of four nature of drugs, were mainly flat and cold, and the five flavors were mainly sweet, bitter and pungent. In the meridian tropism, the drugs of heart channel and liver channel were mainly used. Forty groups of high-frequency drug combinations, 54 drug association rules and three potential new prescriptions were excavated. Conclusion Professor Yuan Zhaokai treats insomnia mainly by nourishing yin and tonifying blood, dispersing stagnated liver qi for relieving qi stagnation, clearing heat and dissipating phlegm, and calming the mind at the same time, treating both manifestation and root cause of disease.

121 Characteristic and data mining of Zhulinsi Nvke Zhengzhi for metrorrhagia and metrostaxis

Objective To explore the syndrome differentiation thinking in treatment of metrorrhagia and metrostaxis in Zhulinsi Nvke Zhengzhi, and to explore the regularity of medication in metrorrhagia and metrostaxis. Methods Took Zhulinsi Nvke Zhengzhi as the retrieval source. The syndrome treatment literatures related to the collapse of the book were collected and sorted out. The clinical thoughts of Zhu Lin′s buddhist doctor in the treatment of the collapse were analyzed, the names of the formulas included were extracted, the recipe database was established, and frequency analysis of the data was conducted. SPSS Statistic 22.0 and SPSS Modeler 14.1 were used to cluster analysis and correlation analysis for high frequency drugs. Results Through screening, 24 prescriptions for metrorrhagia and metrostaxis were obtained. Hematonic drug, qi-tonifying drug, superficies-relieving drug, antipyretic drug, bleeding-arresting preparation drug were high frequency medication. Liquorice root, Chinese angelica, prepared rehmannia root, Sichuan lovage rhizome, debark peony root, baical skullcap root, unprocessed rehmannia root, ect. were specific high-frequency drugs. Chinese angelica was the core drug. Five core drug groups were observed by cluster analysis. According to the correlation analysis, the traditional Chinese medicine with potential association rules were Chinese angelica and debark peony root, Chinese angelica and Sichuan lovage rhizome. Conclusion Based on the analysis of the literatures and prescriptions in the treatment of metrorrhagia and metrostaxis in Zhulinsi Nvke Zhengzhi, it is concluded that the treatment of metrorrhagia and metrostaxis by Zhu Lin′s buddhist doctor attaches great importance to benefiting qi for promoting production of blood, coordinating spleen and stomach, and the data mining results are consistent with the traditional theory, which is reasonable and effective.

Objective To observe the effect of Shenyuan Granules on the serum fibroblast growth factor 23 (FGF 23), fibroblast growth factor receptor 4 (FGFR 4) and Klotho protein in non-dialysis patients with chronic kidney disease (CKD) of stage 3-5, so as to explore the possible mechanism of Shenyuan Granules on vascular calcification in CKD patients. Methods A total of 60 patients with CKD admitted to Hubei Provincial Hospital of Traditional Chinese Medicine from June 2018 to June 2019 were selected and they were divided into the control group (28 cases) and the treatment group (32 cases) by random number table method. The control group was treated with Niaoduqing Granules, while the treatment group was treated with Shenyuan Granules. After 3 months of continuous treatment, the levels of biochemical indexes (serum calcium, phosphorus, parathyroid hormone [PTH] and estimated glomerular filtration rate [GFR]), and serum FGF23, FGFR4 and Klotho protein content were observed before and after treatment. Results Before treatment, there were no statistically significant differences in calcium, phosphorus, PTH and eGFR between the two groups (P > 0.05). After treatment, serum phosphorus and PTH in the treatment group were lower than those inthe control group, while calcium and eGFR were higher than that in the control group, with statistically significant differences (all P < 0.05). Before treatment, there was no significant difference in FGF23, FGFR4, and Klotho protein content between the two groups (P > 0.05). After treatment, serum FGF23 and Klotho protein content in the treatment group were higher than those in the control group, and FGFR4 content was lower than that in the control group, with statistically significant differences (all P < 0.05). Conclusion Shenyuan Granules can inhibit FGF23 and FGFR4 levels and increase Klotho protein content, which may be one of the mechanisms of Shenyuan Granules in the treatment of vascular calcification in patients with CKD.

129 Clinical observation of thunder-fire moxibustion combined with electroacupuncture in the treatment of neurogenic bladder after suprasacral spinal cord injury

Objective To observe the clinical effect of thunder-fire moxibustion combined with electroacupuncture in the treatment of neurogenic bladder after suprasacral spinal cord injury. Methods A total of 60 cases of neurogenic bladder after suprasacral spinal cord injury patients from January 2017 to August 2019 in the Guangxi Liuzhou Iron and Steel Group Company Limitde Hospital were selected and divided into observation group and control group according to the method of random number table, with 30 cases in each group. Both groups were treated with bladder management. The observation group was treated with thunder-fire moxibustion combined with electroacupuncture, the control group was treated with electroacupuncture, once per day, 10 times was as one course, three courses were observed. Before and after treatment, the total effective rate, maximum flow rate, bladder volume, residual urine volume, daily urination frequency and subjective quality of life profile (SQLP) scores of the two groups were recorded and compared. Results After treatment, the total effective rate of the observation group was higher than that of the control group, the difference was statistically significant(P < 0.05). After treatment, the daily urination frequency, residual urine volume and SQLP scores in the two groups were lower than those before treatment, and the indexes in the observation group were lower than those in the control group, the differences were statistically significant (P < 0.05). The maximum flow rate and bladder volume of two groups after treatment were higher than those before treatment, and the indexes in the observation group were higher than those in the control group, the differences were statistically significant (P < 0.05). Conclusion Thunder-fire moxibustion combined with electroacupuncture was a safe and effective treatment for neurogenic bladder after suprasacral spinal cord injury, which has advantages in improving bladder functions and patients′ quality of life.

In the process of long-term diagnosis and treatment, director Li Quanwang takes root in the classical collateral disease theory of traditional Chinese medicine, combined with the clinical characteristics of pulmonary nodules, and thinks that pulmonary nodules belong to pulmonary collateral disease. Based on the epidemiological data and treatment progress of pulmonary nodules, combined with the psychological and physical bias of patients, it is considered that traditional Chinese medicine should be applied to the early diagnostic treatment of pulmonary nodules. It is put forward that the pathogenesis of pulmonary nodules is “deficiency of lung qi and obstruction of lung collaterals by toxin”, and the treatment principle of “regulating qi and strengthening vital qi, detoxification and dredging collaterals”. 140 Effect of modified Xijiao Dihuang Decoction on primary immunologic thrombocytopenic purpura and the level of T lymphocyte subsets

Objective To analyze the effect of modified Xijiao Dihuang Decoction on primary immunologic thrombocytopenic purpura and the level of T lymphocyte subsets. Methods A total of 76 patients with primary immunologic thrombocytopenic purpura admitted to the First Hospital Jiaxing from January 2016 to February 2018 were selected for this study. According to the random number table method, they were randomly divided into conventional group and traditional Chinese medicine group, 38 cases in each group. The conventional group was treated with Prednisone Acetate Tablets, while the traditional Chinese medicine group was treated with modified Xijiao Dihuang Decoction (buffalo horn instead of rhino horn). After treatment, the effects total effective rate and relapse rate, traditional Chinese medicine symptom score, platelet count and T lymphocyte subsets CD3+, CD4+ and CD8+ of the two groups were compared. Results After treatment, there was no significant difference in the total effective rate and relapse rate between the two groups (P > 0.05). After treatment, the total effective rate of traditional Chinese medicine symptom improvement in traditional Chinese medicine group was higher than that in the conventional group (P < 0.05). After treatment, the platelet count of the two groups increased (P < 0.05), but there was no significant difference between the two groups (P > 0.05). After treatment, the levels of CD3+ and CD4+ in the two groups were higher, and the traditional Chinese medicine group were higher than those in the conventional group (P < 0.05); the levels of CD8+ in the two groups were lower, and the traditional Chinese medicine group was lower than that in the conventional group (P < 0.05). There were no adverse reactions in the two groups during the study. Conclusion Modified Xijiao Dihuang Decoction has a significant effect in the treatment of primary immunologic thrombocytopenic purpura, it can obviously improve the clinical symptoms of patients and the level of T lymphocyte subsets.

144 Analysis of ultrasonographic features of primary thyroid lymphomas

Objective To summarize the characteristics of each type of image for primary thyroid lymphomas (PTLs), and to discuss the diagnosis and differential diagnosis of PTLs. Methods From August 2008 to May 2019, a total of 65 patients with PTLs confirmed by biopsy or pathology after preoperative ultrasound examination were retrospectively analyzed. The ultrasonic classification and sonographic features were discussed. Results Of the 65 patients with PTLs, 39 cases (60.0%) were diffuse type. The thyroid gland was enlarged in varying degrees, the capsule was intact, and the echo of thyroid parenchyma was diffusely decreased, showing heterogeneous hypoechoic to extremely low echo. No calcification or liquefaction was found in the lesions of all patients with diffuse PTLs. Abundant blood flow signals were detected in 28 cases (71.8%). Cervical lymph node enlargement was found in 22 cases (56.4%). 26 cases (40.0%) were non diffuse type with 47 lesions, of which 11 cases were single lesion, 15 cases were multiple lesions, 16 cases (61.5%) were hypoechoic, and 10 cases (38.5%) were extremely hypoechoic. 22 cases (84.6%) had irregular shape, hypoechoic or extremely hypoechoic lesions. 19 cases (73.1%) had abundant blood flow. Cervical lymph node enlargement was found in 19 cases (73.1%). Conclusion PTLs have certain ultrasonographic features. If necessary, biopsy or biopsy should be performed to avoid misdiagnosis and missed diagnosis.

147 Effect of Yiligan Capsules combined with Tenofovir on liver function and liver fibrosis in patients with hepatitis B cirrhosis

Objective To explore the effect of Yiligan Capsules combined with Tenofovir on liver function and liver fibrosis in patients with hepatitis B cirrhosis. Methods Ninety patients from May 2015 to March 2018 in Cangzhou Infectious Disease Hospital of Hebei Province were selected and divided into control group and observation group according to random number table method, 45 cases in each group. Control group was treated with Tenofovir, and observation group was given Yiligan Capsules combined with Tenofovir. Clinical efficacy and laboratory examination indexes of two groups were compared. Results Total clinical effective rate of observation group was higher than that of control group (P < 0.05). After treatment, serum total bilirubin (TBil), alanine aminotransferase (ALT), aspartate aminotransferase (AST) levels of two groups were lower than those before treatment, prothrombin time (PT) of two groups was shorter than that before treatment, and the levels of TBil, ALT and AST in observation group were lower than those in control group, and PT in observation group was shorter than that in control group (P < 0.05). After treatment, the levels of hyaluronidase, blood laminin, type Ⅳ collagen and type Ⅲ procollagen in two groups were lower than those before treatment, and those in observation group were lower than those in control group (P < 0.05). The negative conversion rate of HBV-DNA in observation group was higher than that in control group at 23 and 46 weeks of treatment, and the differences were statistically significant (P < 0.05). Conclusion Yiligan Capsules combined with Tenofovir has significant clinical effect on patients with hepatitis B cirrhosis, it can protect the activity of liver cells, significantly improve the liver function, reduce liver fibrosis and improve the quality of life of patients, which is worthy of clinical application.

Objective To explore clinical effect of Anluohuaxian Pills combined with Entecavir in the treatment of hepatitis B cirrhosis. Methods Sixty-eight patients with hepatitis B cirrhosis admitted to Hebei Traditional Chinese Medicine and Liver Disease Hospital from June 2017 to June 2018 were selected and divided into two groups according to random number table method, 34 cases in each group. Control group received Entecavir treatment, observation group received Anluohuaxian Pills combined with Entecavir treatment, the course of treatment lasted for one year. The negative conversion rate of HBV-DNA and HBeAg, Child-Pugh score and liver function index, serum liver fibrosis index, liver spleen imaging index and adverse reactions were compared between two groups. Results The negative conversion rates of HBV-DNA and HBeAg in observation group after treatment were higher than those in control group (P < 0.05). After treatment, Child-Pugh score, albumin and total bilirubin levels of observation group were lower than those of control group, and alanine aminotransferase level of observation group was higher than that of control group, the differences were statistically significant (P < 0.05). After treatment, the serum levels of hyaluronic acid, human laminin and procollagen type Ⅲ in observation group were lower than those in control group (P < 0.05). The diameter of portal vein trunk, the length of spleen and the proportion of splenic vein in observation group were significantly higher than those in control group (P < 0.05). No adverse reactions occurred during the treatment. Conclusion Anluohuaxian Pills combined with Entecavir in the treatment of hepatitis B cirrhosis has exact effect, it can significantly improve the liver function of patients, and relieve the deterioration trend of liver fiber related indicators, it is a kind of therapy worthy of promotion.

155 Application effect of Neuman intervention model in patients with hyperglycemia after liver transplantation

Objective To investigate the effect of Neuman intervention model in patients with hyperglycemia after liver transplantation. Methods From January 2018 to January 2019, 86 patients with liver transplantation admitted to Nanjing Drum Tower Hospital of Nanjing University Medical College were selected and divided into control group and observation group by random number table method, 43 in each group. The control group was given routine intervention mode, while the observation group was given Neuman intervention mode. The blood sugar control, the change of treatment-related indicators and the incidence of postoperatre complications and familys′ satisfaction were compared between the two groups. Results After intervention, fasting blood glucose and 2 h postprandial blood glucose levels in both groups were lower than those before intervention, and the observation group was lower than the control group, with high statistically significant differences (P < 0.01); the incision healing time, blood glucose compliance time, and hospital stay time of observation group were all shorter than the control group, and the insulin consumption was lower than that of the control group when the blood glucose reached the standard, the differences were statistically significant (all P < 0.01); the incidence of postoperative complications in the observation group was less than in the control group, the difference was statistically significant (P < 0.05). The satisfaction of the family in observation group was higher than the control group, and the difference was statistically significant (P < 0.05). Conclusion Neuman intervention model can effectively control the blood sugar of patients after liver transplantation, improve the changes of treatment-related indicators, and reduce the incidence of postoperative complications, it is worth promoting.

159 Effect of special care on the effect of total knee arthroplasty and postoperative blood glucose in elderly diabetic patients

Objective To explore the effect of special nursing on elderly patients with diabetic total knee replacement (TKA). Methods From June 2016 to June 2018, 120 elderly patients with diabetic TKA admitted to the First People′s Hospital of Lianyungang City, Jiangsu Province were selected. According to random alphabet method, the patients were divided into control group and observation group, 60 cases in each group. The control group was given routine nursing intervention, and the observation group was given special nursing intervention based on routine nursing. Comparison of perioperative blood glucose indexes, nursing quality, nursing satisfaction, hospital for special surgery (HSS), knee of motion (ROM), and self-rating anxiety scale (SAS), self-rating depression scale (SDS) score and differences in complications. Results Comparison of glycosylated hemoglobin (HbA1c), blood glucose (2hPG) and fasting blood glucose (FPG) before nursing intervention between the two groups showed no statistically significant differences (P > 0.05); HbA1c, 2 hPG, and FPG were lower in the two groups after the nursing intervention than before the intervention, and the observation group were lower than those in the control group (P < 0.05); efficacy evaluation was performed six months after operation, and the nursing quality, nursing satisfaction, HSS score, and ROM scores of the observation group were higher than those in the control group (P < 0.05); SAS and SDS scores in both groups were lower than those before intervention, and those in the observation group were lower than the control group (P < 0.05); the incidence of complications and TKA revision in the observation group was lower than that in the control group (P < 0.05). Conclusion Special nursing intervention has obvious influence on elderly diabetic TKA patients, which can promote the curative effect, improve the blood sugar level, reduce the incidence of complications, and improve the quality of care.

165 Effect of targeted psychological intervention on anxiety, depression and quality of life in patients with refractory thyroid carcinoma targeted therapy

Objective To investigate the effect of targeted psychological intervention on anxiety, depression and quality of life in patients with refractory thyroid carcinoma targeted therapy. Methods A total of 60 patients who received targeted therapy for refractory thyroid cancer in Union Hospital, Tongji Medical College, Huazhong University of Science and Technology, from January 2015 to January 2017 were recorded as odd and even numbers according to the treatment time, they were randomly divided into the intervention group and the control group, with 30 patients in each group. The control group adopted general health education, and the intervention group carried out targeted psychological intervention on the basis of the control group. Anxiety, depression and quality of life were evaluated by the self-rating anxiety scale (SAS), self-rating depression scale (SDS) and the cancer quality of life questionnaire European Organization for Research and Treatment of Cancer (EORTC) QLQ-C30 (V3.0). Results Before intervention, there were no significant differences in SAS, SDS and EORTC QLQ-C30 (v3.0) between the two groups (P > 0.05). After intervention, SAS and SDS in the intervention group were all decreased, and the differences were statistically significant (P < 0.05); SDS score in the control group was lower than that before intervention, and the difference was statistically significant (P < 0.05). Physical function, role, emotion, society, total health level, fatigue, pain, shortness of breath, and insomnia in the intervention group were statistically significant (all P < 0.05). In the EORTC QLQ-C30 questionnaire, there was no statistically significant difference (P > 0.05) except for the economic difficulties and the comparison with the control group, and the other indicators showed statistically significant differences (all P < 0.05). Conclusion Targeted psychological intervention can improve anxiety, depression and quality of life in patients with refractory thyroid cancer targeted therapy.

169 Influence of positive psychological intervention based on internet technology on mental state and quality of life of patients after breast cancer chemotherapy

Objective To explore the application effect of positive psychological intervention based on internet technology in patients after breast cancer after chemotherapy. Methods From January 2018 to July 2019, 110 patients with breast cancer received postoperative chemotherapy in the First Affiliated Hospital of Nanjing Medical University were selected and divided into the experimental group and the control group by random number table method, with 55 cases in each group. The control group was given routine nursing intervention, and the experimental group was given positive psychological intervention based on internet technology in addition. The mood state, quality of life and satisfaction with psychological intervention were compared between the two groups after six months of follow-up. Results Scores of tension-anxiety, depression-depression, fatigue-dullness in the experimental group were lower than those in the control group, and scores of energy-vitality were higher than those in the control group, with high statistically significant differences (P < 0.01); physiological status, social/family status, emotional status, additional attention, and total score of breast cancer quality of life scale were higher than those of the control group, with statistically significant differences (P < 0.05 or P < 0.01); satisfaction with psychological intervention in the experimental group was higher than that in the control group, with statistically significant difference (P < 0.05). Conclusion The positive psychological intervention based on internet technology is helpful to correct the mood state of breast cancer patients after chemotherapy, improve the quality of life, and improve nursing satisfaction.

173 Effect of nursing intervention based on the concept of fast rehabilitation surgery on the efficacy, fatigue and quality of life of patients with lung cancer

Objective To explore the role of nursing intervention based on the concept of fast rehabilitation surgery (FTS) in improving the clinical effect, fatigue level and quality of life of patients with lung cancer undergoing surgery. Methods From August 2017 to August 2019, Cangzhou Central Hospital, Hebei Province, a total of 216 patients with thoracoscopic-assisted tumor resection for lung cancer were treated. They were divided into FTS group and control group by random number table method, with 108 cases in each group. The control group was given routine nursing intervention, and the FTS group was given nursing intervention based on the concept of FTS. The postoperative recovery indicators and postoperative complications in severn days were recorded for the two groups; self-made questionnaires were used to assess the patient′s satisfaction rate with care, the Piper fatigue scale to assess the degree of fatigue, and the quality of life questionnaire (QLQ-C30) scale to assess the quality of life. Results The extubation time, exhaust time, and time to get out of bed in the FTS group were earlier than those in the control group, and the differences were statistically significant (P < 0.05); the antibiotic use time and hospital stay were shorter than the control group, and the differences were statistically significant (P < 0.05). The incidence of complications in the FTS group was lower than that in the control group, and the difference was statistically significant (P < 0.05). The nursing satisfaction rate of the FTS group was higher than that of the control group, and the difference was statistically significant (P < 0.05). The behavioral, emotional, physical, cognitive scores and total scores of the two groups after intervention were all lower than before the intervention, while the FTS group was lower than the control group, and the differences were statistically significant (P < 0.05). The QLQ-C30 function scores of the two groups after intervention were higher than those before the intervention, while the symptom scores were lower than before the intervention, and the differences were statistically significant (P < 0.05). The functional score of the FTS group was higher than that of the control group, while the symptom score was lower than that of the control group, and the differences were statistically significant (P < 0.05). Conclusion Nursing intervention based on the concept of FTS can promote postoperative recovery of lung cancer patients, reduce the incidence of complications, improve fatigue, and improve the quality of life.

177 Clinical analysis of acceptance and commitment therapy intervention for post-traumatic stress disorder of parents in children with developmental dislocation of the hip

Objective To investigate the effect of acceptance and commitment therapy intervention for post-traumatic stress disorder of parents in children with developmental dislocation of the hip (DDH), and to provide the guidance for the development of future intervention programs of this disease. Methods A total of 108 parents of DDH children who admitted in the Hu′nan Children′s Hospital from May to December 2019 were selected as the research subjects. One person was randomly selected from the parents of each child for observation. The number parity method was used to divide the children′s parents into observation group (odd numbers) and control group (even numbers), with 54 cases in each group. All selected cases received routine nursing, while observation group was given acceptance and commitment therapy based on routine nursing. The general information of the children′s parents in two groups were investigated; the post-traumatic stress disorder, coping style, acceptance and action of the children′s parents of two groups were evaluated by relevant scale or questionnaire before intervention, after intervention for three and six weeks; at the end of intervention, the incidence of post-traumatic stress disorder was compared between the two groups. Results There were statistically significant differences in post-traumatic stress disorder of the index group, time point and interaction comparison between the children′s parents of the two groups (P < 0.05). Intra-group comparison: after three and six weeks of intervention, the scores of post-traumatic stress disorder of the children′s parents of the two groups were lower than those before the intervention, while after six weeks of intervention was lower than after three weeks of intervention, and the differences were statistically significant (P < 0.05); comparison between groups: parental intervention of the observation group after three and six weeks, the scores of post-traumatic stress disorder were lower than those of the control group, and the differences were statistically significant (P < 0.05). There were statistically significant differences in the coping style and acceptance and action scores of children′s parents of the two groups between the groups, time points and interactions (P < 0.05). Further pairwise comparison, intra-group comparison: after three and six weeks of intervention, the scores of the parents′ coping styles of the two groups were higher than those before the intervention, while the scores of acceptance and action were lower than those before the intervention, and the score of coping style after six weeks of intervention was higher than that of three weeks after intervention, while the score of acceptance and action was lower than that of three weeks after intervention, and the differences were statistically significant (P < 0.05); comparison between groups: the scores of coping styles of children in the observation group after three and six weeks of intervention were higher than those of the control group, while the scores of acceptance and action were lower than those of the control group, and the differences were statistically significant (P < 0.05). The incidence of post-traumatic stress syndrome in the observation group was lower than that in the control group, and the difference was statistically significant (P < 0.05). Conclusion Acceptance and commitment therapy in parents of children with DDH can improve the acceptance of DDH-related disease thought and feeling. Parents can respond in a positive and optimistic manner, reduce the degree of post-traumatic stress disorder, and decrease the occurrence of stress syndrome.

182 Application of comprehensive health management based on timing theory in elderly patients after valve replacement

Objective To explore the impact of comprehensive health management based on timing theory on medication compliance and quality of life of elderly patients after valve replacement. Methods From January 2018 to June 2019, a total of 94 elderly patients who underwent heart valve replacement surgery in the Department of Cardiovascular Surgery of Affiliated Hospital of Zunyi Medical University were divided into control group (48 cases) and intervention group (46 cases) by using random number table method. Patients in both groups were given routine care and guidance before discharge. The intervention group implemented comprehensive health management based on timing theory on the basis of the control group. The medication compliance and quality of life of the two groups were evaluated before discharge and one, three and six months after discharge. Results The medication compliance scores of patients in the intervention group at one, three and six months after discharge were higher than those in the control group, and the differences were statistically significant (P < 0.05). There was no statistically significant difference in medication compliance scores between the two groups after discharge from the hospital (P > 0.05). Comparing the scores of time factors between the two groups, and the difference was highly statistically significant (P < 0.01). There were no interaction in the dimensions of physiological field, social relations, family friction, and appetite (P > 0.05), while there were interactions in other dimensions (P < 0.05). The results of the comparison between groups showed that the differences were statistically significant in the total score of quality of life, overall condition, psychological field, social relationship, environmental field, and family friction score between two groups in each time point after discharge (P < 0.05). Conclusion Comprehensive health management based on the timing theory can effectively improve medication compliance and quality of life of elderly patients after valve replacement.

186 Application of empathy concept-oriented nursing strategies to elderly women in the early stages of cesarean delivery

Objective To explore the application effect of nursing strategies based on empathy-oriented concept in elderly primiparas with cesarean section. Methods From July 2017 to July 2019, a total of 94 elderly primiparous women who were admitted to Huai′an First People′s Hospital were selected as the research objects. According to the random number table method, they were divided into intervention group (48 cases) and control group (46 cases). The control group was given routine nursing intervention, while the intervention group was combined with a care strategy based on empathy concept on the basis of routine nursing. The mental health status, hope level, and nursing satisfaction of the two groups of senile primipara after cesarean section were compared. Results After the intervention, the scores of maternal somatization, interpersonal sensitivity, depression, anxiety, and horror in the two groups were significantly lower than those before the intervention, while the intervention group were lower than those in the control group, and the differences were statistically significant (P < 0.05 or P < 0.01). After the intervention, attitudes towards reality and the future, maintaining close relationships with others, adopting positive actions, and the total score of hope of the two groups were significantly higher than those before the intervention, while the intervention group were higher than those in the control group, and the differences were statistically significant (P < 0.05 or P < 0.01). The nursing satisfaction of the intervention group was significantly higher than that of the control group, and the difference was statistically significant (P < 0.05). Conclusion The nursing strategies based on the concept of empathy can help improve the mental health of senile cesarean primiparas, promote the development of hope levels, and improve nursing satisfaction.

The policy of traditional Chinese medicine is an important guarantee for the development of Chinese medicine. Further improving the policy system of traditional Chinese medicine is an important basic work to realize the modernization of Chinese medicine governance ability and governance system. As a province enjoying a reputation of traditional Chinese medicine establishment, Yunnan still has some problems in traditional Chinese medicine policies, such as incomplete traditional Chinese medicine policy system, incomplete establishment of policy framework, lack of overall planning for policy research, insufficient investment and cooperation, lack of research strength and insufficient talent cultivation. This article tries to give advice on the government policy framework for the overall planning and perfecting the system, increasing investment, strengthening the cooperation between departments, establishing linkage mechanism. Only by strengthening construction of Chinese medicine research base, policy system, personnel training to promote a leapfrog development of traditional Chinese medicine for the health industry in Yunnan.

194 Present situation and prospect of urban community health management in China

In the context of the advance of the population ageing and the chronic disease barrier, urban community health management has become an effective measure to prevent disease. The city community health management has been developed and improved continuously from the introduction of various measures in our country, but there are still deficiencies in government investment, personnel training, service mode, market demand, information platform and so on. This paper analyzes the research progress and current situation of urban community health management, and gives some thoughts and prospects for its existing problems.
