DOI: 10.20047/j.issn1673-7210.2023.36.01
Objective To observe the effect of electro-acupuncture intervention on mitochondrial fission in diabetic mice with postoperative cognitive dysfunction (POCD). Methods Thirty-nine SPF-grade C57BL/6 male mice of body weight(20±2) g aged four to six weeks injected intraperitoneally with streptozotocin after 16 weeks of high-fat and high-sugar diet with fasting blood glucose≥11.1 mmol/L after one week, that is, they were experimental animals with diabetes mellitus type 2 (no mice died). Then they were divided into the control group, the model group, and the electro-acupuncture group by random number table method, 13 mice in each group. The control group did not do any other intervention except grasp and fixation. POCD model was established by tibial fracture intra- medullary fixation between model group and electroacupu- ncture group (no mice death). Compared with the control group, the cognitive ability of the model group decreased and the model was considered successful. In the electro-acupuncture group, shéntíng, bǎihuì, nèiguān, and zúsānlǐ were treated with electro-acupuncture intervention two weeks before surgery, and the needles were retained for 30 min each time for five consecutive days every week. The cognitive function of mice was evaluated by open field test and fear conditioning test one day after operation. Hematoxylin-eosin staining was used to observe the pathology of hippocampal CA1 region. The number of neurons was observed by Nissl staining. The content of adenosine triphosphate (ATP) was detected by chemiluminescence. Mitochondrial membrane potential (MMP) was detected by flow cytometry. Western blot analysis was performed to detect the protein expression levels of dynamin-related protein 1(Drp1), p-Drp1 Ser616, and p-Drp1 Ser637 in the hippocampus of mice. Results Compared with the control group, the freezing time of the context test in the model group was reduced (P<0.05). Compared with the model group, the freezing time of the context test in the electro-acupuncture group was increased (P<0.05). Compared with the control group, the damage of neurons in CA1 region of the model group was aggravated and the number of neurons was decreased (P<0.05). Compared with the model group, the damage of neurons in CA1 region was reduced and the number of neurons was increased in electro-acupuncture group (P<0.05). Compared with the control group, ATP content and MMP in the model group were decreased (P<0.05). Compared with the model group, ATP content and MMP in electro-acupuncture group were increased (P<0.05). Compared with the control group, the expression level of p-Drp1 Ser616 protein in hippocampus of model group was increased, while the expression level of p-Drp1 Ser616 protein was decreased (P<0.05). Compared with the the model group, the expression level of p-Drp1 Ser616 protein in hippocampus of the electro-acupuncture group was decreased, and the expression level of p-Drp1 Ser616 protein was increased (P<0.05). Conclusion Electro-acupuncture can improve the learning and memory ability of POCD in diabetic mice and reduce the neuronal damage. The mechanism may be related to the reduction of Drp1-mediated mitochondrial hyperfission and the improvement of hippocampal mitochondrial function.

Methods A total of 102 7-week-old male SD rats with body weight of (195±20) g were divided into normal group, model group, Ambroxol positive control group, and Chinese medicine low-dose, medium-dose, high-dose groups by random number table method, with 17 rats in each group. Except for the normal group, the other groups were treated with lipopolysaccharide drops combined with cigarette smoke exposure to establish COPD airway mucus hypersecretion rat model. After successful modeling, Ambroxol positive control group was given Ambroxol solution (54 mg/[kg·d]), Chinese medicine low-dose, medium-dose, high-dose groups were given 4.375, 8.750, and 17.500 g/(kg·d) Mashe Anjin Pills (decoction), respectively, normal group and model group were given equal volume of normal saline, and all six groups were given continuous gavage for 28 days. After the experiment, hematoxylin-eosin staining and periodic acid Schiff stain were used to observe the pathological changes of lung tissue in each group. Serum inflammatory factors and alveolar lavage fluid cytokine levels were detected by enzyme-linked immunosorbent assay. The contents of MUC5AC and GABAARα in lung tissue were detected by Western blot. Results The pathological changes of airway and lung tissues in the model group were more serious. The levels of interleukin-1β (IL-1β), neutrophil elastase (NE), tumor necrosis factor-α (TNF-α), transforming growth factor-β1 (TGF-β1), tissue metalloproteinase-1 (TIMP-1), matrix metalloproteinase-9 (MMP-9), MUC5AC, GABAARα of model group were higher than those of normal group, the differences were statistically significant (P<0.05). The pathological changes of airway and lung tissues in Chinese medicine low-dose, medium-dose, high-dose groups were less than those in model group, and the levels of IL-1β, NE, TNF-α, TGF-β1, TIMP-1, MMP-9, MUC5AC, and GABAARα were lower than those in model group, the differences were statistically significant (P<0.05). The levels of IL-1β, NE, MMP-9, and GABAARα in Chinese medicine medium-dose group were lower than those in Chinese medicine low-dose group. The levels of IL-1β, NE, TNF-α, TGF-β1, TIMP-1, MMP-9, MUC5AC, and GABAARα in Chinese medicine high-dose group were lower than those in Chinese medicine low-dose and medium-dose groups, and the differences were statistically significant (P<0.05). Conclusion Mashe Anjin Pills can improve airway mucus hypersecretion and inhibit airway remodeling in COPD, which may be related to reducing inflammatory injury and inhibiting the expression of MUC5AC and GABAARα.

DOI: 10.20047/j.issn1673-7210.2023.36.08
A lot of studies have shown that Rhei Radix Et Rhizoma has good efficacy in the treatment of sepsis. In order to further clarify the related role and mechanism of Rhei Radix Et Rhizoma in the treatment of sepsis, this paper reviews the understanding of sepsis in traditional Chinese medicine, the negative effect of sepsis on intestinal flora, the significance of intestinal flora regulation in the treatment of sepsis, and the effect of Rhei Radix Et Rhizoma on sepsis by regulating intestinal flora. Studies have found that the occurrence and development of sepsis is closely related to the changes of intestinal flora. Single drug Rhei Radix Et Rhizoma, Rhei Radix Et Rhizoma compound and Rhei Radix Et Rhizoma monomer can effectively treat sepsis, and its mechanism may be related to improving intestinal flora imbalance, protecting gastrointestinal mucosal barrier, preventing intestinal bacterial translocation, and endotoxin absorption into the blood, and reducing inflammatory response. This review further clarified the therapeutic effect of Rhei Radix Et Rhizoma on sepsis and its related mechanism, and provided a theoretical basis for the clinical prevention and treatment of sepsis.

DOI: 10.20047/j.issn1673-7210.2023.36.10
Insulin resistance is related to the occurrence and development of various metabolic diseases such as polycystic ovary syndrome, non-alcoholic fatty liver disease, and type two diabetes mellitus. With the improvement of living standards, the probability of obesity and insulin resistance is increasing. The research on insulin sensitivity has received increasing attention, with a focus on the mechanism of insulin resistance. At present, the mechanism of insulin resistance is not only lipid overload and inflammation, but also a series of stress reactions, molecular level mutations of substances in the insulin signaling pathway, autophagy, and intestinal microbiota imbalance. The published articles on the mechanism of insulin resistance are not comprehensive enough, therefore, this article reviews the existing mechanisms of insulin resistance from multiple aspects mentioned above.

DOI: 10.20047/j.issn1673-7210.2023.36.23
This paper summarizes the clinical experience of Professor Lü Peiwen in the treatment of acute mastitis during lactation using Furong Ointment. The incidence of acute mastitis during lactation has increased significantly in recent years, and the probability of progression from mastitis to breast abscess increases greatly if the treatment is untimely and incorrect. Professor Lü Peiwen believes that this disease is mostly caused by the accumulation of breast milk, and the feeling of external evils, and the treatment is to clear away heat, remove toxins and dissipate lumps. Furong Ointment, as a classic prescription of yanjing school of surgery, has been used clinically for more than 50 years, which is based on the traditional formula and skillfully adds drugs to enhance the effectiveness and prevent therapeutic side effects, and is highly effective in treating acute mastitis during lactation, and it has the advantages of being easy to use, affordable, and with fewer adverse reactions. Professor Lü Peiwen emphasizes syndrome differentiation, focusing on the treatment modalities as well as the timing of entry, the use of Furong Ointment in the early stage of the disease is simple but specialized to reduce the patient’s symptoms and control the development of the disease effectively, reflecting the traditional Chinese medicine thinking of “people oriented and preventing transform when sickness”. And with one experimental case.

DOI: 10.20047/j.issn1673-7210.2023.36.24
Objective To analyze the correlation between preoperative multimodal ultrasound features and recurrence risk of breast cancer and to explore its value. Methods A total of 286 cases (286 lesions) with breast cancer patients who underwent surgery in Mudan People’s Hospital of Heze City, Shandong Province from January 2020 to January 2023 were selected. They were divided into training set (200 cases) and validation set (86 cases) according to the ratio of 7∶3. The training set was divided into high risk group (68 cases) and a low-medium risk group (132 cases) according to the Guidelines and Norms for Diagnosis and Treatment of Breast Cancer of the Chinese Anti-Cancer Association 2021 Edition. The factors influencing the risk of breast cancer recurrence after surgery were analyzed and the prediction model of nomogram was established. The performance of the model was evaluated by receiver operating characteristic curve and calibration curve. Results There was no significant difference in general data between the training set and verification set (P>0.05). TNM stage, pathological grade, lymph node metastasis, tumor diameter, boundary, hyperechoic halo, internal echo, blood flow grade, and elastic score were significantly different between high risk group and low-medium risk group (P<0.05). Tumor diameter (OR=1.428), boundary (OR=3.425), hyperechoic halo (OR=3.300), internal echo (OR=9.631), and blood flow grade (OR=1.725) were independent risk factors for postoperative recurrence of breast cancer patients (P<0.05). Based on this, a nomogram prediction model was constructed. The area under the curve (AUC) in the training set was 0.832, and the AUC in verification set was 0.800, which showed good performance. Calibration curve consistency was high. Conclusion It is of high practical value to construct a nomogram model for predicting the recurrence risk of breast cancer based on preoperative multimodal ultrasound.

117 Literature Identification of the treatment of moxibustion to cure cholera after death in the Zhouhou Beiji Fang
FENG Jialei XIAO Yongzhi
DOI: 10.20047/j.issn1673-7210.2023.36.25
Ge Hong, a physician of the Jin Dynasty, discussed in detail the moxibustion treatment of cholera in Zhouhou Beiji Fang, and Beiji Qianjin Yao Fang and Waitai Miyao quoted the text a lot. By studying the content of moxibustion to cure cholera after death in the three books, it was found that there were textual differences in the descriptions of the “moxibustion therapy for resurrecting cholera” and “Hua Tuo’s treatment for cholera after death”. Referring to the Yixin Fang, it was believed that the acupoints measurement in the “moxibustion therapy for resurrecting cholera” should be based on the “renkou” standard. The positioning for moxibustion in “Hua Tuo’s treatment for cholera after death” should be more accurately based on the recorded information in Beiji Qianjin Yao Fang and Waitai Miyao. Moreover, the measurement of the jiaji should be one cun, which is more reasonable. By examining the relationship between Zhouhou Beiji Fang, Beiji Qianjin Yao Fang, Waitai Miyao, and Yixin Fang, this paper aims to analyze the accuracy of the texts, explore the reasons for the differences, and provide references for clinical applications.

127 Professor Guo Jun’s experience in the treatment of benign prostatic hyperplasia with Guiyi Tonglong Formula based on the “brain-heart-kidney-essence chamber” axis theory
YANG Jiutian1 GUO Jun2 ZHAO Ziwei2 MA Dongyue2 REN Kai2 WANG Fu2
DOI: 10.20047/j.issn1673-7210.2023.36.27
Traditional Chinese medicine believes that benign prostatic hyperplasia is often caused by kidney deficiency and instability, as well as bladder disturbance in functioning of qi. Professor Guo Jun proposes the “brain-heart-kidney-essence chamber” axis theory based on this and combined with modern psychological factors such as high work pressure, emphasizing the differentiation and treatment of both body and mind, as well as the combination of overall and local factors. Professor Guo Jun believes that the kidneys and seminal chambers reside together in the lower jiao, which is responsible for water and regulates urine metabolism locally. The heart and brain reside together in the upper jiao, which hides the mind and regulates the physiological functions of the kidneys and seminal chambers as a whole. Any disorder of “brain-heart-kidney-essence chamber” axis can lead to the occurrence of this disease, mainly including three types: loss of nourishment of heart and brain, confusion of the mind, kidney deficiency and instability, disturbance in functioning of qi, stagnation of lower jiao and dysfunction of seminal chamber. Guiyi Tonglong Formula is formulated under the guidance of the “brain-heart-kidney-essence chamber” axis theory. This prescription revolves around tonifying the kidney and warming yang, promoting blood circulation and resolving blood stasis, and has both dredging and tonifying functions. It can provide reference for the clinical treatment of benign prostatic hyperplasia.

DOI: 10.20047/j.issn1673-7210.2023.36.40
Hemophilia C (factor Ⅺ deficiency) is a rare coagulation dysfunction disorder. Spontaneous bleeding is rare in patients with hemophilia C, and significant bleeding usually occurs after trauma or surgical procedures. According to the literature review, there have been no reports of patients with hemophilia C undergoing laparoscopic cholecystectomy. The patient sought medical treatment due to “gallstone discovery for more than four years and intermittent dull pain in the abdomen for more than ten months”, and had a previous history of hemophilia C. Preoperative coagulation function examination showed that the activated partial thromboplastin time was significantly prolonged (67.4 s), and the factor Ⅺ activity was 2.4%. According to the consultation of hematology department, the patient was treated with fresh frozen plasma and tranexamic acid in perioperative period. In the end, the operation was successful, and there was no obvious bleeding during and after the operation. This case report suggests that it is safe for hemophilia C patients to receive laparoscopic cholecystectomy after symptomatic treatment, and can provide reference for clinical diagnosis and treatment of similar cases.
