4 Study on chronic toxicity of freeze-dried powdered Cassiae Semen in Sprague-Dawley rats

Objective  To evaluate the chronic toxicity of Semen Cassiae freeze-dried powder  in rats. Methods 480 SD rats were randomly divided into 4 groups (60 male and 60 female per group): control group (pure water), low-dose group (0.5 g/kg BW), mid-dose group (2.2 g/kg BW) and high-dose group (10.0 g/kg BW). The volume of drug for gavaging was 1.0 mL/100 g. animals were administrated with drug by 6 days every week for 52 weeks. Subsequently, the weight and foodintake were examined per week. Meanwhile, hematology, blood biochemistry, organ weight, organ coefficient and histopathology were detected on 13-week, 26- week, 39-week and 52-week, and stopping drug at the end of 26-week, 52-week following by 4 weeks recovery period with killing 20 rats in each group. Results ①The result showed there was no death case in each group. ②The weight of rats in dose groups and control group were increased, and it showed dose-related. ③Compared with the control group, the total bilirubin of male rats in mid-dose and high-dose group were lower after 26 weeks (P < 0.01), after recovery period the TBIL of male rats in high-dose group was still lower (P < 0.01). After 39 weeks treatment, the TBIL in high-dose group lower compared with the control group (P < 0.05 or P < 0.01); The triglyceride of male rats in high-dose group was lower after 52 weeks(P < 0.05) and returned to normal after recovered 4 weeks. On 26 weeks and 39 weeks, there were significant increased in relative kidney weights of male rats in mid-dose group and high-dose group compared with control group (P < 0.05 or P < 0.01). ④The toxic target organ in high-dose group was kidney on 26, 39, 52 weeks, and the  toxicity was irreversible, no toxic showed in low-dose group and mid-dose group. Conclusion The no-observed-adverse-effect level for Semen Cassiae freeze-dried powder was 2.2 g/kg BW in rats, and the toxic target organ in high-dose (10.0 g/kg BW) group was kidney.

10 Prognostic significance of CD38 in M2 subtype of acute myeloid leukemia

Objective  To investigate the expression level of CD38 in the M2 subtype of acute myeloid leukemia and its effect on the prognosis of patients. Methods From January 2012 to December 2015, in PLA General Hospital, 47 patients with primary M2 subtype of acute myeloid leukemia were collected. The expression level of CD38 on leukemic cells was detected by flow cytometry. The clinical data and laboratorial characteristics and the survival status were compared between these patients, and the prognostic significance of CD38 was analyzed retrospectively with the combination of the most common genetic aberration of t(8;21) translocation in this type of leukemia. Results  The rate of CD38+ patients was 100% in the M2 subtype of patients with t (8;21) translocation, which was higher than the 73.9% of CD38+ patients in non-t (8;21) patients (P = 0.02). The remission rate after the induction therapy in the CD38- patients with non-t (8;21) was lower than those in the CD38+ patients with or without t (8;21) translocation (50.0%, 88.2%, 95.8%, P = 0.03). The survival analysis showed that the 3-year EFS rate of these three cohorts was 16.7%, 38.1% and 58.7%, respectively, the differences were not statistically significant (P > 0.05), while the 5-year OS rate of the CD38-patients was much lower than those in the CD38+ patients with or without t (8;21) translocation (16.7%, 44.0%, 47.2%, P = 0.03). Conclusion CD38 expression has a positive correlation with t(8;21) in the M2 subtype of acute myeloid leukemia, and the downregulation of CD38 expression is one of the hallmarks of poor prognosis in this subtype, which is expected to guide the prognostic stratification and personalized therapy of the patients.

14 Study of STAT1 knockout mice increasing sensitivity in EV71 infection

Objective To analyze the differential expression of immune-associated factors in STAT1 knockout mice, and observe the symptoms and viral load after EV71 virus infection, in order to provide a data base for anti-EV71 therapy. Methods Cytometric bead assay (CBA) was used to quantitative analysis the levels of serum immune-associated factors in STAT1 knockout mice after LPS-induced treatment. The symptom and tissue pathological features were observed and the level of muscle virus RNA was quantitated by real-time PCR in STAT1 knockout mice after EV71 virus infection. Results The levels of multi- immune-associated factors were decreased in STAT1 knockout mice after LPS stimulating (P < 0.01), and pathological damage was aggravated accompanied by the higher viral load in muscle (P < 0.01), also severe symptoms and higher mortality when compared to wide type mice after EV71 infection. Conclusions STAT1 plays an important role in the immune responseafter EV71 infection.Thus, STAT1 gene deletion can improve the sensitivity of mice to EV71 virus.

18 Change and significance of HIF-1α mediated by TLR4 signal pathway in the model of lung ischemia reperfusion injury rats

Objective To investigate the changes and significance of hypoxia inducible factor-1α (HIF-1α) mediated by Toll like receptor 4 (TLR4) signal pathway in the model of lung ischemia reperfusion injury (IRI) rats. Methods A total of 48 healthy male SD rats were selected and randomly divided into sham operation group, IRI model group, TLR4 activation group (LPS intervention) and TLR4 inhibition group (TAK-242 intervention) with 12 rats in each group. 3 weeks before model, IRI model group and sham operation group were injected with saline, TLR4 activation group was injected with lipopolysaccharide, TLR4 inhibition group was injected with TAK-242, once a week for three weeks. IRI model group, TLR4 activation group and TLR4 inhibition group were established a model of lung ischemia-reperfusion injury after last injection 30 minutes, sham operation group was treated same as the other groups except for not block the lung door. 3 hours after reperfusion injury, TLR4, HIF-1α, tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) mRNA expression of lung tissue were tested by reverse transcriptase polymerase chain reaction (RT-PCR). TLR4, HIF-1α, TNF-α and IL-6 protein expression of lung tissue were tested by Western-blot. Results The lung wet weight/dry weight ratio of the IRI model group was higher than that of the sham operation group (P < 0.05), the TLR4 activation group was higher than that of the model group (P < 0.05), and the TLR4 inhibition group was lower than that of the model group (P < 0.05). The mRNA and protein expression levels of TLR4, HIF-1α, TNF-α, IL-6 in the lung tissues of IRI model group were higher than those of the sham operation group (P < 0.05), the TLR4 activation were higher than those of the model group (P < 0.05), and the TLR4 inhibition group were lower than those of the model group (P < 0.05). Conclusion The TLR4 signal pathway in the lung tissue mediates the HIF-1α expression increased significantly after lung ischemia-reperfusion injury in rats, and increases the degree of inflammatory damage to the lung tissue.

26 The preparation and evaluation of experimental autoimmune neuritis rat model

Objective To explore the establishment of experimental autoimmune neuritis (EAN) rat model and the evaluation of related indexes. Methods Twenty female Lewis rats weighing 140-160 g were randomly divided into EAN group (n = 10) and control group (n = 10), and the number of each group was 1-10. The EAN group was immunized with P0180-199 peptide. The control group was injected with saline. After 42 days of immunization, the behavioral changes were recorded and scored. At the eighteenth days after immunization, the nerve-muscle action potential, sciatic nerve electronic microscope and immunohistochemistry and HE staining of gastrocnemius muscle and other indicators of the rats in the two groups were detected. Results The sciatic nerve conduction velocity in the peak of the compound action potential in the EAN group was lower than that of control group, with statistically significant difference (P < 0.05), and the amplitude was obviously lower than that of control group, with highly statistically significant difference (P < 0.01). The results of electron-microscope showed swell, honeycomb changes and miss out of myelin sheath, with an attenuation or disappearance of axon in the EAN group. The results of HE staining showed thinning of gastrocnemius-myofibers and stained cytoplasm and other degenerative changes in the EAN group. The results of immunohistochemistry showed that inflammatory factors Iba-1 (macrophage) and CD3 (T cells) were increased in those with swell and miss out of myelin sheath. The axonal protein NF 200 was decreased in those with miss out of myelin sheath. The myelin protein S100 was decreased in those with honeycomb changes and miss out of myelin sheath. Conclusion The autoimmune neuritis model induced by the P0180-199 has high incidence and easy copy, and the pathological changes are close to clinical idiopathic neuritis. In this study, the evaluation indexes of EAN during peak hours provide a basis for the further study of the physiological process of this disease.

31 Clinical analysis of multiple myeloma with pleural effusion

Objective To analyze the clinical characteristics of multiple myeloma (MM) with pleural effusion. Methods This was a retrospective study. Patients diagnosed of MM from January 2000 to December 2016 in Peking University First Hospital were included. Demographic, clinical and laboratory tests data were collected prospectively and analyzed statistically. Results The overall rate of pleural effusion was 37.4%(130/348) in MM patients, and myelomatous pleural effusion(MPE) rate showed to be 2.6%(9/348). The levels of total protein (TP), albumin, calcium, and complement 3 (C3) in the pleural effusion group were lower than those in the non-pleural effusion group, and the LDH level in the pleural effusion group was higher than that in the non-pleural effusion group (P < 0.05). Hypocomplementemia of C3(OR=0.282, 95%CI 0.096-0.830, P < 0.01) and lower level of TP(OR=0.979, 95%CI 0.966-0.993, P < 0.01) were independent risk factors of pleural effusion production according to multiple Logisitic regression. The levels of LDH and adenosine deaminase (ADA) were higher level in the MPE group than those in the non-MPE group (P < 0.05). Conclusion Pleural effusion is one of the complications of MM, in which MPE is rare. Hypocomplementemia of C3 and lower level of TP seem to be independent risk factors of pleural effusion production in MM. High level of LDH and ADA in pleural effusion remind the high risk of MPE. The pathophysiology of pleural effusion, especially MPE in MM needs further research in the future.

35 Advances of the relationship between blood pressure variability and target organ damage in patients with high blood pressure

There is sufficient evidence that hypertension caused target organ damage, especially in the cardiovascular system. The damage degree of target organs is positively correlated with BP variability (BPV). A growing body of evidences show that the more hypertension patient's BPV increased, the more serious target organ damaged, so it can reduce the risk of target organ damage in some degree by reducing the BPV. This article reviews the literature about the methods of BPV monitoring and relationship between BPV and target organ damage, looks forward to reduce the target organ damage rate of the hypertension patients and the incidence of clinical diseases by reducing the BPV.

39 Progression of PD-1/PD-L1 in non-small cell lung cancer therapy

The traditional treatment of lung cancer is not satisfactory, so it is urgent to explore new therapy to break the bottleneck. Immunotherapy is a new treatment with low toxicity and high specificity. Suppressing the PD-1/PD-L1 pathway is considered as one of the most promising treatments. A number of studies reported that blockade of PD-1 and its ligand PD-L1 achieved favorable outcomes for advanced non-small cell lung cancer patients. Nonetheless, the anti-PD-1/PD-L1 antibody as monotherapy or in combination with other antineoplastic therapy in non-small cell lung cancer remains controversial. In this article, we reviewes the current state of research and advancement of PD-1/PD-L1 in the treatment of non-small cell lung cancer.

43 Mechanism research on targeted autophagy therapy of non-alcoholic fatty liver disease in type 2 diabetes mellitus

Non-alcoholic fatty liver disease (NAFLD) is an independent risk factor for advanced liver disease, type 2 diabetes mellitus (T2DM), and cardiovascular diseases. T2DM is always accompanied by NAFLD as result of a common pathogenesis. Hepatic lipid accumulation and mitochondrial dysfunction proved to be two essential mechanisms for the pathogenesis of T2DM with NAFLD. Regulation of lipophagy and mitophagy in hepatocyte are potential targets for disease therapy. However, autophagy is considered to be a double-edged sword. Moderate autophagy protects liver, yet excessive or failure one aggravates its injury. The evolution of NAFLD could be associated with dynamic regulation of autophagy and additional studies are required. This review aims to investigate the potential value of autophagy regulation in the treatment of NAFLD in type 2 diabetes mellitus, in order to provide innovative targets for therapeutic intervention.

47 Application status of 5-hydroxymethylfurfural in quality standard of traditional Chinese medicine

Considering the research status of current quality standard of traditional Chinese medicine, this paper put forward the necessity of searching common composition as indicators and summarizes the source, formation, influence factors, general presence of 5-hydroxymethylfurfural and its wide application in the traditional Chinese medicine injection, warehousing and storage of Chinese medicine, Chinese herbal medicine compound preparation by searching literatures. Through reviewing the relevant research reports in recent years, this paper investigates and analyzes the feasibility of common composition of general presence as a new indicator of quality assessment and standardizing processing technology in the field of Chinese herbal medicine processing, so as to expand new thought for the quality evaluation system of Chinese herbal medicine processing products and the establishment and improvement of standard Chinese herbal medicine processing technology.

51 Research progress of midwives' retention in foreign countries and its revelation to China

Midwives play an important role in ensuring the safety of mother and baby. With the opening of the two child policy, the number of high-risk pregnant women increased, the quality and quantity of midwives are put forward higher requirements. However, the midwives' intention is not ideal in China. The overall stability of midwives is poor, and there is less research about the retention of midwives. This paper summarizes the influential factors and intervention measures of foreign midwives' intention, puts forward some advice to promote the role of China midwives' intention, in order to meet the current two child policy needs. Finally, hope to improve the maternal and child health.

55 Research progress on strategies for the treatment of hyperlipidemia in the elderly

Hyperlipidemia is an independent risk factor for atherosclerotic cardiovascular disease, and the risk increased with age. Active treatment of hyperlipidemia has great significance for the prevention and treatment of atherosclerotic cardiovascular disease in the elderly people. The physiological characteristics and the distribution characteristics of hyperlipidemia in the elderly people are different from those of the young people. In the treatment of hyperlipidemia, elderly patients should be emphasized on adopting different treatment methods, treatment intensity, treatment plan and treatment goals, on the basis of atherosclerotic cardiovascular disease risk, in order to ensure the effectiveness and safety of treatment.

59 Research progress of acupuncture applied in the postoperative analgesia of total knee arthroplasty

In recent years, the total knee arthroplasty (TKA) has been widely used in patients with fracture in the Department of Orthopedics, the purpose of TKA is relieving pain of knee joint and reconstruction of knee joint function, but some patients still have abnormal pain after operation. In the past, multimodal analgesia, such as intravenous analgesia, epidural analgesia and continuous peripheral nerve block, is used for TAK analgesia, but all of them have certain disadvantages. In recent years, clinical studies have found that acupuncture analgesia in the treatment of postoperative pain of TKA in both domestic and international is gradually getting wide attention and recognition, and it has achieved good effects. As an adjuvant therapy of clinical surgery analgesia, acupuncture and related technologies play an important role in relieving the postoperative pain of patients. This paper summarizes the cause of postoperative pain of TKA, common traditional analgesic methods during perioperative TKA and its mechanism of acupuncture analgesia, the analgesic effect of acupuncture analgesia for postoperative TKA, aimed at providing reference for performing a new analgesic program in clinic.

63 The influence of demographic factors on the quality of life of HIV/AIDS patients

Objective To analyze the impact of demographic factors on life quality of HIV/AIDS infected people, in order to improve the life quality of patients. Methods 500 cases of HIV/AIDS infection patients were randomly selected from HIV/AIDS patients who were hospitalized in Love Home, Beijing You'an Hospital, Capital Medical University by means of simple random sampling method. A questionnaire survey was conducted among 500 individuals with HIV/AIDS. Their QOL were assessed by Chinese version of SF-36, and demographic features related to QOL were also analyzed. Single factor analysis and multivariate analysis of variance were used to analyze the demographic characteristics that might affect the quality of life. Results 423 HIV/AIDS patients were enrolled successfully. The average scores of PF, SF, RP, BP, MH,RE, VT, GH and total quality of life was (93.02±10.97), (49.18±10.85), (57.53±30.82), (93.21±15.43), (61.74±11.51), (58.05±33.76), (57.29±9.69), (52.36±7.66), (522.38±73.55) points respectively. Single factor analysis showed that physiology (F = 8.346, P = 0.015), mental health (F = 7.190, P = 0.027) and overall health (F = 7.127, P =0.028) were obviously affected by education. Physical function (F = 6.363, P =0.012), physical function (F = 11.961, P = 0.001), physical pain (F = 62.257, P =0.000), mental health (F = 85.943, P =0.000), viability (F = 15.497, P =0.000), overall health (F = 27.787, P =0.000) were significantly different between the different payment methods. Multivariate analysis showed that the degree of education had a significant effect on physiological function, physical pain, while health care had an effect on viability (P < 0.05). Conclusion Education level and medical expenses have significant impacts on the quality of life of HIV/AIDS patients, thereby strengthening the knowledge of HIV/AIDS patients and improving health care benefits will help improve their quality of life.

68 Effect of family centered health education on family function and self-efficacy for patients with type 2 diabetes mellitus

Objective To explore the effect of family centered health education on family function and self-efficacy for patients with type 2 diabetes mellitus. Methods 80 cases with type 2 diabetes mellitus admitted to the First People's Hospital Affiliated to Shanghai Jiaotong University during January to December 2014 were divided into experimental group and control group, 40 cases in each group. The patients in control group were received routine diabetic health education, and the patients and their direct caregivers in experimental group were received family centered health education. Diabetes knowledge level, family function, self-efficacy and glycosylated hemoglobin levels between two groups were compared. Results After intervention, before and after intervention, the diabetic knowledge score, glycosylated hemoglobin levels between two groups were compared, the differences were statistically significant (P < 0.05). Problem solving, behavior control scores of experimental group before and after intervention were compared, the differences were statistically significant (P < 0.05). Problem solving, communication, role, behavior control scores after intervention of 3 months between two groups were compared, the differences were statistically significant (P < 0.05). Problem solving, communication, role, emotional response, emotional intervention, behavior control scores after intervention of 6 months between two groups were compared, the differences were statistically significant (P < 0.05). There was no significant difference in self-efficacy score between two groups before and after intervention (P > 0.05). The self-efficacy scores were statistically significant between two groups before and after intervention (P < 0.05). Conclusion It is beneficial to improve the knowledge level, family function and blood glucose level, applying family centered health education for the patients with type 2 diabetes mellitus.

72 Correlation study of ischemia modified albumin, glycosylated albumin and glycated hemoglobin A1c in patients with type 2 diabetes mellitus and retinopathy

Objective To explore risk factors and correlation of ischemia modified albumin (IMA), glycosylated albumin (GA) and glycated hemoglobin A1c (HbA1c) on patients with type 2 diabetes mellitus and retinopathy. Methods 226 patients with type 2 diabetes mellitus in Endocrinology Department of the First Central Hospital of Baoding City in Hebei Province ("our hospital" for short ) from May 2015 to March 2016 were recruited. Among them, 144 patients without retinopathy as non-retinopathy group, 82 patients with retinopathy as retinopathy group. During the same period, 120 cases of health examination were collected as healthy examination group. The levels of IMA, GA, HbA1c were examined and analyzed. Results The levels of IMA, GA, HbA1c in non-retinopathy group and retinopathy group were significantly higher than those in healthy examination group, the differences were statistically significant (P < 0.05). The levels of IMA, GA, HbA1c in retinopathy group were significantly higher than those in non-retinopathy group, the differences were statistically significant (P < 0.05). Age is an important risk factor for type 2 diabetes mellitus with retinopathy, the morbidity increased with age. Logistic regression analysis showed that GA and HbA1c were independent risk factors for retinopathy in patients with type 2 diabetes mellitus, and IMA was not an independent risk factor for diabetic retinopathy. Conclusion The levels of IMA, GA and HbA1c increasing are risk factors of patients with type 2 diabetes mellitus combined with retinopathy. Routinely testing these indicators can help do earlier intervention and treatment in patients with diabetic retinopathy.

76 Diagnostic value of sex hormone, prostate specific antigen and relevant parameters in prostate cancer

Objective To explore serum sex hormone T, FSH, LH, E2, P, PRL, PSA levels and evaluate the diagnostic value of the patients with prostate cancer (PCa). Methods 30 patients with PCa (PCa group), 45 cases of benign prostatic hyperplasia (BPH group) and 45 medical normal cases (control group) were selected to examine the six indicators of sex hormone(T, FSH, LH, E2, P, PRL) and PSA levels from the Affiliated Hospital of Chengde Medical College from October 2012 to December 2016. The diagnostic value of each parameter to detect PCa was assessed by the receiver operating characteristic (ROC) curve. Results Compared with the BPH group and control group, the serum T level in PCa group was significantly decreased, while FSH and LH were significantly increased (P < 0.05), E2, P, PRL levels were no significant differences (P > 0.05). Compared with the control group, the various sex hormone levels in BPH group were no significant differences (P > 0.05). Compared with the control group, the serum total PSA (TPSA), free PSA (FPSA) and PSA density (PSAD) in PCa group and BPH group were significantly increased, while F/T was significantly decreased (P < 0.05). Compared with the BPH group, the serum TPSA, FPSA and PSAD in patients with PCa group were significantly increased, while F/T was significantly decreased (P < 0.05). The areas under the ROC curves (AROC) of TPSA, FPSA, PSAD, T, FSH and LH were 1.000, 0.894, 0.991, 0.920, 0.753, 0.868. The testing parameters AROC comparison from big to small for TPSA, PSAD, T, FPSA, LH and FSH, differences between TPSA and PSAD, FPSA and T, FPSA and LH, PSAD and T, T and LH, FSH and LH were no statistically significance (P > 0.05). The remaining differences between paired were statistically significant (P < 0.05). Conclusion The serum T level in patients with PCa is decreased but FSH and LH levels are increased. The combined detection of sex hormone and serum PSA is a valuable early diagnostic tool for PCa.

80 Clinical observation of total resection of bladder tumor with transurethral fluoroscopy

Objective To investigate the clinical method of transurethral resection of bladder tumor with cystoscopy in the treatment of bladder tumor. Methods From January 2014 to December 2015, 40 patients with bladder cancer who were treated at Xuzhou Central Hospital were observed. All the patients were treated with sacral obstruction anesthesia. 45 cases of tumor were removed, including 36 cases of tumor were single tumor, and the tumor diameter was 0.5~2 cm, which were located in the posterior wall of the bladder or the location of the neck. Firstly, the tumor size and location of the bladder were observed by fluorescent cystoscopy. Then the bladder wall full of bladder fat was cut by the plasma along the tumor proximal, along the tumor in full-thickness resection, the final resection of the tumor distal complete resection of the tumor and its basal bladder full-thickness pathology. Results 40 patients were successfully completed operation, and the operation time was 10-40 min, the mean time was (23.95±8.39) min. The intraoperative blood loss was very few, and there was no secondary bleeding after operation. 34 cases were postoperative tumor stage T1 and 6 cases were postoperative tumor stage T2. Patients were followed up 12-24 months after operation, the mean time was (14.89±1.97) months, no tumor case was recurred. Conclusion Transurethral resection of bladder tumor under cystoscopy can completely remove the tumor and tumor base, which can prevent the removal of the suspected bladder wall under fluorescence, and the surgery effect is exact.

83 Evaluation of surgical treatment of ureterocele complicated with stone in adult

Objective To evaluate the efficacy of surgical treatment for the ureterocele complicated with stone in adults. Methods The clinical data of 10 ureterocele patients complicated with stone, that were admitted in Beijing Jishuitan Hospital from January 2010 to January 2016, were retrospective analyzed. There were 8 males and 2 females in the study. The patient age ranged from 22 to 50 years old and the mean age was 40 years old. 8 cases were unilateral ureterocele and 2 patient was bilateral. 6 patients received transurethral resection of ureterocele and endoscopic stone removal with nephroscope. The other 4 patients received transurethral incision of ureterocele and endoscopic stone removal with urteroscope. Results The operations were successful in this group of patients. The mean operation time was 30 minutes. The average postoperative hospital stay was 3 days. Patients were followed-up regularly, urine test, B ultrasound and cystography were done every three months. Urinary CT and cystoscopy were performed each year. Preoperative symptoms that of complained by 5 patients disappeared 2 weeks after the operation. There hydronephrosis were significantly reduced 3 months after the operation, with no vesicoureteral reflux shown by the cystography. Conclusion Transurethral resection or incision of ureterocele and endoscopic stone removal are an effective procedure for the treatment of ureterocele complicated with stone in adults.

86 Efficacy evaluation of two minimally invasive procedures for the treatment of varicocele

Objective To investigate the efficacy of two minimally invasive procedures for the treatment of varicocele. Methods From June 2014 to June 2016, 80 patients with varicocele at the 105th Hospital of PLA were randomly divided into laparoscopic group and microscope group according to random number table, witn 40 cases in each group, and treated by microscopic and laparoscopic varicocelectomy respectively. The semen parameters before and after surgery, operation time, intraoperative blood loss, postoperative hospital stay and postoperative complications and recurrence rate were measured respectively. Results Semen quality was significantly increased in both groups after surgery, and the improvement of semen quality was better than that of the laparoscopic group (P < 0.05). The operation time of the microscope group was significantly longer than that of the laparoscopic group, with statistically significant difference (P < 0.05). There was no significant difference in intraoperative blood loss and postoperative hospital stay (P > 0.05). The incidence of postoperative omplications in the microscope group was significantly lower than that in the laparoscopic group, with statistically significant difference (P < 0.05). Conclusion Microscopic versus laparoscopic varicocelectomy are safe and effective ways to treat varicocele. Microscopic varicocelectomy is superior to laparoscopic varicocelectomy for its significantly improved semen quality and lower rates of postoperative complications.

90 Outcomes of unilateral fixation on lumbar spinal stenosis

Objective To compare the clinical outcomes of unilateral versus bilateral pedicle screw fixation with transforaminal lumbar interbody fusion in lumbar stenosis. Methods From February 2015 to February 2016, in Zhuhai Hospital of Guangdong Hospital of Triditional Chinese Medicine, 60 lumbar spinal stenosis patients were selected, according to random number table, they were divided into two groups, treatment group (30 cases) were given unilateral versus bilateral pedicle screw fixation, and control group (30 cases) were given transforaminal lumbar interbody fusion. Clinical outcomes after surgery were evaluate by JOA and VAS, meanwhile, the lumbar spine lordosis angle (LL) and the pelvic incidence Angle (PI) were measured before and after surgery. Results JOA and VAS of two groups at same time were compared, the differences were not statistically significant (P > 0.05); JOA and VAS of two groups before surgery were compared with those after surgery, after surgery 3, 6, 12 mooths, the differences were statistically significant (P < 0.05). PI, LL of two groups at same time were compared, the differences were not statistically significant (P > 0.05); PI, LL of two groups before surgery were compared with those after surgery, after surgery 3, 6, 12 mooths, the differences were not statistically significant (P > 0.05). Conclusion Compared with ransforaminal lumbar interbody fusion, unilateral pedicle screw fixation has the same safe and effective outcome.

Objective To investigate the feasibility and superiority of minimally invasive in treatment of pelvic fractures by 3D assisted lateral rectus incision. Methods From September 2015 to October 2016, in Maoming Chinese Medicine Hospital, 15 patients with minimally invasive treatment of pelvic fractures with 3D assisted lateral rectus abdominis incision were selected as 3D group, another 15 patients underwent traditional surgery in the treatment of pelvic fracture patients were selected as control group, operation time, intraoperative blood loss, hospitalization, and time the complications of fracture reduction quality of two groups were observed. Results The operation time of 3D group was significantly less than that of control group, the difference was statistically significant (P < 0.05). The blood loss of 3D group was significantly less than that of control group, the difference was statistically significant (P < 0.05). The incidence of postoperative complications of 3D group was significantly less than that of control group, the difference was statistically significant (P < 0.05). The level of pain of 3D group was significantly lower than that of control group, the difference was statistically significant (P < 0.05). Conclusion The application of 3D printing auxiliary lateral rectus approach for minimally invasive treatment of pelvic fracture is feasible, with shorter operation time, it is worthy of promotion and application.

98 Significances of changes of matrix metalloproteinases-13, TNF-α in serums and synovial fluids of patients with knee osteoarthritis

Objective To explore the changes of matrix metalloproteinases 13 (MMP-13), TNF-α in serums and synovial fluids of patients with knee osteoarthritis (KOA). Methods From May 2014 to May 2016, in Traditional Chinese Medicine Hospitals of Chengde, 84 patients with KOA and 80 healthy persons (control group) were selected, according to Kellgren & Lawrence (K-L) grading, they were divided into KOA 1~2 (n = 45) and KOA 3~4 (n = 39). the levels of TNF-α, MMP-13 in the serums and synovial fluids were measures by ELISA. Results The WOMAC score of KOA 3~4 patients was higher than KOA1~2 patients, the difference was statistically significant (P < 0.05); the levels of TNF-α, MMP-13 in serum and synovial fluid of KOA 3~4 patients were higher than KOA 1~2 patients, the differences were statistically significant (P < 0.05). the level of MMP-13 in serum and synovial fluid of KOA 1~2 patients were higher than control group, the differences were statistically significant (P < 0.05), but there was no statistically significant difference in the TNF-α levels (P > 0.05). The level of MMP-13 in serum of KOA patients was higher than synovial fluid, the difference was statistically significant (P < 0.05), but there was no statistically significant difference in TNF-α levels (P > 0.05). Correlation analysis showed that the serum and synovial fluid levels of MMP-13 in KOA patients were positively correlated with WOMAC score (r = 0.599, 0.641, P < 0.05), but the serum and synovial fluid levels of TNF-α in KOA patients were no correlated with WOMAC score (P > 0.05). Conclusion The serum and synovial fluids levels of TNF-α and MMP-13 elevate in KOA patients, and then MMP-13 levels show correlation with WOMAC scores.

Objective To compare the effect of treatment agedness unstable fracture between femur rotor by proximal screw intramedullary nail and proximal locking plate. Methods From February 2011 to January 2016, in Ningde Hospital Affiliated to Fujian Medical University, clinical data of agedness unstable fracture between femur rotor patients (120 cases) were retrospectively analyzed, they were divided into two groups by different treatment methods, proximal screw intramedullary nail group (60 cases) and proximal locking plate group (60 cases). The incision length, operation time, intraoperative blood loss, load time after operation, hospital stay, fracture healing time, functional recovery effect after operation, postoperative complications of agedness unstable fracture between femur rotor patients in two groups were detected. Results The The incision length, operation time, intraoperative blood loss, load time after operation, hospital stay, fracture healing time of proximal screw intramedullary nail group were lower than proximal locking plate group, the functional recovery good rate after operation of proximal screw intramedullary nail group were higher than proximal locking plate group, the incision infection, LDVT, big rotor pain caused discomfort, hip varus of proximal screw intramedullary nail group were lower than proximal locking plate group, the differences were statistically significant (P < 0.05). Conclusion The treatment agedness unstable fracture between femur rotor by proximal screw intramedullary nail, trauma is little, curative effect is good, complication is little, it is worth to be used.

106 Effect of comprehensive intervention on improving medical fear of school-age children

Objective To explore the role of comprehensive intervention in improving medical fear in hospitalized children. Methods From July to December in 2016, 240 cases hospitalized children in Affiliated Hospital of North Sichuan Medical College were selected and divided into non-intervention group and intervention group according to random number table, with 120 cases in each group. The children in the non-intervention group were stayed in public ward, and the children in the intervention group were stayed in the transformed fairyland ward. Besides, the children in the intervention group were given to comprehensive intervention. The scores of CMFS, length of stay and hospitalization costs of the two groups were compared. Results In the CMFS, the average scores of the four dimensions selected from highest to lowest were the medical environment fear, interpersonal fear, medical operation fear and self fear. The scores of CMFS, length of stay and hospitalization costs of the intervention group were shorter than those of the non-intervention group (P < 0.05). Conclusion Comprehensive intervention can decrease the medical fear of hospitalized school-age children, reduce length of stay and hospitalization costs. It is worthy of clinical application.

109 Epidemiological analysis on virus etiology of acute respiratory infection from 2014 to 2016 in Wenling

Objective To investigate the epidemiological characteristics of influenza virus A (IVA), influenza virus B (IVB), respiratory syncytial virus (RSV), adenovirus (ADV), parainfluenza virus type 1(PIV 1), PIV 2 and PIV 3 in children of Wenling, so as to provide the data for the clinical diagnosis, treatment, epidemic surveillance, and preventive strategy. Methods From January 2014 to December 2016, Nasopharyngeal swab samples from 1826 children presenting influenza-like symptoms in Wenling were collected, and the samples were determined by direct fluorescent antibody staining (DFA) for the 7 common respiratory viruses. The positive rates, epidemiological characteristics of different month and age were analyzed. Results There were 496 cases of positive virus specimens in the specimens, with the total positive rate of 27.16%. Among the seven kinds of virus, the highest infection rate was RSV, and the lowest infection rate was PIV 2. There was no significant difference in terms of the constituent ratio for seven kinds of virus among 1826 cases of infected children (including girls and boys) (P > 0.05). The positive ratio of viruses were detected highly in January, February, March, which was found the highest in February (48.6%) and the lowest in October (21.4%). There was statistical significance for the positive rates of RSV, PIV3, ADV, and PIV1 among 0~1, 1~3, 3~6, and beyond 6 age groups (P < 0.05). Conclusion RSV and PIV 3 are the most common virus among children with influenza-like symptoms in Wenling. The epidemic of respiratory viruses could be obvious seasonal characteristics, and the highest rate of respiratory infection might be in February children under 3 years old are susceptible to these viruses.

113 Correlation between alveolar arterial oxygen pressure or D-dimer and pulmonary function in children with severe pneumonia

Objective To investigate the relationship between alveolar arterial oxygen partial pressure (PA-aO2) or plasma D-dimer (DD) and pulmonary function in children with severe pneumonia. Methods From January 2011 to December 2016, in Third Hospital of Xingtai, 95 children with severe pneumonia were selected as the observation group, and 50 healthy children were selected as control group at the same period. The first second forced expiratory volume (FEV1), forced vital capacity (FVC), peak expiratory flow (PEF) and maximal midexpiratory flow (MMF), PA-aO2 and plasma DD in health children and children with severe pneumonia in acute phase and 3, 6 months after discharge were monitored, and the correlation between PA-aO2, DD and the changes of pulmonary function indexes were analyzed. Results The pulmonary function indexes (FEV1, FVC, PEF, MMF) in children with severe pneumonia were significantly lower than the control group, the differences were statistically significant (P < 0.01), and the levels of PA-aO2 and DD was significantly higher than the control group, the differences were statistically significant (P < 0.01). The FEV1, FVC, PEF and MMF in children at 3, 6 months after discharge were higher than those in the acute phase, the differences were statistically significant (P < 0.01), PA-aO2 and plasma DD were lower than those in the acute stage, the differences were statistically significant (P < 0.01). The PA-aO2 in children with severe pneumonia showed a negatively correlation with FVC, FEV1, PEF and MMF (r = -0.459, -0.476, -0.475, -0.689, P < 0.05), and the DD level also showed a negatively correlation with FVC, FEV1, PEF and MMF (r = -0.481, -0.384, -0.463, -0.693, P < 0.05). Conclusion PA-aO2 and DD in children with severe pneumonia has significant correlation with the degree of pulmonary function impairment. It has important value in the assessment, treatment and prognosis of children with severe pneumonia.

117 Clinical significance of detection of serum hepatitis B virus transcription in maternal and infant vertical transmission

Objective To explore the clinical significance of the test of serum hepatitis B virus transcription in maternal-infant vertical transmission. Methods From October 2014 to October 2016, 40 maternity patients with positive-HBV surface antigen (HBsAg) and their newborns in 302th Hospital of PLA were selected. All the patients divided into HBeAg positive group (21 cases) and HBeAg negative group (19 cases) according to the HBeAg negative/positive. 2 mL maternal peripheral blood and newborn navel blood were collected, the RNA was extracted, then the HBx DNA, fRNA, trRNA were got through reverse transcription and RT-PCR amplification, the products were showed by agarose gel electrophoresis. Results The positive rates of HBx DNA and fRNA of maternity patients in the HBeAg positive group were higher than those in the HBeAg negative groups, with statistically significant difference (P < 0.05). Then the cases were divided into HBx DNA positive group and HBx DNA negative group according to the HBx DNA negative/positive. The positive rates of fRNA and trRNA of maternity patients and trRNA of newborns in the HBx DNA positive group were higher than those in the HBx DNA negative groups, with statistically significant difference (P < 0.05). Conclusion TrRNA appeared in the process of mother-to-child transmission of HBV is earlier than fRNA, HBx DNA, which is helpful to determine neonatal HBV infection status, and has a certain auxiliary diagnostic value.

121 Efficacy of Tranexamic Acid in treatment of irregular uterine bleeding caused by Depot-medroxyprogesterone acetat

Objective To explore the efficacy and safety of Tranexamic Acid in treating irregular uterine bleeding secondary to Depot-medroxyprogesterone acetat. Methods From December 2015 to December 2016, in First Affiliated Hospital of Chinese Medical Sciences University, 80 patients with irregular uterine bleeding secondary to Depot-medroxyprogesterone acetat treatment were recruited. According to random number table, they were divided into treatment group and placebo group with 40 cases in each group, and they were treated with Tranexamic Acid and placebo respectively. All patients were followed up for 4 weeks, the outcomes were evaluated by the number of blood termination within 7 days and 28-day bleeding-free days and side effects reports were observed by information table. Results One case of the placebo group lost during follow-up, 33 cases (82.5%) in the treatment group and 10 cases (25.6%) in the placebo group stopped bleeding within 7 days after treatment initiation, the difference was statistically significant (P < 0.01). The 28-day bleeding-free proportion  in treatment group  was (20.73±4.07) d, that  in the placebo group was (11.90±5.78) d, the difference was statistically significant (P < 0.01). One case of mild nausea was reported. Conclusion Tranexamic Acid is an effective and safe choice in dealing with irregular uterine bleeding secondary to Depot-medroxyprogesterone acetat treatment.

124 Effects of weight changes during pregnancy of gestational diabetes mellitus on neonatal birth weight

Objective To analyze the effects of the weight changes during pregnancy of gestational diabetes mellitus on neonatal birth weight. Methods From January to December 2016, in Tongzhou Maternal and Child Health Hospital of Beijing, the dates of 1240 pregnant woman with gestational diabetes mellitus and complete single birth and complete medical records were retrospectively. They were divided into(BMI <18.5 kg/m2, groupⅠ), normal body weight group (BMI 18.5-<24.0 kg/m2,  groupⅡ),overweight group (BMI 24.0-<28.0 kg/m2, groupⅢ), obese (group B), obesity group (BMI≥28.0 kg/m2, group Ⅳ). They were divided into groupⅠ+Ⅱ and group Ⅲ+Ⅳ, because of the low proportion of low weight group. The effects of the weight changes of gestational diabetes mellitus on neonatal birth weight in two groups were compared. Results In groupⅠ+Ⅱ and group Ⅲ+Ⅳ, the weight [(61.41±4.34) vs (70.41±3.90) kg], pre-pregnancy BMI [(22.34±3.19) vs (26.79±3.72)kg/m2], body weight gain during pregnancy [(16.08±5.42) vs (12.55±6.59) kg], neonatal body weight [(3414.2±460.7) vs (3480.1±542.7) g] were compared, the differences were statistically significant (P < 0.05). In groupⅠ+Ⅱpregnancy weight gained≥20 kg, the risk of LGA increased (P < 0.05); pregnancy weight gained 2.5-< 10.0 kg, SGA increased risk (P < 0.05); in group Ⅲ+Ⅳ: gestational weight gained≥12.5 kg, LGA increased risk (P < 0.05); pregnancy weight gained 2.5-<7.5 kg, SGA increased risk (P < 0.05). Conclusion In the management of gestational diabetes, blood glucose should be monitored at the same time, it should pay attention to the management of its weight, the weight of the changes in the weight of newborns have a significant impact on low birth weight and normal weight of gestational diabetes maternal pregnancy weight gain should be controlled at 10-20 kg, for pre-pregnancy overweight and obese gestational diabetes mellitus pregnancy weight gain should be controlled at 7.5-12.5 kg.

127 The effect of Dopamine and Dobutamine in the treatment of children with severe pneumonia and its effects on cytokines

Objective To analyze the effect of Dopamine combined with Dobutamine in the treatment of children with severe pneumonia. Methods From January 2015 to March 2016, 94 children with severe pneumonia treated in Yantai Zhifu Hospital were selected. According to the randomized double blinded principle, they were divided into control group and observation group, with 47 cases in each group. The children in the two groups were received the same conventional treatment, and the observation group was given to Dopamine and Dobutamine based on the conventional treatment. The clinical effect and the inflammatory cytokine levels of the two groups were compared. Results After treatment, the levels of IL-6, IL-8, soluble cell adhesion molecule -1, C reactive protein and tumor necrosis factor alpha in the two groups were decreased (P < 0.05); and the inflammatory cytokine levels in the observation group were lower than those in the control group (P < 0.05). The time of symptoms and signs improved in the observation group was shorter than that in the control group (P < 0.05). The total efficiency of the observation group was higher than that of the control group (P < 0.05). Conclusion Dopamine combined with Dobutamine can reduce the of inflammatory cytokines levels, and improve the curative effect of children with severe pneumonia.

131 Effect of bandage contact lens on pain and corneal epithelium healing condition after pterygium excision combined with autologous conjunctival flap graft transplantation

Objective To assess the effect of bandage contact lens on the degree of pain and corneal epithelium healing condition after pterygium excision combined with autologous conjunctival flap graft transplantation. Methods From July 2015 to July 2016, in the First Affiliated Hospital of Huzhou University, 116 patients with pterygium were divided into two groups, according to using bandage contact lens or not, group A (60 cases, 60 eyes) was given bandage contact lens at 1 week after surgrey, group B (56 cases, 56 eyes) was not given bandage contact lens. The degree of pain after surgery was evaluated by visual analogue score (VAS) between  two groups at 2 h, 1 d and 1 week after surgery. The healing status of corneal epithelium was observed between the two groups at 2 h, 1 d and 1 week after surgery by fluorescent staining. Results VAS 2 h, 1 d and 1 week after surgery in group A were compared with group B, the differences were statistically significant (P < 0.01). The score of corneal epithelium healing condition at 2 h after surgery of group A and group B were compared, the differences were not statistically significant (P > 0.05). The scores of corneal epithelium healing condition at 1 d and 1 week of two groups were compared, the differences were statistically significant (P < 0.01). Conclusion Bandage contact lenses can significantly release pain response and promote the healing status of corneal epithelium after pterygium excision combined with autologous conjunctival flap graft transplantation.

135 Research on the equity of health resource allocation in Jiangsu Province

Objective To analyze the equity of health resource allocation in Jiangsu Province, find existing problems, and put forward corresponding countermeasures. Methods The changes of health resource allocation from 2011 to 2015 in Jiangsu Province were statistically described. The equity of health resources allocation in population was analyzed with Theil index, and the contribution rate among different regions and areas was calculated. Results The amount of health resources per capita increased year by year, but gaps among different areas were existed. The north of Jiangsu had more serious internal problems. The equity of health resource allocation was improved overall, but the human resource allocation was obviously unfair. Conclusion To increase health investment, and focus on reversing the unbalance of doctor-nurse ratio; establish the reasonable talent flow mechanism, and improve equity of human resource allocation; develop the economy in north of Jiangsu, provide financial support for the health resources; adjust and optimize the pattern of resource allocation, and promote the balance between supply and demand.

139 Determination of ten illegal dyes in Gejie Dingchuan Pills

Objective To establish a quantification method of HPLC for determining illegally added auramine O, lemon yellow, carmine, sunset yellow, brilliant yellow, acid red, methyl orange, orange Ⅱ, etythrosine and alkaline orange in Gejie Dingchuan Pills, and validate with LC-MS/MS method. Methods The sample was extracted by 50% ethanol, and determined by HPLC with a gradient elution of acetonitrile-0.01 mol/L ammonium acetate (contain 0.5% glacial acetic acid) and the detection wavelength was set at 420 nm and 500 nm. The identification was performed by LC-MS/MS, using a gradient elution of acetonitrile-0.01 mol/L ammonium acetate. The analytes were detected by an electro spray ionization tandem mass spectrometry with dynamic MRM. Qualitation of the auramine O based on the retention time and mass charge ratio. Results The calibration curve of ten dyes showed good linearity and correlation coefficients were all above 0.999, the limits of quantitation for ten dyes in samples were between 0.3-2.8 μg/g, and the average recoveries ranged from 96.3% to 101.1% with RSD of 0.6%-1.5%. Auramine O could be detected in 33 of 123 samples. Conclusion LC-MS/MS method is accurate and reliable for the qualitative identification and the HPLC method is simple, rapid and accurate for the content determination. The combination of HPLC and LC-MS/MS can detect ten dyes in Gejie Dingchuan Pills rapidly, accurately and sensitively.

144 Study on quality standard of Fufang Suanzaoren Granules

Objective To establish the quality standard for Fufang Suanzaoren Granules. Methods Ziziphi spinosae semen, Anemarrhenae rhizome, Glycyrrhizae radix et rhizoma were identified by TLC. The contents of mangiferin were determined by HPLC. HPLC method on mangiferin was adopted on a Welchrom C18 (4.6 mm×200 mm, 5 μm) column with Methanol-acetonitrile-0.5% acetic acid (7∶7∶86) as the mobile phase, the flow rate was 1.0 mL/min, the detection wavelength was 258 nm, the column temperature was 30℃ and the injection volum was 10 μL. Results TLC spots of Ziziphi spinosae semen, Anemarrhenae rhizome, Glycyrrhizae radix et rhizoma were clear with good resolution and without interference of negative samples. There was a good linearity within the range of 1.072-167.5 μg/mL, with the linear equation of Y=42 888X-30 400, r = 0.999 9. The average recovery of berberine hydrochloride was 99.15% and the RSD was 2.82% (n = 6). The content of Mangiferin in three batchs samples were satisfactory. Conclusion The method is simple, accurate and reproducible. It can be used as the quality control method of Fufang Suanzaoren Granules.

148 Effect of Infliximab in the treatment of ulcerative colitis and its influence on the levels of inflammatory factors

Objective To investigate the effects of Infliximab in the treatment of ulcerative colitis and its influence on the levels of inflammatory factors. Methods Two hundred and forty patients with moderate ulcerative colitis treated in Shengjing Hospital Affiliated to China Medical University from July 2013 to October 2015 were selected and randomly divided into observation group and control group, with 120 cases in each group. The control group was given routine drug Azathioprine, on basis of which, the observation group was added with Infliximab, continuous treatment for 30 d. The clinical efficacy of the two groups was observed, and the levels of serum C-reactive protein (CRP), erythrocyte sedimentation rate (ESR), albumin (Alb) hemoglobin (Hb) and inflammatory factors before and after treatment between the two groups were compared. Results The total effective rate of observation group (80.83%) was significantly higher than that of the control group (61.67%), the difference was statistically significant (P < 0.05). After treatment, the levels of CRP, ESR, Alb, Hb in the two groups were all improved compared with those before treatment, the differences were all statistically significant (P < 0.05), and the levels of CRP and ESR in the observation group were lower than those of the control group, while the levels of Alb and Hb were higher than those of the control group, the differences were all statistically significant (P < 0.05). The levels of interleukin-6 (IL-6), interleukin-8 (IL-8), interleukin-23 (IL-23) and tumor necrosis factor alpha (TNF-α) in the two groups after treatment were all significantly lower than those before treatment, the differences were statistically significant (P < 0.05), and the levels of IL-6, IL-8, IL-23 and TNF-α in the observation group after treatment were lower than those of control group, there were statistically significant differences (P < 0.05). Conclusion Infliximab in the treatment of severe ulcerative colitis has exact effects, which can effectively improve the patient's intestinal mucosa, and it has important significance for decreasing the level of inflammatory factors, it is conducive for the prognosis of patients and worthy of clinical promotion and application.

152 Effect of Salbutamol Aerosol combined with hormone in the treatment of asthma

Objective To investigate the clinical efficacy of Salbutamol Aerosol combined with hormone in the treatment of asthma. Methods Ninety-four patients with bronchial asthma admitted to the First Affiliated Hospital of Dali University from March 2015 to March 2016 were selected as research objects, and they were divided into observation group (47 cases) and control group (47 cases) by random number table method. The patients of two groups were all treated with conventional treatment, including reducing phlegm, relieving cough, anti-inflammation and others. The control group was treated with Aminophylline Tablets 0.2 g, 2 times/d; Budesonide Aerosol, aerosolized inhalation, once 10 min, 3 times/d. On basis of control group, the observation group was treated with Salbutamol Aerosol, aerosolized inhalation, once 10 min, 3 times/d. Both groups were treated for 2 weeks. The clinical efficacy of the two groups after treatment was observed, the levels of lung function and the changes of C-reactive protein (CRP), tumor necrosis factor-α (TNF-α), transforming growth factor-β1 (TGF-β1) and other indexes before and after treatment between the two groups were compared. Results The total effective rate of observation group was 93.6%, which of control group was 76.6%, the difference was statistically significant (χ2=5.371, P < 0.05). After treatment, the forced expiratory volume in the first second (FEV1) and the percentage of forced expiratory volume in the first second occupied in the vital capacity (FEV1/FVC) in the observation group were all higher than those of control group, the differences were all statistically significant (t = 5.214, 6.104, all P < 0.05). After treatment, CRP, TNF-α, TGF-β1 in the observation group were all lower than those of control group, the differences were all statistically significant (t = 22.383, 4.510, 18.996, all P < 0.05). Conclusion β-receptor agonist Salbutamol combined with hormone Budesonide in the treatment of patients with asthma has exact effect, which can improve the lung function of patients, reduce the inflammatory reaction, so it can be as a reference for the clinical treatment.

This paper analyzes the understanding of western medicine on bone hyperplasia and its deficiency, leading to a new understanding of acupotomy for bone hyperplasia, talks about the etiology, pathological mechanism and the process of bone hyperplasia, analyzes the relationship between the location of bone hyperplasia and biomechanics, the it comes to conclusion that the bone hyperplasia is the result of chronic soft tissue injury, and all of these are the result of second sets of self repair and compensation in order to compensate the abnormal stress of soft tissue. So the author holds the view that hyperosteogeny is unresectable, the method for treating hyperosteogeny is mainly through the combination of acupotome lysis of the string, adhesion of soft tissue of stress site, scar, contracture, release of local abnormal stress, restore the body bowstringmechanics anatomical system of force balance. If the human body's compensatory ability, through the needle knife therapy, bone hyperplasia can gradually absorb or even disappear.

159 Clinical study of acupotomy therapy on the calcaneodynia with abnormal plantar pressure

Objective To explore the effect of specific acupotomy therapy on mechanical changes of plantar in patients with calcaneodynia, and to provide theoretical basis for specific acupotomy therapy. Methods Thirty patients diagnosed with unilateral calcaneodynia in Orthopedics and Traumatology of Hospital of Integrated Chinese and Western Medicine, Southern Medical University from June 2015 to June 2016 were included in the this experiment. The abnormal area of plantar pressure in each patient was detected, and the abnormal area of plantar pressure was treated by acupotomy therapy. The peak force of 10 zones of affected pelma [the 1st base of phalanx (T1), the 2nd-5th base of phalanx (T2-T5), 1th metatarsal (M1), 2nd metatarsal (M2), 3rd metatarsal (M3), 4th metatarsal (M4), 5th metatarsal (M5), middle foot bottom (MID), heel lateral (HL) and heel medial (HM)] was tested with Footscan USB2 system before treatment and after treatment for 2 weeks. Results Before treatment, the peak pressure of M1, HL and HM between the affected side and normal side was significantly different (P < 0.05). After treatment for 2 weeks, the plantar pressure of M1, HL and HM area was significantly decreased (P < 0.05), but there were no significant differences in other zones (P > 0.05). Conclusion Acupotomy therapy for the abnormal area of plantar pressure in patients with calcaneodynia can improve the plantar pressure of abnormal area, and make the plantar pressure become normal. Clinically, we may try to treat the patients' plantar pressure in abnormal area with acupotomy therapy, and it may achieve the purpose of treating the calcaneodynia.

163 The randomized controlled study of acupotomy in treating cervical spinal hypertension

Objective To observe the clinical efficacy of acupotomy in treating cervical spinal hypertension, and to investigate the pathogenesis of it. Methods Sixty-two cases of patients diagnosed as cervical hypertension in General Hospital of Armed Police Forces from February 2014 to December 2016 were collected and divided into two groups in accordance with the random number table, each group had 31 cases. The experimental group was treated with the acupotomy therapy every three days, 2 times a week, for 8 times. The control group was treated with acupuncture, once a day, 7 times were as a course, total for 4 courses. The changes of blood pressure of the two groups before and after treatment were observed, and the clinical efficacy of the two groups was evaluated. Results There were no significant differences of systolic and diastolic blood pressure between the two groups before treatment (P > 0.05). The systolic blood pressure and diastolic blood pressure in the two groups after treatment were significantly lower than those before treatment (P < 0.01), and the systolic blood pressure and diastolic blood pressure in the experimental group were significantly lower than those in the control group (P < 0.01). The total effective rate of the experimental group was higher than that of the control group, the difference was statistically significant (P < 0.05). Conclusion The treatment of cervical hypertensive disease by manipulation with acupotomy for cervical soft tissue can significantly release the blood pressure and cervical symptoms, the curative effect is definite.

167 A case analysis of a long-term oral taking of Warfarin and literature review of perioperative anticoagulation therapy

Clinical pharmacists and clinicians help to analyze and develop practical strategy for 1 case of patient with iron deficiency anemia and high blood pressure, and with long-term oral taking history of Warfarin after heart valve replacement surgery. During the process of operation, no obvious bleeding happened, no thromboembolism and bleeding occured, and high blood pressure is stable after operation. Patient has discharged within a week, followed up to 9 months, the patient is in stable condition and has no hemorrhage symptom. According to review of the literatures, the choice of Warfarin in perioperative period is analyzed.

174 Changes and clinical significance of HIF-1α, MBP and hs-CRP in patients with diabetic peripheral neuropathy

Objective To detect the levels of serum hypoxia inducible factor-1α (HIF-1α), myelin basic protein (MBP), high sensitive C reactive protein (hs-CRP) and oxidative stress response in patients with diabetic peripheral neuropathy, and analyze the correlation among them. Methods 90 cases of patients with diabetic peripheral neuropathy treated in the Fifth Atfiliated Hospital of Xinjiang Medical University ("our hospital" for short) from July 2015 to July 2016 were selected as the study group. 80 healthy cases matched by age and gender in our hospital at the same time were selected as the control group. General biochemical indexes, levls of serum HIF-1α, MBP, hs-CRP, oxidative stress levels of super oxide dismutase (SOD), reduced glutathione (GSH), and malonaldehyde (MDA) were detected and compared between the two gorups. The multivariate Logistic regression analysis was used to analyze the influence factors of diabetic peripheral neuropathy. Spearman correlation test was used to analyze the correlation among serum HIF-1α, MBP, hs-CRP and SOD, GSH, MDA in the study group. Results The levels of total cholesterol (TC), triglyceride (TG), fasting blood glucose (FPG), glycosylated hemoglobin (HbA1c), fasting insulin (FINS) in the study group were significantly higher than those in the control group (P < 0.05). The levers of serum HIF-1α, MBP and hs-CRP in the study group were significantly higher than those in the control group, the differences were statistically significant (P < 0.05). The levels of serum SOD and GSH in the study group were significantly lower than those in the control group (P < 0.05), and the serum MDA level in the study group was significantly higher than that in the control group (P < 0.05). The multivariate Logistic regression analysis showed that age, duration of diabetes, TC, TG, FPG, HbA1c, FINS, MBP, hs-CRP, HIF-1α, MDA were independent risk factors of diabetic peripheral neuropathy (P < 0.05), SOD and GSH were protective factors of diabetic peripheral neuropathy (P < 0.05). Spearman correlation analysis showed that serum HIF-1α, MBP and hs-CRP in study group were positively correlated with MDA level (r=0.547, 0.624, 0.685, P < 0.05), and which were negatively correlated with SOD and GSH levels (r=-0.565, -0.583, -0.592, r=-0.492, -0.551, -0.572, P < 0.05). Conclusion The higher levels of serum HIF-1α, MBP and hs-CRP levels and oxidative stress are involved in the occurrence and development of diabetic peripheral neuropathy, which can be taken as clinical diagnostic bio-markers in patients with diabetic peripheral neuropathy, and provide theoretical basises for clinical intervention.

179 The design and realization of psoriasis PASI scoring system based on the access database

This paper develops the psoriasis area and severity index (PASI) scoring system based on the access database. According to the international common used PASI evaluation method of psoriasis skin lesions area and severity degree, this system is designed. By using this system, doctors can easily input data and automatically calculate the score of PASI, which reduces the tedious procedures that doctor manually recorded data and calculated the score of PASI previously, improves the doctor′s work efficiency and accuracy. This system can also record and track the treatment status of psoriasis patients, improve the treatment efficiency and quality of psoriasis patients. This system has been tested and can run normally and stably.

183 Refinement of fine needle aspiration cytological morphologic criteria of papillary thyroid carcinoma

Objective To validate the efficiency of the cytological diagnostic criterion of papillary thyroid carcinoma (PTC) defined by of Beijing Friendship Hospital ("BFH" for short). Methods All thyroid fine needle aspiration (FNA) cases with histological results of PTC were reviewed from March 2015 to December 2016 by Nikiforov′s, Kini′s and BFH′s cytological diagnostic criterias of PTC. The sensitivities and specificities of 3 criterias were calculated with histological results as the gold standard. Results Total 339 thyroid FNA cases with histological results as control were reviewed, including 337 cases of PTC. With definite diagnosis as positive standard, the sensitivities of 3 kinds of PTC FNA cytological diagnostic criteria was 86.9% (219/337), 34.1% (115/337) and 65.0% (219/337), respectively. The specificities were all 100%. The sensitivity of BFH criterion increased 52.8% and 21.9% compared with Nikiforov and Kini criterion. Conclusion The BFH′s PTC cytologic diagnostic criterion has highly sensitive, specific and worthy of the recommendation.

187 Comprehensive exploration of associations between environmental factors and congenital diseases by bibliometric analysis

Objective To evaluate the characteristics and popular themes in the field of environmental pollution and congenital diseases by bibliometric analysis. Methods The database including all 4687 articles in the field of environmental pollution and congenital diseases was constructed by searching PubMed/Medline using MeSH terms. The bibliographic information was analyzed to summarize the overall research characteristics. MeSH terms were sorted by their normalized frequency (NF) and classified into three categories (environmental factors, congenital outcomes and biological mechanisms) and analyzed by regressive analysis to investigate the popular themes and their tendency. Results All 4687 articles were retrieved in this field. The 65.8% of the articles were published in recent 20 years (1997-2016), and 72.2% of the studies were conducted on the population (3384 articles). The researchers from the USA conducted 23.9% of the studies (1119 articles), only 3.1% of the studies were conducted by researchers from China (144 articles). For the popular themes of the environmental factors in this field, the NFs of chemical water pollutants, tobacco smoke pollution, noise, endocrine disruptor was 5.62%, 3.38%, 2.88%, 1.33% respectively. For the congenital outcomes, the NFs of congenital abnormalities, fetal death, neoplasm, disorders of sex development and inborn genetic disease were 14.82%, 4.32%, 5.08%, 2.47% and 2.22%. For the related biological mechanisms, the NFs of chromosome aberration, mutation and maternal-fetal exchange were 7.95%, 3.92% and 4.14%. Conclusion The study presents a data-based overview of the issues in field of environmental factors & congenital diseases, and concludes the popular themes in the environmental factors, the congenital outcomes, and the mechanisms, which can provide reference for future research.
