DOI: 10.20047/j.issn1673-7210.2023.26.01
Objective To analyze the vaginal microbiota of patients with missed abortion. Methods Patients attending Beijing Obstetrics and Gynecology Hospital, Capital Medical University from September 2020 to June 2021 were selected, 30 patients with missed abortion were included in the experimental group, and 30 normal early pregnancy who planned in the control group. Furthermore, the patients with missed abortion were further divided into 16 patients with normal chromosome group, six patients with abnormal chromosome group, and eight patients with unknown chromosome group according to the results of fetal villus chromosome examination. The questionnaire was filled in and the microecology detection of vaginal secretions was performed. The baseline data and vaginal microecology indexes of the four groups were compared by single factor. Meanwhile, 16S rRNA high-throughput sequencing technology was used to analyze the differences of vaginal microflora between the four groups. Results The comparison of baseline data and microecological indicators of the four groups showed that there were statistically significant differences in whether the four groups had a history of pelvic inflammatory disease, bacterial diversity, and dominant bacteria (P<0.05), among which the proportion of patients with a history of pelvic inflammatory disease and the age of first sexual life in the chromosome abnormality group were higher than those in the control group (P<0.05). The proportion of patients with previous abortion history was lower than that of control group (P<0.05). The results of OTU cluster analysis showed that, compared with the missed abortion group, the number of OTUs in the control group was more, and the bacteria community was more abundant. The results of species classification analysis showed that, compared with the control group, normal chromosome group had imbalance of different lactobacillus strains (increased Lactobacillus inners, decreased Lactobacillus unclassified), and the relative abundance of other bacteria except Lactobacillus (Gardnerella, Prevotella, Escherichia-Shigella, etc.) increased. There was no significant difference in the abundance and proportion of Lactobacillus strains in the abnormal chromosome group. The results of Alpha diversity analysis showed that there were significant differences in the flora abundance among the four groups (P<0.05). The flora abundance of the normal chromosome group was lower than that of the control group (P<0.05), there was no difference in the flora abundance between the abnormal chromosome group and the control group (P>0.05). The results of β-diversity analysis and line discriminant analysis effect size analysis showed that there was not significant difference in vaginal flora among the four groups (P>0.05). Conclusion The imbalance of vaginal microflora in patients with missed abortion may be related to the decreased relative abundance of Lactobacillus and / or the imbalance in the proportion of different Lactobacillus strains and the increased relative abundance of other bacteria except Lactobacillus (Gardnerella, Prevotella, Escherichia-Shigella, etc.) in the vaginal microflora, but its direct etiology and mechanism need further study.

DOI: 10.20047/j.issn1673-7210.2023.26.07
As an important messenger RNA, SPRY2 is involved in the pathological process of many diseases, plays a key role in maintaining cell homeostasis, and is even considered as a potential therapeutic target for studying cancer pathogenesis. Although SPRY2 has been extensively and deeply studied in many diseases, there is no systematic report on its role in cell phenotype. The research status and dynamic analysis of SPRY2 showed that it is specific in cell proliferation, apoptosis, migration, invasion, autophagy, epithelial mesenchymal transformation, and angiogenesis. Especially on cell proliferation, it provides ideas for SPRY2 follow-up research.

43 Research progress on molecular mechanism of macrophage polarization in atherosclerosis
PENG Chaojie WU Linke WU Hong
DOI: 10.20047/j.issn1673-7210.2023.26.08
Macrophages are one of the important immune cells in the development of atherosclerosis, and their phenotype and function play an important role in the occurrence and development of atherosclerosis. M1-type macrophages, by destroying their mitochondrial function, promote glycolysis and the release of reactive oxygen species, interfere with the balance of lipid metabolism of macrophages, maintain and expand the inflammatory response, and reduce the ability of macrophages to eliminate phagocytic necrotic cells. Accelerate the formation of necrotic cores and the occurrence of unstable plaque rupture. M2 type macrophages can maintain the normal respiratory function of mitochondria, promote oxidative phosphorylation metabolism, enhance intracellular lipid outflow, reduce inflammatory reaction, repair damaged tissues, clean necrotic cells, maintain plaque stability, and inhibit the progression of atherosclerosis. At present, the drugs used to prevent and treat atherosclerosis also show the potential to regulate the polarization of macrophages. Therefore, to further explore and summarize the molecular mechanism and effective target of regulating macrophage polarization in the pathological process of atherosclerosis is the potential direction of in-depth research on effective measures to prevent and treat atherosclerosis.

51 Research progress on the antipruritic effect of qūchí and xuèhǎi acupoint pair
LI Jiquan1 WANG Wei1 DAI Jianyu1 MA Tieming1 LI Sijia2
DOI: 10.20047/j.issn1673-7210.2023.26.10
Acupuncture and moxibustion has a definite antipruritic effect, but due to many diseases involved, the selection of acupoints is diverse and the use of acupoints is complex. With the key symptom of “skin itching” as the main concern, this paper clarifies that “disharmony between nutrient qi and defensive qi” is the common pathogenesis of itching. Combined with the high-frequency use of acupuncture points to stop itching, it is believed that the prescription composed of qūchí and xuèhǎi acupoint pair is the basic formula for acupuncture to “harmonize yingfen and weifen” to play an antipruritic effect. From the perspective of clinical research, it fully demonstrates the universal applicability of qūchí and xuèhǎi acupoint pair to the antipruritic effect, further summarizes the research progress of qūchí and xuèhǎi acupoint pair on the antipruritic effect mechanism, and believes that the evidence-based medical evidence of acupuncture antipruritic effect should be improved in the future, while deepening the mechanism research of antipruritic effect.

DOI: 10.20047/j.issn1673-7210.2023.26.15
Objective To investigate the effect of left bundle branch pacing and right ventricular septal pacing in the treatment of atrioventricular block. Methods One hundred and twenty-one patients with atrioventricular block treated from June 2020 to June 2021 in Shijiazhuang People’s Hospital were selected, and a total of 88 cases were enrolled after screening. They were divided into control group and study group by random number table method, with 44 cases in each group. The control group was given right septal pacing and the study group was given left bundle branch regional pacing. The cardiac function indexes, including left atrial anterior posterior diameter, left ventricular short axis shortening rate, left ventricular ejection fraction, brain natriuretic peptide (BNP) index, and QRS wave group width, and the occurrence of adverse effects in the two groups were compared. Results Three months after surgery, left ventricular short axis shortening rate, left ventricular ejection fraction in the two groups were higher than those before surgery, and left atrial anterior and posterior atrial diameter was lower than those before surgery, and left ventricular short axis shortening rate, left ventricular ejection fraction in study group were higher than those of control group, and left atrial anterior and posterior atrial diameter were lower than those of control group (P<0.05). Three months after operation, BNP levels in two groups were lower than those before operation, QRS wave widths were longer than those before operation, BNP in study group was higher than that of control group, QRS wave width was shorter than that of control group (P<0.05). There was no significant difference in the total incidence of adverse reactions between the two groups (P>0.05). Conclusion Compared with pacing in the right ventricular septum, the left bundle branch region pacing has less effect on cardiac function in patients with atrioventricular block, especially in patients with preoperative heart dysfunction, pacing in the left bundle branch region has more obvious improvement in cardiac function than pacing in the right ventricular septum region, and has significant clinical effect and effectively reduces the occurrence of adverse reactions, which is worthy of clinical promotion and application.

DOI: 10.20047/j.issn1673-7210.2023.26.17
Objective To analyze the effect of psychological distress on sleep quality in patients with multiple myeloma (MM) during chemotherapy. Methods A total of 100 MM patients who received chemotherapy in West China Hospital of Sichuan University from July 2020 to December 2021 were retrospectively collected and analyzed. All patients were evaluated by the degree of psychological distress (Kessler psychological distress scale [K10]) and sleep quality (Pittsburgh sleep quality index scale [PSQI]) after three courses of chemotherapy. The sleep quality of patients with different data characteristics and psychological distress was compared, and the relationship between psychological distress and sleep quality was analyzed by Spearman bivariate correlation. The effect of psychological disturbance on sleep quality was analyzed by linear regression. Results After three courses of chemotherapy, the average PSQI score of 100 patients was (9.02±2.37) points. The average K10 score was (25.60±6.76) points, among which 14, 29, 38, and 19 cases had low, lower, high, and higher degree of psychological distress, respectively. The total PSQI scores of patients with different age, disease ISS stage, education level and psychological distress were statistically significant (P<0.05). Spearman bivariate correlation analysis showed that the degree of psychological distress of MM patients during chemotherapy was positively correlated with the total score of PSQI (rs=0.515, P<0.05). Multiple linear regression analysis showed that age and degree of psychological distress were related to sleep quality of MM patients during chemotherapy (B=1.089, 1.353, P<0.05). Conclusion The more serious the psychological distress of MM patients during chemotherapy, the worse their sleep quality. In order to improve their sleep quality and life quality, targeted measures should be taken to alleviate the psychological distress of MM patients and improve their sleep quality.

148 Professor Li Hao’s experience in treating alopecia from “deficiency of blood and excess of retained fluid” with Danggui Shaoyao Powder
LI Zehui YANG Yang LIU Nanyang ZHANG Zhen PEI Hui LI Hao
DOI: 10.20047/j.issn1673-7210.2023.26.32
Alopecia is a common disease in clinics, which can be treated with syndrome differentiation in traditional Chinese medicine and achieve better results. Professor Li Hao considers that the main pathogenesis of alopecia in traditional Chinese medicine is “deficiency of blood and excess of retained fluid”. When yin blood is insufficient, hair is lost nourishment, because hair roots are empty and withered. In addition, water and dampness erode hair roots, accelerating their decay and shedding. The liver stores blood and regulates the operation of blood, and the spleen is the source of qi and blood. The stagnation of  wood is stronger than earth, which causes dysfunction of spleen in transportation, so water and dampness invade the top of the head. Blood deficiency will also cause blood flow poorly. Unsmooth blood circulation results in water retention. The pathogenesis is a combination of liver and spleen dysfunction with abnormalities in blood and water. Danggui Shaoyao Powder is composed of three “blood medicines” Angelicae Sinensis Radix, Paeonia lactiflora Pall., and Chuanxiong Rhizoma, and three “water medicines” Poria, Alismatis Rhizoma, and Atractylodis Macrocephalae Rhizoma. It has the effect of invigorating the spleen and dispersing the stagnated liver-energy, nourishing blood and invigorating blood circulation, eliminating dampness and diuresis. It harmonizes the liver and spleen and treats blood and water simultaneously. When using Danggui Shaoyao Powder to treat alopecia,  Professor Li Hao usually reduces the amount of Paeonia lactiflora Pall. increases the amount of Angelicae Sinensis Radix and Poria, and takes into account invigorating the liver and kidney as appropriate, or accompanied by eliminating heat and removing stasis.

165 Discussion on the three-dimensional dynamic diagnosis and treatment model based on the Qisexingtai hand diagnostic method combination of “symptom-differentiation-diagnosis”
LIU Qian1 WANG Shiheng2 LI Yu2 LIU Jianfeng2
DOI: 10.20047/j.issn1673-7210.2023.26.36
In the current clinical diagnosis and treatment process, the traditional medical diagnosis and treatment system and the modern medical diagnosis and treatment system have their own advantages and disadvantages. The understanding of diseases is mainly carried out through three aspects and three dimensions: symptoms, traditional medical syndrome differentiation, and modern medical diagnosis, and the information obtained from the three has its limitations. How to better combine the three is a question that Chinese medical practitioners have been thinking about and have put forward many ideas. For example, the combination of disease and syndrome, integrative medicine, etc. The combination of disease and syndrome is a two-dimensional system that combines modern medicine and traditional medicine. In his long-term clinical practice, Professor Liu Jianfeng published Observing the Hand to Know the Disease - Essential Manual Diagnosis of Complexion Shape in 1991, creatively proposed the theory and method of manual diagnosis of complexion shape. On this basis, he combined the patient’s symptoms, traditional medical syndrome differentiation and modern medical diagnosis to form a three-dimensional dynamic diagnosis and treatment model, which solved the theoretical problems of incompatible three cognitive systems. It has improved the efficiency and effect of clinical diagnosis and treatment, and enhanced the service capacity and level of medicine.
