Protective effect of Irisin on damage of H9C2 cardiomyocytes under hypoxia-reoxygenation

Objective To observe the protective effect of Irisin on H9C2 cardiomyocytes after hypoxia reoxygenation. Methods Rat H9C2 cardiomyocytes were divided into control group, control+Irisin group, hypoxia-reoxygenation group, hypoxia-reoxygenation+Irisin group. The control group was cultured normally. The control +Irisin group was cultured normally and given 10 ng/mL of Irisin. The hypoxia-reoxygenation group was treated with hypoxia-reoxygenation, and the cells of hypoxia-reoxygenation+Irisin group were treated with hypoxia-reoxygenation and 10 ng/mL of Irisin. Cell viability was detected by CCK-8. Reactive exygen species (ROS) fluorescence probe was used to detect intracellular ROS, and mitochondrial membrane potential JC-1 was detected by flow cytometry. The mRNA levels of caspase-3 and caspase-9 in cardiomyocytes were detected by RT-PCR. Results The cell viability of the hypoxia-reoxygenation group was significantly lower than control group (P < 0.01). The cell viability of the hypoxia-reoxygenation + Irisin group was higher than hypoxia-reoxygenation group (P < 0.05). The intracellular ROS level in the hypoxia-reoxygenation group was higher than that in the control group (P < 0.01). The level of ROS in the hypoxia-reoxygenation + Irisin group was lower than that of the hypoxia-reoxygenation group (P < 0.05). The mitochondrial membrane potential JC-1 level in hypoxia-reoxygenation group was lower than control group (P < 0.05). The level of mitochondrial membrane potential JC-1 in hypoxia-reoxygenation + Irisin group was higher than hypoxia-reoxygenation group (P < 0.05). The mRNA levels of caspase-3 and caspase-9 in the hypoxia-reoxygenation group were higher than the control group (P < 0.05). The mRNA levels of caspase-3 and caspase-9 in hypoxia-reoxygenation + Irisin group were lower than hypoxia-reoxygenation group (P < 0.05). Conclusion Irisin can significantly improve the cell viability of cardiomyocytes after hypoxia-reoxygenation, reduce ROS production, restore mitochondrial membrane potential, and inhibit apoptosis pathway. It can be used as a potential therapeutic agent for improving myocardial ischemia-reperfusion injury.

Experimental study on anti- ovarian carcinoma effect of minicircle DNA expressing anti-EpCAM/anti-CD3 bispecific BiTE in vitro

Objective To study the effect of anti-EpCAM/anti-CD3 BsAb protein inhibition on ovarian cancer cells in vitro. Methods Non-viral vector microcircle DNA was used to obtain the anti-EpCAM/anti-CD3 BsAb protein. Different concentrations (1×10-3, 1×10-4, 1×10-5, 1×10-6, 1×10-7, 1×10-8 μg/mL) of anti-EpCAM/anti-CD3 BsAb protein was added to ovarian cancer cells and T cells. After 6 h and 12 h, the number of live cells and dead cells were counted and morphological changes were observed by inverted fluorescence microscopy. Results Under the inverted microscope, ovarian cancer cells and T cells were obviously aggregated after the addition of anti-EpCAM/anti-CD3 BsAb, and a large number of dead cells were found around them. At the same time, the killing effect of ovarian cancer cells mediated by T cells did not always increased with the increase of concentration. When the concentration reached 1×10-4 μg/mL, the killing rate reached the maximum in both 6 h and 12 h time periods, and the difference were both statistically significant (P < 0.05). Conclusion The anti-EpCAM/anti-CD3 BsAb expressed by the novel microcircle DNA expression system can inhibit the growth of ovarian cancer cells in vitro, and it may be used as an effective targeted anti-ovarian cancer tumor in the future.

Protective effect of ginsenoside Rb1 on regulating endoplasmic reticulum stress in important organs of septic mice

Objective To observe the effects of ginsenoside Rb1 on endoplasmic reticulum stress in important organs of sepsis mice, and to explore the molecular mechanism of the protective effect of ginsenoside Rb1 on sepsis. Methods The sepsis model was reproduced by cecal ligation and perforation (CLP). Sixty adult (4-6 weeks old) C57BL/6 female mice were randomly divided into the normal group, the false CLP group, the ginsenoside group and the CLP group, with 15 mice in each group. The general state of the mice was observed after successful model preparation. The general state of the mice was observed. The mice were sacrificed 24 h after the operation, and the cardiac muscle and lung tissue were taken for pathological examination. The expressions of glucose regulated protein 78 kD (GRP78) and caspase-12 proteins in cardiac muscle and lung tissue were detected by Western blotting. Results Compared with the CLP group, the general status of mice in the ginsenoside group was significantly improved. Compared with the CLP group, the pulmonary alveoli tissue structure of the ginsenoside group was relatively clear and complete, inflammatory cell infiltration was reduced, and pulmonary alveoli size was uniform. In the ginsenoside group, there were fewer inflammatory cells, striations and necrotic cells in the cardiac muscle tissue. Compared with the CLP group, GRP78 and caspase-12 protein expression levels in cardiac muscle and lung tissue of the ginsenoside group were significantly decreased, with highly statistically significant differences (all P < 0.01). Conclusion Ginsenoside Rb1 can reduce the severity of infection in sepsis mice, and has a protective effect on the injury of heart and lung tissue in mice. Its mechanism may be related to the inhibition of endoplasmic reticulum stress overexpression by ginsenoside Rb1.

Pharmacokinetic study of toosendanin in Toosendan Fructus extract in rats

Objective To study the pharmacokinetic characteristics of toosendanin in Toosendan Fructus in rats, through high performance liquid chromatography-mass spectrometry detection of the plasma concentration of toosendanin inrats (LC-MS/MS). Methods ZORBAX Eclipse Plus C18 (4.6 mm×100 mm, 3.5 μm) column was used as the column, acetonitrile-0.01% formate water was used as the mobile phase of the gradient elute, (0-4 min: acetonitrile 5%-55%; 4-6.5 min: acetonitrile 55%; 6.5-9 min: acetonitrile 55%-5%) and the flow rate of 0.6 mL/min, injection volume was 5 μL. The scanning mode was multi-response detection scanning (MRM), the ionization mode was electrospray ion source (ESI) and it was performed by negative ion. Twenty-four male SD rats were randomly divided into three groups according to their body weight, with 8 rats in each group. They were low dose group (15 g crude drug/kg), medium dose group (30 g crude drug/kg) and high dose group (60 g crude drug/kg). Blood was collected from the orbit at each time point after administration. After sample treatment, the pharmacokinetic parameters were calculated by DAS 2.0 software. Results The toosendanin in the range of 1.02-306 ng/mL showed good linear relationship with the peak (r = 0.9953). After the rats were given different doses of Toosendan Fructus extract, compared with the middle and low dose groups, the maximum serum concentration of high dose increased, the area under the curve, clearance rate and apparent distribution volume decreased (P < 0.05). However, there was no statistical difference between the medium dose group and the low dose group (P > 0.05). Conclusion A LC-MS/MS method for the determination of toosendanin in rat plasma is established. The method is simple, accurate and can be used for the pharmacokinetic study of toosendanin in rats plasma.

Effect of Galium verum L. Solution combined with acupuncture and moxibustion therapy on chronic skin ulcer of lower limbs

Objective To research the effect of Galium verum L. Solution combined with acupuncture and moxibustion therapy on chronic skin ulcer of lower limbs. Methods From January to May 2019, eighty-six patients with chronic skin ulcer of lower limbs who met the experimental criteria were selected from the Department of Peripheral Vascular Diseases, the First Affiliated Hospital of Heilongjiang University of Chinese Medicine, they were divided into experimental group and control group by using random number table method, with 43 cases in both group. Two groups were active in the treatment of primary diseases, while the debridement and dressing change were needed daily. Galium verum L. Solution was sprayed on the ulcer surface in the experimental group. At the same time, multiple acupuncture points of affected limbs were selected and moxibustion was carried out once a day. The patients in the control group were sprayed with Benazine solution on the ulcer surface. Two groups were treated continuously for four weeks. At the beginning and end of the experiment, the ulcer area was calculated by a sterile transparent biofilm method, and the ulcer healing of the two groups was compared. The healing time of ulcer after treatment was recorded and compared between the two groups. The numbers of patients at all levels after treatment in the two groups were recorded according to the evaluation criteria of wound granulation tissue changes. The changes of wound granulation tissue were compared. Results In the experimental group, the total effective rate was higher than that in control group, and the changes of new granulation tissue after treatment were significantly higher than those in the control group, while the healing time of the experimental group was significantly shorter than that in the control group, and the differences were statistically significant (P < 0.05). Conclusion The clinical effect of Galium verum L. Solution combined with acupuncture and moxibustion therapy on chronic skin ulcer of lower limbs is significant and better than that of Benazine solution. It is worthy of clinical application.

Research progress of microglia polarization in ischemic stroke

The researchers aim to develop new technologies and new drugs, hoping to improve cerebral ischemia related symptoms by extending the therapeutic time window, promoting neurogenesis and reducing neuroinflammation. In recent years, although academic-industrial collaborations have invested a lot in the field of stroke, no breakthrough has been achieved in translational medicine, suggesting that a deeper understanding of the pathogenesis of cerebral ischemia is required. In physiological state, microglial plays an important regulatory role in monitoring microenvironment in brain, promoting synaptic development, maintaining normal synaptic function and maintaining neuroplasticity. Cerebral ischemia can lead to microglia polarization. The regulation of the dual function of injury and repair after microglial neuronal polarization are considered an important target for brain protection therapy, and related drugs have shown considerable neuroprotective effect in animal experiments. In this review, the characteristics and regulations of microglia polarization after cerebral ischemia and related drug development are comprehensively reviewed.

Research progress of the relationship between integrin β1-FAK-Ras-MAPK signal transduction pathway and tumor

Integrin β1 is the major member of the integrin family, it can activate a variety of signal transduction pathways. The FAK-Ras-MAPK signal transduction pathway is an important signal transduction pathway mediated by integrin β1, which is closely related to the occurrence and development of many diseases, and it is also one of the important mechanisms of tumorigenesis and development. This is mainly related to the fact that it can induce epithelial-mesenchymal transformation, promote the proliferation of tumor cells, inhibit the apoptosis of tumor cells, and promote the formation of tumor blood vessels. Therefore, this pathway has become a potential target for the treatment of tumors. This paper reviews the components, transduction mechanism, and the related research progress on tumorigenesis and development of β1-FAK-Ras-MAPK signal transduction pathway.

Research progress of solomonseal rhizome treatment of cognitive impairment

This paper reviews the literature reports on pharmacological effects of solomonseal rhizome at home and abroad in recent years. The mechanism of action of solomonseal rhizome in the treatment of cognitive dysfunction is analyzed and summarized, it is concluded that solomonseal rhizome and its effective components and solomonseal rhizome compound preparations can effectively improve learning and memory ability of cognitively disabled organisms, alleviate the symptoms, such as insomnia, fatigue, forgetfulness, regulate the physiological and biochemical indexes, protect heart head blood-vessel, anti-inflammatory, anti-aging, improve immune function. It shows that solomonseal rhizome is effective in preventing and treating cognitive disorders, which can provide a reference for clinical prevention and treatment of cognitive dysfunction, and provide a new idea for the research and development of brain-strengthening and brain-beneficial new drugs.

Research progress of traditional Chinese and Western medicine in the rehabilitation of chronic heart failure

Chronic heart failure (CHF) has become the most important public health problem in the world. The decline of exercise ability and exercise tolerance is one of the main symptoms of CHF, which seriously affects the quality of life of patients with chronic heart failure. Studies have proved that it can effectively improve the cardiac function and quality of life of patients with CHF. Exercise training is the core content of cardiac rehabilitation. In Western medicine, cardiac rehabilitation model mainly includes aerobic training, resistance training and flexibility training. In traditional Chinese medicine, it mainly includes traditional Chinese exercise skills, including Taijiquan, Baduanjin, Wuqinxi and so on. In this paper, the research progress of traditional Chinese and Western medicine on cardiac exercise rehabilitation of CHF is summarized as follows.

Study on professional attitude of male nursing students in different grades and related factors in Guizhou Province

Objective To understand the professional attitudes and related factors of male nursing students in different grades in Guizhou Province. Methods From September to October 2017, 102 male nursing students from Zunyi Medical University, Guizhou Medical University and Guiyang College of Traditional Chinese Medicine in Guizhou Province were selected by random sampling method and investigated with general information questionnaire and professional attitude scale. Related factors were analyzed by multiple stepwise linear regression. Results The scores of professional attitude of male nursing students were (4.81±0.96)scores, which were significantly higher in Grade one and Grade two than those in Grade four (P < 0.05). The results of multiple stepwise linear regression showed that grades, whether boys were considered suitable for nursing, and whether nursing teachers had a positive attitude towards nursing were the related factors of professional attitude scores(P < 0.05). Conclusion The professional attitude of male nursing students is neutral and positive. Educators should pay attention to the professional emotional education of male nursing students during their internship to promote their positive and stable professional attitude.

Analysis on correlation between knowledge-attitude-behavior level of self-management and quality of life in middle-aged and elderly type 2 diabetes patients in community

Objective To explore the correlation between knowledge-attitude-behavior level of self-management and quality of life in middle-aged and elderly type 2 diabetes patients in communities. Methods Totally 233 patients were investigated by using brief version of self-management knowledge, attitude, and behavior assessment scale and diabetes specific quality of life scale. The study was conducted in the communities of Qingshan, Tianmashan, Huangchaling and Songmutang in Hengyang City from November 2018 to March 2019. Results A total of 237 questionnaires were distributed, and 233 valid questionnaires were finally collected. The effective recovery rate was 98.31%. The overall scores of quality of life was (53.03±11.78) scores. The mean score of self-management knowledge was (16.93±3.80) scores, of self-management attitude was (4.36±0.51) scores, of self-management behavior was (9.82±1.93) scores. The three dimensions were positively correlated with psychological function, social relationship, and total quality of life. Conclusion The quality of life of Middle-aged and elderly patients with type 2 diabetes in communities is at the upper-middle level, and knowledge-attitude-behavior of self-management and quality of life are positively correlated.

Analysis on the use of IUD and the influencing factors of adverse reactions in some areas of Jiangsu Province

Objective To investigate the status of side effect in women using intrauterine devices (IUD) after health and family planning combined in Jiangsu. Methods From January to December 2016, a county-level maternal and child health and family planning service center in Northern, Central and Southern Jiangsu was selected as the research unit. 269 women of childbearing age who were required to place IUD and voluntarily participated in the study were taken as the research objects. The adverse reactions of one year were followed up and observed, including 88 cases in Northern Jiangsu, 90 cases in Central Jiangsu and 91 cases in Southern Jiangsu. Results There were significant differences in occupation and education level among the subjects in different regions (P < 0.05). There were a lot of objects for the first time in Northern Jiangsu, and the difference was statistically significant (P < 0.05). There were a lot of high-risk devices in the middle of Jiangsu Province, the difference was statistically significant (P < 0.05). The high risk group was significantly higher than the first group (P < 0.05). Multivariate analysis showed that the incidence of adverse reactions of IUD users in southern Jiangsu was 5.41 times higher than that in Northern Jiangsu (OR = 5.41, 95%CI:2.27-12.93). Conclusion After the integration of health and family planning resources, the situation in different regions is different. Some regions should pay attention to the medical follow-up of high-risk objects, timely find and deal with adverse reactions, improve the satisfaction of women of childbearing age with contraceptive device service, provide high-quality reproductive health services for the masses, and improve the reproductive health level of people of childbearing age.

Correlation Between MRP 8/14 and thyroid hormone in patients with type 2 diabetes mellitus

Objective To analyze the correlation between MRP 8/14 and thyroid hormone in patients with type 2 diabetes mellitus. Methods Eighty patients with type 2 diabetes mellitus admitted to the Department of Endocrine Metabolism, the Affiliated Huai′an No.1 People′s Hospital of Nanjing Medical University from December 2014 to September 2016 were collected. According to the median of MRP 8/14, they were divided into group A (MRP 8/14≤5.41 μg/mL) and group B (MRP 8/14 >5.41 μg/mL), with 40 patients in each group. The gender, age, body mass index (BMI), duration of diabetes, systolic blood pressure, diastolic blood pressure, fasting blood glucose, glycosylated hemoglobin (HbA1c), total cholesterol, triacylglycerol, low density lipoprotein cholesterin, high density lipoprotein cholesterin, serum creatinine and urinary albumin/creatinine ratio (ACR), thyroid stimulating hormone (TSH), thyroid hormone [serum free triiodine-methylamine (FT3), serum free thyroxine (FT4)], MRP 8/14 and other clinical data of all participants were recorded and analyzed. Results There were no statistically significant differences in gender, age, BMI, duration of diabetes, systolic blood pressure, diastolic blood pressure, fasting blood glucose, HbA1c, total cholesterol, triacylglycerol, low density lipoprotein cholesterin, high density lipoprotein cholesterin, FT3, FT4 between the two groups (all P > 0.05). Serum creatinine, ACR and TSH in group A were significantly lower than those in group B (all P < 0.05). Serum MRP 8/14 level was positively correlated with BMI, systolic blood pressure, serum creatinine, ACR and TSH (P < 0.05). Conclusion Serum MRP 8/14 is positively correlated with TSH level in patients with type 2 diabetes mellitus, so MRP 8/14 may be involved in thyroid hormone changes in patients with type 2 diabetes mellitus

Application evaluation of different equations for estimated glomerular filtration rate in the solitary functioning kidney population

Objective To evaluate the efficacy of different equations for estimated glomerular filtration rate in the solitary functioning kidney subjects. Methods A total of 258 patients with solitary functioning kidney who underwent kidney dynamic imaging to evaluate kidney function in the Second Affiliated Hospital of Soochow University from January 2009 to December 2018 were included. The accuracy and precision of 9 glomerular filtration rate (GFR) evaluation equationes (MDRD, Peking, Macisaac, Steven 1, Steven 2, Steven 3, CKD-EPIScr, CKD-EPIScys C and CKD-EPIScr-Scys C) were measured by using the indexes of bias, interquartile interval of difference, and misclassification. Results The three estimation equations based on serum creatinine (Scr) at least overestimated the glomerular filtration rate of reference (rGFR) 10 mL/(min·1.73m2) in the whole cohort, especially Peking equation precision and accuracy are poor, the bias is 33.8 mL/(min·1.73m2). All three equations overestimated the estimated glomerular filtration rate (eGFR) at CKDl-3 phase, and underestimated eGFR at CKD4-5 phase. The two prediction equations of combined by Cys C and serum cystatin C (Scys C) showed good accuracy and consistency. The absolute value of median bias of CKD-EPIScr-Scys C equation was the smallest in the overall cohort, severe impaired kidney function, and acquired isolated kidney subjects, which were 11.1, 7.3 and 11.1 mL/ (min·1.73m2), respectively. And it had the highest accuracy which were 76.7%, 85.7% and 81.8%, respectively, and P < 0.001 compared with rGFR. Conclusion In this study, CKD-EPIScr-Scys C equation is recommended as the first choice for kidney function evaluation in the solitary functioning kidney subjects. The accuracy of GFR estimation equation may be related to GFR level and kidney function markers, but not to solitary functioning kidney type.

Observation on the effect of preserved anterior lobe in plasmakinetic transurethral enucleation of prostate

Objective To study the effect of preserving anterior lobe in plasmakinetic transurethral enucleation of prostate. Methods From October 2017 to January 2019, the clinical data of 80 patients diagnosed with benign prostatic hyperplasia in Department of Urology, the Second Affiliated Hospital of Kunming Medical University were collected. All patients underwent plasmakinetic transurethral enucleation of prostate. According to whether the prostatic anterior lobe was retained or not, the patients were divided into non-retained group and retained group, with 40 cases in each group. Post void residual urine volume (PVR), maximum urinary flow rate (Q-max), international prostate symptom score (IPSS), quality of life score(QOL) after 6 months after surgery, the incidence of postoperative urinary incontinence, patient quality of life score and clinical treatment effect were compared between the two groups. Results There was no significant difference in PVR and Q-max scores between the two groups (P > 0.05). IPSS and QOL scores of the retained group were lower than those of the non-retained group, and the differences were statistically significant (all P < 0.05). The incidence of postoperative urinary incontinence in the retained group was lower than that in the non-retained group, and the difference was statistically significant (P < 0.05). The improvement of quality of life in the retained group was higher than that in the non-retained group, and the difference was statistically significant (P < 0.05). The treatment effect of the retained group was better than that of the non-retained group, and the difference was statistically significant (all P < 0.05). Conclusion Plasmakinetic transurethral enucleation of prostate to protect the anterior lobe of the prostate is an effective surgical method, which can reduce the incidence of postoperative urinary incontinence.

Clinical effect comparison of three different operative methods for treatment of chronic Doyle typeⅠa mallet finger

Objective To compare the clinical effect among reconstruction of tendon insertion, skin tightening suture and distal interphalangeal joint fusion in the treatment of chronic Doyle typeⅠa mallet fingers. Methods A retrospective analysis was conducted on the clinical data of 81 patients with chronic Doyle typeⅠa mallet fingers treated in the First Department of Orthopaedics of the First People′s Hospital of Yibin in Sichuan Province from May 2007 to February 2017. The patients were divided into reconstruction group (40 cases), tightening suture group (27 cases) and fusion group (14 cases) base on the treatment method selected by the patients. They were followed up for 12 months. The basic conditions, general conditions of surgery and hospitalization, Dargan function evaluation of the affected fingers, Carroll scoring of hand functions, the visual analog scale (VAS) and subjective satisfaction evaluation of patients were compared among the three groups. Results The duration of operation, and the hospitalization cost of tightening suture group were lower than those of the reconstruction group and the fusion group, and the differences were statistically significant (P < 0.05). There were no significant differences in the hospitalization time and the total incidence of complications among the three groups (P > 0.05). Twelve months after operation, the excellent and good rate of Dargan function evaluation in the fusion group was significantly lower than that in the reconstruction group and the tightening suture group, and the difference was statistically significant(P < 0.05). There were no significant differences in Carroll scores, VAS scores and the excellent and good rate of patients′ satisfaction among the three groups (P > 0.05). Conclusion All the three surgical methods are available to achieve high patient satisfaction and good hand functions, and can be used for the treatment of chronic mallet fingers. However, reasonable selection should be made according to the condition and patients′ requirements for treatment.

A systematic review and Meta-analysis of the clinical efficacy of Duhuo Jisheng Decoction in the treatment of lumbar disc herniation

Objective To systematically evaluate the clinical efficacy of Duhuo Jisheng Decoction in the treatment of lumbar disc herniation (LDH). Methods The randomized controlled trial (RCT) of the treatment of LDH by Duhuo Jisheng Decoction in Chinese databases such as Wanfang, CNKI, VIP and SinoMed was searched. The retrieval time was from the establishment of the database to January 30, 2019. Meta-analysis was conducted with Review Manager 5.3 software. The observation group was treated with Duhuo Jisheng Decoction plus or minus, while the control group was treated with traction, acupuncture, Fenna chlorate capsule, Meroxicam and epidural closure. Total effective rate, visual analogue score (VAS) and adverse reactions were included in outcome indexes. Results A total of 449 articles were retrieved, 13 articles were selected and included, and 1143 patients were involved. The total effective rate of the observation group was higher than that of the control group, and the difference was highly statistically significant (OR=5.37,95%CI:3.63,7.96, P < 0.01). There was no difference in the improvement of VAS score between the two groups [MD=-1.20,95%CI:-2.69,0.28, P > 0.05). Adverse reactions in the observation group were less than that of the control group, and the difference was statistically significant (OR=0.15,95%CI:0.03,0.82, P < 0.05). Conclusion Duhuo Jisheng Decoction can effectively alleviate the clinical symptoms of LDH patients with good safety. However, due to the shortcomings of the included literature methodology, this conclusion has some limitations and needs to be further confirmed by more high-quality clinical studies.

Clinical comparison of fibular graft with vascular pedicle and tantalum rod implantation in the treatment of early osteonecrosis of the femoral head

Objective To compare the clinical efficacy of fibular graft with vascular pedicle (FVFG) and tantalum rod implantation (TRI) in the treatment of early osteonecrosis of the femoral head (ONFH). Methods Data of 23 cases (24 hips) of early ONFH treated with FVFG from January 2017 to July 2018 in the Affiliated Hospital of Traditional Chinese Medicine of Xinjiang Medical University were collected (FVFG group), in addition, data of 22 patients (23 hips) receiving TRI surgery from January 2015 to December 2016 were collected (TRI group). Harris score of hip joint and X - ray performance index were compared 6 months after operation. Results In FVFG group, 2 patients were lost to follow-up because of losing contact. In the end, 21 patients were followed up for 6 to 18 months, with an average of (12.71±2.30)months. All patients in TRI group were followed up for 7 to 18 months, with an average of (13.31±1.33)months. The Harris score of hip joint of the two groups at 6 months after operation were higher than those before operation (P < 0.05),and the Harris score of hip joint in FVFG group was higher than that in TRI group (P < 0.05). The excellent and good rate of Harris score of hip joint in TRI group was 54.55% (12/22), while that in FVFG group was 85.71% (18/21), and there was statistically significant difference in the excellent and good rate between the two groups (P < 0.05). The X-ray performance index of the two groups at 6 months after operation were higher than those before operation (P < 0.05), and the X-ray performance index of FVFG group at 6 months after operation was better than that of TRI group (P < 0.05). In FVFG group, 5 cases had imaging progress, 3 cases had total hip replacement, 11 cases had imaging progress in TRI group and 11 cases had total hip replacement. There was no statistically significant difference in the incidence of complications between the two groups (P > 0.05). Conclusion FVFG can improve hip function in early ONFH patients and is superior to TRI in preventing long-term femoral head collapse.

Detection and analysis of bone metabolic indexes in elderly female patients with distal radius fractures and discussion of risk factors for recurrent fractures

Objective To investigate the bone metabolism of elderly female patients with distal radius fracture and the risk factors of recurrent fracture. Methods From February 2017 to October 2018, retrospective analysis was conducted on the data of 182 elderly female patients with distal radius fracture admitted to Shijiazhuang No. 3 Hospital. Bone metabolism indexes were detected after admission. The risk factors of recurrent fracture were analyzed. Results Patients with serum alkaline phosphatase (ALP), bone gta-protein (BGP), collagen type Ⅰ N terminal peptide (NTX-Ⅰ) level were (37.56±5.83)U/L,(3.89±1.05)μg/L, (38.47±5.56)nmol/L, respectively. The incidence recurrent hair fracture was 18.68%, older age, body mass index (BMI) < 18 kg/m2, BMI > 25 kg/m2, with type 2 diabetes, osteoporosis, with lower limb joint disease, serum ALP < 40 U/L, serum BGP < 4.8 μg/L, serum NTX-Ⅰ > 20 mmol/L, live alone are independent risk factors for recurrence of bone (OR = 5.512, 6.265, 5.697, 6.659, 7.776, 6.062, 7.367, 6.699, 6.966, 7.135, P < 0.05). Bone grafting, early load exercise, and calcium salt use were all independent protective factors for recurrent fractures (OR = 0.511, 0.457, 0.475, P < 0.05). Conclusion Elderly women with distal radius fracture are often accompanied by abnormal bone metabolism, and advanced age can increase the risk of recurrent fracture. Abnormal bone metabolism is an independent risk factor, while bone grafting is a protective factor.

Relationship between intrauterine device and incidence of ectopic pregnancy:a Meta-analysis

Objective To explore the relationship between intrauterine device (IUD) and ectopic pregnancy using a meta-analysis and provide evidence for prevention and treatment of ectopic pregnancy. Methods From the database built to May 10, 2019, Embase, PudMed, the Cochrane Library, China National Knowledge Infrastructure, WanFang Database, VIP Database were searched for randomized case-control studies on the relationship between IUD retention and ectopic pregnancy. Women with ectopic pregnancy were selected as the case group and women with normal pregnancy were selected as the control group. The number of patients with IUD in both groups were recorded. Data were extracted with 3 researchers independently from literatures that met the inclusion criteria. NOS score was used for literature quality evaluation. The sources of heterogeneity were explored with subgroup analysis or sensitivity analysis. Results A total of 19 case-control studies were included. There were 2736 cases in the case group and 3863 cases in the control group. The results showed that the probability of ectopic pregnancy with the case group was 3.11 times higher than that in the control group, the difference is highly statistically significant (P < 0.000 01). Conclusion IUD is one of the risk factors of ectopic pregnancy.

Influence of Bushen Huoxue Decoction on HOXA10 expression in uterine flushing fluid of patients with endometriosis-related infertility

Objective To investigate the changes of HOXA10 in uterine flushing fluid after treatment with Bushen Huoxue Decoction in patients with endometriosis (EMs) caused by kidney-deficiency and blood stasis type EMs-related infertility. Methods From December 2016 to June 2018, 20 cases who diagnosed EMs-related infertility as kidney deficiency and blood stasis in the Department of Gynecology, Affiliated Hospital of Integrated Traditional Chinese and Western Medicine Nanjing University of Chinese Medicine were selected. They were treated with Bushen Huoxue Decoction for three menstrual cycles. The endometrial thickness of the implantation window and blood flow parameters were evaluated by transvaginal Doppler ultrasound before and after treatment. The concentrations of HOXA10 in uterine flushing fluid of before and after treatment were analyzed by enzyme liked imrnunosorbent assay. Results After Bushen Huoxue Decoction treatment, the thickness of the implantation window endometria was obviously higher than that before treatment, blood flow parameters of Uterine artery resistance index (PI) and pulsatility index (RI) were decreased significantly, the expression level of HOXA10 was significantly higher than that before treatment, and the differences were statistically significant (P < 0.05). Condusion Bushen Huoxue Decoction can increase the thickness of endometrium, improve the intrauterine hemodynamic parameters of patients with EMs-related infertility caused by kidney deficiency and blood stasis, improve the expression level of HOXA10 in uterine flushing fluid and improve endometrial receptivity.

Effect of gestational diabetes mellitus on blood gas analysis of neonatal umbilical cord

Objective To investigate the effect of gestational diabetes mellitus (GDM) on blood gas analysis in neonates. Methods A total of 2913 GDM and 3216 non-GDM women with single pregnancy in Foshan Women and Children′s Hospital from January 2017 to August 2018 were randomly sampled and collected. All indexes of neonatal umbilical cord blood were tested by blood gas electrolyte analyzer, and further analyzed between groups and subgroups, including subgroups of blood glucose abnormality at different time point of oral glucose tolerance test (OGTT) and subgroups of delivery mode. Results The results of base excess extracellular fluid (BEecf), BEblood (BEb), standard bicarbonate concentration (SBC), total carbon dioxide (TCO2), partial pressure of carbon dioxide (PCO2) and sodium (Na+) between GDM group and non-GDM group were compared, and the differences were tstatistically significant (P < 0.05). There were significant difference in BEecf, BEb, TCO2, partial alveolar arterial oxygen pressure (A-aDO2) and Na+ among GDM neonates with abnormal blood glucose at different time points of OGTT (P < 0.05). In addition to oxygen partial pressure (PO2), oxyhemoglobin saturation (SaO2) and Na+ between GDM cesarean section group and GDM vaginal delivery group, there were significant differences in other detection indexes (P < 0.05). Conclusion Under the ideal condition of blood glucose control in GDM pregnant women, BEb, TCO2 and SBC changed, but not enough to affect the pH value of umbilical cord blood, and had no significant effect on PO2 and SaO2.

Analysis on type of HPV infection and detection of trace elements in patients with cervical diseases

Objective To investigate the typing of human papillomavirus (HPV) infection in patients with cervical diseases and the effect of trace elements. Methods From December 2017 to December 2018, 600 patients with abnormal cervical cytology admitted to Chengde Central Hospital in Hebei Province were selected as the study group, and 360 patients with chronic cervicitis at the same time were selected as the control group. HPV typing and serum trace elements levels were compared and the clinical characteristics of HPV typing and serum trace elements in patients with cervical diseases were analyzed. Results The positive rate of HPV in the study group was higher than that in the control group (P < 0.05). The types of HPV in the study group were HPV18, 16, 31, 33, 52, 58, 59, 6. The types of HPV in the control group were HPV33, 52, 58, 66, 68, 6, 11, 43. The positive rate of high-risk HPV in the study group was higher than that in the control group. HPV infection types was compared between two groups, the difference was statistically significant (P < 0.05). Spearman correlation analysis showed that cervical cytology abnormalities were positively correlated with HPV infection and high-risk HPV infection (r = 0.403, 0.468, all P < 0.05) . The positive rate of single infection in the study group was higher than that in the control group (P < 0.05). The levels of serum cadmium, nickel, arsenic and copper in the study group were higher than those in the control group, the levels of serum zinc and selenium in the study group were lower than those in the control group, the differences were highly statistically significant (P < 0.01). Logistic regression analysis showed that copper > 28.73 μmol/L was an independent risk factor for cervical cytological abnormalities, while zinc > 71.96 μmol/L and selenium > 36.47 μg/L had protective effects (P < 0.01). Conclusion Cervical cytological abnormalities in patients with cervical diseases are closely related to HPV infection typing, serum zinc, selenium and copper levels.

Differential expression of microRNA in atypical endometrial hyperplasia based on high-throughput sequencing

Objective To investigate the differentially expressed microRNAs (miRNAs) and significance in endometrium from atypical endometrial hyperplasia (AEH) and normal women through high-throughput sequencing. Methods From January 2016 to January 2017, three patients diagnosed with AEH (experimental group) and three normal women (control group) admitted to Beijing Obstetrics and Gynecology Hospital, Capital Medical University were collected. Second-generation high-throughput sequencing was used to screen the differentially expressed miRNAs. Databases was used to predict its target genes and biological functions, and understand the closely related miRNAs and their co-regulated target genes. Results There were 18 differentially expressed miRNAs in two groups, which 17 were up-regulated and 1 was down-regulated. Most of the target genes with differential miRNA were concentrated in the signal transduction pathways related to cancer or body metabolism. Among the differentially expressed miRNAs, miR-577, miR-182-5p and miR-4286 are closely linked. They co-regulate the expressions of CACNA1C, MECP2, FOXK1, LPP, SH3TC2, Smad and MDM4. Conclusion There are significantly differential expressions of miRNAs in endometrium from AEH and Normal endometrial tissue. These miRNAs may be involved in the occurrence and development of AEH via adjusting different target genes to regulate the signal pathway.

Quality evaluation of guidelines on otitis media in children based on AGREEⅡ

Objective To evaluate the quality of clinical practice guidelines for otitis media in children, and to identify high quality guidelines for clinicians and pharmacists. Methods China National Knowledge Infrastucture, Wanfang Chinese Periodical Database, VIP Chinese Periodical Database, CBM database, PubMed database, Excerpta Medica Database and relevant websites, such as medlive.cn, GIN, AAP, NGC, NICE were searched by computer ranged from January 2000 to June 2019. Chinese search terms were otitis media, guidelines for clinical practice and children while English search terms were otitis media, middle ear, glue ear, guideline, guidance, child and children. Clinical practice guidelines for otitis media in children were included, appraisal of guidelines research and evaluation in Europe (AGREEⅡ) instrument was used to evaluate the quality of clinical practice guidelines. Results Ten guidelines were included, 5 of them were evidence-based (EB) guidelines, and 5 were non-EB guidelines. The consistency of 10 guidelines rated by three researchers was high. The evaluation results were as followed: the UK (ICC = 0.876, F = 22.271, P < 0.001), the USA 1(ICC = 0.861, F = 19.509, P < 0.001),the USA 2 (ICC = 0.819, F = 14.587, P < 0.001),Japan (ICC = 0.853, F = 18.531, P < 0.001),Korea (ICC = 0.856, F = 18.894, P < 0.001),New South Wales (ICC = 0.865, F = 20.277, P < 0.001),China (ICC = 0.836, F = 16.258, P < 0.001), Canada 1 (ICC = 0.894, F = 26.342, P < 0.001),Canada 2 (ICC = 0.880, F = 22.997, P < 0.001), the international guideline (ICC = 0.795, F = 12.605, P < 0.001), the differences were statistically significant. The overall quality of the 10 guidelines was not high. One was at grade A, four were at grade B, and five were at grade C. In each field, “clarity” and “scope and purpose” scored high (70.2% and 67.8% respectively), while “editorial independence” and “application” scored relatively low (34.0% and 29.7% respectively). Conclusion The quality of the included guidelines is relatively low. The quality of clinical practice guidelines for otitis media in children in China is not high. It is recommended to adopt EB methods to develop EB guidelines that are consistent with China′s national conditions.

Establishment of microbial limit test for Adefovir Dipivoxil Tablets

Objective To establish a method for microbial limit test of Adefovir Dipivoxil Tablets, through microbial limit test for Adefovir Dipivoxil Tablets and method suitability test based on Chinese Pharmacopoeia 2015 edition. Methods According to the four general principles of the Chinese Pharmacopoeia 2015 edition, microbial limit test of 1105 non-sterile products: microbial count method, microbial limit test of 1106 non-sterile products: control bacteria test and microbial limit test standard of 1107 non-sterile drugs. Sample 10 g was taken and pH 7.0 aseptic sodium chloride-peptone buffer was added to 100 mL. The sample was shaken vigorously and dissolved to obtain 1∶10 test solution. The test was conducted according to the plate method (pour method). Results The microbiological limit of Adefovir Dioxidel Tablets was tested by the dish method (pour method), and the lowest dilution level (1:10) was also effective in detecting viable aerobic bacteria, molds and yeast in the drug. The recovery ratio of all tested bacteria was within the range of 0.5-2. And the controlled bacteria grew well. Conclusion A microbial limit test method for Adefovir Dipivoxil Tablets is established.

Clinical study of Lamotrigine combined with Escitalopram Oxalate in the treatment of post-stroke epilepsy comorbid depression patients

Objective To explore the clinical effects of Lamotrigine combined with Escitalopram Oxalate in the treatment of post-stroke epilepsy comorbid depression patients. Methods From January 2015 to August 2018, 126 cases of patients with post-stroke epilepsy comorbid depression treated in Nanchong Central Hospital of Sichuan Province were selected as research objects and divided into the control group and the observation group by the method of randomized digital table, with 63 cases in each group. The control group was treated with Lamotrigine, and the observation group was given Lamotrigine combined with Escitalopram Oxalate, for 3 months of treatment. The frequency of seizures, adverse drug reactions, Hamilton depression scale (HAMD), National Institutes of Health Stroke scale (NIHSS), Barthel index (BI) and MMSE scores were compared between the two groups. Results After treatment, the seizure frequency, HAMD and NIHSS scores in the observation group were significantly lower than those before treatment, and lower than the control group (P < 0.05). The MMSE score and BI were significantly higher than those before treatment (P < 0.05), and significantly higher than the control group (P < 0.05). There was no statistically significant difference in the incidence of adverse reactions between the two groups (P > 0.05). Conclusion Lamotrigine combined with Escitalopram Oxalate can not only improve the depressive symptoms, the cognitive function, the neurological function and the quality of life of post-stroke epilepsy comorbid depression patients, but also does not increase the risk of recurrence of epilepsy and has high safety.

Pathway analysis of plasma metabolomics in the treatment of hypertensive left ventricular hypertrophy with Uncaria Compound Antihypertensive Tablets

Objective To explore the plasma metabolomics pathway of Uncaria Compound Antihypertensive Tablets in rats with spontaneous hypertension and left ventricular hypertrophy. Methods KEGG, HMDB, MetaboAnalyst and other methods were used to carry out the research on the signal pathway, metabolite transporter, related enzyme or molecular annotation and related properties, as well as the visualization of metabolite pathway. Results The metabolic pathways analyzed by MetaboAnalyst showed that 11 metabolites participated in 14 metabolic pathways. Influence values of 2 pathways (pentose phosphate pathway and purine metabolism) were higher (P < 0.05). Conclusion Uncaria Compound Antihypertensive tablet intervenes the pathologic processes of pentose phosphate pathway and purine metabolic biosynthesis pathway in spontaneous hypertensive left ventricular hypertrophy rats.

Mechanism of Sanqi Oral Liquid in the treatment of diabetic nephropathy based on network pharmacology

Objective To explore the mechanism of Sanqi Oral Liquid in the treatment of diabetic nephropathy. Methods According to the ADME characteristics of the drug, TCMSP platform was used to screen the active ingredients and targets of Sanqi Oral Liquid, which was developed with the compound of Sanqi and Huangqi, and the component target network was constructed. Retrieval of diabetic nephropathy related targets based on GeneCards database, DisGeNET database and OMIM database. The target of active drugs, active ingredients and genes was constructed by using the software of Cytoscape, and the target information was analyzed by using the database of DAVID and the biological process and pathway enrichment were analyzed. Results There were 27 active components were screened, involving 191 targets, and 118 potential targets related to disease targets. The therapeutic effect of diabetic nephropathy was mainly achieved by regulating PPARG, IL-6 and age-range signaling pathway, fluid shear stress and atherosclerosis, IL-17 signaling pathway, HIF-1 signaling pathway, etc. Conclusion Sanqi Oral Liquid can treat diabetic nephropathy through multi-target and multi-channel, which provides theoretical basis and scientific basis for explaining its therapeutic mechanism.

Visual analysis of knowledge mapping of Xiaozhiling in the treatment of rectal prolapse based on CiteSpace

Objective To explore the development status, research hotspots and frontier dynamics in the field by using systematic bibliometric analysis and visual analysis based on “Xiaozhiling in the treatment of rectal prolapse”. Methods Based on all relevant documents collected by CNKI from 1985 to 2017, the econometric analysis was carried out. At the same time, CiteSpace V was used to draw knowledge map from the aspects of document time distribution, author, organization, key words, etc. Results A total of 317 related literatures were obtained. Through econometric analysis, high-yield literature organization, representative researchers and their teams were found. The results of visual analysis showed that the main research directions and trends were “point injection” and “efficacy observation”. Conclusion Although Xiaozhiling is effective in the treatment of rectal prolapse, there are few studies on its mechanism, indicating the direction for future research.

Effect of Wenyang Huayu Tuihuang Decoction in the treatment of hepatitis B-related chronic and acute liver failure

Objective To observe clinical efficacy of Wenyang Huayu Tuihuang Decoction in the treatment of hepatitis B-related chronic and acute liver failure (ACLF). Methods From March 2016 to March 2018, 70 patients with hepatitis B-related ACLF were selected from Department of Hepatology in the First Affiliated Hospital of Guangxi University of Traditional Chinese Medicine. They were divided into control group and observation group by using random number table method with 35 cases in each group. Control group was treated with western medicine. Observation group was treated with Wenyang Huayu Tuihuang Decoction on basis of control group. The treatment course was 8 weeks, and observation period was 24 weeks. Clinical symptom score, laboratory indexes [total bilirubin (TBiL), glutamic-pyruvic transaminase (ALT), glutamic oxalacetic transaminase (AST), albumin (ALB), prothrombin altivity (PTA), blood amrnonia] efficacy, adverse reactions and survival rate after 24 weeks were observed between groups. Results After treatment, clinical symptom scores of two groups were lower than before treatment, and observation group was lower than control group, the difference was statistically significant (P < 0.05). After treatment, TBiL, ALT, AST and blood ammonia of two groups were lower than before treatment, ALB and PTA of two groups were higher than before treatment, observation group improved significantly than control group, the difference was statistically significant (P < 0.05). After treatment of 8 weeks, total effective rate of observation group was higher than that of control group, the difference was statistically significant (P < 0.05). No significant adverse reactions occurred in two groups during treatment. The survival rate after treatment of 24 weeks in observation group was significantly higher than that in control group (P < 0.05). Conclusion Wenyang Huayu Tuihuang Decoction is helpful to improve the clinical symptoms of patients with hepatitis B-related ACLF, improve the quality of life and efficacy, and the survival rate is high, which is suitable for clinical promotion.

Clinical observation of acupuncture combined with medicine in the treatment of hypertension with atherosclerosis

Objective To observe the clinical effect of Yangmai Huatan Decoction combined with acupuncture in the treatment of hypertension with atherosclerosis. Methods From June 2017 to September 2018, 100 cases of vertigo patients were selected from the outpatient department and ward of Department of Acupuncture and Moxibustion, Heilongjiang Academy of Sciences of Traditional Chinese Medicine. The patients were divided into combined treatment group and control group, each group with 50 cases. The final effective cases were 84 cases, 43 cases in combined treatment group and 41 cases in control group. The patients in control group were treated with Western medicine standardized antihypertensive, antiplatelet, hypolipidemic and other symptomatic treatment. On this basis, the patients in combined treatment group were treated with Yangmai Huatan Decoction and acupuncture. Patients of two groups were treated continuously for two months. Before and after treatment, blood pressure, arterial media thickness (IMT), resistance index (RI), serum anti oxidant and inflammatory factors were observed in two groups. Results The levels of systolic and diastolic blood pressure in two groups were lower than those before treatment, the difference was statistically significant (P < 0.05). IMT of bilateral common carotid artery in two groups after treatment was thinner than that before treatment, and the change in combined treatment group was more significant than that in control group, the difference was statistically significant (P < 0.05). RI of bilateral renal arteries in two groups after treatment was lower than that before treatment, and the change in combined treatment group was more significant than that in control group, the difference was statistically significant (P < 0.05). Levels of superoxide dismutase and malondialdehyde in two groups after treatment were higher than those before treatment, the levels of tumer necrosis faltor-α and interleukin-6 were lower than those before treatment, and the changes in combined treatment group were more significant than those in control group, the difference was statistically significant (P < 0.05). Conclusion The combination of acupuncture and medicine can effectively delay the progression of atherosclerosis in patients with hypertension, which is significantly better than western medicine alone in reducing blood pressure, lipid and antiplatelet therapy. This therapeutic effect is independent of the decrease of blood pressure, and may play a therapeutic effect by inhibiting the inflammatory response and oxidative stress injury in patients.

Professor Yu argued from the wet, for deficiency of element kidney, external feeling of wind heat, dampness in IgA nephropathy, treatment to cleaning stagnated and promoting diuresis; for spleen and kidney deficiency of both qi and yin, fluid-dampness intrinsic nephrotic syndrome, treatment to dispelling wind-evil and promoting diuresis; for spleen and kidney deficiency of both qi and yin, damp-turbidity blockade chronic renal insufficiency, treatment to supperting healthy energy and clearing heat-dampness; for upper hyperactivity of liver yang, phlegm dampness blocking collaterals hypertensive nephropathy, treatment to humidification expectorant; for deficiency of both qi and yin, internal obstruction of wet blood stasis diabetic nephropathy, treatment to eliminate dampness and dredging channels, while has achieved good results.

Professor Cui Yinglin uses the prescription to treat three cases of insomnia

Professor Cui Yinglin used evidence to discuss treatment of insomnia, returning to classic treatments, and dialectical flexibility, while the use of drugs is subtle and appropriate. The professor treats the evidence with the method of stagnation and evil, while treats the deficiency syndrome with the method of supplementation, taking into account the appearance and nature of the disease, and based on meridians. According to the patient′s different conditions, he ingeniously increases or decreases the drug based on the classic prescription, the effect is very rapid. Discussion on the new strategy of Compound Chinese Medicine for Reinforcing Kidney for preventing and treating atherosclerosis from the perspective of lipophagy

Atherosclerosis (AS) is a multifactorial disease closely related to aging, and lipid accumulation is a key pathological factor. Lipophagy as an important sub-process of autophagy, it can selectively degradate intracellular lipid droplets via lysosome-mediated and reduce lipid deposition, which can be used as a new target for AS research. Adhering to the basic pathogenesis of deficient root and excessive branch, and the basic idea of the combination of disease and syndrome of AS, based on the interaction between autophagy and lipid metabolism in AS process, combined with the preliminary research results that the intervene therapy of AS, the Compound Chinese Medicine for Reinforcing Kidney can effectively delay aging and regulate lipid metabolism, this artical intends to further explore the research strategy of the Compound Chinese Medicine for Reinforcing Kidney prevents and treats AS from the perspective of lipophagy.

Study on traditional Chinese medicine syndromes of ulcerative colitis based on factor analysis and cluster analysis

Objective To explore the classification and distribution of traditional Chinese medicine (TCM) syndromes of ulcerative colitis. Methods The cases come from the Department of Gastroenterology of outpatients and inpatients, Dongfang Hospital of Beijing University of Chinese Medicine from November 2016 to January 2019. According to the quality control requirements of statistics, a questionnaire for TCM syndromes of ulcerative colitis was compiled, and four diagnostic data were collected according to the clinical design plan. The four diagnostic information of 120 patients was studied by factor analysis and cluster analysis, so as to get the classification and distribution rule of the main TCM syndromes of ulcerative colitis. Results Based on the factor analysis and cluster analysis of the syndrome information of ulcerative colitis patients, three main syndrome types were obtained: large intestine damp heat and spleen deficiency damp accumulation syndrome (54 cases, 45%), cold heat mixed and liver stagnation spleen deficiency syndrome (34 cases, 28%), liver stagnation spleen deficiency and spleen yang deficiency syndrome (32 cases, 27%). Conclusion Factor analysis and cluster analysis can objectively analyze the classification and distribution of TCM syndromes of ulcerative colitis, and the characteristics of TCM syndromes of ulcerative colitis can be found.

Medical record review of 1 case of idiopathic pulmonary arterial hypertension and literature review

The diagnosis and treatment process of a patient with idiopathic pulmonary arterial hypertension who was repeatedly hospitalized in the Department of Respiratory and Critical Care Medicine of West China Hospital of Sichuan University (“our hospital” for short) from October 2013 to June 2016 was retrospectively analyzed, and the treatment and nursing experience of pulmonary arterial hypertension was summarized. The patient was a young female, who had chest tightness after the activity, for more than 3 years, aggravated with syncope and sought treatment in our hospital. The patient was diagnosed as severe pulmonary hypertension by right cardiac catheterization, through cardiac strengthening, diuresis and targeted treatment. After treatment, the patient′s condition improved and was discharged. Because the patient was diagnosed as severe pulmonary arterial hypertension at the first time in our hospital, the best time for treatment has been missed, resulting in repeated aggravation of hospitalization. Pulmonary arterial hypertension as a rare disease, there are relatively few clinical cases, and the medical staff in primary hospitals have a serious lack of understanding of this disease and grasp of the key points of treatment and nursing, leading to missed diagnosis or misdiagnosis of the disease and delayed treatment opportunity. This paper reviews the diagnosis, treatment and nursing progress of pulmonary arterial hypertension through case review and literature review. It is expected to provide reference and guidance for the future clinical treatment of pulmonary arterial hypertension.

Evaluation of the effect of PDCA circulation method on reducing the intensity of antimicrobial use under JCI-HIMSS7 double standard

Objective To probe into the PDCA cycle method to reduce the effect of the use intensity of antibiotics (AUD), taking the opportunity of the hospital JCI re-examination and under the premise of fully implementing the JCI standard of the International Medical and Health Agency Certification Joint Commission. Methods The information on the AUD from January to November of 2018 of Huangshi Central Hospital, Edong Healthcare Group by the Sichuan Meikang PASS Clinical Pharmacy Management System were collected. The PDCA team was build, the current problems were analyzed through the current situation survey and the PDCA cycle method was implemented to intervene in conjunction with the JCI standard. The data in pre-control phase (from January to March 2018), control phase (from May to July 2018), and post-control phase (from September to November 2018) were compared. Results After implemented the PDCA cycle, the AUD was decreased from 53.38 in January 2018 to 38.86 in November 2018, with a decrease of 27.20%, there was a statistically significant difference (P < 0.05). Conclusion Combined with the JCI standard to analyze the high AUD, the use of PDCA cycle method has a significant effect on effectively to reduce the AUD , which plays an important role in promoting clinical rational drug use and ensure patient drug safety.
