Analysis of correlation between depression and motor dysfunction in Parkinson′s disease

Objective To investigate the correlation between depression and motor dysfunction in patients with Parkinson′s disease (PD). Methods A retrospective case-control study was adopted in this study. 800 PD patients in neurology clinic were enrolled in Beijing Tiantan Hospital from April 2008 to August 2015. All patients were assessed with Center for Epidemiologic Studies Depression Scale (CES-D) and divided into the depression group and the non-depression group. The two groups were compared with general data on demographics, unified Parkinson′s disease rating scale (UPDRS), Hoehn-Yahr stage and mini-mental state examination (MMSE) scores respectively. The Spearman rank correlation coefficient was used to analyze the correlation between depression and motor dysfunction. Logistic regression was applied to explore risk factors of PD depression. Results 362 cases (45.3%) were identified as suffering from depression. In the depression group, the proportion of patients with Hoehn-Yahr stage greater than 2.5 was higher than that of the non-depression group (PD). The UPDRS part Ⅲ scores, the UPDRS scores of rigidity items, tremor items and bradykinesia items of the depression group were significantly higher than those of the non-depression group (P < 0.01). There was significant correlation between CES-D scores and UPDRS motor scores (r=0.405, P < 0.01). Logistic regression analysis showed: female, higher score in UPDRS partⅠ/Ⅱ, gait and posture disturbance, faster progression of motor symptoms were risk factors of PD depression (P < 0.05). Conclusion Depression is one of the common symptoms of PD and correlates with motor dysfunction.

Effect of NF-κB p65 gene silencing on proliferation and apoptosis of rheumatoid arthritis synovial cells

Objective To investigate the effect of NF-κB p65 gene silencing on proliferation and apoptosis of rheumatoid arthritis synovial HFLS-RA cells. Methods NF-κB p65 siRNA was transfected into HFLS-RA cells by cationic liposome LipofectamineTM2000, and the expression of NF-κB p65 mRNA and protein in HFLS-RA cells was detected by RT-PCR and Western blot respectively. The proliferation and apoptosis of HFLS-RA cells were separately detected by MTT and flow cytometry assay. Results Compared with the blank control group and the negative control group, the expression of NF-κB p65 mRNA, protein and cell proliferation rate in the HFLS-RA cells transfected with siRNA was decreased, while the apoptosis rate was increased, and the differences were statistically significant (all P < 0.05). Conclusion NF-κB p65 gene plays an important role in the proliferation and apoptosis of synovial HFLS-RA cells in rheumatoid arthritis, and silencing its expression can inhibit the proliferation of synovial HFLS-RA cells and induce its apoptosis.

Expression and clinical significance of p-gp, MRP and BCRP in glioma tissues

Objective To investigate the expression and clinical significance of P- glycoprotein (P-gp), multidrug resistance associated protein (MRP) and multidrug resistance associated protein (BCRP) in brain gliomas. Methods Fifty-six cases of brain glioma resected in the First People's Hospital of Wuan from July 2007 to July 2016 were selected as the observation group, and 10 normal brain tissues of brain trauma in the same period were selected as control group. The expressions of P-gp, MRP and BCRP in specimens of two groups were detected by immunohistochemistry (SP), and the relationship between the expression and clinical pathological features of glioma was analyzed. Results The P-gp, MRP, BCRP positive expression rates of the observation group was 42.86% (24/56), 76.79% (43/56), 46.43% (26/56), respectively, was significantly lower than the control group of 0% (0/10), 10% (1/10), 0% (0/10) (P < 0.01). The positive rates of P-gp, MRP and BCRP in glioma was related to the pathological stage and histological grade of glioma, the higher the pathological stage, the higher the positive expression rate of P-gp, MRP and BCRP. The lower the histological grade, the higher the positive expression rates of P-gp, MRP and BCRP, and the differences were statistically significant (P < 0.01). Spearman rank correlation analysis showed that the positive rate of P-gp expression in glioma tissues was positively correlated with the positive rates of MRP and BCRP (Rs = 0.612, 0.537, P < 0.05). Conclusion The expressions of P-gp, MRP and BCRP are highly expressed in gliomas, and the expression level is related to the pathological stage and histological grade, and the three expressions are positively correlated.

Analysis on FGFRS genetic mutation of fetuses with short limbs deformity

Objective To identify the fibroblast growth factor receptor 3 (FGFR3) mutation of fetuses with short limbs deformity and carry out genetic counseling by fetuses found in ultrasound screening. Methods The genomic information of FGFR3 full 17 exon amplification from UCSC Genome Bioinformatics, including 2 genome transcripts were extracted as a standard sequence, 8 pairs of primers were designed. 5 fetuses suspected to be with short limb deformities in the First Affiliated Hospital of Hebei North University from January 2014 to December 2016 were selected and the amniotic fluid of the fetuses in high-risk was collected for detection of mutation of FGFR3 gene by polymerase chain reaction and Sanger gene sequencing. The mutation of the sequencing of FGFR3 gene was carried out by CodonCode Aligner software. Results Among the 5 fetuses with short limbs deformity, 1 case was carried mutations C.1138G> A (P.Arg380Gly) on the 7th (FGFR3-7-IS1, transcript NM_000142.4). FGFR3 mutation were not found in the other 4 cases. Genetic counseling was conducted on 5 cases of fetal parents. Conclusion The detection of prenatal FGFR3 gene mutation in suspected patients using gene sequencing could effectively prevent the birth of short limb deformity patients.

The research progress of Th17/Treg imbalance in pathogenesis of pulmonary disease

T helper 17 cells (Th17) with pro-inflammatory activity and regulatory T cells (Treg) cells that maintain immune tolerance are important immune cells in humans. They are closely related in differentiation and functional constraints to each other, and there is a dynamic balance. Imbalance between the two can lead to autoimmune diseases, tumors, persistent infection diseases. In recent years, with the study of the common diseases of the respiratory system deeply, of Th17 cells and Treg cells were found to be associated with more lung disease pathogenesis, immune imbalance of Th17/Treg cells in patients with chronic obstructive pulmonary disease, lung cancer, pulmonary sarcoidosis and asthma among the adjustment mechanism. This article describes the progress of Th17 cells, Treg cells and Th17/Treg imbalance in lung related diseases.

Experimental research progress in the effect of autophagy on cells by electroacupuncture

Cell autophagy is widely used in the field of molecular biology recently, the related experimental studies of Alzheimer′s disease, cerebral ischemia reperfusion, peripheral nerve regeneration and spinal cord injury were reviewed in this paper. The purpose of this paper is to explore the effect of the electroacupuncture on autophagy in the disease process and induces its possible mechanism and way, to find new theoretical basis for clinical electroacupuncture treatment.

Research progress on the current treatment status and chemotherapeutic drugs of glioblastoma multiforme

Glioblastoma multiforme (GBM) is the most deadly type of heterogeneous brain tumor. The survival time of patients with pleomorphic glioblastoma is 8 to 15 months. GBM is driven by complex signaling pathways, the treatment of which has always been considered as the most challenging, the standard treatment for GBM includes surgery, radiation therapy, chemotherapy, and combination therapy. In this paper, the tumor interneoplasms of GBM and its heterogeneity and the latest chemotherapy drugs and mechanism are reviewed, this paper focuses on some natural synthesis and new anti-glioma drugs, which are also the main research focus of researchers today.

Investigation on the status of medical students' health literacy and analysis of influencing factors

Objective To understand the status of health literacy and analyze its influencing factors among medical college students, and provide evidence for targeted implementation of health literacy education. Methods The questionnaire was designed by ourselves on basis of 66 Questionnaires of Chinese Citizens′ Health Literacy. A stratified cluster sampling method was used to randomly select 500 medical college students of North Sichuan Medical College on October 2015. Results The overall level of medical college students′ health literacy was 25.31%. In the 3 dimensions of basic health knowledge and concept, healthy lifestyle and behavior and basic skills, the literacy rate was 51.66%, 23.24% and 49.38% respectively. The single factor analysis showed that the health literacy level of female students was higher than that of male students (χ2=4.003, P < 0.05), non rural students was higher than of rural students (χ2=4.286, P < 0.05), and senior students was higher than that of lower grade students (χ2=49.649, P < 0.05). Multivariate Logistic regression analysis showed that the rate of health literacy among medical students was related to gender (OR = 2.064, 95%CI: 1.240-3.434, P < 0.05) and grade (OR = 7.013, 95%CI: 3.900-12.614, P < 0.05). Conclusion The general level of health literacy of medical college students needs to be improved, especially boys and junior medical students, and schools should pay attention to health education work and carry out targeted interventions according to different objects.

Investigation on the medication compliance of patients with chronic hepatitis B receiving free Entecavir

Objective To investigate status of the medication compliance of patients with chronic hepatitis B receiving free Entecavir. Methods A total of 401 patients with chronic hepatitis B receiving free Entecavir and participated in the clinical study of Hubei Provincial Hospital of Traditional Chinese Medicine from June 2012 to April 2016 were taken as objects, followed-up for 48 weeks, the investigation and quality control was conducted according to the dose of drug use and the amount of drug recovery from the patients' medication log card, so as to evaluate the medication compliance of patients. Results The medication compliance of 24, 36 weeks in patients with chronic hepatitis B was better than those of 12, 48 weeks (P < 0.01). The compliance rate of one year was 98.4%, full compliance was accounted for 32.0%, and the percentage of compliance >95% d was 92.4%. The medication compliance of patients with hepatitis B cirrhosis was better than that of mild to moderate chronic hepatitis B (P < 0.01). The main reason of missing drugs was forgotten. Conclusion Although the medication compliance of patients with chronic hepatitis B receiving free Entecavir is high, but the patients still lack the knowledge about the disease. It is necessary to strengthen the patients' cognitive education of the disease and improve the compliance.

The correlation study between serum C1q, high sensitivity C-reactive protein and coronary atherosclerotic heart disease

Objective To explore the correlation between serum C1q, hypersensitive C-reactive protein(hs-CRP) and coronary atherosclerotic heart disease(CHD). Methods From October 2016 to April 2017, 406 clinical data of patients treated in the Department of Cardiology, Dongfeng General Hospital, Hubei University of Medicine were analyzed retrospectively and divided into the control group (200 cases) and the CHD group (206 cases) according to the history of percutaneous coronary intervention coronary angiography or whether the coronary artery stenosis was greater than 50% by coronary angiography. The levels of C1q and hs-CRP were compared between the two groups. The risk factors of CHD were analyzed by Logistic regression analysis. Results The serum C1q in the CHD group was lower than that of control group, and the hs-CRP in the CHD group was higher than that of control group, with statistically significant differences (P < 0.05). Logistic regression analysis indicated that in the coronary heart disease group the age (OR = 1.036, 95%CI =1.009-1.064, P < 0.05), diabetes (OR = 4.629, 95%CI = 2.159-9.924, P < 0.05), LDL-C (OR = 2.965, 95%CI = 1.286-6.835, P < 0.05), Glu (OR = 1.033, 95%CI = 1.246-1.856, P < 0.05), hs-CRP (OR = 1.509, 95%CI = 1.225-1.859, P < 0.05) were the risk factors for CHD and the C1q (OR = 0.991,95%CI = 0.983-0.998,P < 0.05) was the protective factor. Conclusion Serum C1q is the protective factor and hs-CRP is the risk factor in CHD and both can be used as clinically reliable diagnosis and evaluation index for CHD.

The correlation of retinol binding protein 4, lipoprotein (a) level and the severity of coronary artery disease in Uygur elderly patients with acute coronary syndrome

Objective To investigate the relationship between the levels of retinol binding protein 4, lipoprotein (a) and the severity of coronary artery disease in Uygur elderly patients of acute coronary syndrome (ACS). Methods From January 2013 to December 2016, 220 cases of Uygur elderly patients underwent coronary angiography and ≥ 65 years old were retrospectively analyzed and divided into Uygur elderly ACS group (150 cases) and non ACS group (70 cases) according to the clinical diagnostic criteria. Patients in the Uygur elderly ACS group were divided into low-scored groups (45 cases, 0-29), middle-scored group(44 cases, 30-59) and high-scored group(61 cases, ≥60) according to the Gensini score. The retinol binding protein 4 and lipoprotein(a)levels were compared between the Uygur elderly ACS group and non ACS group. The correlation between retinol binding protein 4, lipoprotein (a) and the Gensini score were analyzed. Results The Gensini score, retinol binding protein 4 and lipoprotein(a) in the Uygur elderly ACS group were higher than those of non ACS group. The retinol binding protein 4 and lipoprotein(a) level were increased with the increasing of Gensini score. The Gensini score was positively correlated with the level of retinol binding protein 4 and lipoprotein(a)(r = 0.501、0.704, all P = 0.000). Logistic regression analysis showed that high retinol binding protein 4 and lipoprotein (a) levels were the independent risk factors of high Gensini scores in Uygur elderly patients. Conclusion The elevated levels of retinol binding protein 4 and lipoprotein(a) are associated with the severity of coronary artery disease and those are risk factors for the aggravation of coronary artery disease. In clinical work, the indicators of retinol binding protein 4 and lipoprotein (a) should be detected in Uygur elderly patients of ACS and early prevention should be carried out and that have important clinical significances for Uygur elderly patients of ACS.

Nutritional status and ECOG score in patients after hematopoietic stem cell transplantation

Objective To analyze the nutritional status and eastern cooperative oncology group (ECOG) scores of patients underwent hematopoietic stem cell transplantation. Methods From June 2012 to March 2017, 95 patients with hematopoietic stem cell transplantation in Hebei Yanda Lu Daopei Hospital were selected as research objects. ECOG scale was used to evaluate the patients′performance status. The weight of the patients was recorded before and after operation of hematopoietic stem cell transplantation. Nutritional status of patients were analyze on the bases of ECOG scores and weight changes. The platelet (PLT), red blood cell (RBC), hemoglobin, total protein and albumin of peripheral blood were compared before and after transplantation. Results ①After operation of hematopoietic stem cell transplantation, 82.11% of patients lost weight, while 17.89% gained weight. ②After transplantation, 71.58% of the patients in ECOG score were concentrated at 2-3 levels, 2.10% of patients had no change in physical status and 4.21% of the patients had totally lost the living ability. ③After transplantation, RBC and PLT in the patients were lower than those of before transplantation, with statistically significant differences (P < 0.05). Conclusion Hematopoietic stem cell transplantation can significantly affect the nutritional status of patients and subsequently affect the life quality. Therefore, the preoperative of nutritional status should be valued in patients who underwent hematopoietic stem cell transplantation and appropriate nutritional interventions should be taken.

Comparative study of ABCD2 and ABC2D2 in predicting risk of stroke after transient ischemic attack

Objective To explore the predictive value of ABCD2 criteria and ABC2D2 criteria method for stroke risk in transient ischemic attack (TIA). Methods One hundred and thirty-nine patients with TIA from March 2014 to September 2016 in Suzhou Municipal Hospital Affiliated to Anhui Medical University were divided into low risk, moderate risk and high risk group according to ABCD2 and ABC2D2 criteria, the incidence of stroke in 7 d and 1 year was observed. The stratification of the risk of stroke risk in patients with TIA was evaluated. The prediction value of the risk of stroke in 7 d, 1 year after TIA was compared under the ROC curve (AUC). Results According to the ABCD2 criteria, the patients were divided into 37 cases of low-risk group (0-3 points), 86 cases of middle-risk group (4-5 points) and 16 cases of high-risk group (6-7 points). The risk of stroke in 7 days was 5.41%, 26.74% and 56.25%, the risk of stroke in 1 year was 13.51%, 40.70% and 68.75%, the differences were statistically significant (P < 0.05). According to the ABC2D2 criteria, the patients were divided into 24 cases of low-risk group (0-3 points), 78 cases of middle-risk group (4-6 points) and 37 cases of high-risk group (7-9 points). The risk of stroke in 7 d was 0.00%, 16.67% and 56.76%, the risk of stroke in 1 year was 4.17%, 30.77% and 70.27%, the differences were statistically significance (P < 0.05). ABC2D2 criteria were used to predict that the area of the ROC curve of the risk of stroke in patients with TIA in 7 d (1 year) was higher than that of ABCD2, the differences were statistically significance (P < 0.05). Conclusion The prediction value of ABC2D2 criteria is higher than that of the ABCD2 scoring method, which is a more effective method to predict the risk of stroke after TIA.

Clinical effect of early hyperbaric oxygen combined with standard large trauma craniotomy in the treatment of severe traumatic brain injury

Objective To study the clinical effect of early hyperbaric oxygen combined with standard large trauma craniotomy in the treatment of patients suffered from severe traumatic brain injury. Methods The clinical data of 70 cases with severe traumatic brain injury in Hebei People′s Hospital from March 2013 to January 2016 was analyzed retrospectively. 35 cases of control group received standard large trauma craniotomy treatment and 35 cases of observation group received early hyperbaric oxygen combined with standard large trauma craniotomy treatment. The recovery conditions of disease of the two groups were assessed by Glasgow coma scale (GCS) before treatment and after treatment for 4 months; the nerve function indices in serum and amino acid neurotransmitters were measured by enzyme-linked immunosorbent assay (ELISA). Results The GCS scores, serum nerve function indices [nerve growth factor (NGF), glial fibrillary acidic protein (GFAP), S-100B protein (S-100B)], and the contents of amino acid neurotransmitters [glutamic acid (Glu), γ-aminobutyric acid (GABA), aspartic acid (Asp), glycine (Gly)] of both groups after treatment were significantly improved compared with those before treatment (P < 0.05), and the improvement of the indices above in the observation group was significantly higher than those of the control group, the differences between the two groups were statistically significant (P < 0.05). Conclusion Early hyperbaric oxygen combined with standard large craniotomy can improve the efficacy of patients with severe traumatic brain injury, optimize the nerve function and inhibit the toxic effect of excitatory amino acids.

Predictive value on head and neck CTA combined with ABCD2 scale for the cerebral infarction in patients with transient ischemic attack of vertebral basilar artery

Objective To investigate the predictive value of head and neck CT angiography (CTA) combined with ABCD2 scale for the cerebral infarction in patients with transient ischemic attack (TIA) of vertebral basilar artery. Methods The clinical data of 92 patients with TIA admitted to the Affiliated Hospital of Inner Mongolia Medical University from February 2014 to February 2016 were collected. By using ABCD2 score method, CTA evaluation method and CTA combined with ABCD2 score method, according to patients' clinical symptoms and head and neck MRI diffusion-weighted, TIA was judged whether it was developed for cerebral infarction, and the proportion of cerebral infarction within 7 d after TIA was recorded. Results ABCD2 score method showed that, the incidence of cerebral infarction in high-risk group was the highest, followed by moderate and low risk group, the difference was highly statistically significant (Z = 8.394, P < 0.01); head and neck CTA showed that the incidence of cerebral infarction in patients with cerebrovascular stenosis was related to its degree of stenosis, followed by severe>moderate>normal/low, with significant difference (Z = 10.231, P < 0.05); according to CTA joint ABCD2 score, the incidence of cerebral infarction for low risk was 9.5%, which was significantly lower than that of high risk (27.6%), the difference was statistically significant (χ2=19.256, P < 0.05); the area under curve of ROC of the risk of cerebral infarction after 1 year predicted by ABCD2 score was 0.683 (95%CI: 0.606-0.794, P < 0.05), which of CTA combined with ABCD2 score was 0.801 (95%CI: 0.740-0.842, P < 0.05). Conclusion ABCD2 score can effectively predict the early development of cerebral infarction after TIA, and it is possible to further improve the prediction accuracy by checking with head and neck CTA inspection, which is also feasible for predicting the cerebral infarction of patients with TIA of vertebral basilar artery.

Predictive value of homeostasis model assessment insulin resistance on pregnancy outcome in infertility patients with PCOS undergoing IVF-ET

Objective To investigate the predictive value of homeostasis model assessment insulin resistance (HOMA-IR) on the pregnancy outcome in infertility patients with polycystic ovary syndrome (PCOS) undergoing in vitro fertilization and embryo transfer (IVF-ET). Methods The clinical data of 86 patients with PCOS treated with IVF-ET from August 2014 to August 2016 at the Reproductive Medicine Center of People′s Hospital of Hubei Medical College were analyzed retrospectively. According to HOMA-IR, those patients were divided into two groups including the observation group (insulin Resistant group, HOMA-IR≥1.66, n = 44) and control group (non-insulin resistance group, HOMA-IR <1.66, n = 42). The general situation, controlled ovarian hyperstimulation, pregnancy outcome and complications were compared in the two groups. Logistic regression analysis was performed on the factors influencing the outcome of IVF-ET in patients with PCOS. Results The levels of BMI, FTI, Glu, INS and HOMA-IR in the observation group were significantly higher than those in the control group (P < 0.01), and the number of oocytes retrieved, the clinical pregnancy rate and live birth rate was significantly lower in the observation group (P < 0.05). There was no significant difference in the normal fertilization rate, high quality embryo rate, embryo implantation rate, OHSS incidence, and early abortion rate between the two groups (P > 0.05). The BMI, age and infertility duration were risk factors on pregnancy outcome of patients with PCOS undergoing IVF-ET (P < 0.05), however AMH was protective factor (P < 0.05). Conclusion The lower HOMA-IR value may be a better predictor for a better pregnancy outcome in patients with PCOS undergoing IVF-ET.

Relationship between SFRP5 expression and insulin resistance in pregnant women with gestational diabetes mellitus

Objective To discuss the relationship between the expression of SFRP5 and insulin resistance of pregnant women with gestational diabetes mellitus (GDM). Methods Fifty cases of normal pregnant women in Maternal and Child Health Hospital of Inner Mongolia from January 2016 to March 2017 were selected as control group, and 50 cases with GDM were selected as experimental group. The level of fasting insulin, fasting blood glucose and blood lipid from 24 to 28 weeks of pregnancy was compared between the two groups. Enzyme-linked immunosorbent method was used to test the expression of SFRP5 in cubital vein serum of the puerperas in the two groups during cesarean section. Subcutaneous adipose of 30 cases between the two groups was taken respectively, and the expression of SFRP5 mRNA was tested by RT-PCR. Results SFRP5 was expressed in serum and adipose of the experimental group and the control group. The expression of SFRP5 in serum and adipose of the experimental group was lower than that in control group significantly (P < 0.01). The serum SFRP5 was negatively correlated with HOMA-IR, cholesterol, triglyceride, LDL-C (r = -0.5706, -0.1479, -0.1541, -0.1366, all P < 0.05), the serum SFRP5 and high density lipoprotein have the positive correlation relationship (r = 0.0866, P < 0.01). Conclusion The abnormal expression of SFRP5 is related to the occurrence and development of GDM. Through clinical monitoring the expression of SFRP5, the occurrence of insulin resistance can be discovered early, and reduce the morbidity of GDM. During pregnancy, offer good nutrition guidance and strict weight-control to pregnant women can reduce the occurrence of insulin resistance, and improve the pregnancy success rate in GDM.

Discussion of relationships between hormone, IGF-1 and growth and development in young girls with central precocious puberty

Objective To investigate the relationships between growth hormone, inslin-like growth inslin-like factor-1 (IGF-1) with growth and development in children with central precocious puberty. Methods Each 26 cases of central precocious puberty and normal healthy young girls in Xiangtan Central Hospital of Hunan Province from January 2016 to March 2017 were selected as the disease group and the healthy group respectively. The levels of sex hormones and IGF-1 were measured by peripheral venous blood, and the stature, bone age, ovary and uterus volume were measured and compared. The Pearson correlation analysis method was used to analyze the clinical relationships between the neutral hormone, IGF-1 and the growth , bone age, ovary and uterus volume in the disease group. Results The levels of serum FSH, luteinizing hormone (LH), estradiol (E2) and IGF-1 in the disease group were higher than those in the healthy group, and the differences were statistically significant (P < 0.01). The height, bone age, ovary and uterus volume in the disease group were higher than those in the healthy group, the differences were statistically significant (P < 0.01). In the disease group, the serum FSH, LH, E2 and IGF-1 levels were positively correlated with height, bone age, ovary and uterus volume (r > 0.3, P < 0.05). Conclusion In girls with central precocious puberty, the serum levels of FSH, LH, E2 and IGF-1 are abnormally high, which can affect growth and development, lead to short stature, and increase bone age, ovary and uterus volume.

Influence of laparoscopic and open radical nephrectomy for the effects, complications and survival rate of renal carcinoma

Objective To explore the influence of laparoscopic and open radical nephrectomy for the effects, complications and survival rate of renal carcinoma. Methods One hundred patients with renal carcinoma admitted to Guang′an People′s Hospital of Sichuan Province from March 2013 to March 2014 were selected as the study objects and divided into observation group (50 cases) and control group (50 cases) by random number table method. The observation group was treated with laparoscopic radical nephrectomy, the control group was treated with traditional open radical nephrectomy. The clinical effect, main postoperative complications and survival rates within 3 years of the two groups were observed. Results The length of surgical incision in the observation group was significantly shorter than that of control group, the intraoperative blood loss was less than that of control group, and the recovery time of intestinal function, leaving bed time, hospital stay after operation were all shorter than those of control group, the differences were all statistically significant (P < 0.05). The incidence of postoperative complications in the observation group was significantly lower than that of control group (P < 0.05). The postoperative 1-year survival rate of the observation group was higher than that of control group, but the difference was not statistically significant (P > 0.05). The postoperative 2-year, 3-year survival rates were significantly higher than those of control group (P < 0.05). Conclusion Laparoscopic radical nephrectomy has the advantages of simple operation, small pain and remarkable efficacy, which can significantly reduce surgical complications and improve the long-term survival rate of patients. It's worthy of clinical promotion.

Clinical value of thrombelastogram for the evaluation of coagulation function and prediction of metastasis in patients with moderate and advanced breast cancer

Objective To study and investigate the clinical value of thrombelastogram for the evaluation of coagulation function and prediction of metastasis in patients with moderate and advanced breast cancer. Methods One hundred and fourteen patients with moderate and advanced breast cancer admitted to Liaoning Cancer Hospital ("our hospital" for short) from January 2016 to January 2017 were selected and divided into two groups, with 56 cases in group Ⅰ (Ⅰ+Ⅱ stage of patients with breast cancer) and 58 cases in group Ⅱ (Ⅲ+Ⅳ stage of patients with breast cancer), of which, there were 18 cases with metastasis (metastasis group) and 40 cases without metastasis (no metastasis group) in group Ⅱ. Another 50 healthy controls in our hospital at the same time were selected as normal control group. The thrombelastogram was used to detect response time (R value), coagulation time (K value), maximum amplitude (MA value), angle. The prothrombin time (PT), activated partial thromboplastin time (APTT), fibrinogen (FIB) and thrombin time (TT) were tested by routine examination. Results The R value, K value, MA value, angle in group Ⅰ, Ⅱ were all higher than those of normal control group, and R value, K value, MA value, angle in group Ⅱ were all higher than those of group Ⅰ, the differences were all statistically significant (all P < 0.05). The PT, APTT, TT, FIB in group Ⅰ, Ⅱ were all higher than those of normal control group, but the differences were not statistically significant (P > 0.05), there were also no statistically significant differences of PT, APTT, TT, FIB between group Ⅰ and group Ⅱ (P > 0.05). The R value, K value, MA value, angle in metastasis group were all higher than those of no metastasis group, the differences were all statistically significant (P < 0.05), while there were no statistically significant differences of PT, APTT, TT, FIB between metastasis group and no metastasis group (P > 0.05). Conclusion Compared with routine coagulation function examination, thrombelastogram can evaluate the coagulation function of breast cancer more effectively, which has an important value for prediction of breast cancer metastasis.

A Meta-analysis of the effectiveness and safety of hyperthermia combined with intravenous chemotherapy for ovarian cancer

Objective To evaluate the effectiveness and safety of hyperthermia combined with intravenous chemotherapy for ovarian cancer. Methods The databases of PubMed, Cochrane library, EMbase, China national knowledge internet, the Chinese biomedical literature database were searched from building to June 22, 2017. Randomized controlled studies on the efficacy and safety of hyperthermia combined with intravenous chemotherapy in the treatment of ovarian cancer were included, the quality of the included studies was evaluated using the quality evaluation criteria of the Cochrane Handbook version 5.1.0, and the Review Manager 5.1.0 software was used for data analysis. Results Twenty studies including 1275 cases of ovarian cancer were included. The results of the Meta-analysis exhibited that compared with intravenous chemotherapy alone, hyperthermia combined with intravenous chemotherapy could increase effective rate [RR = 1.58, 95%CI: 1.42-1.77, P < 0.00 001], effective rate of ascites[RR = 1.63, 95%CI: 1.32-2.02, P < 0.00 001], effective rate of CA125 [RR = 1.33, 95%CI: 1.14-1.55, P = 0.0003], the improvement rate of life quality [RR = 1.60, 95%CI: 1.24-2.06, P = 0.0003].Meanwhile, hyperthermia combined with intravenous chemotherapy did not significantly increase the incidence of myelosuppression, nausea and vomiting and liver and kidney dysfunction (P > 0.05). Conclusion Hyperthermia combined with intravenous chemotherapy may be more effective for ovarian cancer, improve life quality whereas adverse reactions are similar.

Correlation analysis on the expression of serum IL-8, Fractalkine, MIP-3α and clinical features of lung cancer in patients with newly diagnosed lung cancer

Objective To analyze the correlation of the expression of serum interleukin-8 (IL-8), chemotactic factor (Fractalkine), macrophage inflammatory protein-3α (MIP-3α) and clinical features of lung cancer in patients with newly diagnosed lung cancer. Methods The clinical data of 90 patients with newly diagnosed lung cancer (newly diagnosed lung cancer group, 21 cases of small cell carcinoma and 69 cases of non small cell carcinoma) admitted to Zhongshan Hospital Affiliated to Dalian University from January 2013 to December 2015 and 90 healthy controls with normal results of outpatient physical examination at the same time (healthy group) was analyzed retrospectively. Firstly, the levels of serum IL-8, Fractalkine, MIP-3α between newly diagnosed lung cancer group and healthy group were compared. Secondly, the newly diagnosed lung cancer group was divided into different groups according to Ⅰ+Ⅱ stage and Ⅲ+Ⅳ stage, small cell carcinoma and non-small cell carcinoma, and the levels of serum IL-8, Fractalkine and MIP-3 were compared. At last, Pearson correlation test was used to analyze the relationship among the levels of serum IL-8, Fractalkine and MIP-3 in newly diagnosed lung cancer group. Results ①The levels of IL-8, Fractalkine, MIP-3α in newly diagnosed lung cancer group were higher than those in healthy group (P < 0.01). ②The levels of IL-8, Fractalkine, MIP-3α in Ⅰ+Ⅱ stage group were lower than those in Ⅲ+Ⅳ stage group (P < 0.01). ③The levels of IL-8, Fractalkine, MIP-3α in small cell carcinoma group were higher than those in non-metastasis group, but the differences were not statistically significant (P > 0.05). ④Pearson correlation analysis showed that there was linear correlation of IL-8 and Fractalkine (r = 0.598, P < 0.05), IL-8 and MIP-3α (r = 0.641, P < 0.05), Fractalkine and MIP-3α (r = 0.682, P < 0.05). Conclusion The levels of IL-8, Fractalkine, MIP-3α in patients with newly diagnosed lung cancer are higher than those of healthy persons, and increase with the escalation of the disease, but there are no significant differences between different pathological types. Three indicators are linearly related to each other and can be used for the condition judgment of patients with newly diagnosed lung cancer.

Long-term clinical effects of 2.0 mm microincision phacoemulsification for the patients with cataract and high myopia

Objective To study the long-term clinical effects of 2.0 mm microincision phacoemulsification for the patients with cataract and high myopia. Methods In this consecutive observational noncomparative clinical trial, a total of 40 patients (68 eyes) with cataract and high myopia were enrolled in the study from October 2015 to January 2016, who underwent 2.0 mm microincision cataract surgery. Preoperatively, all patients underwent a comprehensive systemic and ophthalmic examination including slit-lamp microscopy, uncorrected distance visual acuity (UCVA), best-corrected distance visual acuity (BCDVA), best-corrected near visual acuity (BCNVA), intraocular pressure (IOP), biometry (IOLMaster 500; Carl Zeiss Meditec, Jena, Germany), ocular ultrasound, corneal topography, corneal specular microscopy, and optical coherence tomography (OCT). Follow-up visits were conducted at 1 day, 1 week, 3 months and 1 year postoperatively. The outcomes were uncorrected corrected distance visual acuity(UDVA), best corrected distance acuitiy(BCDVA), best corrected near acuity (BCNVA ), contrast sensitivity(CS), surgically induced astigmatism (SIA), IOL position stability, endothelial cell count loss, corneal incision thickness and complications. Results A statistically significant improvement was observed in UDVA between preoperative and postoperative (P < 0.05), as well as for BCDVA (P < 0.05). Contrast sensitivity was significantly improved(P < 0.05), and the mean SIA was only (0.154±0.214)D at 1 year postoperatively. Effective lens positions of all IOLs remained well though 1 year postoperatively. Mean endothelial cell count loss was (6.21±19.79)% at 3 months postoperatively. There was statistically significant difference in CCT at the incision site between postoperative 1 week and 3 months (P < 0.05). Postoperatively major complications included 2 eyes (2.94%) with anterior capsule contraction at 3 months and 2 eyes (2.94%) with posterior capsule opacification at 1 year. Conclusion The 2.0 mm microincision phacoemulsification is safe and effective for cataract and high myopia,and the long-term clinical effects are excellent.

The comparison of effect, blood glucose and lipid metabolism in patients of schizophrenia in the treatment of Aripiprazole and Olanzapine

Objective To compare the curative effect, blood glucose and blood lipid metabolism of Aripiprazole and Olanzapine in the treatmen oft patients with schizophrenia. Methods From January 2016 to June 2017, 78 cases of patients with schizophrenia in the First Affiliated Hospital of Xinjiang Medical University were selected as research objects and divided into Aripiprazole group and Olanzapine group by random number table, with 39 cases in each group. The Aripiprazole group was given Aripiprazole with 10-30 mg/d and the Olanzapine group was given Olanzapine with 5-20 mg/d. Both groups were treated for 6 weeks. The positive and negative symptom scale was used to evaluate the curative effect before and after treatment. The blood biochemical index, body mass index(BMI) and the incidence of adverse reactions were compared between the two groups. Results The curative effect in the Aripiprazole group (84.62%) was higher than that of Olanzapine group (82.05%), but with no statistically significant difference (P > 0.05). After treatment, the blood glucose level, BMI in Olanzapine group were significantly higher than those of before treatment and those of Aripiprazole group, with statistically significant differences(P < 0.05). After treatment, triglycerides and alanine aminotransferase in Olanzapine group were significantly higher than those of before treatment, triglycerides in Olanzapine group was higher than that of Aripiprazole group, with statistically significant differences (P < 0.05). The incidence of adverse reactions such as akathisia, tremor, insomnia, excitement or agitation in the Aripiprazole group were high than those of Olanzapine group, with statistically significant differences (P < 0.05). Conclusion Aripiprazole has the same effect as olanzapine. Olanzapine has an effect on blood glucose, lipid metabolism and BMI in patients with schizophrenia. Aripiprazole tends to lead to a higher incidence of partial adverse reactions. In the process of clinical drugs use, individual differences in patients should be aware in the treatment of schizophrenia.

Clinical effect of Gangshu Lotion in rehabilitation of mixed hemorrhoid after operation

Objective To investigate the safety and efficacy of Gangshu Lotion in rehabilitation of mixed hemorrhoid after operation, to observe its influence on the change of prostaglandin E2 (PGE2) content in serum, in order to clarify its possible analgesic mechanism. Methods Sixty patients with mixed hemorrhoid admitted to Affiliated Hospital of Zunyi Medical College from January to July in 2017 were selected and divided into Chinese medicine group and western medicine group by random number table, with 30 cases in each group. Both groups were treated by Milligan-Morgan operation. After operation, the Chinese medicine group was treated by fumigation and sitz bath with self-made Gangshu Lotion. After operation, the western medicine group was treated by fumigation and sitz bath with povidone, both treatment until the wounds healed. All patients were assessed about wound pain, edema, hemafecia at 1 day, 3 days, 5 days after operation, and the levels of serum PGE2 were measured by enzyme-linked immunosorbent assay. Results The regression time of edema and wound healing of the Chinese medicine group were shorter than those of the western medicine group, with statistically significant difference (P < 0.05). 5 days after operation, the bleeding score of the Chinese medicine group was lower than that of the western medicine group, with statistically significant difference (P < 0.05). The scores of pain and the levels of PGE2 in the two groups at 3 days and 5 days after operation were significantly higher than those at 1 day after operation, and those of the Chinese medicine group were lower than those of the western medicine group, with statistically significant difference (P < 0.05). Conclusion Gangshu Lotion can effectively improve rehabilitation of mixed hemorrhoid after operation. The analgesic mechanism may be related to the decrease of the levels of serum PGE2.

Objective To investigate the value of (1,3)-beta-D-glucan dynamic detection in evaluating the efficacy of antifungal drugs in invasive fungal infections. Methods The levels of (1,3)-beta-D-glucan from 42 patients with invasive fungal infections were detected by fungi (1,3)-beta-D-beta-glucan assay kit, who were treated in the People′s Hospital of Wuzhou from March 2013 to March 2017, with analyzing the clinical data of the cases. Results After antifungal treatment, the levels of plasma (1, 3)-beta-D-dextran were significantly reduced in 40 patients, of which the levels of 5 patients decreased to normal levels after a small increase. Besides, the levels of 2 patients were not decreased to normal levels. Conclusion Dynamic detection of plasma (1,3)-beta-D-glucan in patients with invasive fungal infection is an effective indicator to evaluate the efficacy of antifungal drugs, which can guide the rational use of antifungal drugs in clinic.

Study of the methods of reducing the use of pain-relieving drugs during perioperative period of total knee arthroplasty

Objective To investigate the intervention of non-drug method for postoperative pain in total knee arthroplasty, in order to clarify whether perioperative non-drug therapy can relieve pain in postoperative patients and reduce the use of Celecoxib. Methods 203 patients underwent unilateral total knee arthroplasty in Department of Joint Surgery, Honghui Hospital Affiliated to Xi′an Jiaotong University Medical College from January 2016 to September 2017 were selected, then they were divided into group A (Celecoxib < 800 mg, 105 cases) and group B (Celecoxib ≥ 800 mg, 98 cases), according to whether the dosage of oral Celecoxib was ≥ 800 mg. The risk factors of increase the use of Celecoxib were investigated by single-factor analysis and multifactor Logistic regression analysis. Results The cases of general anesthesia + femoral nerve block anesthesia, "cocktail" intra-articular injection and postoperative psychological comfort, electrotherapy, cryotherapy in the group A were more than those in the group B, with statistically significant difference (P < 0.05). Multifactor Logistic regression analysis showed: general anesthesia + femoral nerve block, "cocktail" intra-articular injection and postoperative psychological comfort, electrotherapy were independent risk factors of increase the use of Celecoxib (P < 0.05). Conclusion The application of general anesthesia + femoral nerve block anesthesia, "cocktail" joint cavity injection and postoperative psychological comfort, electrotherapy can alleviate postoperative pain and reduce the use of analgesic drugs after total knee arthroplasty.

Effect of transrectal ultrasound conductance-guided administration drug in treating chronic bacterial prostatitis

Objective To compare the effect of transrectal ultrasound conductance-guided administration drug with oral medication in treating chronic bacterial prostatitis. Methods 80 patients with chronic bacterial prostatitis in the outpatient and hospitalization of the Nanjing Pukou Hosptial from January 2016 to January 2017 were selected and divided into two groups by envelope method. There were transrectal ultrasound conductance-guided administration drug group (group A) and oral administration group (group B), with 40 cases in each group. Both groups were treated with 21 days. The number of prostatic fluid leukocytes (WBC), the total integral of chronic prostatitis symptom (NIH-CPSI) and the total efficiency follow-up 1 month after treatment in the two groups were compared. Results The total efficiency, the number of prostatic fluid WBC and the total integral of NIH-CPSI in the group A were better than those in the group B, with statistically significant difference (P < 0.05). Conclusion Compared with oral medicine, the effect of transrectal ultrasound conductance-guided administration drug in treating chronic bacterial prostatitis is better.

Effect of concentrated Naikan therapy on implicit cognition bias in Alcohol dependent patients

Objective To investgate the effect of concentrated Naikan therapy on implicit cognition bias in Alcohol dependent patients using go/no-go association task (GNAT). Methods From October 2013 to December 2015, 28 patients met the criteria of alcohol dependence were enrolled as research group and matched sexual, age and education degree of 28 healthy persons were enrolled as control group. The research group was treated with concentrated Naikan therapy for 4 weeks. The right rate, response time and false rate were measured and compared by GNAT between the two groups. Results In the control group, there were no significant differences in right rate, response time and false rate between before and after treatment in test 1 and test 2 (P > 0.05). Before treatment, in the research group test 1, the response time was shorter, the right rate was higher and the false rate was lower than those of control group, with statistically significant differences (P < 0.05); in the research group test 2, the response time was longer, the right rate was lower and the false rate was higher than those of control group, with statistically significant differences (P < 0.05). After treatment, in the research group test 1, the response time was longer, the right rate was lower and the false rate was higher than those of before treatment, with statistically significant differences (P < 0.05); in the research group test 2, the response time was longer, the right rate was higher and the false rate was lower than those of before treatment, with statistically significant differences (P < 0.05). Conclusion Concentrated Naikan therapy can improve the implicit cognition of patients with alcohol dependent, which might be a useful methods for preventing the re-drinking behavior.

Effect of Reteplase combined with Tirofiban for the microcirculation and coagulation function of elderly patients with acute myocardial infarction

Objective To observe the effect of Reteplase combined with Tirofiban for the microcirculation and coagulation function of elderly patients with acute myocardial infarction. Methods The data of total 120 patients with acute myocardial infarction treated in Baoji Hospital of Traditional Chinese Medicine from March 2014 to March 2016 was analyzed retrospectively, and they were divided into control group and observation group according to different therapeutic methods, with 60 cases in each group. The control group was given Reteplase by intravenous drip, the observation group was given Reteplase combined with Tirofiban by intravenous drip. The indices of microcirculation, coagulation function before and 4, 24 h after treatment and related indices of fibrinolysis before and after treatment for 4 h between the two groups were observed and compared. Results Compared with before treatment, the peak intensity (PI) of the two groups 4, 24 h after treatment was enhanced, the area under curve (AUC) was increased and the QTd was shortened, the differences were all statistically significant (P < 0.05). 4, 24 h after treatment, the PI, AUC of observation group were higher than those of control group, the QTd was shorter than that of control group, the differences were all statistically significant (P < 0.05). At the same time, the falling range of ST-elevation in the observation group 4, 24 h after treatment was higher than that of control group, the time to get to the enzyme peak was faster than that of control group, the differences were all statistically significant (P < 0.05). Compared with before treatment, the thrombin time (TT), prothrombin time (PT), activated partial thromboplastin time (APTT) of the two groups 4, 24 h after treatment were prolonged, the contents of serum fibrinogen (FIB) in the two groups were decreased, while the contents of D-Dimer (D-D) were increased, the differences were all statistically significant (P < 0.05). 4, 24 h after treatment, the TT, PT, APTT of observation group were longer than those of control group, the content of serum FIB was lower than that of control group, the content of D-D was higher than that of control group, the differences were all statistically significant (P < 0.05). 4, 24 h after treatment, the levels of tissue plasminogen activator (t-PA) of the two groups were increased, while the levels of plasminogen (PLG), α2-plasmin inhibitor (α2-PI) and plasminogen activator inhibitor (PAI-1) were decreased, and the level of t-PA in observation group was higher than that of control group, the levels of PLG, α2-PI and PAI-1 were lower than those of control group, the differences were all statistically significant (P < 0.05). Conclusion Reteplase combined with Tirofiban can effectively improve the coronary microcirculation and coagulation function of elderly patients with acute myocardial infarction, which is useful to improve the blood supply of ischemic myocardium, reduce myocardial damage, it is worthy of clinical promotion and application.

Effects of DPP-4 inhibitor for the cognitive function of elderly patients with type 2 diabetes mellitus and Alzheimer disease

Objective To study the impact of dipeptidyl peptidase 4 (DPP-4) inhibitor Sitagliptin Phosphate on the cognitive function of elderly patients with type 2 diabetes mellitus and Alzheimer disease. Methods Eighty patients with type 2 diabetes mellitus and Alzheimer disease in Community Health Service Management Center of Dongcheng District in Beijing City from February 2016 to February 2017 were divided into two groups by random number table method, with 40 cases in each group. On basis of original therapeutic regimen, the control group was added with placebo, while the observation group was added with Sitagliptin Phosphate. The levels of blood glucose and cognitive function of the two groups were evaluated and analyzed before treatment and after treatment for 6 months. Results There were no significant differences of fasting blood glucose (FBG), glycated hemoglobin (HbA1c), and scores of mini-mental state examination (MMSE) between the two groups before treatment (P > 0.05). After treatment, the FBG and HbA1c in observation group were significantly lower than those before treatment (P < 0.05), the scores of MMSE were significantly higher than those before treatment (P < 0.05). After treatment, the FBG and HbA1c in control group were significantly lower than those before treatment (P < 0.05), while there was no statistically significant difference of MMSE scores before and after treatment (P > 0.05). After treatment, the scores of MMSE in observation group were significantly higher than those of control group (P < 0.05), but there were no statistically significant differences of FBG and HbA1c between the two groups (P > 0.05). Conclusion DPP-4 inhibitor can significantly improve the cognitive function of elderly patients with type 2 diabetes mellitus and AD.

Application effect of the health education pathway on cancer patients with chemotherapy:a Meta-analysis

Objective To systematically evaluate the application effect of the health education pathway on cancer patients with chemotherapy. Methods Studies of the randomized controlled trials of health education pathway on cancer patients receiving chemotherapy were researched in databases from establishment of the databases to December 2016, including Wanfang databases, CNKI, VIP. RevMan 5.3 software was used for statistical analysis. Results Twenty-four literatures and 2 494 patients had been incorporated into this project, including 1242 cases in health education pathway group and 1252 cases in the control group. The results of Meta analysis showed that the mastering of health knowledge (RR = 1.36,95%CI:1.22-1.51,P < 0.01), patients'satisfaction (RR = 1.19,95%CI:1.12-1.26,P < 0.01) in the health education pathway group were higher than those of control group, the anxiety score (SMD= - 1.90,95%CI:-2.44--1.37,P < 0.01) and phlebitis incidence (RR = 0.20,95%CI:0.05-0.77,P < 0.05) were lower than those of control group, with statistically significant differences. Conclusion The health education pathway has a better implementation effect compared with traditional health education, but it is still required for verification through multicenter and large sample randomized clinical trials.

The effects of non-gap nursing on the perioperative stress response in patients with glioma

Objective To explore the effects of non-gap nursing on perioperative stress response in patients with glioma. Methods From April 2015 to April 2017, 80 cases of patients with glioma treated in the First Affiliated Hospital of Harbin Medical University were selected and divided into the control group and the observation group by random number table, with 40 cases in each group. The control group was given routine nursing care and the observation group was given non-gap nursing on the bases of the control group. The perioperative stress indexes and the nursing satisfaction were compared between the two groups. Results The nursing satisfaction in the observation group(95.00%) was higher than that of control group (80.00%), with statistically significant difference (P < 0.05). There were significant differences in the stress indexes of heart rate and blood pressure in the two groups at the different times of admission time, 1 d before operation and 3 d after operation (P < 0.05). At the time of 1 d before operation, the indexes above in the two groups were all higher than those of admission time, and the control group increased more significantly. At the time of 3 d after operation, the indexes above in the two groups were all lower than those of 1 d before operation, and the observation group decreased more significantly, with statistically significant difference (P < 0.05). Conclusion The non-gap nursing can control the stress indexes better than the routine nursing on the perioperative patients with glioma. It is easier to be accepted by patients and the nursing satisfaction is more higher.

The intervention effects of Orem self-care model in patients of lymphoma with PICC tube during the intermittent period

Objective To explore the effects of Orem self-care model in patients of lymphoma with PICC tube during the intermittent period. Methods From September 2013 to January 2016, 68 patients with lymphoma treated in Harbin Medical University Cancer Hospital were selected and divided into Orem group and control group by random number table, with 34 patients in each group. The control group was given routine care during the intermittent period and the Orem group was given Orem self-care model on the basis the control group. The incidence of complications, self-care ability and the score of life quality were compared between the two groups. Results The incidence of complications in the Orem group (8.8%) was lower than that of control group (32.4%), with statistically significant difference (P < 0.05). After nursing, the score of self-care ability in the two groups were all higher than that of before nursing, and that in Orem group was higher than that of control group, with statistically significant difference (P < 0.05). After nursing, the scores of life quality such as social relationships, environment, physiology, psychology in the Orem group were all higher than those of control group, with statistically significant differences(P < 0.05). Conclusion Orem self-care model can improve the self-care ability of the patients of lymphoma with PICC tube during the intermittent period, reduce the incidence of complications, so as to promote the quality of life.

Correlation between anxiety and depression and self - management in elderly patients with type 2 diabetes mellitus

Objective To explore the relationship between anxiety, depression and self-management in elderly patients with type 2 diabetes mellitus. Methods From September to December 2016, a total of 78 elderly patients with type 2 diabetes mellitus in the Department of Endocrinology, the Affiliated Hospital of Southwest Medical University were selected as research objects. The hospital anxiety and depression scale and summary of diabetes self-care activities were used to investigate the level of anxiety, depression and self-management behavior of the patients and the correlation between anxiety, depression and self-management level were analyzed. Results The incidence of anxiety and depression were 41.03% and 44.87%. The total score of self-management was (18.35±3.64). Anxiety, depression and the total score of self-management were negatively correlated (r = -0.517, -0.507, all P < 0.05). Conclusion The psychological assessment should be enhanced and corresponding psychological nursing intervention measures should be taken in oder to improve the patients′ self-management level.

Influences of personalized continuation nursing intervention on anxiety and depression in patients with hypertensive intracerebral hemorrhage

Objective To explore the influences of personalized continuation nursing intervention on anxiety and depression in patients with hypertensive intracerebral hemorrhage. Methods From December 2015 to September 2016, 86 patients with hypertensive intracerebral hemorrhage treated in Harbin Medical University were divided into two groups according to the draw order, with 43 cases in each group. The control group received routine nursing care and the observation group received personalized continuation nursing intervention on the basis of routine nursing care. The blood pressure and the incidence of complications were compared between the two groups. The depression self-rating scale (SDS) and anxiety self-rating scale (SAS) were used to assess the psychological status of the patients. The independent living ability score were compared between the two groups. Results After intervention, the levels of systolic pressure and diastolic pressure, the incidence of complications, scores of SDS and SAS in the observation group were all lower than those of control group, with statistically significant difference (P < 0.05). After intervention, the score of sphincter control, social cognitive function, communication ability and activity ability in the observation group were significantly higher than those of control group, with statistically significant differences (P < 0.05). There was no statistically significant difference in the score of self-care ability between the two groups (P > 0.05). Conclusion Personalized continuous care intervention can effectively reduce the blood pressure of patients with hypertensive intracerebral hemorrhage, relieve anxiety and depression, reduce the incidence of complications and promote independent living ability.

The application of medical care integration in elderly patients with hip fracture

Objective To explore the effects of medical care integration in elderly patients with hip fracture. Methods From August 2012 to August 2016, 1200 cases of elderly patients with hip fracture in Traditional Chinese Medicine Hospital of Meishan City were selected as research objects and divided into the control group and the observation group by different intervention model, with 600 cases in each group. The scores of nursing quality, the excellent and good rate of hip function, the clinical treatment effects and the incidence of postoperative complications were compared between the two groups. Results The scores of diagnostic information mastery, nursing operation, professional nursing knowledge, medical and nursing measures implementation in the observation group were higher than those of control group, with statistically significant differences (P < 0.05). The excellent and good rate of hip function in the observation group was higher than that of control group, with statistically significant differences (P < 0.05). The protein reached the normal value time, indwelling catheter time, hospitalization time, the leaving bed time were all shorter than those of control group, with statistically significant differences (P < 0.05). There was no complication occurred in the observation group and the incidence of complications in the control group was 6.1%. Conclusion The medical care integration can promote the recovery of hip function and the clinical treatment effects in elderly patients with hip fracture, improve the nursing quality, reduce the incidence of complications. It is worthy of promotion and application.

Comparison of diagnostic value of breast MRI and ultrasonography in axillary lymph node metastasis of breast cancer

Objective To study the clinical value of breast MRI and axillary ultrasonography in the differential diagnosis of axillary lymph node metastasis in breast cancer patients. Methods The data of 229 patients with breast cancer underwent radical mastectomy, modified radical mastectomy or radical mastectomy in Xuanwu Hospital from October 2016 to September 2017 were retrospectively analyzed. All patients underwent preoperative MRI and axillary ultrasound examination, underwent radical mastectomy and axillary lymph node dissection, and underwent routine pathological examination of lymph nodes. The correlation between MRI and axillary ultrasound findings and postoperative pathological findings were observed. Results Axillary lymph node metastasis was detected in 102 of 229 patients, and no metastasis was detected in 127 patients. 108 cases of axillary lymph node metastasis were detected by MRI, of which 91 cases were consistent with the cases, 17 cases were misdiagnosed, and 11 cases were missed. Axillary lymph node metastasis was detected in 98 cases by ultrasound, 80 cases were consistent with pathological diagnosis, 18 cases were misdiagnosed, and 22 cases were missed diagnosis. The sensitive of MRI in diagnosis of axillary lymph node metastasis was higher than that of axillary ultrasound, with statistically significant difference (P < 0.05). There was significant difference in specificity and accuracy of diagnosis between MRI and axillary ultrasound (P > 0.05). The specificity of MRI positive and axillary ultrasound positive in diagnosis of axillary lymph node metastasis was higher than that of MRI, axillary ultrasound and MRI positive or axillary ultrasound positive (P < 0.05). The sensitivity of MRI positive or axillary ultrasound positive was higher than that of MRI, axillary ultrasound, MRI positive and axillary ultrasound positive (P < 0.05). Conclusion The breast MRI and axillary ultrasonography are important means of detecting lymph node metastasis in breast cancer, both positive is helpful for the diagnosis of axillary lymph node metastasis, one positive in the two indicator is helpful for the diagnosis of axillary lymph node metastasis, clinical can flexibly according to the actual situation.

Clinical observation of Chinese medicine fumigation in the treatment of lumbar disc herniation

Objective To observe clinical effect of Xunxi NO.1 empirical formula of Chinese medicine fumigation on the patients with lumbar disc herniation (LDH). Methods Sixty patients with LDH treated in South Campus of Guang′anmen Hospital, China Academy of Chinese Medical Sciences, from January to August in 2017 were selected as the study subjects, and they were divided into two groups by random number table, with 30 cases in each group. The fumigation group was treated with Xunxi NO.1 Chinese medicine fumigation, 1 time per day, one dose a day, 30 minutes each time. The Hantongle group was treated with Hantongle Compression Formula, 1 times per day, one stick a day for 2 weeks. The adverse reaction, VAS score, JOA score and clinical effective of the two groups were compared. Results No obvious adverse reaction occurred in treatment group, 1 case appeared ambustion in the Hantongle group. The clinical effect of the fumigation group was better than that of the Hantongle group (P < 0.01). After treatment, in the two groups, the VAS scores were decreased, and the JOA scores were increased, and the improvement of the fumigation group was markedly better than that of the Hantongle group (P < 0.01). Conclusion Xunxi NO.1 of Chinese medicine fumigation can effectively relieve the lumbago and leg pains of patients with LDH, improve the lumbar function, and has significantly therapeutic effect to LDH.

The measure in the connotation development of children′s hospital based on the fine management

Fine management is not only a management method, but also a modern management concept. The application in the hospital connotation development is to establish the predetermined result achieved in a certain period of time by according the external environment and internal conditions, and to develop the overall goal, and it needs some method for realizing the goal of organization, incentive, control and inspection. The connotation development mode of Beijing Children′s Hospital asked for improving the utilization efficiency of medical resources and reducing the burden of patients. This paper firstly introduces the meaning of fine management, expounds the practical meaning of implementing large-scale hospital fine management, discusses the fine management system of large-scale hospital model, puts forward based on the connotation of fine management leveraging development measures. The outstanding professional construction is a prerequisite for fine management, strengthening the construction of information technology is an effective means of meticulous management, establishment of "quantitative" management culture is the only way which must be passed of fine management, building quality culture is the fine management to upgrade security, and helping implement grading diagnosis and treatment is the innovation of fine management.
