Effect of expression of autophagy related gene Atg7 on replication of hepatitis B virus

Objective To investigate the effect of Atg7 expression on HBV replication in hepatocytes infected with hepatitis B virus. Methods The expression of Atg7, LC3 and HBs in paraffin section of liver tissue of patients with chronic hepatitis B virus infection in Beijing You′an Hospital from April to May 2019 was observed by immunofluorescence staining. Atg7 overexpression group and Atg7 reduction group were established in HepG2.2.15 cells by using Atg7 expression plasmids and small interfering RNAs. The changes of autophagy and HBV replication level were observed by Western blot and real-time quantitative PCR. Data was analyzed by SPSS 17.0. Results The expression of Atg7 and LC3 in HBs positive cell was higher than that in HBs negative cell; after Atg7 overexpression and starvation, autophagy level, HBc and HBV-DNA expression were higher than those of control group, the replication level of autophagy and HBV increased most significantly in 6 h of starvation, and the differences were highly statistically significant (P < 0.01). The level of autophagy, the expression of HBc and HBV DNA in Atg7 treated cells were lower than those in control group, the decrease was most significant at 6 h after starvation, and the differences were highly statistically significant (P < 0.01). Conclusion HBV infection increases Atg7 expression and autophagy, early intervention of Atg7 expression in autophagy can significantly affect HBV replication.

Study on the repair mechanism of Yiguanjian Decoction in DNA oxidative damage of L02 cells induced by H

Objective To investigate the repair mechanism of Yiguanjian Decoction in hydrogen peroxide (H2O2)-induced DNA oxidative damage of human normal liver cells (L02). Methods L02 cells were randomly divided into 6 groups, including normal control group, model control group, low dose Yiguanjian Decoction group, middle dose Yiguanjian Decoction group, high dose Yiguanjian Decoction group and positive control group, five mice in each group. The normal control group and the model control group were cultured with medium containing 20% normal rat serum, simultaneously, low dose Yiguanjian Decoction group, middle dose Yiguanjian Decoction group, high dose Yiguanjian Decoction group were respectively cultured with medium containing 20% low, middle, high dose Yiguanjian Decoction of serum, and positive control group was cultured with 20% normal rat serum containing 0.1 mmol/L Vc. After 24 hours, except the normal control group, the cells of other groups were treated with 0.6 mmol/L H2O2 for 1 hour. The cell viability was detected by CCK-8; the content of 8-hydroxydeoxyguanosine (8-OHDG) was determined by enzyme linked immunosorbent assay (ELISA); the reactive oxygen species (ROS) contents and cell cycle were tested by flow cytometry; the mRNA levels of ATM, CHK2, Cdc25A and Cdc25C were assessed by revers transcriptation-polymerase chain reaction (RT-PCR). Results L02 cell viability was decreased with increasing H2O2 concentration and prolonged action time. Compared with the normal control group, cell viability of the model control group was decreased (P < 0.05); 8-OHDG contents were increased (P < 0.05); the mean fluorescence intensity of ROS was increased significantly (P < 0.05); the cell proportion of G1 phase was decreased, while the cell proportion of S phase and G2/M phase were increased (P < 0.05); the mRNA levels of ATM and CHK2 were up-regulated, and Cdc25A and Cdc25C were down-regulated (P < 0.05). Compared with the model control group, the cell viability of the low dose Yiguanjian Decoction group, middle dose Yiguanjian Decoction group, high dose Yiguanjian Decoction group and positive control group were increased (P < 0.05); the 8-OHDG content were decreased (P < 0.05); the mean fluorescence intensity of ROS were decreased (P < 0.05); the cell proportion of G1 phase were increased, the cell proportion of S phase and G2/M phase were decreased (P < 0.05); the mRNA levels of ATM and CHK2 were down-regulated, while the expression of Cdc25A and Cdc25C mRNA were up-regulated (P < 0.05). Conclusion Yiguanjian Decoction could repair the DNA oxidative damage of L02 cells by inhibiting the cell cycle through the ATM/CHK2/Cdc25A/Cdc25C pathway.

Effect of dihydroquercetin on the phosphoinositide-3-kinaset/glycogen synthase kinase-3β signaling pathway in the myocardium of rats with acute myocardial ischemia induced by isoproterenol

Objective To explore the effect of dihydroquercetin on the phosphoinositide-3-kinaset (PI3K)/glycogen synthase kinase-3β (GSK-3β) signaling pathway in the myocardium of rats with acute myocardial ischemia induced by isoproterenol. Methods Sixty clean SD rats were randomly divided into blank group, model group, Di′ao Xinxuekang group, high dose (60 mg/kg) of dihydroquercetin group, medium dose (30 mg/kg) of dihydroquercetin group, low dose (15 mg/kg) of dihydroquercetin group by random number table method. The drug was administered by continuous gavage for 14 d. The rat models of acute myocardial ischemia were established with intraperitoneal injection of isoproterenol from day 12. The hemorheology was observed, the levels of creatine kinase (CK), lactate dehydrogenase (LDH), nitric oxide (NO) in rat serum and the levels of superoxide dismutase (SOD), catalase (CAT), glutathione peroxidase (GSH-Px) and malonaldehyde (MDA) in rat myocardium were measured; the protein expression levels of PI3K, phosphorylation protein kinase B (PKB) and GSK-3β in rat myocardium were detected by Western blot. Results Compared with the blank group, the whole blood viscosity, plasma viscosity, haematocrit was significantly increased in model group (P < 0.05), serum CK and LDH levels increased (P < 0.05), NO level decreased (P < 0.05), the myocardial SOD, GSH-Px, CAT level and the protein expression of PI3K were reduced (P < 0.05), MDA level and the protein expression of GSK-3β were increased (P < 0.05). Compared with the model group, the levels of plasma viscosity in high, medium, low dose of dihydroquercetin group decreased (P < 0.05), the haematocrit in medium, low dose of dihydroquercetin group decreased (P < 0.05), and the levels of serum LDH and CK in high, medium dose of dihydroquercetin group decreased (P < 0.05), NO level in each dose of dihydroquercetin group was increased (P < 0.05), the SOD, GSH-Px, CAT level and the protein expression of PI3K were increased in each dose of dihydroquercetin group (P < 0.05), the myocardial MDA level was reduced (P < 0.05), and the protein expression of GSK-3β in high, medium dose of dihydroquercetin group decreased (P < 0.05). Conclusion Dihydroquercetin can inhibit oxidative stress cascade, regulate myocardial blood flow, block the progress of myocardial ischemia in rats with acute myocardial ischemia through regulating PI3K/GSK-3β signaling pathway, so as to play an anti-myocardial ischemia role.

Expressions, clinical significance and correlation of DNMT1 and NHERF1 in colorectal cancer

Objective To explore the expressions, clinical significances and their correlation of DNA methyltransferase-1 (DNMT1) and natrium hydrogen exchanger regulator factor 1 (NHERF1) in colorectal adenocarcinoma tissues. Methods From February 2017 to February 2018, 50 specimens of colorectal adenocarcinoma tissues (cancer group) and 50 specimens of normal colorectal tissues (normal group) in the First Affiliated Hospital of Hebei North University were selected. The expression levels of DNMT1 mRNA and NHERF1 mRNA were detected by real-time fluorescent quantitative reverse transcription polymerase chain (qRT-PCR), and the expression levels of DNMT1 and NHERF1 protein were detected by immunohistochemistry. The clinical data was collected and the relationship between their expression and clinicopathological characteristics was analyzed. The correlation was analyzed by Spearman test. Results The qRT-PCR results showed that the expression level of DNMT1 mRNA in the cancer group was higher than that in the normal group, while the expression level of NHERF1 mRNA in the cancer group was lower than that in the normal group, and the differences were highly statistically significant (all P < 0.01). The immunohistochemical results showed that the positive rate of DNMT1 protein in cancer group was higher than that in the normal group, while the expression level of NHERF1 protein in cancer group was lower than that in the normal group, and the differences were highly statistically significant (all P < 0.01). The positive expression of DNMT1 and NHERF1 protein were correlated with the depth of tumor invasion, TNM stage, lymph node metastasis, and liver metastasis (all P < 0.05), and were not related with gender, age, tumor size and degree of differentiation (all P > 0.05). Spearman correlation analysis showed that there was a negative correlation between DNMT1 and NHERF1 (r = -0.491, P < 0.01). Conclusion The abnormal expression levels of DNMT1 and NHERF1 play an important role in the occurrence and development of colorectal cancer, which can be used as potential biological indicators for early diagnosis and treatment monitoring of colorectal cancer.

Advances in the mechanism of traditional Chinese medicine regulating invasion and metastasis of tumor cells by EMT

Epithelial-mesenchymal transition (EMT) is a key step in the process of differentiation and metastasis of solid cancer. Studies have shown that EMT is involved in a variety of signaling pathways and transcription factors. Therefore, finding the relevant influencing factors of EMT as the therapeutic targets of tumor invasion and metastasis plays an important role in the prevention and treatment of cancer. This paper takes the molecular mechanism of EMT process as the starting point, the development and progression of tumor cell invasion and metastasis regulated by EMT are reviewed. It is beneficial to find therapeutic targets for EMT process, thus providing new therapeutic ideas and directions for malignant tumor development.

Advances in the function reserch of MicroRNA in breast cancer drug resistance

Breast cancer is one of the most common malignant tumors in women, which is a serious threat to human health. In recent years, relying on the continuous improvement of early diagnosis and treatment strategies, breast cancer mortality has been declining year by year. However, drug resistance and relapse are the main obstacles to the treatment of breast cancer. The drug resistance of breast cancer is regulated by a variety of mechanisms, among which microRNAs (miRNA) may play an important role in all aspects of cancer progression. miRNAs are endogenous non-coding RNAs involved in the regulation of various biology processes, including cell proliferation, invasion, metastasis and drug resistance. Abnormal expression of specific miRNA can affect the expression of cell-associated proteins, the binding of anti-tumor drugs to targets and apoptosis-related pathways, leading to drug resistance. This paper reviews the studies on functional miRNAs involved in breast cancer drug resistance and their targets and related mechanisms, believing that miRNA can become be a biomarker and a new therapeutic target for clinical diagnosis and treatment of breast cancer.

Research progress of psychological distress and psychological intervention in lung cancer patients

Lung cancer is one of the major diseases threatening human life. The psychological distress of patients with lung cancer is closely related to the treatment coordination and quality of life. Therefore, how to implement psychological intervention for patients has become the focus of medical researchers in the field of cancer. This article reviews the current status and causes of psychological distress in patients with lung cancer (demographic factors, disease-related factors), and psychological interventions at home and abroad (cognitive intervention, mindfulness decompression, etc.), in order to reduce the psychological problems and improve the quality of life of patients with lung cancer. At the same time, this article also provides lessons and reference for the optimization of tumor clinical nursing work.

Research progress on mitochondrial mechanism of septic cardiomyocytes apoptosis

Sepsis-induced myocardial dysfunction (SIMD) is the initial cause of multiple organ dysfunction and has always been a difficult point in intensive care unit (ICU) treatment. As the energy factory of cardiomyocytes, mitochondria is involved in occurrence and development of SIMD. And the occurrence of cardiomyocyte apoptosis is closely related to mitochondria. At present, it is believed that the mitochondrial mechanism of cell apoptosis is mainly related to mitochondrial permeablity transition pore (mPTP) opening, mitochondrial respiratory disorder, intracellular Ca2+ overload, mitochondria oxidative/nitrosative stress, caspase family and Bcl-2/Bax apoptosis/inhibition of apoptotic protein release. However, these mechanisms often do not occur independently, but cause and effect each other and promote each other, and eventually induce cardiomyocyte apoptosis, which will lead to or aggravate SIMD. This article will review the mechanism of mitochondria in the apoptosis of sepsis cardiomyocytes, in order to provide a theoretical basis for further research.

Analysis on medical device recall from 2010 to 2018 in China

Objective To explore the status quo of medical device recall in China and to make recommendations for reference. Methods All information were collected in “Medical Device Recall” section of the official websites of China Food and Drug Administration (CFDA) and National Medical Products Administration (NMPA) from May 2010 to December 2018. Excel method was used for classification analysis in terms of number of recalls, reasons, levels, methods, equipment classification, country of origin, and manufacturing enterprises. Results The total number of effective medical device recalls was 1533 cases, all of which were active recalls, including 1251 cases of imported instruments (81.60%) and 282 cases of domestic instruments (18.40%). The main recall reasons were design defects in 349 cases (22.77%) and manufacturing defects in 752 cases (49.05%). 48 cases (3.13%) were recalled with first level, 379 cases (24.72%) with second level, while 740 cases (48.27%) with third level, and 366 cases (23.87%) with un-indicated recall level. The number of Clinical analytical instruments recalls was 403 cases (26.29%), which was the largest number in category of devices recalls. 18 countries were involved in the recall process, while the top 3 were USA, China and Germany. Conclusion The recall regulation has achieved remarkable results, however, the system still needs to be standardized to improve the scientific and efficient supervision system.

Research on the curriculum setting of higher vocational nursing majorunder the demand orientation

Objective To constructed a course system for the elderly care professional from the needs of the elderly, elderly caregivers, and managers of the elderly care institutions. Methods By consulting the relevant literature and the previous research results, a questionnaire for the needs and curriculum of senior nursing professionals was developed. The Delphi method was used to conduct two rounds of consultations with 14 nursing education experts, clinical nursing experts and senior care institution managers in Jiangxi Province from January to June 2018. Results The demand rate of professional nurses for elderly institutions had reached 100%, 50% of them need secondary education and college education. In the two rounds of expert consultation, the effective rates of questionnaire recovery efficiency were 93% and 100%, the expert authoritative coefficient was 0.83, the coefficients of variation were 0.165 and 0.156, respectively, while Kendall′s W values were 0.253 and 0.413. The course system for the elderly nursing profession was initially determined, including 6 course modules, 26 courses at school and post placements. Conclusion Curriculum setting of senior nursing specialty in higher vocations not only reflects the professionalism of the curriculum, but also organically integrates the basic, humanities and elective courses. In addition, the practical part of the course uses post-training internships so that students can apply what they have learned. At the same time, the construction of the curriculum system will meet the needs of the employers in the quantity and quality of nursing talents.

Relationship between serum HMGB1 and sTLT-1 levels and adverse cardiovascular events after percutaneous coronary intervention in patients with acute myocardial infarction

Objective To investigate the relationship between serum high-mobility group box-1 (HMGB1) and soluble triggering receptor expressed on myeloid cells-like transcript-1 (sTLT-1) levels and major adverse cardiovascular events (MACE) in patients with acute myocardial infarction (AMI) after percutaneous coronary intervention (PCI). Methods From March 2016 to March 2018, 198 cases with AMI patients after PCI admitted to the Fourth People′s Hospital of Langfang City of Hebei Province were selected. According to the whether or not MACE happens, they were divided into MACE group (n = 57) and non-MACE group (n = 141). The control group included 50 patients who had no history of AMI and acute or chronic infection. Heart rate (HR), Killip cardiac function grading, operation time, serum levels of cholesterol, glycosylated hemoglobin (HbA1c), HMGB1, sTLT-1, high-sensitivity C-reactive protein (hs-CRP) and cardiac troponin I (cTnI) were compared among the three groups. Risk factors of MACE after PCI in AMI patients and correlation analysis between HMGB1, sTLT-1 levels and hs-CRP, cTnI levels were analyzed. ROC curve was used to evaluate the predictive value of serum HMGB1 and sTLT-1 for the occurrence of MACE after operation. Results There were no statistical significance differences in HR, Killip cardiac function grading, operation time, HbA1c and cholesterol among the three groups (all P > 0.05). The levels of HMGB1, sTLT-1, hs-CRP and cTnI in MACE group and non-MACE group were higher than those in control group, the levels of HMGB1, sTLT-1, hs-CRP and cTnI in MACE group were higher than those in non-MACE group, and the differences were statistically significant (all P < 0.05). Logistic regression analysis showed that HMGB1 sTLT-1, hs-CRP and cTnI levels were independent risk factors for MACE after PCI in AMI patients. After PCI, serum HMGB1 levels in AMI patients were positively correlated with hs-CRP and cTnI levels (r = 0.422, 0.491, P < 0.05), and sTLT-1 levels were positively correlated with hs-CRP and cTnI levels (r = 0.404, 0.520, P < 0.05). When the diagnostic threshold of HMGB1 were 79.75 μg/L, the sensitivity and specificity of HMGB1 were 0.719 and 0.759. When the diagnostic threshold of sTLT-1 was 595.72 pg/mL, the sensitivity was 0.544 and the specificity was 0.794. The sensitivity and specificity of the two indexes were 0.842 and 0.858. Conclusion Elevated serum HMGB1 and sTLT-1 levels in AMI patients after PCI are also risk factors for the occurrence of MACE in the near future after PCI. Early combined detection can effectively predict the occurrence of MACE in AMI patients after PCI.

Change and clinical significance of serum Lp(a) and NEFA levels in patients with coronary heart disease

Objectives To analyze the change and clinical significance of serum lipoprotein a [Lp(a)] and non-esterified fatty acid (NEFA) levels in patients with coronary heart disease (CHD). Methods From May 2016 to January 2019, 143 cases with CHD diagnosed in Chenzhou First People′s Hospital of Hu′nan Province were selected as the study group. According to the lesion branch, they were divided into single branch lesion subgroup (54 cases), double branch lesion subgroup (49 cases) and multi-branch lesion subgroup (40 cases). According to Gensini scores, they were divided into low score subgroup (34 cases), middle score subgroup (47 cases) and high score subgroup (62 cases). And 106 patients who underwent coronary angiography at the same time, with mild stenosis and coronary artery occlusion less than 30% were selected as the control group. Serum Lp(a) and NEFA levels were compared between the two groups. Pearson correlative analysis was used to analysed the correlation between Lp(a), NEFA levels and lesion branch, Gensini scores, the level of Lp(a) and NEFA. Results The serum levels of Lp(a) and NEFA in the study group were higher than those in the control group, and the differences were statistically significant (all P < 0.05). The levels of serum Lp(a) and NEFA in the study group increased with the number of coronary artery lesion branchesand the degree of lesions (P < 0.05). Pearson correlative analysis showed that serum Lp(a) and NEFA levels were positively correlated with Gensini scores (r = 0.482, 0.503, P < 0.05). Serum Lp(a) and NEFA levels were positively correlated with the number of lesions (r = 0.513, 0.472, P < 0.05). The serum Lp(a) in CHD patients was positively correlated with NEFA (r = 0.469, P < 0.05). Conclusion The levels of serum Lp(a) and NEFA in CHD patients are significantly increased, and are positively correlated with the number of lesions and Gensini scores. The detection of Lp(a) and NEFA are of great significance for the clinical diagnosis and treatment of CHD patients.

Effect of Tian Meng Capsules combined with Sertraline Hydrochloride Tablets in the treatment of post-stroke depression

Objective To explore the clinical efficacy of Tian Meng Capsules combined with Sertraline Hydrochloride Tablets in the treatment of post-stroke depression (PSD). Methods Ninety-five patients with PSD who were admitted to Guangzhou Red Cross Hospital from January to November 2018 were selected as subjects. The patients were divided into control group (47 cases) and study group (48 cases) by random number table method. The control group was given Sertraline Hydrochloride Tablets, the study group was given Tian Meng Capsules combined with Sertraline Hydrochloride Tablets. The clinical efficacy, serum 5-hydroxytryptamine (5-HT), brain-derived neurotrophic factor (BDNF), depression and anxiety, sleep quality and adverse reactions were compared between the two groups. Results The total clinical effective rate in the study group was higher than that in the control group (P < 0.05). After treatment, the levels of 5-HT and BDNF in the study group were higher than those in the control group (P < 0.05). The scores of Hamilton depression scale, Hamilton anxiety scale and Pittsburgh sleep quality index in the study group were lower than those in the control group (P < 0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P > 0.05). Conclusion Tian Meng Capsules combined with Sertraline Hydrochloride Tablets is effective in the treatment of PSD. It can improve depression and anxiety, and enhance sleep quality, the adverse reactions are low. It can be widely used in clinic.

Effectiveness of Edaravone combined with Sodium Valproate in the treatment of post-stroke epilepsy

Objective To investigate the efficacy of Edaravone combined with Sodium Valproate in the treatment of post-stroke epilepsy (PSE). Methods The clinical data of 102 patients with PSE who were treated in Tianjin Huanhu Hospital from January 2016 to March 2018 was analyzed retrospectively. According to the different treatment methods, the patients were divided into control group (55 cases) and study group (47 cases). The control group was treated with Sodium Valproate, and the study group was treated with Edaravone on the basis of the control group. The clinical efficacy, seizure frequency, duration, levels of neuron specific enolase (NSE), inflammatory factors and adverse reactions were compared between the two groups. Results The total effective rate of the study group was significantly higher than the control group (P < 0.05). Compared with those before treatment, the number of epileptic seizures decreased and the duration of epileptic seizures shortened in the two groups after treatment. After treatment, the number of epileptic seizures in the study group was less than the control group, and the duration of epileptic seizures was shorter than the control group (P < 0.05). Compared with those before treatment, the levels of serum interleukin-1β (IL-1β), interleukin-2 (IL-2) and tumor necrosis factor-α (TNF-α) in the two groups were decreased, after treatment, which in the study group were lower than the control group after treatment (P < 0.05). There was no significant difference in serum NSE level between the two groups after 3, 6 months and 12 months of treatment (P > 0.05). The serum NSE level in the study group was lower than the control group after 1 month of treatment (P < 0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P > 0.05). Conclusion Edaravone combined with Sodium Valproate in the treatment of PSE can rapidly improve clinical symptoms and reduce the level of NSE with definite curative effect. It can also reduce the level of inflammatory factors without increasing the incidence of adverse reactions.

Application of transthoracic B-ultrasound positioning in neonatal PICC catheterization

Objective To explore the effect of early transthoracic wall ultrasonography in neonatal peripherally inserted central catheter (PICC) catheterization. Methods A total of 66 neonates who underwent PICC catheterization in Jieyang People′s Hospital of Guangdong Province from June 2018 to May 2019 were enrolled. According to the random number table method, they were divided into experimental group (33 cases) and control group (33 cases). After the conventional puncture was placed in the control group, the X-ray film was positioned at the tip position; after the B-ultrasound of the chest wall was placed in the experimental group, the conventional puncture was performed, and the X-ray film was positioned at the tip position. The catheterization time, catheterization completion and complications were compared between the two groups. Results The operation time of PICC in the experimental group was shorter than that in the control group, and the difference was statistically significant (P < 0.05). The PICC catheterization in the experimental group was better than the control group, and the difference was statistically significant (P < 0.05). The incidence of complications after PICC catheterization was lower than that of the control group, and the difference was statistically significant (P < 0.05). Conclusion The application of B-ultrasound positioning in the chest wall is effective in the placement of neonatal PICC, and its complication rate is lower but the safety is stronger.

Effect of high volume hemofiltration combined with Alprostadil in the treatment of neonatal septic shock complicated with acute respiratory distress syndrome

Objective To analyze the application value of high volume hemofiltration (HVHF) combined with Alprostadil and the effect on high mobility group box 1 (HMGB1) and matrix metalloproteinase-9 (MMP-9) levels. Methods From January 2016 to June 2018, 80 cases of neonatal septic shock complicated with ARDS in West China-Guang′an Hospital, Sichuan University were divided into observation group and control group according to random envelope method, 40 cases in each group. The control group was treated with routine treatment combined with continuous venous hemofiltration intervention, while the observation group was treated with HVHF combined with alprostadil regimen was used to treat the patients. The basic treatment conditions of the patients were counted, the changes of respiratory function index [respiratory rate (RR), arterial oxygen partial pressure (PaO2), inhaled oxygen concentration (FiO2), oxygenation index (OI), positive end-expiratory pressure (PEEP), airway resistance (AR)], inflammatory index in blood [procalcitonin (PCT), C-reactive protein (CRP), interleukin-6 (IL-6) and CD4+/CD8+] and the levels of HMGB1 and MMP-9 in blood were detected. Results Respiratory ventilation time and ICU hospitalization time in the observation group were shorter than those in the control group, the differences were statistically significant (P < 0.05); RR, FiO2, PEEP and AR in the observation group were lower than those in the control group at 3 d and 6 d of treatment, and PaO2 and OI levels were higher than those in the control group, the differences were statistically significant (P < 0.05). The levels of PCT, CRP, IL-6 and CD4+/CD8+ in the observation group were lower than those in the control group, the differences were statistically significant (P < 0.05); the levels of HMGB1 and MMP-9 in the observation group were lower than those in the control group at 3 d and 6 d after treatment, the differences were statistically significant (P < 0.05). Conclusion The combination of HVHF and alprostadil can effectively improve the clinical efficacy of the traditional regimen in the treatment of neonatal septic shock complicated with ARDS, improve the pulmonary function and inflammatory state in vivo, and reduce the levels of HMGB1 and MMP-9 in blood.

Expression and clinical significance of miR-29b and Tiam1 in colon cancer

Objective To investigate the expression and clinical significance of microRNA-29b (miR-29b), T lymphoma invasion and metastasis inducing gene 1 (Tiam1) in colon cancer tissues. Methods The cancer tissues and paracancerous tissues of 81 patients with colon cancer treated in the Second People′s Hospital of Yibin from May 2016 to May 2018 were selected. Real-time fluorescent quantitative PCR was used to detect the expression of miR-29b and Tiaml in colon cancer tissues. The relationship between the expression of miR-29b and Tiaml and its relationship with the clinicopathological characteristics of the patient were analyzed. Results The expression of miR-29b in colon cancer tissue was significantly lower than that in paracancerous tissue (P < 0.05), and the expression level of Tiam1 in colon cancer tissue was significantly higher than that in paracancerous tissue (P < 0.05). The expression of miR-29b and Tiam1 was correlated with the degree of cell differentiation, tumor TNM stage and lymph node metastasis, while the expression of miR-29b was negatively correlated with the expression of Tiam1 in cancer tissues (r = -0.381, P < 0.05). Conclusion The expression of miR-29b is low expressed in colon cancer tissues, while the Tiam1 was high. The expression of miR-29b and Tiam1 is closely related to TNM stage, cell differentiation and lymph node metastasis. The interaction between them is involved in the occurrence, development and metastasis of colon cancer.

The value of Caprini score combined with D-dimer in the diagnosis of deep venous thrombosis in gynecological malignant tumors

Objective To investigate the value of Caprini score combined with D-dimer in the diagnosis of deep venous thrombosis (DVT) in patients with gynecologic malignancies. Methods A retrospective analysis of 95 patients with DVT after gynecologic malignancies and 95 patients with concurrent DVT who were admitted to the First Affiliated Hospital of Shandong First Medical University from January 2017 to January 2019. The general data of the two groups were retrospectively analyzed. The values of Caprini score, D-dimer value and the combined diagnosis of DVT in gynecologic malignant tumor were compared. Results The Caprini score and D-dimer in the DVT group were significantly higher than those in the control group (P < 0.01). The ROC curve of DVT in patients with gynecologic malignant tumors was diagnosed by Caprini score, D-dimer and the combination of the two. The lower areas were 0.817, 0.860 and 0.884, respectively. The sensitivity was 67.4%, 90.5% and 61.1%, respectively. The specificities were 87.4%, 69.5% and 95.8%, respectively. The false negative rates were 32.6%, 9.5% and 39.0%, respectively. The positive rates were 12.6%, 30.5% and 4.2%, respectively. The positive predictive values were 85.3%, 74.8% and 93.6%, respectively. The negative predictive values were 72.8%, 88.0% and 71.1%, respectively. The positive likelihood ratios were 5.35, 2.97 and 14.50, respectively. The negative likelihood ratios were 0.37, 2.97 and 0.42, respectively. The likelihood ratio of Caprini score combined with D-dimer in the diagnosis of gynecologic malignant tumor after DVT was compared with Caprini score alone the difference was statistically significant (P < 0.01), the difference was not statistically significant compared with D-dimerm alone (P > 0.05). Conclusion Caprini score combined with D-dimer is an economic and safe early screening method with certain clinical value.

Predictive value of RBC volume distribution width on myocardial perfusion level and short-term prognosis in patients with STEMI

Objective To study the predictive value of RBC volume distribution width (RDW) on post-operative myocardial perfusion level and short-term prognosis in patients with ST-segment elevation myocardial infarction (STEMI). Methods One hundred and thirty-eight patients with first-episode acute STEMI who were enrolled in Chengdu Third People′s Hospital from January 2017 to January 2018 were selected for the study. The patients were divided into observation group (no reflow, no complete ST segment resolution after operation, 47 cases) and control group (normal reflow, complete ST segment resolution after operation, 91 cases). The clinical indexes of two groups were detected and compared, the predictive effect of RDW and high sensitivity C-reactive protein (hs-CRP) on no reflow after percutaneous coronary intervention (PCI) was analyzed; RDW=14.48% was taken as the best cut-off point, the patients with first-episode acute STEMI was divided into the RDW≥14.48% group (52 cases) and the RDW<14.48% group (86 cases), and the incidence of no reflow, adverse cardiovascular events (MACEs) and survival prognosis between two groups were compared. Results The levels of RDW and hs-CRP in observation group were significantly higher than those in control group, and the ratio of infarct related artery to TIMI blood flow grade 3 after PCI in observation group was significantly lower than that in control group (all P < 0.05). ROC curve analysis showed that RDW was better than hs-CRP in the prediction of no reflow after PCI. When the area under the curve was 0.794 and RDW was 14.48%, the prediction value was the highest. The incidence of no reflow and MACEs in RDW≥14.48% group was significantly higher than that in RDW<14.48% group (all P < 0.05). The incidence of no reflow and MACEs in RDW≥14.48% group was significantly higher than that in RDW<14.48% group (all P < 0.05). There was no significant difference between two groups in survival and prognosis (P > 0.05). Conclusion RDW has a high predictive value for myocardial perfusion level and MACEs in patients with STEMI. Early monitoring of RDW level can be used to evaluate and judge the prognosis of STEMI patients. Ultimately, it helps to choose more scientific treatment options.

The relationship between serum thrombospondin-1 level and severity of systemic lupus erythematosus

Objective To investigate the relationship between the level of serum thrombospondin-1 (TSP-1) and the disease severity of patients with systemic lupus erythematosus (SLE). Methods One hundred and ten SLE patients admitted to Central War Zone General Hospital of Chinese People′s Liberation Army (“our hospital” for short) from January 2010 to December 2018 were selected as observation group. According to the SLE disease activity index (SLEDAI-2000) score, the patients were divided into remission period (49 cases, SLEDAI-2000 score < 5 points) and active period (61 cases, SLEDAI-2000 score ≥ 5 points). Another 50 volunteers who underwent physical examination in our hospital during the same period were selected as control group. The levels of serum TSP-1 in control group and observation group, remission period and active period were compared with other clinical indicators. The relationship between serum TSP-1 level and SLE complications was analyzed. Pearson correlation analysis was used to analyze the correlation between serum TSP-1 level and other clinical indicators. Results The level of serum TSP-1 in observation group was higher than that in control group, while the levels of hemoglobin, platelet, C3 and C4 in observation group were lower than those in control group (P < 0.05). The level of TSP-1 in active phase was higher than that in remission phase, while levels of hemoglobin, platelet, C3 and C4 in active phase were lower than those in remission phase (P < 0.05). The serum TSP-1 level of SLE patients with oral ulcer, rash, alopecia, arthritis, lupus nephritis and hemolytic anemia was higher than that of those without symptoms (P < 0.05). Pearson correlation analysis showed that TSP-1 was negatively correlated with hemoglobin, platelet, C3 and C4 (r = -0.471, -0.462, -0.492, -0.537, P < 0.05), but not with creatinine, urea nitrogen and 24h urinary protein (P > 0.05). Conclusion There is an abnormal increase in serum TSP-1 level of SLE patients, and the degree of increase is closely related to the disease severity of patients. At the same time, the abnormal release of TSP-1 may have some effect on the complications of SLE.

Comparative analysis of clinical characteristics between pulmonary tuberculosis patients with HIV and simple pulmonary tuberculosis

Objective To analyze the clinical characteristics of human immunodeficiency virus (HIV) patients with pulmonary tuberculosis and simple pulmonary tuberculosis, and to further evaluate the accuracy of G-xpert test, PPD test, Mycobacterium tuberculosis(MTB) routine sputum examination, sputum culture and T cell spot test in diagnosis of two groups of patients. Methods The clinical characteristics, clinical manifestations and diagnostic methods of 49 patients with HIV and tuberculosis and 57 patients with simple tuberculosis admitted to the Third People′s Hospital of Kunming from January 2017 to December 2018 were analyzed. Results The positive rates of sputum examination, PPD test and G-xpert test were 12%, 23% and 54% in 49 HIV patients with pulmonary tuberculosis, respectively. The positive rates of sputum examination, PPD test and Xpert/MTB RIF test in 57 patients with pulmonary tuberculosis were 25%, 51% and 48% respectively. The diagnostic accuracy of G-xpert test was higher in HIV patients with pulmonary tuberculosis(P < 0.05). Conclusion Different diagnostic methods, such as G-xpert test, PPD test, conventional sputum MTB test, sputum culture, T-cell spot test, have different diagnostic accuracy in patients with HIV and tuberculosis and patients with simple tuberculosis. Xpert/MTB RIF examination has more advantages in the diagnosis of HIV patients with tuberculosis.

Clinical application value of ACTH, CORT, IL-18 and TNF-α in predicting depression

Objective To investigate clinical application value of ACTH, CORT, IL-18 and TNF-α in predicting depression. Methods A total of 60 patients with first-diagnosis depression admitted to Ningbo Kangning Hospital (“our hospital” for short) from June 2017 to June 2019 were selected as depression group, a total of 60 healthy persons who underwent routine physical examination in our hospital during the same period were selected as the control group. Serum levels of ACTH, CORT, IL-18 and TNF-α in two groups were detected respectively to analyze the correlation with depression. Results The serum levels of ACTH, CORT, IL-18 and TNF-α in depression group were significantly higher than those in the control group (P < 0.05). The serum levels of ACTH was positively correlated with total score of Hamilton depression scale (HAMD) and HAMD factors (P < 0.05), such as depression, guilt, insomnia at the end of early sleep, mental anxiety, somatic anxiety, diurnal and nighttime changes, and impaired ability. CORT was positively correlated with total score of HAMD and factors (P < 0.05), such as depression, insomnia at the end of early sleep, work interest, somatic anxiety, weight loss, diurnal and nighttime changes, and paranoid symptoms. The total score of HAMD was positively correlated with IL-18 (P < 0.05). IL-18 was positively correlated with HAMD factors such as depression, early insomnia with difficulty in falling asleep, somatic anxiety, general symptoms, paranoid symptoms, obsessive-compulsive symptoms, and impaired ability (P < 0.05). TNF-α was also positively correlated with total score of HAMD and its factors such as depression, insomnia at the end of early sleep, work interest, mental anxiety, somatic anxiety, general symptoms and impaired ability (P < 0.05). Conclusion The serum levels of ACTH, CORT, IL-18 and TNF-α in patients with depression are higher. Direct detection of the above indicators has certain clinical application value in predicting the occurrence of depression.

Effect of Huatan Tongsui Decoction for patients with stroke hemiplegia due to phlegm congestion and stasis obstructing meridian

Objective To analyze the clinical effect and the possible mechanism of Huatan Tongsui Decoction in the treatment of the stroke patients with hemiplegia. Methods A total of 120 stroke hemiplegia patients in Yiling Hospital of Hebei Province from January 2015 to December 2016. According to the random number table method they were divided into control group and treatment group, with 60 cases in each group. The treatment group was given basic treatment plus rehabilitation training and oral Huatan Tongsui Decoction, one dose a day, taken twice in the morning and evening for 45 days. The control group was given the same treatment as the treatment group except for Huatan Tongsui Decoction. The clinical symptoms were scored before and after treatment, and the upper limb motor function was evaluated by Fugl-Meyer scale. The electromyography score (iEMG) of deltoid muscle, triceps brachii muscle and extensor muscle group of forearm were detected by the surface electromyography. The serum total cholesterol (TC), triglyceride (TG), and hemorheology indexes (including whole blood viscosity, fibrinogen and plasma viscosity) were measured before and after treatment in two groups. Results Compared with before treatment, the clinical symptom scores of two groups were decreased after treatment (P < 0.05). There were no significant differences in clinical symptom scores between two groups (P > 0.05). The scores of upper limb motor function and iEMG in two groups were both increased, and those in treatment group were higher than those in control group (P < 0.05). The levels of TC and TG in two groups were decreased and those in treatment group were lower than those in control group (P < 0.05). There were no significant differences in hemorheology indexes in the control group after treatment (P > 0.05). Total blood viscosity, plasma viscosity and fibrinogen content were decreased in treatment group (P < 0.05), and those in treatment group were lower than those in control group (P < 0.05). Conclusion The mechanism of Huatan Tongsui Decoction treating stroke may be related to reducing serum triglyceride, improving blood rheology, so as to improve the clinical symptoms of the patients.

Relationship between the levels of serum Angptl 2 and Angptl 4 and severity of coronary artery stenosis in patients with coronary heart disease

Objective To explore the relationship between the levels of serum angiopoietin-like protein 2 (Angptl 2) and angiopoietin-like protein 4 (Angptl 4) and the severity of coronary artery stenosis in patients with coronary heart disease. Methods From June 2016 to December 2018, 98 csaes with coronary heart disease examined by coronary angiography in Liaoning Health Industry Group General Hospital of Fushun Mining Bureau were selected as coronary heart disease group. According to Gensini scores, they were divided into low-risk subgroup (36 cases), medium-risk subgroup (32 cases) and high-risk subgroup (30 cases). At the same time, 62 healthy persons with normal results of coronary angiography were enrolled as control group. The levels of serum of blood glucose, blood lipids, blood pressure, Angptl 2 and Angptl 4 between two groups were compared. Pearson correlative analysis was used to analyze the correlation between the levels of serum Angptl 2, Angptl 4 and Gensini scores in patients with coronary heart disease. Results The serum Angptl 2 levels in coronary heart disease group were higher than those in control group, while the serum Angptl 4 levels were lower than those in control group, and the differences were highly statistically significant (all P < 0.01). There were no significant differences in blood lipids, blood glucose and blood pressure between two groups (P > 0.05). Angptl 2, TG and LDL-C levels in high-risk subgroup were higher than those in medium-risk subgroup and low-risk subgroup, and medium-risk subgroup were higher than low-risk subgroup, the levels of Angptl 4 in high-risk subgroup were lower than those in medium-risk subgroup and low-risk subgroup, and medium-risk subgroup was lower than low-risk subgroup, and the differences were statistically significant (all P < 0.05). Pearson correlative analysis showed that Angptl 2 levels were positively correlated with Gensini scores (r = 0.504, P < 0.01), and Angptl 4 levels were negatively correlated with Gensini scores (r = -0.598, P < 0.01). Conclusion Angptl 2 and Angptl 4 are expected to provide new ideas for the diagnosis of coronary heart disease.

Effect of blood purification in poisoned children

Objective To analyze and summarize the effect of 15 poisoned children receiving blood purification treatment in Kunming children′s Hospital ("our hospital" for short). Methods The clinical data of 15 children treated with concurrent blood purification due to poisoning admitted to our hospital from May 1, 2018 to April 30, 2019 was analyzed retrospectively. Clinical data including blood purification methods, adverse reactions and treatment outcomes. Results Thirteen children recovered, two improved, and no death. Compared with before treatment, the indexes of alanine transaminase, alpaca transaminase, creatine kinase isoenzyme, troposin and urea were decreased after treatment, there were statistically significant differences (P < 0.05). The length of hospitalization ranged from 7 to 19 days, and the average length of stay was (9.5±2.3) d. Conclusion Blood purification is an effective method for the treatment of poisoned children. Which is worthy of active promotion in the field of pediatrics, so as to improve the cure rate, survival rate and prognosis of children with poisoning.

Correlation between serum hepatitis C virus RNA titers and liver fibrosis in chronic Hepatitis C virus patients

Objective To investigate the relationship between serum hepatitis C virus (HCV) and liver fibrosis in patients with chronic HCV infection. Methods A total of 162 chronic HCV patients without other liver or major diseases admitted to the Dalian Sixth People′s Hospital from April to December 2015 were selected. The association between HCV ribonucleic acid (RNA) titers and liver fibrosis was studied by using serum collagen Ⅳ (C-Ⅳ), laminin (LN), Type-Ⅲ pro-collagen (PⅢNP), hyaluronic acid (HA) and FibroScan transient elasticity. In order to further assess the correlation between HCV RNA titers and liver fibrosis, chronic HCV patients were divided into three groups according to the results of lg-transformed HCV RNA titers: lg(HCV RNA) ≤5 group, 5<lg(HCV RNA) ≤7 group and lg(HCV RNA) >7 group. Four serum indices of liver fibrosis were collected from 162 patients, of which only 54 patients were tested for FibroScan transient elasticity. The serum HCV RNA titers were automatically calculated by the instrument software and used for the 10-base logarithmic representation. Results Among 162 patients with chronic HCV, there were no statistically significant differences in hepatic fibrosis among different viral load groups (P > 0.05). There was no statistically significant difference in Fibroscan among 54 patients with chronic HCV in different viral load groups (P > 0.05). FibroScan transient elasticity was correlated with C-Ⅳ, LN, PⅢNP and HA (r = 0.530, 0.340, 0.289, 0.438, P = 0.000, 0.007, 0.023, 0.000). The HCV RNA titers were not correlated with four serum indices of liver fibrosis (P > 0.05). Conclusion Serum HCV RNA titers in patients with chronic HCV are not correlated with C-Ⅳ, LN, PⅢNP, HA and FibroScan transient elasticity, serum HCV RNA viral titers can not reflect the degree of liver fibrosis.

Discussion on the pathogenesis of rectal prolapse from “middle qi collapse”

In order to deepen the understanding of the pathogenesis of rectal prolapse in “middle qi collapse”, this paper expounds the two aspects of traditional Chinese medicine and modern medicine, in order to better guide the clinical practice, and also to point out the direction for the modeling of rectal prolapse in scientific research. In traditional Chinese medicine, “middle qi collapse” usually refers to a kind of disease, which is caused by qi deficiency, such as tissue relaxation and organ prolapse, indicating that “middle qi collapse” is directly related to the incidence of rectal prolapse. In modern medicine, the pathogenesis of rectal prolapse is the relaxation of rectal mucosa or the relaxation and separation of the rectum and its surrounding supporting tissue, and the rectum loses its support and slides down, which is basically consistent with the cognition of traditional Chinese medicine; as well as the experimental research on the establishment of artificial rectal prolapse model, the modeling method of chronic diarrhea coincides with the cognition of traditional Chinese medicine; in addition, rectal prolapse is common in fibulin-5 the weak mice, such as gene deficient mice, transgenic mice with low immune function or colitis mice, are also in line with the description of “qi deficiency can not be fixed, but it also falls off”, which indirectly indicates that the pathogenesis of rectal prolapse is related to the theory of “middle qi collapse”.

Professor Xie Ning′s experience in treating aged insomnia based on “spirit governed by heart and brain”

Insomnia is one of the common diseases that perplex the elderly. The primary insomnia can be attributed to the category of mental diseases in traditional Chinese medicine. According to the theory of “spirit governed by heart and brain” which has become a consensus in the field of traditional Chinese medicine, Professor Xie Ning combs the pathogenesis of insomnia in traditional Chinese medicine according to the physical characteristics of the elderly. It is suggested that we should start from two key points of treatment, namely, mental and medullary dystrophy, and take into account the changes of pathogenesis, such as disharmony between camp and health, phlegm and blood stasis disturbing the spirit. Based on the traditional Chinese medicine prescription Dihuang Yinzi, we should establish the targeted treatment ideas of tonifying, nourishing, regulating and clearing to treat senile insomnia.

Clinical syndrome and adjustment of internal injuries and miscellaneous diseases under the guidance of Gezhi Yu Lun

Zhu Danxi is one of the four great masters in Jin-Yuan dynasties, and is famous for his good treatment of miscellaneous diseases. Based on the theory of pathogenesis of fire-heat and the physiological state of “relative equilibrium of yin-yang” in Neijing, he inherites the medical theories of Liu Wansu, Zhang Zihe and Li Dongyuan, and combined the regional characteristics with the criticism of current disadvantages in clinical practice, write his representative work Gezhi Yu Lun. This paper sorts out the important academic thoughts of the whole book and the drugs contained in the medical records, and summarizes that Zhu Danxi′s treatment of miscellaneous diseases mostly starts with spleen and stomach. It emphasizes that the physiological characteristics of miscellaneous diseases are “exuberant yang and insufficiency of yin”. The etiology and pathogenesis of miscellaneous diseases are “fire disorder” and “stagnation of qi, blood and phlegm”. It is established that “taking spleen and yin as the basis” is the major method of miscellaneous diseases treatment. Nourishing spleen and Yin does not forget to protect temper (Yang). Through nourishing one′s emotions, calming one′s temper and keeping one′s temper in peace, distinguishing between deficiency and reality before curing fire, the prescriptions are guided by different therapies such as “fire-depression”“heat and cold” and “nourishing yin and reducing fire”. The methods of “blood-dispersing qi, phlegm-inducing stagnation” are used to dispel pathogenic factors, and the methods of “unobstructed mood, festive diet, and adjusting to the four seasons” are used throughout the whole process. Finally, Zhu Danxi′s unique opinion of “supporting internal injury, dispelling external evil and re-adjusting photography” in Gezhi Yu Lun provides valuable experience and important reference for the clinical treatment of miscellaneous diseases in modern Chinese medicine.

Analysis of professor Shen Shuwen′s prescription for diarrhea based on data mining

Objective To analyze the rules of professor Shen Shuwen′s prescriptions for diarrhea by using data mining. Methods The prescriptions of the patients diagnosed as “diarrhea” in the Famous Medical Studio of Affiliated Hospital of Shaanxi University of Chinese Medicine and the Famous Medical Center of the Second Affiliated Hospital of Shaanxi University of Chinese Medicine from January 2016 to December 2018 were collected. With the aid of the auxiliary system of traditional Chinese medicine inheritance, the methods such as association rules, entropy clustering of complex systems were used to analyze the frequency of drug use, the rules of formula formation, core drug pairs and potential new prescriptions. Results One hundred and ninety-two prescriptions were included, involving 170 flavors of traditional Chinese medicine, and 8 groups of core drugs and 4 potential new prescriptions were used. Conclusion According to the results of data analysis, professor Shen Shuwen used properties of drugs for diarrhea are mainly based on warm and mild drugs, such as Fried Rhizoma Atractylodis Macrocephalae, Pericarpium Citri Reticulatae, Radix Paeoniae Alba, Fructus Psoraleae, Semen Myristicae, etc. In addition to pungent, bitter and sweet drugs are main flavours of the five flavours. Therefore, Jianpi Yiqi is put in the first in the treatment of diarrhea.

Clinical effect of Shihu Yangwei Decoction combined with Western medicine in the treatment of chronic atrophic gastritis

Objective To explore clinical effect of Shihu Yangwei Decoction combined with Western medicine in the treatment of chronic atrophic gastritis (CAG). Methods One hundred patients with CAG who were admitted to Nanchang Hongdu Hospital of Traditional Chinese Medicine from January 2018 to January 2019 were selected. The patients were randomly divided into control group (50 cases, Teprenone + Colloidal Bismuth Pectin therapy) and study group (50 cases, Shihu Yangwei Decoction + Teprenone + Colloidal Bismuth Pectin therapy) according to stratified randomization method. The clinical efficacy, traditional Chinese medicine symptom score, the levels of inflammatory factor [C-reactive protein (CRP), tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6)], oxidative stress index [nitric oxide (NO), superoxide dismutase (SOD), malondialdehyde (MDA) and serum gastrin-17 (G-17), pepsinogen Ⅰ (PGⅠ), pepsinogen Ⅱ (PGⅡ) indicators and adverse reactions between two groups were compared. Results The total effective rate in study group was higher than that in control group (P < 0.05). After treatment, the scores of stomach tingling, food shortage, wasting, fatigue and fullness of stomach in two groups were lower than those before treatment, and the scores in study group were lower than those in control group (P < 0.01). After treatment, CRP, TNF-α and IL-6 in two groups were lower than those before treatment, and that in study group was lower than that in control group (P < 0.01). After treatment, the levels of G-17, PGⅠ and PGⅡ in two groups were higher than those before treatment, and the level in study group was higher than that in control group (P < 0.01). The levels of NO and SOD in two groups were higher than those before treatment, and the level in study group was higher than that in control group (P < 0.01). MDA level of two groups after treatment was lower than that before treatment, and that of study group was lower than that of control group (P < 0.01). There was no significant difference in incidence of adverse reactions between two groups (P > 0.05). Conclusion Shihu Yangwei Decoction combined with Western medicine has definite curative effect on CAG. It can significantly improve the clinical symptoms, it has high safety. It can provide reference for clinical treatment of CAG.

Effect of acupoint application of traditional Chinese medicine on constipation related to Oxkangding in cancer patients

Objective To evaluate the effect of acupoint application of traditional Chinese medicine on constipation related to Oxkangding in cancer patients. Methods Seventy-two cases of cancer patients with constipation caused by Oxycontin were selected in Oncology Department of Beijing Hospital of Traditional Chinese Medicine, Capital Medical University from January 2017 to June 2019. The patients who met the criteria were divided into treatment group (36 cases, acupoint application of traditional Chinese medicine) and control group (36 cases, oral Lactulose) by random number table method. The incidence of constipation, bowel function, quality of life [Karnofsky performance status (KPS)] and pain [numeric rating scale (NRS)] score between two groups was observed. Results There was no significant difference in incidence of constipation between two groups (P > 0.05). There was significant difference in incidence of constipation between treatment period and follow-up period in treatment group (P < 0.05). There was significant difference in incidence of constipation between treatment period and follow-up period in control group (P < 0.05). The differences were statistically significant between two groups in incidences of fecal restlessness, abdominal distention and abdominal pain before defecation (P < 0.05), and there was no statistically significant difference between two groups in incidence of hard stool (P > 0.05). There was no significant difference in KPS scores between two groups before treatment (P > 0.05). There was no significant difference in KPS scores between two groups after treatment (P > 0.05). The KPS scores in two groups after treatment were higher than those before treatment, the differences were statistically significant (P < 0.01). There was no significant difference in NRS score between two groups before treatment (P > 0.05). There was no significant difference in NRS score between two groups after treatment (P > 0.05). The NRS scores in two groups after treatment were lower than those before treatment, the defferences were statistically significant (P < 0.01). Conclusion Acupoint application of traditional Chinese medicine can effectively prevent and treat constipation caused by Oxycontin, and its effect is similar to Lactulose.

Clinical study of Xingnaojing Injection combined with Alteplase in the treatment of acute ischemic stroke

Objective To investigate the clinical effect of Xingnaojing Injection combined with Ateplase in the treatment of acute ischemic stroke. Methods From March 2015 to March 2019, a total of 86 patients with acute ischemic stroke admitted to 3201 Hospital Affiliated to Medical College of Xi′an Jiaotong University were selected. They were divided into the control group and the study group according to the random number table method, with 43 cases in each group. The control group was treated with Alteplase, while the study group was treated with Xingnaojing Injection on the basis of the control group. The clinical efficacy, inflammatory factors, internal carotid artery blood flow, degree of nervous functional defects, daily activity ability and adverse reactions of the two groups were compared. Results The total effective rate of the study group after treatment was significantly higher than that of the control group, and the difference was statistically significant (P < 0.05). There was no statistically significant difference in interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α) and high-sensitivity C-reactive protein (hs-CRP) between the two groups of patients before treatment (P > 0.05). After treatment, IL-6, TNF-α and hs-CRP of patients in both groups decreased compared with before treatment, and those in the study group were lower than those in the control group, the differences were statistically significant (P < 0.05). The blood flow of basilar artery, anterior, middle and posterior cerebral artery and vertebral artery of patients of the two groups before treatment showed no significant difference (P > 0.05). After treatment, the blood flow of basilar artery, anterior, middle and posterior cerebral artery and vertebral artery increased of patients in both groups, and those in the study group were higher than the control group, the differences were statistically significant (P < 0.05). There was no significant difference in the score of neurological deficit degree (NIHSS), the score of modified Barthel index (mBI) between the two groups of patients (P > 0.05). After treatment, the NIHSS score of patients of the two groups decreased compared with that before treatment, while the mBI score increased compared with that before treatment. After treatment, the NIHSS score of the study group was lower than that of the control group, while the mBI score was higher than that of the control group, the differences were statistically significant (P < 0.05). There was no significant difference in the incidence of adverse reactions of patients between the two groups (P > 0.05). Conclusion Xingnaojing Injection combined with Alteplase in the treatment of acute ischemic stroke has a definite effect, can effectively improve the level of inflammatory factors and internal carotid artery blood flow, and has a good safety, with certain clinical application value.

Clinical study of Pediatric compound amino acids combined with Ribavirin in the treatment of infantile herpetic pharyngitis

Objective To investigate the clinical study of Pediatric compound amino acids combined with Ribavirin in the treatment of infantile herpetic pharyngitis. Methods From June 2017 to December 2018, 80 cases of infantile herpetic pharyngitis admitted to Yan′an University Affiliated Hospital were selected and divided into the control group and the study group by random number table method, with 40 cases in each group. The control group was treated with Ribavirin. On the basis of the control group, the study group was treated with Pediatric compound amino acids. The clinical efficacy, clinical symptom disappearance time, myocardial injury index and inflammatory factor index of the two groups were compared, and the occurrence of adverse reactions in the two groups was recorded during the treatment. Results After 5 days of treatment, the total effective rate of the study group was higher than that of the control group, the difference was statistically significant (P < 0.05). After 5 days of treatment, creatine kinase isoenzymes (CK-MB), cardiac troponin Ⅰ (cTnⅠ), lactate dehydrogenase (LDH) of children in the two groups were all lower than those before treatment, and those in the study group was lower than those in the control group, the differences were statistically significant (P < 0.05). After 5 days of treatment, the levels of high-sensitivity C-reactive protein (hs-CRP) and tumor necrosis factor-α (TNF-α) of children in the two groups were lower than those before treatment, and those in the study group was lower than those in the control group, the differences were statistically significant (P < 0.05). The time of fever abatement, sore throat and salivation disappearance, herpes and ulcer disappearance and resumption of eating in the study group was shorter than those in the control group, the differences were statistically significant (P < 0.05). There was no significant difference in the total incidence of adverse reactions of children between the two groups(P > 0.05). Conclusion Pediatric compound amino acids combined with Ribavirin in the treatment of infantile herpetic pharyngitis have a definite curative effect. It can effectively improve the indexes of myocardium and inflammatory factors, without increasing the incidence of adverse reactions, and has high clinical value.

Effect of systematic family follow-up on treatment compliance and complications in patients after small-caliber vascular implantation

Objective To investigate the effect of systematic family follow-up on the treatment compliance and complications of patients after small-caliber vascular implantation. Methods From January 2015 to January 2018, 80 patients underwent stenting for lower limb atherosclerosis in Nanjing Drum Tower Hospital the Affiliated Hospital of Nanjing University Medical College were randomly selected and divided into control group and observation group according to the random number table method, with 40 cases in each group. The control group was given routine discharge nursing, and the observation group was given systematic family follow-up nursing on the basis of the control group. After 1 year of nursing, compliance, limb function and complications of patients in the two groups were observed. Results Ankle-brachial index (ABI), percutaneous oxygen partial pressure (TcPO2), and claudication distance (DOC) were not significantly different between the two groups before treatment (P > 0.05). The levels of ABI, TcPO2 and DOC in the two groups increased significantly after 1 year of nursing care, and the difference was statistically significant in the observation group compared with the control group (P < 0.05). The compliance of the observation group with reasonable diet, rehabilitation training, strict smoking cessation, medication adherence, regular reexamination and psychological adjustment were significantly higher than those of the control group, with statistically significant differences (P < 0.05). The total complication rate in the observation group (5.00%) was significantly lower than that in the control group (17.50%), the difference was statistically significant (χ2 = 5.000,P = 0.025). Conclusion Systematic family follow-up nursing effect is better, can effectively improve the function of the affected limb, enhance the patient′s compliance with the doctor′s advice, and reduce the occurrence of complications, promote the recovery of the patient′s condition, can be widely used in clinical.

Influence of personalized nutrition management based on network platform on the patients with hemodialysis malnutrition

Objective To explore the influence of personalized nutrition management based on network platform on maintenance hemodialysis (MHD) malnutrition patients. Methods From January 2016 to December 2017, 100 patients with MHD malnutrition in Renmin Hospital of Wuhan University were selected. According to the method of random number table, the patients were divided into control group and observation group, 50 cases in each group. The control group was given routine nutrition management (oral education and nutrition education manual) and follow-up for 6 months, the observation group was given routine nutrition management + network extension personalized nutrition management and follow-up for 6 months (WeChat and QQ platform). Nutritional assessment were conducted before and 6 months after nutrition management, modified quantitative subjective global assessment (MQSGA) scores, anthropometry, blood biochemical test. The mean execution days of individual recipes were also compared. Results There was no significant difference in MQSGA score between the two groups before nutrition management (P > 0.05); after 6 months of nutrition management, MQSGA scores in both groups were lower than before nutrition management (all P < 0.05), and the MQSGA score in the observation group was significantly lower than that in the control group (P < 0.01). There was no significant difference in TSF between the two groups before nutrition management (P > 0.05); TSF values in both groups increased significantly 6 months after nutrition management compared with those before nutrition management, and the observation group was significantly higher than that in the control group (P < 0.05). There was no significant difference in upper arm muscle circumference (AMC) between the two groups before nutrition management (P > 0.05); after 6 months of nutrition management, there was significantly increased in AMC the observation group compared with that before nutrition management (P < 0.05), while AMC was no significantly difference in the control group compared with that before nutrition management (P > 0.05), and AMC in the observation group was significantly higher than that in the control group (P < 0.05). Before nutrition management, there was no statistically significant difference in serum albumin (ALB), transferrin (TF) and hemoglobin (Hb) between the two groups (P > 0.05). After 6 months of nutrition management, all biochemical indexes in the observation group were higher than those in the control group (all P < 0.05). After 6 months of nutrition management, ALB, TF and Hb in the observation group were increased significantly compared with those before nutrition management (P < 0.05). Compared with the ALB value before nutrition management in the control group (P > 0.05), the TF and Hb value in the control group increased compared with those before nutrition management (P < 0.05). After 6 months of nutrition management, the average number of days of nutrition diet execution per month in the observation group [(29.4±0.5) d] was higher than that in the control group [(22.6±5.7) d] (P < 0.05), and the compliance of nutrition management in the observation group was significantly better than that in the control group. Conclusion Personalized nutrition management based on network platform can carry out out-of-hospital continuous nursing for MHD patients, which can improve nursing compliance and improve patients′ nutritional status.

Practice effect of accelerated rehabilitation surgical nursing in perioperative nursing of patients with hepatic cystic echinococcosis

Objective To explore the effect of accelerated rehabilitation surgical nursing in perioperative nursing of patients with hepatic cystic echinococcosis. Methods From October 2017 to February 2019, 106 patients in the Department of Hepatobiliary Echinococcus of the First Affiliated Hospital of Xinjiang Medical University were selected. Experimental methods were used to study patients with hepatic cystic echinococcosis who met the inclusion criteria. A total of 53 patients from October 2017 to May 2018 were selected as the control group. Fifty-three patients from June 2018 to February 2019 were selected as the intervention group. The pain score, self-care ability score and postoperative recovery of the two groups were compared. Results The pain score and self-care ability score of the two groups at different time points after the operation were higher than those before the operation, and the score of the intervention group was lower than that of the control group, the differences were statistically significant (P < 0.05). There were statistically significant differences between the two groups in the time of anal exhaust, defecation, postoperative first out-of-bed activity, hospital stay and cost (P < 0.05). Conclusion The practice of accelerated rehabilitation surgical care in patients with hepatic cystic echinococcosis is safe and effective.

Effect of intensive care on respiratory failure in children

Objective To explore the effect of severe nursing on respiratory failure in children. Methods From May 2016 to May 2017, 106 children with respiratory failure admitted to the Paediatric Care Unit of the First Affiliated Hospital of PLA Air Force Medical University were enrolled in the study. According to the random number table method, the patients were divided into control group and study group, with 53 cases in each group. The control group received routine care, the study group received intensive care. The intervention effect, treatment, respiratory function and nursing satisfaction of the two groups were compared. Results After intervention, the total effective rate of the study group was higher than that of the control group (P < 0.05). The duration of ventilation, hospitalization and intensive care in the study group were lower than those in the control group (P < 0.05). Peak airway pressure and airway resistance were lower in the study group than those in the control group (P < 0.05). The peak airway pressure and airway resistance in the study group were lower than those in the control group (P < 0.05), and the static compliance in the study group was higher than that in the control group (P < 0.05). The satisfaction of the study group was higher than that of the control group (P < 0.05). Conclusion In the nursing process of pediatric respiratory failure, intensive care nursing can effectively improve the nursing effect, shorten the duration of intensive care for patients, promote the recovery of children, improve nursing satisfaction, clinical application should be further promoted.

Application of salvage stent in interventional treatment of splenic aneurysm: a case repport

This paper retrospectively analyzes a case of splenic aneurysm patient in Department of Vascular Surgery, Xuanwu Hospital, Capital Medical University. After the implantation of the covered stent to cover the tumor cavity, the treatment process of the proximal splenic artery dissection and the implantation of the salvage bare stent are conducted due to the difficulty in operation. The final results show that after splenic artery stent implantation, there is no internal leakage in the aneurysm cavity, and the blood flow in the stent is unobstructed, and the distal blood flow is unobstructed. The treatment methods and efficacy of splenic aneurysm are further discussed, and the characteristics and diagnosis and treatment methods of this disease are summarized and reviewed in the literature. It is concluded that endovascular stent implantation is safe, minimally invasive and effective for splenic aneurysms.

Application of standardized clinical trial mode in clinical teaching for professional postgraduate of oncology

In order to meet the needs of the society for high-quality oncology professionals, it is imperative to improve the quality of postgraduate education of oncology. The new teaching model based on the standardized drug clinical researches was taught by invites teachers who have passed GCP training to give lectures on anti-tumor drug clinical trials and lead graduate students to participate in clinical work. The invites teachers guide postgraduates to use the clinical trial researcher′s manual to self-study and participate in subject management and all aspects of clinical trials. This teaching model is not only beneficial for the graduate students to receive systematic, standardized, comprehensive and strict clinical training of oncology and improve their thinking and ability of clinical trial, but also conducive for the tutor to integrate clinical teaching and scientific research. It is an effective way to promote the standardized development of postgraduate education of oncology and the continuous improvement of education quality, and is also a key link of the talent pool for the sustainable development of China′s drug research and development.

Research on the regulation path of rational exercise of emergent medical right

When patients are facing life-threatening emergency circumstances, they are badly in need of receiving surgery or other specific treatment, while medical institutions are often unable to obtain the consent from patients or their family members. In order to protect the basic human right of life and health, the laws empower medical institutions to exercise the right of emergency medical treatment, which is conducive to ease the tension between medical institutions and patients to some extent. However, in medical practice, the limits of exercising the emergency medical right are unclear, and there are some problems, such as exercising the right over actively or indolently, and the failure of communication strategy. It is necessary to take reasonable regulation by means of accurately recognizing the nature and application scenarios of emergency medical treatment, setting up emergency response system and establishing the normalized feedback mechanism of rights.

Screening and application of English core words in basic medicine

Objective To screen a core medical English vocabulary in basic medical subject and improve teaching effect of medical English vocabulary. Methods A basic medical English corpus of about 2 million words was built from the end of 2014 till the end of 2018 and a core medical word list was screened out by using both quantitative and qualitative research methods and consulting professional teachers from January to May 2019. Results A total of 1748 (actually 2357 unreduced vocabulary) word families were screened as the core medical word list, which had a coverage of 17.5% in the entire corpus. Conclusion The screening of the core medical word list in basic sciences of medicine can not only improve the pertinence of classroom vocabulary teaching, but also has important research value in the aspects of textbook writing, dictionary compilation and learning software development.

Combining with the practice that Beijing Tongren Hospital, Capital Medical University used WeChat official accounts to build a platform for medical popularization science, drew support from WeChat multimedia typies to enrich the expression forms of medical popularization science, used message function of WeChat official account to extend the accessibility of medical popularization science, and established WeChat matrixes to enhance the effects of medical popularization science, this paper proposes some suggestions that public hospitals ought to establish new media popularization science teams, achieve precision medical popularization science, build a brand of medical popularization science and pay attention to copyright protection of popular science, as well as exploring a new mode of medical popularization science work and promoting citizens′ health literacy.

Comments and intervention analysis of unreasonable outpatient prescription pharmacists before dispensing

Objective To investigate comments and intervention analysis of unreasonable outpatient prescription pharmacists before dispensing. Methods 192 300 prescriptions were collected from January to June 2018 in Ma′anshan People′s Hospital of Anhui Province. Pareto chart was used to analyze the main types of irrational prescriptions, the causes and formulate intervention strategies were analyzed. Prescription intervention was conducted from July to December 2018 to observe the change of the incidence of irrational prescriptions. Results There were 14 042 irrational prescriptions among 192 300 prescriptions before intervention, accounting for 7.30%. Pareto chart showed that the main types of irrational prescription are no diagnosis or incomplete diagnosis, unsuitable usage and dosage, and unsuitable drug selection. There were 8245 irrational prescriptions among 196 310 prescriptions after intervention, accounting for 4.20% after intervention. The incidence of irrational prescriptions decreased significantly (P < 0.05). Conclusion The intervention of pharmacists before prescription dispensing can effectively reduce the incidence of irrational prescriptions and improve the level of pharmaceutical care.
