ZHU Chaoyu XIAO Yuanyuan YE Yang LIU Mengdan ZENG Yuanyuan WEI Li▲
Objective To detect the effects of Fenofibrate on the expression of adipose triglyceride lipase (ATGL) in tissues of insulin resistant rats. Methods Eight-week old male SD rats were randomly divided into normal diet group (NC group, n = 8) and high fat diet group (HFD group, n = 16). After 8 weeks, the rats in the HFD group were divided into untreated (HFD-UT) group and Fenofibrate treatment (HFD-FT) group. The biochemical indices of all groups were detected, and the insulin sensitivity was detected by oral glucose tolerance (OGTT) and insulin tolerance (ITT). Western blot was used to assay ATGL protein in epididymal adipose, liver and muscle. Results High fat diet led to insulin resistance, ATGL in adipose tissue, liver and muscle in HFD-UT group were decreased compared with those of NC group, the differences were highly statistically significant (P < 0.01). After Fenofibrate treatment, the insulin sensitivity was improved, and the ATGL levels in adipose tissue, liver and muscle were increased compared with those of HFD-UT group, the differences were statistically significant (P < 0.01 or P < 0.05). Conclusion Fenofibrate can enhance the ATGL levels in adipose tissue, liver and muscle of insulin resistant rats.

The role of DEPTOR-mTOR signaling pathway-mediated autophagy on osteoclasts differentiation and maturation in multiple myeloma
ZHANG Haoran1 QIAO Xuxu1 BI Minghong1 ZHAI Yunzhi1 QIAN Liyu2 PAN Chengwu2 ZHAO Lun1
Objective To study the role of DEPTOR knockdown in RPMI-8226 cells on the protein expression of RANKL and differentiation of THP-1 into osteoclast-like cells in a contactless co-culture system and its possible mechanism. Methods Constructed DEPTOR shRNA expression vector GV115-shRNA was transferred into RPMI-8226 cell to produce packaged lentivirus. Western blot was applied to measure the protein levels of DEPTOR and RANKL. The expression of autophagy-associated proteins LC-3 and Atg5 were confirmed by Western blot analysis. Three groups were divided:THP-1 group, THP-1 + RPMI-8226 group and THP-1 + DEPTOR shRNA group. Osteoclast-like cells were identified by TRAP. The mRNA levels of calcitonin receptor (CTR) and Cathepsin-K were examined using RT-PCR. Results The results showed that protein expression levels of DEPTOR and RANKL were significantly lower in RPMI-8226 cells transfected with GV115 DEPTOR shRNA compared with that in untransfected cells (P < 0.05). The expression levels of autophagy-associated proteins LC-3 and Atg5 in the DEPTOR shRNA group were significantly lower than those in the control shRNA group and the parental group (P < 0.05). In the co-culture system, THP-1 cell could differentiate into TRAP positive multinuclear cells. RPMI-8226 promoted mRNA expression of CTR and Cathepsin-K (P < 0.05). DEPTOR shRNA suppressed osteoclast-like cells formation and decreased CTR and Cathepsin-K mRNA expression in co-cultures, the differences were statistically significant (P < 0.05). Conclusion In the coculture system, DEPTOR shRNA inhibits the differentiation of THP-1 cells into TRAP positive multinuclear cells, which may be due to its inhibition on RANKL expression in RPMI-8226 cells, and the inhibition of autophagy will restrain osteoclast maturation.

LIU Dehua1 WANG Jian2 GAO Zhiqiang1 ZOU Liansheng1 XU Ying3
Objective To investigate the mechanism of Toll like receptor 4 (TLR4)/ nuclear factor κB (NF-κB) signal pathway in rats with incisional pain. Methods One hundred healthy adult male Sprague-Dawley (SD) SPF rats of 20 weeks old were selected to establish incision pain models. Before operation and after operation for 0.5 h, 1 h, 2 h, 8 h, 12 h, 1 d, 2 d, 3 d, 5 d, 7 d, the mechanism withdraw threshold (MWT) in non-operative side and operative side of rats were detected by electronic von Frey pressure pain threshold detector. Before operation and after operation for 2 h, 8 h, 1 d, 2 d, 3 d, 5 d and 7 d, the expression of TLR4, NF-κB, interleukin 1β (IL-1β) and tumor necrosis factor α (TNF-α) in the spinal cord were detected by real-time fluorescence quantitative PCR (real-time PCR). Results After operation, the MWT in the operative side of rats was lower than that before operation, which was the lowest after operation for 2 h, then increased gradually, and they were significantly lower than those of non-operative side (P < 0.05). After operation, the expression levels of TLR4, NF-κB, IL-1β and TNF-α in spinal cord of rats were increased compared with those before operation, which of postoperative 2 h, 8 h, 1 d, 2 d and 3 d had statistically significant differences compared with those before operation (P < 0.05), which of postoperative 5 d and 7 d had no statistically significant differences compared with those before operation (P > 0.05). The expression levels of TLR4 and NF-κB in the spinal cord reached the highest after operation for 1 d, and the expression levels of IL-1β and TNF-α reached the highest after operation for 8 h. Pearson correlation analysis showed that there was a negative correlation of MWT in operative side with the expression of TLR4 and NF-κB (r = -0.473, P < 0.05, r = -0.398; P < 0.05), the TLR4 levels in spinal cord of rats were positively correlated with the expression levels of IL-1β and TNF-α (r = 0.302, P < 0.05; r = 0.538, P < 0.05), and the NF-κB levels in spinal cord of rats were positively correlated with the expression levels of IL-1β and TNF-α (r = 0.283, P < 0.05; r = 0.467, P < 0.05). Conclusion The signal pathway of TLR4/NF-κB in spinal cord of rats with incisional pain is activated, the expression levels of TLR4 and NF-κB are increased, and the expression levels of downstream inflammatory cytokines IL-1β and TNF-α are up-regulated, which is involved in the regulation process of incisional pain.

The exploration for development of medical alliance in psychiatric hospitals in Shanghai
XIE Fei XIE Bin▲
According to medical reform policy requirements, medical alliances in China were established one after another in health institutions and achieved the initial success. But there are still many problems to be solved, a number of "test water breaking" medical couplet presents a state of "incompatibility", which led to the alliances could not play the functions completely, even affected the formation of the domestic hierarchical medical system. There are many problems between insufficient government investment, self-governance among specialist hospitals, poor coordination and unreasonable distribution of mental health resources in Shanghai city mental health management and service, to carry out establish psychiatric medical alliances is one of the effective ways to solve these problems, and it has realistic significance. It is suggested that in the establish psychiatric medical alliances, government should play a leading and coordinating role in the construction of the psychiatric medical alliances, it is responsible for the top-level design and coordination of the organization, clarifying the four basic principles of fairness, accessibility, hierarchical medical care and the leading role of public hospitals. Give different supporting measures and policy support according to different needs, and increase the investment on the construction of medical alliance. So that the psychiatric medical alliances can return to the initial purpose of the establishment of the hierarchical medical system and reach the goal of the new medical reform, which aims to effectively alleviate "difficulties and expensive medical treatment".
