4 A primary culture method of human renal carcinoma cells and normal cells based on RNA-seq validation

Objective To establish a more rapid and convenient method for primary culture of renal tumor and normal renal tubular epithelial cells. Methods Fresh clear renal cell carcinoma tissue and renal cortical tissue from patients undergoing radical nephrectomy and partial nephrectomy in Beijing Chaoyang Hospital, Capital Medical University from January to March 2019 were selected. The tumor and normal primary cells were purified by mechanical segmentation, collagenase digestion and continuous cell screening and purification. The genetic background of the two primary cells was identified by transcriptomic sequencing (RNA-seq) technique, the expression of cell markers was identified by immunocytochemistry, and the ultrastructure of the primary cells was observed by transmission electron microscopy. Finally, the tumorigenesis characteristics of tumor cells were identified by the model of nude mice bearing tumors. Results A total of three human proximal tubular epithelial primary cells and eight renal clear cell carcinoma primary cells were isolated and purified. RNA-seq showed that the sequencing results were consistent with the genetic background of the two types of cells, and immunocytochemistry could identify the expression of marker proteins in the proximal renal tubular epithelial cells. Two kinds of cell specific ultrastructure could be observed by transmission electron microscope. The tumor bearing model of nude mice confirmed the tumorigenic properties of primary renal cell carcinoma cells. Conclusion A rapid and convenient processes and methods for primary culture of renal carcinoma cells and renal tubular epithelial cells is successfully established.

8 Analysis of prognostic factors of extensive disease-small cell lung cancer

Objective To investigate the prognostic factors of extensive disease-small cell lung cancer. Methods The clinical data of 157 patients with extensive disease-small cell lung cancer who were hospitalized in China-Japan Friendship Hospital and Wangjing Hospital of Chinese Academy of Chinese Medical Sciences from January 2011 to January 2019 were analyzed retrospectively. Log-Rank test was used for univariate analysis, Kaplan-Meier method was used for comparison of survival curve between groups, and multivariate COX proportional hazard regression model was used for multivariate analysis of variables with statistically significant difference indicated by univariate analysis. Results Six factors, including patient gender, whether have presence of obstructive pneumonia at the time of diagnosis, liver metastasis, whether have thoracic radiation therapy, number of cycles of first-line chemotherapy, and number of cycles of second-line chemotherapy, were independent influencing factors for prognosis of extensive disease-small cell lung cancer (P < 0.05). Among them, female, the first-line chemotherapy ≥one cycle, number of the second-line chemotherapy cycles four to six cycles, and have thoracic radiation therapy were protective factors for the prognosis of extensive disease-small cell lung cancer patients (P < 0.05), and presence of obstructive pneumonia at the time of diagnosis and liver metastasis were risk factors for the prognosis of extensive disease-small cell lung cancer patients (P < 0.05). Conclusion Extensive disease-small cell lung cancer patients with female gender, the first-line chemotherapy cycles ≥one cycle, and the second-line chemotherapy cycles four to six cycles, have thoracic radiation therapy, have better prognosis, while extensive disease-small cell lung cancer patients with obstructive pneumonia and liver metastasis at the time of diagnosis have worse prognosis.

13 Study on the interventional effect of acupuncture nerve entering points on musculi quadriceps femoris of cerebral apoplexy muscle spasm based on sEMG evaluation

Objective To observe the effect of surface electromyography (sEMG) on the control of the knee joint of the lower extremities with spasm in patients with hemiplegia by using acupuncture nerve entering points on musculi quadriceps. Methods A total of 52 cerebral apoplexy patients confirmed in Taizhou Hospital of Zhejiang Province and the Second Affiliated Hospital of Wenzhou Medical University from January 2016 to December 2017 were selected and divided into study group and control group according to the random number table method, with 26 patients in each group. The two groups were given the same basic treatment, the control group was treated with traditional acupuncture therapy with acupuncture points following the meridian, and the study group was treated with acupuncture nerve entering points on musculi quadriceps femoris. In both groups, the Holden walking functional classification, modified Ashworth scale (MAS), clinic spasticity index (CSI), the lower limb simplified Fugl-Meyer movement scale and Berg balance scale (BBS) were evaluated, electromyographic signals guided by the surface electrode were recorded and the changes of the measured maximum amplitude of H wave (Hmax), maximum amplitude of M wave (Mmax) and the ratio of the maximum amplitude of H wave to M wave (Hmax/Mmax) were analyzed and compared before and four weeks after treatment. Results After four weeks of treatment, the scores of Holden functional walking ability, lower limb simplified Fugl-Meyer movement scale and BBS in the two groups were all improved compared with those before treatment, and the study group was higher than the control group, with statistically significant differences (all P < 0.05). After four weeks of treatment, lower limb MAS and CSI scores of both groups were lower than those before treatment, and the study group was lower than the control group, with statistically significant differences (all P < 0.05). After four weeks of treatment, Hmax, Mmax and H/Mmax of the two groups were all lower than those before treatment, and the study group was lower than the control group, with statistically significant differences (all P < 0.05). Conclusion Using acupuncture nerve entering points on musculi quadriceps femoris can alleviate side lower limb spastic hemiplegia patients quadriceps femoris muscle tension, improve the coordination between anterior and posterior muscles of the thigh, and improve the knee joint control ability of patients with spastic hemiplegia, so as to further improve the quality of life of patients, its curative effect is better than traditional acupuncture method.

17 Evaluation of the effect of PDCA cycle applied in drug management in ward

Objective To evaluate the role and significance of PDCA cycle in drug management in ward. Methods The problems in the drug management examination in ward from January to June, 2018 in Chengdu Fifth People′s Hospital were retrospective analyzed. From July to December 2018, the PDCA cycle management method was used to intervene the existing problems in drug management in the ward, and the application effects before and after the implementation of PDCA cycle management were compared. Results Compared with before the implementation of PDCA cycle management, after the implementation of PDCA cycle management, the number of defects in effective period management decreased by 68.33%, the number of defects in use management decreased by 73.42%, and other defects not as the main target also improved significantly, with a total number of defects decreased by 71.09%. Conclusion PDCA cycle management method in ward can improve the quality of drug management, reduce medication errors, promote the standardization and scientization of drug management, and enhance the safety and effectiveness of medication.

21 Breast cancer ultrasound SWE technical analysis and its correlation with ER, PR, Ki-67 expression and molecular typing

Objective To explore the analysis results of breast cancer ultrasound shear wave elastography (SWE) technical analysis and its correlation with estrogen receptor (ER), progesterone receptor (PR), Ki-67 antigen expression and molecular typing. Methods From June 2017 and October 2019, 100 patients with pathologically confirmed breast cancer who admitted in Wanbei Coal and Electricity Group General Hospital of Anhui Province were selected as research objects. Before the treatment, routine examination and SWE examination were performed in order to obtain the results of SWE technical analysis of the patients′ lesions. Postoperatively, the tumor tissue grade, ER, PR, Ki-67 antigen expression level and molecular typing of all patients were observed and recorded, and the correlation between the results of SWE technical analysis before operation and the expression levels of ER, PR, Ki-67 antigen and molecular typing in postoperative patients were analyzed. Results The differences of tumor size, Ki-67 antigen expression, tissue grade and elastic maximum square root [Sqrt(Emax)], elastic maximum square root [Sqrt(Emean)] and elastic ratio number [Log(Eratio)] were statistically significant (P < 0.05). However, there were no significant differences between ER expression level, PR expression level, Human epidermal growth factor receptor-2 (HER-2) expression level, molecular typing and Sqrt(Emax), Sqrt(Emean), Log(Eratio)(P > 0.05). The tumor size and Ki-67 antigen expression were correlated with Sqrt(Emax) and Sqrt(Emean) (P < 0.05), but not with Log(Eratio) (P > 0.05). There were no correlation between tumor tissue grade and Sqrt(Emax), Sqrt(Emean) and Log(Eratio) (P > 0.05). Conclusion SWE elasticity parameter can reflect the biological characteristics of breast cancer to a certain extent. The larger the mass, the higher the SWE elasticity value, and the higher the expression of Ki-67.

26 Effect of endocrine therapy on hormone receptor positive breast cancer and its relationship with ESR1 gene locus

Objective To study the effect of endocrine therapy on hormone receptor positive breast cancer and its relationship with ESR1 gene locus (rs2077647 and rs3798759). Methods Eighty-six patients with hormone receptor positive breast cancer admitted to the Fifth People′s Hospital of Changshu City in Jiangsu Province from January 2014 to December 2018 were selected as study objects. All patients were treated with endocrine therapy and ESR1 single nucleotide polymorphisms (rs2077647 and rs3798759) were detected. The short-term efficacy of endocrine therapy was analyzed and its relationship with ESR1 single nucleotide polymorphisms was observed. Results Forty five patients of 86 patients with complete remission, partial remission and stable disease after endocrine therapy were included in ETR group, 41 patients with disease progression were classified into ETNR group. The proportion of genotype CC at rs2077647 in ETR group was significantly higher than that in ETNR group (P < 0.05), the proportion of TT genotype of rs3798759 was significantly lower than that of ETNR group (P < 0.05), and the proportion of TG genotype was significantly higher than that of ETNR group (P < 0.05). The proportion of allele C at rs2077647 in ETR group was significantly higher than that in ETNR group (P < 0.05), and the proportion of allele G at rs3798759 was significantly higher than that in ETNR group (P < 0.05). ESR1 gene polymorphism was an independent factor affecting the effect of endocrine therapy (P < 0.05). Conclusion The effect of endocrine therapy on hormone receptor-positive breast cancer is closely related to ESR1 single nucleotide polymorphisms (rs2077647 and rs3798759).

30 Application status of defect repair of abdominal free dermal fat graft in breast-conserving surgery

For early-stage breast cancer, both the safety of tumor treatment and the breast shape after surgery are the same goals pursued by doctors and patients. At present, the repairing of tissue defect after breast-conserving therapy mainly depends on oncoplastic breast surgery techniques, including glands displacement and autologous tissue breast replacement. Because the breast size of the Asian women are not big enough to undergo repairing by simple glands displacement after breast-conserving surgery, using an autologous tissue to fill up the defect can produce good cosmetic outcomes. However, the latissimus dorsi muscle flap, transverse rectus abdominis-myocutaneous flap, inferior abdominal artery perforator flap, and so on, which have been tried in the past, are difficult to carry out due to the difficulty of surgery and requirement of professional microsurgical techniques. Japanese scholar Kijima is the first one who use an autologous free dermal fat graft for breast defect tissue repair, which shows good cosmetic outcomes and simple to operating. This method is expected to be promoted clinically. Due to the short history of this operation, it still needs a long-term follow-up with large samples to scientifically evaluate the long-term postoperative cosmetic outcomes and complications.

34 Progress on individualized administration of Linezolid

Due to Vancomycin resistance and adverse reactions, Linezolid has become increasingly important in the treatment of infections caused by Methicillin-Resistant Staphylococcus Aureus and other multi-resistant and pan-resistant bacteria. However, research on how Linezolid can be personalized medicine is still insufficient. Studies have shown that the pharmacokinetics of Linezolid are affected by various factors such as age, gender, weight, liver and kidney function, and genes, which can lead to individual differences in efficacy and adverse reactions. Many studies have confirmed the role of therapeutic drug monitoring in the individualized administration of linezolid, but its application value remains to be further confirmed. Pharmacokinetics/pharmacodynamics studies have shown that individualized dosing regimens should be designed based on the patient′s disease status and drug sensitivity results. Therefore, clinical pharmacists should use therapeutic drug monitoring and pharmacokinetics/pharmacodynamics actively participate in clinical treatment to improve the effectiveness and safety of Linezolid anti-infective treatment.

37 Correlation study between health empowerment and coping style in elderly patients with cardiovascular disease

Objective To explore the correlation between health empowerment and medical coping style of elderly patients with cardiovascular disease in hospital. Methods Convenient sampling method, the elderly chronic disease health empowerment scale and medical coping style questionnaire were used. From March to July 2019, 233 elderly patients with cardiovascular disease were selected and surveyed from the cardiovascular group of Geriatrics Department of the Second Hospital of Shanxi Medical University. Results Compared with national norm, the scores of facing and avoiding dimensions of hospitalized elderly patients with cardiovascular disease were lower, and the scores of yielding dimension were higher, the differences were statistically significant (P < 0.05). Health empowerment score of elderly patients with cardiovascular disease was (3.422±0.364) scores. Facing was positively correlated with of support, knowledge growth, treatment participation, self reconstruction and health empowerment total score (P < 0.01). Avoiding was negatively correlated with knowledge growth and positively correlated with self reconstruction (P < 0.05). There was a negative correlation between surrender and responsibility belief, support, knowledge growth, treatment participation, self reconstruction and health empowerment total score (P < 0.05 or P < 0.01). Knowledge growth and treatment participation were the main factors of health empowerment influencing disease coping, and responsibility belief and self reconstruction were the main factors of health empowerment influencing yield coping. Conclusion Health empowerment level of elderly patients with cardiovascular disease affects the choice of medical coping style. Medical staff can improve the self-health management and promote positive response to the disease of elderly patients with cardiovascular disease by improving their health empowerment level.

41 Correlation of epicardial adipose volume and coronary calcification score with coronary stenosis in 320-row CT

Objective To investigate the relationship between epicardial adipose tissue volume (EATV) measurement and coronary calcification score (CACS) with coronary stenosis in 320-row CT. Methods A total of 200 patients with coronary heart disease underwent 320-row CT examination at Taihe County Traditional Chinese Medicine Hospital of Anhui Province from October 2017 to April 2019 were selected as coronary heart disease group, and 40 patients underwent 320-row CT examination without coronary artery stenosis were selected as control group. The patients in coronary heart disease group were divided into low score group (Gensini score < 41 points, 85 cases), middle score group (41 points ≤ Gensini score < 64 points, 65 cases), and high score group (Gensini score ≥ 64 points, 50 cases) according Gensini score system. EATV and CACS of each group were recorded and compared. Results EATV and CACS in coronary heart disease group were higher than those in control group, the differences were statistically significant (P < 0.05). There were significant differences in EATV and CACS among three subgroups (P < 0.05). EATV and CACS in high score group were higher than those in low score group and middle score group, while EATV and CACS in middle score group were higher than those in low score group, the differences were statistically significant (P < 0.05). EATV was positively correlated with CACS, number of diseased vessels and Gensini score (r = 0.332, 0.421, 0.451, P < 0.05). CACS was positively correlated with the number of vessels and Gensini score (r = 0.359, 0.436, P < 0.05). The area under curve (AUC) of EATV was 0.75 (P < 0.05), the specificity was 65.2%, the sensitivity was 84.2%, and the cut-off value was 191.25 cm3. AUC of CACS was 0.75, the specificity was 61.8%, the sensitivity was 88.3%, and the cut-off value was 286.05 points. Conclusion The 320-row CT EATV and CACS are positively correlated with coronary artery stenosis in patients with coronary heart disease, and have certain value in predicting severe coronary artery stenosis.

45 Relationship study between homocysteine, troponin T, high sensitivity C-reactive protein and cardiac function in patients with chronic heart failure

Objective To investigate the relationship between homocysteine (Hcy), troponin T (TNT), high sensitive C-reactive protein (hs-CRP) and cardiac function in patients with chronic heart failure. Methods One hundred and fifteen patients with chronic heart failure admitted to the Department of Cardiology, Rocket Army General Hospital from January 2016 to January 2019 were selected as heart failure group. According to the New York Heart Association (NYHA) cardiac function classification standard, the heart failure group was divided into NYHA class Ⅱ group (26 cases), NYHA class Ⅲ group (35 cases) and NYHA class Ⅳ group (54 cases). Ninety-five healthy elderly people who had physical examination at the same time were selected as control group. The levels of Hcy, TNT, hs-CRP, NT-proBNP and left ventricular ejection fraction (LVEF) in each group were compared, and the correlation between Hcy, TNT, hs-CRP and NT-proBNP, LVEF were analyzed. Results The levels of TNT, hs-CRP, Hcy and NT proBNP in heart failure group were higher than those in control group, and LVEF was lower than that in control group (P < 0.01). The cardiac function was graded from grade Ⅱ to grade Ⅳ, the levels of serum TNT, hs-CRP, Hcy and NT-proBNP increased gradually, LVEF decreased gradually, the differences were statistically significant (P < 0.01). The levels of serum TNT, hs-CRP and Hcy were positively correlated with NT-proBNP (r = 0.75, 0.61, 0.69), and negatively correlated with LVEF (r = -0.71, -0.59, -0.64), and the differences were statistically significant (P < 0.05). Conclusion The levels of Hcy, TNT and hs-CRP in patients with heart failure are significantly increased, and the levels of Hcy, TNT and hs-CRP are positively correlated with NT-proBNP, and negatively correlated with LVEF. Therefore, they can be used as important indicators to evaluate the severity of heart failure.

49 Correlation between the disorder of peripheral blood lymphocyte subsets ratio and Parkinson′s disease

Objective To explore the correlation between neural immune mechanisms and the occurrence of Parkinson′s disease (PD) by detecting the percentage differences of lymphocyte subtypes in peripheral blood of patients with PD. Methods Fluorescence staining method was used to determine the periphery of primary PD patients (case group, 83 cases) admitted to the First Affiliated Hospital of Xinjiang Medical University (“our hospital” for short) from June 2014 to July 2017 and healthy physical examination patients (control group, 100 cases) in our hospital during the same period, while the differences in the percentage of CD3+, CD4+, CD8+, CD19+, CD56+, Fas and Tregs cells in the blood were measured, and the correlation between whether the patients in the case group took the drug Compound Levodopa, Hoehn-Yahr (HY) grade difference and the changes of lymphocyte subsets were analyzed. Results CD3+, CD4+, and CD8+ in the case group were lower than those in the control group, while the positive subsets of CD56+ cells, Fas and Tregs were higher than those in the control group, and the differences were highly statistically significant (P < 0.01). Fas and Tregs cell subsets in the medication group were lower than those in the non-administration group, and the differences were statistically significant (P < 0.05). Correlation analysis showed that the number of Fas and Tregs positive cells were positively correlated with HY grade (r = 0.620, 0.761, P < 0.05). Conclusion Peripheral blood lymphocyte subsets of PD patients have a disorder in the proportions. The the proportion of Fas and Tregs cells are related to the severity of the disease in PD patients. For PD patients who are not treated with oral Compound Levodopa, the proportion will further increase.

53 Study of Methazolamide in five patients with normal pressure hydrocephalus

Objective To study the effectiveness and safety of Methazolamide (MTZ) in the treatment of patients with normal pressure hydrocephalus (NPH), to explore its mechanism of action, to enrich NPH treatment methods, and to broaden new ways of MTZ application. Methods A total of four cases of idiopathic normal pressure hydrocephalus (iNPH) and one case of secondary normal pressure hydrocephalus (sNPH) were collected from 2019 to 2020 in Aviation General Hospital, China Medical University. All iNPH patients met the diagnostic criteria of the 2005 International iNPH Guidelines. The five patients were all tested positive for a single lumbar puncture and were not suitable for or refused surgical treatment for various reasons. After obtaining the consent of the patient and their family members, they were given oral MTZ 25-50 mg, two times per day for treatment. The gait score, cognitive assessment, bladder function measurement, head MRI examination, iNPH grading score before and after oral MTZ were evaluated. After oral administration of MTZ, an improvement in iNPH score is greater than or equal to one point was defined as effective, and adverse drug reactions could be closely observed to evaluate the effectiveness and safety of MTZ on NPH. Results Among the five patients, four case were effective, including one case was slightly improved, and three cases were improved significantly (two patients with iNPH and one patient with sNPH), showed improvements in gait, cognition or bladder function. None of the five patients found clear MTZ-related adverse reactions. Conclusion MTZ may become one of the potential drugs for the treatment of NPH, but it still needs to be further confirmed by large randomized controlled trials.

57 Mediating effect of ruminant meditation on self-efficacy and coping style of young CSR patients after emergency surgery

Objective To study the mediating effect of ruminating meditation on self-efficacy and coping styles of young patients with cervical spondylotic radiculopathy (CSR) after emergency surgery. Methods From February 2016 to December 2019, a total of 120 young patients with severe CSR who were treated at Dezhou People′s Hospital were treated as the research object. After training, all researchers used a one-to-one questionnaire survey to analyze the level of ruminant contemplation, self-efficacy, and coping styles in the first week after surgery, and the correlation between ruminant meditation and self-efficacy, coping style scores and mediating effects were studied. Results The questionnaire response rate in this study was 100%. The scores of ruminant meditation, self-efficacy, and coping style of patients with different genders and educational levels were compared, and the differences were statistically significant (P < 0.05). Patients′ ruminating meditation, invasive rumination meditation, purpose rumination meditation and self-efficacy (r = 0.458, 0.521, 0.620) and coping ability (r = 0.521, 0.662, 0.851) showed positive correlation (P < 0.05). Self-efficacy had a direct predictive effect on the purposeful ruminant meditation (r = 0.226, P = 0.000). Through the purposeful ruminant meditation of the patient, the indirect analysis of the patient affects the patient′s coping style. The mediating effect of ruminant meditation on patients indirectly affects the patient′s self-efficacy and coping style. The indirect effect was 0.226 and the mediating effect was 21.56%. The fitting equation of ruminating meditation for the evaluation of patient′s self-efficacy was as follows: Y=0.99+0.33X1+0.56X2 (Y: patient′s self-efficacy; X1: ruminating meditation; X2: coping ability). Conclusion The mediating effect of rumination meditation in young patients with CSR after emergency surgery indirectly affects the patient′s self-efficacy and coping style. After the operation, the patient′s ruminant meditation level can be used to guide clinical treatment.

61 Banxia Baizhu Tianma Decoction combined with western medicine in the treatment of vertebrobasilar ischemic vertigo:a meta-analysis and sequential analysis

Objective To systematically evaluate the clinical efficacy of Banxia Baizhu Tianma Decoction combined with Western medicine on vertebrobasilar ischemic vertigo (VBIV). Methods PubMed database, Cochrane Library database, EMbase database, CBM database, CNKI database, VIP database, and WangFang database were searched to screen clinical study about Banxia Baizhu Tianma Decoction combined with traditional Western medicine in the treatment of VBIV, and the search time was from the establishment to October, 2019. Meta analysis and trial sequential analysis of included studies were carried out using Stata 13.0 software. Results A total of 28 studies involving 2494 patients were included. The comprehensive clinical efficacy of the combination of Banxia Baizhu Tianma Decoction and traditional western medicine (RR = 1.19, 95%CI[1.15,1.23], P < 0.001), left vertebral artery average blood flow velocity (WMD = 5.74,95%CI[4.64, 6.84], P < 0.001), right vertebral artery average blood flow velocity (WMD = 5.58, 95%CI[4.32, 6.84], P < 0.001), mean flow velocity of basilar artery (WMD = 5.04, 95%CI[3.91, 6.18], P < 0.001), whole blood viscosity (low cut) (WMD = -1.30, 95%CI[-2.23, -0.38], P < 0.001), whole blood viscosity (high cut) (WMD = -1.22, 95%CI[-1.48,-0.96], P < 0.001), plasma-viscosity (WMD = -0.26,95%CI[-0.33,-0.18], P < 0.001), fibrinogen(WMD = -0.7,95%CI[-0.99, -0.55], P < 0.001) were better than the control group, and the differences were statistically significant (P < 0.01). Trial sequential analysis further indicated that Banxia Baizhu Tianma Decoction combined with Western medicine had a definite clinical effects in the treatment of VBIV. Conclusion Combining with Banxia Baizhu Tianma Decoction has better comprehensive effect for VBIV. This conclusions still need to be verified by high quality and large sample randomized controlled trials.

66 Application value of thromboelastography in the antithrombotic treatment of acute atherosclerotic cerebral infarction

Objective To investigate the value of thromboelasto graph (TEG) in antithrombotic therapy for acute atherosclerotic cerebral infarction (CI). Methods A total of 160 patients with acute atherosclerotic CI admitted to Hebei Petro China Central Hospital from December 2017 to July 2019 were selected as the study subjects, according to random number table method, they were divided into the Aspirin group (n = 40, Aspirin antithrombotic therapy), Clopidogrel Bisulfate group (n = 40, Clopidogrel Bisulfate Antithrombotic therapy), the joint group (n = 40, Aspirin combine with Clopidogrel Bisulfate antithrombotic therapy), TEG guide group (n = 40, based on the guidance of TEG antithrombotic therapy, high platelet inhibition rate of antithrombotic drug treatment individualized programs was chosen). Clinical efficacy, microembolus (MES) monitoring, daily life ability and neurological function impairment, adverse reactions, bleeding events and recurrence were compared in each group. Results After treatment, the total clinical response rate of the combined group and the TEG guide group were higher than those of the Aspirin group and Clopidogrel Bisulfate group (P < 0.05). After treatment, the MES positive rate in all four groups were lower than those before treatment (P < 0.05), and the MES positive rate in both the combined group and the TEG guide group were lower than those in the Aspirin group and Clopidogrel Bisulfate group (P < 0.05). After treatment, the improved Barthel index scores of the four groups were higher than those before treatment, and the NIHSS scores were lower than those before treatment (P < 0.05), the improved Barthel index score of the combined group and the TEG guide group were all higher than those of the Aspirin group and Clopidogrel Sulfate group (P < 0.05), the NIHSS scores were lower than those of Aspirin group and Clopidogrel Bisulfate group (P < 0.05). The incidence of bleeding events in Clopidogrel Bisulfate group and TEG guide group were lower than those in the combined group (P < 0.05). After treatment, the recurrence rates of the combined group and the TEG guide group were all lower than those of the Aspirin group and Clopidogrel Bisulfate group (P < 0.05). Conclusion In the antithrombotic therapy of acute atherosclerosis CI patients, TEG-guided individualized therapy has better clinical effect and higher safety than Aspirin and Clopidogrel Bisulfate alone.

71 Relationship between serum β-amyloid protein and cognitive dysfunction in patients with liver failure

Objective To study the relationship between serum β-amyloid protein (Aβ) and cognitive dysfunction in patients with liver failure and provide guidance for clinical research. Methods From April 2019 to April 2020, 104 patients with liver failure admitted to the Zhejiang Hangzhou Xixi Hospital (hereinafter referred to as “our hospital”) were selected for study. The cognitive function was assessed by the Montreal cognitive assessment scale (MoCA) and the patients were divided into the cognitive-normal group (n = 56, MoCA score ≥ 26 points) and the cognitive-impaired group (n = 48, MoCA score <26 points), in addition, 52 healthy controls in the Medical Center in our hospital were selected as the control group. Serum Aβ was detected by enzyme-linked immunosorbent assay to determine the relationship between serum Aβ and cognitive dysfunction in patients with liver failure. Results MoCA score, levels of Aβ 1-40 and Aβ 1-42 were compared among the three groups, and the differences were statistically significant (P < 0.05). MoCA score and Aβ 1-42 levels in the cognitive impairment group were significantly lower than those in the cognitive-impaired and control groups, and Aβ 1-40 levels in the cognitive-impaired group were significantly higher than those in the normal and control groups, with statistically significant differences (P < 0.05). Logistic regression analysis showed that, age > 55, high Aβ 1-40 level, and low Aβ 1-42 level were independent risk factors for cognitive dysfunction in patients (P < 0.05). Correlation analysis showed that cognitive dysfunction in patients with liver failure was positively correlated with Aβ 1-42 level (r = 0.685, P = 0.000) and negatively correlated with Aβ 1-40 level (r = -0.714, P = 0.000). Conclusion Serum Aβ 1-42 levels are significantly decreased in patients with cognitive dysfunction of liver failure, which is positively correlated with MoCA score; serum Aβ 1-40 levels are significantly increased, which is negatively correlated with MoCA score. Both of them are risk factors for the occurrence of cognitive dysfunction, and their detection combined with MoCA assessment could provide certain reference for clinical diagnosis.

75 Difference analysis of clinical characteristics of polycystic ovary syndrome patients with different body mass index

Objective To study clinical characteristics, sex hormone level, glucose metabolism level and lipid metabolism level of polycystic ovary syndrome (PCOS) patients with different body mass index (BMI), and to explore the effect of obesity on PCOS. Methods From January 2015 to August 2018, 660 patients with PCOS were selected from Gynecology Clinic in the First Affiliated Hospital of Heilongjiang University of Chinese Medicine. According to different BMI, the patients were divided into normal group (BMI 18.5-<23.0 kg/m2) of 243 cases, overweight group (BMI 23.0-<25.0 kg/m2) of 100 cases, obesity group (BMI ≥ 25 kg/m2) of 317 cases. Clinical characteristics, sex hormone, glucose and lipid metabolism were compared among three groups. Results There were no significant differences in age and hirsutism among three groups (P > 0.05). There were significant differences in systolic blood pressure, diastolic blood pressure, waist circumference, hip circumference, waist hip ratio, acne and black acanthosis among three groups (P < 0.05). The levels of follicle stimulating hormone, luteinizing hormone, follicle stimulating hormone/luteinizing hormone, testosterone, rostenedione and sex hormone binding globulin were significantly different among three groups (P < 0.05). There were no significant differences in estradiol and dehydroepiandrosterone sulfate among three groups (P > 0.05). There were significant differences in fasting blood glucose, fasting insulin and insulin resistance index among three groups (P < 0.05). There were significant differences in triglyceride, total cholesterol, high density lipoprotein cholesterol and low density lipoprotein cholesterol among three groups (P < 0.05). Conclusion In obese PCOS patients, fat distribution is abdominal obesity, androgen level is higher, glucose and lipid metabolism disorder is more obvious.

79 Expression of myeloid-derived suppressor cells in targeted phosphatase of regeneratiing liver-3 mouse breast tumor gene immunity

Objective To observe the expression of myeloid suppressor cells (MDSC) in targeted phosphatase of regeneratiing liver-3 (PRL-3) mouse breast tumor gene immunity, and to explore the relationship between the number of MDSC and tumor growth. Methods Forty female BALB/c mice were divided into five groups (eight groups): no-load control group (pVAX1 [-] group), adjuvanted PRL-3 group (K-PRL3 group), adjuvanted PRL-3 fusion protein group (K-T-PRL3 group and K-PRL3-MT group), and MDSC control antibody deletion group (K-T-PRL3+Ab control group and K-PRL3-MT+Ab control group). In the MDSC specific antibody deletion group (K-T-PRL3+ GR-1AB group and K-PRL3-MT+ GR-1 Ab group), immunophenotypes CD45, GR-1 and CD11b were detected by flow cytometry to analyze the expression levels of MDSC in peripheral blood, tumor and spleen of each immune group, and the correlation between MDSC and tumor growth was observed. Results On the 28th day after tumor-bearing, the tumor volume of K-PRL3 group, K-T-PRL3 group and K-PRL3-MT group was all smaller than pVAX1 (-) group, with statistically significant differences (all P < 0.05).There was no significant difference in tumor volume between the K-PRL3-mt group and K-T-PRL3 group and K-PRL3 group (P > 0.05). The expression of MDSC in peripheral blood or tumor tissues of the adjuvant -PRL-3 fusion protein group was higher than that of the K-PRL3 group, and the difference was statistically significant (P < 0.05). There was no significant difference in tumor volume between MDSC specific antibody deletion group (K-T-PRL3 + GR-1 Ab group and K-PRL3-mL + GR-1 Ab group) and MDSC Control group (K-T-PRL3 +Ab control group and K-PRL3-MT +Ab control group) (P > 0.05). The tumor volume of the MDSC specific antibody deletion group and the MDSC control antibody deletion group were all larger than those of the adjuvant-free PRL-3 group and the adjuvant-PRL-3 fusion protein group, with statistically significant differences (P < 0.01). The expression of MDSC in spleen and peripheral blood of the MDSC specific antibody deletion group was lower than that of the control antibody deletion group, while the difference between the expression of MDSC in tumor tissues and that of the MDSC control antibody deletion group was not statistically significant (P > 0.05), and was greater than that of the adyuvanted-PRL-3 fusion protein group, with statistically significant difference (P < 0.01). Conclusion MDSC in tumor tissues in the adjuvant-PRL-3 fusion protein group is correlated with tumor growth in breast tumor by DNA vaccination against PRL-3, providing a theoretical basis for further optimization of anti-tumor immunotherapy against PRL-3 and research on the related effects of MDSC.

86 Dosimetry and side effects comparison of prone with Belly board and supine position in intensity modulated radiotherapy for cervical cancer

Objective To explore the optimal position in radiotherapy for cervical cancer, by comparing the dose-volume relationship of the mid-intestinal pouch, bladder, rectum, femoral head and the occurrence of acute radiation enteritis in the prone position + Belly board and the supine position. Methods A total of 40 patients with cervical cancer who underwent intensity modulation radiotherapy in Xiangtan Central Hospital, Hu′nan Province from June 2017 to March 2019 were randomly selected. CT scans were performed in Belly board and Belly position respectively. The target area and organs at risk were mapped under the same conditions and the treatment plan was optimized to compare the dose-volume relationship between the target area and organs at risk in two positions. Then the patients were divided into prone position group (20 cases) and supine position group (20 cases) for treatment by stratified sampling and random number table method according to age and stage, and the occurrence of acute radiation enteritis was observed in the two groups. Results Compared with the supine position group, the volume of intestinal pouch of organs at risk in the prone position group was significantly reduced (P < 0.05), the average volume of bladder and rectum and the average volume of target area showed no statistical significance (P > 0.05), and the comparison of target area fitness index and uniformity index showed no statistical significance (P > 0.05). Compared with the supine group, the volume of intestinal pouch receiving 25-45 Gy dose was significantly reduced in the prone position group (P < 0.01), while the dose-volume relationship of other organs at risk was not statistically significant (P > 0.05). The incidence of acute radiation enteritis in prone position group was significantly lower than that in supine position group, and the severity was reduced, with statistically significant difference (P < 0.05). Conclusion For intensity-modulated radiotherapy of cervical cancer, the Belly board in prone position can significantly reduce the dose of intestinal pouch and reduce the occurrence of intestinal side effects.

91 Effect of Modified Bingpeng Power Mouthwash on immune function in patients with head and neck cancer radiotherapy

Objective To observe the effect of Modified Bingpeng Power Mouthwash on immune function of patients with head and neck cancer radiotherapy. Methods A total of 98 eligible head and neck cancer radiotherapy patients admitted to the Department of Radiotherapy, Gansu Provincial Cancer Hospital from October 2017 to December 2018 were divided into the traditional Chinese medicine group and the Western medicine group according to the order of admission and the random number table method. Among them, 12 patients fell off in the test, and 43 patients in the traditional Chinese medicine group and the Western medicine group were included in the statistics. From the first day of radiotherapy to the end of radiotherapy, the traditional Chinese medicine group was given the gargle of Modified Bingpeng Power Mouthwash and the Western medicine group was given the conventional Western medicine gargle. Flow cytometry was used to detect CD3+, CD4+, CD8+ and CD4+/CD8+ subsets of T lymphocytes in the two groups before and after radiotherapy, and they were compared. Results After treatment, CD4+ and CD4+/CD8+ in both groups were lower than before treatment, and the Western medicine group was lower than the traditional Chinese medicine group, with statistically significant differences (P < 0.05). There were no significant differences in CD3+ and CD8+ of the two groups before and after treatment (P > 0.05). Conclusion Radiotherapy of head and neck tumors can damage the cellular immune function of patients, resulting in the decrease of CD4+ and CD4+/CD8+ values in T cell subsets. Modified Bingpeng Power Mouthwash can reduce the decrease of CD4+ and CD4+/CD8+ values in T cell subsets caused by radiotherapy.

95 Observation of preoperative oral rehydration salts combined with magnesium sulfate on C-reactive protein, GFV and prognosis of elderly breast cancer patients undergoing radical mastectomy

Objective To investigate the effect of preoperative oral rehydration salts combined with magnesium sulfate on C-reactive protein (CPR) and residual gastric fluid volume (GFV) and prognosis in elderly breast cancer patients undergoing radical mastectomy. Methods Eighty patients undergoing radical mastectomy in the First Affiliated Hospital of Hebei North University from November 2018 to April 2019 were selected and divided into the combined group and the magnesium sulfate solution group according to the random number table method, with 40 cases in each group. In the magnesium sulfate solution group, 50 mL of 25% magnesium sulfate solution was taken orally at 15∶00 one day before operation. The combined group was given 50 mL of oral 25% magnesium sulfate solution at 15∶00 on one day before operation and oral rehydration salt at 17∶00, dissolved in 300 mL warm water, and was taken within one hour. The levels of CRP at 8∶00 on one day before operation (T1) and fasting at 8:00 on the day of operation (T2) were calculated, as well as GFV at 14∶00 on one day before operation (T3) and 23∶00 on one day before operation (T4). The times of defecation, postoperative exhaust time and postoperative hospital stay of the two groups were counted from 8∶00 on the day of operation after magnesium sulfate solution was given. Visual analogue scales (VAS) was used to calculate the VAS scores of the two groups at the room resting state (T5), immediately after surgery (T6) and one day affer surgery (T7). The occurrence of postoperative adverse reactions was compared between the two groups. Results Compared with T1 time point, the levels of CRP were decreased in both groups at T2 time point, and the combined group was lower than that in the magnesium sulfate solution group at T2 time point, the differences were highly statistically significant (all P < 0.01). Compared with T3 time point, GFV in both groups was reduced at T4 time point, and the combined group was lower than magnesium sulfate solution group at T4 time point, the differences were highly statistically significant (all P < 0.01). The times of defecation in the combined group was higher than that in the magnesium sulfate solution group, and the postoperative exhaust time and postoperative hospital stay were shorter than those in the magnesium sulfate solution group, and postoperative adverse reactions were lower than that in the magnesium sulfate solution group, with statistically significant differences (all P < 0.05). VAS score in T7 of the combined group was lower than that of the magnesium sulfate solution group, and the difference was statistically significant (P < 0.05). Conclusion Preoperative oral rehydration salts combined with magnesium sulfate can reduce the preoperative the levels of CRP and GFV in elderly breast cancer patients and reduce the incidence of postoperative adverse reactions, which is worthy of clinical application.

99 Effect of laparoscopic radical prostatectomy in the treatment of prostate cancer and its effect on bladder function

Objective To observe the effect of laparoscopic radical prostatectomy in the treatment of prostate cancer and its effect on bladder function. Methods A total of 96 patients with prostate cancer who were treated in the Second People′s Hospital of Wuhu, Anhui Province from February 2016 to January 2019 were selected as the research objects, and were divided into laparoscopic group and open group according to the surgical method, with 48 patients in each group. The differences of operation general condition, bladder function, quality of life, complication rate and recurrence rate were compared between the two groups. Results The intraoperative blood loss in the laparoscopic group was significantly lower than that in the open group, and the difference was statistically significant (P < 0.05). There was no significant difference in operation time between the two groups (P > 0.05). Six months after the operation, the international scores of prostate symptoms in both groups were lower than three months after surgery, and the differences were statistically significant (all P < 0.05). Six months after the operation, the maximum urine flow rate of both groups was higher than that of the three months after surgery, and the laparoscopy group was higher than the open group, with statistically significant differences (all P < 0.05). There was no statistically significant difference in the quality of life between the two groups before the operation (P > 0.05). Six months after the operation, the scores of physical function, physical role, overall health, vitality, social function, emotional role and mental health in the open group were higher than those before the operation, with statistically significant differences (all P < 0.05). But the differences in the physical pain scores were not statistically significant compared with those before the operation (P > 0.05). All the life quality scores of the laparoscopic group were improved compared with those before the operation, and all the life quality scores of the laparoscopic group were higher than those of the open group, with statistically significant differences (all P < 0.05). There were no significant differences in postoperative complication rate and recurrence rate between the two groups (P > 0.05). Conclusion Laparoscopic radical prostatectomy has a low operative blood loss, little influence on the urine function of patients, low incidence of complications, and has a good application value.

104 Clinical effect of protein nutrition support combined with antiviral therapy in HBV-related decompensated liver cirrhosis

Objective To observe the clinical efficacy of protein nutrition support combined with antiviral therapy in Hepatitis B virus (HBV) related decompensated liver cirrhosis. Methods From March 2017 to March 2019, 112 cases with HBV decompensated liver cirrhosis of hepatitis B combined with malnutrition who admitted to the First Affiliated Hospital of Xinjiang Medical University were selected as study objects. They were divided into the observation group (56 cases) and the control group (56 cases) according to random number table method. The control-group was treated with routine antiviral therapy, and the observation group was treated with antiviral combined with protein nutrition diet. The levels of hepatitis B virus dexoyribonucleic acid (HBV-DNA), alanine aminotransferase (ALT), aspartate aminotransferase (AST), albumin (ALB), total bilirubin (TBil), liver function Child-Turcotte-Pugh (CTP) score, hyaluronic acid (HA), laminin (LN), type Ⅲ procollagen (PCⅢ), type Ⅵ collagen (Ⅵ-C) were compared, and the improvement of ascites and HBV-DNA negative rate between the two groups were compared. Results Before treatment, there were no statistically significant differences in HBV-DNA, TBil, ALT, AST, HA, LN, PCⅢ, Ⅵ-C, ALB level and CTP score between two groups (P > 0.05). After treatment, HBV-DNA, TBil, ALT, AST, HA, LN, PCⅢ, Ⅵ-C levels and CTP scores of the observation group were lower than those before treatment, and the observation group was lower than the control group (all P < 0.05). After treatment, ALB levels in two groups were higher than those before treatment, and the observation group was higher than the control group (all P < 0.05). The HBV-DNA negative conversion rate and ascites improvement rate in the observation group were higher than those in the control group (all P < 0.05). There were no statistically significant differences in the incidence of complications between the two groups (P > 0.05). Conclusion Patients in the decompensated liver cirrhosis of hepatitis B are treated with protein nutrition diet and antiviral therapy can effectively improve the liver function, inhibit the progress of liver fibrosis, promote the negative of HBV-DNA, improve the nutritional status and reduce ascites in patients with liver cirrhosis.

Objective Meta analysis was used to identify the risk factors of falls in patients with stroke during rehabilitation period. Methods PubMed, EMbase, web of science, Weipu database,Wanfang and other databases were searched by computer. The retrieval time was from the establishment of each database to October 20, 2019. Meta analysis of the extracted data was carried out using RevMan 5.3 software. Results A total of 13 articles and 3704 patients were included, and the fall group was 1311 cases, the non-fall group was 2393 cases. Hearing impairment (OR = 2.87, 95%CI:2.10-3.92, P < 0.000 01), cognitive impairment (OR = 3.92, 95%CI:1.55-9.93, P = 0.004), balance impairment (OR = 1.60, 95%CI:1.01-2.52, P = 0.04), hypertension (OR = 1.79, 95%CI:1.02-3.14, P = 0.04), history of stroke (OR = 1.70, 95%CI:1.31-2.20,P < 0.0001), osteoarthritis (OR = 1.73, 95%CI:1.25-2.39, P = 0.0008), sedative hypnotic use (OR = 2.20, 95%CI:1.73-2.80, P < 0.000 01), antidepressant use (OR = 1.98, 95%CI:1.29-3.06, P = 0.002) were all risk factors for falls in stroke patients during rehabilitation period. Conclusion The incidence of falls is high in stroke patients during rehabilitation period, so risk factors should be identified and preventive measures should be formulated to reduce the incidence of falls.

113 Relationship between dynamic blood glucose and diurnal dynamic blood pressure fluctuation in patients with type 2 diabetes mellitus with hypertension

Objective To investigate the relationship between dynamic blood glucose and diurnal dynamic blood pressure fluctuation in type 2 diabetes mellitus with hypertension. Methods From June 2015 to June 2019, 96 cases with type 2 diabetes mellitus with hypertension who admitted to Taihu Sanatorium of Jiangsu Province were selected as the study subjects. According to the blood glucose fluctuation, they were divided into hyperglycemic drift group (42 cases) and hypoglycemic drift group (54 cases). General Information, standard deviation of blood glucose (SDBG), rate of diastolic blood pressure fall (DBPF) and rate of systolic blood pressure fall (SBPF) of the two groups were compared, and the relevant factors of SBPF were analyzed by multiple linear regression, and the correlation between SDBG, DBPF and SBPF were analyzed by Spearman method. Results The duration of diabetes and SDBG in hyperglycemic drift group were higher than those in hypoglycemic drift group, while SBPF and DBPF were lower than those in hypoglycemic drift group (all P < 0.05). The duration of diabetes and SDBG were the influencing factors of SBPF (P < 0.05). In hypoglycemic drift group, SDBG was negatively correlated with DBPF and SBPF(r = -0.689,-0.579, P < 0.05). Conclusion There is a negative correlation between blood glucose fluctuation and diurnal dynamic blood pressure fluctuation in patients with type 2 diabetes mellitus with hypertension. Reducing blood glucose fluctuation in patients is conducive to maintaining blood pressure rhythm, which has certain clinical reference value in the treatment of patients with type 2 diabetes mellitus with hypertension.

117 Changes of serum sTfR level in children with HbH disease and its clinical significance

Objective To study the expression of soluble transferrin receptor (sTfR) in the serum of children with hemoglobin H (HbH) disease and its clinical significance. Methods From January 2017 to September 2018, 79 patients with HbH disease who diagnosed and treated in Women and Children′s Hospital Affiliated to Medical College of Electronic Science and Technology University (“our hospital” for short) were selected as case group, and they were divided into non-deletion group (26 cases) and deletion group (53 cases) according to the genotype test results. A total of 40 healthy children in our hospital during the same period were selected as control group. The sTfR, hemoglobin (Hb), mean corpuscular hemoglobin (MCH), red blood cell (RBC), mean corpuscular volume (MCV) and serum ferritin level berween three groups were statistically analyzed and the correlation between serum sTfR level and hematological indexes in children with HbH disease were analyzed. Results In case group, deletion type (-α3.7/--SEA, -α4.2/--SEA) accounted for 67.1%, non-deletion type (αCSα/--SEA, αQSα/--SEA, αΔ30α/--SEA) accounted for 32.9%. The serum levels of sTfR and SF in deletion group and non-deletion group were higher than those in control group, while the levels of Hb, RBC, MCH and MCV were lower than those in control group, with statistically significant differences (all P < 0.05). The serum levels of sTfR, MCH, MCV and SF in non-deletion group were higher than those in deletion group, while the levels of Hb and RBC were lower than those in deletion group, with statistically significant differences (all P < 0.05). In case group, the serum sTfR level was significantly positively correlated with serum MCH, MCV and SF (r = 0.621, 0.734, 0.521, P < 0.05), but negatively correlated with RBC and Hb (r = -0.721, -0.675, P < 0.05). Conclusion Serum level of sTfR is related to the severity of anemia and iron metabolism in children with HbH disease, which may be a serological indicator of the disease.

120 Phenotypic distribution of bronchial asthma airway inflammation, airway inflammation markers detection and correlation analysis of asthma control

Objective To investigate the phenotypic distribution of bronchial asthma airway inflammation, airway inflammation markers detection and correlation analysis of asthma control, so as to provide clinical treatment guidance for patients. Methods The diagnosis and treatment of 110 asthma patients who diagnosed in Cangzhou Central Hospital of Hebei Province from May 2016 to May 2019 were retrospectively analyzed. According to the asthma control level, they were divided into complete control group (20 cases), partial control group (41 cases) and uncontrolled group (49 cases). The phenotypic distribution of airway inflammation in three groups of patients with bronchial asthma were analyzed, markers of airway inflammation were detected and the correlation between asthma control level and airway inflammation markers (exhaled nitric oxide [FeNO], serum total immunoglobulin [IgE], sputum eosinophils, blood eosinophils) were analyzed by Pearson correlation analysis. Results There were no significant differences in phenotype distribution of airway inflammation among three groups (P > 0.05). There were no significant differences in markers of airway inflammation among three groups (P > 0.05). There were no correlation between bronchial asthma control level and sputum eosinophils, FeNO, blood eosinophils and serum total IgE (P > 0.05). Conclusion There is no significant correlation between asthma control level and airway inflammation markers, whether airway inflammation markers should be used to determine the control level of bronchial asthma still needs to be determined based on individual patient conditions and subsequent studies.

124 Prospective clinical observation of Mediation of Zhongzhou Prescription in the treatment of obese polycystic ovary syndrome with insulin resistance

Objective To explore the clinical effect and security of the empirical formula of Professor Wang Guohua, Mediation of Zhongzhou Prescription, in the treatment of obese polycystic ovary syndrome (PCOS) with insulin resistance. Methods A total of 59 PCOS patients with insulin resistance and obesity with syndrome of phlegm-dampness due to spleen deficiency in the Gynecology Department of the Third Affiliated Hospital of Beijing University of Chinese Medicine from September 2016 to March 2018 were selected as the research subjects. Patients were divided into control group (29 cases) and experimental group (30 cases) according to the random number table method. Metformin Tablets were taken orally in the control group, Mediation of Zhongzhou Prescription were taken orally in the experimental group. Basal body temperature (BBT) was recorded daily during treatment. Body mass index (BMI), waist-to-hip ratio (WHR), homeostasis model assessment-insulin resistance (HOMA-IR) and testosterone (T), luteinizing hormone/follicle-stimulating hormone (LH/FSH) were measured before and after treatment. The clinical efficacy of the two groups was compared before and after treatment, and the occurrence of adverse reactions was observed. Results After treatment, the total effective rate of clinical syndromes in the experimental group was higher than that in the control group (P < 0.05). Before treatment, there were no significant differences in WHR, BMI, HOMA-IR indexes and sex hormone (T, LH and FSH) levels between the two groups (P > 0.05). After treatment, in the experimental group, the levels of WHR, BMI, HOMA-IR and LH/FSH were significantly lower than before treatment (P < 0.05), in the control group, the levels of BMI and HOMA-IR were significantly lower than before treatment (P < 0.05), and LH/FSH in the experimental group was lower than that in the control group (P < 0.05). There were four cases of gastrointestinal reactions in control group, while no obvious adverse reactions occurred in treatment group. Conclusion Professor Wang Guohua′s empirical formula in the treatment of obese PCOS with insulin resistance has significant effect and certain clinical application value, which is worthy of promotion, application and further research.

129 Meta-analysis on the efficacy of acupoint application of traditional Chinese medicine in the treatment of primary dysmenorrhea

Objective To evaluate the curative effect of acupoint application of traditional Chinese medicine (TCM) on primary dysmenorrhea. Methods After two researchers screened the literature independently, extracted the data and evaluated the risk of bias, the meta-analysis was performed by Stata 14.0 software. Chinese databases were searched including CNKI, Wanfang Data, CBM, VIP, and English databases included PubMed, Embase, the Cochrane Library and Web of Science. The retrieval period was from database construction to July 2019. Results There were 16 studies included, with 1366 patients. Meta-analysis showed that acupoint application of TCM was superior to nonsteroidal anti-inflammatory drugs in terms of clinical total response rate (RR = 1.34, 95%CI [1.18, 1.53], P < 0.0001), reduction of dysmenorrhea symptom score (WMD = 3.03, 95%CI [2.56, 3.49], P < 0.0001), reduction of visual analogue score (VAS) (WMD = 1.11, 95%CI [0.79, 1.43], P < 0.0001) in the treatment of primary dysmenorrhea. In addition, acupoint application of TCM was better than placebo in reducing VAS score (WMD = 2.26, 95%CI [1.90, 2.62], P < 0.0001), reducing duration of dysmenorrhea (WMD = 10.93, 95%CI [7.99, 13.87], P < 0.0001) and severity of onset level (WMD = 6.52, 95%CI [5.05, 7.99], P < 0.0001). Compared with traditional Chinese patent analgesics, there was a certain advantage in reducing dysmenorrhea symptom score (WMD = 3.56, 95%CI [2.33, 4.78], P < 0.0001), but there was no significant difference in clinical efficacy (RR = 1.17, 95%CI [0.99, 1.39], P = 0.06). Conclusion Acupoint application of TCM has certain curative effect on primary dysmenorrhea, but the results need to be further verified by more high-quality RCTs.

134 Effect of kidney-tonifying method on lipid metabolism in patients with carotid atherosclerosis

Objective To evaluate the effect of kidney-tonifying method on lipid metabolism in elderly patients with carotid atherosclerosis (CAS). Methods One hundred and twenty-two CAS patients with syndrome of deficiency of kidney essence from January 2015 to July 2016 in Longhua Hospital of Shanghai University of Traditional Chinese Medicine were randomly divided into control group and treatment group by random number table method, with 61 cases in each group. The control group was treated with Simvastatin Tablets, the treatment group was treated with Yishen Oral Liquid. The treatment duration of both groups was 24 weeks. The traditional Chinese medicine (TCM) syndrome scores of both groups were evaluated and compared before and after treatment, and the levels of the blood lipid indexes (triacylglycerol [TG], total cholesterol [TC], low density lipoprotein cholesterol [LDL-C] and high density lipoprotein cholesterol [HDL-C]) were detected. At the same time, the adverse reactions were observed. Results In the control group, three cases fell off with 58 cases completed and in the treatment group, four cases fell off with 57 cases completed, 115 cases were involved in statistical analysis. There was no significant difference of the total TCM syndrome scores in the control group before and after treatment (P > 0.05). After treatment, the total TCM syndrome score in the treatment group was lower than that before treatment (P < 0.05), and lower than that in the control group (P < 0.05); after treatment, the efficacy of TCM syndrome in the treatment group was better than that in the control group (P < 0.05). After treatment, the HDL-C level in the control group was lower than before treatment (P < 0.05), the HDL-C level in the treatment group was higher than before treatment (P < 0.05), and higher than that in the control group (P < 0.05). After treatment, the TC levels in both groups were lower than that before treatment (P < 0.05), and the TC level in treatment group was higher than that in control group (P < 0.05). Before and after treatment, there were no significant differences in adverse reactions between the two groups (P > 0.05). Conclusion Yishen Oral Liquid can regulate blood lipid metabolism and reduce cholesterol in patients with CAS, and it has significant advantages compared with the control group in terms of increasing HDL-C level. It can effectively improve TCM syndromes with no obvious adverse reactions.

139 Meta-analysis of blood activating therapy combined with acupuncture in the treatment of vascular cognitive impairment

Objective To systematically evaluate the effect of blood activating therapy combined with acupuncture in the treatment of vassular cognitive impairment (VCI). Methods Based on the key words of “promoting blood circulation”, “removing blood stasis”, “dredging collaterals”, “acupuncture”, “vascular cognitive impairment”, “vascular dementia” and “post stroke dementia”, this study searched CBM, CNKI, Wanfang Data, VIP, PubMed and Web of Science from the establishment of the database to November 2019. Randomized controlled trials (RCTs) published in these database were studied. Meta-analysis was performed by RevMan 5.3 software. Results Thirteen trials involving 1150 patients were included. The meta-analysis results showed that the method of promoting blood circulation combined with acupuncture could improve the curative effects of VCI patients (OR = 3.65, 95%CI [2.49, 5.35], P < 0.000 01), mini mental status examination scores (MD = 3.49, 95%CI [2.18, 4.80], P < 0.000 01), Montreal cognitive assessment scores (MD = 1.73, 95%CI [0.78, 2.69], P = 0.0004) and reduce the scores of National Institute of Health stroke scale (MD = -3.36, 95%CI [-4.25, -2.47], P < 0.000 01) as well as whole blood viscosity (MD = -0.78, 95%CI [-1.04, -0.53], P < 0.000 01). Conclusion On basis of the conventional Western medicine treatment, combination of promoting blood circulation and acupuncture for VCI is significantly better than using Western medicine alone in improving cognitive dysfunction, restoring defective nerve function and reducing whole blood viscosity.

145 The clinical study of wěizhōng point bloodletting combined with kneeling to take fútù point in the treatment of third lumbar transverse process syndrome

Objective To study the effect of wěizhōng point bloodletting combined with kneeling to take fútù point in the treatment of third lumbar transverse process syndrome. Methods From January to November 2019, 66 patients with lumbar transverse process syndrome were selected as the research objects. After informed consent and ethical review, the patients were randomly divided into treatment group (33 cases) and control group (33 cases) according to the random table method. The treatment group was treated with wěizhōng point bloodletting combined with kneeling to take fútù point. The control group was treated with Diclofenac Sodium Enteric-coated Tablets orally. After treatment, Japanese Orthopedic Association (JOA) score, Oswestry dysfunction index (ODI) scale and the clinical efficacy of traditional Chinese medicine (TCM) was compared within and between the two groups, and the conclusions were drawn. Results After treatment, JOA scores of the two groups were higher than those before treatment (P < 0.05), and the treatment group was higher than that of the control group (P < 0.05); after treatment, ODI score of the two groups were lower than those before treatment, and the treatment group was lower than that of the control group (P < 0.05). After treatment, the total effective rate of the efficacy of TCM syndrome in the treatment group was higher than that in the control group (P < 0.05). Conclusion Wěizhōng point bloodletting combined with kneeling to take fútù point in the treatment of third lumbar transverse process syndrome has obvious clinical effect, the effect is fast, the safety is high, and it is better than taking Diclofenac Sodium Enteric-coated Tablets orally.

149 Effect of Tacrolimus Ointment combined with Compound Muniziqi Granules in the treatment of facial hormone dependent dermatitis

Objective To investigate the efficacy of Tacrolimus Ointment combined with Compound Muniziqi Granules in the treatment of facial hormone dependent dermatitis (FHDD). Methods Eighty patients with FHDD admitted to the First Affiliated Hospital of Xinjiang Medical University from March 2018 to December 2019 were selected and divided into control group (n = 40, Tacrolimus Ointment) and study group (n = 40, Tacrolimus Ointment combined with Compound Maniziqi Granules) according to the random number table method. Clinical efficacy, symptom score, skin lesions, immunoglobulin E (IgE), eosinophil count, adverse reactions and recurrence rate were compared between two groups. Results Total clinical effective rate of study group was higher than that of control group (P < 0.05). After treatment, the scores of pruritus, telangiectasia, skin atrophy, swelling, flushing and acne like lesions in two groups were lower than those before treatment (P < 0.05), and the scores in study group were lower than those in control group (P < 0.05). After treatment, IgE and eosinophil count in both groups were lower than those before treatment, and the lesion area was smaller than that before treatment (P < 0.05), and IgE and eosinophil count in study group were lower than those in control group, and the lesion area was smaller than that in control group (P < 0.05). There was no significant difference in the incidence of adverse reactions between two groups (P > 0.05). The recurrence rate of study group was lower than that of control group (P < 0.05). Conclusion Tacrolimus Ointment combined with Compound Muniziqi Granules in the treatment of FHDD can effectively improve clinical symptoms of patients, improve the immune function, with good safety and definite effect.

153 Application of establishing of a hospital-level nutritional catheterization subgroup in improving the blind insertion of the naso-intestinal tube for critically ill patients

Objective To explore the application of establishing a hospital-level nutritional support catheterization subgroup in improving bedside blind insertion of naso-intestinal tubes for critically ill patients. Methods In the Affiliated Huai′an First People′s Hospital of Nanjing Medical University, a total of 256 patients and 31 hospital nutrition support team liaison nurses who performed bedside blind intubation registration from January 2017 to December 2018 were selected as the research subjects. With the establishment of the nutritional support catheterization subgroup in January 2018 as the demarcation point, a total of 86 patients registered for blind insertion of the naso-intestinal tube from January to December 2017 were used as the control group; from January to December 2018, a total of 170 patients registered for bedside blind insertion of the naso-intestinal tube was implemented  were the observation group. The catheterization subgroup conducted systematic training and effect evaluation on the theory and practical skills related to blind insertion of naso-intestinal tubes for 31 contact nurses. The success rate and satisfaction degree of blind insertion of nasal intestine tube between the two groups were compared. Results One year after the establishment of the standardized training of the catheterization sub group, the contact nurses who mastered blind insertion of the nasao-intestinal tubes had higher theoretical scores than before the establishment, and the difference was highly statistically significant (P < 0.01). The practice skill scores of blind insertion of the nasao-intestinal tubes in the catheterization subgroup nine months after the standardized training was higher than those three months after the training, and the difference was highly statistically significant (P < 0.01). The success rate of blind insertion of the naso-intestinal tube and patient satisfaction in the observation group were higher than those in the control group, and the differences were highly statistically significant (P < 0.01). The overall satisfaction of doctors, head nurses and ward nurses with liaison nurses was 96.7%, 100.0% and 100.0% respectively. Conclusion Establishing a subgroup of nutrition catheterization technology can significantly improve the clinical nurses′ bedside blind insertion of the naso-intestinal tube skills and catheterization success rate, and promote the application of enteral nutrition technology in multi-disciplinary critically ill patients.

157 Application of internet plus blood glucose management mode in the education and management of diabetic foot patients

Objective To explore the effect of Internet plus blood glucose management mode in the education and management of Wanger 0-grade diabetic foot patients. Methods A total of 150 diabetic foot patients of Wagner 0-grade who were hospitalized in the Department of Vascular Surgery, Beijing Luhe Hospital, Capital Medical University from January to December 2017, were divided into two groups using random number table method, including 100 cases in the intervention group and 95 cases in the control group. The control group was used conventional blood glucose management mode, and the intervention group was used the Internet plus blood glucose management mode. The Hemoglobin A1c (HbA1c), body mass index (BMI), diabetes self-management behavior ability score, average hospital stay and hospital satisfaction of patients before intervention and 12 months after intervention were compared between two groups. Results After the intervention for 12 months, the HbA1c and BMI of the two groups were lower than those before the intervention, while the intervention group was lower than the control group, and the differences were highly statistically significant (P < 0.01); the scores of each dimension of self-management behavior were better than those before the intervention, while the intervention group was better than the control group, and the differences were highly statistically significant (P < 0.01); the average hospital stay of the intervention group was shorter than that of the control group, while the discharge satisfaction level was higher than that of the control group, and the differences were statistically significant (P < 0.05). Conclusion The Internet plus blood glucose management model extends the management of diabetic foot patients, and improves the patient′s self-behavior management ability and satisfaction with medical treatment.

161 Status quo of knowledge, attitude and behavior of insulin injection among nurses of non-endocrinology department and analysis of structural equation model

Objective To analyze the current status of insulin injection knowledge, attitude and behavior among non-endocrinology nurses, and explore the mechanism of action of knowledge, attitude, and behavior. Methods A total of 1233 non-endocrine nurses in a class three grades hospital in Nanjing Province from June to July 2019 were selected by convenience sampling method. The general information questionnaire and insulin injection technology knowledge, attitude and behavior questionnaire were used, and SPSS 23.0 and Amos 23.0 software were applied to analyze the status quo and construct the structural equation model. Results In this study, a total of 1235 questionnaires were collected, 1233 valid questionnaires, and the effective response rate was 99.8%. The scores of non-endocrinology nurses′ insulin injection knowledge and attitude, knowledge and behavior, attitude and behavior were positively correlated (r = 0.13, 0.10, 0.39, P = 0.000). The differences in comparison of insulin injection knowledge scores among non-endocrinology nurses with different genders, working years, education background, job title, marital status, whether they were members of the diabetes team, and the number of times of training in the past one year were statistically significant (P < 0.05). There were statistically significant differences in the score of insulin injection knowledge among non-endocrine nurses of whether it was a member of the diabetes group and received relevant training for different times in the past one year (P < 0.05). There was a statistically significant difference in the scores of insulin injection behavior at different times of training in the past one year (P < 0.05). The structural equation model fits well, knowledge had a direct effect on attitude, and the effect coefficient was 0.14; attitude had a direct effect on behavior, and the effect coefficient was 0.56; the total effect coefficient of knowledge on behavior was 0.15, of which the direct effect coefficient was 0.07, and the indirect influence coefficient through attitude on behavior was 0.08. Conclusion Non-endocrinology nurses need to improve their knowledge of insulin injection, and their attitudes and behaviors are good. The guidance of a positive attitude is the key to improving nurses′ knowledge, attitude, and behavior of insulin injection.

166 Application effect of out-of-hospital home nursing intervention preschool children with asthma in outpatient clinic

Objective To analyze the application effect of out-of-hospital home nursing intervention in outpatient preschool children with asthma. Methods A total of 360 preschool children with moderate persistent asthma who attended the Pediatric Clinic of the First Affiliated Hospital of Guangxi Medical University from November 2015 to May 2019 were divided into control group and study group according to random number table method, with 180 cases in each group. The control group was received out-of-hospital routine nursing guidance, while the research group was implemented home nursing interventions outside the hospital. The percentage of peak respiratory flow rate of the predicted value (PEF%), the percentage of forced expiratory volume in the first second (FEV1%), the children′s asthma quality of life questionnaire score (PAQLQ), and compliance sexuality, frequency of acute attacks, number of re-visits, asthma day and night symptom score were compared between two groups after six months. Results After six months of home care intervention, the PEF% and FEV1% of the two groups were higher than those when they were enrolled, while the study group were higher than those of the control group, and the differences were highly statistically significant (P < 0.01); the scores of symptom dimension activity dimension and affective function were higher than those at the time of enrollment, while the study group were higher than those of the control group, and the differences were statistically significant (P < 0.05). The differences in compliance between the two groups at different times were statistically significant (P < 0.05). The compliance of the study group for three and six months of intervention were higher than that of the control group, and the differences were statistically significant (P < 0.05). After six months of intervention in the study group, the frequency of acute asthma attacks and the frequency of revisiting doctors were less than those of the control group, and the differences were statistically significant (P < 0.05); the asthma daytime symptom score and nighttime symptom score of the study group were lower than those of the control group, and the differences were statistical significance (P < 0.05). Conclusion Out-of-hospital home care intervention can improve the compliance of children with doctors, reduce the frequency of acute attacks and the frequency of re-visiting the children, improve their quality of life, and provide reference for the prevention and treatment of chronic diseases in children.

170 Effect of multilevel comprehensive nursing intervention model on elderly patients with frailty after percutaneous coronary intervention

Objective To investigate the effect of multilevel comprehensive nursing intervention model on elderly patients with frailty after percutaneous coronary intervention (PCI). Methods A total of 100 elderly patients with frailty after PCI admitted to Haikou People′s Hospital from February to November 2018 were selected as the study subjects,and they were divided into experiment group (50 patients) and control group (49 patients) by random number table method. The experimental group implemented multilevel comprehensive nursing intervention model, and the control group implemented routine nursing and health education after PCI. Frailty, walking speed, standing up time, depression and anxiety of patients were evaluated before and three months after intervention. Results After intervention, the total scores of frailty and all dimensions of the experimental group were lower than those of before intervention and the control group, and the differences were statistically significant (all P < 0.05). The walking speed of the experimental group was faster than that of before intervention and the control group, and the differences were statistically significant (P < 0.05). The standing and walking time of the experimental group was shorter than that of before intervention and the control group, and the differences were statistically significant (all P < 0.05). The scores of anxiety and depression in the experimental group were lower than those before intervention and the control group, and the differences were statistically significant (all P < 0.05). Conclusion The multilevel comprehensive nursing intervention model can improve the frailties of senile patients after PCI, improve the ability of walking and standing up, and reduce the level of anxiety and depression.

174 Application of TTT training mode in training of traditional Chinese medicine nursing knowledge in county-level traditional Chinese medicine hospital

Objective To explore the application of train-the-trainer (TTT) training mode in the training of traditional Chinese medicine nursing knowledge in county-level traditional Chinese medicine hospitals. Methods This study is a self-controlled experimental study. From December 2017 to December 2019, 15 head nurses from Xintai Hospital of Traditional Chinese Medicine were selected as trainers and 149 nurses from the trainer′s department were selected as target nurses for training by convenient sampling method. The trainer will be trained by the tutor first, and then the trainer will train the target nurse twice. After the training, the trainers shall be evaluated for their training ability and the contents of the training courses. The theoretical knowledge of traditional Chinese medicine nursing and traditional Chinese medicine nursing skills of the target nurses were evaluated before and after the training. Results The second training ability score after training was higher than that before training, and the difference was highly statistically significant (P < 0.01). The trainer′s evaluation of the training course indicated that the training of traditional Chinese medicine nursing knowledge had strong practicability, comprehensive training content and high clinical suitability. Target nurses: the scores of traditional Chinese medicine nursing theory knowledge assessment after training were higher than those before training, with statistically significant difference (P < 0.05). The assessment result of nursing operation of traditional Chinese medicine after training was higher than that before training, and the difference was highly statistically significant (P < 0.01). Conclusion TTT training mode has good effect in traditional Chinese medicine nursing knowledge training and can be widely used in clinical nursing training, which is beneficial to cultivate and reserve all kinds of nursing talents.

178 Influence of traditional Chinese medicine characteristic extended nursing on daily activity level and physical function of stroke patients

Objective To investigate the influence of traditional Chinese medicine characteristic extended nursing on daily activity level and physical function of stroke patients. Methods A total of 82 stroke patients admitted to the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from August 2018 to October 2019 were selected as the study subjects, they were divided into control group and intervention group according to random number table method, with 41 cases in each group. The control group was given routine nursing, while the intervention group was given characteristic extended nursing of traditional Chinese medicine, including dialectical nursing, health education, rehabilitation training, acupoint massage, follow-up after discharge, etc., and the course of treatment in both groups was three months. The scores of traditional Chinese medicine symptoms, daily life activity (Barthel) and stroke lesion assessment (SIAS) were compared between the two groups before and after treatment, the total clinical response rate was calculated after treatment. Results After treatment the scores of traditional Chinese medicine symptoms in the two groups were lower than before treatment (P < 0.05), and the score of traditional Chinese medicine symptoms in the intervention group was lower than that in the control group (P < 0.05). After treatment, Barthel score and SIAS score in the two groups were higher than those before treatment (P < 0.05), and Barthel score and SIAS score in the intervention group were higher than those in the control group (P < 0.05), the total clinical effective rate of the intervention group was higher than that of the control group (P < 0.05). Conclusion Traditional Chinese medicine characteristic extension nursing can significantly improve the level of daily activities of patients, restore physical function, improve clinical symptoms, clinical treatment of stroke effect is satisfactory, worthy of promotion.

182 Effects of cognitive intervention and psychological intervention on puncture the central vein through the peripheral vein catheterization related complications and mood in patients with breast cancer

Objective To explore the effects of cognitive intervention and psychological intervention on the complications and emotions associated with puncture the central vein through the peripheral vein (PICC) in patients with breast cancer after peripheral venipentesis. Methods From October 2018 to December 2019, 52 patients with ultrason-guided PICC catheter after breast cancer surgery in the Second Hospital of Yulin City were selected and divided into control group (routine nursing intervention) and observation group (enhanced cognitive and psychological intervention based on routine nursing intervention), with 26 patients in each group. Anxiety, depression, incidence of PICC catheter-related complications and nursing satisfaction were compared between the two groups. Results After intervention, the scores of anxiety self-rating scale and depression self-rating scale in the observation group decreased compared with that before intervention, and the scores of the observation group were significantly lower than those of the control group (P < 0.05). The incidence of PICC catheter-related complications in the observation group was lower than that in the control group (P < 0.05). Nursing intervention satisfaction in the observation group was significantly higher than that in the control group (P < 0.05). Conclusion In the nursing intervention for breast cancer patients after operation, cognitive intervention combined with psychological intervention plays a very important role in the improvement of anxiety and depression in patients, which can reduce the incidence of PICC catheter-related complications catheterization and improve the nursing satisfaction of patients.

Knee bursitis belongs to the category of “Jinbi” and “Xibi” in traditional Chinese medicine. It is a common and frequent occurring disease of knee joint. Its condition is complex, the course is long, the deficiency and excess is mixed, and the treatment is difficult. Professor Sun Chengxiang believes that knee synovitis is mostly caused by deficiency of spleen and kidney, which leads to the appearance of qi stagnation and blood stasis caused by exogenous pathogens, and the obstruction of channels and collaterals, which is fundamentally caused by exogenous pathogens. The clinic should pay attention to the pathogenesis, pay attention to syndrome differentiation, tonic deficiency and remove pathogens, syndrome differentiation and medication, flexible compatibility, use the method of promoting blood circulation and removing blood stasis, as appropriate to strengthen muscles and bones, expel wind, relieve pain, eliminate dampness and diuresis. This paper gives a preliminary summary and explanation of Professor Sun Chengxiang′s academic viewpoints, and provides a preliminary work basis for the sequential academic research.

190 Research on the dilemma and countermeasures of traditional Chinese medicine culture communication from the perspective of media convergence

With the rapid iteration of modern media technology, traditional media and new media show a trend of integration, which has a profound impact on the communication structure and influence space of traditional Chinese medicine culture. Under the background of media convergence, the reasons that restrict the effectiveness of traditional Chinese medicine cultural communication are mainly reflected in three aspects, system and mechanism, from “addition” to “integration”, and talent team building. How to resolve dilemmas and problems faced by traditional Chinese medicine culture communication from the perspective of media convergence, it is necessary to provide a new “prescription” for comprehensive treatment from four aspects, namely, management convergence, channel platform convergence, media content convergence and talent team convergence, so as to enhance the influence and credibility of traditional Chinese medicine culture omnimedia communication.

193 Analysis of medical expenses in public hospitals in Shandong Province after the elimination of drug price addiction

Objective To analyze the changes in the medical expenses of public hospitals in Shandong Province after the elimination of drug price addiction, and to provide reference suggestions for optimizing the structure of medical expenses and ensuring the public welfare and sustainable development of public hospitals. Methods Statistical analysis was made on the medical expenses of public hospitals from 2010 to 2018, and the number of public hospitals included in the tertiary, secondary, primary and unrated public hospitals in Shandong Province was 155, 335, 210, and 109, respectively. Results From 2010 to 2018, the average outpatient expenses increased from 167.3 yuan to 257.9 yuan, with an average annual growth rate of 6.8%. The per capita hospitalization expenses increased from 6415.9 yuan to 9845.7 yuan, with an average annual growth rate of 6.7%. The proportion of drugs decreased, but the proportion of outpatient drugs was still higher than 30%. All levels of hospitals accounted for the highest proportion of drug costs, and the primary hospitals accounted for the highest proportion of drug costs. Conclusion It is of positive significance to cancel drug price addiction, but the government investment must be timely and sink, otherwise the excessive compensation mechanism will increase the burden on patients. At the same time, strengthen the standardized operation of the pharmaceutical market, reduce the cost of medicines and health materials, and deepen the policy of tiered diagnosis and treatment.
