4 Effect of the exosomes extracted from adipose-derived mesenchymal stem cells on hypoxia/reoxygenation-induced cardiomyocyte apoptosis
LIANG Zheng ZHONG Jianfeng WU Yancong WEN Wen CHEN Can LI Bo
Objective To investigate the effect of mouse adipose-derived mesenchymal stem cells (mASCs) derived exosomes (Exo) on hypoxia/reoxygenation (H/R) induced apopotosis of cardiomyocytes neonatal mouse cardiomyocytes. Methods The mascs-derived Exo (mascs-exo) was isolated and extracted by ultra-high speed centrifugation. The Exo was observed and identified by transmission electron microscopy (TEM), nanoparticle tracer analysis (NTA) and Western blot (WB). C57BL/6 suckled rat cardiomyocytes were extracted by differential adherent method. Expression of CTnI and conexin43 was detected by immunofluorescence. Pkh-26 was labeled Exo to indicate the uptake capacity of cardiomyocytes. The experiment was divided into three groups: normal control group (control group), hypoxia/reoxygenation group (H/R group) and hypoxia/reoxygenation + mascs-exo group (H/R+ mascs-exo group). Apoptosis of was detected by TUNEL staining. The expression level of apoptosis-related protein was detected by WB. Results compared with the control group, the apoptosis rate of cardiomyocytes in the H/R group was increased (P < 0.01), and the apoptosis rate of cardiomyocytes in the H/R+ mascs-exo group was decreased (P < 0.05). Compared with the control group, p-PI3K /PI3K, p-Akt /Akt, and Bcl-2 /Bax were decreased in the H/R group (P < 0.01), while the cleaved caspase-9/caspase-9 ratio was increased (P < 0.01). Compared with H/R group, the ratio of p-PI3K/PI3K, p-Akt /Akt and Bcl-2 /Bax in H/R+ mascs-exo group was increased (P < 0.05 or P < 0.01), and the ratio of cleaved caspase-9/caspase-9 was decreased (P < 0.01). Conclusion mASCs-Exo can inhibit H/R induction of cardiomyocytes apoptosis, which may be mediated by activation of the PI3K/Akt signaling pathway.

Objective To study on the improvement of insulin resistance (IR) in rats with type 2 diabetes mellitus (T2DM) was conducted based on AMPK/GLUT4/GSK3β/PPARα signaling pathway. Methods Sixty Wistar rats were divided into control group and high-fat experimental group (n = 50), and 40 rats which were successfully modeled and met the requirements of the model in the high-fat experimental group were divided into, model group, positive control group [Rosiglitazone 2 mg/(kg·d)], Cortex Lycii Radicis low dose group [5 mg/(kg·d)] and high dose group [10 mg/(kg·d)] by the random number table method, with 10 rats in each group. Each groups were given continuous administration for 6 weeks. Fasting blood glucose (FBG), total cholesterol (TC), triglyceride (TG), low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), aminotransferase (AST) and alanine aminotransferase (ALT) levels of rats in each group were compared and liver morphology was observed. Protein mass concentration in liver was determined by BCA method. Results FBG levels in model group were higher than those in control group, while FBG levels in Cortex Lycii Radicis low dose group and Cortex Lycii Radicis high dose group were significantly lower than those in model group, with statistically significant differences (all P < 0.05). The levels of TC, TG and LDL-C in model group were higher than those in control group, while the levels of HDL-C were lower than those in control group, with statistically significant differences (all P < 0.05). After 6 weeks of intervention, the levels of TC, TG and LDL-C in Cortex Lycii Radicis low dose group and Cortex Lycii Radicis high dose group were lower than those in model group, while the levels of HDL-C were higher than those in model group, with statistically significant differences (all P < 0.05). The levels of ALT and AST in model group were higher than those in control group. After 6 weeks of intervention, the levels of ALT and AST in Cortex Lycii Radicis low dose group and Cortex Lycii Radicis high dose group were lower than those in model group, with statistically significant differences (all P < 0.05). HE staining showed clear hepatic lobule structure in the control group. In model group, the arrangement of hepatocytes was disordered and lipid vacuoles were obvious. The hepatic fat vacuoles of rats in Cortex Lycii Radicis high dose group were significantly reduced and the structural changes of hepatocytes and steatosis were significantly improved. Oil red O staining showed liver was significant accumulation of lipid droplets in the liver of rats in model group and was decreased lipid deposition in Cortex Lycii Radicis high dose group. The levels of p-AMPK, PPARα and GLUT4 of liver in model group were lower than those in control group, and the GSK3β levels were higher than those in control group, with statistically significant differences (all P < 0.05). The levels of p-AMPK, PPARα and GLUT4 of liver were significantly up-regulated and GSK3 levels were significantly down-regulated in Cortex Lycii Radicis high dose group (P < 0.05). Conclusion The FBG of T2DM rats can be significantly reduced by Cortex Lycii Radicis, improve dyslipidemia and liver pathological changes. It may up-regulate the expression of GLUT4 and PPARα protein and down-regulate the over-expression of GSK3β protein through AMPK signaling pathway to improve IR.

141 Examples of experience in Xie Xinke′s treatment for hyperbilirubinemia and coagulation disorders
LI Yumin1 XIE Xinke2 BIAN Qian2
Hyperbilirubinemia is equivalent to the category of “jaundice” in traditional Chinese medicine. The effect of Western medicine on jaundice is not ideal. However, the understanding of jaundice in traditional Chinese medicine has a long history. On the basis of summing up the experience of predecessors, combined with the characteristics of modern jaundice, the chief physician of Xie Xinke thinks that dampness, heat, blood stasis and stagnation are the key to the onset of jaundice, and he makes Jianghuang Mixture by himself. The main symptom of yellow body and eyes urination is accompanied by hypochondriac pain, bitter mouth, greasiness, pruritus and a small amount of bleeding in the mouth and nose (or gums). The syndrome differentiation is damp heat type with remarkable clinical effect. The clinical experience is analyzed in this case.

192 Discussion on diagnosis and treatment of pulmonary nodules in Chinese and Western medicine
ZHAO Yuanchen LIU Shigang
As the rate of examination of pulmonary nodules increased recent years, the method to deal with or treat pulmonary nodules is coming to a controversial and challenging question. There is incrementally progresses in radiography and non-operative biopsy in Western medicine last several years, which provides an important reference on the differentiation of benign and malignant nodules. Meanwhile, the traditional Chinese medicine, which already has good clinical efficacy in the lung cancer prevention and treatment, is gradually beginning to treat pulmonary nodules patients, and has made good therapeutic effect. Chinese herbs can improve the organism′s microenvironment with its integrality and multi-targets features, in order to make a healthy lung microenvironment so that pulmonary nodules won′t grow. Therefore, according to the writer′s theory of resolving phlegm and softening hardness to dissipate stagnation used in Chinese herbs, plenty of pulmonary nodules patients were treated effectively. With the theory of “preventive treatment of disease” in traditional Chinese medicine, pulmonary nodules, known as precancerous lesions, if could be early detection and intervention, which can be helpful for the early prevention and treatment of lung cancer.
