Research progress on the application of ternary linkage nursing model in patients with chronic diseases

Abstract  With the increasing incidence of chronic diseases year by year, the quality of life of patients with chronic diseases has been seriously affected, bringing heavy pressure to the family and society. The ternary linkage nursing model of hospital-community-family has a significant effect in the treatment of chronic diseases, which can improve patients’ compliance behavior, reduce patients’ readmission rate, improve patients’ quality of life and improve social and economic benefits. By consulting the relevant literature at home and abroad, this paper summarizes the concept of hospital-community-family ternary linkage nursing model and its application effect in patients with chronic diseases. To provide a theoretical basis for effective intervention programs to improve the quality of life of patients with chronic diseases in the future.
