Abstract Exosomes are rich in genomic and non-genomic biomolecules that conduct cell communication activity. It is widely present in the body fluids, not only participates in the pathogenesis of systemic multi-system diseases, but also can be used as therapeutic agent and drug delivery media, so it is called “natural drug carrier”. Traditional Chinese medicine plays its therapeutic role of targeted regulation and long-distance transmission by regulating exosomes. This paper reviews the application of exosomes in cardiovascular disease, liver disease, respiratory system disease and tumor system disease in traditional Chinese medicine, providing new ideas for the research of traditional Chinese medicine.
