社区整合照护模式及需求评估工具的研究进展

Abstract With the old and disabled population growth, the community care system is gradually getting more integrated rather than fragmented, and integrated care has become the general direction of the development of modern health service system. This article explains the types and characteristics of integrated care models in international communities, and introduces the content and application of common community integrated care needs assessment tools. It is proposed to build a people-centered integrated care model, develop an integrated care needs assessment tool suitable for national conditions, and build an integrated care information system for information sharing, aiming at providing a reference basis for developing the community integrated care models and needs assessment tools in China.
