Abstract Objective To compare the molecular characteristics of Bacillus cereus from bloodstream infection (BSI) and food poisoning (FP), to establish a database of Bacillus cereus toxin carrying status and genotyping. Methods From January 2015 to June 2019, 18 strains of Bacillus cereus were isolated from blood of inpatient children in Sichuan Provincial Maternity and Child Health Care Hospital, and 21 strains of Bacillus cereus isolated from fecal samples of food poisoning patients in Sichuan Provincial Maternity and Child Health Care Hospital and Sichuan Provincial Center for Disease Control and Prevention during the same period. The antimicrobial susceptibility test for 14 antibiotics was performed by micro-broth dilution method. Ten virulence genes (hbl A, hbl C, hbl D, nhe A, nhe B, nhe C, ent FM, cyt K, bce T and EM1) were detected by PCR. Genotyping was performed by ERIC-PCR. Results The drug resistance rate of 39 strains of Bacillus cereus to Penicillin G and Ampicillin was more than 95%. The drug resistance rate of the isolates from BSI to Compound Sulfamethoxazole was higher than that from FP (P < 0.05). All strains carried at least 3 virulence genes, and no vomiting related genes were found in the isolates from BSI strains. Thirty-nine isolates Bacillus cereus were divided into 31 genotypes by ERIC-PCR, showing high diversity. Conclusion Bacillus cereus from BSI and FP may have differences in drug resistance spectrum and virulence spectrum, and therefore active monitoring and tracing should be performed.
