Anesthetic effect of lumbar plexus-sciatic nerve and L1-L2 paravertebral nerve block combined with general anesthesia and its influence of cognitive function in elderly patients with operation of intertrochanteric fracture of femur

Abstract  Objective To investigate the anesthetic effect of lumbar plexus-sciatic nerve and L1-L2 paravertebral nerve block combined with general anesthesia and its influence of cognitive function in elderly patients with operation of intertrochanteric fracture of femur. Methods A total of 80 elderly patients with operation of intertrochanteric fracture of femur from January 2018 to December 2019 in Quzhou Municipal Hospital of Traditional Chinese Medicine, Zhejiang Province were selected as the research subjects, and they were divided into observation group (40 cases) and control group (40 cases) according to random number table method. The observation group was given lumbar plexus-sciatic nerve and L1-L2 paravertebral nerve block combined with general anesthesia, while the control group was given laryngeal mask airway general anesthesia. The mean arterial pressure (MAP), heart rate (HR), anesthesia effect, and mini mental state examination (MMSE) score of the two groups were observed. Results The overall analysis found that there were significant differences in time, inter group and interaction between the two groups (P < 0.05). Further pairwise comparisons were made. There was no significant differences in MAP and HR between T1-T4 and T0 in observation group (P > 0.05). MAP of T1, T2 and T4 was significantly higher than that of T0, and HR of T1-T3 was significantly higher than that of T0 in the control group, with statistical significance (all P < 0.05). Comparison between groups showed that MAP of T1-T2 and HR of T1-T3 in observation group were lower than those in control group at the same time point (all P < 0.05). The excellent and good rate of anesthesia in observation group was higher than that in control group, and the difference was statistically significant (P < 0.05). The MMSE scores at three days after operation in both groups were lower than those before operation, but the observation group was higher than the control group, with statistical significance (all P < 0.05). Conclusion Lumbar plexus-sciatic nerve and L1-L2 paravertebral nerve block combined with general anesthesia in elderly patients with operation of intertrochanteric fracture of femur has good anesthesia effects and little impact on cognitive function.
