Clinical significance and plasma levels of Copeptin, PCT and D-dimer in patients with acute pancreatitis |

Abstract Objective To investigate the clinical significance and levels of plasma C-terminal portion of pro-vasopressin (Copeptin), procalcitonin (PCT) and D-dimer in patients with acute pancreatitis (AP). Methods From January 2016 to October 2019, 152 AP patients admitted to Danzhou People’s Hospital, Hainan Province were selected as the AP group, and they were divided into mild acute pancreatitis (MAP) group (70 cases), moderate acute pancreatitis (MSAP) group (40 cases) and severe acute pancreatitis (SAP) group (42 cases) according to the severity of their disease. According to their prognosis, SAP patients were divided into good prognosis group (25 cases) and poor prognosis group (17 cases). Another 50 healthy people were selected as control group at the same period. Plasma Copeptin, PCT and D-dimer levels in different groups were compared. Receiver operating characteristic (ROC) curve were used to analyze the prognostic value of Copeptin, PCT and D-dimer in patients of SAP group. Pearson correlation analysis were used to analyze the correlation on Copeptin, PCT and D-dimer levels in patients of SAP group. Results The levels of plasma Copeptin, PCT and D-dimer in AP group were significantly higher than those in control group (P < 0.05). The levels of plasma Copeptin, PCT and D-dimer in SAP group were significantly higher than those in MSAP group and MAP group (P < 0.05). The levels of plasma Copeptin, PCT and D-dimer in the poor prognosis group were significantly higher than those in the good prognosis group (P < 0.05). ROC curve showed that the area under the curve of Copeptin, PCT and D-dimer combined to predict the poor prognosis of SAP patients was the largest. Correlation analysis showed that the Copeptin were positively correlated with PCT (r = 0.845,P < 0.05) and D-dimer(r = 0.783,P < 0.05), and D-dimer was also positively correlated with PCT (r = 0.802,P < 0.05). Conclusion The levels of plasma Copeptin, PCT and D-dimer are increased in AP patients, the combination of the three indexes has a good value in predicting the poor prognosis of SAP patients.
