Research progress of the application of artery-first approach in laparotomy and minimally invasive pancreaticoduodenectomy |

Abstract Pancreaticoduodenectomy (PD) is currently the only effective operation for patients with pancreatic head cancer to have a chance of cure, and the key of operation is whether R0 resection can be achieved. The improvement of R0 resection rate is directly related to the reasonable choice of operative approach, the technique of the operator and the individual condition of the patient. The latter two factors vary from person to person and are not conducive to generalization as experience. Therefore, it is particularly important to continuously optimize the operative approach. Artery-first approach is based on this appeal, which advocates that the operation should start from peripancreatic artery, and the tumor should be identified as early as possible to the invasion of the artery and be dealt with. In this paper, the research status of the application of artery-first approach in laparotomy and minimally invasive PD in recent years is reviewed, in order to provide reference for clinical practice.
