Correlation between GDF-15 expression and left ventricular remodeling after myocardial infarction |

Abstract Objective To investigate the correlation between GDF-15 and left ventricular remodeling after myocardial infarction. Methods From January 2018 to May 2019, 50 cases with acute myocardial infarction who admitted to the First Affiliated Hospital of Baotou Medical College (“our hospital” for short) were selected as the study group. 50 elderly healthy people who came to physical examination center of our hospital for physical examination during the same period were selected as the control group. Clinical data of two groups were collected, and the level of N-terminal pro-brain B-type natriuretic peptides (NT-proBNP), growth differentiation factors-15 (GDF-15) and left ventricular remodeling correlation index value were compared. According to NYHA classification and heart failure stage, the study group was divided into 4 subgroups respectively, NT-proBNP, GDF-15, left ventricular mass index (LVMI), left ventricular ejection fraction (LVEF), left ventricular end-diastolic diameter (LVEDD) levels of two groups were compared. Pearson correlation analysis was used for correlation analysis, and ROC curve analysis was used for diagnostic efficacy evaluation. Results NT-proBNP, GDF-15, LVEDD and LVMI levels of the study group were significantly higher than those of the control group, while LVEF values were significantly lower than those of the control group (P < 0.01). The higher the NYHA classification, the higher the levels of NT-proBNP, GDF-15, LVEDD and LVMI, and the lower the levels of LVEF (all P < 0.05). The higher the heart failure stage, the higher the levels of NT-proBNP, GDF-15 and LVMI (all P < 0.05). Serum GDF-15 level was negatively correlated with LVEF (r = -0.421, P < 0.001) and positively correlated with NT-proBNP, LVEDD and LVMI (r = 0.424, 0.478, 0.396; P < 0.001). The area under NT-proBNP curve was 0.789, and the optimal cut-off point was 199.45 pg/mL. The area under the curve of GDF-15 was 0.796, and the optimal cut-off point was 144.36 pg/mL. Conclusion Serum GDF-15 is closely related to left ventricular remodeling and increases with the increase of heart failure staging and cardiac function classification, and its diagnostic value for left ventricular remodeling is basically equal to that of NT-proBNP.
