距下关节撑开器辅助开放复位接骨板内固定术在跟骨粉碎性关节内骨折中的疗效

Abstract Objective To evaluate the effect of subtalar joint distractor assisted open reduction and internal fixation in comminuted intra-articular fractures of the calcaneus. Methods The clinical data of 76 patients with comminuted intra-articular fractures of the calcaneus admitted to Chongqing Sixth People′s Hospital from January 2014 to August 2017 was analyzed retrospectively. According to different treating methods, the patients were divided into control group (36 cases) and the research group (40 cases). The control group was treated with Steinmann pin traction and percutaneous reduction by leverage, while the research group was treated with open reduction internal fixation with bone plate assisted by subtalar joint retractor. The curative effect, surgical indexes, injured feet function, complications and the pain scores of the two groups were observed and compared. Results The total effective rate of the research group was significantly higher than that of the control group (P ＜ 0.05). The hospital stay and fracture healing time in the research group were significantly shorter than those in the control group, and the score of American Orthopaedic Foot and Ankle Society ankle-hindfoot scale was significantly higher than that of the control group, the differences were statistically significant (P ＜ 0.05). The incidence of complications in the research group was significantly lower than that of the control group (P ＜ 0.05). 3, 6, 12 months after operation, the pain scores in the research group were significantly lower than those in the control group, the differences were statistically significant (P ＜ 0.05). Conclusion The open reduction internal fixation with bone plate assisted by subtalar joint retractor in the treatment of comminuted intra-articular fractures of the calcaneus has ideal effects, the possibility of complications is low, and it is worthy of further popularization and application in clinic.
