Effects of continuous renal replacement therapy combined with Ulinastatin on the level of pulmonary and renal function in patients with burn complication of sepsis |

Abstract Objective To investigate the effect of continuous renal replacement therapy(CRRT) combined with Ulinastatin on the level of pulmonary and renal function in patients with severe burn complication of sepsis. Methods A total of 30 patients with severe burn complication of sepsis were enrolled in this study, from January 2014 to June 2017 in Affiliated Hospital of Nantong University. They were divided into two groups CRRT (control group) and CRRT combined with Ulinastatin (observation group) treatment were used in the two groups respectively. According to random number table method with 15 cases in each group. The two groups′ hemodynamics were monitored by PICCO catheter. The levels of pulmonary and renal function were tested before and 1, 3, 5, 7 d after treatment. Results The observation group had decrease of ITBVI, EVLWI, SVRI and improvement of PaO2/FiO2 after 1 d treatment (P < 0.05). There was improvement in the control group as well, the expression of ITBVI, EVLWI decreased within 48 h and PaO2/FiO2 increased in the 3rd day of treatment, the differences in the same group and between the two groups were statistically significant (P < 0.05). After treatment, the serum uric acid and creatinine of the two groups decreased while the 24 h urine volume increased in two groups (P < 0.05). At different time points, the serum uric acid and creatinine in the observation group were lower than those in the control group, and 24 h urine volume was higher than the control group (P < 0.05). Conclusion CRRT combined with Ulinastatin on burn sepsis could change the hemodynamics status, and improve the pulmonary and renal function in patients with severe burn complication of sepsis.
