The effect of bedside lung ultrasound in the diagnosis of patients with acute dyspnea |

Abstract Objective To explore the effect of bedside lung ultrasound in the diagnosis of patients with acute dyspnea. Methods Clinical data of 84 cases with dyspnea from June 2016 to May 2018 in Chongqing People′s Hospital (the 3rd ward) were analyzed retrospectively. They were divided into bedside ultrasound group and routine inspection group according to different checking methods, with 42 cases in each group. The sensitivity, specificity, positive predictive value, negative predictive value, diagnostic accuracy, inspection time and inspection cost of the two groups were calculated. Results The examination time of the bedside ultrasound group was shorter than that of the routine inspection group (P < 0.01), and the examination expenses of the two groups had no statistical difference (P > 0.05). The sensitivity of the etiology and diagnostic accuracy in bedside ultrasound group were higher compared with those in the routine inspection group(P < 0.05), the diagnosis specificity of severe pneumonia, pulmonary embolism, acute cardiogenic pulmonary edema was higher (P < 0.05), but the diagnosis specificity of COPD, pneumothorax had no statistical difference (P > 0.05). Conclusion Bedside ultrasound with the characteristics of efficiency, economy, and high diagnostic accuracy, can give individualized and dynamic assessment for severe patients, and it is safe with no radiation. It can provide timely and accurate guidance of management and treatment for acute, critical and serious syndrome.
