Predictive value of hemoglobin level during early pregnancy in gestational diabetes mellitus |

Abstract Objective To explore the predictive value of hemoglobin (Hb) level during early pregnancy gestational diabetes mellitus. Methods A total of 124 gestational diabetes mellitus in Chengdu Women′s and Children′s Center Hospital ("our hospital " for short) from March 2016 to December 2017 were selected as case group. At the same time, 50 healthy pregnant women who were admitted to our hospital for prenatal examination were randomly selected as control group. The basic data such as age, height, weight, etc were recorded, the glucose tolerance (GTT), glycosylated hemoglobin and the blood routine were detected, the predictive value of Hb for gestational diabetes mellitus was analyzed. Results The pregnancy women over 35 years old and body mass index (BMI)≥24 kg/m2 had a higher proportion in case group than those in control group, the levels of fasting blood glucose, blood glucose, HbA1c, Hb, platelet (PLT), red blood cell (RBC) in case group were higher than those in control group, the differences were statistically significant(P < 0.05). Multivariate Logistic regression analysis showed that age, BMI, fasting blood glucose, 2 h post-meal blood glucose, HbA1c, Hb, PLT and RBC were the independent risk factors for gestational diabetes mellitus (OR = 1.147, 1.096, 1.229, 1.191, 1.353, 1.195, 1.226, 1.124, all P < 0.05). The area under the ROC curve was used to assess the predictive value of Hb in gestational diabetes mellitus during the first trimester, the results showed that the area under ROC curve was 0.882 (95%CI: 0.753, 0.921), and the sensitivity, specificity, accuracy were 0.76, 0.83, 0.79, respectively, the best cutoff value for Hb was 129.76 g/L. Conclusion The detection of Hb in early pregnancy can effectively predict gestational diabetes mellitus, so as to provide reference for early intervention and formulate corresponding measures in clinic.
