Error optimization and effectivenes analysis in initial period of informationalized PIVAS |

Abstract Objective To explore the errors in initial period of informationalized pharmacy intravenous admixture services (PIVAS) for reinforcing the security of vein transfusion. Methods From April 2016 to March 2017, a total of 511 cases of errors in initial period of informationalized PIVAS in Jiangwan Hospital in Hongkou District (“our hosipital” for short) were analyzed, meanwhile the types of errors, causes, preventive measures were summarized, and the effects were evaluated by two stages. Results The PIVAS errors were mainly occurred in prescription check, drug dispensing, drug admixture. Considering the errors in each step, improvement measures had been implemented. In the second stage (2016.10-2017.3), the overall error rate decreased by 37.28% compared with that in the first stage (2016.4-2016.9), and the rate of irrational prescription reduced gradually. Conclusion By optimizing information system, developing independent platform of prescription evaluation, strengthening personnel management and system standordized mauagement can reduce the occur of error, and insure the security, rationality, effectiveness of clinical drug use.
