Abstract  Objective To analyze the effect of traditional Chinese medicine external treatment on constipation in patients with heart failure by network meta-analysis. Methods The randomized controlled trials on constipation in patients with heart failure treated by traditional Chinese medicine external treatment from the databases of CNKI, Wangfang, VIP, SinoMed, EMbase, Cochrane Library, PubMed databases, the retrieval time was from inception to October 2021, the data were analyzed by Stata. Results A total of 21 randomized controlled trials were included, covering nine traditional Chinese medicine external treatment methods. In terms of clinical efficacy, abdominal massage (OR = 8.8, 95%CI [3.2, 24.2], P < 0.05), point massage combined with traditional Chinese medicine sticking (OR = 4.0, 95%CI [1.7, 9.2], P < 0.05), abdominal massage combined with foot bath (OR = 4.0, 95%CI [1.6, 9.6], P < 0.05), auricular application pressure (OR = 3.9, 95%CI [1.7, 9.0], P < 0.05}, traditional Chinese medicine sticking (OR = 4.4, 95%CI [2.6, 7.5], P < 0.05), abdominal massage combined with point massage (OR = 21.8, 95%CI [5.6, 85.5], P < 0.05), moxa-wool moxibustion (OR = 0.1, 95%CI [0, 1.0], P < 0.05) were better than conventional treatment; traditional Chinese medicine sticking (OR = 0.2, 95%CI [0.1, 0.9], P < 0.05), auricular application pressure (OR = 0.2, 95%CI [0, 0.9], P < 0.05), abdominal massage combined with foot bath (OR = 0.2, 95%CI [0, 0.9], P < 0.05), point massage combined with traditional Chinese medicine sticking (OR = 0.2, 95%CI [0, 0.9], P < 0.05) and point massage (OR = 0.1, 95%CI [0, 0.3], P < 0.05) were better than abdominal massage combined with point massage. Point massage (OR = 0.2, 95%CI [0.1, 0.6], P < 0.05) was better than abdominal massage. Point massage(OR = 0.3, 95%CI [0.2, 0.8], P < 0.05) was better than traditional Chinese medicine sticking. In terms of traditional Chinese medicine syndrome score, traditional Chinese medicine sticking (OR = 0, 95%CI [0, 0.2], P < 0.05) and auricular application pressure combined with traditional Chinese medicine sticking (OR = 0, 95%CI [0, 0.2], P < 0.05) were better than conventional treatment. Conclusion Traditional Chinese medicine external treatment has different effects on improving the clinical symptoms of constipation and reducing the score of traditional Chinese medicine syndromes, but this conclusion needs further research to verify.
