Abstract  Exendin-4 is a short incretin polypeptide, belonging to a long-acting agonist of glucagon-like peptide-1 (GLP-1) receptor. In recent years, it has been found that Exendin-4 can act on islet β cells in various ways and play a role in the treatment of diabetes. On the one hand, Exendin-4 regulates insulin secretion by specifically binding to GLP-1 receptors on islet β cells. On the other hand, Exendin-4 can also promote the proliferation of islet β cells, regulate autophagy and inhibit apoptosis, and protect islet function. This paper reviews the recent studies on the effects of Exendin-4 on islet β cells and related molecular mechanisms, in order to promote the further application of Exendin-4 in the treatment of diabetes mellitus.
