Clinical effect of acupotomy for treatment of subacromial impingement syndrome based on anatomic structure-selected#br# |

Abstract Objective To observe the clinical effect of acupotomy for treatment of subacromial impingement syndrome based on anatomic structure-selected. Methods A total of 120 patients with subacromial impingement syndrome in the Department of Minimally Invasive Joint Arthrology Clinic of Beijing University of Chinese Medicine Third Affiliated Hospital from April 2020 to May 2021 were selected and they were divided into control group and experimental group according to random number table method, with 60 patients in each group. The control group was given conventional oral Diclofenac Sodium Sustained-release Tablets combined with plaster application, and the experimental group was given acupotomy treatment based on the control group. Shoulder pain and shoulder function were assessed using visual analogue scale (VAS) and Constant-Murley shoulder function score before and at 1, 2, and 4 weeks after treatment. Results Overall analysis: there were statistically significant differences in VAS scores between the two groups, time point comparison and interaction (P < 0.05). Intra-group comparison: VAS scores of both groups at 1, 2 and 4 weeks after treatment were lower than those before treatment, VAS scores at 2 and 4 weeks after treatment were lower than those at 1 week after treatment, VAS score at 4 weeks after treatment was lower than that at 2 weeks after treatment, the differences were statistically significant (P < 0.05). Comparison between groups: at 1, 2 and 4 weeks after treatment, VAS score of experimental group was lower than that of control group, the differences were statistically significant (P < 0.05). Overall analysis: there were statistically significant differences in Constant-Murley scores between the two groups, time point comparison and interaction (P < 0.05). Intra-group comparison: Constant-Murley scores of both groups at 1, 2 and 4 weeks after treatment were higher than those before treatment, Constant-Murley scores at 2 and 4 weeks after treatment were higher than those at 1 week after treatment, Constant-Murley score at 4 weeks after treatment was higher than that at 2 weeks after treatment, the differences were statistically significant (P < 0.05). Conclusion Acupotomy treatment of subacromial impact syndrome based on specific anatomical structure-selected can effectively relieve shoulder pain and improve shoulder joint abduction, flexion and internal rotation, and the improvement effect is obvious immediately after treatment, the effect is significantly better than oral Diclofenac Sodium Sustained-release Tablets combined with local plaster application.
