Abstract Objective To investigate the factors affected the severity of acute ischemic stroke and the relationship between lymphocyte/monocyte ratio (LMR) and acute ischemic stroke severity. Methods From January to December 2017, clinical data of 242 patients with acute ischemic stroke who admitted to the Department of Neurology or Geriatrics, Nanjing Drum Tower Hospital were analyzed retrospectively, including 154 males and 88 females. According to the National Institute of Health Stroke Scale (NIHSS), they were divided into mild cerebral infarction group (NIHSS ≤ 5, n = 181) and moderate-severe cerebral infarction group (NIHSS > 5, n = 61) score at the time of admission. Logistic regression analysis was used to analyze the influence factors of stroke severity. Results There was a statistically significant difference in age, gender, atrial fibrillation, alcohol consumption, fasting glucose, LMR, and CRP between the two groups (P < 0.05). LMR was significantly lower in the moderate-severe cerebral infarction group compared with the mild cerebral infarction group (3.25±2.20 vs. 4.37±1.87, P < 0.05). The multivariate Logistic regression analysis showed that LMR was influence factor of the severity of cerebral infarction patients (OR = 0.8, 95%CI: 0.650-0.985, P < 0.05). Conclusion LMR on admission can reflect the severity of cerebral infarction patients. The lower level of LMR on admission, the more severer condition of the cerebral infarction patients.
