Abstract Ischemic stroke is a disease with high incidence and disability rate. Intravenous thrombolysis and mechanical thrombectomy are two most effective therapies for acute ischemic stroke. However, due to the limitations of therapy time window and surgical techniques, there are many patients who cannot fully or largely recover after the treatment of ischemic stroke. Neural stem cell transplantation provides a new method for the treatment of ischemic stroke. By promoting neurogenesis, angiogenesis, synaptic remodeling, cell replacement, and other mechanisms, neural stem cells play a good effect in the treatment of ischemic stroke. Although there are some problems such as insufficient source of neural stem cell, the optimal transplantation dose and route of administration, unclear time window, and tumorigenicity, neural stem cell transplantation is still a promising therapeutic method.
