The effect comparison of two different internal fixation methods in the treatment of humeral shaft spiral fracture |

Abstract Objective To explore the clinical efficacy of interlocking intramedullary nail combined with the shape memory embracing fixator and the locking compression plate technique in the treatment of humeral shaft spiral fracture. Methods A total of 51 patients with humeral shaft spiral fracture in the Xiaogan Hospital Affiliated to Wuhan University of Science and Technology from January 2014 to January 2016 were selected as the research objects and divided into the IMN group (25 cases) and LCP group (26 cases) by random number table. The IMN group was treated with interlocking intramedullary nail combined with the shape memory embracing fixator and the LCP group was treated with locking compression plate. The operation time, the amount of bleeding, the function of the shoulder and the elbow joint [Neer score, hospital for special surgery knee score (HSS)] and the incidence of complications were compared between the two groups. Results There was no statistically significant difference in the operation time between the two groups (P > 0.05). The total amount of bleeding in the IMN group was lower than that of LCP group, with statistically significant difference (P < 0.05). After operation, the scores of Neer was lower than that of LCP group, and HSS was higher than that of LCP group, with statistically significant differences (P < 0.05). One case of wound infection occurred in both groups after surgery. No delayed fracture healing, nonunion or radial nerve injury occurred 16 weeks after surgery in the IMN group. Conclusion The efficacy of interlocking intramedullary nail combined with the shape memory embracing fixator and the locking compression plate technique in the treatment of humeral shaft spiral fracture are good, but the former has better reduction, less trauma, less bleeding than those of the latter.
